ABSTRACT

Objective To analyze the effect of the new type of Enteral Nutritional Emulsion on the blood glucose and serum level of serum calcitonin in patients with acute respiratory failure.Methods 62 acute respiratory failure with stress hyperglycemia patients were selected and randomLy divided into observation group(n=31) and control group(n=31) in our hospital from April 2013 to August 2016, the control group was treated with Enteral Nutritional Suspension, the observation group was treated with New Enteral Nutritional Emulsion.We compared the two groups before treatment and after 10 days of fasting blood glucose(FBG), 2 hour postprandial blood glucose(2hPBG), serum procalcitonin(PCT) and interleukin-6(IL-6), C reactive protein(CRP) level, and statistics of the incidence of adverse reactions of the two groups.Results Comparing with the control group, the FBG and 2hPBG of the observation group was significantly lower than that of the control group after 10 days treatment, and the difference was statistically significant(P0.05).Conclusion The new type of Enteral Nutritional Emulsion in the treatment of acute respiratory failure with stress hyperglycemia can effectively control the blood glucose level and reduce the concentration of serum calcitonin.

ABSTRACT

Objective To observe the serum calcitonin original (PCT) detection in the diagnosis of children with infectious pneumonia.Methods From March 2014 to April 2015,96 children with infectious pneumonia were selected,including bacterial pneumonia in 43 cases,26 cases of viral pneumonia,27 cases of Mycoplasma pneumonia.50 healthy children were selected as control group.All children received serum calcitonin original test,PCT test was applied in the diagnosis of children with infectious pneumonia.Results The PCT level in children with bacterial pneumonia group was(3.84±3.57)μg/L,which was obviously higher than (0.12±0.03)μg/L of healthy children,(0.74±0.13)μg/L of children with viral pneumonia and (1.07±0.08)μg/L of pure chlamydia pneumonia patients(F=6.120,P=0.013).Before treatment,the level of serum PCT in children with bacterial pneumonia was significantly higher than that in normal children[(3.84±3.57)μg/L vs.(0.12±0.03)μg/L,t=6.987,P=0.000].After treatment,the results of PCT detection in serum of children with bacterial pneumonia was significantly lower[(3.84±3.57)μg/L vs.(0.16±0.03)μg/L,t=6.978,P=0.000)].Conclusion Serum calcitonin original detection can effectively help the differential diagnosis of pediatric infectious pneumonia,has clinical application value.

ABSTRACT

Objective To investigate the application of combined detection of serum calcitonin(PCT),interleukin-6 (IL-6)and C reactive protein(CRP)in early diagnosis of sepsis in intensive care unit(ICU).Methods ICU ward diagnosed 89 cases of patients with early sepsis(sepsis group),fever and did not peak use of antibiotics,send blood to blood culture and detection of PCT,IL-6,CRP level,and at the same time to select 132 cases of healthy subjects(healthy control group),blood samples were collected to detect PCT,IL-6,CRP;PCT,IL-6 for the detection of electrochemical luminescence method.The detection of CRP by immunoturbidimetric method.Results ICU patients with sepsis in PCT,IL-6,CRP detection value is significantly higher than the control group,and the difference was statistically significant(P<0.01);combined detection sensitivity and specificity was significantly higher than that of single detection,PCT+ IL-6,PCT+ CRP,PCT+ IL-6-+-CRP sensitivity of three kinds of combined detection were 91.3 %,90.5% and 91.9%,the specificity was 89.5%,88.3% and 89.4%,and accuracy were 85.5%,85.3% and 85.6%.Conclusion Combined detection of PCT+ IL-6 + CRP or PCT+ IL-6 or PCT+CRP,is helpful to the clinical recognition of early sepsis.

ABSTRACT

Objective To study the clinical effect of serum calcitonin in the treatment of pediatric respiratory tract infections.Methods A retrospective analysis of our hospital in the patient's medical records from April 2015 to April 2016 were 80 cases of pediatric respiratory tract infection, were randomly divided into control group and observation group, 40 cases of respiratory tract infection were treated with standard antibiotic therapy in the control group, the observation group was treated with antibiotics in the treatment of serum calcitonin under the guidance of the comparative analysis of two groups of children with clinical efficacy, clinical symptoms disappear time, antibiotic use and hospital economy.Results The effective rate of control group was 70%, and the observation group was 90%.The clinical efficiency of the observation group was relatively higher than control group ( P <0.05).Compared with the control group, the clinical symptoms of the patients in the observation group were significantly lower than that of the control group(P<0.05).Patients in the observation group used antibiotics was 16 cases(40%), the antibiotic use rate was, 32 cases(80%) in the control group, the average usage time of antibiotics and the average cost of observation group was significantly lower than control group(P<0.05)..The time of hospitalization and the hospitalization expenses of,observation group were lower than control group(P<0.05).Conclusion The treatment of pediatric respiratory infection in serum calcitonin can help improve the clinical efficacy, reduce the use of antibiotics in children with high economic.

ABSTRACT

Objective To study the change of serum diamine oxidase (DAO) level in patients with severely acute pancreatitis (SAP) in order to explore the role of DAO in assessing the severity of SAP and the magnitude of gastrointestinal dysfunction.Methods From January 2012 through December 2013,56 SAP patients with 33 male and 23 female and average age (45-± 14) years admitted within 3 days after onset were enrolled for this study.At admission,serum diamine oxidase (DAO) was detected,and APACHE Ⅱ score,computed tomography severity index (CTSI) score and Balthazar grading and gastrointestinal dysfunction score were calculated.And at the corresponding time,serum procalcitonin (PCT) was detected.The correlations between serum DAO level and 4 other markers were analyzed.Results The high level of serum DAO was found at admission in SAP patients correlating positively with serum PCT concentrations (r =0.516,P< 0.01),APACHE Ⅱ score (r =0.631,P< 0.01),CTIS score (r=0.640,P < 0.01),and the degree of gastrointestinal dysfunction (r =0.730,P < 0.01).Conclusions The role of serum DAO in assessing the severity of SAP and magnitude of gastrointestinal dysfunction in SAP patients is really valid.
