ABSTRACT

The aim is to evaluate the effect of therapeutic communication on cervical cancer patients’ preoperative anxiety and hope level. The convenience sampling method was used to select the inpatients who will receive radical surgery for cervical cancer in the department of obstetrics and gynecology of the Second Affiliated Hospital of Xi’an Jiaotong University from November 2016 to November 2019 as the research object. 50 patients were grouped by the random number table method: 25 patients were in the intervention group, and 25 patients were in the control group. Patients in the intervention group were given therapeutic communication on the basis of routine nursing, and patients in the control group were given routine nursing. Both groups were investigated with the Self-Rating Anxiety Scale (SAS) and Herth Hope Index (HHI) on the first day of admission and the day before surgery. Before the intervention, there was no statistically significant difference between the two groups (P>0.05) . After the intervention, the anxiety level of the intervention group was lower than that of the control group (P<0.05), and the hope level was higher than that of the control group (P<0.05). It can be seen that therapeutic communication can alleviate preoperative anxiety of cervical cancer patients, improve their hope level, promote patient recovery, and ease tense medical relationship.

ABSTRACT

Objective:To explore and analyze the effect of therapeutic communication system (TCS) intervention mode on Therapeutic effect and quality of life in patients with cardiac valve replacement.Methods:A total of 103 patients with cardiac valve replacement admitted from February 2018 to February 2019 were selected and divided into observation group (51 cases) and control group (52 cases) according to the random number table method. Patients in the control group were treated with staged rehabilitation intervention, while patients in the observation group were treated with staged rehabilitation combined with TCS intervention mode. Patients' prognosis, the score of Short Form 36-item Health Survey (SF-36) before and after intervention, the changes in the score of the Morisky label Scale (MMAS-8) before and after intervention, and perioperative indicators were recorded and compared between the two groups.Results:The quality of life scores of the observation group at 1 month,3 months and 6 months after intervention were (64.42±9.51), (76.23±9.19), (87.24±9.21) points, which were significantly higher than (58.73±9.38), (61.23±9.29), (76.29±9.42) points of the control group ( t values were 3.057, 8.237, 5.964, P<0.05);and also higher than those before intervention (54.29±9.14, 54.45± 9.31), the difference was statistically significant ( F values were 11.358, 7.581, P<0.001); the ICU hospitalization time (43.25±1.72),bed rest time (42.13±8.32) and hospitalization time (16.32±4.20) days in the observation group were significantly lower than (54.34±1.93), (72.33±8.54), (21.24±4.36) days in the control group, and the difference was statistically significant ( t values were 30.766, 18.174, 5.831, P<0.001); the drug compliance (7.21±0.17) was significantly higher than that of the control group (6.01±0.34) ( t value was 22.588, P<0.001). Conclusion:Stage rehabilitation combined with TCS intervention mode can effectively improve the prognosis and perioperative indicators of patients with heart valve, and improve the quality of life of patients, which is worthy of clinical application.

ABSTRACT

BACKGROUND: This study was conducted to develop therapeutic communication skills for nursing students by using video clip.METHODS: The development of therapeutic communication contents were carried out in three steps: The first step is to extract the methods of therapeutic communication described in the case report prepared by a nursing student during mental clinical practice which is highly frequently used and prone to confusion. The second step is to extract cases and create scenarios that reflect the techniques extracted. The third step is to film the developed scenarios with expert advice and then edit it to reflect the content utilization to enhance the efficiency of teaching.RESULTS: Eight video content reflecting therapeutic communication techniques and eight video contents that improperly reflected therapeutic communication, a total of 16 video contents were finally developed.CONCLUSION: The developed video clip based on real error cases in this study can be utilized as an educational contents for nursing students to apply therapeutic communication skills.

Este artigo fundamenta-se nos princípios da comunicação terapêutica, do psiquiatra Jürgen Ruesch, aliados aos processos da comunicação organizacional, com vistas ao estudo e à implementação de uma comunicação específica para as organizações de saúde. A pesquisa, de caráter qualitativo, foi realizada na unidade de Onco-Hematologia Pediátrica de um hospital infantil público terciário do Distrito Federal, entre 25 de setembro e 6 de novembro de 2015, por meio de entrevistas em profundidade, aplicadas em dois grupos ­ profissionais da saúde e mães das crianças hospitalizadas ­ com o objetivo de verificar que pressupostos da comunicação terapêutica eram aplicados no tratamento das crianças e por quais categorias profissionais. Foram observados comportamentos comunicacionais conscientes, com potencial terapêutico, indicando a possibilidade de estabelecimento de um modelo comunicacional que atenda às especificidades das organizações de saúde. This article is based on the principles of therapeutic communication, by the psychiatrist Jürgen Ruesch, allied to the processes of organizational communication, with a view to studying and to implementing a specific communication for health organizations. A qualitative research was carried out at the Pediatric Onco-Hematology unit of a public tertiary children's hospital in the Federal District, Brazil, between September 25 and November 6, 2015, through in-depth interviews, applied to two groups ­ health professionals and mothers of hospitalized children ­, whose objective was to verify what assumptions of the therapeutic communication were used in the treatment of the children and by which professional categories. Conscious communicational behaviours with therapeutic potential were observed, indicating the possibility of establishing a communication model satisfying the specificities of the health organizations. Este artículo se fundamenta en los principios de la comunicación terapéutica, del psiquiatra Jürgen Ruesch, en los procesos de comunicación organizacional, con miras al estudio y a la implementación de una comunicación específica para las organizaciones de salud. La investigación, de natulareza cualitativa, fue realizada en la unidad de Onco-Hematología Pediátrica de uno hospital infantil público en el Distrito Federal, entre 25 de septiembre y 06 de noviembre de 2015, por medio de entrevistas en profundidad, aplicadas a dos grupos ­ profesionales de salud y madres de los niños hospitalizados ­ con el objetivo de verificar qué presupuestos de la comunicación terapéutica eran aplicados al tratamiento de los niños y por cuáles categorías profesionales. Se han observado comportamientos comunicacionales conscientes, con potencial terapéutico, que indican la posibilidad de crear un modelo de comunicación que satisfaga las especificidades de las organizaciones de salud. ABSTRACT

INTRODUCTION Carcinomas arising in the male breast is a rare occurrence, with a known incidence of only 0.6% of all the reported cases of breast carcinomas. CASE HISTORY A36year old male presented with a hard retro-areolar mass in the left breast since 6 months. FNAC smears were cellular and showed atypical cells lying singly as well as in dyscohesive clusters with abundant mucinous material in the background. Cytological diagnosis of Mucinous Carcinoma of breast was suggested and were confirmed onhistopathology. DISCUSSION Male breast cancers are rare aggressive lesions reported mainly in the older age groups. It is important to recognize these lesions for focusedtreatment strategies and an overall improved survival. CONCLUSION This case is being presented due to uncommon nature of tumour presenting at an early age. FNAC is a useful tool for diagnosis with high sensitivity but histopathology remains gold standard.

Resumo OBJETIVO Analisar a comunicação terapêutica na interação entre profissional de saúde e paciente hipertenso na Estratégia Saúde da Família. MÉTODOS Estudo descritivo com abordagem qualitativa. A amostra constituiu-se de 14 pacientes hipertensos e dois profissionais da ESF em um município do Estado do Ceará, Brasil, em 2016. Na coleta de dados, utilizou-se um checklist para observação sistemática não participante contendo as estratégias de comunicação terapêutica: expressão, clarificação, validação e um diário de campo, sendo tratados pela análise de conteúdo. RESULTADOS Verificou-se que os profissionais da ESF não utilizam de forma adequada a comunicação terapêutica, reconhecendo a necessidade de investimento nesse dispositivo, que atua como ponte de acesso aos usuários, potencializa as práticas assistenciais e abre caminhos que instrumentalizam as relações interpessoais. CONCLUSÕES Desvelou-se que as estratégias de comunicação terapêutica são pouco exploradas pelos profissionais de saúde, sendo necessário, portanto, desenvolver habilidades para empregá-las adequadamente no cuidado ao hipertenso.


Resumen OBJETIVO Analizar la comunicación terapéutica entre el profesional de la salud y pacientes hipertensos en la Estrategia de Salud de la familia. MÉTODOS Estudio descriptivo con enfoque cualitativo. La muestra estuvo constituida por 14 pacientes hipertensos y dos profesionales del Estrategia de Salud de la Familia (ESF - "Estratégia Saúde Família") de un municipio del estado de Ceará, Brasil, en 2016. En la recopilación de datos, seutilizó una lista de verificación para la observación sistemática no participante la cual contiene las estrategias de comunicación terapéutica: expresión, claridad, validación y un diario de campo que trata del análisis de contenido. RESULTADOS Se comprobó que los profesionales del ESF no utilizan de manera adecuada la comunicación terapéutica, reconociéndose la necesidad de inversión en este dispositivo que actúa como puente para el acceso a los usuarios, realza las prácticas asistenciales y abre caminos que instrumentalizan las relaciones interpersonales. CONCLUSIONES Se reveló que las estrategias de comunicación terapéutica son poco aplicadas por los profesionales de la salud que las requieren, por lo tanto, se hace necesario desarrollar destrezas para utilizarla correctamente en el cuidado de los pacientes hipertensos.


Abstract OBJECTIVE To analyze the therapeutic communication in the interaction between health professionals and hypertensive patients in the Family Health Strategy. METHODS Descriptive study with qualitative approach. The sample consisted of 14 hypertensive patients and two health professionals of the Family Health Strategy (ESF - "Estratégia Saúde Família") in a city of the state of Ceará, Brazil, in 2016. In the data collection, a checklist was used for non-participant systematic observation containing the strategies of therapeutic communication, namely: expression, clarity, validation, and a field diary, being these subjected to content analysis. RESULTS It was noted that ESF professionals do not adequately use therapeutic communication, indicating the need of investment in this device, which acts as a bridge for users, enhances care practices and opens paths that instrumentalize interpersonal relationships. CONCLUSIONS It was realized that health professionals are not fully exploring therapeutic communication strategies, therefore being necessary to develop skills to use these techniques correctly when caring for hypertensive patients.

ABSTRACT

Objective: To investigate the application of therapeutic communication in patients with foreign body in digestive endos?copy. Methods: A total of 31 patients with foreign body undergoing endoscopic treatment from Jun 2014 to Feb 2015 were selected as control group and received conventional education. A total of 31 patients with foreign body undergoing endoscopic treatment from Mar 2015 to Oct 2015 were selected as observation group and received therapeutic communication. The number of mind state, nursing sat?isfaction and adverse events were analyzed. Result: There was significant difference in the number of mind state, nursing satisfaction and adverse events in 2 groups. Conclusion: Therapeutic communication can help to resolve the patient?s sense of fear, improve the mind state, active cooperation, reduce operation risk and adverse events, ensure the treatment effect, improve nursing satisfaction.

ABSTRACT

Objective To evaluate the effect of therapeutic communication on negative emotion and quality of life of stoke patients. Methods A total of 120 stroke patients were divided into the control group and the observation group equally. The former received routine nursing and the latter routine nursing plus therapeutic communication. After 4 weeks, they were evaluated by self-rating anxiety scale (SAS), self-rating depressive scale (SDS), trait coping style questionnaire (TCSQ) and the short-form health survey-36 (SF-36). Results After the intervention, the scores on anxiety, depression and negative coping in the observation group were lower than those in the control group (P<0.01), while the scores on positive coping and quality of life were higher in the observation group than those in control group (P<0.01). Conclusion Therapeutic communication can effectively improve stroke patients′negative emotion and quality of life.

ABSTRACT

Objective To evaluate the effect of therapeutic communication on perioperative anxiety of cancer patients by using the Meta-analysis.Methods The randomized controlled studies of therapeutic communication for perioperative anxiety of cancer patients were gathered from Pubmed,Embase,Cochrane Library,China Biology Medicine Disc (CBM),China Scientific Journal Database by VIP (CSJD),China National Knowledge Infrastructure (CNKI) and Wangfang Data,while other relative researches were also searched by hand.According to inclusion and exclusion criteria articles were evaluated by two reviewers independently.The studies were assessed according to the Juni assessment.RevMan 5.2 was used for statistical analysis.Results Seven studies with 506 patients were included,of which therapeutic intervention group contained 256 patients,the control group included 250 patients.Meta-analysis showed that compared with the control group,therapeutic communication system could reduce perioperative anxiety of cancer patients [MD=-8.00,95%CI (-8.91--7.10),P <0.01],improve positive perioperative coping of patients [MD=5.50,95%CI(4.05-6.95),P < 0.01],reduce negative perioperative coping of patients [MD=-7.62,95%CI (-9.13--6.11),P < 0.01],increase the satisfaction degree of patients [MD=0.55,95%CI (0.33-0.77),P <0.01],the differenccs were statistically significant.Conclusions Therapeutic communication can contribute to relieve perioperative anxiety of cancer patients,optimize their coping styles and improve the satisfaction degree of patients with nursing work.

ABSTRACT

O cuidado de enfermagem a pacientes com agitação psicomotora e/ou comportamento agressivo deve ser permeado por abordagem segura e eficaz para evitar e/ou minimizar os agravos físicos e psíquicos para pacientes e para a equipe de saúde que o atende. Contudo, é preciso refletir como é desenvolvido o cuidado de enfermagem a esses pacientes no Pronto Atendimento. Trata-se de pesquisa qualitativa, exploratória com os seguintes objetivos: verificar a compreensão da equipe de enfermagem sobre comportamento agitado e/ ou agressivo manifestado pelo paciente e descrever o cuidado de enfermagem prestado ao paciente com comportamento agressivo e/ou agitado no Pronto Atendimento. Os participantes foram: seis enfermeiros, dois técnicos de enfermagem e nove auxiliares de enfermagem. Os dados foram coletados por meio de encontros coletivos gravados, com a técnica de discussão de grupo. Esses encontros foram mensais, três no turno da manhã e três no turno da tarde. Para discussão, foram lançadas as perguntas: 1. Como você define o comportamento agitado e/ou agressivo manifestado por um paciente?; 2. Como o cuidado de Enfermagem ao paciente com manifestação de comportamento agitado e/ou agressivo é desenvolvido no P.A.?; e 3. Como você considera que deve ser feita a abordagem pela equipe de enfermagem ao paciente com comportamento agitado e/ou agressivo? Para a análise das informações obtidas das transcrições dos encontros com os participantes, foi utilizada a Proposta Operativa de Análise Qualitativa. Emergiram as seguintes categorias: Comportamento agitado e agressivo manifestado pelo paciente; Cuidado de enfermagem ao paciente com comportamento agitado e agressivo; e Contenção física. Para os sujeitos, a agitação psicomotora ocorre quando o paciente agitado interfere no ambiente e no comportamento dos outros pacientes, contudo, não agride, apenas se movimenta, representando maior risco para a própria integridade física e saúde do que para a dos outros. No cuidado ao paciente com comportamento agitado e agressivo, a equipe de enfermagem deve dispor da observação, escuta e diálogo como ações primordiais em busca da construção do vínculo, sem os quais se tornam difíceis a eficácia das intervenções e a tentativa de mudança do comportamento do paciente. A contenção física é a última opção utilizada no cuidado ao paciente com manifestação de agitação e agressão, sendo somente realizada quando o paciente oferece risco para si ou para outros e que, também, traga benefício a ele. A partir dos encontros, surgiu a construção de uma Ficha de Acompanhamento de Contenção Física na Unidade de Pronto Atendimento, com o intuito da melhoria do cuidado de enfermagem ao paciente com comportamento agitado e/ou agressivo. A comunicação terapêutica permeou as discussões como um cuidado do antecipatório a todas as demais práticas. Concluiu-se que o cuidado de enfermagem a pacientes com comportamento agitado e/ou agressivo pode ser melhorado na instituição apenas se houver a inserção de protocolos, educação continuada, discussões e construções coletivas.


Nursing care for patients with psychomotor agitation and/or aggressive behavior should be permeated by safe and effective approach to prevent and/or minimize the physical and psychological injuries to patients and their caregivers. However, the way as the nursing care is developded for these patients in the Emergency Services Units needs to be analyzed. It is a qualitative and exploratory study aiming to check the understanding of the nursing staff on agitated and/or aggressive behavior manifested by the patient, and to describe the nursing care provided to these patients in Emergency Services. Participants for this study were six nurses, two nursing technicians, and nine nursing assistants. Data were collected through collective meetings recorded with the technique of group discussion. These meetings were performed monthly, three in the morning and three in the afternoon. For discussion, the following questions were released: "1. How do you define the agitated and/or aggressive behavior manifested by a patient? 2. How is the nursing care developed in Emergency Services for patients who show agitated and/or aggressive behavior? 3. How do you consider that the nursing staff should approach the patient with agitated and/or aggressive behavior?" Operative Proposal of Qualitative Analysis was used for anlyzing information obtained from transcritions of meetings with participants. The following categories emerged: "Agitated and aggressive behavior manifested by the patient, Nursing care for patients with aggressive and agitated behavior, and Physical containment". For individuals, psychomotor agitation occurs when the patient affects the environment and other patients' behavior, but do not attack them, just moves, representing greater risk to his own health and physical integrity than for others. For treatment of patients with aggressive and agitated behavior, the nursing staff needs to observe, listening, and dialoguing as the primary actions in pursuit of creating the link, essential for the interventions effectiveness and attempt to change patient behavior. Physical restraint is the last option used for treatment of patients that show agitation and aggression, being performed only when the patient presents risk to himself or others; and when this procedure also brings benefit to patient. Based on the meetings, Data Monitoring Physical Restraint in the Emergency Services Unit was constructed aiming to improve the nursing care of the patient with agitated and/or aggressive behavior. Therapeutic communication permeated the discussions as an anticipatory care to all other practices. It was concluded that nursing care to patients with agitated and/or aggressive behavior can only be improved in the institution, if there is inclusion of protocols, continuing education, discussions, and collective constructions.

O abuso sexual infantil e suas consequências à vítima e à família têm sido alvo de constantes pesquisas e discussões. Este trabalho objetiva apresentar um estudo de caso de uma criança que sofreu esse tipo de violência, a fim de discutir a comunicação terapêutica do processo e pensar em estratégias de trabalho com esta temática. Ao longo da psicoterapia, pode-se perceber que o paciente demonstrava medo, receio de contato com o outro e percepção de que era uma pessoa sem energia. No entanto, a comunicação terapêutica possibilitou novas formas de expressão, criatividade e espontaneidade. O processo foi prejudicado pelo elevado número de faltas, que pode ser entendido pela dificuldade dos pais da criança em entender a necessidade de psicoterapia para ele. Entende-se que toda a família deve ser engajada neste processo, de forma que as dificuldades familiares também possam ser trabalhadas...


Sexual abuse against children, and its consequences to the victim and family, has been the subject of ongoing research and discussion. This study presents a case study of a child who suffered such violence in order to discuss the therapeutic communication process and think about strategies for working with this theme. Over the course of psychotherapy, one can see that the patient showed fear, apprehension of contact with others and the perception that he was a person nerveless. However, therapeutic communication made possible new forms of expression, creativity and spontaneity. The process has been hampered by the high number of faults that can be understood by the child's parents' difficulty in understanding the need for psychotherapy for him. ABSTRACT

Objective To investigate the effect of therapeutic communication system on alleviation of depression emotion of postoperative tumor patients undergoing chemotherapy.Methods 56 cases of cancer patients undergoing chemotherapy after surgery were randomly divided into the observation group and the control group with 28 cases in each group.The observation group was given therapeutic communication system based on routine care,the control group only received routine care.The self-rating depression scale (SDS),automatic thoughts questionnaire (ATQ) and quality of life evaluation questionnaire (QLQ-C30 3.0) were used for evaluation in two groups before and after intervention.Results The depression mood,negative automatic thoughts and quality of life in the observation group were better than the control group,the difference was statistically significant.Conclusions Application of therapeutic communication system on postoperative tumor patients undergoing chemotherapy can alleviate depression,negative automatic thoughts and quality of life significantly.It implies the effectiveness and feasibility of psychological intervention for postoperative tumor patients undergoing chemotherapy,which is worthy of clinical application.

Objective To explore the clinical practice and patient experience of perioperative therapeutic communication in rectal cancer patients, in order to provide theoretical basis for the further clinical application of therapeutic communication. Methods To adopt phenomenology method of qualitative research, collecting data by the direct participation, observation, half structural formula thorough interview and other methods. To establish coding and refine theme. Results Four themes of the rectal cancer patients experience of perioperative therapeutic communication were generalized: emotional adaptation, actively coping with surgery, intraoperative cooperation, postoperative rehabilitation and self-care. Conclusions Application of therapeutic communication system can effectively interfere with patients cognitive behavior, improve communication ability of nurses and extended the nursing service connotation.

ABSTRACT

Esta pesquisa foi realizada com a participação de cinco enfermeiros assistenciais, em um hospital geral, localizado na região Oeste do Paraná, no período de 14/07 à 03/09 de 1999. Teve como objetivo descrever como enfermeiros assistenciais vivenciam a relação de ajuda terapêutica com o paciente internado em hospital geral, após um programa de educação continuada. Adotou-se como referencial teórico, conceitos de comunicação e relação de ajuda terapêutica. A opção metodológica foi a modalidade convergente-assistencial. Nas reuniões de grupo, para discussão dos conceitos sobre comunicação e relação de ajuda terapêutica, utilizamos um jogo educativo sobre o assunto. A análise dos dados evidenciou a adequação dos referenciais teórico e metodológico com o todo da pesquisa. Podemos concluir que os enfermeiros assistenciais foram capazes de construir conceitos e desenvolverem-se no uso da relação de ajuda terapêutica.


This research took place with five nurses' participation in a general hospital, in the west of Paraná state, from 14/07 to 03/09/1999. It aimed to describe how nurses experience the therapeutic relationship with patients in hospital, after a continuous educational program. Communication and therapeutic relationship concepts were adopted for the theory. The methodology used was the care-convergent . In the meeting of the groups, which aimed discussing the concepts about communication and the therapeutic helpful relationship , we used an educational game about the subject. The data analysis was focused on the agreement of the theoretical and methodological framework with the whole research . We could conclude that nurses were able to build concepts and to improve their therapeutic help relationship.
