ABSTRACT

OBJECTIVE To investigate the efficacy of Methotrexate (MTX) injection combined with Mifepristone tablets in the treatment of ectopic pregnancy. METHODS A retrospective cohort study method was used to collect data on patients with ectopic pregnancy who visited the Affiliated Hospital of Chengdu University from January 1, 2013 to December 31, 2021. Control group was given MTX injection alone, and exposure group was given MTX injection combined with Mifepristone tablets. Baseline variables were balanced between the two groups using a propensity score matching (PSM), and the outcomes of the matched cohorts were compared. RESULTS A total of 125 patients were included, involving 68 in the control group and 57 in the exposure group. After PSM, 46 cases in each group were successfully matched. There was no significant difference in the treatment response rate and the incidence of adverse drug reaction (ADR) between the exposure group and the control group, which were 73.91% and 63.04%, 21.74% and 13.04% (P>0.05). Compared with before treatment, serum levels of β-HCG were all significantly decreased in 2 groups after treatment (P<0.05); there was no significant difference in the serum level of β-HCG after treatment, the time to return to normal, the interval time of second pregnancy, second ectopic pregnancy and second intrauterine pregnancy between 2 groups (P>0.05). The mean diameter difference of B-ultrasound pregnancy mass before and after treatment, duration of clinical symptom remission and length of hospital stay in the exposure group were significantly higher than those in the control group (P< 0.05), but drug costs during hospitalization and total hospitalization costs were not significantly increased (P>0.05). There were no significant difference between single-dose group and multi-dose group of MTX in the treatment response rate and the incidence of ADR (P>0.05). The hospitalization time, drug cost and total cost of the single dose group were significantly less than those of the multi-dose group (P<0.05). CONCLUSIONS MTX injection combined with or without Mifepristone tablet can both effectively treat ectopic pregnancy, with comparable efficacy and safety. MTX single-dose regimen is similar to the multi-dose regimen in efficacy, but the multi-dose regimen increases the hospital stay and hospitalization costs.

ABSTRACT

Objective To probe into the safety and feasibility of vaginal delivery of scar uterus in pregnancy. Methods 2048 cases of secondary pregnancy women in cesarean section in our hospital from Jan. 1 to Nov. 15, 2017, were selected, 1 case of women was randomly selected from each 15 cases as the sample in this study according to the inclusion criteria. A total of 136 cases were randomly allocated to 68 cases in the observation group and 68 cases in the control group. The observation group was treated with vaginal delivery again, while the control group was treated with cesarean section again. Then, the delivery situation, the amount of blood in the process of labor, the length of the hospitalization time, the Apgar score of the newborn and the puerperal infection of two groups of patients were observed, and the results of the indicators between the two groups were compared and analyzed. Results In the observation group, 63 cases of vaginal delivery were successful(92. 64%), and 5 cases (7. 36%) were failed. In the control group, 68 cases (100%) were successful in cesarean section again, and there was no significant difference between the two groups (P>0. 05). There was a statistically significant difference in the amount of blood produced during labor, neonatal Apgar score and puerperal infection between the two groups(P<0. 05). There was no significant difference in hospitalization days (P>0. 05). Conclusion It is safe and feasible for the clinical scar uterus to be pregnant through vaginal delivery.

ABSTRACT

To establish reference values for the cervical length (CL) measurement by transvaginal ultrasound between 20 and 24+6 weeks of gestation in a large Brazilian population. A retrospective cross-sectional study was performed with 996 singleton pregnancies. The CL measurement (mm) using the transvaginal ultrasound was obtained in a sagittal view and the calipers positioned to measure the linear distance between the triangular area of echodensity at the external os and the internal os. The median±standard deviation and ranges for the CL measurement (mm) was 37.0±10.7 (range, 8 to 51). CL measurement did not modify significantly with gestational age. The observed percentiles for the CL measurement (mm) considering all number case were the following: 5th, 28 mm; 50th, 37 mm; and 95th, 45 mm. Reference values for the CL measurement by transvaginal ultrasound between 20 and 24+6 weeks of gestation in a large heterogeneous Brazilian population were established.

ABSTRACT

Pregnancy is the most remarkable time in every woman’s life. The upsurge in levels of estrogen and progesterone leads to the plethora of changes in various parts of human body, including the oral cavity. In the oral cavity, changes are commonly seen on the gingiva. These include pyogenic granuloma, peripheral giant cell granuloma and also peripheral ossifying fibroma. Not many cases have been reported in literature of the latter. Peripheral ossifying fibroma can interfere with normal tooth position, may become a plaque retentive factor and can be unaesthetic. The pathogenesis of such lesions still remains an enigma. Many believe that peripheral ossifying fibroma may be a progressive stage of pyogenic granuloma. Here, we present a rare case of a peripheral ossifying fibroma seen in a pregnant female in the lower left front tooth region and have discussed the possible etiopathogenesis of the same.
