ABSTRACT

SUMMARY: Temporomandibular joint dysfunction interferes with the quality of life and activities of daily living among patients. The symptoms of temporomandibular dysfunction, including pain and clicking and popping sounds, are worsened during stressful events, and patients report increased pain around the temporomandibular joint. Stress-related behaviors, such as teeth clenching and teeth grinding, are commonly reported as increasing during stress. The prevalence of temporomandibular dysfunction and stress-related behaviors is reported differently in the literature. Stress in higher education is common. The purpose of this pilot study was to investigate the prevalence of temporomandibular joint dysfunction and stress-related behaviors among staff members at a local University. The study also sought to explore pain patterns described by people experiencing temporomandibular joint dysfunction and the relationship between stress-related behaviors and pain symptoms experienced. Further, the impact of stress on symptoms experienced by people with temporomandibular dysfunction was investigated in this pilot study.


La disfunción de la articulación temporomandibular interfiere con la calidad de vida y las actividades de la vida diaria entre los pacientes. Los síntomas de la disfunción temporomandibular, incluidos el dolor y los chasquidos, empeoran durante los eventos estresantes, y los pacientes informan un aumento del dolor alrededor de la articulación temporomandibular. Los comportamientos relacionados con el estrés, como apretar y rechinar los dientes, suelen aumentar durante el estrés. La prevalencia de la disfunción temporomandibular y los comportamientos relacionados con el estrés se informa de manera diferente en la literatura. El estrés en la educación superior es común. El propósito de este estudio piloto fue investigar la prevalencia de la disfunción de la articulación temporomandibular y los comportamientos relacionados con el estrés entre los miembros del personal de una universidad local. El objetivo del estudio además fue explorar los patrones de dolor descritos por personas que experimentan disfunción de la articulación temporomandibular y la relación entre los comportamientos relacionados con el estrés y los síntomas de dolor experimentados. Además, en este estudio piloto se investigó el impacto del estrés en los síntomas que experimentan las personas con disfunción temporomandibular.

ABSTRACT

With the maturity of kidney transplantation, introduction of new immunosuppressive drugs and improvement of immunosuppressive regimen, the short-term survival rate of kidney transplant recipients has been significantly improved, whereas the long-term survival rate has not been significantly elevated. Kidney transplant recipients may have the risk of renal graft loss. Clinical management after renal graft loss is complicated, including the adjustment of immunosuppressive drugs, management of renal graft and selection of subsequent renal replacement therapy. These management procedures directly affect clinical prognosis of patients with renal graft loss. Nevertheless, relevant guidelines or consensuses are still lacking. Clinical management of patients after renal graft loss highly depend upon clinicians’ experience. In this article, the adjustment of immunosuppressive drugs, management of renal graft and selection of subsequent renal replacement therapy were reviewed, aiming to provide reference for prolonging the survival and improving the quality of life of these patients.

ABSTRACT

Lung cancer is the fastest-growing cancer type in terms of incidence and mortality worldwide, posing a huge threat to the health and life of the population. Radiation therapy is one of the main methods for treating lung cancer, and there is a clear dose-effect relationship between the radiation dose and local control rate of lung cancer. However, the lung is a radiation dose-limiting organ, and the radiation resistance of lung cancer tissues and the radiation damage to normal tissues limit the radiation efficacy for lung cancer. The pathogenesis of lung cancer in traditional Chinese medicine (TCM) is characterized by an initial deficiency in vital Qi, followed by the internal invasion and gradual accumulation of pathogenic Qi. After radiation therapy for lung cancer, the body's vital Qi becomes weaker, and syndromes of phlegm coagulation, Qi stagnation, and static blood blocking collaterals become more severe, leading to radiation resistance of lung cancer tissues. Therefore, the key issue to better clinical efficacy of radiation therapy for lung cancer patients is to use drugs to enhance the radiation sensitivity of lung cancer cells and improve the radiation tolerance of normal lung tissues. TCM can be used as a radiation sensitizer by regulating the cell cycle to increase the proportion of cells in the radiation-sensitive phase, promoting upregulation of pro-apoptotic genes and downregulation of anti-apoptotic genes to induce cell apoptosis, enhancing DNA damage caused by radiation and inhibiting damage repair, improving blood circulation and tissue oxygen supply, and so on, to enhance the sensitivity of tumor cells to radiation and amplify the toxicity of radiation to tumor tissues. TCM can also be used as a radiation protector by inhibiting cell damage, regulating cytokines and immune balance, reducing the release of inflammatory and fibrotic factors, and inhibiting the activation of related signaling pathways to prevent and treat radiation-induced lung injury. This article systematically reviewed the research results of TCM on radiation sensitization and radiation protection in lung cancer in recent years, aiming to elucidate the mechanism of TCM in regulating the effect of radiation therapy for lung cancer and provide more theoretical and practical basis for TCM to participate in improving the prognosis of lung cancer patients undergoing radiation therapy.

Food allergy is a common allergic disease threatening the growth and development of infants and children.n-3 polyunsaturated fatty acids (PUFAs) are common nutrients in the diet, which have important structural functions and immunomodulatory effects.Their protective effect in food allergy has gradually become a potential research hotspot.This review highlights the function and immune regulation of PUFAs, the regulation of n-3 PUFAs on immunological indexes, the mechanism of food allergy, and the relationship between food allergy, and the impact of n-3 PUFAs on other allergic diseases.

ABSTRACT

[Introduction] We report a case of patient with migraine that improved after acupuncture and additional treatment for comorbidities of dry eye.[Case presentation] Subject was a 59-year-old female patient who was 156 cm tall and 49 kg in weight (BMI: 20.1). Her blood pressure was 122/69 mmHg, and her pulse was 70 beats per minute. She had suffered from headaches for about ten years and had been previously diagnosed with migraine. Due to the worsening of the headaches, she began acupuncture treatments. Since the onset of the headaches, she was also diagnosed with dry eye.[Treatment] At the initial visit, the subject's Headache Impact Test (HIT-6) score was 65. Manual acupuncture in her back and neck area and C2 peripheral nerve field stimulation with electroacupuncture were performed for migraine. At week four of treatment, the HIT-6 score was 60 and the headaches appeared 14 times a month, which improved to 48 and 7, respectively, at week 12. However, her headaches worsened after treatment interruption. At week 40, the HIT-6 score and headache frequency worsened and increased to 60 and 13, respectively. Acupuncture treatment for migraine was resumed, and treatment for comorbidities of dry eye, that included manual acupuncture around the orbits and self-care, was also performed. Subsequently, her headaches ameliorated significantly. At week 56, the HIT-6 score was 48 and the headaches appeared only five times a month. Migraine relief persisted and the symptoms of dry eye also improved.[Discussion and Conclusion] The additional dry eye treatment had a positive effect in improving the symptoms of migraine. It has been speculated that the pathogenesis of both diseases is related to central sensitization of the optical and trigeminal nervous systems. If so, the improvement of dry eye symptoms may have had an effect on migraine relief as well.

ABSTRACT

Aim To investigate the effect of protein kinase C (PKCs) on midazolam-induced relaxation of aortic smooth muscle in spontaneously hypertensive rats (SHR) and the underlying mechanism. Methods U-sing the isolated vessel tension measurement system, the relaxant effect of midazolam on aortic smooth muscle of SHR and Wistar-Kyoto (WKY) rats was observed. After preincubation with GF109203X (GF, a broad-spectrum inhibitor of PKCs), LY333531 (LY, PKCp2 specific inhibitor) and PKC6 pseudo-substrate inhibitor (PPS), the changes of midazolam-induced relaxation amplitude were observed. Western blot was used to detect the effect of midazolam on the phosphorylation level of PKC [32 in SHR aortic smooth muscle. The effect of midazolam, LY, GF and PPS on the phosphorylation level of the key proteins (CPI-17/ MYPT1/MLC) in calcium sensitization pathway in SHR aortic smooth muscle was also examined. Results Midazolam concentration-dependently relaxed aortic smooth muscle in SHR and WKY. GF significantly inhibited midazolam-induced relaxation amplitude of SHR and WKY aortic smooth muscle. LY and PPS had no significant effect on midazolam-induced relaxation amplitude of WKY aortic smooth muscle. In contrast, LY markedly inhibited midazolam-induced relaxation amplitude of SHR aortic smooth muscle. Midazolam significantly inhibited the phosphorylation level of PKC [32 enhanced by NE in SHR aortic smooth muscle. Midazolam, LY and GF all evidently inhibited the phosphorylation level of the key proteins in calcium sensitization pathway enhanced by NE in SHR aortic smooth muscle. Conclusions Midazolam induces excessive relaxation of SHR aortic smooth muscle by inhibiting calcium sensitization pathway mediated by PKC [32.

ABSTRACT

Bacteria-mediated anti-tumor therapy has received widespread attention due to its natural tumor-targeting ability and specific immune-activation characteristics. It has made significant progress in breaking the limitations of monotherapy and effectively eradicating tumors, especially when combined with traditional therapy, such as radiotherapy. According to their different biological characteristics, bacteria and their derivatives can not only improve the sensitivity of tumor radiotherapy but also protect normal tissues. Moreover, genetically engineered bacteria and bacteria-based biomaterials have further expanded the scope of their applications in radiotherapy. In this review, we have summarized relevant researches on the application of bacteria and its derivatives in radiotherapy in recent years, expounding that the bacteria, bacterial derivatives and bacteria-based biomaterials can not only directly enhance radiotherapy but also improve the anti-tumor effect by improving the tumor microenvironment (TME) and immune effects. Furthermore, some probiotics can also protect normal tissues and organs such as intestines from radiation via anti-inflammatory, anti-oxidation and apoptosis inhibition. In conclusion, the prospect of bacteria in radiotherapy will be very extensive, but its biological safety and mechanism need to be further evaluated and studied.

ABSTRACT

Brachial plexus avulsion (BPA) is a combined injury involving the central and peripheral nervous systems. Patients with BPA often experience severe neuropathic pain (NP) in the affected limb. NP is insensitive to the existing treatments, which makes it a challenge to researchers and clinicians. Accumulated evidence shows that a BPA-induced pain state is often accompanied by sympathetic nervous dysfunction, which suggests that the excitation state of the sympathetic nervous system is correlated with the existence of NP. However, the mechanism of how somatosensory neural crosstalk with the sympathetic nerve at the peripheral level remains unclear. In this study, through using a novel BPA C7 root avulsion mouse model, we found that the expression of BDNF and its receptor TrκB in the DRGs of the BPA mice increased, and the markers of sympathetic nervous system activity including α1 and α2 adrenergic receptors (α1-AR and α2-AR) also increased after BPA. The phenomenon of superexcitation of the sympathetic nervous system, including hypothermia and edema of the affected extremity, was also observed in BPA mice by using CatWalk gait analysis, an infrared thermometer, and an edema evaluation. Genetic knockdown of BDNF in DRGs not only reversed the mechanical allodynia but also alleviated the hypothermia and edema of the affected extremity in BPA mice. Further, intraperitoneal injection of adrenergic receptor inhibitors decreased neuronal excitability in patch clamp recording and reversed the mechanical allodynia of BPA mice. In another branch experiment, we also found the elevated expression of BDNF, TrκB, TH, α1-AR, and α2-AR in DRG tissues from BPA patients compared with normal human DRGs through western blot and immunohistochemistry. Our results revealed that peripheral BDNF is a key molecule in the regulation of somatosensory-sympathetic coupling in BPA-induced NP. This study also opens a novel analgesic target (BDNF) in the treatment of this pain with fewer complications, which has great potential for clinical transformation.

ABSTRACT

Chemotherapy is an important adjuvant treatment of glioma, while the efficacy is far from satisfactory, due not only to the biological barriers of blood‒brain barrier (BBB) and blood‒tumor barrier (BTB) but also to the intrinsic resistance of glioma cells via multiple survival mechanisms such as up-regulation of P-glycoprotein (P-gp). To address these limitations, we report a bacteria-based drug delivery strategy for BBB/BTB transportation, glioma targeting, and chemo-sensitization. Bacteria selectively colonized into hypoxic tumor region and modulated tumor microenvironment, including macrophages repolarization and neutrophils infiltration. Specifically, tumor migration of neutrophils was employed as hitchhiking delivery of doxorubicin (DOX)-loaded bacterial outer membrane vesicles (OMVs/DOX). By virtue of the surface pathogen-associated molecular patterns derived from native bacteria, OMVs/DOX could be selectively recognized by neutrophils, thus facilitating glioma targeted delivery of drug with significantly enhanced tumor accumulation by 18-fold as compared to the classical passive targeting effect. Moreover, the P-gp expression on tumor cells was silenced by bacteria type III secretion effector to sensitize the efficacy of DOX, resulting in complete tumor eradication with 100% survival of all treated mice. In addition, the colonized bacteria were finally cleared by anti-bacterial activity of DOX to minimize the potential infection risk, and cardiotoxicity of DOX was also avoided, achieving excellent compatibility. This work provides an efficient trans-BBB/BTB drug delivery strategy via cell hitchhiking for enhanced glioma therapy.

Esta revisión busca proporcionar a los profesionales de la salud una mayor comprensión del dolor para su actividad clínica-asistencial. Basados en la hipóte-sis de neuroplasticidad presentada inicialmente por Ramón y Cajal y la teoría de la compuerta en la vía dolorosa presentada por Melzack y Wall, se ha ela-borado una revisión bibliográfica con el objetivo de abordar la modulación de la vía nociceptiva desde un punto de vista fisiopatológico. Asimismo, se presen-tan los principales resultados obtenidos durante los últimos años en nuestro laboratorio usando ratas Wistar hembras como modelo de dolor experimental. Finalmente, se describe un circuito original de modu-lación central a nivel del subnúcleo caudal del trigé-mino con una visión integral de los componentes del sistema nociceptivo orofacial, para ayudar al clínico a comprender situaciones de sensibilización central con perpetuación del dolor y cómo paulatinamente el sistema nervioso central pone en marcha un sistema de modulación para adaptarse y alcanzar un estado similar al basal (AU)


This review aims to provide health professionals with a better understanding of pain for their clinical-care activity. Based on the neuroplasticity hypothesis initially presented by Ramón and Cajal, and the gate theory in the pain pathway presented by Melzack and Wall, a literature review has been carried out with the aim of addressing the modulation of the nociceptive pathway from a pathophysiological point of view. The main results obtained in recent years in our laboratory using female Wistar rats as an experimental pain model are also presented. Finally, an original central modulation circuit at the level of the caudal trigeminal subnucleus is described with a comprehensive view of the components of the orofacial nociceptive system, to help the clinician to understand situations of central sensitization with perpetuation of pain and how the central nervous system gradually sets in motion a modulation system to adapt and reach a state similar to the basal one (AU)

Abstract Interventions based on pain education and self-management are dependent on factors such as attention, memory, concentration, and executive function. Objective To explore the relationship between cognitive performance and the variables pain intensity, central sensitization, catastrophizing, and hypervigilance in women diagnosed with chronic pain-related TMD. Methodology This is a cross-sectional study. A total of 33 women (mean age: 38±4.6 years; range: 18 to 66 years) with chronic pain-related TMD (myalgia and/or arthralgia) diagnosed according to the Diagnostic Criteria for Temporomandibular Disorders (DC/TMD). Specific questionnaires were used to evaluate cognitive performance, overall pain intensity, central sensitization, hypervigilance, and pain catastrophizing. The data were analyzed using Pearson's correlation coefficient and backward stepwise multiple linear regression (statistical significance at 5% alpha). Results Approximately 53% of the study sample showed decreased cognitive performance. High central sensitization, hypervigilance, and pain catastrophizing were observed. A significant negative correlation was observed between cognitive performance and hypervigilance (p=.003, r=−.49), cognitive performance and catastrophizing (p<.001, r=−.58), and cognitive performance and pain intensity (p<.001, r=−.58). Regarding the partial regression coefficients, only catastrophizing and pain intensity showed statistical significance (t=−2.12, p=.043; t=−2.64, p=.014, respectively), indicating a significant role in explaining cognitive performance at the sample. Conclusion High pain intensity and the presence of catastrophic thoughts regarding pain can predict impaired cognitive performance in women with chronic pain-related TMD. Management strategies addressing psychosocial dimensions such as reducing catastrophizing and ensuring complete understanding of the condition are important.

A nivel mundial, los aeroalérgenos más frecuentes causantes de enfermedades alérgicas son los granos de polen anemófilos. Estos han sido capaces de desencadenar crisis que han reflejado una elevada morbilidad. En Cuba los estudios de sensibilización a granos de polen han sido escasos. Con el objetivo de determinar la sensibilización a granos de polen en pacientes alérgicos y su relación con la presencia de enfermedades alérgicas, se realizó un estudio observacional descriptivo de corte transversal, no aleatorizado, de marzo a junio del 2019. La muestra se constituyó de 33 pacientes con asma, rinitis, rinoconjuntivitis alérgica, dermatitis atópica y conjuntivitis alérgica. A todos los pacientes se les realizó historia clínica alergológica y prueba cutánea por punción con extractos alergénicos de: Helianthus annus, Cosmos bipinnatus, Cynodon dactylon, Quercus sp, Eucaliptus sp. Se aplicaron las frecuencias absolutas, porcientos, desviación estándar, promedio, edad media y test de Spearman para su análisis. La edad media de la muestra fue de 36,9 años, con predominio de mujeres. Más del 50por ciento de los pacientes presentaron sensibilización a granos de polen; y de ellos, el 24,24por ciento resultaron polisensibilizados. El mayor porcentaje de sensibilización fue a Cynodon dactylon. La rinitis alérgica fue la enfermedad que prevaleció en la población estudiada(AU)


Nowadays, the most frequent aeroallergens causing allergy diseases have been anemophilous pollen grains. They have been able to triggers crises that have reflected a high morbidity. In Cuba, studies of sensitization to pollen grains have been scarce. The objective of our research was to determine the sensitization to pollen grains in allergic patients and its relationship with the presence of allergic diseases. A non-randomized, descriptive, cross-sectional, observational study was conducted from March to June 2019. The sample consisted of 33 patients with asthma, allergic rhinitis, rhinoconjunctivitis, atopic dermatitis and allergic conjunctivitis. All patients underwent allergic history and skin prick test testing with allergenic extracts of: Helianthus annus, Cosmos bipinnatus, Cynodon dactylon, Quercus sp, Eucalyptus sp. Absolute frequencies, percentages, standard deviation, mean, average age, and Spearman´s test were applied for analysis. The average age of the sample was 36.9 years, with a predominance of women. More than 50percent of the patients presented sensitization to pollen grains; of them, 24.24percent polysensitized. The highest percentage of sensitization was to Cynodon dactylon. Allergic rhinitis was the disease that prevailed in the population studied(AU)

ABSTRACT BACKGROUND AND OBJECTIVES: In the teacher's professional practice, there is a high level of stress, anxiety and incidence of pain. With the advent of COVID-19 and the emergence of remote teaching, it is possible that this scenario has been aggravated. The objective of this study was to evaluate chronic pain, central sensitization and pain catastrophizing among primary education teachers during remote teaching offered due to the pandemic. METHODS: A cross-sectional analytical observational study. Participants were 200 teachers from different regions of Brazil who responded through an online form a sociodemographic questionnaire and a questionnaire about their working conditions. In addition, pain intensity was assessed using the Visual Analog Pain Scale (VAS), central sensitization state was assessed using the Central Sensitization Questionnaire (CSI), and pain catastrophizing was assessed using the Catastrophic Thinking Scale about Pain (PCS). RESULTS: Most of the teachers were female, white, from the southeast region, with lato sensu specialization, had an income of 2 to 2.5 minimum wages, working 21 to 40 hours a week. Most of the teachers reported pain intensity equal to eight and the body region most affected was the lumbar spine. It was observed that teachers with low salary, uncomfortable environment and longer days dedicated to remote teaching showed a greater tendency to central sensitization and pain catastrophizing. Most teachers reported pain intensity equal to eight and the most affected body region was the lumbar spine. Teachers with uncomfortable furniture reported increased pain, especially in the lumbar spine and neck. They also showed higher levels of central sensitization to pain, inversely proportional to their salary income and, in sum, teachers with a greater feeling of discomfort catastrophized more which reflects the physical and emotional damage that pain can cause. CONCLUSION: Remote teaching during the pandemic of COVID-19 impacted physical and emotional changes in teachers of primary education. The professionals perceived that their furniture was not the most adequate for the high amount of time they had to work on academic activities using computers (in general, over 40 hours a week), they reported increased pain, especially in the lumbar spine and neck, they presented higher levels of central pain sensitization, which was influenced by low salary income, and, in sum, teachers with higher feelings of discomfort catastrophized more, which reflects the physical and emotional damage that pain may cause. All these affections tend to reduce the teachers' quality of life and, consequently, affect the teaching and learning processes.


RESUMO JUSTIFICATIVA E OBJETIVOS: Na atuação profissional do professor, há elevado nível de estresse, ansiedade e incidência de dor. Com o advento do COVID-19 e a emergência do ensino remoto, é possível que este cenário tenha sido agravado. O objetivo deste estudo foi avaliar quadros de dor crônica, sensibilização central e catastrofização da dor em professores da rede básica durante o ensino remoto ofertado devido à pandemia. MÉTODOS: Trata-se de um estudo observacional analítico do tipo transversal. Participaram 200 professores de diferentes regiões do Brasil que responderam, através de um formulário online, perguntas sobre aspectos sociodemográficos e suas condições de trabalho, além de serem avaliadas a intensidade de dor por meio da escala analógica visual de dor, o estado de sensibilização central por meio do Questionário de Sensibilização Central (CSI) e a catastrofização da dor por meio da Escala de Pensamento Catastrófico sobre a Dor. RESULTADOS: A maioria dos professores eram do sexo feminino, brancos, da região sudeste, com especialização lato sensu, apresentavam renda de 2 a 2,5 salários-mínimos e jornada de trabalho de 21 a 40 horas semanais. A maioria dos professores relatou intensidade de dor igual a oito e a região do corpo mais afetada foi a coluna lombar. Observou-se que professores com baixo salário, ambiente desconfortável e maior jornada dedicada ao ensino remoto apresentaram maior tendência à sensibilização central e à catastrofização da dor. Professores com mobiliário desconfortável relataram aumento da dor, em especial, na coluna lombar e no pescoço. Apresentaram também maiores níveis de sensibilização central à dor, inversamente proporcional à sua renda salarial e, de maneira somatória, professores com maior sensação de desconforto catastrofizaram mais o que reflete os prejuízos físicos e emocionais que a dor pode causar. CONCLUSÃO: O ensino remoto durante a pandemia do COVID-19 impactou alterações físicas e emocionais nos professores da rede básica de ensino. Os profissionais perceberam que seu mobiliário não era o mais adequado para a alta permanência de tempo que tiveram de trabalhar em atividades acadêmicas pelo computador (em geral, acima de 40 horas semanais), relataram aumento da dor, em especial na coluna lombar e no pescoço, apresentaram maiores níveis de sensibilização central à dor, que foi influenciado por baixa renda salarial e, de maneira somatória, professores com maior sensação de desconforto catastrofizaram mais, o que reflete os prejuízos físicos e emocionais que a dor pode causar. Todos estes acometimentos tendem a reduzir a qualidade de vida do professor e, consequentemente, afetar o processo de ensino e aprendizagem.

SUMMARY OBJECTIVE: This study aimed to compare pain intensity, stiffness, functionality, central sensitization, and self-efficacy, between individuals with bilateral knee osteoarthritis and unilateral knee osteoarthritis. METHODS: We included sedentary participants with knee osteoarthritis. The diagnosis was defined by a specialist, in which there was a complaint of pain and/or altered function in the lower limbs (duration ≥3 months); morning stiffness; pain intensity ≥3; Kellgren-Lawrence 2-3° associated with X-ray; persistence of symptoms >3 months. We used the following tools: Western Ontario and McMaster Universities Arthritis Index, Numerical Pain Scale, Central Sensitization Inventory, and Pain Self-Efficacy Questionnaire. Intergroup comparisons were performed using the t-test. RESULTS: The sample consisted of 118 adult individuals, divided into two groups: bilateral knee osteoarthritis (n=59) and unilateral knee osteoarthritis (n=59). We observed a significant difference (p<0.05) and a large effect size (d≥0.8), in the comparisons between: stature, body mass index, physical function, central sensitization, and self-efficacy. CONCLUSION: Individuals with bilateral knee osteoarthritis have higher levels of central sensitization, impaired functionality, and a lower level of self-efficacy.

ABSTRACT

Objective To specify clinical and immunological parameters of the mechanisms, which may lead to development of persistent asthma, or regression of the disease symptoms. Methods Eighty children with childhood asthma, diagnosed in the past by using the modifed Asthma Predicted Index (mAPI), were divided into two groups: remission group and persistent group. There were 3 study visits (baseline, at 6 mo, and at 12 mo). Clinical remission of asthma was defned as the absence of asthma symptoms for at least 12 mo without treatment. The patients could switch from one group to another during the 12 mo of follow-up. Clinical, infammatory, and immunoregulatory predictors of asthma remission/persistence were analyzed. Results The presence of mAPI criteria as well as house dust mite (HDM) allergy and allergic rhinitis at 7–10 y, were associated with a reduced prevalence of asthma remission. The increased eosinophil blood count in mAPI criteria was associated with a lower expression of CD25 positive cells. HDM allergy was associated with a higher fractional exhaled nitric oxide (FeNO) level (p=0.0061) and higher expression of CD25CD71 (p=0.0232). Allergic rhinitis was associated with a higher expression of PPAR (p=0.0493) and CD25CD71 (p=0.0198), and lower expression of glycoprotein A repetitions predominant (GARP). Conclusions Persistence of childhood asthma was largely determined by the presence of allergic rhinitis and sensitization to HDM. Additionally, API criteria but not immunoregulation processes, were related to asthma persistence.

ABSTRACT BACKGROUND AND OBJECTIVES: The design of research with monozygotic twins discordant for the disease has emerged as a powerful tool for the detection of phenotypic risk factors. The aim of this study is to report a clinical case of monozygotic twins discordant for pain-related temporomandibular joint disorder (TMD) from a cognitive-behavioral-emotional phenotypic analysis, from the comparison of clinical variables of pain, history of exposure to painful procedures in early childhood, and coping with pain. CASE REPORT: TMD-Twin presented a diagnosis of painful (myofascial pain with referral) and joint (disk displacement with reduction) TMD according to the criteria of the DC/TMD. Control-Twin did not show TMD, however she presented other chronic pains. TMD-Twin showed reduced pressure pain threshold, hyperalgesia in trigeminal and extra-trigeminal regions compared to the Control-Twin. TMD-Twin was more exposed to painful procedures and emotional events due to congenital heart problems. Both had central sensitization based on the Central Sensitization Inventory, although TMD-Twin had more catastrophic thoughts about pain. TMD-Twin presented an internal locus of control. CONCLUSION: Both monozygotic twins presented a chronic pain phenotype, although they were discordant with the TMD-related pain. The main differences were the lower pressure pain threshold and higher hyperalgesia locally presented by TMD-Twin. The internal locus of control indicates greater pain sensitivity, with better coping of the painful experience for the TMD-Twin. One possible explanation for this clinical condition can be that painful experiences in early childhood have shaped a phenotype of greater sensitivity with better coping and resilience to the painful condition.


RESUMO JUSTIFICATIVA E OBJETIVOS: O desenho da pesquisa com gêmeos monozigóticos discordantes para a doença surgiu como uma ferramenta poderosa para a detecção de fatores de risco fenotípicos. O objetivo deste estudo foi relatar um caso clínico de gêmeas monozigóticas discordantes para disfunção temporomandibular (DTM) dolorosa a partir de análise fenotípica cognitivo-comportamental-emocional entre elas, por meio de comparação de variáveis clínicas de dor, histórico de exposição a procedimentos dolorosos na primeira infância e enfrentamento de dor (autoeficácia e lócus de controle). RELATO DO CASO: A gêmea-DTM apresentou diagnóstico de DTM dolorosa (dor miofascial com referência) e articular (deslocamento do disco com redução) segundo os critérios do Critérios de Diagnóstico para Distúrbios Temporomandibulares. A gêmea--controle não apresentou DTM, contudo apresentou manifestação clínica de outras dores crônicas. A gêmea-DTM apresentou limiar de dor à pressão reduzido, hiperalgesia em regiões trigeminais/extra-trigeminais quando comparados à gêmea-controle, que na primeira infância foi mais exposta a procedimentos dolorosos devido a problemas cardíacos congênitos. Ambas apresentaram sensibilização central de acordo com o Inventário de Sensibilização Central, embora a gêmea-DTM apresentou mais pensamentos catastróficos sobre a dor. A gêmea-DTM apresentou lócus de controle interno. CONCLUSÃO: Ambas as gêmeas apresentaram fenótipo de dor crônica, apesar do fato de serem discordantes para a DTM. Dentre as avaliações, as que mais diferiram entre o par foram o baixo limiar de dor à pressão e hiperalgesia local presentes na gêmea com DTM. O lócus de controle interno associado à maior sensibilidade indicou melhor enfrentamento da experiência dolorosa para a gêmea-DTM. Uma possível explicação para esta manifestação clínica está pautada na hipótese de que experiências dolorosas na primeira infância vivenciadas por ela tenham moldado um fenótipo de maior sensibilidade com melhor enfrentamento e resiliência frente à condição dolorosa.

Abstract Previous research has shown that exposure to nicotine and other drugs of abuse stimulate dopaminergic neurons in the mesolimbic circuit. Sustained activation of this circuit by prolonged exposure to drugs promotes locomotor sensitization. However, there are inconsistent reports about nicotine-induced locomotor sensitization when assessed among different developmental stages. We evaluated exploratory behavior on specific areas of the open field as an indicator of behavioral disinhibition and general locomotor activity as an indicator of nicotine-induced locomotor sensitization, to further explore the mechanisms underlying behavioral adaptations to nicotine exposure in animals from different developmental stages. We found that while adolescent and adult rats are equally responsive to nicotine-induced loco-motor sensitization, nicotine disrupts inhibition of risk-related behavior only in adolescent rats. Together, our results suggest that chronic daily exposure to nicotine promotes potentiation of its stimulant effects on locomotor activity. In adolescents, this effect is accompanied by a decreased capacity to inhibit risk-related behaviors under the acute effect of the drug.


Resumen Estudios previos han demostrado que exposición a la nicotina y otras drogas de abuso estimula las neuronas dopa-minérgicas del circuito mesolímbico. La activación sostenida de este circuito por exposición a las drogas promueve la sensibilización locomotriz. La evaluación de este efecto en diferentes etapas del desarrollo ha mostrado evidencia contradictoria sobre la susceptibilidad de adolescentes. En este trabajo exploramos las adaptaciones conductuales a la exposición crónica a nicotina en ratas adolescentes y adultas; para esto, evaluamos el comportamiento exploratorio en áreas específicas del campo abierto como indicador de desinhibición comportamental y el desplazamiento general como indicador de sensibilización locomotriz. Encontramos que, ambos grupos etarios muestran igual sensibilización locomotriz inducida por la nicotina y que la nicotina altera la inhibición del comportamiento relacionado con el riesgo sólo en adolescentes. Estos resultados sugieren que la exposición crónica diaria a la nicotina promueve la potenciación de sus efectos estimulantes sobre la actividad locomotriz y en los adolescentes, este efecto se acompana de una disminución de la capacidad para inhibir conductas relacionadas con el riesgo bajo el efecto agudo de la droga.

ABSTRACT BACKGROUND AND OBJECTIVES: Temporomandibular joint (TMJ) disorder is a musculoskeletal disorder that causes the exacerbation of painful sensation during function and the presence of sensitivity/pain to palpation of the masticatory muscles and/or TMJ. People with pain complaints in the head, neck, shoulder or lower back may present signs and symptoms of TMJ disorders, sharing sleep disturbances, forgetfulness or difficulty concentrating, abdominal pain and differences in fecal consistency. Studies have also shown that TMJ disorders can be associated with emotional distress and multiple comorbidities related to central sensitization (CS). This, in turn, is responsible for producing hypersensitivity to pain, altering the sensory response. The pathophysiology of CS in TMJ disorders is not yet well understood. Thus, it is the scope of this review to synthesize knowledge about the relationship between CS and Temporomandibular Disorder (TMD) and describe the comorbidities most frequently found in this profile of patients. CONTENTS: TMJ disorders do not seem to occur in isolation. Comorbidities such as migraine, tension headache, fatigue, dizziness, tinnitus and allergies have been reported. Hyperexcitability in central nociceptive processing is part of the pathophysiology of TMJ disorder, which could explain the greater sensitivity to pain in other areas of the body in these individuals, characterizing a CS process. CONCLUSION: The most frequently reported comorbid conditions are headache, psychological factors, allergies, irritable bowel syndrome, and sleep disturbances. It is admitted that CS is a neurophysiological phenomenon present in some chronic pain disorders, including TMD.


RESUMO JUSTIFICATIVA E OBJETIVOS: O transtorno da articulação temporomandibular (ATM) apresenta-se como uma desordem musculoesquelética que causa a exacerbação da sensação dolorosa durante a função e a presença de hipersensibilidade/dor à palpação da musculatura mastigatória e/ou da ATM. Pessoas com queixas álgicas na cabeça, pescoço, ombro ou lombar podem apresentar sinais e sintomas de transtornos da ATM, compartilhando de distúrbios do sono, dificuldade de concentração ou esquecimento, dor abdominal e diferenças na consistência fecal. Estudos também têm mostrado que os transtornos da ATM podem estar associados a sofrimento emocional e múltiplas comorbidades relacionadas à sensibilização central (SC). Esta, por sua vez, é responsável por produzir hipersensibilidade à dor, alterando a resposta sensorial. A fisiopatologia da SC nos transtornos da ATM ainda não está bem esclarecida. Desse modo, torna-se escopo dessa revisão sintetizar o conhecimento sobre a relação entre SC e disfunção temporomandibular e descrever as comorbidades mais frequentemente encontradas nesse perfil de pacientes. CONTEÚDO: Os transtornos da ATM parecem não ocorrer isoladamente. Comorbidades como enxaqueca, cefaleia tensional, fadiga, tontura, zumbido e alergias têm sido relatadas. Uma hiperexcitabilidade no processamento nociceptivo central faz parte da fisiopatologia da desordem da ATM, o que poderia explicar a maior sensibilidade à dor em outras áreas do corpo nesses indivíduos, caracterizando um processo de SC. CONCLUSÃO: As condições de comorbidades mais frequentemente relatadas são cefaleias, fatores psicológicos, alergias, síndrome do intestino irritável e distúrbios do sono. Admite-se que a SC é um fenômeno neurofisiológico presente em alguns distúrbios de dor crônica, incluindo as disfunções temporomandibulares.

ABSTRACT

Objective:To evaluate the effect of esketamine on the efficacy of postoperative patient-controlled intravenous analgesia (PCIA) in the patients with moderate central sensitization undergoing high tibial osteotomy.Methods:Fifty-four patients of both sexes with moderate central sensitization, aged 45-64 yr, with body mass index of 18.0-32.5 kg/m 2, of American Society of Anesthesiologists Physical Status classification Ⅰ or Ⅱ, undergoing elective high tibial osteotomy, were divided into 2 groups ( n=27 each) using a random number table method: control group (group C) and esketamine group (group ES). Ultrasound-guided femoral nerve block was performed with 0.5% ropivacaine 30 ml on the operated side at 30 min before induction of anesthesia.In C and ES groups, midazolam 0.1 mg/kg, sufentanil 0.2 μg/kg, propofol 1.5 mg/kg, and cisatracurium besilate 0.15 mg/kg were intravenously injected in turn during induction of anesthesia, and in addition esketamine hydrochloride 0.5 mg/kg was injected in ES group, and the equal volume of 0.9% sodium chloride was injected in C group, and then a laryngeal mask airway was placed.Anesthesia was maintained with intravenous infusion of remifentanil 0.1-0.3 μg·kg -1·min -1 and propofol 4-6 mg·kg -1·h -1.Esketamine hydrochloride 0.2 mg/kg was intravenously injected once every 20 min until 30 min before the end of operation in ES group, the equal volume of 0.9% sodium chloride was injected according to the amount of esketamine hydrochloride injected for the same body weight at the same time point in C group, and additional cisatracurium besilate was administered intermittently according to the degree of muscle relaxation.Intraoperative BIS values were maintained at 40-60.Postoperative PCIA was performed, and the patient was admitted to the post-anesthesia care unit.When the efficacy of PCIA was not good, ketorolac tromethamine 30 mg was intravenously injected for rescue analgesia.The intraoperative consumption of remifentanil and propofol and emergence time in the anesthesia recovery room were recorded.The pressing times of PCA and the number of rescue analgesia in each group were recorded within 2 days after operation.The Chinese Richards-Campbell Sleep Questionnaire was used to assess the nighttime sleep quality on the night of surgery and 1 and 2 days after surgery.The Chinese Quality of Recovery was used to assess the early recovery quality at 1 and 2 days after surgery.The first postoperative off-bed time and first walked distance were recorded.The adverse reactions were recorded. Results:Compared with group C, the consumption of remifentanil and propofol was significantly reduced, the emergence time in the anesthesia recovery room was prolonged, the pressing times of PCA and the number of rescue analgesia were decreased within 2 days after operation, the quality of nighttime sleep was improved on the night of surgery and 1 and 2 days after operation, the quality of early recovery on 1 and 2 days after operation was increased, the first postoperative off-bed time was shortened, the first walked distance was prolonged, and the incidence of postoperative adverse effects was decreased in group ES ( P<0.05). Conclusions:Esketamine can enhance the efficacy of postoperative PCIA in the patients with moderate central sensitization undergoing high tibial osteotomy.

ABSTRACT

Background@#Allergen sensitization, symptom severity, pulmonary function test, and bronchodilator response are important in the diagnosis and treatment of asthma. However, the relationship between these factors remains unclear.@*Objective@#The objective of this study was to investigate the relationship between aeroallergen sensitization and asthma severity, pulmonary function, and bronchodilator response among pediatric patients with bronchial asthma.@*Methods@#This was a prospective study where 155 pediatric patients aged 7–18 years old with bronchial asthma were recruited from Outpatient Clinics. Patients who met the inclusion criteria proceeded with spirometry and aeroallergen skin prick test.@*Results@#There was a significant degree of sensitization, wherein 100% of the patients had sensitization to one or more aeroallergens. Among these children, 106 (68%) were polysensitized. The polysensitized group had more severe and persistent asthma severity profile (p<0.001) and worse pulmonary function (p<0.001). The frequency of abnormal pre-bronchodilator lung function of the polysensitized group was higher than the monosensitized group (p<0.001). A positive bronchodilator response was higher among polysensitized children as compared to monosensitized children (p<0.001). Sensitization to Dermatophagoides farinae, Dermatophagoides pteronyssinus and dog correlated with impairment of both the large airways and distal small airways while sensitization to cat, cockroach, and horse correlated only with impairment of the large airways (p<0.05). Patients sensitized to D. farinae, D. pteronyssinus, cat and dog had significant bronchodilator response (p<0.05).@*Conclusion@#Polysensitized asthmatic children had a more persistent and severe asthma profile, worse pulmonary function, and higher bronchodilator reversibility compared to the monosensitized group.
