ABSTRACT

Introducción: promover autonomía significa transitar desde un modelo paternalista hacia uno que posi-cione en un rol activo a las personas durante el encuentro clínico, con participación en la toma de deci-siones relativas al cuidado de su salud. Este artículo describe la percepción de usuarios que viven con multimorbilidad respecto del ejercicio de su autonomía durante la atención clínica. Método: estudio de caso cualitativo en usuarios con multimorbilidad atendidos en un centro de salud familiar de Santiago (Chile).Se realizó análisis de contenido según Krippendorf. Resultados: la muestra quedó conformada por 19 participantes adultos con un promedio de 2.7 condiciones crónicas de salud. Del análisis de contenido de las entrevistas emergieron tres categorías: a) significado atribuido por los usuarios a la autonomía en la atención de salud, b) elementos que debe considerar una atención en salud que respete la autonomía del usuario y c) participación durante la atención clínica. Conclusiones: frente al aumento de las condicio-nes crónicas de salud es imperativo repensar la forma de brindar atención de salud, relevando el valor de la participación usuaria a través de la toma de decisiones compartida como expresión de respeto de su autonomía y una forma de fomentar el cuidado centrado en las personas


Aim: Promoting autonomy means changing from a paternalistic model to one in which individuals play an active role in their healthcare, which their participation in medical decision-making will reflect. This issue needs to be sufficiently explored in Chile, so this article aims to describe the perception of users liv-ing with multimorbidity regarding their ability to exercise autonomy in clinical care. Method: Qualitative case study conducted in a sample of patients with multimorbility from a family health center in Santiago de Chile. Content analysis was performed according to the Krippendorf method. Results: The sample com-prised 19 adult participants with an average of 2.7 chronic health conditions. Three categories emerged from the content analysis of the interviews: (a) Meaning attributed by users to autonomy in health care, (b) Elements that health care respecting user autonomy should consider, and (c) Participation during clinical care. Conclusions: Considering the sustained increase in chronic health conditions, it is impera-tive to rethink how health care is provided, highlighting the value of user participation through shared decision-making as an expression of respect for individuals' autonomy and the promotion of patient-cen-tered care

Este manuscrito descreve a percepção de usuários que convivem com multimorbidade quanto ao exercício de sua autonomia durante o atendimento clínico. Método: estudo de caso qualitativo em usuários com multimorbidade atendidos em um Centro de Saúde da Família de Santiago, no Chile. A análise de conteúdo foi realizada segundo Krippendorf. Resultados: a amostra foi composta por 19 participantes adultos com média de 2.7 condições crônicas de saúde. Da análise de conteúdo das entrevistas emergem três categorias: a) Significado atribuído pelos usuários à autonomia no cuidado em saúde, b) Elementos que um cuidado de saúde que respeite a autonomia do usuário deve considerar, e c) Participação durante o atendimento clínico. Conclusões: face ao aumento das condições crónicas de saúde, é imperativo repensar a forma de prestar cuidados de saúde, destacando o valor da participação dos pacientes através da tomada de decisão partilhada como expressão de respeito pela sua autonomia e forma de promover o cuidado centrado nas pessoas

ABSTRACT

Introduction We aimed to develop a decision aid to support shared-decision making between physicians and women with average breast cancer risk when deciding whether to participate in breast cancer screening. Methods We included women at average risk of breast cancer and physicians involved in supporting the decision of breast cancer screening from an Academic Hospital in Buenos Aires, Argentina. We followed the International Patient Decision Aid Standards to develop our decision aid. Guided by a steering group and a multidisciplinary consultancy group including a patient advocate, we reviewed the evidence about breast cancer screening and previous decision aids, explored the patients' information needs on this topic from the patients' and physicians' perspective using semi-structured interviews, and we alpha-tested the prototype to determine its usability, comprehensibility and applicability. Results We developed the first prototype of a web-based decision aid to use during the clinical encounter with women aged 40 to 69 with average breast cancer risk. After a meeting with our consultancy group, we developed a second prototype that underwent alpha-testing. Physicians and patients agreed that the tool was clear, useful and applicable during a clinical encounter. We refined our final prototype according to their feedback. Conclusion We developed the first decision aid in our region and language on this topic, developed with end-users' input and informed by the best available evidence. We expect this decision aid to help women and physicians make shared decisions during the clinical encounter when talking about breast cancer screening.

ABSTRACT

Com base em uma experiência de atendimento psicológico on-line a mulheres negras quilombolas, no contexto da pandemia, pretendemos: 1) refletir sobre casa e rua na realidade da população negra, guiados pelas noções de "forma social escravista" e de "reterritorialização", ambas de Muniz Sodré, destacando, do interior dessas reflexões e a partir das elaborações teóricas de Carl Gustav Jung (no campo da psicologia analítica), e de Achille Mbembe e Muniz Sodré (no campo das relações étnico-raciais), dois conceitos em construção para a compreensão do sofrimento sociorracial: trauma racial e sombra branca; 2) refletir sobre o lugar da "partilha do comum" no cuidado à saúde mental da população negra de zonas rurais e urbanas. Do ponto de vista teórico-metodológico, trabalhamos ao modo de uma comunicação rapsódica, no sentido grego da palavra (raptein = coser): costurando história de caso e proposições conceituais.


Based on an online psychological care experience for black quilombola women in the context of the pandemic, we intend to: 1) reflect upon home and street in the reality of the black population, guided by the notions of "slavery social form" and "re-territorialization", both by Muniz Sodré, highlighting, from within these reflections and from the theoretical elaborations of Carl Gustav Jung (in the field of analytical psychology), and of Achille Mbembe and Muniz Sodré (in the field of ethnic-racial relations), two concepts under construction to understand socio-racial suffering: racial trauma and white shadow; 2) reflect on the place of "sharing of common things" in mental health care for the black population in rural and urban areas. From a theoretical-methodological point of view, we worked in the form of a rhapsodic communication, in the Greek sense of the word (raptein = to sew): weaving together case histories and conceptual propositions.


Con base en una experiencia de atención psicológica en línea con mujeres negras quilombolas en la pandemia, pretendemos: 1) reflexionar sobre hogar y calle en la realidad de la población negra, guiándonos por nociones de "forma social esclavista" y "reterritorialización", de Muniz Sodré, destacando, en de esas reflexiones y a partir de las elaboraciones teóricas de Carl Gustav Jung (en el campo de la psicología analítica), y de Achille Mbembe y Muniz Sodré (en el campo de las relaciones étnico-raciales), conceptos en construcción para comprender el sufrimiento socio-racial: trauma racial y sombra blanca; 2) reflexionar sobre el lugar de "vida compartida" en el cuidado de la salud mental de la población negra de zonas rurales y urbanas. Desde el punto de vista teórico-metodológico, trabajamos sobre la base de una comunicación rapsódica, en el sentido griego de la palabra (raptein = coser): tejiendo el relato del caso y proposiciones conceptuales.

ABSTRACT

To explore the barriers and facilitators of shared decision making, 23 clinicians were selected for semi-structured interviews by purpose sampling and convenience sampling with phenomenological methods in qualitative research, and 7-step of Colaizzi was used to analyze the interview data. Three themes and twelve subthemes were extracted, included: individual factors of doctors (role cognition, perceived outcomes, communication skills, clinical expertise, cognitive bias) , individual factors of patients (general information, lack of disease knowledge, willingness to participate in decision making) and environmental factors (clinical situation, social environment, resources and social influence) . There were many barriers and facilitators in the implementation of doctor-patient shared decision making. It is necessary to scientifically analyze and actively deal with the influence of each factor, and find reasonable countermeasures to promote the clinical implementation of shared decision making.

ABSTRACT

Discrimination against patients with mental disorders and the resulting stigma will not only affect patients’ medical treatment, but also bring about community isolation and lack of resources. Mental health problems have become a major public health problem and a prominent social problem. From the perspective of bioethics, the existence of public mental disorders stigma violates the principles of justice and respect. This paper quantitatively described the status quo of public mental disorders stigma in China, and explored its influencing factors through factor analysis and binary logistic regression analysis. The public stigma of mental disorders score was (54.64±11.048). Factor analysis extracted 4 common factors, namely isolation, pain, contact, and empathy, with a cumulative explained variance of 68.948%. The results showed that age and contact history were the main factors affecting the public stigma of mental disorders. It is recommended to reduce discrimination by enhancing understanding and improving empathy. Specifically, it is to implement the personal liability for discrimination through ethical regulation and legal construction, and strengthen the concept of a community of shared future for mankind by creating a tolerant social atmosphere, so as to achieve an appropriate balance between public safety and individual rights and interests.

ABSTRACT

El objetivo general describir los retos y desafíos de enfermería en la actualidad. Método: Se planteó desde el paradigma positivista, con un enfoque cuantitativo, apoyado en la tipología documental-bibliográfico. Resultados: Los desafíos que enfrenta la profesión de enfermería crean la necesidad de continuar el proceso educativo basado en la idoneidad, el conocimiento y la calidad, que permita a los profesionales posicionarse y recuperar la autonomía y el liderazgo en la disciplina asistencial. En conclusión: El profesional de enfermería se enfrenta, en conjunto, con el equipo de salud, es importante destacar la necesidad de repensar la gestión primaria y hospitalaria desde la cooperación, integración y uso de herramientas que propicien a los pacientes y personas sanas una atención de calidad desde una visión humanista e integral.


The general objective was to describe the challenges of nursing today. Method: It was approached from the positivist paradigm, with a quantitative approach, supported by the documentary-bibliographic typology. Results: The challenges faced by the nursing profession create the need to continue the educational process based on suitability, knowledge and quality, which allows professionals to position themselves and recover autonomy and leadership in the care discipline. In conclusion: The nursing professional faces, together with the health team, the need to rethink primary and hospital management from the cooperation, integration and use of tools that provide patients and healthy people with quality care from a humanistic and comprehensive vision.

ABSTRACT

Introducción: El trastorno psicótico compartido se caracteriza por la aparición de síntomas psicóticos en personas que tienen un vínculo afectivo estrecho con un sujeto que padece un trastorno mental; este caso es el primer reporte de lesiones por quemaduras en el contexto de este trastorno. Caso: Se trata de una pareja joven, con un patrón similar de quemaduras causadas por el contacto con una plancha. Las lesiones son el resultado de la agresión causada por un familiar de uno de ellos, que presentaba síntomas psicóticos relacionados con el espectro de esquizofrenia no diagnosticado previamente. Conclusiones: El impacto de esta afección abarca los componentes social, físico y psicológico y requiere un tratamiento multidisciplinario y un alto índice de sospecha diagnóstica.


Introduction: Shared paranoid disorder is characterised by the development of psychotic symptoms in people who have a close affective bond with a subject suffering from a mental disorder. This case is the first case of burn injuries reported in the context of this disorder. Case: We describe a young couple, with a similar pattern of burns caused by contact with Paranoid disorders a griddle. The injuries are the result of the aggression caused by a relative of one of them, who presented psychotic symptoms, related to the previously undiagnosed spectrum of schizophrenia. Conclusions: The impact of this condition encompasses social, physical and psychological components, requiring multidisciplinary management and a high index of diagnostic suspicion.

ABSTRACT

En materia de Derecho de familia, la legislación peruana contempla la posibilidad de una tenencia compartida, regulada expresamente en el Código de los Niños y Adolescentes, sin establecer precisiones sobre las condiciones y formas de aplicación, lo que es similar en el Derecho comparado. Sustentado en principios bioéticos y jurídicos, en el presente artículo se analiza la posibilidad de generar una regulación especial para la tenencia compartida, enfocada en la necesidad de establecer criterios especiales que puedan tener en cuenta los jueces en los casos concretos que involucren a niños y niñas. Estos criterios deben partir de un modelo social dirigido a quebrar las barreras impuestas, dotando de las condiciones más adecuadas para un desarrollo integral y pleno. En esa línea de pensamiento, es fundamental el interés superior del niño, el compromiso de los progenitores y la implementación de ajustes razonables.


In terms of family law, Peruvian legislation contemplates the possibility of shared custody, expressly regulated in the Code of Children and Adolescents, without establishing details on the conditions and forms of application, which is similar in comparative law. Based on bioethical and legal principles, this article analyses the possibility of generating a special regulation for shared custody, focusing on the need to establish special criteria that judges can take into account in specific cases involving children. These criteria should be based on a social model aimed at breaking down the barriers imposed, providing the most appropriate conditions for an integral and full development. In this line of thought, the best interests of the child, the commitment of the parents and the implementation of reasonable accommodations are fundamental.


Em matéria de Direito de família, a legislação peruana contempla a possibilidade de uma guarda compartilhada, regulada expressamente no Código das Crianças e Adolescentes, sem estabelecer especificações sobre as condições e formas de aplicação, o que é similar no Direito comparado. Apoiado em princípios bioéticos e jurídicos, no presente artigo se analisa a possibilidade de gerar uma regulação especial para a guarda compartilhada, focada na necessidade de estabelecer critérios especiais que juízes possam ter em conta nos casos concretos que envolvem meninos e meninas. Esses critérios devem partir de um modelo social dirigido a quebrar as barreiras impostas, dotando as condições mais adequadas para um desenvolvimento integral e pleno. Nessa linha de pensamento, é fundamental o interesse superior da criança, o compromisso dos pais e a implementação de ajustes razoáveis.

El material abordado se conformó por tesis de grado, investigaciones científicas, trabajos arbitrados y, con ello, descubrir características y relaciones entre los elementos. En conclusión: El rol de enfermería en el desarrollo de los modelos de gestión del cuidado en los servicios hospitalarios, es de gran importancia ya que establecen los mecanismos de comunicación para su implementación en conjunto con el resto del equipo de salud, que hace vida en los centros hospitalarios.

Resumen La lectura compartida es frecuente en la infancia. Sin embargo, la participación de infantes menores de un año en esta actividad ha sido poco explorada. Este trabajo describe y analiza las intervenciones de bebés de 9 meses en la lectura conjunta materno-infantil en hogares argentinos. Transcribimos y analizamos filmaciones realizadas por 10 madres. Clasificamos las intervenciones en: acción responsiva o espontánea; vocalización responsiva o espontánea; acción responsiva + vocalización; acción espontánea + vocalización. Los bebés realizaron mayoritariamente acciones espontáneas (con o sin vocalizaciones). Luego, clasificamos las acciones (con o sin vocalizaciones) en: convencionales (pasar la página, señalar) o manipulativas (morder, sacudir los libros); otras no orientadas (tomar otro objeto, mirar fuera del escenario). Encontramos que las acciones fueron principalmente manipulativas. Finalmente, clasificamos las acciones en: inespecíficas (morder el libro, sacudirlo) u orientadas (intentar agarrar los objetos representados). Predominaron las acciones inespecíficas. Los hallazgos muestran bebés sensoriomotores en exploración. Sin embargo, algunas intervenciones revelaron también un incipiente desarrollo simbólico al usar los libros convencionalmente y reconocer las imágenes como objetos simbólicos. La lectura compartida es un escenario en el que se accede a diversos sistemas de representación (imágenes, números, letras), permitiendo desarrollar tempranamente un potencial simbólico en contextos sociales relevantes.


Abstract Shared reading is a significant and frequent activity in early childhood. However, the interventions in this kind of activities by babies and infants under one-year have been little explored. The main objective of this work was to describe 9-month-old babies' interventions in reading situations with their mothers at Argentinian homes. The focus of the analysis was babies' actions during reading with their mothers. Ten mothers filmed shared reading sessions with their babies. Then we transcribed and analyzed babies' interventions in three levels. First, we classified the interventions in: responsive action, spontaneous action, responsive vocalization, spontaneous vocalization, responsive action + vocalization, spontaneous action + vocalization. Isolated vocalizations were rare. The babies performed mostly spontaneous actions (with or without vocalizations). Responsive actions were less frequent (with or without vocalizations). Second, we classified babies' actions (with or without vocalizations) in 1-conventional, like turn the page, point to the book, put the book in reading position, 2-manipulative, like bite, shake, put the book in the mouth, open and close the book, try to grab/suck/ touch objects represented in pictures, 3- other actions- actions not oriented to books, like shake another object present in the interaction or looking to some object or person outside the scene. We found that babies' actions were mainly manipulative. Finally, we categorized manipulative actions in 1- unspecific actions, like bite, shake, put the book in the mouth, open and close the book, and 2- pictures-oriented ones, like try to grab, suck, touch objects present in pictures. Unspecific actions predominated. Findings show sensory motor babies in exploration. Nevertheless, the presence of conventional actions and picture-oriented actions are signs of an incipient symbolic development. That is, babies use books conventionally and recognize pictures as symbolic objects. In sum, shared reading is one of the richest scenarios for early symbolic development. In this context babies and children access to diverse representational systems (words, pictures, numbers, letters). This activity allows children to development their symbolic potential in relevant social contexts.

ABSTRACT

Introduction: The most important genetic association in rheumatoid arthritis (RA) is presented with some alleles from the HLA-DRB1 gene that encode the shared epitope (SE). Objectives: To apply the SE classification methods of Gregersen, de Vries, Raychaudhuri, Mattey, and Tezenas du Montcel in a group of Colombian patients with RA and determine the most common HLA-DRB1 alleles in the population. Methods: RA diagnosis, genetic study of the HLA-DRB1 region using Luminex technology in 50 RA and 50 healthy subjects. For the classification analysis, Fisher's exact test and chi-squared test were applied. Tables were created to count the RA-related alleles. We used odds ratio to determine the risk between the presence of the shared epitope (SE) and anti-cyclic citrullinated peptides (Anti-CCP). Results: Gregersen and de Vries methods were suitable for the characterization of RA in this population (p = .006). The most prevalent HLA-DRB1 alleles in the RA group were 14:02,04:04, 08:02,04:05, and 10:01. High frequencies of the 07:01, 03:01,13:02,01:02, and 12:01 HLA-DRB1 alleles were found in the healthy population. HLA-DRB1 alleles with similar distribution in both populations were 04:07, 15:01, 11:01, 16:02, and 01:01. A high frequency of SE + was observed in Anti-CCP + individuals (63.15%); however, this was not statistically significant [OR2.4 (.63-9.01); p = .19]. Conclusion: The SE classification methods of Gregersen and de Vries were adequate in characterizing RA in a Colombian population group. An equivalence of 100% was verified between the susceptibility alleles defined by de Vries and the alleles assigned as SE according to Gregersen.


Introducción: La asociación genética más importante en artritis reumatoide (AR) se presenta con algunos alelos del gen HLA DRB1 que codifican el epítope compartido (EC). Objetivos: Aplicar los métodos de clasificación de EC de Gregersen et al., de Vries et al., Raychaudhuri et al., Mattey et al., y Tezenas du Montcel et al., en un grupo de pacientes colombianos con AR, y determinar los alelos HLA DRB1 más frecuentes en esta población. Métodos: Diagnóstico para AR, estudio genético de la región HLA DRB1 por tecnología Luminex® de 50 sujetos AR y 50 sanos. Para análisis comparativos de clasificaciones EC, se aplicaron las pruebas test exacto de Fisher y Chi-cuadrado y se realizaron tablas de conteos para los alelos relacionados con AR. Se estimó la razón de odds para determinar el riesgo entre la presencia de EC y los anticuerpos antipéptidos cíclicos citrulinados (anti-PCC). Resultados: Los métodos de Gregersen et al. y de Vries et al. fueron adecuados para la caracterización de AR en esta población (p = 0,006). Los alelos HLA DRB1 más prevalentes en el grupo AR fueron 14:02, 04:04, 08:02, 04:05 y 10:01. Se encontraron altas frecuencias de los alelos HLA DRB1 07:01, 03:01,13:02, 01:02 y 12:01 en población sana. Alelos HLA DRB1 con distribución similar en ambas poblaciones fueron: 04:07, 15:01, 11:01, 16:02 y 01:01. Se observó alta frecuencia de individuos EC+ en el grupo AR anti-PCC+ (63,15%); no obstante, sin asociación estadística (OR: 2,4 [0,63-9,01]; p = 0,19). Conclusión: Los métodos de clasificación para EC de Gregersen et al. y de Vries et al. fueron adecuados caracterizando AR en un grupo de población colombiana. Se corroboró equivalencia del 100% entre los alelos de susceptibilidad definidos por de Vries y los alelos asignados como EC según Gregersen et al.

ABSTRACT Objective To analyze the refusal rate of elective aortic aneurysm surgery in asymptomatic patients after the presentation of a detailed informed consent form followed by a meeting where patient and their families can analyze each item. Methods We conducted a retrospective analysis of 49 patients who had aneurysms and were offered surgical treatment between June 2017 and February 2019. The patients were divided into two groups: the Rejected Surgery Group, which was composed of patients who refused the proposed surgical treatment, and the Accepted Surgery Group, comprising patients who accepted the proposed surgeries and subsequently underwent them. Results Of the 49 patients, 13 (26.5%) refused surgery after reading the informed consent and attending the comprehensive meeting. We observed that patients who refused surgery had statistically smaller aneurysms than those who accepted surgery (9% versus 26%). These smaller aneurysms were above the indication size, according to the literature. Conclusion One-quarter of patients who were indicated for elective surgical repair of aortic aneurysms rejected surgery after shared decision-making, which involved presenting patients with an informed consent form followed by a clarification meeting for them and their families to analyze each item. The only factor that significantly influenced a rejection of the procedure was the size of the aneurysm; patients who rejected surgery had smaller aneurysms than those who accepted surgery.

ABSTRACT

This research aimed to understand the structuring of psychological assessments in custody actions and, on the other hand, to identify the existence of homogeneity or heterogeneity in these practices. Twenty-nine Brazilian legal psychologists participated in this study and answered an online questionnaire built based on the literature. The analysis of the results was carried out through deductive and semantic Thematic Analysis, whose previous categories were created based on national and international guidelines. The most salient results indicate some divergences from the literature, but that its actions and dimensions are compatible with those found in the literature. Based on these results, a guide of systematized practices is proposed, aiming to promote greater uniformity in parental assessment.


Resumo Esta pesquisa teve como objetivos, por um lado, compreender a estruturação das avaliações psicológica em ações de guarda e, por outro, identificar a existência de homogeneidade ou heterogeneidade nessas práticas. Participaram neste estudo 29 psicólogos jurídicos brasileiros, que responderam a um questionário online construído com base na literatura. A análise dos resultados foi realizada através da Análise Temática dedutiva e semântica, cujas categorias prévias foram criadas com base em orientações nacionais e internacionais. Os resultados mais salientes indicam algumas divergências face à literatura, mas que as suas ações e dimensões são compatíveis com as encontradas na literatura. Com base nestes resultados, propõe-se um roteiro de práticas sistematizadas, visando promover uma maior uniformidade na avaliação parental.

ABSTRACT

Introducción: El estudio de procesos que inciden en la calidad de enfermería en oftalmología con la utilización de la referenciación competitiva requiere la validación de instrumentos para el alcance de los resultados esperados. Objetivo: Describir el proceso de construcción y validación de instrumentos para el estudio de procesos relacionados con la calidad de enfermería en oftalmología. Métodos: Estudio instrumental realizado en el Instituto Cubano de Oftalmología "Ramón Pando Ferrer", en el período de enero-julio 2020, que contó con dos fases, la construcción de los instrumentos y la validación de comprensión o de apariencia y de contenido. Para el diseño se tuvieron en cuenta las tareas que cada proceso estudiado incluye en su ficha y para la validación de comprensión y de contenido se utilizó la consulta a profesionales y expertos, así como el método Barraza que integra la valoración cuantitativa de los ítems que integran los instrumentos. Resultados: En la validación de comprensión o apariencia más del 94 % de los ítems no sufrieron cambios desde su primera ronda de consulta, en tanto solo cuatro ítems requirieron modificaciones en la forma de enfocar las preguntas propuestas. Los instrumentos recibieron en la valoración de los expertos una evaluación mayor a 2,6, lo que les confiere una fuerte validez de contenido. Conclusión: La validación realizada permitió constatar la pertinencia de los instrumentos diseñados para el estudio en la referencia de buenas prácticas, en la Clínica Central "Cira García", de los procesos de hospitalización, atención a urgencias y esterilización(AU)


Introduction: The use of competitive benchmarking for the study of processes that influence on ophthalmology nursing quality requires the validation of instruments for the achievement of the expected results. Objective: To describe the construction and validation of instruments for the study of processes related to nursing quality in ophthalmology. Methods: A instrumental study was carried out at Instituto Cubano de Oftalmología "Ramón Pando Ferrer" in the period January-July 2020. It had two phases: the construction of the instruments, and the validation of comprehension or appearance and content. For the design, the tasks that each studied process includes in their respective files were taken into account, while the validation of comprehension and content was performed by means of consultation with professionals and experts, as well as the Barraza method, which integrates the quantitative evaluation of the items making up the instruments. Results: In the validation of comprehension or appearance, more than 94% of the items did not have any changes since the first consultation round, while only four items required modifications in the way of approaching the proposed questions; the instruments received an evaluation of more than 2.6 according to the experts' assessment, which provides them with strong content validity. Conclusion: The performed validation allowed to confirm the relevance of the instruments designed for the study in reference to good practices, as well as the hospitalization, emergency care and sterilization processes, at Clínica Central "Cira García"(AU)

Resumo O Centro de Estudos e Pesquisas em Reabilitação "Prof. Dr. Gabriel O. S. Porto" (CEPRE) participou do Programa de Aprimoramento (PAP)2 para profissionais não médicos da área da saúde entre 1994 e 2019. O objetivo deste estudo foi conhecer e verificar a ocorrência da interprofissionalidade e o papel da terapia ocupacional entre os participantes do PAP atuantes no CEPRE. Esta é uma pesquisa descritiva, qualitativa com a participação de 18 aprimorandos das áreas de fonoaudiologia, pedagogia, psicologia, serviço social e terapia ocupacional que ingressaram nos PAPs realizados no CEPRE em 2018. Os dados foram coletados por meio de entrevistas semiestruturadas, contendo perguntas fechadas e abertas, que abordaram questões referentes às experiências e vivências no PAP. Essas entrevistas foram registradas em áudio e posteriormente transcritas. Optou-se pela análise de conteúdo para examinar os dados. Os resultados apontaram que o aprimoramento profissional contribuiu positivamente para a ocorrência do conhecimento sobre outras áreas, especialmente a terapia ocupacional, oportunizado através de aulas interdisciplinares. A análise de conteúdo revelou que os momentos compartilhados foram importantes para a ocorrência da prática interprofissional centrada no paciente com enfoque na integralidade, e que esse espaço fortaleceu ainda mais o trabalho.


Abstract The "Prof. Gabriel O. S. Porto" Center for Studies and Research in Rehabilitation (CEPRE) participated in the Professional Improvement Program (PAP) for non-medical health professionals from 1994 to 2019. This study aimed to understand and assess the occurrence of interprofessionality and the role of occupational therapy among the PAP participants working at CEPRE. This descriptive, qualitative research involved 18 interns from the areas of speech therapy, education, psychology, social service, and occupational therapy who joined PAP at CEPRE in 2018. Data were collected through semi-structured interviews that included both closed and open questions addressing experiences and insights during PAP. These interviews were audio recorded and later transcribed. The data were assessed by content analysis. The results showed that professional improvement positively influenced knowledge about other areas, especially occupational therapy, which was facilitated through interdisciplinary classes. The content analysis revealed that shared moments were important for the occurrence of patient-centered interprofessional practice with a focus on comprehensiveness, and that this space further strengthened the overall work.

Resumo Este estudo teve como objetivo descrever os métodos de pesquisa e instrumentos utilizados na avaliação de estratégias de educação interprofissional para a tomada de decisão compartilhada em farmacoterapia. Os tipos de avaliação empregados foram categorizados segundo o modelo adaptado de Kirkpatrick. Foi conduzida uma revisão de escopo, seguindo as recomendações do Preferred Reporting Items for Systematic Reviews and Meta-Analyses Extension for Scoping Reviews (Prisma-ScR). Entre os 21 estudos selecionados, houve predomínio dos métodos quantitativos na avaliação das experiências educacionais (n=18). Destes, o aspecto mais comum avaliado foi "atitudes e percepções dos estudantes em relação à educação e à prática interprofissional", por meio de instrumentos validados (n=13). Foram identificados dez diferentes instrumentos, que se mostraram em conformidade com as competências colaborativas do Interprofessional Education Collaborative. A variedade de instrumentos sinaliza a crescente produção de conhecimento acerca do assunto, mas aponta o desafio de realizar análises comparativas entre experiências educacionais ao redor do mundo. Abstract This study aimed to describe the research methods and instruments used in the evaluation of interprofessional education strategies for shared decision making in drug therapy. The types of evaluation employed were categorized according to Kirkpatrick's adapted model. A scoping review was conducted, following the PRISMA-ScR recommendations. Among the 21 selected studies, there was a predominance of quantitative methods in the evaluation of educational experiences (n=18). Of these, the most common aspect evaluated was "students' attitudes and perceptions towards interprofessional education and practice" by means of validated instruments (n=13). Ten different instruments were identified and found to be in line with the Interprofessional Education Collaborative competencies. The variety of instruments signals the growing production of knowledge about this topic, but points to the challenge of conducting comparative analyses between educational experiences around the world. Resumo A premissa de que todo ser humano é vulnerável por natureza advém da noção de vulnerabilidade enquanto suscetibilidade ao dano, assim como da inter-relação e interdependência intrínseca do ser humano com seu contexto socioambiental, cabendo à sociedade estabelecer instituições capazes de promover tanto a proteção quanto a autonomia das pessoas, em equidade de condições. Para além do dever profissional de informar benefícios e malefícios das intervenções possíveis, incluindo as consequências de negar qualquer intervenção, o cuidado centrado no paciente integra uma abordagem colaborativa que considera fundamental a tomada de decisão compartilhada. A partir de uma revisão teórica, neste artigo analisa-se o conceito de vulnerabilidade e sua relação com o respeito e a promoção dos direitos humanos dos pacientes, propondo e justificando a tomada de decisão compartilhada no que se refere a uma boa estratégia de mitigação de vulnerabilidades acrescidas.


Abstract The premise that every human being is vulnerable by nature stems from the notion of vulnerability as susceptibility to harm, as well as the interrelations and intrinsic interdependence of human beings with their socio-environmental context. It is up to society to establish institutions capable of promoting both the protection and autonomy, under equal conditions. Besides the professional duty to inform patients of the benefits and harms from possible interventions, including the consequences of denying any intervention, patient-centered care integrates a collaborative approach that considers shared decision-making essential. This theoretical review analyzes the concept of vulnerability and its relations with respect for and promotion of patient rights, proposing and justifying shared decision-making as a good strategy for mitigating increased vulnerabilities.


Resumen La premisa de que todo ser humano es vulnerable por naturaleza parte de la noción de vulnerabilidad como susceptibilidad al daño, así como de la interrelación e interdependencia intrínseca del ser humano con su contexto socioambiental, y a la sociedad le corresponde establecer instituciones capaces de promover tanto la protección como la autonomía de las personas en igualdad de condiciones. Además del deber profesional de informar de los beneficios y perjuicios de las posibles intervenciones, incluyendo las consecuencias de negar cualquier intervención, la atención centrada en el paciente integra un enfoque colaborativo que considera fundamental la toma de decisiones compartida. A partir de una revisión teórica, este artículo analiza el concepto de vulnerabilidad y su relación con el respeto y la promoción de los derechos humanos de los pacientes, proponiendo y justificando la toma de decisiones compartida como una buena estrategia para mitigar el aumento de las vulnerabilidades.

@#BACKGROUND: Shared decision-making (SDM) has broad application in emergencies. Most published studies have focused on SDM for a certain disease or expert opinions on future research gaps without revealing the full picture or detailed guidance for clinical practice. This study is to investigate the optimal application of SDM to guide life-sustaining treatment (LST) in emergencies. METHODS: This study was a prospective two-round Delphi consensus-seeking survey among multiple stakeholders at the China Consortium of Elite Teaching Hospitals for Residency Education. Participants were identified based on their expertise in medicine, law, administration, medical education, or patient advocacy. All individual items and questions in the questionnaire were scored using a 5-point Likert scale, with responses ranging from “very unimportant” (a score of 1) to “extremely important” (a score of 5). The percentages of the responses that had scores of 4-5 on the 5-point Likert scale were calculated. A Kendall’s W coefficient was calculated to evaluate the consensus of experts. RESULTS: A two-level framework consisting of 4 domains and 22 items as well as a ready-to-use checklist for the informed consent process for LST was established. An acceptable Kendall’s W coefficient was achieved. CONCLUSION: A consensus-based framework supporting SDM during LST in an emergency department can inform the implementation of guidelines for clinical interventions, research studies, medical education, and policy initiatives.

Objective:This paper introduces the concepts of " patient-centered cell therapy research management model", to provide reference for domestic medical institutions which conduct cell therapy clinical trials.Methods:We reviewed and summarized the experiences of conducting cell therapy at Peking University Shenzhen Hospital, including challenges and response plans regarding to the technology assessment, ethical evaluation risks and regulatory compliance. According to which, this paper aims to explore the reflections and practical experience of establishing a patient-centered, multi-stakeholder shared decision-making research management system.Results:The " patient-centered cell therapy research management model" ensures the reliability of research results through multi-stakeholder engagement in decision-making and management, adequate technical evaluation, effective ethical review and harmonized scientific research management, which not only meets the urgent health needs of the patient, but also promotes the standardized development of emerging technologies.Conclusions:The " patient-centered cell therapy research management model" is tailored for the cell therapy research, it is important to promote its further assessment and applications.
