ABSTRACT

AIM:To compare the curative effect of different surgical methods combined with Toric intraocular lens(IOL)implantation on age-related hard nuclear cataract.METHODS:According to retrospective study, 104 patients(104 eyes)with age-related hard nuclear cataract confirmed in the hospital between January 2020 and December 2021 were enrolled. They were divided into phacoemulsification group(52 eyes, phacoemulsification combined with Toric IOL implantation)and small-incision group(52 eyes, small-incision split nuclear technique in horizontal space combined with Toric IOL implantation)according to different surgical methods. The best corrected distance visual acuity(BCDVA), corneal astigmatism, number of corneal endothelial cells, proportion of normal hexagonal cells, tear film function and complications were compared between the two groups.RESULTS:There was no significant difference in BCDVA(LogMAR)between the two groups before and at 3mo after surgery(all P>0.05), while BCDVA(LogMAR)was better in small-incision group than phacoemulsification group at 1wk after surgery(0.15±0.04 vs. 0.20±0.05, P<0.001). The corneal astigmatism of the patients in both groups was lower at 1wk and 3mo after surgery than that before surgery, and it was lower at 3mo than 1wk after surgery(all P<0.05), while there was no significant difference in corneal astigmatism between the two groups before and after surgery(all P>0.05). At 1wk and 3mo after surgery, number of corneal endothelial cells in small-incision group was more than that in phacoemulsification group(1wk after surgery: 2363.8±315.3 vs. 2231.4±326.4 cells/mm2, P<0.05; 3mo after surgery: 2414.6±245.7 vs. 2322.9±221.0 cells/mm2, P<0.05). Before and at 1wk after surgery, there was no significant difference in the proportion of normal hexagonal cells between the two groups(all P>0.05). At 3mo after surgery, proportion of normal hexagonal cells in small-incision group was higher than that in phacoemulsification group(21.77%±1.91% vs. 20.59%±1.65%, P<0.001). Before and at 3mo after surgery, there was no difference in break up time(BUT)or ocular surface disease index(OSDI)score between the two groups(P>0.05). At 1wk after surgery, BUT in small-incision group was longer than that in phacoemulsification group(6.8±0.8 vs. 5.9±1.0s, P<0.001)and OSDI score was lower than that in phacoemulsification group(17.62±5.47 vs. 20.34±6.18 points, P<0.05). The incidence of postoperative complications in small-incision group was lower than that in phacoemulsification group(3.9% vs. 17.3%, P<0.05).CONCLUSION: Small-incision split nuclear technique in horizontal space combined with Toric IOL implantation can significantly improve visual acuity and astigmatism in patients with age-related cataract, with slight damage to corneal endothelium and tear film function.

ABSTRACT

AIM: To investigate the changes of morphology and function of meibomian glands in patients with type 2 diabetes mellitus and its influence on the tear film. METHODS: A total of 52 patients(104 eyes)with type 2 diabetes mellitus who came to our hospital from January 2018 to January 2020 were selected. Then they were divided into non-diabetic retinopathy group(NDR group, 31 cases with 62 eyes)and diabetic retinopathy group(DR group, 21 cases with 42 eyes)according to the fundus changes. While 38 cases(76 eyes)of diabetic-free cataract patients who treated at the same time were selected as the control group. The differences of three groups were compared with the morphology and the scores of the function of lid edge and meibomian glands, the scores of fluorescence staining of cornea, break-up time(BUT)of tear film, lipid layer thickness(LLT), blink times(BT)and partial blink rate(PBR).RESULTS: The morphology and the scores of function of lid edge and meibomian glands, the scores of fluorescence staining of cornea were significantly higher than the control group, and the DR group was significantly higher than the NDR group(all P<0.05). The BUT in the DR group and NDR group was significantly lower than that in the control group, and the DR group was significantly lower than that in the NDR group(all P<0.05). There were differences in LLT, BT and PBR among the three groups(P<0.05). The LLT and BT in the DR group and NDR group were significantly lower than those in the control group, and PBR was significantly higher than that in control group(all P<0.05), but there was no significant difference between the DR group and the NDR group(all P>0.05). Type 2 diabetes mellitus patients with morphology abnormalities of meibomian gland have a higher incidence of abnormal tear film function.CONCLUSION: Patients with type 2 diabetes mellitus are prone to shortening and loss of meibomian glands, which is easy to cause the dysfunction of the meibomian gland and decrease the stability of the tear film. While the patients with DR, the morphology abnormalities and dysfunction of the meibomian glands are more pronounced, and the stability of the tear film is worse.

ABSTRACT

@#AIM: To analyze the influence of limbal conjunctival autograft and amniotic membrane transplantation on vision, corneal astigmatism(CAD)and tear film function in patients undergo pterygium surgery.<p>METHODS: Totally 96 patients(96 eyes)who had undergone pterygium surgery in the Department of Ophthalmology at Pengzhou People's Hospital were enrolled in this retrospective study between August 2018 and March 2020. According to random number table method, the patients were divided into observation group(48 patients with 48 eyes treated with pterygium excision combined with limbal conjunctival autograft)and control group(48 patients with 48 eyes treated with pterygium excision combined with amniotic membrane transplantation). General surgical indicators, uncorrected visual acuity(UCVA), tear film function \〖break-up time(BUT), SⅠt\〗, CAD, ocular surface disease index(OSDI), recurrence rate and incidence of complications were compared between the two groups.<p>RESULTS: The surgical time of observation group was longer than that of the control group, and the corneal epithelial healing time was shorter than that of the control group(<i>P</i><0.001). There was no significant difference in hospital stay or conjunctival hyperemia and edema remission time between the two groups(<i>P</i> >0.05). The UCVA of the two groups increased at 1, 3 and 6mo after surgery(<i>P</i><0.05), without statistically significant difference between groups(<i>P</i> >0.05). Compared with pre-operation, the BUT and SⅠt of both groups also increased, while CAD and OSDI scores decreased at 1, 3 and 6mo after surgery(<i>P</i><0.05). Besides, the BUT and SⅠt of observation group at 1mo after surgery were shorter/lower than control group, which were longer/higher than control group at 3 and 6mo after surgery(<i>P</i><0.05). However, there was no significant difference in CAD between the two groups(<i>P</i> >0.05). OSDI scores of the observation group were lower than those of the control group at 1, 3 and 6mo after surgery(<i>P</i><0.05). The recurrence rate of observation group within 6mo after surgery was lower than that of control group(<i>P</i><0.05), but the difference in incidence of complications was not significant(<i>P</i>>0.05).<p>CONCLUSION: Compared with amniotic membrane transplantation, limbal conjunctival autograft combined with pterygium excision applied in treatment of patients with pterygium can significantly shorten the corneal epithelial healing time, and reduce the influence on tear film function and recurrence rate, without affecting vision and corneal astigmatism. It is also safe and reliable.

They were divided into two groups according to the random number table method. 62 cases(64 eyes)in the conjunctival transplantation group were treated with pterygium excision combined with free conjunctival flap transplantation. 58 cases(62 eyes)in the amniotic membrane transplantation group were treated with pterygium excision combined with amniotic membrane transplantation. The length of Schirmer's-Ⅰ test(SⅠt), tear film break-up time(BUT), corneal fluorescein staining(FL)scores, noninvasive Keratograph tear breakup time(NIBUT), determine tear river height(TMH)and conjunctival goblet cell count were determined before surgerry, at 3mo and 6mo after surgery. Patients' quality of life was evaluated with ocular surface disease index(OSDI), and the therapeutic effect and recurrence were evaluated at 6mo after surgery.<p>RESULTS: There was no significant difference in the average repair time of corneal epithelium or the duration of symptoms between the conjunctival transplantation group and the amniotic membrane transplantation group(<i>P</i>>0.05). At 6mo after surgery, the BUT \〖(11.11±2.77)s <i>vs</i>(10.01±2.41)s\〗, NIBUT \〖(10.01±1.52)s <i>vs </i>(9.52±0.98)s\〗 in conjunctival transplantation group was longer than that in the amniotic membrane transplantation group(<i>P</i><0.05), and the FL score was lower than the amniotic membrane transplantation group \〖(0.44±0.10)points <i>vs</i>(0.50±0.11)points\〗(<i>P</i><0.05). The conjunctival goblet cell counts in both groups were increased significantly after surgery(<i>P</i><0.05). At 3mo and 6mo after surgery, the counts in conjunctival transplantation group were larger than those in the amniotic membrane transplantation group \〖(311.41±58.45)/mm2 <i>vs</i>(285.46±68.96)/mm2,(342.41±66.89)/mm2 <i>vs</i>(314.41±70.12)/mm2\〗(<i>P</i><0.05). The OSDI scores of both groups were significantly decreased after surgery(<i>P</i><0.05), but there was no significant difference between groups(<i>P</i>>0.05). There was no statistically significant difference in the overall curative effect between the two groups(<i>P</i>>0.05). The recurrence rates in the conjunctival transplantation group and the amniotic membrane transplantation group were 6.3% and 6.5%, respectively(<i>P</i>>0.05).<p>CONCLUSION: Pterygium resection combined with autologous conjunctival flap transplantation is equivalent to amniotic membrane transplantation in terms of postoperative recovery and prevention of pterygium recurrence. Both can improve the stability of patients' tear film function.

Objective To compare the tear related indexes of patients with type-2 diabetes and the healthy subjects, analyze the functions of tear film of these two groups and provide significant theoretical evidences for the causes of the increased prevalence of dry eye in patients with type-2 diabetes and their pathogenesis. Methods A case-control study was designed. During the epidemiological investigation of dry eye in communities,79 patients with diabetes and 74 normal healthy subjects were randomly selected as participants in this case-contol study. Dry eye tests including tear film break-up time (BUT),SchirmerⅠtest (SⅠt) and corneal fluorescein staining were performed. The tears were collected to detect mucoprotein-5AC (MUC-5AC),matrix metalloproteinase 9 (MMP-9),receptor of advanced glycation endproducts ( RAGEs ) and the content of insulin. The diagnosis standard referred to Experts Consensus on Clinical Diagnosis and Treatment of Dry Eye published in 2013. The study was followed the Declaration of Helsinki. This study protocol was approved by Medical Ethics Committee of the Shanghai General Hospital, Shanghai Jiaotong University(No. 2016KY005),and written informed consent was obtained from all subjects. Results There were significant differences between 79 diabetes patients and 74 normal healthy subjects in the value of BUT,SⅠt and corneal fluorescein staining (all at P<0. 05). The value of BUT and SⅠt in patients with diabetes were lower than those in the control group, while the corneal fluorescein staining scores of patients were higher than those of normal healthy subjects, with significant differences ( P<0. 05 ) . In addition, compared with normal healthy subjects, the diabetic patients were higher in the value of MMP-9 and RAGEs,but lower in the content of insulin and MUC-5AC,with significant differences (P<0. 05). There were significant differences in gender, diabetes duration, MMP-9,and MUC-5AC between the dry and non-dry eyes in the diabetic group (P<0. 05). Women were more likely to develop dry eyes with a significant difference (t=4. 35,P=0. 04). The duration of diabetes in dry eye group was higher than that in non-dry eye group, with a significant difference (t=2. 56,P=0. 01);the value of MMP-9 in dry eye group was (1052. 37±157. 68)pg/ml,which was significantly higher than that in non-dry eye group ([459. 11± 258. 67]pg/ml)(t=11. 92,P<0. 01);the value of MUC-5AC in dry eye group was (867. 83±121. 82)pg/ml,which was significantly lower than in non-dry eye group ([972. 93±153. 52] pg/ml)(t=-3. 30,P<0. 01). There was no statistical difference in age and insulin between the two groups. Pearson correlation analysis showed that MMP-9 was negatively correlated with BUT (r=-0. 349,P<0. 01),and positively correlated with corneal fluorescein staining (r=0. 181,P=0. 026). MUC-5AC was positively correlated with SⅠt (r=0. 367,P<0. 01). Conclusions Dry eye prevalence is significantly related with the decrease in MUC-5AC and the increase in MMP-9 in community-based diabetic patients.

ABSTRACT

·AIM: To explore the effect of different incision phacoemulsification on corneal endothelial function and tear film function in senile cataract (SC) patients. ·METHODS: Totally 90 SC patients (90 eyes) admitted to our hospital were randomly divided into the 2. 2mm group ( n = 47, 47 eyes, 2. 2mm corneal micro - incision phacoemulsification) and the 3. 0mm group ( n= 43, 43 eyes, 3. 0 mm clear corneal micro - incision phacoemulsification). The indexes of corneal endothelial function and tear film function were compared between the two groups before and after operation. ·RESULTS: There was no statistical difference on the average phacoemulsification time (APT), mean effective phacoemulsification time( EPT ),average phacoemulsification energy ( AVE) and preoperative and postoperative intraocular pressure between the two groups (P>0. 05). There were statistical differences on the tear break-up time (BUT), basal tear secretion test (ST-Ⅱ), corneal sodium fluorescein staining (SCSF) score, corneal endothelial cell count, central corneal thickness and the variation coefficient of corneal endothelial cells between the two groups (P<0. 05). At 1wk and 1mo after operation, the BUT in the two groups was significantly lower than that before operation (P<0. 05) while the ST-Ⅱ was significantly higher than that before operation ( P< 0. 05), and the SCSF first increased and then decreased (P < 0. 05 ), and corneal endothelial cell count was significantly lower than that before operation (P<0. 05), and the central corneal thickness and the variation coefficient of corneal endothelial cells significantly increased (P<0. 05). The changed degrees of BUT, ST-Ⅱ, SCSF score, corneal endothelial cell count, central corneal thickness and corneal endothelial cell variation coefficient in the 2. 2mm group were less than those in the 3. 0mm group (P<0. 05). ·CONCLUSION: Phacoemulsification for SC patients can cause some damage in corneal endothelial function and tear film function, and 2. 2mm corneal micro-incision have less damage and faster early recovery.

ABSTRACT

@#AIM: To explore the correlation between eccentricity in the optical treatment area and tear film function and corneal morphology after orthokeratology. <p>METHODS:Eighty patients(80 eyes)with myopia who were treated with orthokeratology were selected as subjects. The eccentric distance, tear film function and corneal morphology of the patient's optical treatment area were detected at different time points before and after wearing the lens. According to the median eccentric distance, the patients were divided into low eccentricity group and high eccentricity group. The differences of tear film function and corneal morphology at different time points before and after wearing were compared between the two groups. Pearson correlation coefficient was used to analyze the correlation between total eccentricity and tear film function and corneal morphological parameters at 12mo after wearing glasses. <p>RESULTS:Compared with before wearing lenses, the corneal surface asymmetry index and corneal surface regularity index of the two groups were significantly increased at 1wk, 3, 6 and 12mo after wearing lenses(<i>P</i><0.05). Patients in the high eccentric group were more significantly elevated than in the lower eccentric group, and the differences were statistically significant(<i>P</i><0.05). The time of tear film rupture, corneal curvature, basic tear secretion, and central corneal thickness were significantly lower, and the differences were statistically significant(<i>P</i><0.05). Compared with the patients with lower eccentricity, the patients with high eccentricity group had more significant reduction in tear film rupture time and basic tear secretion, and the difference was statistically significant(<i>P</i><0.05), however, the degree of corneal curvature and central corneal thickness decreased less(<i>P</i><0.05). There was no significant difference in corneal endothelial cell density between 1wk after wearing lenses and 3mo after wearing lenses(<i>P</i>>0.05). The difference was statistically significant at 6mo and 12mo after wearing lenses(<i>P</i><0.05). At 12mo after wearing the lens, the total eccentric distance was positively correlated with the asymmetry index of the corneal surface and the regularity index of the corneal surface(<i>r</i>=0.869, 0.815; <i>P</i><0.05). At 12mo after wearing the lens, the total eccentric distance was negatively correlated with tear film rupture time, corneal curvature, basic tear secretion, and central corneal thickness(<i>r</i>=-0.865, -0.745, -0.912, -0.713; all <i>P</i><0.05), and there was no significant correlation with corneal endothelial cell density(<i>r</i>=0.185, <i>P</i>=0.199). <p>CONCLUSION:The eccentricity of the optical treatment area occurs at the beginning of wearing lenses. And the total eccentric distance has a significant correlation with the changes of tear film function and corneal morphology after keratoplasty.

ABSTRACT

AIM: To evaluate the effects induced by topical antiglaucomatous drugs, Travoprost eyedrops on tear film. METHODS: Eighteen patients ( 32 eyes ) with primary open-angle glaucoma or ocular hypertension were all treated with Travoprost eyedrops once every night. The symptom score, Schirmer's test ( S Ⅰ t ) , corneal fluorescein staining ( FL ) , tear film break - up time (BUT), were observed before the treatment and 1, 2 and 3mo after the treatment. RESULTS: The average symptom score, FL of all patients were 1. 34 ± 1. 56 and 0. 44 ± 0. 73 before the treatment, and 2. 75±1. 63, 1. 08±0. 84; 5. 10±1. 68, 1. 53±0-67;6. 33±1. 40, 1. 98±0. 50 respectively after 1, 2 and 3mo of the treatment. There was significant increase in symptom score and FL after the treatment for 1, 2 and 3mo (P=0. 00). The average BUT, SⅠt of all patients were (7. 76±0. 92s), (8. 47±2. 73mm/5min) before the treatment, and (7. 08±1. 15s), (7. 73±3. 44mm/5min);(5-59±1. 33s), (6. 82±3. 05mm/5min); (4. 29±1. 87s), (6-04±3. 15mm/5min) respectively after 1, 2 and 3mo of the treatment. There was significant decrease in BUT and ST after the treatment for 1, 2 and 3mo (P=0. 00). CONCLUSION: Travoprost eyedrops can obviously aggravate patients’ corneal irritation after treatment. Our results show abnormal decreased tear secretion and stability of tear film induced by Travoprost eyedrops over the short term.

ABSTRACT

PURPOSE: In this study we evaluated the changes in tear film after primary pterygium operation in patients with pterygium. METHODS: We investigated 43 eyes of 42 subjects who showed successful results 3 months after pterygium operation performed by one surgeon. The changes in tear film thickness, tear break-up time (BUT), Schirmer I test, and ocular surface disease index (OSDI) were evaluated. All values were compared before and after surgery. RESULTS: The mean age was 58.0 +/- 11.1 years (34-81 years). Preoperative tear film thickness, tear BUT, and Schirmer I test in eyes which underwent pterygium operation were 21.53 +/- 5.93 microm, 4.84 +/- 2.21 seconds, and 11.67 +/- 6.75 mm, respectively. Three months after the operation, the respective values were 24.23 +/- 4.19 microm (p < 0.05), 5.81 +/- 1.89 seconds (p < 0.05), and 13.02 +/- 7.54 mm (p = 0.094). Tear film thickness and BUT score increased significantly after pterygium operation. There was no statistically significant difference in Schirmer I test, before and 3 months after pterygium operation. The subjective parameter (OSDI) improved 3 months after pterygium operation (p = 0.015). CONCLUSIONS: Pterygium operation can partially restore the tear film function into a normal state by improving tear film thickness and tear BUT after pterygium operation.

ABSTRACT

Objective To investigate the tear fdm function with Graves disease. Methods Seven-ty-eight patients with Graves disease were studied respectiveIy,40 cases healthy person as contr01.Tear film function Was evaluated by tear break-up time(TBUT),Schirmer I test(SIT),corneal fluorescein staining, dry eye and unwell symptoms. Results The difference of TBUT and SIT resultsbetweenpatients and con-trois were significant(P<0.05).The rate ofTBUT,corneal fluorescein staining and symptom ofdry eye in patients were significantly higher than those in controls. Conclusions Graves disease patients have higher rate of abnormal tear film function in comparison with controls.Graves disease are correlated with xerophthalmia.
