ABSTRACT

@#Abstract: Objective - - To establish a pre column derivatization high performance liquid chromatography method for detecting Methods dimethyl sulfate (DMS) in workplace air. DMS in workplace air was collected with mercaptopyridine impregnated ( silicone tube. The derivative of DMS and mercaptopyridine was eluted by mobile phase phase A: water, phase B: acetonitrile, ∶ the volume ratio was 40 60) , and separated with a C18 column, then detected with diode array detector and quantitated by a Results - standard curve. The linear range of DMS was 0.17 40.00 mg/L, with the correlation coefficient of 0.999 95. The detection limit and the lower limit of quantitation were 0.05 and 0.17 mg/L respectively. The minimum detection concentration and minimum quantitation concentration were 0.02 and 0.04 mg/m³, respectively (air sample volume of 4.5 L, 1.0 mL sample - - - solution). The average desorption efficiency was 98.40% 102.00%. The within run and between run relative standard deviations - - were 0.61% 3.92% and 1.71% 6.00%, respectively. The samples could be stored at room temperature for at least 14 days. Conclusion This method can be used to detect DMS in workplace air.

ABSTRACT

@#AIM:To observe the clinical effect of retrograde lacrimal duct drainage tube(RS-1 type)in the treatment of lacrimal canaliculus rupture in children.<p>METHODS: The clinical data of 37 cases(37 eyes)with lacrimal canaliculus rupture admitted to Handan Eye Hospital and Handan Central Hospital from March 2016 to November 2019 were retrospectively analyzed. New silicone tubes were retrograde implanted in all patients during the operation. 8-0 absorbable sutures were used to anastomose the broken end of the lacrimal canaliculus, 6-0 absorbable sutures were used to anastomose the “medial canthus ligament-orbicularis” complex intermittently, and muscle, subcutaneous tissue, skin wounds and conjunctival wounds were sutured intermittently. The catheter was extubated 2-3mo after operation, and the lacrimal passage patency and lacrimal ectorrhea were observed for 6-12mo. <p>RESULTS: Totally 31 eyes(84%)were cured, 5 eyes(14%)were improved, and 1 eyes(3%)was ineffective. The total effective rate reached 97%. None of the patients had lacrimal dot tearing, abnormal location of lacrimal dot, angulation deformity and other related complications, and 1 invalid eyes(3%)was combined with bone lacrimal tubule fracture, without obvious lacrimal ectorsion symptoms.<p>CONCLUSION: The new type of silicone tube combined with retrograde catheterization has the characteristics of simple operation, light postoperative stimulation, safe catheterization state and significant anastomosis effect. The retrograde catheterization can better protect the small lacrimal spots.

Background@#Dacryocystorhinostomy (DCR) is a surgical treatment for site of obstruction in the nasolacrimal duct system [1]. This surgery is performed in cases of congenital nasolacrimal obstruction, partial and complete occlusion of the nasolacrimal duct in adults, chronic tearing disorders, ineffective probing treatment and placement of a silicone tube in the nasolacrimal system [2,3]. the present findings indicate that preservation of lacrimal sac flaps with end-to-end anastomosis to the nasal mucosa using histoacryl has a positive impact on surgical outcome in patients undergoing endonasal endoscopic dacryocystorhinostomy [4]. Histoacryl glue is one of the most commonly surgical materials used recently. The basis of our study is the lack of research on the use of this glue in the practice of oculoplastic and reconstructive surgery.@*Purpose@#The purpose of this study to comparing outcomes of surgical methods of histoacryl glue and silicone tube during endoscopic endonasal dacryocystorhinostomy.@*Methods@#Retrospective, case series study. The study included 83 patients who underwent endonasal endoscopic dacryocystorhinostomy between January 2017 to June 2020. Whole case was divided into two groups. In group 1, an anastomosis was made during surgery by using histoacryl glue to the posterior wall of the lacrimal sac and the wall formed by the nasal mucosa [5]. In group 2, a silicone tube was placed in the nasolacrimal duct during endonasal dacryocystorhinostomy and the results were evaluated in the 1st, 3rd, 6th, and 12th months after surgery. Statistical calculations were performed using SPSS software.@*Results@#The age of the whole cases was between 17 and 82 years. There were no significant difference among the age groups. In terms of gender, n=48 (78.7%) were female and n=13 (21.3%) were male, indicating that dacryocystitis was predominant in women. Study reports no significant difference in intraoperative and postoperative complication. The surgical success rate was significantly higher in histoacryl group 94.9%, than silicone tube group 79.5%. Recurrent cases n=2 (5.1%) in group 1 and n=9 (20.4%) in group 2 respectively. @*Conclusion@#Histoacryl glue is highly effective, commodity, inexpensive and easy to apply during endonasal endoscopic dacryocystorhinostomy surgery.

The basis of our study is the lack of research on the use of this glue in the practice of oculoplastic and reconstructive surgery.@*Purpose@#To report the use of fibrin glue as an adjunctive procedure in endoscopic dacryocystorhinostomy (En-DCR) surgery and compare it to silicone tubing@*Material and Method@#Patients with primary nasolacrimal duct obstruction were retrospectively reviewed for endo-DCR between January 2017 and December 2019. Group I patients applied fibrin glue and Group II used silicone tube insertion. The fibrin glue group received end-to-end anastomosis of the nasal mucosa and lacrimal sac using fibrin glue at the end of surgery. The demographics, recurrence, causes of failure, formation of scars, final ostium size and the success rate were compared. This study was approved by the Institutional Review Board of the Chang Gung Memorial Hospital Clinical Research Ethnics Committee and followed the Declaration of Helsinki. All statistical analysis was performed with SPSS version 24.0 software (SPSS Inc., Chicago, IL, USA), and a P value < .05 was considered statistically significant. Continuous variables were presented as mean ± standard deviation, while categorical data was represented as numbers and percentages. Independent t-tests were used to compare the differences in parametric variables. The Mann-Whitney U test was performed to compare the follow-up period. Pearson’s chi-squared and Fisher’s exact tests were used to compare categorical variables. Multivariate logistic regression was used to identify the risk factors associated with recurrence. @*Result@#The age of the whole cases was between 17 and 82 years. ABSTRACT

@#AIM: To evaluate the clinical effecacy of the RS-1 silicone tube and the conventional silicone tube in the management of pediatric canalicular lacerations.<p>METHODS: We retrospectively reviewed the medical records of 48 pediatric patients(48 eyes)with canalicular lacerations admitted for primary repair in the Department of Ocular Trauma of Hebei Eye Hospital from January 2015 to June 2018. The patients were divided into two groups according to the treatment of method: Group A(25 patients, the conventional silicone tube), Group B(23 patients, the RS-1 silicone tube). The management of pediatric canalicular lacerations were performed under general anesthesia. The silicone tube was removed about 3mo after surgery. The surgical time, clinical efficacy and complications were compared.<p>RESULTS: The mean surgical time between the two groups was 44.92±14.45min in Group A and 31.78±7.40min in Group B, which was statistically significant difference(<i>t</i>=4.02, <i>P</i><0.01). The clinical efficacy of the two groups was compared. Group A: 20 patients were cured(80%), 2 patients were improved(8%), 3 patients were failed(12%), and the functional success rate was 88%. Group B: 19 patients were cured(82%), 2 patients were improved(9%), 2 patients were failed(9%), the functional success rate was 91%, which was not statistically significant difference(χ2=0.14, <i>P</i>>0.05). The incidence of complications(32% <i>vs</i> 9%)including lower lacrimal point and mild eyelid eversion(1 patient 4% <i>vs</i> 1 patient 4%), slit canaliculus(2 patients 8% <i>vs</i> 1 patient 4%), early tube extrusion(3 patients 12% <i>vs</i> 0%), and nasal mucosal injury(3 patients 12% <i>vs</i> 0%)in Groups A and B, respectively, were comparable. There was statistically significant difference in the incidence of complications between the groups(χ2=3.94, <i>P</i><0.05).<p>CONCLUSION: There is no statistically significant difference with the two different types of silicone tube, which have good clinical efficacy, in the management of pediatric canalicular lacerations. But the RS-1 silicone tube is easier to operate, shorter surgical time, less complications and easier to removed.

It can act as both tracheostomy tube and stent to keep the tracheal airway open after repair of tra-cheal stenosis.Here we report a successful management of a case of 45-year old male who had Montgomery tube in situ posted for Direct Laryngoscopy assessment to rule out subglottic steno-sis before decannulation trial.

@#AIM: To explore the usefulness of RS-type lacrimal drainage tube versus double silicone tube for children's lacrimal canaliculi rupture.<p>METHODS: A retrospective analysis from August 2016 and July 2018 were taken. Totally 37 children(37 eyes)of lacrimal canaliculi rupture were enrolled in the study and divided into two groups according to admission time sequence: treatment group(20 eyes)and control group(17 eyes). The treatment group was treated with RS-type lacrimal drainage tube. The control group underwent the implantation of double silicone tube and their efficacy was analyzed.<p>RESULTS: It was significant difference that the epistaxis rate between the treatment group and the control group, which was 0% and 100%. The effective rate was 95% and 88%, respectively, for the treatment group and the control group, the difference between the two groups has no significant(<i>P</i>=0.584). And the postoperative complication rate was 5% in treatment group, 12% in control group respectively, there was no significant statistical difference between the two groups(<i>P</i>=0.584).<p>CONCLUSION: The implantation of the RS-type lacrimal drainage tube can effectively prevent epistaxis, is safe and simple, easy to operate and extubate easily with good clinical effect in the treatment of lacrimal canaliculus fracture in children. So it can be widely carried out in basic hospitals.

Objective@#To development of ethylene glycol samples in silicone tubes, and the samples were applied to inter-laboratory comparison, through it to evaluate the detection capacity of occupational health testing laboratories.@*Methods@#Three content levels of ethylene glycol samples in silicone tubes were prepared, and the homogeneity and stability of samples were investigated, the results were statistically analyzed. A robust statistical four-point distance method was used to calculate the results submitted by each participant laboratory and the test capability of the laboratory was assessed by the z-score method.@*Results@#The statistic of the homogeneity of ethylene glycol samples in silicone tubes were less than the critical value (P>0.05) , and the samples were stable at room temperature for one month. The satisfactory rate of comparison between 204 laboratories was 88.24%.@*Conclusion@#The homogeneity and stability of the ethylene glycol samples in the silicone tubes can meet the requirements of proficiency testing and can be used for laboratory comparison. Most of the laboratories that participated in the comparison had the ability to detect ethylene glycol.

@#AIM:To explore the clinical outcomes of RS lacrimal tube for lacrimal canaliculi rupture without nasal endoscope.<p>METHODS:A retrospective analysis from September 2017 to March 2018 were taken. According to admission time sequence. Totally 52 patients with lacrimal canaliculi rupture was divided into two groups. One group was treated with RS lacrimal tube, the other was treated with annular silicone tube. The treatment results were comparatively analyzed. <p>RESULTS:There were 15 eyes of nasal mucosa hemorrhage in annular silicone tube group. There was no eye with this in RS tube group, the difference between the two groups was significant(<i>P</i><0.001). In RS tube group, 26 eyes were cured, 3 eyes improved, 3 eyes failed. The effective rate was 91%. In annular silicone tube group, 15 eyes were cured, 3 eyes improved, 2 eyes failed. The effective rate was 90%, the clinical efficacy was not significantly different between groups(<i>P</i>=0.877). There were 2 eyes in RS tube group occurred fall off in 2wk to 2mo after operations; 1 eye of lacrimal puncture were removed from the lacrimal puncture without 2wk, and was anastomosed again. The other eye was removed from the lacrimal puncture within 1-2mo after the operation, the lacrimal canal flushing was improved during follow-up. There were 3 eyes in annular silicone group occurred fall off without 2wk, and were not anastomosed again. The lacrimal duct flushing was improved during follow-up. No statistical significance was found in the two groups(<i>P</i>=0.361). <p>CONCLUSION:Lacrimal canaliculi rupture should be timely given surgical treatment. RS lacrimal tube can not only achieve good operation effect and reduce the probability of complications, but also has the advantages of simple surgical methods, good histocompatibility and no influence on the appearance of patients.

ABSTRACT

@#AIM: To evaluate the clinical efficacy of a newly designed silicone tube for the repair of canalicular lacerations.<p>METHODS: This was a prospective clinical study. Data were analyzed from 47 eyes of 47 patients presenting with traumatic canalicular laceration in our hospital from January 2013 to October 2015. The newly designed silicone tubes were inserted through the lacrimal punctum and left in the bicanaliculi for 3mo. Data of the lacrimal duct patency, epiphora, and eyelid shape were recorded.<p>RESULTS: Surgery was performed successfully in all cases. No complications associated with the silicone tubes occurred. In total, 41 patients(87.23%)achieved complete success, 4 patients(8.51%)achieved partial success, and 2 patients(4.26%)had surgical failure.<p>CONCLUSION: The newly designed silicone tube is an effective and atraumatic tool for the management of canalicular lacerations. The operation process is simple, and is easy to grasp for the surgeon.

ABSTRACT

PURPOSE: We evaluated the clinical efficacy of lacrimal endoscopy-assisted silicone tube intubation in patients with a nasolacrimal duct obstruction. METHODS: We conducted a retrospective chart review of 86 eyes of 67 patients who underwent lacrimal endoscopy (RUIDO fiberscope; Fibertechco, Tokyo, Japan)-assisted silicone tube intubation from December 2014 to March 2017. We compared clinical characteristics, irrigation test results, and dacryocystographic and lacrimal endoscopic findings, and analyzed factors related to surgical success. RESULTS: In total, 86 eyes of 67 patients underwent lacrimal endoscopy-assisted silicone tube intubation. The success rate was 87.2%. There was a significantly lower preoperative tear meniscus height (420.5 ± 198.1 µm vs. 639.0 ± 224.3 µm, p < 0.001). In the surgically successful group, narrowing was frequently observed (29.0% vs. 0%, p = 0.030). Dacryolith findings were associated with surgical failure (10% vs. 29%, p = 0.043). CONCLUSIONS: Lacrimal endoscopy-assisted silicone tube intubation is considered an effective and successful operative procedure and enables the observation of real-time findings inside the lacrimal drainage passage for the treatment of pathological lesions. Narrowing observed during lacrimal endoscopy indicated successful treatment as opposed to dacryolith findings, which were associated with a failed outcome.

ABSTRACT

PURPOSE: To compare the success rates of performing only silicone tube intubation versus carrying out both conjunctival resection and silicone tube intubation. METHODS: The subjects of this study involved 62 patients (96 eyes) between October 2015 and May 2017 who were diagnosed as having punctal stricture or nasolacrimal duct stenosis. Out of 96 eyes, 47 underwent only silicone tube intubation, and 49 underwent both silicone tube intubation and conjunctival resection. Three parameters were measured at 1, 3, and 6 months after the surgery: the area of the tear meniscus using RTVue-100 anterior segment optical coherence tomography, the height of the tear meniscus using a slit lamp microscope, and the subjective satisfaction of patients as a result of improved sympotms like epiphora. The surgery was considered successful when the patients' experienced the resolution of symptoms and reduction of the area and height of the tear meniscus. RESULTS: The area of the tear meniscus, height of the tear meniscus, and subjective satisfaction of patients was superior in the group that underwent both silicone tube intubation and conjunctival resection compared silicone tube intubation only. Based on these results, the success rate of the surgery was 68.9% in the group that underwent only silicone tube intubation and 78.7% in the group that underwent both silicone tube intubation and conjunctival resection. CONCLUSIONS: The resection of relaxed plica semilunares seems to increase the success rate of silicone tube intubation through the reduction of the area and height of the tear meniscus. Therefore, after determining the degree of conjunctivochalasis, if it was found to be severe, a combination with conjunctival resection was expected to increase the success rate of the surgery.

· AIM:To observe the curative effect of different operative methods on acquired lacrimal point stenosis or adhesion,and to investigate the better treatment method for them.· METHODS:Totally 52 cases (72 eyes) suffered from acquired lacrimal point stenosis or adhesion,were divided into treatment group and control group by random number table,in which 27 cases (36 eyes) in treatment group were treated by modified tear incision combined with silicone tube implantation,25 cases (36 eyes) in control group were treated by silicone tube implantation.All cases were followed up for 12mo.· RESULTS:The total effective rate of the treatment group was 94%.The total effective rate of the control group was 78%.The difference between two groups was statistically significant (P<0.05).Silicone tube shedded in two eyes in treatment group and three eyes in control group.The rate of silicone tube shedded in two was 6% and 8%,respectively.The difference between two groups was not statistically significant (P>0.05).None case had lacrimal point tear in two groups.· CONCLUSION:Modified tear point incision combined with silicone tube implantation is the better operative methods on acquired lacrimal point stenosis or adhesion.It is easily operating,less damage with quick recovery.And effective rate is high.

ABSTRACT

Endoscopic dacryocystorhinostomy (DCR) is a widely used procedure for nasolacrimal duct obstruction. Because endoscopic DCR has shown higher success rate, fewer complications, and better cosmetic outcome compared to the conventional external approach, it has replaced the external approach. However, since the openings of the nasal cavity formed during surgery are small, recurrence often occurs due to stenosis caused by granuloma formation or the silicone tube. Hence, it is important to remove the silicone tube before granuloma formation around the openings of the nasal cavity after surgery. Failure to remove the silicone tube at the appropriate time can cause inflammation, resulting in granuloma formation. We recently experienced two cases of recurrent nasolacrimal duct obstruction caused by a remaining silicone tube. Here, we present these cases with a brief review of the literature.

ABSTRACT

OBJECTIVE To compare the costeffectiveness of two surgical approaches:endoscopic silicone tube intubation(ESTI) and conventional blind silicone tube intubation(CBSTI),in the management of chronic dacryocystitis(CDC).METHODS There were 46 cases of CDC from the Third Affiliated Hospital of Sun Yat-sen University from 2014 to 2015.Randomly,22 CDC patients were included in ESTI,24 patients were performed CBSTI.We analyzed both the final success rate,operating time,intraoperative visual analogue scale(VAS) and the rate of post-operative complications,as well as the final therapeutic effect.RESULTS In ESTI group,17 cases were cured,5 cases were improved and 3 cases were invalid.The success rate was 88.00%.Correspondingly for CBSTI group,14 cases were cured,6 eases were improved and 5 cases were invalid,and the success rate was 80.00%.ESTI was better,but there was no significant in success rate between the two groups (x2=0.881,P=0.644).Besides,the operating time and intraoperative VAS score in ESTI group was (10.32±2.30)min and 2.02±0.86,and they were(25.32 ± 4.87)min and 4.11 ± 1.44 in CBSTI group.So ESTI was better than CBSTI(t=-13.918,P=0.000;t=-6.012,P=0.000).ESTI had fewer complications(x2=4.878,P=0.027).CONCLUSION Compared to CBSTI,ESTI is a minimally invasive and highly effective technique for the treatment of CDC.The visualization of nasal endoscopy is the optimization of CBSTI,and this method need to be popularization and application.

@#AIM: To observe the clinical effects of modified endoscopic dacryocystorhinostomy(EDCR)for chronic dacryocystitis and try to find out an effective adjuvant method to improve the long-term effect of the modified surgery.<p>METHODS: Totally 136 cases(158 eyes)of chronic dacryocystitis were enrolled in the study and randomly divided into two groups:treatment group(80 eyes)and control group(78 eyes).The treatment group underwent the modified EDCR(intraoperative mytomycin C combined with silicone tube implatation and the use of tobramycin and dexamethasone eye ointment).The control group was only applyed mytomycin C during EDCR.The postoperative follow-up period was for 3-12mo.<p>RESULTS: At 6mo postoperatively,the cure rate and total effective rate of the treatment group were 95.0%, 98.8% respectively,and the control group of it were 75.6%, 93.6%.The effective rate of the treatment group was higher than that of the control group significantly(<i>χ</i><sup>2</sup>=11.90,<i>P</i><0.05).<p>CONCLUSION: The modified EDCR is a new surgical method which can prevent postoperative cicatricial adhesion and elevate surgical effective rate, and it has clear field, minimal invasion, quick recovery, exact effect and less recurrence.

AIM: To explore the curative effect of different ways of silicone tube catheter for the treatment of children`s lacrimal canaliculi rupture.METHODS: A retrospective analysis from January 2014 to December 2015 were taken.According to admission time sequence,42 children with lacrimal canaliculi rupture was divided into two groups.One group was treated with single silicone tube insertion,the other was treated with annular silicone tuber insertion.The treatment results were comparatively analyzed.RESULTS: There were 4 eyes in single silicone tube group occurred fall off in 2wk to 1mo after operations because of improper nursing and the second operations were taken.There was no eye with this in annular silicone tuber groups,the difference between the two groups was significant(P<0.05).In single silicone tube group,16 eyes were cured,1 eye improved,3 eyes unchanged which were the 3 eyes with fall off.Those 3 eyes were 1 patient bitten by dog,2 bombed;the effective rate was 85%.In annular silicone tuber insertion,20 eyes were cured,2 eyes improved and the effective rate was 100%,the difference between the two groups was significant (P<0.05).CONCLUSION: Children`s lacrimal canaliculi rupture should be timely given surgical treatment,reasonable surgical treatment can achieve good operation effect.Annular silicone tube has good stability,fewer complications than single silicone tube,and clinical treatment effect is good.

ABSTRACT

AIM: To investigate the feasibility and clinical effect of punctoplasty by using trabeculectomy punch combined with a novel RS tube for the treatment of punctal stenosis.METHODS: Totally 39 patients (39 eyes) with punctual stenosis were selected from October 2013 to October 2015 in the Second People`s Hospital of Foshan.All patients underwent punctoplasty by using trabeculectomy punch combined with a novel RS tube.These tubes were removed at 3mo after operation.A follow-up of 6mo was taken for final analysis.The fluorescein dye disappearance test score was recorded before the operation and at 1,3 and 6mo after the extubation.The curative effect of the operation at 6mo after the extubation was assess.RESULTS: Fluorescein dye disappearance test: the scores at 1,3 and 6mo after the extubation all decreased compared with the preoperative ones.The difference was statistically significant(P<0.05).At the last following up, 35 eyes (90%) were cured completely, 4 eyes (10%) were improved significantly, no patients recurred.Effective rate was 100%.No serious intraoperative and postoperative complications happened.CONCLUSION: Punctoplasty by using trabeculectomy punch combined with novel RS tubes is a safe and effective method for the punctul stenosis, which is easy to perform, with high success rate.

ABSTRACT

Objective To explore the effect of pigtail and silicone catheters on thoracic close drainage. Methods Sixty-one patients undergoing closed thoracic drainage hospitalized in our department from June 2013 to November 2016 were divided into the control group (n=31) and observation group (n=30). The control group was treated with closed pleural drainage using silicone catheter and the observation group using pigtail catheter. The two groups were compared in respect of drainage effect nursed the same. Results The extubation time, wound healing time of the observation group were all significantly shorter than those of the control group ( P<0.05). The pain score, complication rate and hospitalization cost were all significantly lower as well ( P < 0.05). Conclusions The pigtail catheter is more effective in the treatment of pneumothorax. It can alleviate pains, reduce the complication rate and surgical scar, and improve patient's satisfaction.

ABSTRACT

PURPOSE: To compare surgical success rates and complications of silicone tube intubation using 0.64-mm- and 0.94-mm-diameter tubes in adult patients with nasolacrimal duct stenosis. METHODS: In 62 eyes of 36 patients diagnosed with nasolacrimal duct stenosis who had undergone silicone tube intubation, we surveyed sex, age, symptom duration, time of tube removal, mean follow-up period, and surgical success rate between two groups divided by silicone tube diameter, 0.64 mm and 0.94 mm. By preoperative syringing test and dacryocystography, we examined the surgical results and complications of the two groups. RESULTS: The silicone tube diameter was 0.64 mm in 43 eyes (69.4%) and 0.94 mm in 19 eyes (30.2%). The surgical success rates in the two groups were 84.1% (36 eyes) and 78.9% (15 eyes), respectively, but the difference was not significant statistically. The surgical success rate was higher in partial stenosis than in complete stenosis but was not different according to silicone tube diameter according to preoperative syringing test and dacryocystography. Silicone tube disconnection and bending were only observed in 0.94-mm-diameter silicone tubes. CONCLUSIONS: In silicone tube intubation for nasolacrimal duct stenosis in adults, silicone tube diameter does not affect operation success. However, silicone tube disconnection and bending were observed only in the 0.94 mm silicone tube.
