ABSTRACT

Objective:To explore the effect of epalrestat combined with calcium dobesilate on cardiovascular and cerebrovascular diseases and oxidative stress in diabetes patients with peripheral neuropathy.Methods:In a retrospective analysis, 92 patients with diabetic peripheral neuropathy were admitted to the Department of Endocrinology and Metabolism Hospital of Bozhou People’s Hospital from Jun. 2018 to Jun. 2021, which were divided into observation group and control group according to the different treatment methods, with 46 cases in each group. The control group were treated with epalrestat. On this basis, the observation group were given calcium dobesilate combined treatment. The two groups were compared in terms of the Michigan diabetes neuropathy score (MDNS) Michigan neuropathy screening instrument (MNSI) score, oxidative stress reaction [serum superoxide dismutase (SOD), reduced glutathione (GSH) ], clinical efficacy, complications and adverse reactions Using t-test and chi square test.Result:After treatment, the MDNS and MNSI scores of both groups decreased, and the observation group [ (23.49±3.73), (8.49±1.97) ] were lower than the control group [ (28.49±3.85), (11.53±2.28) ] ( t=8.337, 6.843, P<0.05) ; After treatment, the levels of SOD and GSH in both groups increased compared to those before treatment, and the observation group [ (38.96±4.89), (89.79±6.92) ] were higher than the control group those [ (36.42±4.61), (86.74±6.20) ] ( t=2.563, 2.226, P=0.012, 0.028) ; The clinical efficacy of the observation group was higher than that of the control group ( Z=1.592, P=0.042) ; The incidence of complications in the observation group after 1 year was significantly lower than that in the control group ( χ2=4.389, P=0.036) ; The incidence of adverse reactions in the observation group was slightly lower than that in the control group ( χ2=0.155, P=0.694) . Conclusion:Epalrestat combined with calcium dobesilate is effective in the treatment of diabetes peripheral neuropathy, can effectively reduce complications, and has high treatment safety, which is worthy of clinical application.

ABSTRACT

Objective:To investigate the application value of oral stimulation combined with maternal milk sucking in the pain caused by PICC in children with congenital digestive tract malformations.Methods:Using convenience sampling method, 80 children with congenital digestive tract malformations who were admitted to Hunan Children's Hospital from July 2018 to December 2019 were selected and divided into the experimental group and the control group of 40 cases in each group according to the random number table method. The control group was given routine nursing mode, while the experimental group was given oral stimulation combined with maternal milk sucking intervention on the basis of routine nursing. The intervention effects were evaluated by the Neonatal Infant Pain Scale (NIPS), the onset time of the first cry, the duration of crying, the onset time of the first painful face, the duration of the painful face, and indicators such as heart rate, respiratory rate and SpO 2. Results:The NIPS scores of children in the experimental group were (5.60 ± 1.24) and (4.10 ± 0.31) in the puncture period and the recovery period, which were significantly lower than those in the control group (6.10 ± 0.84), (4.93 ± 0.35), the differences were statistically significant (t = 2.12, 3.00, both P<0.05). The heart rate and respiratory rate of the children in the experimental group were (151.10 ± 12.37), (147.00 ± 12.16) times/min and (47.48 ± 7.59), (45.58 ± 6.51) times/min in the puncture period and the recovery period, which were significantly lower than those in the control group (159.75 ± 13.81), (156.05 ± 12.99) times/min and (52.38 ± 8.12), (49.75 ± 7.59) times/min, the differences were statistically significant (t values were 2.64-3.22, all P<0.05). The SpO 2 in the experimental group were 0.980 5 ± 0.009 0, 0.982 5 ± 0.010 8 in the puncture period and the recovery period, which were significantly higher than those in the control group 0.970 8 ± 0.014 2, 0.971 8 ± 0.018 1, the differences were statistically significant (t = 3.66, 3.23, both P<0.05). The onset time of the first cry, the duration of crying, the onset time of the first painful face, the duration of the painful face in the experimental group were (41.55 ± 6.38), (22.05 ± 4.47), (35.08 ± 5.94), (24.65 ± 5.46) s, significantly shorter than those in the control group (46.58 ± 5.26), (29.50 ± 6.48), (44.68 ± 13.31), (30.65 ± 9.42) s, the differences were statistically significant (t values were 2.63-5.98, all P<0.05). Conclusions:Oral stimulation combined with maternal milk sucking can effectively relieve the pain caused by PICC in children with congenital digestive tract malformation, and reduce the physiological stress response of children.

ABSTRACT

"Timely, near, and expectation" is the main principle of battlefield rescue for military combat stress reaction (CSR). Post-traumatic stress disorder (PTSD) is the most common form of CSR and a long-term persistent mental disorder that is caused by unusual threatening or catastrophic psychological trauma. Chinese medicine (CM) has abundant resources, is simple, easy to master, with few side effects. This article summarizes the cellular and animal experimental mechanisms of CM treatment on PTSD, suggesting that traditional Chinese herbs and acupuncture can protect brain functional areas, and adjust hypothalamus-pituitary-adrenal axis. Traditional Chinese herbs and acupuncture have shown good anti-stress efficacy and fewer side effects in clinical application, which may improve the CSR in the battlefield.

ABSTRACT

Bidirectional regulation is one of the key function of acupuncture. The stimulator, mediator and receptor are the basis while the specificity of acupoints and the multi-target regulation of receptors receiving stimulation signals are the essential link of the bidirectional regulation of acupuncture. The possible mechanisms of bidirectional regulation of acupuncture are discussed in 4 aspects, i.e. homeostasis mechanism, stress reaction, central adaptive regulation and autonomic nerve regulation. Knowing the limitations of bidirectional regulation and exploring suitable researchmethods are proposed to be the key points in future researches.

ABSTRACT

Objective:To explore the effect of Qizhu Zhenwutang on renal interstitial fibrosis in rats ligated with unilateral ureter, transforming growth factor-β1 (TGF-β1)/Smads and oxidative stress. Method:A total of 30 male SD rats were randomly divided into sham operation group, model group, high-dose group, low-dose group and irbesartan group (n=6). The left ureter ligation was performed in the model group and the treatment group. In the sham operation group, the ureter was not ligated, only the ureter was separated, and the abdominal cavity was closed. Rats in each group were given drugs by gavage on the next day after operation. Sham operation group and model group were given aseptic distilled water 10 mL·kg-1 by gavage, high-dose Qizhizhenwu Tang group was given 22.2 g·kg-1 by gavage, low-dose group was given 11.1 g·kg-1 by gavage, and irbesartan group was given 0.02 g·kg-1 by gavage. Rats in each group were sacrificed on the 14th day after operation, 24-hour urine was collected before sampling, and the total amount of 24 hour urine protein (24 h-Upr) was detected. Blood samples were collected from the abdominal aorta to detect serum creatinine(SCr) and blood urea nitrogen (BUN). The tissues were stained with htoxylin eosin (HE) and Masson, and the pathological changes were observed under light microscope, immunohistochemical method was used to detect α-SMA, FN and Col-Ⅰ expressions. Western blot method was used to detect the expressions of TGF-β1, Smad3, p-Smad3 and NOX4. Result:Compared with sham group, SCr, BUN and collagen volume fraction (CVF),24 h-Upr in model group were all increased (P<0.05, P<0.01). The expressions of α-SMA, Col-Ⅰ, FN, TGF-β1, p-Smad3, NOX4 were higher (P<0.05). Compared with the model group, SCr, BUN and CVF were lower in high-dose group and irbesartan group (P<0.05). 24 h-Upr was lower in high-dose group (P<0.05), the expressions of α-SMA, Col-Ⅰ, FN, TGF-β1, Smad3, p-Smad3, NOX4 in traditional Chinese medicine treatment group were less (P<0.05). Conclusion:Qizhi Zhenwutang can reduce the urinary protein of UUO rats, protect the renal function, and inhibit the occurrence and development of renal interstitial fibrosis, the mechanism may be related to the inhibition of TGF-β1/Smads signaling pathway and oxidative stress response.

ABSTRACT

Objective@#To investigate the effects of mindfulness relaxation training on stress response and postoperative rehabilitation of patients undergoing general surgery.@*Methods@#A total of 110 patients who underwent general surgery were randomly divided into study group and control group, with 55 cases in each group. The control group received routine peri-operational period nursing, and the study group carried out mindfulness relaxation training on the basis of control group for 4 weeks. The levels changes of serum insulin, cortisol were detected between two groups. The psychological stress of two groups were evaluated with Symptom Checklist 90 (SCL-90). The off-bed activity time, anus exhaust time, bowel sounds recovery time and hospital stay time were recorded to evaluate the quality of patients′ postoperative rehabilitation.@*Results@#The levels of insulin, cortisol were at minimum on admission in two groups, and then increased, the highest level was at 2 hours after operation. There was no significant difference in insulin, cortisol level on admission in two groups (P>0.05), however, the insulin [(120.15±22.63) μg/L, (122.34±15.75) μg/L], cortisol [(10.41±3.07) mmol/L, (12.69±4.50) mmol/L] levels were significantly decreased in the study group compared to the control group [(129.85±15.43) μg/L, (131.34±22.79) μg/L and (11.69±2.99) mmol/L, (14.84±4.04) mmol/L] before anesthesia and at 2 hours after operation (t value was 2.206-2.638, P<0.05). After intervention, the total scores of SCL-90 (120.91±11.84), somatization (1.26±0.13), compulsion (1.43±0.08), depression (1.18±0.12), anxiety (1.14±0.10) and fear (1.34±0.08) were all lower in the study group than those in the control group (130.47±14.13, 1.83±0.53, 1.65±0.58, 1.71±0.12, 1.76±0.13, 1.41±0.12), and the differences were statistically significant (t value was 2.828-28.277, all P<0.05). Besides, off-bed activity time [(1.55±0.18) d] and hospital stay time[ (6.51±2.20) d] in the study group were significantly less than in the control group [(1.73±0.25) d and (7.65±2.19 d)] (t value was 2.737,4.592, P<0.05).@*Conclusion@#The mindfulness relaxation training in nursing general surgery patients during peri-operational period is helpful to ameliorate the patients′ degree of physiological and psychological stress reactions, and this mode has important significance in promoting the quality of patients' postoperative rehabilitation.

ABSTRACT

AIM: To study the effect of low-dose baclofen on inflammatory factors and oxidative stress in patients with trigeminal neuralgia. METHODS: A total of 112 patients with trigeminal neuralgia treated in our hospital from Jan. 2017 to Jan. 2019 were selected as the subjects; they were divided into test group (n=56) and control group (n=56) according to random number table method. The control group was treated with oxcarbazepine, while the test group was treated with low-dose baclofen on the basis of the control group; the clinical total effective rate, visual analogue scale (VAS), tumor necrosis factor-α (TNF-α), interleukin-6 (IL-6), malondialdehyde (MDA), superoxide dismutase (SOD), glutathione peroxidase (GSH-Px) levels and the incidence of adverse reactions were compared between the two groups. RESULTS:The clinical total effective rate of the test group was significantly higher than that of the control group (P0.05). CONCLUSION:Low-dose baclofen adjuvant therapy has good clinical efficacy for patients with trigeminal neuralgia, which can effectively relieve pain, reduce inflammation and oxidative stress.

Resumen Introducción La reacción de estrés de combate (REC), que hoy en día podría ser denominada «reacción a estrés agudo¼ (ICD-10), se refiere a una reacción de ansiedad transitoria en el soldado, la cual históricamente llevó a una incapacidad para afrontar la lucha de la guerra. La mayoría de los análisis militares, históricos y médicos de la actividad realizada por la Wehrmacht § en la Segunda Guerra Mundial reportan que no se presentaron serios problemas debido al «quiebre¼ por estrés. Las razones se fundamentan en la calidad excepcional y el liderazgo de las pequeñas unidades militares del ejército alemán. Sin embargo, con el paso de los años, ya no fue posible evitar el reporte de un elevado número de bajas secundarias a la REC, lo que ha puesto en duda el mito de la inmunidad del soldado alemán al estrés de la guerra. Conclusiones Al final de la WWII, la reacción de estrés de combate fue un tópico común en las guías oficiales. Los psiquiatras fueron quienes subestimaron de forma continua el papel de este trastorno en la Wehrmacht. La percepción generalizada de que la Wehrmacht no sufría los serios problemas del quiebre emocional en los soldados puede ser modificada hasta cierto punto.


Abstract Introduction Combat stress reaction (CSR), now called «acute stress reaction¼ (ICD-10), refers to a transient anxiety reaction in the soldier, which has historically led to an inability to face the struggle of war. Most military, historical and medical analyzes of the activity carried out by the Wehrmacht in World War II reported no serious problems due to stress. The reasons are based on the exceptional quality of the small military units and the leadership of the German Army. However, over the years, it was no longer possible to avoid reporting the high number of casualties secondary to combat stress reactions, which has cast doubt on the myth of the German soldiers' immunity to the stress of war. Conclusions At the end of WWII, the CSR was a common topic in official reports, meetings and guides. The psychiatrists who had the opportunity to report it were those who continuously underestimated the role of this stress disorder in the Wehrmacht. Thus, the widespread perception that the Wehrmacht did not suffer serious problems of emotional breakdown in soldiers, typical of combat stress, can be modified to some extent.

ABSTRACT

Diaphoretic processing is one of the essential methods aiming to speed up the drying herbs and help the preservation of medicinal materials in the initial processing of Chinese materia medica. In addition, modern research showed that diaphoretic processing also affects the chemical components pharmacological activities and pharmacodynamic action of medicinal materials. During the diaphoretic processing, the internal temperature of medicinal materials rose, which increased the enzyme activity and improved the enzyme-catalyzed reaction, and then these lead to the change of chemical components of medicinal materials. With the diaphoretic processing continues, continually elevated internal temperature of medicinal materials started a series anti-heat stress reaction and generated some secondary metabolites, which considered as main effective components of medicinal materials. In this paper, the mechanism of diaphoretic processing was revealed by analysis the biological enzyme action and anti-heat stress reaction, in order to provide a theoretical basis for the application and extension of the diaphoretic processing.

Objective@#To explore the statistical differences of the vocational exhaustion positive rate among various occupational stress factors, occupational stress reaction, personality characteristics and relieving factors, to lay a foundation for reducing occupational stress of the medical staffs. @*Methods@#656 medical staffs in Yangzhong and Chifeng hospitals were included to conduct the evaluation of the occupational stress and job burnout, analyze the differences among the various factors by comparing them. @*Results@#Various levels of vocational exhaustion positive rate among different occupational stress factors, occupational stress reaction, personality characteristics and relieving factors show statistical difference (P<0.05) . Among occupational stress factors, the vocational exhaustion positive rate of high level groups of conflict in groups was the highest (73.4%) , among occupational stress reaction, the vocational exhaustion positive rate of high level groups of body complain was the highest (80.4%) , among personality characteristics, the vocational exhaustion positive rate of low level groups of self-esteem was the highest (74.4%) , among relieving factors, the vocational exhaustion positive rate of high level groups of family support was the highest (76.4%) . @*Conclusion@#The vocational exhaustion rate of medical staffs show very high in the profession, comprehensive factors should be considered to create harmonious working atmosphere and to reduce vocational exhaustion positive rate of the medical staffs, to reduce the positive rate of vocational exhanstion in medical staffs.

Objective@#To explore the relationship between occupational stress factors and psychological stress reaction among petrochemical workers, detecting the effects caused by the occupational stress and putting forward solutions to reduce the occupational stress degree. @*Methods@#392 petrochemical workers in Oil refining enterprise were included to conduct the evaluation of the occupational stress, correlation analysis and ridge regression were used to analyze the relationship between occupational stress and five aspects of psychological stress reaction and to detect the influencing factors. @*Results@#Mission control, decision control, environmental control, resources control, promotion, and participation in decision making were positively correlated with job satisfaction (P<0.01) , technology utilization degree, work order tonality, job prospects were negatively correlated with job satisfaction (P<0.05) ; mission control, resource control, promotion, participation in decision-making were positively correlated with mental health (P<0.01) , work order tonality, job prospects were negatively correlated with mental health (P<0.05) ; load change risk, work, work order tonality, job prospects were positively correlated with depression (P<0.05) resource control, promotion opportunities were negatively correlated with depression (P<0.05) ; quantitative risk load, work, work order tonality, job prospects were positively correlated with anxiety (P<0.05) , the task control, promotion, and participation in decision-making were negatively correlated with anxiety (P<0.05) ; work, load change, work order tonality, risk scoring for job prospects were positively correlated with body complaint (P<0.01) , mission control, decision control, resource control were negatively correlated with body complaint (P<0.05) . @*Conclusion@#Different stress factors make different effects in different psychological stress reaction, the main predictors and predictive power of each psychological stress reaction were not the same.

ABSTRACT

Objective To explorepredictive factors of psychological stress reaction of medical staff members with blood-occupational exposure and to provides scientific basis for the hospital administrators toprovide psychological support and interventionfor medical staff members with blood-occupational exposure. Methods Seventy-eight medical staff members with blood-occupational exposure were recruited as participants. The Socio-demographic Questionnaire, Posttraumatic Stress Disorder Checklist-5, the Perceived Stress Scale, the Hospital Anxiety and Depression Scale, the Ruminative Responses Scale and the Emotion Regulation Questionnaire were used to evaluate the participants' psychological stress reactionat different stages and the styles of their emotion regulation. Correlation analysis and Hierarchical Regression analysis were used to analyze the predictive factors of psychological stress reaction of medical staff members with blood-occupational exposure. Results Ruminative thinking and emotion regulation can predict the psychological stress reaction of medical staff members with blood-occupational exposure at different stages. Conclusions According to the psychological stress reaction and ruminative thinking of medical staff members with blood- occupational exposure, psychological intervention should be implemented within one month to decrease the psychological trauma of medical staff members with blood-occupational exposure.

ABSTRACT

OBJECTIVE: To analyze the occupational stress status and its influencing factors in workers in labor intensive enterprises.METHODS: Two thousand eight hundred and ninety-four workers from 6 labor intensive enterprises were selected by cluster sampling method.The Occupational Stress Inventory-revised Questionnaire was used to evaluate their occupational stress.The influencing factors of occupational stress reaction were analyzed.RESULTS: Among the 2 894 workers,there were 76.1%,72.6% and 57.8% of workers at moderate levels on occupational stress,stress reaction and lack of coping resources,respectively.There were 17.0%,22.4% and 20.8% of workers at medium or above levels on occupational stress,stress reaction and lack of coping resources,respectively.The results of multiple linear regression analysis showed that the primary influencing factors of occupational stress reaction were role overload,role ambiguity,task conflict,sense of responsibility,working environment,recreation,rational handling,daily working hours,weekly working days and occupational hazards exposure(P<0.05).CONCLUSION: Most of the workers in labor intensive enterprises are at a moderate level of occupational stress.Some workers are at high occupational stress level.The occupational stress level,coping resources,working time and occupational hazards exposure can affect of the occupation stress response in these workers.

Objective To investigate the relationships among resilience, perceived social support and acute stress reaction of naval soldiers. Methods Connor-Davidson resilience scale (CD RISC), perceived social support scale (PSSS) and the military acute stress scale (MASS) were used to investigate 1 348 naval soldiers. The relationships among the involved variables were analyzed by structural equation model. Results Perceived social support was positively correlated with resilience in the participants (P<0. 01). and both resilience and perceived social support were negatively correlated with the military acute stress reaction (P<0. 01); both of them could independently explain the variance of acute stress reaction of navy soldiers, in which strength (β = -0. 136, P<0. 01) and external family support (β = -0. 229. P< 0. 01) had significant negative predictive effects on military acute stress reaction. The mediating effect of resilience was significant between military acute stress reaction and perceived social support, accounting 16. 22% of the total effect. The model fitting was good: χ2/df = 27. 91/ 11 = 2. 537, P = 0.003; root mean square error of approximation (RMSEA) = 0. 032, comparative fit index (CFI) =0. 997 and goodness-of-fit index (GFI) = 0. 994. adjusted goodness of fit index (AGFI) =0.986, normed fit index (NFI) =0. 995, relative fit index (RFI) = 0. 986, incremental fitness index (IFI) = 0. 997 and Tucker-Lewis Index (TLI) = 0. 992. Conclusion Perceived social support has direct effect on acute stress reaction of navy soldiers, and it also has indirect influence on the acute stress reaction through the mediating effect of resilience.

ABSTRACT

Objective To investigate the effects and mechanism of dexmedetomidine hydrochloride preconditioning against glutamate-induced neurotoxicity.Methods The model of glutamate-induced neurotoxicity was established by the injection of glutamate into lateral cerebral ventricle.Thirty-six SD rats were randomly divided into control group (C group),glutamate-induced neurotoxicity group (G group),Dex1 group and Dex2 group.Dex1 group and Dex2 group received intraperitoneal injection of dexmedetomidine respectively at a dose of 50 μg/kg or 100 μg/kg before glutamate application.Two hours later,the rats were sacrificed and hippocampus was separated to measure the level of SOD and MDA.The rest of each brain was used to measure the degree of brain edema.Pathological changes were observed under microscope with Nissl's staining.Results In contrast to G group,brain edema and MDA concentration in Dex1 group and Dex2 group were significant lower,while SOD concentrations were significantly increased and the pathological change in Dex1 group and Dex2 group were relieved obviously compared to glutamate-induced neurotoxicity group.Conclusion Dexmedetomidine preconditioning can significantly attenuate glutamate-induced neurotoxicity,which is properly related to the inhibition of oxidative-stress reaction.

ABSTRACT

OBJECTIVE To investigate the cytotoxicity of cyflumetofen for SH-SY5Y cells and the mechanism. METHODS SH-SY5Y cells treated with cyflumetofen 0.03, 0.06, 0.125, 0.25, 0.5, 1, 2, 2.6, 4, 6, 8 and 16 mmol·L-1 for 48 h. Cell survival was measured with MTT assay. The reactive oxygen species (ROS) was determined with the DCFH- DA probe, and mitochondrial membrane potential (MMP) was detected by JC-1 staining. The morphological changes in cell nuclei were observed with Hoechst33258 staining. Cell cycle and apoptosis were determined by flow cytometry. The protein levels of phosphorylated Jun Kinase (p-JNK) and p-P38 were measured by Western blotting. RESULTS Compared with solvent (DMSO) control group, cyflumetofen (≥0.06 mmol · L- 1) inhibited the proliferation of SH- SY5Y cells obviously (P<0.05), and the IC50 was 2.6 mmol·L-1. MMP declined and ROS levels increased significantly in cyflumetofen 1, 2, 4 and 6 mmol·L- 1 groups (P<0.01). Cyflumetofen 2, 4 and 6 mmol·L- 1 induced nucleic accumulation, nuclear shrinkage and disintegration in SH-SY5Y cells. Apoptosis rates of cyflu? metofen 2, 4 and 6 mmol·L- 1 groups increased from (0.7±0.1)% in DMSO control group to (6.7±0.1)%, (72.4±8.6)% and (90.7±3.2)% (P<0.01). Cyflumetofen 4 and 6 mmol·L- 1 induced G1 phase cell cycle arrest (P<0.01). In addition, Western blotting showed that cyflumetofen 4 and 6 mmol·L-1 up-regulated the expression of p-JNK (P<0.01), while the level of p-P38 in SH-SY5Y cells was increased in cyflumetofen 6 mmol · L- 1 group (P<0.01). CONCLUSION Cyflumetofen induces cell damage, apoptosis and G1 phase cell cycle arrest in SH- SY5Y cells. The mechanism may be associated with oxidative damage, and activation of P38 and JNK stress-response pathways.

ABSTRACT

Objective To observe the effects of dexmedetomidine on the perioperative stress in patients undergoing uvulopalatopharyngoplasty (UPPP).Methods Sixty patients scheduled for uvulopalatopharyngoplasty (UPPP), all males, aged 18-65 years, of ASA physical status Ⅰ or Ⅱ, were selected randomly.Then the patients were divided into dexmedetomidine group (group D) and control group (group C), 30 patients in each group.The patients of group D were administered 1.0 μg/kg bolus dose of dexmedetomidine over 10 min before tracheal intubation, followed by 0.5 μg·kg-1·h-1 infusion for 12 hours in ICU after surgery.And the patients in group C were given normal saline instead as in group D.The levels of MAP, HR, PaO2, PaCO2, norepinephrine (NE), epinephrine (E), cortisol (Cor), interleukin-6 (IL-6) and tumor necrosis factor-α (TNF-α) were recorded at time points of entering operating room (T0), tracheal intubation (T1), 30 min after operation (T2), upon entering ICU (T3), and extubation (T4).Results HR of group D was significantly slower than that of group C during T1-T4 (P<0.05).MAP of group D was lower than that of group C during T1-T4 (P<0.05).The levels of NE, E, Cor, IL-6, TNF-α of group D were significantly lower than that of group C during T1-T4(P<0.05).Compared with group C , the incidence of cough reflex during extubation was lower in group D significantly (P<0.05).Conclusion Intravenous infusion of dexmedetomidine can suppress perioperative stress effectively and stabilize hemodynamics for the patients undergoing UPPP without prolonging the recovering time, extubation time and the duration in ICU.

ABSTRACT

Objective To investigate the pre-emptive analgesia effects of hydromorphone on stress reaction in patients undergoing gynecological laparoscopic surgery.Methods Forty ASA Ⅰ or Ⅱ patients [age:45-58 years,body mass index:18-24 kg· (m2)-1] undergoing gynecological laparoscopic surgery were randomly divided into 2 groups (n =20 for each group):treatment group and control group.Hydromorphone (1 mg) was intravenously injected before anesthesia in treatment group.In the two groups,after routine induction and incubation,remifentanil (0.2 μg· min-1 · kg-1) and propofol (0.1 mg· min-1· kg-1) were injected with micro perfusion pump,cisatracurium was injected intermittently.Injection of remifentanil and propofol was stopped when skin suture started.The concentrations of epinephrine(E) and norepinephrine(NE) were obtained before induction (t1),pneumoperitoneum (t2),1 h after pneumoperitoneum (t3) and extubation (t4),respectively.The heart rate,blood pressure and the time of operation to extubation of the patients were recorded.Results There were no significant changes in extubation time after operation among the groups.Heart rate and systolic blood pressure were significantly lower in the treatment group than in the control group (P < 0.05).Plasma E and NE levels at t1 were significantly lower than those at t2,t3 and t4.Plasma E and NE were significantly lower in treatment group than that of control group at t2,t3 and t4 (P < 0.05).Conclusion Pre-emptive analgesia of hydromorphone can significantly decrease the release of epinephrine and norepinephrine during and after laparoscopic surgery,restrain the increase of heart rate and systolic blood pressure during extubation,without influence of revival time.

ABSTRACT

Objective To explore the effect of psychological intervention combined with drug multimodal therapy on orthopedic analgesia and its effect on stress response, and to provide reference for clinical application.Methods 88 cases treated with open reduction and internal fixation for lower limb fracture as the object of study were collected from March 2015 to March 2017 in Baoji traditional Chinese medicine hospital, which were received multi-mode drug treatment.According to whether to receive psychological intervention, 88cases were divided into the intervention group(45 cases) and the control group(43 cases).Postoperative pain was assessed by visual analogue scale(VAS) and simplified McGill questionnaire(SF-MPQ), the changes of plasma CRP and IL-6 levels were compared before and after operation.Results There was no significant difference in VAS score, SF-MPQ score, preoperative plasma CRP and IL-6 levels between the two groups.One and 24 hour after operation, VAS score and SF-MPQ score were higher than those at admission(P<0.05).The VAS score and SF-MPQ score in the intervention group were lower than those in the control group at different time points(P<0.05).One and 24 hour after operation, plasma CRP and IL-6 levels in the two groups were significantly higher than those before operation(P<0.05), the levels of plasma CRP and IL-6 in the intervention group were significantly lower than those in the control group(P<0.05).Conclusion Psychological intervention combined with multimodal drug therapy can effectively reduce the degree of postoperative pain and stress reaction, and is conducive to postoperative rehabilitation.

ABSTRACT

·AIM: To investigate the anti - aging effect and its potential mechanisms of simvastatin on retinas of physiological aging rats.·METHODS:Totally 40 three-month old healthy SD rats which had no eye diseases, were randomly assigned into two groups: simvastatin group ( n = 20 ) and control group ( n=20 ) . All rats were cultivated under the same conditions until they were nine - month old when interventions started to be given. Simvastatin group was given intragastric administration of 5mg/kg simvastatin every day until 17-month old. Control group was given intragastric administration of same amount of saline gavage. Retinal thickness was measured by HE staining, while Cu - Zn - SOD, NOX2, Bcl - 2 and Bax were determined by immunohistochemistry ( IHC) .·RESULTS:HE staining showed that the retinal structure was clearer; the morphology of cell was more homogeneous;the number of cells was more stable and the structure of retinal pigment epithelium was more compact when compared with control group. Thickness of retinal neuroepithelium layer and retinal pigment epithelium increased significantly in the simvastatin group. Immunohistochemistry analysis showed that the expressions of Cu - Zn - SOD and Bax statistically increased while the NOX2, Bcl-2 as well as Bcl-2/Bax decreased in simvastatin group when compared with control group (P<0. 05).·CONCLUSION: Simvastatin plays a protective role in retinal aging by decreasing oxidative stress reaction and promoting cell apoptosis.
