ABSTRACT

Objective To explore the clinical significance of total serum immunoglobulin ( IgE) , peripheral blood eosinophils ( EOS) and urinary leukotriene E4 ( LTE-4 ) detection on pediatric asthma clinical symptoms of the inacute phase .Methods 90 cases with pediatric asthma clinical symptoms of the inacute phase from March 2014 to March 2015 were selected as experimental group, and divided into the general asthma group(n=32), the moderate asthma group(n=31), the sereval asthma group(n=27) and another 30 health children as control group.Give each patients children doses of β2 agonists (LABA) as monotherapy, and inhaled corticosteroids (ICS) used as combination therapy.The use of LABA is 3-4 times/d.After three months' treatment, clinical efficacy were evaluated, and IgE, EOS, LTE-4 content were detected.Results After 3 months' treatment, IgE, EOS, LTE-4 contents in three experiment groups were significantly decreased (P<0.05), improved to the basic condition compared to control groups.The clinical efficacy among three groups had no significant differences.Conclusion IgE, EOS, LTE-4 levels detection of patients with pediatric asthma clinical symptoms of the inacute phase had evaluation significance, can be a good indicator of pediatric asthma health recovery in patients of the inacute phase.

ABSTRACT

Objective To explore the clinical significance of urinary leukotriene E4(LTE4) in children with respiratory syncytial virus(RSV) bronchiolitis.Methods Forty infants with bronchiolitis were enrolled for the identification of RSV-Ag from the nasopharyngeal secretions.Forty cases who were found to be positive of RSV were put into RSV bronchiolitis group.According to the severity of illness,the infants in RSV bronchiolitis group were separated into 2 subgroups:mild illness group (24 cases) and moderate illness group (16 cases).Thirty healthy children were chosen as healthy control group.Urine of the patients in RSV bronchiolitis group was collected in acute phase (on the 1st day) and recovery phase (on the 7th day after the admission).Urine of children in control group was collected .Enzyme-linked immunosorbent assay was used to assay the level of urinary LTE4.Results The levels of LTE4 in healthy control group (M,Q) were 1.28 pmol?mmol-1,1.72 pmol?mmol-1,while the levels of LTE4 in acute phase in RSV bronchiolitis group (M,Q were 30.52 pmol?mmol-1,23.56 pmol?mmol-1,and the levels of LTE4 in recovery phase in RSV bronchiolitis group (M,Q) were 3.98 pmol?mmol-1,3.43 pmol?mmol-1.Urinary LTE4 levels in acute phase in RSV bronchiolitis group were significantly higher than those in healthy control group (P

0.05). Conclusions The LTs synthesis is elevated in RSV bronchiolitis and LTs may participate in the pathogenesis of RSV bronchiolitis. Leukotriene receptor antagonist may play an important role in RSV bronchiolitis. However there still needs further research on the relationship between LTs and eosinophils count in the pathogenesis of RSV bronchiolitis.

ABSTRACT

Objective To investigate the relationship between cysteinyl leukotriene and clinical response to antileukotriene treatment,and to help select a clinical pharmacologic scheme.Methods Seventy-eight cases with acute mild-moderate asthma were treated with montelukast in a four-week trial.Asthmatic symptom score,usage of ?_2 receptor agonist,percentage of eosinophil,serumal IgE concentration,spirometry and urinary leukotriene E_4(uLTE_4)were measured pre- and post-treatment.Logistic analysis was used to access the various clinical parameters correlated with the response to montelukast.Results There were 48 responders and 30 nonresponders.The uLTE_4 level from the responders was higher than that of nonresponders(P0.05).Subjects with a uLTE_4 level of ≥1 200 pg/mL were11.5 times more likely to respond to montelukast than those with

ABSTRACT

BACKGROUND: Cysteinyl leukotrienes are important inflammatory mediators in bronchial asthma because they enhance microvascular permeability, increase nonspecific bronchial hyperresponsiveness, and are potent bronchoconstrictor agents. Leukotriene E4 is a major metabolite of cysteinyl leukotriene excreted in urine and its concentration of urine can be a reliable marker of the overall leukotriene production in the body. OBJECTIVE: To determine whether clinical parameters of bronchial asthma are associated with production of cysteinyl leukotrienes, expressed by urinary leukotriene E4. METHOD: We measured the urinary concentration of leukotriene E4 in bronchial asthma(127 patients) and normal control(15 persons) group by ELISA method. Pulmonary function test, allergic skin test, measurement of peripheral eosinophil count and eosinophil cationic protein were undertaken in bronchial asthma patients. RESULTS: The concentrations of urinary leukotriene E4 were significantly higher in bronchial asthma patients than normal controls (p<0.05). The increased concentrations of leukotriene E4 had a significant correlation with FEV1/FVC or methacholine PC20(p<0.05). However, the concentration of leukotriene E4 did not have any correlation with the presence of atopy, peripheral blood eosinophil count, and eosinophil cationic protein level. CONCLUSION: The concentration of urinary leukotriene E4 that represents the overall leukotriene production in the body is correlated with the degree of airway obstruction and airway hyperresponsiveness. So, the concentration of urinary leukotriene E4 may be used as an adjunctive parameter of airway obstruction and hyperresponsiveness in bronchial asthma.
