ABSTRACT

Purpose: To investigate the clinical effect of sodium hyaluronate eye drops combined with tobramycin, dexamethasone, and pranoprofen eye drops in the treatment of dry eye after phacoemulsification. Methods: Medical records of 188 patients with dry eye after phacoemulsification, treated in our hospital from August 2020 to August 2021, were included and divided into groups based on the type of dry eye treatment. Patients in the control group (n = 90) were treated with tobramycin, dexamethasone, and pranoprofen eye drops. Patients in the test group (n = 98) were treated with sodium hyaluronate, tobramycin, dexamethasone, and pranoprofen eye drops. The tear secretion test (TST), film rupture time (BUT), corneal fluorescence staining (FS) score, levels of interleukin (IL)?6, and tumor necrosis factor (TNF)?? before the treatment and at 1 month follow?up, the overall total effective rate and the number of completely cured cases were compared between the two groups. Multiple linear regression was used to calculate coefficients for predicting clinical variables. Ordinal logistic regression was used to compute coefficients and odds ratios for predicting effective scores. Results: The total effective rate in the observation group (99%) was significantly higher than that in the control group (80.00%). After the treatment, the TST and BUT indexes of the observation group were higher compared to the control group, and the FS score of the observation group was lower than that of the control group. TNF?? and IL?6 levels of the observation group were lower compared to the control group. Logistic regression analysis demonstrated that the differences in the effective rate and clinical variables between the two groups remained significant after adjusting for the effect of age. Conclusion: A combination of sodium hyaluronate, tobramycin, dexamethasone, and pranoprofen eye drops for the treatment of dry eye after phacoemulsification can improve the curative effect, reduce corneal fluorescein staining and inflammatory factor index levels compared to the treatment regimen lacking sodium hyaluronate eye drops.

ABSTRACT

Objective:To investigate the feasibility of nebulizing injectable Tobramycin for the treatment of Pseudomonas aeruginosa (Pa) in pediatric cystic fibrosis (CF) patients. Methods:The clinical data of 3 CF children with Pa infection who were treated by nebulizing injectable Tobramycin in Shenzhen Children′s Hospital from September 2017 to March 2021 were retrospectively analyzed. The efficacy and safety were explored. The nebulized injectable Tobramycin (160 mg/dose) was given twice daily after airway clearance. After one-month treatment course, oral Ciprofloxacin would be given [30 mg/(kg·d)] to patients for another 1 month if Pa was still positive in repeat sputum cultures. Results:There were 2 males and 1 female in 3 cases recruited. The youngest patient was 1-year-old when receiving Tobramycin treatment. After airway clearance and inhalation of injectable Tobramycin, all had improvements in respiratory symptoms and chest CT scan. Two cases took additional oral Ciprofloxacin as Pa was still positive after the 1-month treatment course of Tobramycin.Pa turned to negative in all 3 cases after treatment for 3 months to 1.5 years.Besides, after treatment all the 3 patients had normal liver and renal functions, and normal hearing in multiple follow ups.One patient had a normal brainstem auditory evoked potential in the reexamination. Conclusions:Nebulizing injectable Tobramycin would be a reasonable alternative to inhaled Tobramycin solution for treating pediatric CF patients with Pa in view of the present condition in mainland China. However, it is still worth further study and discussion.

ABSTRACT

OBJECTIVE@#To explore effect of tobramycin (TOB) on healing of femoral fractures in rats.@*METHODS@#Totally 32 male sprague-dawley (SD) rats were selected and randomly divided into sham group (group A), fracture group (group B), fracture with TOB group (group C) and fracture + TOB + IWR-1 group (group D), 8 rats in each group. Close femoral fracture model in rats were established in group B, C and D, group A was sham operation without otherwise process. Group D was intraperitoneal injected 100 μl (8 μM) of Wnt pathway inhibitor IWR-1-endo (IWR-1) before molding at 1 day. At 1 day after molding, 100 μl (100 μM) of TOB was intraperitoneally injected into group C and D at once a day for 7 days. At 7 weeks after modling, fracture healing of group B, C and D were observed by X-ray, Western blotting was appilied to detect alkaline phosphatase(ALP) and Runt related transcription factor 2 (RUNX2) and β-catenin of Wnt passway.@*RESULTS@#X-ray results showed fracture line disappeared, callus formation and fracture healing well in group C compared with begning of molding; while a little fracture line, callus formation and fracture malunion in group B and d could be seen. Western blotting results showed ALP, RUNX2 and expression of β-catenin in group B, C and D were higher than that of group A (@*CONCLUSION@#Tobramycin could promote osteoblast differentiation and fracture healing by stimulating Wnt / β-catenin signaling pathway, up regulating expression of ALP and RUNX2.

ABSTRACT

@#AIM:To find out the pathogenesis of dry eye disease(DED)and the possible mechanisms of available effective treatments.
METHOD:Here we employed a non-targeted technology, ultra-high performance liquid chromatography coupled with quadrupole-time-of-flight mass spectrometry(UHPLC-Q-TOF-MS), to investigate metabolic characterizations for tear samples from 18 patients with DED upon drug or acupuncture treatment.
RESULTS:A total of 190 named metabolites were identified, which presented so far the broadest tear metabolome. Further analysis indicated a significantly distinct metabolomics profile among all patients, but very subtle metabolic differences upon drug or acupuncture treatment. On one hand, only six significantly changed metabolites were determined after drug treatment, five of which including inosine, monopalmitin, urate, propionylcarnitine, and nicotinamide were all increased and involved in inflammatory responses. On the other hand, merely four metabolites including alanine, serine, and homoserine were found to be significantly different. Further pathway analysis of those six and four significantly changed metabolites revealed that only one pathway, aminoacyl-tRNA biosynthesis was significantly influenced in acupuncture-treated patients, which were highly associated with the cause of the disease. The results here indicated acupuncture treatment may address the cause rather than the symptoms for dry eye disease, displaying partially better compared with drug treatment.
CONCLUSION: Collectively, this work extended our understanding on the key regulatory elements or pathways involved in the potential mechanisms of available effective treatments, and would be useful for providing novel potential targets and therapeutic strategies for DED.

METHODS:Medical records of 1000 patients(1250 eyes)with obstructive lacrimal duct disease receiving To-bramycin dexamethasone ointment in Nd∶YAG laser lacrimal duct plasty were selected from our hospital during Jan. 2013-Jan. 2017. They were divided into chronic dacryocystitis group(380 eyes),small tears(total)tube obstruction group(360 eyes),naso-lacrimal duct obstruction group (342 eyes),punctal occlusion group (86 eyes) and traumatic canalicular laceration group (82 eyes)according to the lacrimal duct obstruction. In 5 groups, heated Tobramycin and dexamethasone ophthalmic ointment would be loaded in 1 mL syringe and injected into the lacrimal duct via probe, dropping the probe while pushing the ointment. They received lacrimal duct flushing regularly and were followed up for 3 months. Therapeutic efficacies and safty were compared among 5 groups. RESULTS:Cure rate and response rate of patients with chronic dacryocystitis were 85.00% and 87.63%;those of patients with lacrimal small (total) obstruction were 87.78% and 94.45%;those of patients with nasolacrimal duct obstruction were 89.18% and 95.03%;those of patients with punctal occlusion were 84.88% and 94.18%. All of above were significantly higher than 42.68%and 64.63%of patients with traumatic canalicular laceration, with statistical significance(P<0.05). There was no sta-tistical significance in the incidence of ADR among 5 groups(P>0.05). CONCLUSIONS:In the treatment of chronic dacryocysti-tis,small tears(total)tube obstruction, nasolacrimal duct obstruction and punctal occlusion, application of Tobramycin dexametha-sone eye ointment in Nd∶YAG laser lacrimal duct plasty shows good therapeutic efficacy and has less ADR.

ABSTRACT

OBJECTIVE:To compare the efficacy and safety of Sodium hyaluronate versus Carbomer,Polyethylene glycol,To-bramycin and Dexamethasone in the treatment of glaucoma with cataract. METHODS:464 glaucoma patients with cataract were randomly divided into group A(116 cases),group B(116 cases),group C(116 cases)and group D(116 cases). All patients re-ceived intraocular lens implantation;1-1.5 cm to Tramycin and Dexamethasone eye ointment,3-5 times a day. Based on it,group A received 1-2 Sodium hyaluronate eye drops,4 times a day. Group B received 1 Carbomer eye drop,3-5 times a day. Group C re-ceived 1-2 Polyethylene glycol eye drops,3-5 times a day. Group D received Tobramycin and dexamethasone eye ointment,1-1.5 cm every times,1-3 times a day. They treated for 1 month. Clinical efficacy, recovery of visual acuity and ocular surface functions [dry eye symptom score, tear film break-up time(BUT),corneal fluorescein staining(CFS),basal tear secretion(SIt),ocular sur-face disease index(OSDI)] and the incidence of adverse reactions in all groups were observed. RESULTS:The total effective rate sorted, patients with visual acuity higher than 0.6,BUT,OSDI by group A>group B>group C>group D,with significant differenc-es (P0.05). CONCLUSIONS:Sodium hyaluronate shows superior efficacy and safety to Carbomer,Polyethylene glycol,Tobramycin and Dexamethasone in the treatment of glaucoma with cataract,it can significantly improve visual acuity and ocular surface function.
