ABSTRACT

Introducción: La artritis reumatoide es una enfermedad sistémica crónica inflamatoria que se caracteriza por una reacción autoinmune y puede producir manifestaciones bucofaciales. Objetivo: Describir las manifestaciones maxilofaciales de la artritis reumatoide. Métodos: Se realizó un estudio observacional, descriptivo, de serie de casos, de 25 pacientes con artritis reumatoide, seleccionados por el método aleatorio simple, ingresados en el Servicio de Reumatología del Hospital Provincial Clinicoquirúrgico Docente Saturnino Lora Torres de Santiago de Cuba, de septiembre de 2016 a febrero de 2017, a los cuales se les efectuó un examen físico detallado intra- y extrabucal. Resultados: La localización intrabucal donde existieron más manifestaciones fue el periodonto de protección (84,0 %), seguido del periodonto de inserción (40,0 %). En cuanto a las manifestaciones extrabucales, fueron escasas, con un leve predominio de afectación en la región parotídea (32,0 %). La mayoría de la serie presentó alteraciones de la articulación temporomandibular (80,0 %) y periodontopatías (84,0 %); en tanto, una minoría padeció ulceraciones y manchas negras (con 12,0 %, respectivamente). Conclusiones: En esta investigación se identificaron grandes afectaciones del complejo bucal, como periodontopatías, alteraciones de la articulación temporomandibular, entre otras, que pueden influir negativamente en la calidad de vida de estos pacientes.


Introduction: Rheumatoid arthritis is a systemic inflammatory chronic disease characterized by an autoimmune reaction and it can produce oral and facial manifestations. Objective: To describe the maxillofacial manifestations of the rheumatoid arthritis. Methods: An observational, descriptive series of cases study, of 25 patients with rheumatoid arthritis, randomly selected, who were admitted in the Rheumatology Service of Saturnino Lora Torres Teaching Clinical-Surgical Provincial Hospital in Santiago de Cuba was carried out, from September, 2016 to February, 2017, to whom an intraoral and extraoral detailed physical examination was made. Results: The intraoral localization where more manifestations existed was the protection periodonto (84.0 %), followed by the insertion periodonto (40.0 %). As for the extraoral manifestations, they were scarce, with a mild prevalence of damage in the parotid region (32.0 %). Most of the series presented temporomandibular joint changes (80.0 %) and periodontopathies (84.0 %); while a small minority suffered ulcerations and black stains (with 12.0 % each). Conclusions: In this investigation considerable changes of the oral complex were identified, such as periodontopathies, changes of the temporomandibular joint, among other that can influence negatively in the life quality of these patients.

Se realizó un estudio descriptivo y transversal de 150 habitantes del municipio de Palma Soriano en la provincia de Santiago de Cuba, durante el 2015, quienes pertenecían a 5 consultorios del médico de la familia y fueron seleccionados mediante un muestro estratificado en grupos etarios de interés, con vistas a determinar el estado de salud bucal en ellos, para lo cual se consideraron algunas variables del modelo de encuesta de salud bucodental de la Organización Mundial de la Salud, modificada en el 2003. Entre los resultados sobresalientes de la serie se obtuvo que predominara el sexo femenino, 92,8 % no presentaba alteraciones en la articulación temporomandibular, 75,3 % no padecía enfermedad periodontal alguna, la cual se observó mayormente en las edades de 35-44 años, y el tratamiento protésico parcial fuera el más empleado. Igualmente, el índice de dientes cariados, obturados y perdidos fue de 1,1 a los 12 años y reveló una mayor afectación en el grupo etario de 60-74 años por el componente dientes perdidos.


A descriptive and cross-sectional study of 150 inhabitants from Palma Soriano in Santiago de Cuba, was carried out during 2015 who belonged to 5 family doctor's offices and were selected by means of a stratified sampling in age groups of interest, aimed at determining the state of oral health in them, for which some variables of the dental oral health survey pattern of the World Health Organization were considered and modified in 2003. Among the outstanding results of the series it was obtained that the female sex prevailed, 92.8% didn't present disorders in the temporomandibular joint, 75.3% didn't suffer from any periodontal disease, which was mostly observed in persons aged 35 to 44, and the prosthetic partial treatment was the most used. In the same way, the decayed, filled and lost teeth index was 1.1 at 12 years and it revealed a higher damage in the 60-74 age group due to the lost teeth component.

ABSTRACT

Objective To investigate X‐ray ,CT and MRI features of synovial chondromatosis of the temporomandibular joint (TMJ) .Methods X‐ray ,CT and MRI features of eight patients of synovial chondromatosis of TMJ with histo‐pathologically con‐firmed were analyzed retrospectively .X‐ray examination and CT scanning were performed in all eight patients .Routine MRI scanning was performed in six patients and contrast‐enhanced MRI scanning was performed in two patients synchronously .Results Tumors occured unilaterally in all eight cases ,which occured on the right TMJ in six cases and on the left side in two cases .On X‐ray films , widen joint space and calcificated loose bodies occured in all eight cases .On CT scanning ,cystic‐solid mixed mass around the joint and calcificated loose bodies occured in all eight cases .On MR scanning ,multiple nodular long T1 and short T2 signal occured in six cases . Arthroedema and synovial hyperplasia with iso T1 and iso or slightly long T2 signal in six cases .On contrast‐enhanced MR ,homoge‐neous enhancement occurred in svnovial tissue and the edge of loose bodies in two cases .Conclusion The synovial chondromatosis of TMJ owns typical imaging features .The imaging findings can serve as a reference to improve diagnosis of synovial chondromatosis of TMJ .
