ABSTRACT

OBJECTIVE:To observe clinical efficacy and safety of sitagliptin combined with metformin in the treatment of type 2 diabetes complicating with metabolic syndrome. METHODS:Totally 80 patients with type 2 diabetes complicating with meta-bolic syndrome were divided into the observation group and control group with 40 cases in each group according to simple random sampling method. Both groups were given same diet and exercise plan;control group was additionally given metformin orally,0.5 g each time,tid;observation group was additionally given sitagliptin,100 mg each time,qd,on the basis of control group. Treat-ment course of 2 groups lasted for 12 weeks. The waist circumference(WC),body mass index(BMI),blood glucose,blood lip-id,fasting insulin (FINS) and serum adiponectin were detected in 2 groups,and steady-state model insulin resistance index (HOMA-IR)and whole body insulin sensitivity index(WBISI)were calculated. The occurrence of ADR was observed. RESULTS:Before treatment,there were no statistically significant differences between 2 groups in WC,BMI,systolic blood pressure,diastol-ic blood pressure,triglycerides,cholesterol,high-density lipoprotein cholester,low-density lipoprotein cholesterol,fasting plasma glucose,2 h postprandial blood glucose,FINS,HOMA-IR,WBISI and serum adiponectin(P>0.05). After treatment,the above indexes of 2 groups were obviously improved,and the improvement of observation group was significantly better than that of con-trol group,with statistical significance(P0.05). CONCLUSIONS:Sitagliptin combined with metformin shows significant clinical efficacy in the treatment of type 2 di-abetes complicating with metabolic syndrome. Its mechanism may be related to reducing insulin resistance,enhancing insulin sensi-tivity,decreasing patients’body weight and up-regulating serum adiponectin level.

ABSTRACT

Objective To investigate the relationship of serum adiponectin (APN) gene promoter region single nucleotide polymorphism (SNP) frequency and type 2 diabetes mellitus (T2DM).Methods 120 cases of T2DM were divided into Yin Deficiency Syndromes(n=42), Yin hot (n=38), yin and yang (n=40) and 50 cases of normal volunteers were select as the control group. The diponectin gene (aPM1) promoter polymorphisms of each group were detected with polymerase chain reaction amplification (PCR).Results Serum APN Yin hot levels in patients with T2DM (6.98 ± 1.23 μg/ml) were lower than Qi and yin (2.55 ± 0.78 μg/ml) and yang group (3.48 ± 0.22 μg/ml) (P0.05).Conclusion T2DM patients Yin hot type inherent relationship with aPM1-11377G/C polymorphism, aPM1-11377G/C polymorphism may reflect R2DM Disease Syndromes typing a certain extent, and by influence insulin resistance in patients with arterial plaque and serum APN levels, thereby affecting T2DM disease occurrence and progression.

ABSTRACT

Objective To explore the association between type 2 diabetes mellitus (T2DM)combined with non -alcoholic fatty liver disease (NAFLD)and serum adiponectin (APN)and its clinical significance.Methods A total of 76 patients who were initially diagnosed with T2DM or had been diagnosed with T2DM for less than three years and admitted to the endocrine department and its outpatient clinic of the Affiliated Hospital of Medical School of Ningbo University from August 2011 to July 2013 were selected as the subjects,and were assigned to group A or B,each consisting of 38 patients,based on the presence or absence of associated NAFLD.Another 30 healthy subjects receiving physical examination were assigned to the control group (NC group).The indicators including body mass index (BMI),blood lipids,blood glucose,liver function,insulin resistance index (HOMA -IR)and serum APN,were measured and compared among the groups. Results In group A ,the indicators including BMI,triglyceride (TG),total cholesterol (TC),low density lipoprotein -cholesterol (LDL -C),alanine aminotransferase (ALT),fasting insulin (FINS)and HOMA -IR were significantly higher than those in group B and NC group,and the APN level was significantly lower than that in group B and NC group (P <0.05).Pearson correlation analysis indicated that APN was negatively correlated with BMI,TG,high density lipoprotein -cholesterol(HDL -C),and HOMA -IR,and positively correlated with TC,LDL -C and ALT(P <0.05).Multiple linear regression analysis suggested that HDL -C and HOMA -IR was the independent influencing factors for APN (P <0.05).Conclusion Severe insulin resistance and glucose and lipid metabolic disorders are present in T2DM patients combined with NAFLD,and adiponectin level is of vital significance for the development and progression of NAFLD.

ABSTRACT

Objective To analysis the changes of serum adiponectin level in patients with breast cancer and its clinical signifi‐cance .Methods 35 cases of serum adiponectin levels in breast cancer patients using enzyme‐linked immunosorbent assay ,and 35 healthy serum adiponectin level in peripheral blood of women ,analysis of significance of changes of serum adiponectin levels in breast cancer patients .Results Average serum adiponectin level in patients with breast cancer group(6 .13 ± 1 .55)μg/mL ,health group average serum adiponectin levels (8 .72 ± 1 .86)μg/mL ,the difference was statistically significant(P<0 .05) .Postmenopausal women in breast cancer group and health group compared with premenopausal women ,the serum adiponectin levels were signifi‐cantly increased ,there was a statistically significant difference(P<0 .05) .Lymph node metastasis of breast cancer serum adiponec‐tin levels were significantly lower in patients without lymph node metastasis in patients with breast cancer ,the difference was statis‐tically significant(P<0 .05) .Serum adiponectin level in breast cancer patients with stage Ⅱ and Ⅲ were significantly lower than breast cancer patients with stage Ⅰ ,there was a statistically significant difference(P<0 .05) .Conclusion Serum adiponectin levels as an important marker in the diagnosis of breast cancer ,serum adiponectin level and grade of the tumor ,lymph node metastases . Significantly reduced serum adiponectin level in patients with breast cancer ,and not under the influence of patients with menopause .

ABSTRACT

Aims: To assess the relation between the grade of steatosis and anthropometric measures, lipid profile and serum adiponectin in non-diabetic patients with nonalcoholic fatty liver. Study Design: Cross sectional study. Place and Duration of Study: Department of Tropical Medicine and Gastroenterology, Qena faculty of medicine, South Valley University. Methodology: Fifty patients with US evidence suggestive of fatty liver disease and normal fasting and post-prandial serum glucose were included. No past or current history of alcohol consumption. Blood samples were taken to detect liver function tests, fasting lipogram, complete blood count and serum adiponectin. Body mass index (BMI) and waist circumferences (WC) were measured for all patients. Liver biopsy was done to detect the presence and the degree of steatosis. Results: The mean age of patients was 40+/-12 years. Patients with steatosis showed significantly higher value for BMI and WC than those without (P value =0.000). Cholesterol, triglyceride and Low density lipoprotein-cholesterol (LDL-C) also were significantly higher in patients with steatosis (P value =0.00). High density lipoprotein-cholesterol (HDL-C) and serum adiponectin were significantly lower in patients with steatosis (P value =0.00). Patients with severe steatosis showed significantly higher values for BMI and WC, cholesterol, triglyceride, LDL-C and lower values for HDL-C and adiponectin (P value = 0.05) than those with mild or moderate steatosis. Positive correlations were detected between the age, BMI and WC, cholesterol, triglyceride, LDL-C and the grade of steatosis and negative correlations with HDL-C and adiponectin. Conclusion: Anthropometric measures, lipogram and serum adiponectin are associated with progression of steatosis in nondiabetic patients with NAFLD. So their detection is important for evaluation and management.

ABSTRACT

Objective To evaluate the effect of telmisartan on serum adiponectin and urine microalbumin(mAlb) level by administrating it to mice with simple obesity, so as to explore new therapies for obesity-related kidney diseases.Method A total of 24 8-week-old male OB mice and 8 8-week-old male C57 mice were selected for this study.The genetic background of OB mouse was C57 mouse, but the lepin gene was deleted in OB mouse.OB mice were randomly divided into 3 groups by body weight and fed with high-fat diet for 12 weeks: model group (M group), telmisartan group (T1 group) and losartan group (T2 group).C57 mice acted as control and were fed with general diet for 12 weeks.Serum adiponectin and blood glucose levels were measured before and after treatment.24 h urine was collected to measure urine mAlb.Quantitative real-time PCR (qRT-PCR) was used to measure the expression level of peroxisome proliferators-activated receptors gamma mRNA(PPARγ mRNA).HE stain was used to observe the morphological changes of kidney and measure the glomerular diameter.Body weight, serum adiponectin level, blood glucose level, urine mAlb level, expression level of kidney PPARγ mRNA, kidney wet weight and glomerular diameter of 4 groups were compared and a correlation analysis was carried out by Person correlation coefficient.Results Compared with C group, the urine mAlb level in M group increased (P<0.01), serum adiponectin level and the expression level of kidney PPARγ mRNA in M group decreased (P < 0.01);The urine mAlb level was negatively correlated with serum adiponectin level and expression of kidney PPARγmRNA (r=-0.773,P < 0.01;r=-0.469, P < 0.01).The urine mAlb level in T1 group was lower than M group (P < 0.05),serum adiponectin level and expression of kidney PPARγmRNA in T1 group were higher than M group (P < 0.05).Compared with T2 group, the urine mAlb level in T1 group decreased, serum adiponectin level and expression of kidney PPARγ mRNA in T1 group increased (P < 0.05);Compared with M group, the urine mAlb level in T2 group decreased (P=0.01).The morphological changes of kidney:glomerular volume increase and focal segmental sclerosis were found in some mice in M group and T2 group.No glomerular volume increase and focal segmental sclerosis were observed in T1 group.Conclusions Telmisartan can reduce urine microalbumin, whose mechanism might be that telmisartan can active the PPARγand promote the level of serum adiponectin.

ABSTRACT

Objective To explore the relationship between serum adiponectin concentration and hyperuricemia in Han people in Hunan.Methods Cluster random sampling method was used to carry out the survey during October to December 2008 in Changsha Health Checkup Center of the Second Xiangya Hospital.All subjects completed the questionnaires,physical examination,biochemical measurements,and the data were analyzed by t test,Pearson's correlation analysis and multiple linear regression analysis.Results The Age,WC,TG,SBP,DBP,SUA,BMI were significantly higher in hyperuricemia group compared with the normal uric acid group.The concentration of adiponectin in hyperuricemia group was significantly lower than that in the normal uric acid group (5.0±2.7 vs 6.8±4.2 μg/ml,t=3.961,P<0.05).In the normal serum uric acid group,the serum adiponectin concentration in female was significantly higher than that in male (t=4.99,P<0.05).Pearson's correlation analysis showed that serum adiponectin concentration was negatively correlated with the serum uric acid level,but the adiponectin level was not significantly correlated with Age and blood pressure.Stepwise analysis showed that the main factors that could affect the adiponectin level were SUA,gender and BMI (P<0.05).Conclusion The decreasing of serum adiponectin concentration might be one of the mechanisms of hyperuricemia,therefore,detecting serum adiponectin concentration may provide the basis for the prevention and treatment of hyperuricemia.

ABSTRACT

OBJECTIVE: Adiponectin is an adipocyte-derived hormone with profound insulin sensitizing, anti-inflammatory, and antiatherogenic effects. Apart from its obvious potential as a mediator of adult metabolic syndrome, in pregnancy, adiponectin could have a significant role in regulating energy homeostasis. However, correlations between umbilical cord and maternal serum adiponectin levels and neonatal birthweights are far from understood. The purpose of the present study was to clarify correlations between umbilical cord and maternal serum adiponectin levels and neonatal birthweights. METHODS: The study included 30 healthy mothers who had given birth to healthy neonates. Adiponectin levels in maternal serum and umbilical cord serum were determined by ELISA and analysed. RESULTS: The ranges of adiponectin levels for umbilical cord and maternal serum were 7.12-24.93 microgram/mL and 1.76-8.20 microgram/mL, respectively. Umbilical cord adiponectin levels (14.82+/-3.66 microgram/mL) were significantly higher than maternal serum levels (4.73+/-1.87)(p<0.001). Umbilical cord adiponectin levels were correlated positively with neonatal birthweights (r=0.459, p=0.011). No significant differences in adiponectin levels were found between female and male neonates. In addition, there was no correlation between umbilical cord adiponectin levels and maternal serum adiponectin levels, maternal body mass index, umbilical leptin, or insulin levels. CONCLUSION: The levels of adiponectin were higher in umbilical cord than in maternal serum. The adiponectin levels in umbilical cord were found to correlate positively with neonatal birthweights. Therefore, adiponectin may be involved in fetal energy metabolism in pregnancy.
