ABSTRACT

Objective To explore the establishment and implementation of nursing quality evaluation system in department of orthopedics. Methods Randomly 527 patients hospitalized in the department of orthopedics from December 2011 to December 2012 were selected and assigned into the control group, where a traditional evaluation system was used. Another 528 patients hospitalized from April 2013 to June 2014 as the observation group, where orthopedics specialist evaluation system was used. The two groups were compared in terms of hospital evaluation accuracy, observation accuracy, accuracy rate of specialist intervention complications, the qualified rate of nursing records and patient satisfaction. Result The hospital evaluation accuracy, observation accuracy, the accuracy rate of specialist intervention complications, the qualified rate of nursing records and patients satisfaction of the observation group were significantly higher than those of the control group (P<0.01) and there was no incidence in two groups. Conclusion The establishment of orthopaedic nurse quality evaluation system can make up the shortage of traditional evaluation system and improve the quality of the orthopaedic nursing management.

ABSTRACT

Objective In order to provide information for the health administration and the price authorities to establish the reasonable price, the practical cost of special nursing of the chronic wound be analyzed, and be compared with the present charge, and the deviation be studied. Methods The cost of nursing care for all items was measured, recorded and collected, then calculating the special nursing cost for chronic wound using the method of ladder sharing the project cost.Rusults The true cost on a single special care for chronic wound was (29.42±9.29)yuan,and it reached 2.5 times of present charge ( 12 yuan). Conclusions The present charge on the special care for chronic wound was seriously deviated from the practical cost, and the labor value of special care of nurses could not be embodied.

ABSTRACT

As a result of introducing swallowing rehabilitation at a special nursing home for the elderly, a resident became able to take food orally for the first time in about 4 years since having a PEG tube inserted. Further, the resident's FIM, FG, and DSS values improved to some extent. While nutritional care using the PEG tube is recognized as a long-term enteral nutritional therapy, the tube is mostly used as a “PEG tube for survival” and scarcely used as a “PEG tube for eating”, which indicates a PEG tube for supporting oral ingestion. The result indicates that it is preferable to use the PEG tube “for eating” in addition to its use as an option for long-term enteral nutritional therapy. The result also indicates the need to conduct, with the cooperation of nurses and nursing care workers, the nutritional care and regular follow-ups of all nursing home residents, as well as to maintain and improve the residents' swallowing function and their activities of daily living (ADL) by continuing swallowing rehabilitation, oral care, and measures against gastroesophageal reflux disease.

ABSTRACT

Objective To discuss the influence of establishment of special nursing clinic on medical compliance behavior of diabetic patients. Methods We randomly selected outpatients from the endocrine department and special nursing clinic as the control group and the observation group with 100 cases in each group respectively. The condition of medical compliance behavior and blood glucose control was observed and compared in the two groups. Results The patients visiting special nursing clinic were better in medical compliance behavior and blood glucose control. Conclusions Establishment of special nursing clinic for diabetic outpatients can improve the medical compliance behavior, satisfying the requirement for disease knowledge, enhance the shaping of healthy behavior and facilitate the control of disease progress.

ABSTRACT

@#Objective To evaluated the effect of special nursing after coronary intervention in elderly. Methods 142 cases of consecutive patients with coronary artery disease were treated with percutaneous coronary intervention.According to the nursing models, the patients were divided into special nursing group(86 case) and routine nursing group(56 case). Results The incidence of aypnia, dysphoria, hospital onset of infection or other complication after intervention in special nursing group was less than that of routine nursing group. Conclusion The special nursing may accelerate the recovery of the old patients with coronary artery disease after coronary intervention.

ABSTRACT

Objective To explore special nursing measures for patients with pulmonary edema related to uremia. Methods Treatment and nursing were performed on 101 patients with pulmonary edema related to uremia. Results Of the 101 patients, 84 cases were rescued successfully, 13 cases were dead and 3 cases were discharged automatically. Conclusion When nursing for patients with pulmonary edema related to uremia, besides general nursing measures, the following aspects should be included: ① specially observing the blood pressure, arterial blood oxygen saturation and urine volume of patients; ② controlling the blood pressure of patients effectively; ③ dialysing or ultrafiltrating in proper manner and at proper time, meanwhile nursing carefully during dialysis; ④ ameliorating the general nutritional status of patients to prevent infection of the lungs; ⑤ keeping firmly in mind the care program: postur sucking oxygen drugs of vasodilatation hemodialysis; ⑥ controlling water drinking.

ABSTRACT

This study was performed to evaluate the effects of a special nursing program on the recovery of patients with spontaneous intracranial hemorrhage, and on the level of satisfaction of their families. The experimental group comprised twenty patients selected from A university medical center, and the control group, nineteen patients selected from B university medical center. Patients with cormorbidity were not included. Between 1 October, 1995 and 30 April, 1996, personnel specialized in neurosurgical nursing provided the experimental group with a special nursing program, while the control group was offered general nursing services. The key elements of this special nursing program were close observation to identify potential risk factors ; crisis intervention to prevent gaps in medical service delivery ; patient-centered care emphasizing early recovery, including specific protocols for the prevention of disability and for rehabilitation ; and patient education and counseling to maximize satisfaction. The average length of stay in ICU, neurological status, functional status, impact of complications, and satisfaction level were measured through medical records, the Glasgow Coma Scale, Functional Independence Measurement, patient assessments, and questionnaires, respectively. These data were analyzed by paired t-test, independent-samples t-test, and ANOVA, and the results were as follows : 1) The average length of stay in ICU of the experimental group was significantly shorter than that of the control group. 2) The degree of neurological recovery of the experimental group between admission and the second week after operation was not significantly different from that of the control group. 3) The impact of complications on the control group was greater than on the experimental group. 4) Between the first and second week after operation, the functional status of the experimental group changed more noticeably than that of the control group. 5) The degree of satisfaction with the nursing services was higher among the families of patients in the experimental group than among the other families. These findings indicate that a special nursing program might improve the recovery of patients with spontaneous intracranial hemorrhage and increase the satisfaction of their families. It is therefore suggested that in order to provide comprehensive medical services to patients with spontaneous intracranial hemorrhage, personnel such as neurosurgical nursing specialist be appropriately utilized. In addition, the current medical insurance fee schedule especially in relation to the cost of rehabilitation, should be adjusted to cover broader services.
