ABSTRACT

This report outlines the case of a 51-year-old female patient who had suffered from onycholysis of the thumbs for eight years. The patient complained of experiencing pain when she works in contact with water, affecting her quality of life. Before coming to our clinic, the patient was diagnosed and treated at three dermatology clinics. However, treatment proved ineffective and the cause was not elucidated. As the patient experienced symptoms of onycholysis and dry skin, we diagnosed her with ketsu deficiency and treated her with shimotsuto. The symptoms gradually improved with continuous oral administration, and were no longer present at eight months following the start of treatment. This case suggests that shimotsuto may be effective in treating instances of onycholysis where the cause is unknown.

ABSTRACT

We report a case in which goreisan with hoketsuzai (blood-tonifying formula) was successful in the treatment of a marked nutritional disorder. A 17-year-old woman was screened for prominent emaciation of unknown cause. After the administration of goreisan, edema was reduced and her body weight increased. Subsequently, anemia improved and muscle mass increased with a combination of shimotsuto or sokeikakketsuto. It is possible that goreisan with hoketsuzai improved the water balance of the whole body including the digestive tract, and restored the digestive and absorptive function.

ABSTRACT

We report 4 cases of vulvodynia, vulval pain from unknown causes that showed symptomatic improvements by administrating Kampo medicines. Modern medicines, such as analgesics, treatments for endometriosis or hormone replacement therapy, can't always be effective for vulvodynia and not a few patients go to see different hospitals without improving their pain. We diagnosed these 4 cases of vulvodynia were originated in kekkyo. We administered Kampo medicines, mainly shimotsuto with supporting ones, and the symptoms of vulvodynia improved.

ABSTRACT

Crowned dens syndrome (CDS) is a pseudogout of the cervical vertebra, which shows calcification around the dens of the axis. We report a case of CDS that was successfully treated with daiobotampito and shimotsuto. An 83-year-old woman visited our clinic because of neck pain. She was diagnosed with CDS and was initially administered daiobotampito and eppikajutsuto. Her neck pain improved slightly, but recurred. She was then administered daiobotampito and shimotsuto, after which her neck pain disappeared. We considered that daiobotampito and shimotsuto were effective for this prolonged illness based on persistent stasis and blood deficiency according to her treatment course and laboratory findings.

ABSTRACT

Six patients with psychiatric symptoms were successfully treated with shimotsuto-containing medication. Case 1 was a 13-year-old boy who complained of difficulty in getting up ; however, when he was treated with a focus on the decrease in his concentration, he was able to go to school after treatment. Case 2 was an 18-year-old girl who had also difficulty in getting up, but was able to go to school. Case 3 was a 10-year-old girl who was able to go to school partially after treatment. Case 4, a 42-year-old man with apprehension in socializing, showed improvement in socializing after taking medication containing shimotsuto. Case 5 had various complaints that were relieved after taking medication containing shimotsuto. Case 6 was a 56-year-old woman who had also multiple complaints ; however, the symptoms were alleviated with shimotsuto-containing medication. All patients experienced depression and anxiety. Four patients with decreased cortisol recovered after treatment with shimotsuto. From these results, shimotsuto is considered to improve concentration, judgment, and psychiatric symptoms by treating blood deficiency.

ABSTRACT

We evaluated body constituents patterns of 74 consecutive patients with neurological degenerative disorders. They comprise Parkinson's disease (n = 38), amyotrophic lateral sclerosis (n = 19), and multiple system atro­phy (n = 17). We compared body constituents patterns between them and 149 consecutive patients with other neurological diseases of the same age. We used ki-ketsu-sui scores to evaluate body constituents patterns in all cases. Ki-ketsu-sui scores measure six factors : qi deficiency (kikyo), qi stagnation (kiutsu), qi counterflow (kigyaku), blood deficiency (kekkyo), blood stasis (oketsu), and fluid retention (suitai). As a result of multi­variate analysis, neurological degenerative disorders had large weight of blood deficiency, fluid retention and qi stagnation. Their adjusted odds ratios (95% confidence interval) were 3.02 (1.43-­6.48), 2.37 (1.13-­5.11), 2.33 (1.01-­5.44), respectively. Most relevant factor to neurological degenerative disorders was a blood defi­ciency. Taking into consideration a prescription of “shimotsuto rui” may contribute to alleviate patient's suf­fering. In addition to subjective symptoms, we need an oriental medicine scale such as pulse, tongue, and ab­dominal examinations to judge a therapeutic effect of Kampo medicine.

ABSTRACT

We report three patients with acute lumbago who did not respond to nonsteroidal anti-inflammatory drugs (NSAIDs), but were treated successfully with daiobotampito and shimotsuto. Case 1 (86-year-old man) and Case 2 (56-year-old woman) were diagnosed with acute lumbago. Case 3 (69-year-old man) was diagnosed with acute lumbago and mild left sciatica. All three patients were initially administered orally NSAIDs, but this treatment was ineffective. Therefore, daiobotampito and shimotsuto were administered after choeikatsurakuto according to “Manbyokaishun.” Following this treatment, the patients' acute lumbago and sciatica resolved within 1-2 weeks. In conclusion, daiobotampito in combination with shimotsuto seems to be a useful Kampo medicine for treatment of lumbago.

ABSTRACT

We present 16 cases effectively treated using the traditional Japanese herbal formulation tokishakuyakusan with the crude extract jio (tokishakuyakusan and shimotsuto). All patients were women with a median age of 35.5 years (range : 22-62 years). Cases included infertility (n = 5), dermatological disease (n = 5), gynecological disease (n = 2), excessive sensitivity to cold (n = 2), mental nerve paresthesia (n = 1), and general fatigue (n = 1). All patients had sho for tokishakuyakusan and high degree of ketsu deficiency. All patients with infertility became pregnant within one year. The symptom severity of other patients decreased to less than half compared with their initial visit. Cases of palmoplantar pustulosis and chronic eczema improved without topical corticosteroids. Our results suggest that tokishakuyakusan with jio is a suitable treatment for patients who have sho for tokishakuyakusan and high degree of ketsu deficiency.

ABSTRACT

The effectiveness of the Kampo medicine shakuyakukanzoto extract for patients with muscle cramp is well known, but recently side effects related to pseudoaldosteronism have also been reported. To avoid this issue, Kampo medicines without <i>Glycyrrhizae Radix </i>are needed for treatment of muscle cramp. We investigated the therapeutic effect of shimotsuto extract, which has traditionally been used to treat anemia, in 26 patients (mean age : 70.7 ± 12.1 years) with muscle cramp. Improvement of the symptom was noted in 18 patients (69%), with no change in 8 patients (31%). The abdominal muscle tension of the former patients was significantly lower than the latter. Here, the cases of four representative patients in whom muscle cramp was improved by shimotsuto are described. The response rate to shimotsuto was similar to that of shakuyakukanzoto. Our results suggest that shimotsuto extract is a suitable alternative to shakuyakukanzoto for treatment of muscle cramp in older non-robust type patients not in need of emergency care.

ABSTRACT

From a point of stress, the competitive sports are totally different from exercises for the health promotion. It would be even harmful especially for middle-distance or long-distance women runner. It brings them paramenia, defatigation and other orthopedic troubles which makes them unable to exercise further more.We have investigated the possibility of the preventive use of Kampo Medicine for those athletes and found that it is useful. The nine women who belong to the Tohoku-Region women team of long-distance relay road race had received Kampo medical treatment for 7 months. Serum CPK and AST were elevated with training exercise. In the cases whose CPK level was above 500 IU/l, most of them suffered from fatigue, leg pain, low back pain, and lower abdominal pain, which is very important for Kampo diagnosis. Because these symptoms unable athletes to exercise as planned, we understand that for the improvement of physical capacity, it is important to prevent those symptoms. For the treatment and prevention of the symptoms, we prescribed Keishibukuryogan, Rikkunshito, and Shimotsuto. We defined preventive medication period as “Mibyou” and continued to use Kampo medicine for 7 months. During the period, they were free from any troubles that would make them unable to exercise. These medicines enabled them to improve their records drastically. This study shows that preventive use of Kampo Medicine is extremely effective for any athletes to maintain their good conditions.

ABSTRACT

Idiopathic thrombocytopenic purpura is an autoimmune disease and is caused by a blood antiplatelet antibody that reacts to thrombocyte membrane. The platelet associated immunoglobulin G (PAIgG) is admitted in idiopathic thrombocytopenic purpura.<br>Adrenocortical steroids are our first choice as a treatment for idiopathic thrombocytopenic purpura.<br>However, a sufficient thrombocyte increase is not obtained even when adrenocortical steroids are administered to the idiopathic thrombocytopenic purpura patient.<br>On the other hand, we use Kampo medicine widely clinically. We are admitting the effectiveness of Kampo medicine in treating many refractory diseases. Even if it corresponds to idiopathic thrombocytopenic purpura that is one of a chronic refractory disease effective Kampo medicine is reorted several Kampo pharmaceutical preparations that are assumed when it is effective in idiopathic thrombocytopenic purpura are Sairei-to, Sho-saiko-to, Kami-kihi-to, etc.<br>This time we carried out Kampo therapy (Rikkunshi-to, Shimatsu-to) on patients with digestive and cutaneous problems but no other symptoms of hemopathy. A blood specialist at this university hospital tentatively diagnosed the patients' condition as thrombocytopenic purpura. Kampo therapy increased the thrombocyte count and alleviated the symptoms.<br>This result might suggest the possibility of a therapy for idiopathic thrombocytopenic purpura and even the importance of the therapy in accordance with the oriental medical syndromes.

ABSTRACT

The source of Kami-shoyo-san varies in the literature. The current formula contains ten kinds of medicine. Only Shoyo-san is described in “He ji ju fang”. In “Nü ke cuo yao” and “Nei ke zhai yao”, a formula containing eight medicines without ginger and mentha was descrided. It was in “Wan bing hui chun” after Xue Ji that a formula containing ten kinds of medicine was first specified. The source of its combination with Shimotsu-to also varies in the literature. There is no mention of this combination in the “He ji ju fang”. In the “Nei ke zhai yao”, there is a case where eight-medicine formulations in combination with Shimotsu-to were used. Although ten-medicine plus Shimotsu-to are, in fact, described in “Futsugo-yakushitsu-houkan-kuketsu”, my investigation found that such a combination formula was used for general pruritus in “Ryo-chikeiken-hikki” about 90 years previously. I believe that if the process of establishing a formula is complicated, as in the case of Kami-shoyo-san, every activity contributing to each step in establishing the formula is generally included as a part of its source and that any activity, in which a combination of formulae was very effective, is regarded, in principle, as its source.
