Resumen Introducción: La melina (Gmelina arborea), es una especie de gran interés por su madera y propiedades medicinales. En Costa Rica, existen clones genéticamente superiores que se propagan sin el conocimiento de la edad ontogénica y fisiológica de los materiales. Objetivo: Evaluar la relación del contenido de fenoles y ligninas en hojas, peciolos, tallos y raíces de plantas con diferentes edades. Métodos: Los contenidos de fenoles y ligninas totales se determinaron mediante el método colorimétrico de Folin-Ciocalteu y el método de extracción alcalina, respectivamente. Para la investigación se eligieron plantas in vitro "año cero" y árboles de año y medio, cuatro, siete y 20 años. El muestreo se realizó en marzo y abril del 2021. Resultados: Se demostró que todas las partes de la planta analizadas contienen compuestos fenólicos y ligninas, independientemente de su edad. No hubo una correlación positiva entre la edad con el contenido de fenoles y ligninas para ninguna condición de desarrollo, pues los valores más altos no se obtuvieron en los árboles más longevos. Los extractos de hojas de las plantas in vitro y los árboles de siete años mostraron, respectivamente, los contenidos más altos de fenoles y ligninas para todas las condiciones (P < 0.05). Los valores promedio más bajos de compuestos fenólicos para todas las condiciones se obtuvieron en los árboles de cuatro años. Respecto a las ligninas, el contenido más bajo se presentó en las raíces más longevas, aunque la tendencia no se mantuvo para el resto de las partes de la planta. Conclusiones: La investigación muestra los primeros resultados del contenido de compuestos fenólicos y ligninas presentes en diferentes tejidos de una especie forestal de edades diferentes. Por lo tanto, son los primeros valores de referencia acerca del compromiso bioquímico para la síntesis fenólica según la edad y el estado de desarrollo específico de una planta leñosa.


Abstract Introduction: Melina (Gmelina arborea) is a tree species of great interest for its wood and medicinal properties. In Costa Rica, there are genetically superior clones that are propagated without knowledge of the ontogenic and physiological age of the materials. Objective: To evaluate how age influences the content of phenols and lignins in leaves, petioles, stems, and roots of melina plants. Methods: The total phenolic and lignins contents were determined using Folin-Ciocalteu colorimetric method and alkaline extraction method, respectively. Plants of five different ages were chosen for the investigation (in vitro plants "year 0" and trees of a year and a half, four, seven and 20 years). Sampling was done in March and April 2021. Results: All parts of the plant analyzed contain phenolic compounds and lignins, regardless of their age. There was no positive correlation between age and phenol and lignin content for any development condition, since the highest values were not obtained in the oldest trees. Leaf extracts from in vitro plants and seven-year-old trees showed, respectively, the highest phenol and lignin contents for all conditions (P < 0.05). The lowest average values of phenolic compounds for all conditions were obtained in four-year-old trees. Regarding lignins, the lowest content occurred in the oldest roots, although the trend was not maintained for the rest of the plant parts. Conclusions: This study provides the first results of the content of phenolic compounds and lignins present in different tissues of a forest species of different ages. Therefore, they are the first reference values about the biochemical commitment for phenolic synthesis according to the age and the specific developmental stage of a woody plant.

Abstract Introduction: Secondary hyperparathyroidism (SHPT) is one of the causes for inflammation in CKD. We assessed the impact of parathyroidectomy (PTX) on neutrophil-to-lymphocyte (N/L) and platelet-to-lymphocyte (P/L) ratios in SHPT patients. Methods: A total of 118 patients [hemodialysis (HD, n = 81), and transplant recipients (TX, n = 37)] undergoing PTX between 2015 and 2021 were analyzed. Results: There was a significant reduction in calcium and PTH levels in both groups, in addition to an increase in vitamin D. In the HD group, PTX did not alter N/L and P/L ratios. In the TX group, there was a reduction in N/L and P/L ratios followed by a significant increase in total lymphocyte count. Conclusion: N/L and P/L ratios are not reliable biomarkers of inflammation in SHPT patients undergoing PTX. Uremia, which induces a state of chronic inflammation in dialysis patients, and the use of immunosuppression in kidney transplant recipients are some of the confounding factors that prevent the use of this tool in clinical practice.


Resumo Introdução: O hiperparatireoidismo secundário (HPTS) é uma das causas de inflamação na DRC. Avaliamos o impacto da paratireoidectomia (PTX) nas relações neutrófilo/linfócito (N/L) e plaqueta/linfócito (P/L) em pacientes com HPTS. Métodos: Foram analisados 118 pacientes [hemodiálise (HD, n = 81) e transplantados (TX, n = 37)] submetidos à PTX entre 2015 e 2021. Resultados: Houve redução significativa de cálcio e PTH nos dois grupos, além de elevação de vitamina D. No grupo HD, a PTX não mudou as relações N/L e P/L. Já no grupo TX, houve redução nas relações N/L e P/L acompanhadas de elevação significativa do número de linfócitos totais. Conclusão: As relações N/L e P/L não são marcadores fidedignos de inflamação em pacientes com HPTS submetidos à PTX. A uremia, que induz um estado de inflamação crônica em pacientes dialíticos, e o uso de imunossupressão em pacientes transplantados renais são alguns dos fatores de confusão que impedem o uso dessa ferramenta na prática clínica.

Abstract Introduction: Management of secondary hyperparathyroidism (SHPT) is a challenging endeavor with several factors contruibuting to treatment failure. Calcimimetic therapy has revolutionized the management of SHPT, leading to changes in indications and appropriate timing of parathyroidectomy (PTX) around the world. Methods: We compared response rates to clinical vs. surgical approaches to SHPT in patients on maintenance dialysis (CKD 5D) and in kidney transplant patients (Ktx). A retrospective analysis of the one-year follow-up findings was carried out. CKD 5D patients were divided into 3 groups according to treatment strategy: parathyroidectomy, clinical management without cinacalcet (named standard - STD) and with cinacalcet (STD + CIN). Ktx patients were divided into 3 groups: PTX, CIN (cinacalcet use), and observation (OBS). Results: In CKD 5D we found a significant parathormone (PTH) decrease in all groups. Despite all groups had a higher PTH at baseline, we identified a more pronounced reduction in the PTX group. Regarding severe SHPT, the difference among groups was evidently wider: 31%, 14% and 80% of STD, STD + CIN, and PTX groups reached adequate PTH levels, respectively (p<0.0001). Concerning the Ktx population, although the difference was not so impressive, a higher rate of success in the PTX group was also observed. Conclusion: PTX still seems to be the best treatment choice for SHPT, especially in patients with prolonged diseases in unresourceful scenarios.


Resumo Introdução: O manejo do hiperparat-ireoidismo secundário (HPTS) é uma tarefa desafiadora com diversos fatores que contribuem para o fracasso do tratamento. A terapia calcimimética revolucionou o manejo do HPTS, levando a alterações nas indicações e no momento apropriado da paratireoidectomia (PTX) em todo o mundo. Métodos: Comparamos taxas de resposta às abordagens clínica vs. cirúrgica do HPTS em pacientes em diálise de manutenção (DRC 5D) e pacientes transplantados renais (TxR). Foi realizada uma análise retrospectiva dos achados de um ano de acompanhamento. Pacientes com DRC 5D foram divididos em 3 grupos de acordo com a estratégia de tratamento: paratireoidectomia, manejo clínico sem cinacalcete (denominado padrão - P) e com cinacalcete (P + CIN). Os pacientes com TxR foram divididos em 3 grupos: PTX, CIN (uso de cinacalcete) e observação (OBS). Resultados: Na DRC 5D, encontramos uma redução significativa do paratormônio (PTH) em todos os grupos. Apesar de todos os grupos apresentarem um PTH mais elevado no início do estudo, identificamos uma redução mais acentuada no grupo PTX. Com relação ao HPTS grave, a diferença entre os grupos foi evidentemente maior: 31%, 14% e 80% dos grupos P, P + CIN e PTX atingiram níveis adequados de PTH, respectivamente (p< 0,0001). Com relação à população TxR, embora a diferença não tenha sido tão impressionante, também foi observada uma taxa maior de sucesso no grupo PTX. Conclusão: A PTX ainda parece ser a melhor escolha de tratamento para o HPTS, especialmente em pacientes com doenças prolongadas em cenários sem recursos.

ABSTRACT

Os objetivos deste estudo são: relatar a experiência do desenvolvimento das atividades de sensibilização dos trabalhadores relacionadas à promoção da saúde no ambiente laboral e à humanização no trabalho realizadas em dois serviços de saúde de um município do estado de Minas Gerais e avaliar junto à equipe a realização dessas atividades. As atividades de alongamento, massagens, escalda-pés, dinâmica de grupo e oficina da beleza foram realizadas na atenção primária e secundária. Utilizou-se um questionário e as respostas foram apresentadas por estatística simples. Os trabalhadores avaliaram as atividades como "excelentes"; referiram melhorar o dia de trabalho; sentiram-se valorizados e mais dispostos para o trabalho, além de solicitarem a continuidade de ações de promoção da saúde e humanização no trabalho. Por mais simples que sejam, essas atividades proporcionaram momentos de reflexões e a pausa laboral, tendo em vista a saúde do trabalhador e melhor qualidade de vida no trabalho.


The objectives of this study are: to report the experience of developing awareness activities for workers related to health promotion and humanization at work, carried out in two health services in a municipality in the state of Minas Gerais; and assess with the team these activities. Stretching activities, group dynamics, massages, foot baths, beauty workshop and health promotion were carried out in primary and secondary care. A questionnaire was used and the answers were presented by simple statistics. The workers rated the activities as "excellent"; reported improving their working day; they felt valued and more willing to work, in addition to requesting the continuity of actions to promote health and humanization at work. As simple as they are, these activities provided moments of reflection and a break from work, with a view to the worker's health and better quality of life at work.

ABSTRACT

Enteroparasitosis are diseases caused by parasitic agents present in the environment and in the gastrointestinal tract of living beings. In addition, they are still considered neglected diseases, but of great importance for public health, especially when they are related to secondary infections and currently their co-infection profile with COVID-19. The interaction of protozoa and/or helminths with the SARS-CoV-2 virus is timely and its signs and symptoms are confused with other pathogen relationships. In this way, this study aims to correlate the incidence of enteroparasitosis and COVID-19, in the pandemic period from 2020 to April 2022. This is a documentary and exploratory study of secondary data from laboratory tests of patients who were treated and diagnosed with COVID-19 and enteroparasitosis at Hospital Doutor Cloves Bezerra Cavalcante, Municipal Hospital of Bananeiras, Paraíba, Brazil. In the analysis of the database, a significant increase of approximately 48.85% in the incidence of COVID-19 cases from 2020 to 2021 stands out, remaining high until 2022. In contrast, cases of enteroparasites peaked at 48.74% in 2021, followed by an average reduction of 23.12%, with a deviation of 1.49%, in relation to the years 2020 and 2022. It was concluded that COVID-19 is predominantly associated with an increase in secondary infections, highlighting the crucial need to promote health education, improve basic sanitation and guarantee access to health services as essential components in combating the increase in parasitic infections, especially those related to viral pathologies.


As enteroparasitoses são enfermidades originadas por agentes parasitários presentes no meio ambiente e no trato gastrointestinal dos seres vivos. Ademais, ainda são consideradas doenças negligenciadas, porém de grande importância para a saúde pública, em especial, quando estão relacionadas com infecções secundárias e atualmente seu perfil de coinfecção com a COVID-19. A interação de protozoários e/ou helmintos com o vírus SARS-CoV-2 é oportuna e seus sinais e sintomas são confundidos com outras relações de patógenos. Desta maneira, este estudo visa correlacionar a incidência de enteroparasitoses e COVID-19, no período pandêmico de 2020 a abril de 2022. Trata--se de uma pesquisa documental e exploratória, de dados secundários dos exames laboratoriais de pacientes que foram atendidos e diagnosticados com COVID-19 e enteroparasitoses no Hospital Doutor Cloves Bezerra Cavalcante, Hospital Municipal de Bananeiras, Paraíba, Brasil. Na análise da base de dados, destaca-se um aumento significativo de aproximadamente 48,85% na incidência de casos de COVID-19 de 2020 a 2021, mantendo-se elevado até 2022. Em contraste, os casos de enteroparasitas atingiram um pico de 48,74% em 2021, seguido por uma redução média de 23,12%, com um desvio de 1,49%, em relação aos anos de 2020 e 2022. Conclui-se que a COVID-19 está predominantemente associada ao aumento de infecções secundárias, destacando a necessidade crucial de promover a educação em saúde, melhorar o saneamento básico e garantir o acesso aos serviços de saúde como componentes essenciais no combate ao aumento de infecções parasitárias, especialmente aquelas relacionadas a patologias virais.

ABSTRACT

Introducción: La lumbalgia se presenta en aproximadamente 9,4 % de la población mundial. La acupuntura es empleada por la medicina tradicional china para estimular determinados puntos del cuerpo con diferentes tipos de agujas. Entre sus efectos beneficiosos se describe la remisión del dolor. Objetivo: Evaluar la efectividad del tratamiento acupuntural en pacientes con dolor lumbar asistidos en el cuerpo de guardia. Métodos: Se realizó un estudio cuasiexperimental de intervención terapéutica (sin grupo control) en 35 pacientes con dolor lumbar, los cuales fueron atendidos en el Cuerpo de Guardia de Medicina Natural y Tradicional del Hospital General Docente Dr. Juan Bruno Zayas Alfonso de Santiago de Cuba, desde julio hasta diciembre del 2021. Resultados: En la investigación primaron el grupo etario de 40-49 años (31,4 %), el sexo femenino (57,1 %), las amas de casa, el dolor entre grave y moderado al inicio del tratamiento, así como los pacientes sin dolor una hora después de la terapia. En cuanto a la evolución final, el total de la muestra clasificó en las categorías de aliviados y mejorados. Conclusiones: Se demostró la efectividad de la acupuntura en pacientes con dolor lumbar agudo.


Introduction: Low back pain is presented in approximately 9.4% of the world population. Acupuncture is used by Chinese traditional medicine to stimulate certain points of the body with different types of needles. Among its beneficial effects the pain remission is described. Objective: To evaluate the effectiveness of the acupuntural treatment in patients with lumbar pain assisted in the emergency room. Methods: A quasi-experiment of therapeutic intervention study (without control group) was carried out in 35 patients with lumbar pain, who were assisted in the Natural and Traditional Medicine Emergency Room of Dr. Juan Bruno Zayas Alfonso Teaching General Hospital in Santiago de Cuba, from July to December, 2021. Results: In the investigation there was a prevalence the 40-49 age group (31.4%), female sex (57.1%), housewives, serious and moderate pain at the beginning of the treatment, as well as patients without pain one hour after the therapy. As for the final clinical course, the total of the sample classified in the relieved and improved categories. Conclusions: The effectiveness of acupuncture was demonstrated in patients with acute lumbar pain.

Resumo Fundamento: As diretrizes da Sociedade Europeia de Cardiologia recomendam um nível de colesterol LDL (LDL-C) < 55 mg/dL para pacientes com doença cardiovascular estabelecida. Embora a fórmula de Friedewald ainda seja amplamente utilizada para estimar o LDL-C, a fórmula mais recente de Martin-Hopkins mostrou maior precisão. Objetivos: Nosso objetivo foi avaliar: A) a proporção de pacientes que atingiram a meta de LDL-C e as terapias utilizadas em um centro terciário; B) o impacto da utilização do método de Martin-Hopkins em vez do método de Friedewald na proporção de pacientes controlados. Métodos: Estudo transversal monocêntrico, incluindo pacientes consecutivos pós-infarto do miocárdio, acompanhados por 20 cardiologistas, em um hospital terciário. Os dados foram coletados retrospectivamente de consultas clínicas realizadas após abril de 2022. Para cada paciente, os níveis de LDL-C e o atingimento das metas foram estimados a partir de um perfil lipídico ambulatorial, utilizando as fórmulas de Friedewald e Martin-Hopkins. Um valor-p bicaudal < 0,05 foi considerado estatisticamente significativo para todos os testes. Resultados: Foram incluídos 400 pacientes (com 67 ± 13 anos, 77% do sexo masculino). Utilizando a fórmula de Friedewald, a mediana de LDL-C sob terapia foi de 64 (50-81) mg/dL, e 31% tinham LDL-C dentro da meta. Estatinas de alta intensidade foram usadas em 64% dos pacientes, 37% estavam em uso de ezetimiba e 0,5% estavam em uso de inibidores de PCSK9. A terapia combinada de estatina de alta intensidade + ezetimiba foi utilizada em 102 pacientes (26%). A aplicação do método de Martin-Hopkins reclassificaria um total de 31 pacientes (7,8%). Entre aqueles considerados controlados pela fórmula de Friedewald, 27 (21,6%) teriam LDL-C estimado por Martin-Hopkins acima da meta. Conclusões: Menos de um terço dos pacientes pós-infarto do miocárdio apresentaram LDL-C dentro da meta. A aplicação da fórmula de Martin-Hopkins reclassificaria um quinto dos pacientes presumivelmente controlados no grupo de pacientes não controlados.


Abstract Background: The European Society of Cardiology guidelines recommend an LDL-cholesterol (LDL-C) < 55 mg/dL for patients with established cardiovascular disease. While the Friedewald equation to estimate LDL-C is still widely used, the newer Martin-Hopkins equation has shown greater accuracy. Objectives: We aimed to assess: A) the proportion of patients reaching LDL-C goal and the therapies used in a tertiary center; B) the impact of using the Martin-Hopkins method instead of Friedewald's on the proportion of controlled patients. Methods: A single-center cross-sectional study including consecutive post-myocardial infarction patients followed by 20 cardiologists in a tertiary hospital. Data was collected retrospectively from clinical appointments that took place after April 2022. For each patient, the LDL-C levels and attainment of goals were estimated from an ambulatory lipid profile using both Friedewald and Martin-Hopkins equations. A two-tailed p-value of < 0.05 was considered statistically significant for all tests. Results: Overall, 400 patients were included (aged 67 ± 13 years, 77% male). Using Friedewald's equation, the median LDL-C under therapy was 64 (50-81) mg/dL, and 31% had LDL-C within goals. High-intensity statins were used in 64% of patients, 37% were on ezetimibe, and 0.5% were under PCSK9 inhibitors. Combination therapy of high-intensity statin + ezetimibe was used in 102 patients (26%). Applying the Martin-Hopkins method would reclassify a total of 31 patients (7.8%). Among those deemed controlled by Friedewald's equation, 27 (21.6%) would have a Martin-Hopkins' LDL-C above goals. Conclusions: Less than one-third of post-myocardial infarction patients had LDL-C within the goal. Applying the Martin-Hopkins equation would reclassify one-fifth of presumably controlled patients into the non-controlled group.

Anredera cordifolia (Ten.) Steenis is a vine species native to Brazil that is considered an unconventional food plant and a medicinal species whose phenolic compounds exert antioxidant activity. Since the production of metabolites is determined by environmental factors and leaf maturity, it is important to track these changes in order to determine the best time to harvest. This study aimed to verify whether leaf phenology and seasonality cause variations in the amount of phenolic compounds and in the antioxidant activity of this species. The leaves were collected in different seasons between September 2018 and April 2019, and separated according to maturity: young, mature, and senescent. Daily atmospheric temperature and rainfall data were used to characterize the collection period. The total phenolic content (TPC), determined by Folin-Ciocalteu method, was significantly higher in the young leaves collected in winter, a season of lower temperatures. These leaves showed 54.4 mg of gallic acid equivalents per 100 g of dry matter (mg GAE 100 g-1DM). Other results averaged 25.6 mg GAE 100 g-1DM. The highest antioxidant activity, assessed via the 2,2-diphenyl-1-picrylhydrazyl (DPPH) method, regardless of leaf phenology, was observed in leaves harvested in autumn (73.7%) and winter (71.1%), seasons with lower rainfall. Leaves harvested in summer and spring had lower antioxidant action rates (54.3 e 37.5%, respectively). There was no significant correlation between the total phenolic content and antioxidant activity. Thus, the phenolic composition of A. cordifolia, and consequently its activity on free radicals, varies seasonally in response to temperature and rainfall, and may or may not interact with the age of the leaves.


Anredera cordifolia (Ten.) Steenis é uma trepadeira nativa do Brasil considerada uma planta alimentícia não convencional e uma espécie medicinal, cujos compostos fenólicos exercem ação antioxidante. Como a produção de metabólitos é determinada por fatores ambientais e pela maturidade das folhas, é importante acompanhar essas mudanças a fim de determinar a época de colheita. Neste estudo objetivou-se verificar se a fenologia foliar e a sazonalidade ocasionam variação na quantidade de compostos fenólicos e na ação antioxidante dessa espécie. As folhas foram coletadas em diferentes estações, entre setembro de 2018 e abril de 2019, e separadas conforme a maturidade: jovens, maduras e senescentes. Os dados diários de temperatura atmosférica e pluviosidade foram usados para caracterizar o período de coleta. O conteúdo de fenólicos totais (TPC), determinado pelo método Folin-Ciocalteu, foi significativamente maior nas folhas jovens coletadas no inverno, período caracterizado por temperaturas mais baixas. Estas folhas apresentaram 54,4 mg equivalentes de ácido gálico por 100 g de matéria seca (mg GAE 100 g-1DM) . A média dos outros resultados foi de 25,6 mg GAE 100 g-1DM. A maior ação antioxidante, avaliada pelo método do radical livre 2,2-difenil-1-picrilhidrazil (DPPH), foi observada em folhas colhidas no outono (73,7%) e inverno (71,1%), independentemente da fenologia foliar. Nestes períodos a pluviosidade foi menor. Folhas colhidas no verão e na primavera apresentaram menores percentuais de ação antioxidante (54,3 e 37,5%, respectivamente). Não houve correlação significativa entre o conteúdo de fenólicos totais e a atividade antioxidante. Conclui-se que a composição fenólica de A. cordifolia e, consequentemente, sua ação sobre radicais livres, varia sazonalmente em resposta à temperatura e pluviosidade, podendo ou não interagir com a idade das folhas.

ABSTRACT

Liver failure is a common clinical syndrome of severe liver disease with rapid progression and high mortality. Therefore, early intervention in pre-liver failure or “golden window” is of great significance in improving the prognosis of patients. Hepatitis B virus-related acute-on-chronic liver failure and alcohol-related acute-on-chronic liver failure are the main topics of studies on pre-liver failure. This article discusses the pathogenesis of pre-liver failure and artificial liver support therapy, so as to guide the reasonable and affective applications of artificial liver technology in clinical practice, promote related studies, and thereby reduce the mortality rate of patients with liver failure.

Cervical cancer is a malignant tumor that can be effectively controlled and eventually eliminated through etiological prevention, pathogenic prevention, and clinical prevention. In China, some problems exist in comprehensive prevention and control system for cervical cancer, such as uneven development, insufficient capacity building, and scattered institutions. In 2022, the Hubei Provincial Government launched a province-wide project to provide free cervical cancer screening for 12.67 million women aged 35-64 from 2022 to 2024. It is a historic opportunity and of great significance to fully utilize the scale and team advantages of this full-coverage cervical cancer screening, build integrated outpatient clinic for cervical cancer prevention in grassroots maternal and child health hospitals, and implement standardized construction and management. The expert group of the Cervical Disease Prevention and Control Professional Committee of Hubei Maternal and Child Health Association formulated this consensus based on the latest progress and the actual situation in Hubei. The consensus elucidated the general requirements, HPV vaccination outpatient construction standards and workflow, cervical cancer screening outpatient construction standards and workflow, colposcopy outpatient construction standards and workflow, precancerous lesion treatment outpatient construction standards and workflow, the follow-up after treatment of precancerous lesions, public education, etc. This consensus is suitable for guiding the standardized construction and management of integrated outpatient clinics for cervical cancer prevention and control in grassroots (county, district) maternal and child health hospitals in Hubei province.

ABSTRACT

Secondary metabolites of medicinal plants are extremely important to human health because of their special pharmacological activities or efficacy. They are the main source of drugs, health care products, and cosmetics. As human beings continue to pursue health and longevity, the demand in the pharmaceutical market continues to grow. It becomes especially important to improve the production and quality of secondary metabolites of medicinal plants. Plant secondary metabolites are a kind of adaptation of plants to their environment and are the result of the interaction between plants and biotic and abiotic factors during the long-term evolution process. The production and accumulation of secondary metabolites in medicinal plants are mainly affected by plant genetic factors and environmental factors. Among them, light environment is extremely important for their synthesis. Therefore, light regulation has long been a research focus for many scholars in China and abroad. In this article, we the recent research progress on the effects of light regulation on the secondary metabolites of medicinal plants were reviewed, mainly focusing on the effects of light quality, light intensity and photoperiod, in order to provide theoretical basis and practical guidance for the efficient production of secondary metabolites with important pharmacological activities.

ABSTRACT

@#Excess hormone production from adrenal tumors caused by primary hyperaldosteronism or pheochromocytoma are common etiologies for secondary hypertension. Studies have shown that sustained long-term circulating hormones in excess affect the blood vessels and cardiac structures. Inflammation of cardiomyocytes leads to fibrosis and eventual cardiomyopathy and is clinically presented as arrhythmia, nonfatal myocardial infarction, heart failure, or even death. The tissue changes and/or impaired cardiac function are reversible if early diagnosis and removal of the adrenal tumor by unilateral adrenalectomy is done. However, the condition becomes challenging if the adrenal lesions are bilateral. This article introduces the concept of systemic hormonal unloading and will discuss the philosophy of quality of life in managing bilateral adrenal disease.

ABSTRACT

Sesquiterpenes are natural terpenoids with 15 carbon atoms in the basic skeleton, which mainly exist in plant volatile oil and have important physiological and medicinal value. Cytochrome P450 (CYP450) is a kind of monooxygenase encoded by supergene family, which is one of the largest gene families in plants. It is involved in the synthesis and metabolism of terpenoids, alkaloids and other secondary metabolites. In the process of terpene biosynthesis, CYP450 participates in the post-modification stage of terpenes by introducing functional groups such as hydroxyl, carboxyl and carbonyl, which plays an important role in enriching the diversity of terpenes. The CYP450 enzymes involved in sesquiterpene synthesis and their substrate catalytic specificity mechanisms have been partially investigated. In this paper, the biosynthetic pathway of plant sesquiterpenes, the structure and classification of CYP450 enzymes were briefly introduced, and the CYP450 enzymes involved in sesquiterpene biosynthesis were summarized, in order to provide a reference for intensive study of the role of CYP450 enzymes in the synthesis of sesquiterpenoids.

ABSTRACT

ObjectiveTo study the changing characteristics of secondary metabolic compounds accumulated in Dendrobium nobile stems at different growth years, a simulated wild stone plant, in order to provide a theoretical basis for rational planning of the harvesting period of D. nobile. MethodUltra-high performance liquid chromatography-mass spectrometry(UPLC-MS/MS) was used to detect and analyze the secondary metabolites in the stems of 1-year-old, 2-year-old, and 3-year-old D. nobile. The mass spectrometry data were processed using Analyst 1.6.3 software, and all samples were subjected to principal component analysis(PCA), cluster heat map analysis, partial least squares-discriminant analysis(PLS-DA), and differential secondary metabolites were screened based on variable importance in projection(VIP) values>1, fold change(FC)≥2 and FC≤0.5. Then differential secondary metabolites were identified based on relative molecular weight, fragmentation ions and mass spectrometry database, and enriched pathways were identified based on the Kyoto Encyclopedia of Genes and Genomes(KEGG) database. ResultA total of 1 317 secondary metabolites were identified in the stems of D. nobile at three growth stages, with flavonoids, phenolic acids, alkaloids and terpenoids accounting for 76.55% of the total. Compared with the 1-year-old stems of D. nobile, 289 differential secondary metabolites were identified in the 2-year-old stems, of which 255 were up-regulated and 34 were down-regulated, 682 differential secondary metabolites were identified in the 3-year-old stems, of which 502 were up-regulated and 180 were down-regulated. Compared to the 2-year-old stems, the 3-year-old stems had 602 differential secondary metabolites, with 405 up-regulated and 197 down-regulated. As the growth stage of D. nobile increased, the top 10 up-regulated differential metabolites mainly included flavonoids, phenolic acids, phenylpropanoids and terpenoids, such as kaempferol derivatives, asperulosidic acid, apigenin derivatives, chrysoeriol derivatives, isorhamnetin derivatives, taxifolin derivatives, quercetin derivatives. KEGG enrichment analysis showed significant enrichment of secondary metabolites in the flavonoid biosynthesis, flavone, and flavonol biosynthesis, secondary metabolite biosynthesis, and phenylpropanoid biosynthesis pathways with the increase of growth years. ConclusionWith the increase of the growth years, the levels of secondary metabolites such as flavonoids, phenolic acids, phenylpropanoids and terpenoids in the wild-grown D. nobile have been significantly enhanced. In practical production, grading based on different growth years can be carried out to improve the medicinal and economic values of D. nobile.

Background@#Scabies is the second most common cause of disability among skin diseases in the Philippines as of 2019. There is no large nationwide study describing the epidemiologic profile of scabies in the country. Objective. This study aimed to describe the demographic, seasonal, and geographic profile of scabies in the Philippines.@*Methods@# We compared secondary data of two local patient registries (Philippine Dermatological Society, PDS, 2010 to 2021; and Philippine Pediatric Society, PPS, 2009 to 2021) for reported cases of scabies in the Philippines. We reported the frequency and percentage distribution according to age, sex, month, year, and type of diagnosis, and region. @*Results@#The median annual frequency of scabies cases (mostly outpatient) for PDS (from year 2010) was 4087 (range ([QR], 342-6422 [3271.5]), while it was 183 (range [IQR], 64-234 [96.5]) (all inpatient) for PPS (from year 2009). There was a reduction to one-third (PDS) and one-fourth (PPS) of pre-pandemic numbers during the pandemic years (2020- 2021). The peak months for scabies cases were the cooler months: January (median, 12.1% of annual cases; range [IQR], 2.6%-31.4% [3.6%]) to February (median, 10.0% of annual cases; range [IQR], 1.5%-27.8% [2.5%]) based on PDS data, and November (median, 10.0% of annual cases; range [IQR], 0.0%-24.3% [7.0%]) to January (median, 9.0% of annual cases; range [IQR], 0.0%-24.3% [6.6%]) for PPS data. Overall, for PDS, age 1-4 years is the most affected age group (median, PDS, 17.5% of annual cases; range [IQR], 11.9%-25.4% [8.1%]), while it was the less than 1-yearolds (median annual cases, 48.9%; range [IQR], 29.1%-67.3% [13.20%]) among PPS pediatric population aged 0 to 18 years. Males (median, 53.9% of annual cases; range [IQR], 45.0%-67.2% [8.8%]) were more affected than females in PPS. While for PDS during earlier years (prior to 2015), males (median, 51.6% of annual cases from 2010 to 2014; range [IQR], 47.4%-52.9% [0.2%]) were more affected than females. However, males became less affected than females with median, 44.7% of annual cases from 2015 onwards (range [IQR], 43.4%-46.5% [1.2%]). NCR was the region with the highest frequency of cases in PPS (median, 52.6% of annual cases; range [IQR], 22.7%-75.0% [20.4%]). The 2nd most affected regions were Central/Eastern Visayas (34.2%, 2009-2013; range [IQR], 17.9%-54.1% [5.3%]), Bicol region (12%; 2014 to 2018; range [IQR], 17.9%-54.1% [7.4%]), Central Luzon (18%; 2019), Central/Eastern Visayas (29%, 2020), and Northern/Central Mindanao (17%, 2021). @*Conclusion@#Scabies was commonly seen in the younger age group, slightly more in females in the PDS, while slightly more among males in the PPS, in the cooler months of the year, and in the urbanized NCR.

Abstract Anredera cordifolia (Ten.) Steenis is a vine species native to Brazil that is considered an unconventional food plant and a medicinal species whose phenolic compounds exert antioxidant activity. Since the production of metabolites is determined by environmental factors and leaf maturity, it is important to track these changes in order to determine the best time to harvest. This study aimed to verify whether leaf phenology and seasonality cause variations in the amount of phenolic compounds and in the antioxidant activity of this species. The leaves were collected in different seasons between September 2018 and April 2019, and separated according to maturity: young, mature, and senescent. Daily atmospheric temperature and rainfall data were used to characterize the collection period. The total phenolic content (TPC), determined by Folin-Ciocalteu method, was significantly higher in the young leaves collected in winter, a season of lower temperatures. These leaves showed 54.4 mg of gallic acid equivalents per 100 g of dry matter (mg GAE 100 g-1DM). Other results averaged 25.6 mg GAE 100 g-1DM. The highest antioxidant activity, assessed via the 2,2-diphenyl-1-picrylhydrazyl (DPPH) method, regardless of leaf phenology, was observed in leaves harvested in autumn (73.7%) and winter (71.1%), seasons with lower rainfall. Leaves harvested in summer and spring had lower antioxidant action rates (54.3 e 37.5%, respectively). There was no significant correlation between the total phenolic content and antioxidant activity. Thus, the phenolic composition of A. cordifolia, and consequently its activity on free radicals, varies seasonally in response to temperature and rainfall, and may or may not interact with the age of the leaves.


Resumo Anredera cordifolia (Ten.) Steenis é uma trepadeira nativa do Brasil considerada uma planta alimentícia não convencional e uma espécie medicinal, cujos compostos fenólicos exercem ação antioxidante. Como a produção de metabólitos é determinada por fatores ambientais e pela maturidade das folhas, é importante acompanhar essas mudanças a fim de determinar a época de colheita. Neste estudo objetivou-se verificar se a fenologia foliar e a sazonalidade ocasionam variação na quantidade de compostos fenólicos e na ação antioxidante dessa espécie. As folhas foram coletadas em diferentes estações, entre setembro de 2018 e abril de 2019, e separadas conforme a maturidade: jovens, maduras e senescentes. Os dados diários de temperatura atmosférica e pluviosidade foram usados para caracterizar o período de coleta. O conteúdo de fenólicos totais (TPC), determinado pelo método Folin-Ciocalteu, foi significativamente maior nas folhas jovens coletadas no inverno, período caracterizado por temperaturas mais baixas. Estas folhas apresentaram 54,4 mg equivalentes de ácido gálico por 100 g de matéria seca (mg GAE 100 g-1DM) . A média dos outros resultados foi de 25,6 mg GAE 100 g-1DM. A maior ação antioxidante, avaliada pelo método do radical livre 2,2-difenil-1-picrilhidrazil (DPPH), foi observada em folhas colhidas no outono (73,7%) e inverno (71,1%), independentemente da fenologia foliar. Nestes períodos a pluviosidade foi menor. Folhas colhidas no verão e na primavera apresentaram menores percentuais de ação antioxidante (54,3 e 37,5%, respectivamente). Não houve correlação significativa entre o conteúdo de fenólicos totais e a atividade antioxidante. Conclui-se que a composição fenólica de A. cordifolia e, consequentemente, sua ação sobre radicais livres, varia sazonalmente em resposta à temperatura e pluviosidade, podendo ou não interagir com a idade das folhas.

ABSTRACT Follicular thyroid cancers account for 15%-20% of all thyroid tumors. Choroidal metastases secondary to follicular thyroid cancer rarely occur. Herein, we report the case of an 85-year-old woman who presented choroidal metastasis from a follicular thyroid carcinoma in the right eye 7 years after total thyroidectomy and underwent enucleation. To confirm the diagnosis and primary tumor site, histopathological, and immunohistochemical examinations were performed. One year later, she presented metastasis in the contralateral eye. Few similar cases have been described in the literature.

Introducción: El COVID-19 ha afectado la salud mental de la población pediátrica. Con la transición a la virtualidad, se han identificado posturas positivas y negativas. Debido a la diferencia de características sociodemográficas que posee la República Dominicana, es necesario describir el efecto de dichos cambios en la salud mental dentro de este contexto. Objetivo: Evaluar el efecto de las clases virtuales en la salud mental de los pacientes pediátricos. Metodología: Se utilizó el cuestionario de capacidades y dificultades en los pacientes que asistieron a consulta general. Se asignó una puntuación con la escala de puntuación estandarizada y se analizaron los resultados en gráficos de frecuencia y pruebas de correlación de spearman. Resultados: Existe una frecuencia del 10,4% (n=34) de alto riesgo de desarrollar algún trastorno psiquiátrico. Se observaron correlaciones débiles entre menor edad y trastorno de conducta, r(365) = -0,111, p = 0,034, y sexo masculino y cualquier trastorno pediátrico, r(365) = -0,131, p = 0,012. Hay una incidencia de 51,5% de síntomas físicos. Un 45.5% y un 30.2% identificó mejores e iguales calificaciones. Conclusión: Es necesario investigar los efectos identificados agregando la evaluación de especialistas para mayor precisión de los hallazgos observados.


Introduction: COVID-19 has affected the mental health of the pediatric population. With the transition to virtuality, positive and negative positions have been identified. Due to the difference in sociodemographic characteristics that the Dominican Republic has, it is necessary to describe the effect of these changes on mental health within this context. Objective: To assess the effect of virtual classes on the mental health of pediatric patients. Methodology: Strengths and Difficulties Questionnaire was used in the patients who attended the general consultation. A score was assigned using the standardized scoring scale and the results were analyzed in frequency plots and spearman correlation tests. Results: There is a frequency of 10.4% (n=34) of high risk of developing some psychiatric disorder. Weak correlations were observed between younger age and conduct disorder, r(365) = -0.111, p = 0.034, and male gender and any pediatric disorder, r(365) = -0.131, p = 0.012. There is a 51.5% incidence of physical symptoms. 45.5% and 30.2% identified better and equal qualifications. Conclusion: It is necessary to investigate the effects identified adding the evaluation of specialists for greater precision of the observed findings.

O linfoma difuso de grandes células B (LDGCB) é o subtipo mais comum de linfoma não Hodgkin. A recaída em sistema nervoso central (SNC) é um evento raro, variando de 5% a 10%, de acordo com fatores de risco previamente definidos através do Índice Prognóstico Internacional do SNC (CNS-IPI) e sítios extranodais específicos. Apresenta desfechos insatisfatórios, com sobrevida global mediana de dois a cinco meses. Ao longo dos anos, diversas estratégias para reduzir a recaída em SNC foram avaliadas, e são cada vez mais controversas. As profilaxias para evitar recaída em SNC frequentemente utilizadas diferem na forma de administração, baseados em metotrexato intratecal (IT-MTX) ou de forma sistêmica em altas doses (HD-MTX), associado ou não a outros agentes quimioterápicos. Os estudos até então disponíveis foram realizados em países de alta renda e é questionado se limitações encontradas em países de transição econômica, com maior dificuldade de acesso a métodos diagnósticos e terapêuticos, trariam impacto ou poderiam justificar profilaxia para recaída em SNC. Realizamos um estudo retrospectivo em dois centros de saúde pública em Belo Horizonte, Brasil, entre janeiro de 2018 e julho de 2022, para avaliar a incidência de recaída em SNC em pacientes acometidos por LDGCB. Estimamos sobrevida livre de progressão e sobrevida global. Um total de 120 pacientes, com idade média de 54,4 ± 15,4 anos e predomínio do sexo masculino (60,0%) foram avaliados no estudo. Destes, apenas sete (5,8%) receberam IT-MTX e quatro (3,3%) receberam HD-MTX. Não houve pacientes que receberam as duas vias de profilaxia. O escore prognóstico para risco inicial de recaída do SNC pelo CNS-IPI foi estimado como: baixo [0-1; 37 (30,8%)], moderado [2-3; 53 (44,2%)] ou alto [≥ 4; 27 (22,5%)]. A recaída em SNC foi confirmada em quatro (3,3%) pacientes. Apesar do estudo ter sido realizado em centros de referência oncohematológicas, o n disponível foi pequeno ao considerar a raridade do evento. Não conseguimos demonstrar se há benefício ou não de profilaxia específica para recaída em SNC. Considerando a morbimortalidade desta complicação, sugere-se realizar mais estudos e investigar acometimento oculto de SNC em LDGCB ao diagnóstico.


Diffuse large B-cell lymphoma (DLBCL) is the most common subtype of non-Hodgkin lymphoma. Central nervous system (CNS) relapse is a rare event, varying from 5% to 10%, according to risk factors previously defined through the CNS International Prognostic Index(CNS-IPI) and specific extranodal sites. It presents unsatisfactory outcomes, with a median overall survival of two to five months. Over the years, several strategies to reduce CNS relapse have been evaluated, and they are increasingly controversial. Prophylaxis to prevent CNS relapse frequently used differs in the form of administration, based on intrathecal methotrexate (IT-MTX) or high-dose systemic (HD-MTX), associated or not with other chemotherapeutic agents. The studies available so far were carried out in high-income countries and it is questioned whether limitations found in economic transition countries, with greater difficulty in accessing diagnostic and therapeutic methods, would have an impact or could justify prophylaxis for CNS relapse. We carried out a retrospective study in two public health centers in Belo Horizonte, Brazil, between January 2018 and July 2022, to evaluate the incidence of CNS relapse in patients affected by DLBCL. We estimated progression-free survival and overall survival. A total of 120 patients, with a mean age of 54.4 ± 15.4 years and a predominance of males (60.0%) were evaluated in the study. Of these, only seven (5.8%) received IT-MTX and four (3.3%) received HD-MTX. There were no patients who received both routes of prophylaxis. The prognostic score for initial risk of CNS relapse by CNS-IPI was estimated as: low [0-1; 37 (30.8%)], moderate [2-3; 53 (44.2%)] or high [≥ 4; 27 (22.5%)]. CNS relapse was confirmed in four (3.3%) patients. Although the study was carried out in oncohematological reference centers, the number available was small considering the rarity of the event. We were unable to demonstrate whether or not there is benefit from specific prophylaxis for CNS relapse. Considering the morbidity and mortality of this complication, it is suggested to carry out further studies and investigate occult CNS involvement in DLBCL at diagnosis.
