ABSTRACT

ObjectiveTo evaluate the effectiveness of Gandou decoction (GDD) by analyzing theclinical efficacy of GDD combined with speech training on the treatment of dysarthria with endoretention of damp-heat in Wilson's disease (WD), so as to provide more clinical data and theoretical support for the selection of appropriate treatment schemes for WD patients with dysarthria with endoretention of damp-heat. MethodA total of 60 eligible WD patients with dysarthria with endoretention of damp-heat were selected and divided into a control group and a treatment group according to the random grouping method, with 30 cases in each group. The control group was treated with speech training + sodium dimercaptopropanesulfonate (DMPS), and the treatment group was combined with GDD on the basis of the control group, with eight days as a course of treatment for 32 days. The total clinical effectiveness rate (Goldstein clinical classification), dysarthria grading assessment from China Rehabilitation Research Center, TCM syndrome scores, 24-hour urine copper content, and modified Frenchay dysarthria rating scale scores of the two groups were compared before and after treatment. ResultAfter treatment, the total effective rate of the observation group was 90.0% (27/30), and that of the control group was 70.0% (21/30). The total effective rate of the observation group was significantly higher than that of the control group (Z=-1.986,P<0.05). After treatment, the modified Frenchay dysarthria score, dysarthria grading assessment from China Rehabilitation Research Center, and 24-h urine copper in the two groups were significantly increased (P<0.05, P<0.01), and the TCM syndrome score was significantly decreased (P<0.01). Compared with the control group after treatment, except for the respiratory and jaw score, the modified Frenchay dysarthria score of the observation group was significantly increased (P<0.05, P<0.01). The dysarthria grading from China Rehabilitation Research Center and 24-h urine copper content were significantly increased (P<0.01), and the observation group had better efficacy. During the study period, there were no serious adverse reactions such as fever, rash, oral and eyelid mucosal swelling, exfoliative dermatitis, vomiting, diarrhea, or allergic shock during copper excretion treatment of DPMS and oral administration of GDD. ConclusionGDD combined with speech training can improve the symptoms and efficacy of WD patients with dysarthria with endoretention of damp-heat and enhance the patients' living standard to a certain extent, which can be widely used in clinics.

ABSTRACT

Objective@#Analyzing the characteristics of consonant errors in children with functional dysarthria in different age groups and the effect of speech training provides a reference for clinical treatment. @*Methods @#This study followed medical ethics, and informed consent has been obtained from patients. Speech data from 388 patients with functional dysarthria were retrospectively studied. They were divided into two groups at the age of 6, namely, the preschool group (4-6 years old) of 226 patients and the school age group (6-13 years old, including 6 years old) of 162 patients. The characteristics of consonant pronunciation errors from four aspects were analyzed: average number of errors, pronunciation location, pronunciation method, and error type. One-on-one speech training was conducted, with a training frequency of once a week and once for 30 minutes. The training method was carried out in the order of phoneme training, syllable training, vocabulary training, sentence training, and short text and conversation training. The effects of speech training in the two groups were compared. @*Results@#Analysis by pronunciation location: both age groups had the highest frequency of errors in tongue tip posterior sounds; the school age group had the lowest error frequency for labiodental consonants, and the preschool group had the lowest error frequency for bilabial consonants. According to the analysis of pronunciation mode, both age groups had the highest error frequency of aspirated affricate and the lowest error frequency of nasal sound. Analysis by error type: both age groups are mainly characterized by substitution and omission. Compared with the preschool group, most consonants of patients in the school group tend to improve in terms of pronunciation location, pronunciation mode, and error types. Compared with the preschool group, the two types of errors-palatalization and lateralization-increased in frequency in the school group, but the trend of increased lateralization was not statistically significant. After 6.7 and 5.5 sessions of speech training, the pronunciation of the preschool group and the school-age group significantly improved; the cure rate of the school-age group was 84.9% (118/139), and that of the preschool group was 77.1% (91/118). There was no statistically significant difference in the cure rate between the two groups.@*Conclusion@# Functional dysarthria may improve with age, but it may not completely self-heal. Children of different age groups can achieve good treatment results through scientific and reasonable speech training.

ABSTRACT

Objective:To observe the effect of speech training based on mirror neuron theory on children with functional articulation disorder (FAD).Methods:Fifty children with FAD were randomly divided into a training group and a control group, each of 25. Both groups received 30 minutes of conventional speech training 5 times a week for 24 weeks, while the training group was additionally given 20 minutes of speech training based on mirror neuron theory simultaneously. Before and after the intervention, both groups were evaluated using the articulation speech ability assessment scale and the oral motor ability assessment scale.Results:Before the treatment, there were no significant differences between the 2 groups in any of the measurements. After the treatment, significant improvement was observed in all of the measurements for both groups, but at that point the training group′s articulation, motor ability of the lower jaw, tongue and lips were all significantly superior to the control group′s averages.Conclusions:Speech training based on mirror neuron theory can significantly improve the articulation, intelligibility and oral motor functioning of children with functional articulation disorder.

ABSTRACT

OBJECTIVE@#To verify the clinical efficacy of " acupuncture" combined with speech training for post-stroke aphasia.@*METHODS@#A total of 68 patients with post-stroke aphasia were randomly divided into an observation group and a control group, 34 cases in each one. The patients in the control group were treated with basic therapy and speech rehabilitation exercises (mainly Schuell's aphasia stimulation therapy). Based on the treatment of the control group, the patients in the observation group were treated with " acupuncture" at Shuigou (GV 26), Neiguan (PC 6), Sanyinjiao (SP 6), Lianquan (CV 23), Baihui (GV 20), Fengfu (GV 16), Yamen (GV 15), Hegu (LI 4) and Taichong (LR 3), once a day, six times a week; four-week treatment was taken as one session, and totally 2 sessions were given. The speech function scores, including comprehension, naming, retelling, writing and reading, were observed before and after treatment in both groups, and the efficacy of the two groups was evaluated.@*RESULTS@#The total effective rate was 91.2% (31/34) in the observation group, which was superior to 67.6% (23/34) in the control group (<0.05). Compared before treatment, each index of speech function scores in the two groups were increased significantly after treatment (all <0.05); compared with the control group, each index of speech function scores in the observation group were increased significantly after treatment (all <0.05).@*CONCLUSION@#Based on the basic treatment and speech training, " acupuncture" could improve the early rehabilitation effect of post-stroke aphasia.

ABSTRACT

Objective To explore the effects of the mouth -muscle strengthening training on the phonetic in-telligibility (PI) of children with functional articulation disorders (FAD) .Methods A total of 80 children of 4 to 7 years old with FAD were randomly divided into the common group (n=40) and comprehensive group (n=40) .Both groups accepted routine speech training ,and the comprehensive group was given mouth muscles training additional-ly .All the children received the assessment of speech before the speech training and 3 months after the training .Re-sults Before the speech training ,the PIs of the common and comprehensive groups were 34 .54% and 33 .52% ,re-spectively .The differences between the 2 groups were not statistically significant (P> 0 .05) .After 3 months of speech training ,the PI in the common group increased to 77 .02 ± 4 .21% from 34 .54 ± 7 .28% of the baseline ,while in the comprehensive group ,the PI increased to 87 .54 ± 4 .31% from 33 .52 ± 7 .46% .In both groups ,the PIs after training were significantly higher than those of before the training respectively .The differences were statistically significant (P<0 .05) .The PIs in the comprehensive group were significantly higher than those of in the common group .The differences were statistically significant (t= -8 .862 ,P<0 .001) .Conclusion Strengthening the mouth muscles is beneficial to improve the PI of children with FAD .

ABSTRACT

Objective To observe the clinical efficacy of cluster needling at the anterior oblique line of vertex-temporal plus speech training in treating motor aphasia after cerebral infarction.Method Forty patients with motor aphasia after cerebral infarction were randomized into a treatment group and a control group, 20 in each group. Both groups were given basic neurological treatment. In addition, the treatment group was intervened by cluster needling at the anterior oblique line of vertex-temporal plus speech training, while the control group was by speech training alone. After 4-week treatments, the speech function and communication ability were observed and the clinical efficacies were compared.Result The total effective rate was 90.0% in the treatment group versus 80.0% in the control group, and the difference was statistically significant (P<0.05). After treatment, the speech function score and Chinese Functional Communication Profile (CFCP) score were changed significantly in both groups (P<0.01). There were significant differences in comparing the speech function and CFCP scores between the two groups (P<0.01,P<0.05).Conclusion Cluster needling at the anterior oblique line of vertex-temporal plus speech training is an effective approach in treating motor aphasia after cerebral infarction.

ABSTRACT

@#Objective To discuss the clinical features and speech training of bilabial consonant articulation disorders. Methods 92 patients with functional articulation disorders (FAD) and 85 patients repaired cleft palate were studied. Then 20 patients with bilabial consonant articulation disorders were selected respectively from two types of patients for speech training. Results FAD patients who replaced /p/with /b/ accounted for 78.8% in patients with bilabial consonant articulation disorders, and the patients repaired cleft palate dropped or replaced /b/ accounted for 30.2% and 60.4% respectively. Both types of the patients with bilabial consonant articulation disorders made less mistakes after speech training (P<0.001). Conclusion Bilabial consonant articulation disorders mainly show unaspirated about /p/ in FAD patients, and show dropping and replacement about /b/ in the patients repaired cleft palate. The speech training is significantly effective.

ABSTRACT

Objective To observe the association between therapeutic effect of speech disorder and the age on treatment of children with cerebral palsy,and to explore the best treatment period of speech disorder children with cerebral palsy.Methods One hundred and twenty-one cases of speech disorder children with cerebral palsy aged 1 to 6 years in Maternal and Child Health Center of Hebei Province,Hebei Children's Hospital,were selected as subjects,and the patients were randomly divided into training group,which included 64 cases who received scalp acupuncture combined with language and 57 cases treated with speech training group.After 6 courses of treatment,the therapeutic effect for speech disorder of children between 1 to 3 years old and > 3 to 6 years old were compared,the association between children's age for treatment and treatment effects was explored.Results The total treatment efficiency of 1 to 3 years and > 3 to 6 years old children in scalp acupuncture group combined with language training group were 95.45% and 83.33% respectively,the difference was significant (x2 =6.859 8,P < 0.05) ; while in 1 to 3 years and > 3 to 6 year-old children in speech training group the total treatment efficiency were 84.62% and 77.42%,and the difference was significant(x2 =6.625 7,P < 0.05).In both groups,total efficiency of 1 to 3 years and > 3 to 6 years old children were 89.58% and 80.82%,respectively,and the difference was statistically significant (x2 =9.617 5,P < 0.01).Conclusions The best treatment period of speech disorder children with cerebral palsy is less than 3 years old,as age grows,the effect is gradually decreasing.

Objective To observe the effects of auditory integration training on language development delay in mental retardation chil-dren. Methods 40 cases were devided into observation group (n=20) and control group (n=20). All the children accepted speech training, while the observation group accepted auditory integration training in addition. They were assessed with the development quotient (DQ) of adaptability and language of Gesell developmental schedules before and 6 months after treatment. Results DQ of adaptability and language improved after treatment in both groups (P<0.05), and improved more in the observation group than in the control group (P<0.05). Conclu-sion Auditory integration training may further improve the language development in mental retardation children.

ABSTRACT

@#ObjectiveTo observe the speech outcome of globle aphasia after rehabilitation.Methods9 patients with globle aphasia from Dec. 2008 to June. 2010 in our hospital were assessed with the Standard Aphasia Examination of Chinese(CRRCAE) and trained using the Schuell aphasic stimulation approach, promoting aphasics communication effectiveness (PACE) and functional communication therapy (FCP).Results9 patients were disabled with their faces, 8 cases were disabled with speech. 9 cases improved in understanding, retelling, naming, and copying after speech training(P<0.05). Boston Diagnostic Aphasia Examination (BDAE) showed 2 cases had obvious improvement, and 2 patients had mild improvement.ConclusionThe speech rehabilitation on globle aphasia is very difficult, but speech function can recover to a certain extent and the type of aphasia can tranffer after speech training, and it can raise the ability of daily exchange by combining nonspeech means.

ABSTRACT

Objective To investigate the phonological characteristics of patients with functional speech problems. Methods Ninety patients with functional speech problems were classified by speech analysis. All types of speech problems were analyzed. Results The functional speech problems can be categorized as unaspiration, palatalization, lateralization, fronting of tongue, backing of tongue, plosion, omission of consonants, glottal stop, affrication and backing of tongue and nasalization. Backing of tongue and nasalization was only related to the consonant l, and the unaspiration was often related to such consonants as p, t, k, q, c, ch. Conclusion The functional speech problems were related to consonants. There was regularity with the involvement of the consonants in different types of functional speech problems.
