ABSTRACT

OBJECTIVES@#To investigate the positive rate of enterovirus (EV) nucleic acid in throat swabs of term late neonates hospitalized during the coronavirus disease 2019 (COVID-19) epidemic and the clinical characteristics of the neonates.@*METHODS@#A single-center cross-sectional study was performed on 611 term late infants who were hospitalized in the neonatal center from October 2020 to September 2021. Throat swabs were collected on admission for coxsackie A16 virus/EV71/EV universal nucleic acid testing. According to the results of EV nucleic acid test, the infants were divided into a positive EV nucleic acid group (8 infants) and a negative EV nucleic acid group (603 infants). Clinical features were compared between the two groups.@*RESULTS@#Among the 611 neonates, 8 tested positive for EV nucleic acid, with a positive rate of 13.1‰, among whom 7 were admitted from May to October. There was a significant difference in the proportion of infants contacting family members with respiratory infection symptoms before disease onset between the positive and negative EV nucleic acid groups (75.0% vs 10.9%, P<0.001). There were no significant differences between the two groups in demographic data, clinical symptoms, and laboratory test results (P>0.05).@*CONCLUSIONS@#There is a certain proportion of term late infants testing positive for EV nucleic acid in throat swabs during the COVID-19 epidemic, but the proportion is low. The clinical manifestations and laboratory test results of these infants are non-specific. Transmission among family members might be an important cause of neonatal EV infection.

ABSTRACT

Objective To investigate the false negative rate of throat swab nucleic acid test of severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) and to analyze the causes, so as to provide references for the prevention and control of coronavirus disease 2019 (COVID-19) in China. Methods A retrospective analysis was conducted on the throat swab nucleic acid test results of 1 452 COVID-19 patients admitted to Guanggu Branch of Maternity and Child Healthcare Hospital of Hubei Province from Feb. 19 to Mar. 20, 2020. The negative results before positive results at discharge were judged as false negative results, and the false negative rate was calculated. The discharged patients were followed up to screen for the patients who were positive for nucleic acid test again, and the relationship between the times of consecutive negative nucleic acid tests before discharge and the positive again results was analyzed. Results Among the 1 452 COVID-19 patients, 592 (40.77%) were males and 860 (59.23%) were females. A total of 212 cases (14.60%) had false negative results. Twenty-eight cases (1.93%) were discovered nucleic acid positive again after discharge. Among the 918 patients whose nucleic acid tests were negative for two consecutive times, 24 (2.61%) were positive again, which was significantly higher than that of the patients whose nucleic acid tests were negative for three consecutive times (0.75%, 4/534; χ2=6.21, P=0.012 7). Conclusion The throat swab nucleic acid test of SARS-CoV-2 has a certain proportion of false negative results, which is one of the reasons for COVID-19 patients are found nucleic acid positive again after discharge. Multiple and continuous tests by different testers are recommended before discharge, and negative nucleic acid test for three or more consecutive times can reduce the incidence of nucleic acid positive results again after discharge.

ABSTRACT

The novel coronavirus pneumonia (NCP) has spread from Wuhan to all parts of China since December 2019, and the prevention and control of NCP is a top priority for medical staff. Now report three cases of NCP patients, whose viral nucleic acids still positive in stool after throat swab detection turned negative. In view of the highly homologous and similar clinical manifestations between the 2019 novel coronavirus (2019-nCoV) and the severe acute respiratory syndrome(SARS) related coronaviruses, it is recommended to attach great importance to the detection of the viral nucleic acids in stool, with the reference of SARS prevention and control experience. In order to minimize the risks of gastrointestinal spread, the detection of 2019-nCoV nucleic acids in stool may be recommended as the reference standard of disisolation and discharge.

Objective@#To isolate and identify human rhinovirus (HRV) from throat swab samples from patients with acute respiratory infection in Anhui Bengbu.@*Methods@#The throat swab specimens from 108 patients with acute upper respiratory tract infection diagnosed in the Anhui Bengbu were selected as samples. RNA was extracted and detected by HRV universal primers; H1-Hela cells were infected with the positive samples for virus isolation, and HRV VP1 gene amplification was performed and gene phylogenetic tree analysis of the successfully isolated HRV strains was done.@*Results@#Only 5 samples were positive for HRV by the real-time PCR, and only one sample showed significant cytopathic effects after three passages of H1-Hela cells were infected, and the HRV VP1 gene was amplified by RT-PCR in the sample that was named TYZQ201901. The sequence analysis showed that the VP1 nucleotide homology of the strain with other representative HRV A strains was over 95%. The gene phylogenetic tree analysis showed that the strain had the closest genetic distance to the RMH127/2013 strain, and both were on a branch and was confirmed to be HRV type A virus.@*Conclusions@#An HRV type A strain virus was isolated from throat swab samples from patients with acute respiratory infection in Anhui Province by the H1-Hela cells, and the HRV virus separation technology system were initially established.

ABSTRACT

Objective To investigate the diagnostic value of plasma ,peripheral blood mononuclear cell (PB-MC) and throat swab EB virus (SBV) load in the patients with infectious mononucleosis (IM ) aged over 16 years old .Methods The detection results in 130 patients with suspected IM aged over 16 years old of 3 differ-ent samples of plasma ,peripheral blood mononuclear cell(PBMC) and throat swab EB virus(SBV) load were analyzed retrospectively .Results Among 61 cases of IM verified by clinic and laboratory ,16 cases of IM were detected by plasma sample ,the detection rate of EBV-DNA was 26 .22% ;in 31 cases of PBMC detection ,the EBV-DNA detection rate was 61 .29% ;in 31 cases of throat swab detection ,the EBV-DNA detection rate was 83 .78% .The EBV-DNA detection rate of the combined detection with one indicator or multiple indicators positive of plasma ,PBMC and throat swab as the basis was 100 .00% .Conclusion The combined detection of plasma ,PBMC and throat swab can improve the positive rate in the patients with IM aged over 16 years old , w hich can provide more bases for pathogenic diagnosis and treatment for clinic .

Introducción: las faringoamigdalitis se encuentran entre las enfermedades infecciosas que más afectan a los niños en edad escolar, cuya primera causa bacteriana es el estreptococo beta-hemolítico del grupo A. Objetivo: determinar la presencia de dicha bacteria en exudados faríngeos de niños asintomáticos. Métodos: se realizó un estudio transversal y descriptivo de 80 niños de ambos sexos en el Círculo Infantil Alberto Fernández de Santiago de Cuba, durante el año 2014. Resultados: se obtuvo un elevado porcentaje de infectados por bacterias potencialmente patógenas, con predominio del estreptococo beta-hemolítico del grupo A, así como una mayor frecuencia de niños en las edades de 4 y 5 años y del sexo masculino; asimismo, los antibióticos de mayor sensibilidad fueron la penicilina y amoxicilina, mientras que la kanamicina fue el de mayor resistencia. Conclusiones: esta investigación posibilitó conocer las características de colonización de esta bacteria, así como su sensibilidad antimicrobiana


Introduction: pharyngotonsillitis is among the infectious diseases that most affect children in school age whose first bacterial cause is the group A beta-hemolytic streptococcus. Objective: to be determine the presence of this bacteria in throat swabs of asymptomatic children. Methods: a cross-sectional and descriptive study of 80 children from both sexes was carried out in Alberto Fernández day care center in Santiago de Cuba, during 2014. Results: a high percentage of infected children by potentially pathogens bacterias was obtained, with prevalence of group A beta-hemolytic streptococcus, as well as a higher frequency of children of the male sex aged 4 and 5; also, the antibiotics of more sensitivity were penicillin and amoxicillin, while kanamycin was that of higher resistance. Conclusions: with this investigation the colonization characteristics of this bacteria were known, as well as its antimicrobian sensitivity

ABSTRACT

Background: Sore throat is the one of the commonest complaint of the patients in ENT OPDs. The prevalent cause of sore throat is in India is group A streptococci (GAS).But physicians across the country underestimate the real cause of the sore throat and prescribe the irrational use of antibiotic leading to development of resistance towards the antibiotics by the pathogens. Methods: The100 throat swab samples were collected with all aseptic precautions from sore throat patients in ENT OPD and also from other department of microbiology from March 2014 to December 2015. And were sent to microbiology department for throat swab culture & Sensitivity reporting. Result: A total of 100 patients suffering from sore throats were included in this study of which 57 were males and 43 were females. Out of 100 patients 35 were pathogenic 59 were nonpathogenic & there were no growth in 6 patients. The age range of study is from 2 years to 70 years. The isolated organism were E.Coli, Klebsiella pneumoniae, Streptococcus pneumoniae, Staph aureus,and alpha haemolytic streptococci were found out of 35pathogenic organism. Biomodal peak of more pathogenic growth was observed in the month of September, November and December. Culture sensitivity reports showed high sensitivity in of various pathogens towards erythromycin (mostly), Prisinomycin ,Cotrimoxazole, Linezolid,Vancomycin ,Cefaperazone, Polymyxin B Norfloxacin, Ampicillin and resistance towards Chloramphenicol,Gentamycin, Ampicillin, Cefazolin, Amoxicillin etc. Conclusion: This study gives us an insight to the current state of causative pathogens and their antimicrobial sensitivity from throat swab in Teerthanker Mahaveer Hospital. Alpha haemolytic Streptococci and staphylococcus aureus were the commonest organism isolated from throat swab.

ABSTRACT

Objective To perform pathogenic surveillance of scarlet fever in Xicheng District of Beijing in 2014 ,and provide sci‐entific data for developing reasonable prevention policy for the disease .Methods The throat swab specimens from 212 patients who were diagnosed with streptococcal infection ,tonsillitis and angina in surveillance hospital were collected ,and from which the patho‐gens were isolated and identified .Results In the 212 samples ,the positive rate of A group hemolytic streptococcus were 21 .2%(45/212) .The patients were mainly 4-15 years old ,especially 6- <8 years old .The positive rate was highest in 6- <7 years old children .The peak of disease incidence was observed in May and June .Conclusion Preschools and schools were the key sites for scarlet fever prevention and control ,therefore ,the surveillance and prevention should be further strengthened to prevent the out‐break .

ABSTRACT

OBJECTIVE To investigate Epstein-Barr virus (EBV) infection and clinical symptoms among hospitalized children by detection of EBV in their throat swabs. METHODS During from Oct 2004 to Apr 2006, EBV-DNA was detected from throat swabs of inpatients in pediatric ward by the technology of fluorescence quantitative polymerase chain reaction. RESULTS Totally 3429 cases were detected, 738 cases were positive. The positive rate was 21.5%. In different age groups, the rate of EBV infection was 18.8% for ≤3 years old, 26.5% for 4-7 years old, 29.0% for 8-11 years old, and 31.7% for 12-15 years old, respectively. ABSTRACT

PURPOSE: Nebulizers are commonly used to treat children with asthma. Some pulmonary infections associated with contaminated nebulizer units have been reported. The aims of this study were to determine whether home nebulizers are repositories of bacteria and to ascertain how asthmatic patients use and maintain their nebulizers at home. METHODS: Fifty-five children with asthma aged 2 to 6 years were enrolled in this study. The parents were asked to bring their child's nebulizer units to regular follow-up visits. Bacterial samples which were obtained from the masks(or mouthpieces), nebulizer chambers and throat swabs were inoculated to blood agar, chocolate agar, and McConkey agar. Cultures were incubated at 5 percent CO2 incubator and observed after 24 hours. Parents completed a questionnaire regarding nebulizer usage and care in their child. RESULTS: Bacterial contamination was found in samples from 27(49.1%) of the 55 masks(or mouthpieces) and 24(43.6%) of the 55 chambers. The predominant microorganisms isolated were Acinetobacter baumannii, Bacillus subtilis, Klebsiella pneumoniae, Staphylococcus aureus, and Candida albicans. The questionnaire revealed that 40 parents cleaned the nebulizer units with tap water(and soap) and dried then, as they had received cleaning and drying instructions from a medical staff member. Seven organisms from nebulizer units were indistinguishable from the patient's throat isolates. CONCLUSION: We have demonstrated that home nebulizers in these asthmatic children were frequently contaminated with aerobic bacteria. Aerosolization might transmit these bacteria to their upper respiratory tracts. The education and training of home caregivers should include the principles of cleaning and maintenance of their child's nebulizer units.

ABSTRACT

PURPOSE: To know the true carrier rate and the immunologic responses (antistreptolysin O :ASO) to beta-hemolytic streptococci which obtained from normal school children with or without carrying streptococci. And to study the clinical meaning of the number of organisms found in culture. METHODS: One hundred ninty-eight first grade of elementary school children were included. I obtained throat swab culture from tonsil and tonsilar fossa with sterile cotton ball stick and plated on sheep blood agar dish, counted number of colonies of beta-hemolytic streptococci (BHS) after overnight incubation, examined ASO titer (Rantz-Randall method) simultaneously, and differentiated grouping of BHS with Streptex. RESULTS: 1) Of the 198 normal school children, 34 (17.2%) had cultured BHS. 16 (8.1%) were group A, 4 (2.0%) were group C, 2 (1.0%) were group G, and 12 (6.1%) were non-grouping BHS. 2) 14 (22.2%) out of 63 normal children without carrying BHS and 16 (47.1%) out of 34 with carrying BHS had come out above 250 units of ASO. 13 (59.1%) out of 22 group A,C,G streptococcal carriers had showed above 250 units of ASO. The true carrier rate of BHS were 18 (9.0%) out of 198 elementary school children. 3) When I had counted the number of colonies from 34 carrier of BHS, 1+ was 67.6% and 2+ was 17.6%. It was 85.2% of total children with carrier. 4) There were significant differences between the mean titer of logarithmic ASO obtained from group A BHS carrier and that of normal children without carrying BHS (P=0.001), and differences between that of group A and group B,C,G streptococci (P= 0.0005). CONCLUSIONS: The ASO titer obtained from BHS carrier is higher than normal school children, which suggests the possibility of subclinical infection, the possibility of persistent habor of organism since past infection had contracted, and the possibility of replaced old organism in past infection with new organism. It should be performed follow-up study for identifying the persistence of immunologic response in accordance with or without culturing organism.
