ABSTRACT

Se describe el caso clínico de un paciente de 62 años de edad, quien fue atendido en la consulta de dermatología del Policlínico Aquiles Espinosa Salgado de Las Tunas, por presentar una lesión blanca, de aspecto perlado, de 0,5 cm, en la punta de la lengua. Teniendo en cuenta las características de dicha lesión, se estableció el diagnóstico de sospecha de sífilis secundaria. Se le indicó la prueba serológica para la detección de sífilis, cuyo resultado fue de 128 diluciones, por lo que se confirmó que se trataba de una sifílide opalina, como manifestación de la sífilis en estadio secundario. Se efectuó el control de foco y la fuente de contagio estaba aún sin precisar. Con el tratamiento indicado evolucionó favorablemente, pero se mantuvo el seguimiento clínico y serológico según lo establecido.


The case report of a 62 years patient who was assisted in the Dermatology Service of Aquiles Espinosa Salgado Polyclinic in Las Tunas is described, who presented a 0.5 cm white lesion, of pearly aspect, in the tip of the tongue. Taking into account the characteristics of this lesion the suspected diagnosis of secondary syphilis was established. The venereal disease research laboratory test was ordered and its result was 128 dilutions, reason why an opaline syphilid was confirmed, as a manifestation of syphilis in secondary stage. The focus control was carried out and the source of infection was still without specifying. With the suitable treatment, he had a favorable clinical course, but clinical and serologic follow up was maintained as established.

RESUMEN Introducción: La sífilis es una infección de transmisión sexual causante de grandes estragos durante toda la historia de la humanidad, en la actualidad su prevalencia es preocupante. El compromiso óseo en la sífilis secundaria puede preceder o seguir a las lesiones mucocutáneas y en algunos casos ocurrir como única manifestación. Objetivo: Describir el caso clínico de un paciente con osteítis de calota en el curso de una sífilis secundaria, que permita a los profesionales de la salud asociar estas lesiones con las infecciones de transmisión sexual. Presentación de caso: Paciente masculino de 33 años de edad, estado civil soltero, con antecedentes patológicos personales de ser portador del Virus del herpes simple oral desde los 24 años, quien asiste a la consulta por presentar cefalea frontal izquierda, opresiva, pulsátil, de inicio leve que se intensifica e irradia al oído homolateral. El examen físico, que incluyó el sistema nervioso, fue negativo. Se indicaron estudios imagenológicos y de laboratorio, los que evidenciaron la presencia de imágenes osteolíticas en región parietal izquierda próxima a la sutura sagital y pruebas serológicas positivas que confirman el diagnóstico de manifestaciones óseas de osteítis de calota por sífilis secundaria. Conclusiones: Durante la sífilis secundaria el hueso del cráneo es el más afectado, al sufrir osteítis proliferativa. Es responsabilidad del personal de salud el diagnóstico oportuno y precoz de la enfermedad, a fin de evitar su progresión, morbilidad y posibles complicaciones.


ABSTRACT Introduction: Syphilis is a sexually transmitted infection that has caused great havoc throughout the history of mankind; currently, its prevalence is of great concern. Bone involvement in secondary syphilis may precede or follow mucocutaneous lesions and, in some cases, occur as the only manifestation of the disease. Objective: To describe the clinical case of a patient with calvarial osteitis in the course of secondary syphilis to allow health professionals to associate these lesions with sexually transmitted infections. Case presentation: Thirty-three-year-old male patient, single, with personal pathological antecedents of being carrier of the oral herpes simplex virus since he was 24 years old who attends the consultation due to left frontal, oppressive, throbbing headache of mild onset that intensifies and radiates to the ipsilateral ear. Physical examination, which included the nervous system, was negative. Imaging and laboratory studies were ordered; the imaging studies showed the presence of osteolytic images in the left parietal region near the sagittal suture, and the positive serological tests confirmed the diagnosis of bone manifestations of calvarial osteitis due to secondary syphilis. Conclusions: In secondary syphilis, the skull bone is most often affected by proliferative osteitis. Risky sexual practices and self-medication have a negative influence on the expression of the disease. The health personnel is responsible for conducting a timely and early diagnosis of the disease in order to avoid its progression, morbidity, and possible complications.

La dermatosis IgA lineal del adulto es una enfermedad que pertenece al grupo de las dermatosis ampollares adquiridas. De etiología desconocida e infrecuente, se presenta en adultos mayores y se asocia a múltiples fármacos, neoplasias y enfermedades autoinmunes e infecciosas. Se presenta el caso de un paciente con una dermatosis IgA lineal asociada a una sífilis secundaria.


Linear bullous IgA dermatosis of adult is a disease that belongs to the group of acquired bullous dermatoses. It is of unknown aetiology and infrequent in adults and is associated with drugs, neoplasms, autoimmune and infectious diseases. We present a case of a male patient in whom a linear IgA dermatosis and secondary syphilis coexist.

ABSTRACT

La sífilis es una enfermedad de transmisión sexual causada por una espiroqueta, Treponema palidum. Presentamos el caso de una mujer de 62 años de edad, que consultó por fiebre de 38°, dolor torácico en puntada de costado y lesiones pruriginosas en piel. Se realizó examen de laboratorio de sangre periférica, radiografía y tomografía de tórax. Recibió tratamiento antibiótico y fue diagnosticada como neumonía aguda de la comunidad. Debido a la respuesta parcial de los síntomas y persistencia de lesiones pruriginosas se realizó biopsia de piel que informó Treponema palidum, el cual fue confirmado con test serológico VDRL y FtA-abs positivo. La paciente recibió 4 dosis de penicilina G benzatínica con favorable evolución de las lesiones en piel y mejoría de las imágenes radiológicas.


We present the case of a 62-year-old woman who consulted for fever (38°), stabbing thoracic pain (on one side), and pruritic skin lesions. She underwent peripheral blood tests, chest X-rays and CT. Her symptoms were interpreted as severe community-acquired pneumonia. After a treatment with antibiotics, her skin lesions persisted, and other symptoms were only partially relieved. A skin biopsy was performed, which revealed Treponema pallidum. Such finding was confirmed through positive serum VDRL and FTA-ABS tests. The patient received 4 doses of benzathine penicillin G with favorable evolution of skin lesions and improvement of radiological images.

ABSTRACT

RESUMEN La sífilis es una infección de transmisión sexual causada por la bacteria Treponema pallidum. En los últimos años ha habido un aumento en la incidencia de esta enfermedad debido a la creciente infección por el virus de la inmunodeficiencia humana (VIH) e inmunodepresión. Conocida como "la gran imitadora" son muchos los signos y síntomas que puede simular, siendo la presentación orofaríngea infrecuente. Presentamos el caso de un varón de 31 años de edad que acude a nuestro servicio por clínica de otitis media serosa bilateral. En la exploración física se objetiva una masa en cavum y una ulceración en pared faríngea posterior que se biopsia en consultas externas siendo el resultado un intenso infiltrado inflamatorio crónico de probable origen infeccioso, por lo que se decidió tomar una nueva biopsia bajo anestesia general. El día de la intervención, el paciente refirió la aparición de un exantema eritematoso generalizado, pero de predominio palmo-plantar. Interrogado sobre sus antecedentes, el paciente reconoció conductas sexuales de riesgo y ser portador VIH, por lo que se orientó el diagnóstico hacia una probable sífilis secundaria que se confirmó posteriormente mediante serología. Las lesiones de orofaringe, la masa del cavum y el exantema remitieron con tratamiento antibiótico y corticoideo pautado. Cuando las manifestaciones de cabeza y cuello constituyen la presentación inicial de la sífilis, su diagnóstico se retrasa con frecuencia debido al desconocimiento por parte del médico de primaria, e incluso del otorrinolaringólogo, de las formas típicas de presentación en esta localización. Su diagnóstico precoz es fundamental para prevenir la extensión de la enfermedad.


ABSTRACT Syphilis is a sexually transmitted infection caused by the bacteria Treponema pallidum. Over the last years there has been an increase of the incidence of this disease due to the growing infection by the human inmunodeficiency virus (HIV) and the inmunosupression. Known as "the great imitator", there are many signs and symptoms that can simulate, being the oropharyngeal presentation infrequent. We present the case of a 31-year-old man who came to our service for bilateral otitis media. Physical examination revealed a mass in the cavum and ulceration in the posterior pharyngeal wall that was biopsied in the outpatient clinic. The result was an intense chronic inflammatory infiltrate of probable infectious origin, so it was decided to take a new biopsy under general anesthesia. On the day of the intervention, the patient reported the appearance of a generalized erythematous rash, but predominantly on the bottoms of the feet and on the palms of the hands. Questioned about his background, the patient recognized risky sexual behavior and being an HIV carrier, so the diagnosis was oriented towards a probable secondary syphilis that was subsequently confirmed by serology. The ulcerations of the oropharynx, the mass of the cavum and the rash subsided with antibiotic treatment and corticoid regimen. When the manifestations of the head and neck are the initial presentation of syphilis, its diagnosis is often delayed due to the lack of knowledge of the physicians of the typical presentation forms in this location. Early diagnosis is essential to prevent the spread of the disease and its complications.

ABSTRACT

Se describe el caso clínico de una paciente de 38 años de edad, quien fue atendida en la consulta estomatológica del Policlínico Cuatro Caminos, por presentar múltiples lesiones indoloras en casi toda la mucosa bucal. Teniendo en cuenta estos síntomas se le diagnosticó una estomatitis aftosa recurrente, pero el tratamiento indicado no resultó satisfactorio. Luego se interconsultó con el especialista en Medicina Interna, quien le indicó exámenes complementarios y, según los criterios clínicos y de laboratorio, se confirmó la presencia de sífilis secundaria. Se efectuó el control de foco y se logró la remisión de la enfermedad.


The case report of a 38 year-old patient who was assisted in the stomatological department of Cuatro caminos polyclinic is described due to multiple painless lesions in almost all the oral mucous. Keeping in mind these symptoms a recurrent aphthous stomatitis was diagnosed, but the prescribed treatment was not satisfactory. Then a consultation with the specialist in Internal Medicine was carried out, who indicated her complementary exams and, according to the clinical and laboratory criteria, the presence of secondary syphilis was confirmed. The focus control was carried out and the remission of the disease was achieved.

@#Nodular secondary syphilis is a rare condition with only 12 cases reported from 1980 to 20055 . Cutaneous nodular granulomatous lesions, a positive serologic test for syphilis, and rapid resolution of symptoms with administration of Benzathine Penicillin are usually seen in previous cases 3,4,5,6. We report a case of a 32-year old male with clinical manifestations and laboratory findings consistent with nodular secondary syphilis, and concurrent manifestations of primary and secondary stages. This is the first documented case in East Avenue Medical Center and possibly in the Philippines, highlighting the need for meticulous historytaking, physical examination and laboratory work-up to determine the correct diagnosis.

ABSTRACT

Objective To investigate the expression of TP 0155 mRNA in rabbits with early infection of Treponema pallidum ( T.pallidum ). Methods Three New Zealand white rabbits were subcutaneously injected with T.pallidum Nichols Seattle strains.Each rabbit was inoculated at ten sites with 106 T.pallidum/site.Skin lesions at the primary stage of syphilis were observed at different time points. Biopsy from one of the lesions was obtained from each rabbit every three days for detection of TP 0155 mRNA and house keeping gene TP 0574 mRNA.TP0155 plasmid standard was constructed by molecular cloning technique , and the quantitative PCR was used to continuously detect the expression of TP 0155 mRNA and TP0574 mRNA from lesion at different time points.Kruskal Wallis test and Bonferroni method were used to analyze the data.Results On d6, red papules appeared on the dorsal skins of rabbits ,there were ulcers in the center of the lesions on d19,presenting typical appearance of syphilis chancre.On d24 the scab of ulcer became smaller; on d25 the rabbits showed disseminated secondary syphilis , which became smaller and disappeared on d30.The copy numbers of TP0155 plasmid standards were 7.48 ×109 copies/μL.There were significant differences in expression of both TP 0155 mRNA and TP0574 mRNA at different time points (χ2 =32.756 and 52.344,both P<0.01).The expression levels of TP0155 mRNA and TP0574 mRNA increased in the early stage, and both reached the peak at d15 (both P<0.05), and then rapidly declined. There were significant differences in normalized TP 0155 mRNA ( TP0155 ×1000/TP0574 mRNA ) at different time points(χ2 =19.758,P<0.05),which reached the peak on d24 and d30,respectively (all P<0.05).Conclusion The level of TP0155 mRNA increases with the disappearance of chancre and secondary syphilis lesions , suggesting that TP0155 might be involved in immune escape of T.pallidum.

ABSTRACT

Abstract Syphilis is an infectious disease caused by the bacterium Treponema pallidum. Syphilis has three clinical stages and may present various oral manifestations, mainly at the secondary stage. The disease mimics other more common oral mucosa lesions, going undiagnosed and with no proper treatment. Despite the advancements in medicine toward prevention, diagnosis, and treatment syphilis remains a public health problem worldwide. In this sense, dental surgeons should be able to identify the most common manifestations of the disease in the oral cavity, pointing to the role of this professional in prevention and diagnosis. This study describes a case series of seven patients with secondary syphilis presenting different oral manifestations.

ABSTRACT

<p><b>Objective</b>To analyze the gene expression profiling characteristics of peripheral blood mononuclear cells (PBMCs) from patients with secondary syphilis, and gain an insight into the host molecular immune mechanisms involved in Treponema pallidum infection.</p><p><b>METHODS</b>Using genome-based high-throughput Illumina sequencing technology, we comprehensively determined the transcriptional difference in PBMCs from 4 secondary syphilis patients and 4 healthy controls, followed by real time PCR to validate the results of Illumina sequencing.</p><p><b>RESULTS</b>Totally, 78 differentially expressed genes were found in the PBMCs of the secondary syphilis patients, among which 16 were associated with the immune system. Significant upregulation was observed in the expressions of pro-inflammatory cytokines and related receptors, such as TNF receptor super-family member 17 (TNFRSF17), IL-17C, IL-21, IL-31 receptor A (IL-31RA), chemokine C-X-C motif ligand 10 (CXCL10), and chemokine (C-C motif) ligand 1 (CCL1), as well as the transcripts for the CD4+ T lymphocytes activation markers CD38, Fc-mediated phagocytosis receptors (FcγR1A, FcγR3B), and complement (C2, SERPING1).</p><p><b>CONCLUSIONS</b>Systemic innate and adaptive immune effecter molecules are involved in the host clearance mechanism of secondary syphilis.</p>

ABSTRACT

"Leukoplakia-like" plaque on the tongue is an uncommon skin manifestation of secondary syphilis. Skin lesions of secondary syphilis usually have numerous presentations, which mimic many other skin diseases, especially in the presence of HIV co-infection. Oral hairy leukoplakia (OHL) is characterized by corrugated whitish patches and plaques on the lateral border of the tongue. It is frequently and strongly associated with HIV but may appear in other diseases. A 47-year-old man with HIV and receiving HAART therapy developed a leukoplakia-like plaque on the tongue, which was first suggestive of OHL but was eventually diagnosed as secondary syphilis.

ABSTRACT

Sífilis é uma doença sistêmica e infecto-contagiosa, causada pelo Treponema pallidum. É dividida em duas formas: recente e tardia. A forma dita recente é subdividida em primária, secundária e latente. A forma tardia envolve comprometimento multissistêmico. A incidência e prevalência da sífilis decresceram em vários países após a introdução da penicilina em 1943. Após a década de 60, observou-se incremento de ambas, seguindo a trajetória de outras doenças sexualmente transmissíveis. A despeito dessa tendência, a sífilis é pouco considerada no diagnóstico diferencial de doenças sistêmicas, sobretudo em idosos. O objetivo deste trabalho foi relatar um caso de sífilis secundária com evolução "florida" em paciente idosa, enfatizando seus aspectos clínicos, evolutivos e terapêuticos, no intuito de ressaltar a importância de se considerar essa patologia no diagnóstico diferencial de doenças sistêmicas em idosos com histórico epidemiológico. É importante alertar os médicos que lidam com idosos acerca da necessidade DE se considerar essa patologia no diagnóstico diferencial de doenças sistêmicas nessa população, bem como orientar os pacientes com relação às medidas preventivas e, diante do diagnóstico, instituir terapêutica adequada de forma a impedir manifestações tardias da doença...

Sexually transmitted diseases (STDs) are posing a growing public health problem on a global and national scale. Syphilis is one of the common STD. The dentist is most likely to come across associated oral manifestation of the disease. This is a case report of 60-year-old male patient who presented with multiple erythmatous lesions with shallow ulcerations and crusted appearance on the lip, mimicking few mucocutaneous lesions of oral cavity. He gave history of sexual exposure;further work up revealed it to be a case of secondary syphilis.

ABSTRACT

A 32-year-old man is being presented with a 1-week history of erythematous annular flat plaque with scales on his scalp. He displays only a solitary skin lesion on his scalp without any presence or other histories for other skin lesions or constitutional symptoms. It is difficult to consider the possibilities of syphilis based on such clinical findings. A diagnosis of syphilid is being made on the basis of clinical, serological and histological findings. Herein, we report an interesting case of secondary syphilis which is presented as a solitary annular plaque on the scalp.

ABSTRACT

Although, erythema nodosum is a common skin manifestation associated with syphilis, nodular vasculitis is a rare feature. Here, we describe a case of a 22-year-old, human immunedeficiency virus negative, non-immunocompromised man who developed recurrent oral and scrotal ulcers with nodular lesions of the lower extremitie. Behcet's disease was initially suspected, however, his serologic test for syphilis was positive, and he was thus diagnosed with secondary syphilis, with a skin biopsy showing nodular vasculitis. The patient was treated with benzathine penicillin, and the skin lesions disappeared after treatment.

JUSTIFICATIVA E OBJETIVOS: Na última metade do século XX, o advento da penicilina e a maior atenção às iniciativas que visam prevenção das doenças sexualmente transmissíveis representaram um grande avanço no combate à sífilis. No entanto, a sua prevalência no Brasil ainda é elevada e constitui um importante problema de saúde pública. O objetivo deste estudo foi relatar a apresentação da variedade das manifestações clínicas da sífilis secundária, incluindo formas incomuns e reflexos de sua associação com o vírus da imunodeficiência humana (HIV). RELATO DOS CASOS: São apresentados e discutidos quatro casos de lues secundária tratados com penicilina benzatina, sendo cada um com clínica peculiar: lesões palmo-plantares em conjunto com pápulas disseminadas pelo corpo, sifilides elegantes, sífilis maligna precoce e alopécia. Sintomatologia sistêmica também é discutida quando presente, assim como a relação entre a coinfecção com o HIV e provas laboratoriais. CONCLUSÃO: Observou-se neste estudo a capacidade de polimorfismo das lesões dermatológicas da sífilis secundária reforçando a expressão que a doença recebe de "o grande imitador" e destaca a necessidade de refamiliarização para com as diversas formas clínicas do secundarismo das lues.


BACKGROUND AND OBJECTIVES: In the last half of 20th century, the advent of penicillin and more attention to initiatives aimed at prevention of sexually transmitted diseases represent a major breakthrough in the fight against syphilis. However, the prevalence of syphilis in Brazil is still high and so it is an important public health problem. The main purpose of these reports is the presentation of the variety of clinical manifestations of secondary syphilis, including unusual presentations and aspects of its association with human immunodeficiency virus (HIV). CASE REPORTS: Presented and discussed four cases of secondary syphilis treated with penicillin G benzathine, each with thefollowings presentations: palmoplantar papules and disseminated lesions, syphilids elegant, early malignant syphilis and alopecia. Systemic symptoms when present are also discussed, as well as the relationship between co-infection with HIV and laboratory tests. CONCLUSION: The study reflects the polymorphism of the skin lesions of secondary syphilis enhancing the expression that the disease was "the great imitator" and highlights the need to re-familiarization with the various forms of secondary syphilis.

ABSTRACT

Se presentó un paciente de 40 años, con antecedentes patológicos personales de sepsis urinaria a repetición en los últimos 2 años, que 1 mes antes del ingreso comenzó con decaimiento, pérdida de peso y del apetito, dolor abdominal difuso y orinas oscuras. Se describió la evolución clínica y los estudios realizados mediante los cuales se le diagnosticó sífilis secundaria con afectación hepática y dermatológica


A case is presented of a male 40 year-old patient with a history of recurrent urinary sepsis for the last 2 years. One month before his admission, he started out with weakness, appetite suppression and weight loss, diffuse abdominal pain and dark urine. A description is provided of the patient's clinical evolution and the studies performed, by which he was diagnosed with secondary syphilis with hepatic and dermatologic manifestations

ABSTRACT

A sorologia não treponêmica possui grande valor no diagnóstico e acompanhamento terapêutico da sífilis, porém pacientes coinfectados com o vírus da imunodeficiência humana podem desenvolver respostas que suscitem dúvidas quanto à sua interpretação em relação aos resultados, podendo ser falso-negativas ou falso-positivas. Assim, os clínicos devem estar atentos a manifestações dermatológicas indicativas de sífilis, dando continuidade à conduta diagnóstica, de forma a não retardar o tratamento, evitando maiores danos ao paciente. Este relato avalia a conduta adotada frente a um paciente com clínica sugestiva de sífilis secundária com VDRL inicialmente negativo e HIV-positivo, desenvolvendo, após introdução da penicilina, títulos crescentes de VDRL.


The nontreponemal serology have great value in the diagnosis of secondary syphilis, but the patients coinfected with human immunodeficiency virus may develop abnormal responses before antigenic stimulation and therefore produce false-negative serologic responses or some false-positive infections,including syphilis. Thus, clinicians should be alert to skin lesions suggestive of syphilis and proceed performing diagnostic tests, and not delay treatment to avoid further damage to the patient. This report assesses the conduct adopted front of the patient with symptoms suggestive of secondary syphilis with VDRL initially negative and HIV positive, developing, after the introduction of penicillin, increasing titers of VDRL.

ABSTRACT

La sífilis secundaria se caracteriza por su gran variabilidad clínica, la que puede simular muchas otras enfermedades, retrasando su diagnóstico. Las manifestaciones secundarias desaparecen sin tratamiento en varias semanas o meses. Un 25% de los pacientes no tratados presentan recidiva de las lesiones de secundarismo. Se presenta el caso de una paciente de sexo femenino, con lesiones eritematosas de bordes levemente infiltrados, que se localizan en región palmar y dorso de pie de varios meses de evolución, que luego de realizar análisis clínicos y estudio histopatológico se confirmó el diagnóstico de sífilis secundaria de recidiva.


Secondary syphilis is characterized by great clinical variability, which can simulate many other diseases, delaying his diagnosis. The secondary manifestations disappear without treatment in several weeks or months. 25% of untreated patients present recurrence of lesions by secondary syphilis. It presents the case of a female with injuries erythematous slightly infiltrated edges, located in palmar region and dorsum of foot of several months of development, which after clinical analysis and study histopathologic the secondary syphilis of relapse diagnosis was confirmed.
