ABSTRACT

ObjectiveTo induce the rat model of ulcerative colitis (UC) with spleen-kidney Yang deficiency and liver depression, and explore the efficacy and mechanism of Sishenwan combined with Tongxie Yaofang (SSW&TXYF) based on the therapeutic principles of tonifying spleen, soothing liver, warming kidney, and astringing intestine. MethodSixty male SD rats were randomized into normal, model, mesalazine, and high-, medium-, and low-dose SSW&TXYF groups. The rats in other groups except the normal group were administrated with Sennae Folium decoction and hydrocortisone and received tail clamping for 14 days. On day 14, rats received enema with TNBS-ethanol solution to induce UC. The rats were administrated with corresponding drugs from day 15 of modeling, and the body weight and mental state were observed and recorded. The sucrose preference test was performed from day 25. On day 28, the rectal temperature was measured, and the rats were administrated with 3% D-xylose solution at a dose of 10 mL·kg-1 by gavage. Blood was sampled 1 h later, from which the serum was collected for measurement of the D-xylose content. The serum, hippocampus, and colorectum samples of rats were collected on day 29. The levels of gastrin (GAS), adrenocorticotropic hormone (ACTH), corticosterone (CORT), cyclic adenosine monophosphate (cAMP), cyclic guanosine monophosphate (cGMP), interleukin (IL)-4, tumor necrosis factor (TNF)-α, and interferon (IFN)-γ in the serum and 5-hydroxytryptamine (5-HT) in the hippocampus were determined by enzyme-linked immunosorbent assay. Hematoxylin-eosin staining was employed to reveal the colonic lesions. The mRNA and protein levels of p38 mitogen-activated protein kinase (MAPK), extracellular signal-regulated kinase (ERK), and c-Jun N-terminal kinase (JNK) in the colon tissue were determined by Real-time PCR and Western blot, respectively. ResultCompared with the normal group, the model group showed decreased body weight, anal temperature, and D-xylose content in the serum and increased GAS content (P<0.01). The modeling led to cAMP/cGMP unbalance and decreased the ACTH and CORT content in the serum (P<0.01), the preference for sucrose water, and the 5-HT content in the hippocampus (P<0.01). Moreover, it shortened the colorectal length and caused massive infiltration of inflammatory cells and severe structural damage in the colon tissue. High, medium, and low doses of SSW&TXYF improved above indicators (P<0.05, P<0.01), reduced inflammatory infiltration, and repaired the pathological damage of the tissue. Compared with the normal group, the model group showed lowered IL-4 level (P<0.01) and elevated TNF-α and IFN-γ levels (P<0.05, P<0.01) in the serum, as well as up-regulated expression of p38 MAPK, ERK, and JNK (P<0.05, P<0.01). Compared with the model group, SSW&TXYF elevated the IL-4 level (P<0.01), lowered the TNF-α and IFN-γ levels (P<0.05, P<0.01), and down-regulated the mRNA and protein levels of p38 MAPK, ERK, and JNK (P<0.05, P<0.01). ConclusionA rat model of UC with spleen-kidney Yang deficiency and liver depression was successfully established. SSW&TXYF can significantly mitigate this syndrome by reducing the inflammatory response in the colon and inhibiting the MAPK pathway.

ABSTRACT

ObjectiveTo observe the clinical effect of modified Jichuanjian on senile patients with slow transit constipation of spleen-kidney yang deficiency syndrome and the influence on brain-gut peptide. MethodA total of 150 senile patients with slow transit constipation were randomized into control group (75 cases) and observation group (75 cases) with the random number table method. The observation group was given modified Jichuanjian (oral, 1 dose/day, 4 weeks), and the control group was treated with Biantong Capsules (oral, 3 capsules/time, twice/day, 4 weeks). Data before and after treatment were recorded, including the score of major constipation symptoms, score of Patient Assessment of Constipation Quality of Life (PAC-QOL), TCM syndrome score, spontaneous complete bowel movements (SCBM), colonic transit test, serum 5-hydroxytryptamine (5-HT), 5-HT 4 receptor (5-HT4R), somatostatin (SS), and vasoactive intestinal peptide (VIP), and recurrence. ResultThe total effective rate of the observation group was 93.06% (67/72), as compared with the 74.65% (53/71) in the control group (χ2=8.974 6, P<0.01). After treatment, the scores of major constipation symptoms, scores of four dimensions of PAC-QOL, total score of PAC-QOL, and TCM syndrome score were lower than those before treatment in the two groups (P<0.01), and lower in the observation group than in the control group (P<0.01). The SCBM in the observation group were more than those in the control group at the 2nd, 3rd, 4th weeks after treatment (P<0.01). The proportions of residual markers at 24, 48, 72 h after treatment were smaller than those before treatment in the two groups (P<0.01), and smaller in the observation group than in the control group (P<0.01). After treatment, the levels of serum 5-HT and 5-HT4R were higher (P<0.01) and the levels of serum SS and VIP were lower (P<0.01) than those before treatment in the two groups. In addition, the levels of serum 5-HT and 5-HT4R in the observation group were higher (P<0.01) and the levels of serum SS and VIP were lower (P<0.01) in the observation group than in the control group. The recurrence in the observation group was 29.85% (20/67) in comparison with the 58.49% (31/53) in the control group (χ2=9.932 4, P<0.01). ConclusionModified Jichuanjian is effective for senile patients with slow transit constipation of spleen-kidney yang deficiency syndrome, which can alleviate clinical symptoms, improve quality of life, regulate the level of serum brain-gut peptide, improve the colonic transit function, increase SCBM, and reduce the recurrence.

ABSTRACT

To investigate the intervention effect and mechanism of Zhenwu Decoction on diabetic nephropathy(DN) mice of spleen-kidney Yang deficiency syndrome based on the Rho-associated coiled-coil kinase(ROCK)/IκB kinase(IKK)/nuclear factor-κB(NF-κB) pathway. Ninety-five 7-week-old db/db male mice and 25 7-week-old db/m male mice were fed adaptively for one week. The DN model of spleen-kidney Yang deficiency syndrome was induced by Dahuang Decoction combined with hydrocortisone by gavage, and then the model was evaluated. After modeling, they were randomly divided into a model group, high-dose, medium-dose, and low-dose Zhenwu Decoction groups(33.8, 16.9, and 8.45 g·kg~(-1)·d~(-1)), and an irbesartan group(25 mg·kg~(-1)·d~(-1)), with at least 15 animals in each group. The intervention lasted for eight weeks. After the intervention, body weight and food intake were measured. Serum crea-tinine(Scr), blood urea nitrogen(BUN), fasting blood glucose(FBG), urinary albumin(uALb), and urine creatinine(Ucr) were determined. The uALb/Ucr ratio(ACR) and 24 h urinary protein(UTP) were calculated. Renal pathological morphology was evaluated by HE staining and Masson staining. The levels of key molecular proteins in the ROCK/IKK/NF-κB pathway were detected by Western blot. Enzyme-linked immunosorbent assay(ELISA) was used to detect interleukin-1β(IL-1β), interleukin-6(IL-6), interleukin-8(IL-8), interleukin-10(IL-10), and tumor necrosis factor-α(TNF-α). Compared with the blank group, the model group showed increased content of BUN, uALb, and SCr, increased values of 24 h UTP and ACR, decreased content of Ucr(P<0.05), enlarged glomeruli, thickened basement membrane, mesangial matrix proliferation, inflammatory cell infiltration, and collagen fiber deposition. The protein expression of ROCK1, ROCK2, IKK, NF-κB, phosphorylated IKK(p-IKK), phosphorylated NF-κB(p-NF-κB), and phosphorylated inhibitor of NF-κB(p-IκB) increased(P<0.05), while the protein expression of inhibitor of NF-κB(IκB) decreased(P<0.05). The levels of inflammatory factors IL-1β, IL-6, IL-8, and TNF-α increased(P<0.05), while the level of IL-10 decreased(P<0.05). Compared with the model group, the groups with drug treatment showed decreased levels of BUN, uALb, SCr, 24 h UTP, and ACR, increased level of Ucr(P<0.05), and improved renal pathological status to varying degrees. The high-and medium-dose Zhenwu Decoction groups and the irbesartan group showed reduced protein expression of ROCK1, ROCK2, IKK, NF-κB, p-IKK, p-NF-κB, and p-IκB in the kidneys(P<0.05), increased protein expression of IκB(P<0.05), decreased levels of serum inflammatory factors IL-1β, IL-6, IL-8, and TNF-α(P<0.05), and increased level of IL-10(P<0.05). Zhenwu Decoction can significantly improve renal function and renal pathological damage in DN mice of spleen-kidney Yang deficiency syndrome, and its specific mechanism may be related to the inhibition of inflammatory response by down-regulating the expression of key molecules in the ROCK/IKK/NF-κB pathway in the kidney.

ABSTRACT

Objective::To explore the efficacy of modified Jingui Shenqitang in the treatment of renal hypertension with spleen-kidney yang deficiency syndrome and its effect on blood lipids, renal function and vascular endothelial function. Method::Totally 110 patients were randomly divided into control group and observation group by random number table method, with 55 cases in each group. Control group was given levamlodipine (2.5-5 mg every time, once/day) and enalapril maleate (10 mg every time, once/day), and observation group was given modified Jingui Shenqitang in addition to the therapy of control group (1 dose/day). They were treated for 12 weeks. Blood pressure monitoring was performed, the systolic blood pressure (SBD) and diastolic blood pressure (DBP) were compared before and after treatment, and the blood pressure compliance was calculated. The 24 h urinary protein quantification (24 hUpr), serum creatinine (SCr), albumin (ALB) and urea nitrogen (BUN) were detected before and after treatment, the glomerular filtration rate (eGFR) was calculated, and the triglyceride (TG), total cholesterol (TC), high-density lipoprotein cholesterol (HLD-C) and low-density lipoprotein( LDL-C) were detected before and after treatment. The spleen-kidney Yang deficiency syndromes were scored before and after treatment. The levels of nitric oxide (NO), plasma endothelin (ET) and angiotensin Ⅱ (Ang Ⅱ) were detected before and after treatment. Result::The blood pressure efficacy in observation group was better than that in control group (Z=1.905, P<0.05). The efficacy of traditional Chinese medicine(TCM) syndromes in observation group was better than that in control group (Z=2.416, P<0.01). The compliance rate of causal blood pressure in observation group was higher than that in control group (88.89% vs 71.25%) (χ2=7.861, P<0.01). SBP and DBP in observation group were lower than those in control group (P<0.05). TC and LDL-C in observation group were lower than those in control group (P<0.01). The 24 hUpr, BUN and SCr in observation group were lower than those in control group (P<0.05), while the eGFR was higher than that in control group (P<0.05). The levels of ET-1 and Ang Ⅱ in observation group were lower than those in control group (P<0.05), whereas the NO level was higher than that in control group (P<0.01). Conclusion::In addition to the routine intervention with western medicine, modified Jingui Shenqitang for patients with spleen-kidney Yang deficiency syndrome can further control blood pressure level, improve blood pressure compliance rate, regulate lipid metabolism, protect kidney function, and regulate vascular endothelial function, with a better clinical efficacy than pure western medicine.

Objective: To investigate the effects of Buguzhi Pills and three kinds of simulative Buguzhi Pills on diarrhea rats with spleen-kidney yang deficiency, and to confirm the rationality of the salt-processed composition of Buguzhi Pills. Methods: SD rats were randomly divided into nine groups: normal group, model group, positive drug group, salt-processed Psoraleae ructus group, raw Psoraleae Fructus group, Buguzhi Pills (salt-processed Psoraleae Fructus + salt-processed Foeniculi Fructus) group and three kinds of simulative Buguzhi Pills groups (salt-processed Psoraleae Fructus + raw Foeniculi Fructus; raw Psoraleae Fructus + salt-processed Foeniculi Fructus; raw Psoraleae Fructus + raw Foeniculi Fructus). A complex method of hydrocortisone and sennae folium was adopted to establish the diarrhea model due to spleen-kidney yang deficiency. After administration, the organ indexes of thymus, spleen, kidney, adrenal gland, and testis were measured; The pathological changes of corresponding organs were observed; The serum levels of motilin and testosterone were detected by ELISA. Results: Compared with the normal group, the food intake and body weight reduced; The organ indexes decreased in varying degrees; The above organs showed obvious pathological changes under the microscope; the level of motilin showed an upward trend, and the level of testosterone decreased significantly in model group. The above symptoms and indicators were improved in varying degrees after administration, and the effect of Buguzhi Pills was better than that of Psoraleae Fructus alone and three kinds of simulative Buguzhi Pills on the whole. Conclusion :The efficacy of Buguzhi Pills on warming kidney-spleen yang and relieving diarrhea was superior to that of Psoraleae Fructus alone, and the antidiarrheal effect can be enhanced by salt-processing.

ABSTRACT

Objective To observe the clinical efficacy of acupoint thread embedding plus thunder-fire moxibustion in treating hyperlipidemia due to spleen-kidney yang deficiency. Method Sixty patients with hyperlipidemia due to spleen-kidney yang deficiency were randomized into a treatment group and a control group, 30 cases each. The treatment group was intervened by acupoint thread embedding plus thunder-fire moxibustion, while the control group was intervened by oral administration of Atorvastatin calcium tablets. After 8-week treatment, changes in the serum total cholesterol (TC), triglyceride (TG), high density lipoprotein cholesterol (HDL-C), low density lipoprotein cholesterol (LDL-C) and traditional Chinese medicine (TCM) syndrome score were observed in the two groups, and the clinical efficacies were compared. Result The blood lipids indexes (TC, TG, HDL-C and LDL-C) and TCM syndrome score were significantly changed after the treatment in both groups (P<0.05). After the treatment, the blood lipids indexes and TCM syndrome score of the treatment group were insignificantly different from those of the control group (P>0.05). The total effective rate was 90.0% in improving the blood lipids in the treatment group, versus 93.3% in the control group, and the between-group difference was statistically insignificant (P>0.05). The total effective rate was 83.3% in improving the TCM syndrome score in the treatment group versus 63.3% in the control group, and the between-group difference was statistically significant (P<0.05). Conclusion Acupoint thread embedding plus thunder-fire moxibustion is an effective method in treating hyperlipidemia due to spleen-kidney yang deficiency.

ABSTRACT

Objective To evaluate the efficacy of Shenfu Decoction in treatment of critically ill patients with spleen-kidney yang deficiency type low triiodothyronine syndrome (LT3S). Methods Totally 110 critically ill patients with spleen-kidney yang deficiency type LT3S were randomized into two groups. Control group used standard Western treatment; Treatment group was given the combination of Western treatment and Shenfu Decoction; each group was treated for 7 d. The therapeutic effect indexes were serum T3, serum T4, serum TSH, APACHE II score, hospital day, results of treatment, and therapeutic efficacy of TCM syndromes. Results After treatment, the APACHE II score in control and treatment groups have been significantly improved (P < 0.05, 0.001). Compared with control group, the score in treatment group declined more drastically, and difference between two groups was statistically significant (P < 0.05); The serum T3 levels have been significantly improved in both groups and improved more in treatment group, and the difference between two groups was statistically significant (P < 0.05); The hospital day of treatment group was shorter than control group after treatment, and the difference between two groups was statistically significant (P < 0.05); The symptoms score progressed in both groups after treatment and have been significantly improved in treatment group, and the difference between two groups was statistically significant (P < 0.05); The treatment group had more significant clinical curative effect after treatment (P < 0.05). Conclusion Shenfu Decoction in critically ill patients with spleen-kidney yang deficiency type LT3S could improve the clinical curative effect, progress the TCM syndromes and results of treatment, and shorten the hospital stay.

ABSTRACT

Objective To observe the effects of Jiuxieling Granules on the expressions of MyD88 and IRAK1 in ulcerative colitis model rats with spleen-kidney yang deficiency; To discuss its mechanism of action. Methods Animal models were established by compound methods. 90 Wistar rats were randomly divided into blank group, model group, positive medicine group, and Jiuxieling Granules high-, medium-, and low-dose groups. Each administration group was given relevant medicine for gavage. RT-qPCR, SP immunohistochemistry and Western blot were used to detect mRNA and proteins of MyD88 and IRAK1 in colon tissues. Results Compared with the blank group, mRNA and proteins of MyD88 and IRAK1 in the model group increased (P<0.01). Compared with the model group, mRNA and proteins of MyD88 and IRAK1 in each administration group decreased (P<0.01), especially in Jiuxieling Granules high-dose group. Conclusion Jiuxieling Granules can reduce the expressions of MyD88 and IRAK1, and then influence the transmission of MyD88 signaling pathway and block the release of downstream inflammatory factors to achieve the result of treating ulcerative colitis with spleen-kidney yang deficiency.

ABSTRACT

This study was aimed to observe the clinical curative effect of Wen-Yang Jian-Pi (WYJP) decoction for treating hepatitis B cirrhosis patients with refractory ascites.A total of 81 hepatitis B cirrhosis patients with refractory ascites after admission to our hospital were randomly divided into two groups,41 patients were in the treatment group and 40 were in the control group.Patients in both groups received basic treatment,including antivirus,liver-protective,antiinfective and diuretic therapy,as well as albumin supplementation;while the treatment group were additionally given oral traditional Chinese medicine (TCM) of WYJP decoction at the same time.Changes of subjective symptoms,abdominal circumference,urine volume,main hepatic function indexes,and main indexes of B-ultrasound before and after treatment were measured and compared between two groups.Patients in both groups were treated for 3 one-month therapeutic courses,with 3 to 5 day-intervals,after which the curative effect was evaluated.The results showed that the total effective rate in the treatment group was 87.8%,which was significant higher than that of 70.0% in the control group (P < 0.05).And the treatment group in ascites,urine volume,symptoms,body signs and B ultrasonic improvement were better than those of the control group,with statistical significance (P < 0.05).It was concluded that WYJP decoction has definite therapeutic effect on hepatitis B cirrhosis patients with refractory ascites.

Objective To observe the change rules of expressions of IL-1β, IL-4, IFN-γ and TGF-β1 in serum and colon tissue in rats with ulcerative colitis of spleen-kidney yang deficiency type; To explore the action in the progress of ulcerative colitis of spleen-kidney yang deficiency type.MethodsComposite method was used to establish rat models with ulcerative colitis of spleen-kidney yang deficiency type. 75 Wistar rats were randomly divided into blank group, model 3 d group, model 7 d group and model 21 d group. ELISA was used to test the levels of IL-1β, IL-4, IFN-γ and TGF-β1 in serum and colon tissue.Results Compared with the blank group, contents of IL-1β and IFN-γ in model group increased significantly (P<0.05,P<0.01), and the contents of IL-4 and TGF-β1 content decreased (P<0.05,P<0.01),especially in the model 7 d group (P<0.01).ConclusionThe pro-inflammatory factor IL-1β, IFN-γ and anti-inflammatory factor IL-4 and TGF-β1 play an important role in rats with ulcerative colitis of spleen-kidney yang deficiency type.

ABSTRACT

Objective: To observe the clinical effects of moxibustion at abdominal acupoints for slow transit constipation (STC) due to yang deficiency of the spleen and kidney. Methods: A total of 52 cases with slow transit constipation in conformity with the inclusion criteria were selected and divided into a control group and an observation group according to their visit order and random digital table, 26 cases in each group. Patients in the control group received routine nursing guide. Besides the same routine nursing guide, patients in the observation group received moxibustion at the abdominal acupoints, once every day. The course of the treatment was 4 weeks in the two groups, and the 3-month follow-up was given after the course was finished, for comparing the clinical symptoms, results of colon transit tests, scores of depression/anxiety scale and nursing satisfaction. Results: The total effective rate was 92.3% in the observation group and 69.2% in the control group, with a significant difference between the two groups. After the treatment and during the follow-up checks, the scores of Chinese medical symptoms in the two groups were remarkably decreased than those before the treatment (all P<0.01); the scores of the observation group were obviously lower than those in the control group (all P<0.01). The discharge rates of the markers in the two groups were remarkably increased than those at the same time period before the treatment; moreover, the discharge rates of the markers at various time periods were remarkably better in the observation group than those in the control group (P<0.01). SDS and SAS scores were remarkably decreased after the treatment in the two groups (P<0.01). In comparison of SDS score between the two groups after the treatment, the difference was statistically significant (P<0.01). In comparison of SAS score between the two groups after the treatment, the difference was statistically significant (P<0.05). The nursing satisfaction was 96.2% in the observation group after the treatment, obviously better than that in the control group (73.1%). The recurrence rate was 8.3% in the observation group, remarkably lower than that in the control group (33.3%). Conclusion: Moxibustion at the abdominal acupoints plus routine nursing can remarkably improve the colon transit functions and anxious and depressive emotion in patients with STC, and the therapeutic effects are remarkable. Not only the clinical satisfaction is higher, but the recurrence rate is obviously lower than that of routine nursing.

Objective To study the changes of activities of phase Ⅰ and Ⅱ drug-metabolic enzymes in the spleen of rats with spleen-kidney yang deficiency syndrome.Methods The rat model of spleen-kidney yang deficiency syndrome was established by gastric gavage of Radix et Rhizoma Rhei decoction combined with injection of hydrocortisone for 17 continuous days.And then we detected the activities of 6 kinds of phase Ⅰ drug-metabolic enzymes of CYP2C19,CYP2D6,CYP2C9,CYP1A2,CYP2C8,CYP3A4,and 4 kinds of phase Ⅱ drugmetabolic enzymes of phenol sulfotransferase (PST),uridine diphosphate glucuronosyl transferase 1 (UGT1),glutathione transferase (GST),estrogen sulfotransferase (SULT1E1) in the spleen.Results Compared with the normal control group,the activities of PST,UGT1,GST and SULT1E1 in the model group were significantly decreased (P < 0.05 or P < 0.01),and the activity of CYP1A2 was significantly increased (P < 0.01),while CYP2C19,CYP2D6,CYP2C8,CYP3A4,CYP2C9 enzymes showed no obvious changes(P > 0.05).Conclusion The activities of splenic drug-metabolic enzymes,in particular the phase Ⅱ enzymes,are significantly varied under the state of spleen-kidney yang deficiency.

Objective To evaluate the clinical efficacy of treating functional constipation with spleen-kidney yang deficiency syndrome by TCM hot patching Guanyuan. Methods Totally 61 patients with functional constipation with spleen-kidney yang deficiency syndrome were randomly divided into treatment group (31 patients) and control group (30 patients). Treatment group was given TCM hot patching, heated to 45 ℃, sticking to Guanyuan (RN4), eight hours, one time a day. The control group was treated with polyethylene glycol 4000 powder, 10 g for each time, twice a day, one time for morning and one time for evening. One course lasted for 15 d, 4 courses in total, with six-month follow-up. The improvement of the symptoms, such as difficulty in defecation, interval of defecation, texture of stool, time of defecation, feeling of defecation, weakness of waist and knees, coldness of limbs and so on, were ovserved. The clinical efficacy was evaluated and adverse reaction and recurrence rate were observed. Results After treatment, the symptoms of constipation scores in both groups were significantly improved compared with those before the treatment (P0.05);the incidence of adverse reaction was 6.5%(2/31) in the treatment group, and 16.7%(5/30) in the control group, with statistical significance (P<0.05). In the follow-up of 2, 4 and 6 months, the relapse rates were 0, 3.6%, and 7.1% for the treatment group, and 7.7%, 38.5% and 50.0% for the control group. Relapse rates of 4 and 6 months of treatment group was significantly lower than the control group (P<0.05). Conclusion TCM hot patching Guanyuan has good efficacy for functional constipation with spleen-kidney yang deficiency syndrome, with few adverse reactions and low relapse rate.

ABSTRACT

Objective To observe the therapeutic effect and security of Thermal Moxibustion combined with Yougui Pills in the treatment of premature ovary failure(POF) of spleen and kidney Yang-deficiency.Methods One hundred and forty-two patients with POF of spleen and kidney Yang-deficiency were randomly divided into the treatment group and control group,71 cases in each group.The observation group were treated with Thermal Moxibustion combined with Yougui Pills,while the control group took oral Yougui Pills,12 weeks as a course of treatment.After treatment,the clinical therapeutic effects,K upperman score,related hormones levels(E2,FSH,LH) and occurrence rate of adverse reactions were compared between the two groups.Results The total efficacy rate of the observation group was 95.77%,which of the control group was 77.46%,and the observation group was significantly better than the control group(P<0.05);there were significant improvement in the Kupperman score and levels of related hormones after treatment in the two groups(P<0.05),but the improvement degree of the observation group was superior to the control group(P<0.05).Conclusion Thermal Moxibustion combined with Yougui Pills can alleviate the clinical symptoms with high safety and higher clinical application value

ABSTRACT

Theory of “combination of disease with syndrome” has been widely used in the establishment and evaluation of experimental animal models. The ulcerative colitis animal models with spleen-kidney yang deficiency pattern were established based on the theory according to complex factors. These models were not only required to show the features of “disease” and “syndrome” at the macro characterization, but also combined with microscopic biological indicators and prescription test so as to make comprehensive evaluation. But the problems in the establishment and evaluation of “combination of disease with syndrome” models hindered further development, such as the weak connection between disease and syndrome, multiplicity of models, and limitations in evaluation standards. Therefore, further improvement in the establishment and evaluation of models will make great sense to better ulcerative colitis animal models with spleen-kidney yang deficiency pattern.

ABSTRACT

ObjectiveTo assess the effect of Fuzi-Lizhongdecoction for the diarrhea irritable bowel syndrome (IBS-D) with the Chinese pattern of spleen kidney yang deficiency.MethodsA total of 98 patients diagnosed IBS-D with spleen kidney yang deficiency Chinese patternwere included from April,2014 toMarch, 2015 in our hospital,and were randomly divided into the control group and Traditional Chinese Medicine (TCM) group at ratio of 1:1, 9 paitents in each group, two groups were administrated with the convetional treatment such as correct water, electrolyte disorders. Besides, the control group was added withsulfasalazine treatment, and TCM group withFuzi-Lizhongdecoction. Outcomes includedchanges of symptoms scores, questionnaire scores, TCM syndrome scores, endoscopic scoresand adverse events.Results After treatment, the TCM group showed significantly lower scores of symptoms (1.80 ± 0.19vs. 490 ± 2.24,t=9.653), questionnaire (14.01 ± 7.11vs.31.44 ± 12.91,t=8.278), TCM syndrome (2.33 ± 0.10 vs.4.86 ± 0.93,t=18.934), endoscopic assessment (0.89 ± 0.03 vs.1.60 ± 1.14, t=4.358) than those of the control group (P<0.01). The TCM group showed significantly higher effective rate (93.88%vs.75.51%) than the control groupa (χ2=6.376,P=0.012). The adverse events of TCM group and control group were 0% (0/49) and 12.24% (6/49), and the difference was statistically significant (χ2=6.391,P=0.012).Conclusion Fuzi-Lizhong decoction is safe and effective therapy for IBS-D with Chinese pattern of Yang deficiency of spleen and kidney.

ABSTRACT

Objective To investigate the clinical efficacy of Du meridian moxibustion in treating polycystic ovarian syndrome of spleen-kidney yang deficiency type. Methods Seventy patients with polycystic ovarian syndrome of spleen-kidney yang deficiency type were randomly allocated to treatment and control groups, 35 cases each. The treatment group received Du meridian moxibustion plus medication and stomach and the control group, medication alone. Sex hormones [follicle-stimulating hormone (FSH), luteinizing hormone (IH) and testosterone (T)], fasting insulin (INS), fasting blood glucose (GLU), body mass index (BMI) and changes in menstrual cycle were observed in the two groups before and after three weeks of treatment. The clinical therapeutic effects were compared between the two groups. Results The total efficacy rate was 94.3%in the treatment group and 77.1%in the control group;there was a statistically significant difference between the two groups (P<0.05). There were statistically significant pre-/post-treatment differences in LH, LH/FSH and T in the two groups (P<0.01, P<0.05). There were no statistically significant post-treatment differences in sex hormones between the two groups (P>0.05). There were statistically significant pre-/post-treatment differences in INS, BMI and menstrual cycle in the treatment group (P<0.01, P<0.05). There was a statistically significant pre-/post-treatment difference in menstrual cycle in the control group (P<0.05). There were no statistically significant post-treatment differences in INS, GLU, BMI and menstrual cycle between the two groups (P>0.05). Conclusion Du meridian moxibustion is an effective way to treat polycystic ovarian syndrome of spleen-kidney yang deficiency type. It can improve insulin resistance and insulin sensitivity.

ABSTRACT

Objective To observe the clinical efficacy of Kuijiening Paste acupoint application on ulcerative colitis (UC) patients of spleen-kidney Yang deficiency type, and its effect on serum levels of IFN-γand IL-4. Methods Sixty UC patients were divided into treatment group, control group and combined treatment group, 20 cases in each group, and were given Kuijiening Paste acupoint application plus placebo, oral sulfasalazine plus comfort paste and Kuijiening Paste acupoint application plus sulfasalazine respectively for 60 days. The clinical efficacy, mucosal lesions efficacy, mucosal pathology efficacy, changes of serum IFN-γand IL-4 were observed after treatment. Results There was no significant difference in clinical efficacy, mucosal lesions efficacy and mucosal pathology efficacy among the three groups (P>0.05). After treatment, IFN-γlevels of the three groups significantly reduced, and IL-4 levels significantly increased (P<0.01). The difference between the treatment group and the control group was significant (P<0.05). The difference between the combined treatment group and the control group was significant (P<0.01). Conclusion Kuijiening Paste acupoint application combined with sulfasalazine can regulate the level of immune cytokine in UC patients of spleen-kidney Yang deficiency type, and receive good clinical efficacy.

Objective To explore the essence of polycystic ovary syndrome of spleen-kidney yang deficiency,and to investigate the objective standardization of TCM syndrome diagnosis of the disease.Methods Among the 200 PCOS patients,the Chinese medicine syndrome pattern was differentiated as spleen-kidney yangdeficiency type (n=105 )and Non-spleen-kidney yang deficiency type (n=95).To measure and compare their levels ofBMI、WHR、sexual hormones、FINS、2 hINS,and HOMR-IR.Results The levels of BMI (24.4±2.8)、WHR (0.88±0.07)、LH/FSH (2.02±1.06)、E2 (55.84±23.87) pg/ml、HOMR-IR (2.95±1.52)、FINS (15.18±7.64) IU/ml、2hINS ( 59.53 ±40.61 ) IU/ml in the group of spleen-kidney yang deficiency type were obviously higher than those[ (22.2±2.2)、(0.80±0.06)、(3.50± 1.80)、(45.82±21.31) pg/ml、( 1.90± 1.50)、(9.93±6.59) IU/ml、(50.56±41.34) IU/ml respectively] in the group of Non-spleen-kidney yang deficiency type (P<0.05=,but T and FSH level showed no significant difference between two groups [FSH the group of spleen-kidney yang deficiency type=(6.46±0.95) IU/L、FSH the group of No-spleen-kidney yang deficiency type=(6.40±1.16) IU/L; T the group of spleen-kidney yang deficiency type=(0.81±0.15) μg/L、T the group of No-spleen-kidney yang deficiency type=(0.72±0.13) μg/L,P>0.05].Conclusion BMI、WHR、LH/FSH、E2、FINS、2HINS,and HOMR-IR could be used as objective index in identifying the difference in between spleen-kidney yang deficiency type and Non-spleen-kidney yang deficiency type.
