ABSTRACT

Background: The relation between the extent and progression of baldness and coronary heart disease is not totally clarified yet. Some study reveals that male patients with male pattern baldness are at higher risk of CVD than those without male pattern baldness. The aim of this study was to find the prevalence of Acute Coronary Syndrome in male patients with or without vertex baldness.Material & Methods:This cross-sectional observational study was conducted in the Department of cardiology, Mymensingh Medical College Hospital, Mymensingh from October 2014 to September 2015. A total 100 male patients with or without vertex baldness were included in this study from male patients with a first lack of acute coronary syndrome admitted in the coronary care unit (CCL) of MMCH within 24 hours of onset of symptoms. Results:Mean age was found 45.3±7.2 years in group A and 44.5±6.7 years in group B. The mean age difference was not significant (>0.05). STEMI and NSTEMI were higher in group A than group B on contrast UA was higher in group B than group A. The total mean BMI was observed 22.8±3.0 kg/m2. The mean difference was statistically insignificant in both groups (p=0.31). The mean waist circumference was observed 89.4±8.7 vas 84.5±8.4 cm in group A and group B respectively. The mean was higher in group A than group B with statistically significantly difference (p=0.005). Biochemical status of the study that the mean FBS, total cholesterol, triglyceride and HDL cholesterol level were statistically significant between two groups (p=0.01), but the mean difference of LDL cholesterol was not statistically significant between the two groups (p=0.20).Conclusion:The patients with vertex baldness or male pattern baldness are associated with more risk of CVD than without vertex baldness. Vertex baldness may be a marker for Acute Coronary Syndrome.

ABSTRACT

The present paper endeavors to throw light on the factors that stimulates an individual to commit fraud in the light of the fraud triangle theory. Based on the existing theories and literature, an attempt has been made to recognize the red-?ags of fraud risk related to an individual's behavioral approach. The paper further suggests that of the other elements, a check on the Opportunity vertex of the triangle can control the intensity of the fraud. It also highlights the applicability of SAS-99 and COSO framework for deterring fraud constituents from the system

An example of couple therapy illustrates unchallenging possibilities of clinical handling, based on the analyst's clinical-theoretical options. Departing from the distinction between method, technique and frame, different handlings are justified in a session, always regarding the psychoanalytic vertex. The use of WhatsApp, suggesting a topic, asking, explaining the couple a situation, are discussed aspects, considering psychoanalysis and its boundaries. The couple's story, the biography of each one, the personality types based on anxieties of castration, fragmentation and separation are guiding elements for brief psychoanalytic therapy as advocated by NAPC- Núcleo de atendimento e pesquisa de conjugalidade e da família - from Instituto Sedes Sapieantiae's clinic.

ABSTRACT

@#AIM: To study the postoperative optical zone decentration and visual quality by taking visual axis corneal reflect point(VACRP)and corneal vertex(CV)as the ablation center, exploring the femtosecond laser small incision lenticule extraction(SMILE).<p>METHODS: Prospective randomized controlled trial. Totally 70 myopic patients(140 eyes)who underwent SMILE surgery in our hospital from May to June 2020 were randomly divided into two groups, 68 eyes of 34 cases took the VACRP as the ablation center(VACRP group), and 72 eyes of 36 cases took the CV as the ablation center(CV group). The visual acuity, refractive diopter, offset from corneal ablation center, and high-order corneal aberrations were observed before and 3mo after surgery. <p>RESULTS: Three months after operation, there was no difference in uncorrected visual acuity, best corrected visual acuity and refractive diopter between the two groups(<i>P</i>>0.05). The ablation center deviation in CV group(0.20±0.13mm)was less than that in VACRP group(0.27±0.14mm, <i>P</i><0.01). The total corneal high-order aberration(totHOA), spherical aberration(totZ40), vertical coma(totZ3-1)and horizontal coma(totZ31)in CV group were lower than VACRP group(<i>P</i><0.05). Three months after operation, the totHOA, total high-order aberration change(ΔtotHOA), totZ40, totZ3-1 in VACRP group were correlated with ablation center deviation(<i>r</i>=0.470, 0.486, 0.254, -0.366, <i>P</i><0.001, =0.001, 0.037, 0.002), totZ31 in CV group was correlated with the ablation center deviation(<i>r</i>=-0.352, <i>P</i>=0.002).<p>CONCLUSION: SMILE surgery can obtain satisfactory uncorrected visual acuity and the same level of refractive diopter with the VACRP and the CV as the ablation center, but taking the CV as the ablation center can reduce the postoperative decentration and high-order corneal aberrations and obtain better visual quality.

ABSTRACT

Objective The tests of three types of auditory evoked potentials (AEPs) were performed on normal young adults, to understand the frequency characteristics of different testing methods and the relationship between response threshold and pure tone audiometry threshold of different methods, and to discuss the forensic value of 3 types of AEPs to evaluate hearing function. Methods Twenty normal young adults were selected, their standard pure tone audiometry threshold, short-term pure tone audiometry threshold and the response threshold of 3 types of AEPs (tone burst-auditory brainstem response, 40 Hz auditory event-related potential and slow vertex response) at 0.5 kHz, 1.0 kHz, 2.0 kHz and 4.0 kHz were recorded. The relationship between the response threshold and standard pure tone audiometry threshold, short-term pure tone audiometry threshold of 3 types of AEPs at different frequencies as well as the differences between different types of AEPs were analyzed. Results The short-term pure tone audiometry threshold was higher than the standard pure tone audiometry threshold at each frequency. The response threshold and standard pure tone audiometry threshold of the 3 types of AEPs all had a certain correlation, and the response threshold of the 3 types of AEPs was higher than short-term pure tone audiometry threshold and standard pure tone audiometry threshold at each frequency. The differences in the differences between the response threshold and standard pure tone audiometry threshold of the 3 types of AEPs at different frequencies had statistical significance. Linear regression mathematical models were established to infer the standard pure tone audiometry threshold (hearing level) from response threshold (sound pressure level) of 3 types of AEPs of normal young adults. Conclusion When using response threshold of different types of AEPs to estimate pure tone audiometry threshold, conversion and correction are needed. Combined use of different types of AEPs could improve the accuracy of hearing function evaluation.

ABSTRACT

Vertex epidural hematomas (VEHs) are a special clinical entity due to their clinical presentation, vascular etiology and options of surgical approach. The clinical suspicion involves recognizing the mechanism of the injury and the correct visualization of the hematoma in computed tomography (CT) coronal sequences. In the present article, we describe a case of a very large (146 mL) VEH with central brain herniation, and provide a technical note on the surgical planning and treatment. A 34-year-old male patient was admitted to the hospital after an injury on the left superior parietal region. The Glasgow coma scale score was 6, and the left pupil of the patient was dilated. The CT scan showed a large epidural hematoma on the vertex between the coronal e lambdoid sutures, and a fracture over the sagittal suture. During the surgery, multiple burr holes were made laterally to the sagittal suture, and after inspection and no visualization of bleeding in the superior sagittal sinus (SSS), we performed a standard biparietal craniotomy. The patient was discharged three days after the surgery without any deficits. Currently, with the improvement in imaging modalities, more cases of VEH are being identified. Identifying the etiology prior to the craniotomy is challenging in severe cases. Tears in the SSS can bleed profusely, and they demand strategies during the craniotomy. With multiple burr holes parallel to the sagittal suture, we can visualize whether there is bleeding in the SSS and design a craniotomy with or without a central osseous bridge to anchor the dura. Neurosurgeons must be prepared to plan a surgical strategy in cases of large VEHs. Due to its rare frequency and bleeding risks, VEHs are considered challenging.

ABSTRACT

Objective@#To investigate the electroclinical findings in epilepsy children with epileptic negative myoclonus (ENM) restricted to the lower limb as the first seizure type.@*Methods@#Each retrieved electroencephalogram record performed between March 2011 and March 2018 at the Department of Pediatrics of Peking University First Hospital was searched with "midline" . There were 302 records of 175 patients with "benign" or "functional" midline spikes. A retrospective review of each patient′s hospital record was performed. Thirteen patients had ENM restricted to the lower limb as the first seizure type. The clinical and electroencephalogram characteristics of them were analyzed.@*Results@#Thirteen patients manifested ENM restricted to the lower limb as the first seizure type, diagnosed as benign childhood focal epilepsy with vertex spikes (BEVS). Six patients had ENM as the first and only seizure type during the short-time follow-up. Among them, there were 1 male and 5 females. The age at seizure onset was (2.5±0.7) years. One of them had electrical status epilepticus during sleep (ESES) identified on electroencephalogram at theage of 4 years and 8 months. The last follow-up age was (3.8±1.5) years. The remaining 7 patients developed nocturnal focal motor seizures. Among them, there were 4 males and 3 females. The age at seizure onset was (3.5±0.7) years. Two of them were diagnosed as BEVS evolving into benign childhood epilepsy with centrotemporal spikes (BECTS) and 5 were diagnosed as BEVS concurring with BECTS. The age at focal seizures was (4.1±0.6) years. The interval ranged from 1 month to 1 years. Six of 7 patients had electrical ESES with the age of (5.2±1.0) years. All had developmental regression, further diagnosed as atypical benign partial epilepsy (ABPE). The median age at last follow-up was 5.9 years. Five of 13 patients had repeated electroencephalogram records at our apartment, showing that epileptiform discharges in midline regions were significantly reduced either in frequency or amplitude with the improvement of ENM restricted to the lower limb and that independent epileptiform discharges in Rolandic regions from midline regions were noticed with the onset of nocturnal focal seizures.@*Conclusions@#ENM restricted to the lower limb has a close association with vertex (midline) epileptiform discharges. ENM restricted to the lower limb as the first seizure type is a peculiar phenomenon of BEVS. Some patients could evolve into BECTS or overlap with BECTS, and further into ABPE. The age of seizure onset in BEVS with ENM restricted to the lower limb as the first symptom is a little earlier than in BECTS. Ignorance of the close association between midline spikes and ENM restricted to the lower limb may lead to misdiagnosis of these patients.

ABSTRACT

Objective To studythe cephalocaudal relationship ofabdominal aortic bifurcation relative toumbilicus and iliac crest vertex and their correlations with abdominal adipose tissue thickness and age. Methods The vertical distances, cephalocaudal relationship and other related anatomic parameters of aortic bifurcation relative to umbilicus and iliac crest vertex in 108 patientswere measured by consecutive abdominal CT scanning. The correlations of the acquired data with abdominal adipose tissue thickness and age were analyzed using Pearson correlation coefficient. Results Umbilicus was located at cephalad to aortic bifurcation in 67 patients(62.0%), caudal in 34(31.4%)andthe same level in 7(6.5%),with the vertical distance of(4.53 ± 17.51)mm to the aortic bifurcation. No statistically significant relationship was found between abdominal adipose tissue thickness(P>0.05) or age(P>0.05). Iliac crest vertex relative to aortic bifurcationwas located at cephalad, caudal and the same level in 31,71 and 6 patients, taking up 28.7%,65.7%and 5.6%,respectively. Its vertical distance to the bifurcation was(-6.34 ± 14.49)mm,nonrelated with abdominal adipose tissue thickness(P>0.05),but positively correlated with age(P<0.01). The difference in the cephalocaudal relationship of aortic bifurcation relative to umbilicus and iliac crest vertex was statistically significant(P<0.01). Conclusion Compared with iliac crest vertex, umbilicus is an important landmark of locating abdominal aortic terminal occlusion position in vitro because it mostly lies cephalad to aortic bifurcation in the front of the body, not easy to vary with abdominal adipose tissue thickness and age.

ABSTRACT

The Langerhans cells hystiocitosis is a Granulomatous ill, it has unknown etiology today. This disease can be many places and clinical manifestations, although some authors consider it a neoplasic proliferation of the Langerhans cells which can be on nodular region scenery or not. X Hystiocitosis include three diseases: Eosinophilic Granuloma, Hand-Schuller- Chritian disease and Letterer-Siwe syndrome. Eosinophilic Granuloma is a very uncommon benign bone tumor, which may be present in more than 90 percent in childhood under 10 years old. They have predominance on male patients. Usually it is found on long and flat bones. The case report is about 50 years old, female patient, with allergic history and bronchial asthma II degree, she come to the hospital because she don’t filled well since 8 months ago and she complained mild headache, furthermore she noticed irregularity over her skull, on vertex region, at her admission she was screening with cranial X ray, where was found a radio lucid images over the painful area, moreover it had irregular edge whit more than 10 cm long. After that she was screening on CT-scan, where was found a very large osteolytic lesion without intracranial grow. She was approached with craniectomy and graft of surgical cement. She was diagnosed Eosinophilic Granuloma on pathologic department. After her discharge she was treated by oncology, here she received holocraneal radiotherapy. Her outcome has been satisfactory because she is asymptomatic and her CT-scan showed not evidences of recidivism.

ABSTRACT

Objective To develop a registration method for 3D human atlas models by using geometric information of the vertices so as to lay a foundation for statistical modeling of atlas. Methods Based on CT images of the normal human, thirty 3D models of human atlases were created and marked by the manual selected points, including 1 standard module, 20 training sets and 9 testing samples. The training sets were first registered with the standard module, including calculation on geometric information of the individual vertex and optimization process of weight coefficients in the registration models. By minimizing the energy function defined with the Euclidean distances between the automatic registered points and the manual selected points in training sets, the optimized weight coefficients could be obtained. The testing samples were then registered with the standard module to calculate the Euclidean distances between the automatic registered points and the manual selected points. The results were then compared with the training sets to evaluate the stability of the registration method. Results The registration function and the corresponding optimized weight coefficients were obtained, and the average errors for the training sets and testing samples were 1.983 mm and 2.045 mm, respectively. Further statistical analysis showed that there were no obvious differences in the error distributions among the training sets and testing samples. Conclusions The accuracy and stability of the proposed registration method meet the requirement in medical applications, and it can provide automatic registration of points of interest on human atlas models and be used for element classification in statistical modeling.

ABSTRACT

The present study aims to evaluate the auditory sensory process in the brainstem, thalamocortical and cortical areas by using auditory evoked potentials [auditory brainstem response (ABR), mid latency response (MLR) and slow vertex response (SVR)], cognitive functions by P300 and motor response by reaction time in children with poor academic performance. Thirty children between 6–12 years of age were selected as subjects on the basis of poor academic school records. While thirty children with good academic performance served as controls. The recordings were done using a computerized evoked potential recorder by 10–20 electrode placement system. There was no difference in the anthropometric parameters and IQ of the two groups. There was a significant increase in latency of waves II, III, IV and V, and Inter-peak latency I-V of ABR in poor performer females. All the component waves of MLR and SVR showed increased latency in the subjects but could not reach the level of significance. There was a significant increase in latencies of P300 at Cz and Pz electrode positions with no change in amplitude in poor performer females. The reaction time was also increased in the poor performer females as compared to the controls. The latencies of all the waves of ABR, P300 and reaction time are also increased in male poor performers as compared to male controls but could not reach the level of significance. The conduction of impulses is slower in pontine and midbrain auditory pathway along with inefficient cortical processing of task relevant stimuli and motor response in female children having poor academic performance.

ABSTRACT

Morphea is a subclass of localized scleroderma, and this is a connective tissue disorder that is limited to the skin and subcutaneous tissue. We report here on a case of a 43-year-old woman who presented with a solitary skin colored plaque in the vertex area. The histopathologic findings showed basal hydrophic changes of the basal cells, multiple melanophages in the upper dermis and broadened, compact, eosinophilic collagen fibers in the entire dermis. We finally made the diagnosis of morphea in the vertex, and the vertex is an unusual site for this disease.

ABSTRACT

OBJECTIVE: The purposes of this study were to estimate the success rate of vaginal delivery after trial of labor (TOL) and to analyze the neonatal outcome of vertex-vertex (V-V) and vertex-nonvertex (V-NV) second twin according to the mode of delivery. METHODS: We reviewed retrospectively the medical records of V-V and V-NV twin delivered between December 1996 and February 2006. The patients were classified as TOL group and elective cesarean delivery (ECD) group to compare of the neonatal morbidity and mortality in second twin. Neonatal morbidity included intraventricular hemorrhage, respiratory distress syndrome, disseminated intravascular coagulopathy, sepsis, necrotizing enterocolitis, and birth trauma. Student t-test, Mann-Whtiney U test, Pearson's chi-square, and Fisher's exact were performed for the comparison of the neonatal outcome in second twin according to the groups. RESULTS: There are 349 eligible cases within given period. The proportions of TOL and ECD were 49% (n=170) and 51% (n=179), respectively. The success rates of vaginal delivery after TOL were 75% (n=93) in V-V twin and 70% (n=32) in V-NV twin. There were no significant differences in the neonatal outcome between TOL and ECD group. Additionally there were no significant differences in the neonatal outcome between cesarean delivery after the failure of TOL (n=45) and ECD group. CONCLUSION: Our results suggest that TOL in V-V and V-NV twin may be a safe method and can reduce the rate of ECD without adverse effect on neonatal outcome of second twin unless there are other obstetrical indications for cesarean delivery.

ABSTRACT

We report a case of warty dyskeratoma in a 59-year-old man. For 10 years, he had had an asymptomatic solitary 0.5x0.5 cm sized, erythematous, hard papule with a yellowish, central keratotic plug on the vertex. Histological findings showed a large cup-shaped, keratin-filled invagination of acanthotic epidermis, and a suprabasal cleft formation with corps ronds, grains and numerous villi at the lower portion of the invagination, the characteristic features of warty dyskeratoma.

ABSTRACT

Rudimentary meningocele is a variant of primary cutaneous meningioma and a very rare ectopic meningothelial hamartoma usually found in the subcutis. The prognosis is not so poor as long as it is not associated with major skeletal anomalies and intracranial communication through the bony defect. A 7-year-old male presented a normal-skin appearing, soft, indolent, slightly elevated nodule with alopecia on the vertex, measuring about 1.0X1.0X0.4cm. There was no physical or neurologic defect. The extracranial mass had no intracranial connection although minimal skull bone defect in the sagittal sinus was detected by the roentgenogram. The nodule was removed by excision and there has been no relapse during a period of one-year follow-up.
