ABSTRACT

【Objective】 To analyze the changes in antibody titer of voluntary blood donors after receiving the COVID-19 vaccine, so as to provide reference for blood donation strategy, follow-up vaccine development and COVID-19 prevention and treatment for healthy people. 【Methods】 1) The serum from voluntary blood donors 2, 4, 8, 12, 16 and 20 weeks after inoculation with two-shot vaccines (inactivated vaccine or recombinant protein vaccine) was collected, and SARS-CoV-2 total antibody (IgG+ IgM+ IgA) detection (colloidal gold method) and neutralizing antibody detection (UPT immunoluminescence method) were conducted. 2) The obtained data were grouped according to collection time, age and gender, and differences between groups were analyzed by t test and ANOVA using SPSS 2.0 statistical software to clarify the trend of total antibodies and neutralizing antibodies. 【Results】 Neutralizing antibodies and total antibodies (IgG+ IgM+ IgA) from voluntary blood donors vaccinated inactivated vaccine or recombinant vaccine had the same trend of change, both reached their peak at the 2nd and 4th week, respectively, after inoculation, and then decreased gradually. The antibody produced by the recombinant protein vaccine had a higher titer than that from inactivated vaccine, and had slower decline and more lasting protection. The neutralizing antibody and total antibody (IgG+ IgM+ IgA) from different genders and ages were not statistically different. 【Conclusion】 Neutralizing antibodies reached its peak in the second week after vaccination, and total antibody titer reached its peak in the fourth week; both were independent of age and gender. After receiving the vaccine, voluntary blood donors should follow the latest instructions on blood donation intervals issued by the government.

ABSTRACT

【Objective】 To study the characteristics of HIV infection among voluntary blood donors in Shijiazhuang from 2011 to 2021, so as to provide reference for decision making in the control of HIV for blood centers. 【Methods】 The confirmatory results of HIV reactive samples in initial screening among voluntary blood donors from 2011 to 2021 in our center were statistical analyzed. 【Results】 A total of 2 008 299 samples from 1 667 315 blood donors were detected, among which 3 217 samples were HIV reactive and 234 were confirmed positive, with the positive rate at 11.65/100 000 and the prevalence of 14.03/100 000. The prevalence in men was higher than that in women (16.52/100 000 vs 1.39/100 000), in first-time blood donors higher than that in repeated donors (17.27/100 000 vs 8.12/100 000), in whole blood donors higher than that in plateletpheresis donors (12.01/100 000 vs 8.41/100 000), and the differences were statistically significant (P<0.05). The male homosexual transmission was the main routes of transmission, accounting for 62.39% (146/234). And 72% of double-reagent reactive samples were confirmed positive. Four samples were screened in the serological window period and 6 samples were from HIV positive confirmed donors. 【Conclusion】 The HIV prevalence among voluntary blood donors in Shijiazhuang was low. A certain percentage of repeated blood donors got newly infected. NAT could shorten the detection window period of HIV. Since some confirmed positive samples had not been detected out by NAT, publicity and education should be strengthened to reduce the probability of infected or high-risk groups to participate in blood donation.

ABSTRACT

【Objective】 To evaluate the viability of nucleic acid test(NAT) of human parvovirus B19 (HPV B19), hepatitis A virus (HAV) and hepatitis E virus (HEV) along with routine NAT for voluntary blood donors, and to analyze HPV B19, HAV and HEV prevalence in Nantong, so as to provide reference for rational blood screening programs. 【Methods】 HPV B19 DNA, HAV RNA and HEV RNA of blood donors in Nantong from November 2021 to May 2022 were detected using NAT, and serological antibody testing was performed on NAT reactive samples. 【Results】 Three HPV B19 DNA was yielded out of 3 440 blood donors, with a positive rate of 0.09%, among which 2 were negative for HPV B19-IgM and 1 was undetermined due to insufficient sample size. HAV RNA and HEV RNA were not detected in 3 440 blood donors. HPV B19, HAV, and HEV NAT were conducted simultaneously with routine HBV, HCV and HIV screening, prolonging the test reports by 20 minutes. 【Conclusion】 Although the HPV B19 DNA, HAV RNA and HEV RNA prevalence among voluntary blood donors in Nantong is low, the risk of transfusion transmitted infection still exsits and can be reduced by NAT.

ABSTRACT

【Objective】 To explore the population characteristics, the reasons for deferral and the changing trend of unqualified rate of blood test of voluntary blood donors in Xi′an, so as to provide a data support for recruiting low-risk regular blood donors, reducing unqualified rate, improving blood safety and blood quality. 【Methods】 The age, gender, occupation, education and unqualified blood test of voluntary blood donors in Xi′an area from 2012 to 2021 were classified and analyzed retrospectively. 【Results】 The number of voluntary blood donors in Xi′an increased year by year from 2012 to 2021, while the overall unqualified rate of blood test kept decreasing, with an overall unqualified rate at 2.36%, which was at a middle level in China. There were more male blood donors than female donors, and the highest proportion of blood donors in terms of age, occupation and education was between 21-30 years old group (40.19%), other occupation group(21.75%), and undergraduate and above group (32.61%) respectively. The total unqualified rate was ALT (0.68%)>HBsAg (0.55%) > anti -HCV (0.48%) > anti -TP (0.43%) > anti -HIV (0.17%). The unqualified rate of blood donors aged 31 to 40 was the highest. With the increase of academic qualifications, the unqualified rate generally showed a gradual downward trend. The unqualified rate of soldiers, teachers, civil servants and medical workers was low, while the unqualified rates of various items of students were high. There were seasonal differences in the total number of blood donors, the total unqualified rate and the unqualified rate of each item (except anti -HIV). 【Conclusion】 The voluntary blood donors are mainly males, aged 21 to 30, with other occupations and with bachelor degree or above. Regular voluntary blood donation teams in this area should be established among soldiers, teachers, civil servants, medical workers and highly educated groups, and more attention should be paid to health consultation for workers, farmers and students before blood donation.

ABSTRACT

Objective To investigate the infection of human parvovirus B19 in Suzhou voluntary blood donors under the current blood screening model. Methods A total of 893 blood donor samples from September to December 2022 were randomly collected. Samples were tested to determine the seroprevalence (anti-B19 IgG and IgM) of B19 antibodies by enzyme-linked immunosorbent assay(ELISA), and B19 DNA of positive samples was further detected by real-time polymerase chain reaction(PCR) assay. Results Among 893 samples, the total seroprevalence of B19 antibody was 20.7% (185/893), with anti-B19 IgG and IgM positive rate at 19.4% (173/893) and 1.9% (17/893), respectively, showing significant difference (P0.05). The prevalence of anti-B19 IgG statistically increased with age (P0.05). No statistical difference was not found in anti-B19 IgG and IgM samples among different blood groups. The anti-B19 IgG in repeated blood donors was higher than that in first-time donors(21.5% vs 15.9%)(P0.05). Three cases were found to be positive for B19 DNA in the B19 antibody positive samples, with the positive rate at 1.6%(3/185). Conclusion Although the prevalence of B19 infection in Suzhou was lower than that in other areas and was mostly past infection, there was still a certain proportion of persistent infection and acute infection, which posed the potential risk of blood transfusion transmission. Therefore, attention should be paid to blood transfusions, especially for the high-risk and susceptible groups.

ABSTRACT

【Objective】 To explore the optimization of blood testing strategies for voluntary blood donors in military blood centers. 【Methods】 From January 2017 to December 2020, the testing results of five serological indicators of voluntary blood donors from our center were collected via the blood transfusion management system . The positive NAT results of HBV, HCV and HIV, as well as the unqualified serological indicators, including the unqualified rate over the years, unqualified rate of military/civilian blood donor, reactive rate of dual-ELISA-reagent or one-ELISA-reagent etc., were retrospectively analyzed. 【Results】 The ratio of military blood donors to civilian blood donors in our center was 1: 4 during 2017~2020. The total unqualified rate of blood testing indicators of military blood donors was ALT>anti-HCV>HBsAg>HIV antigen/anti-HIV>anti-TP>NAT, and that of civilian blood donors was HBsAg > anti-TP > ALT > anti-HCV > HIV antigen/anti-HIV > NAT. The total unqualified rate of HBsAg, anti-HCV and anti-TP in our center decreased year by year, the HIV antigen/anti-HIV fluctuated, and the ALT increased year by year (P<0.01). The ALT deferral rate among military blood donors was the highest, accounting for 65.85%(594/902) of the donation deferral. The reactive rate of dual-ELISA-reagent to HBsAg, anti-HCV, HIV antigen/anti-HIV and anti-TP in civilian blood donors were 72.87%, 33.02%, 16.56% and 67.67%, respectively (P<0.05). Among the 123 668 ELISA negative samples during 2017~2020, 56 cases of HBV DNA(+ ) were detected by NAT, including 3 cases of military blood donors. 【Conclusion】 The current blood screening strategy of military blood centers can basically guarantee the safety of blood use in clinical. For military blood donors, ALT deferral, caused by ALT results that are higher than the ALT threshold, was the main reason for blood scrapping. Strengthening the promotion and implementation of NAT, as well as adjusting the critical value of ALT is of positive significance for reducing the risk of blood transfusion-transmitted diseases and the waste of blood sources, thereby improving the safety and security level of the clinical blood supply for the troops, promoting the treatment capabilities of officers and soldiers, and even improving the combat effectiveness of the troops.

ABSTRACT

【Objective】 To investigate the distribution of Hepatitis B virus(HBV)genotypes and the genetic characteristics of genotype B HBV populations among voluntary blood donors in five regions of China. 【Methods】 A total number of 630 plasma samples from blood donors with positive HBV HBsAg neutralization test from 2014 to 2016 in Guangxi Blood Center, Chongqing Blood Center, Urumqi Blood Center, Mianyang Central Blood Station and Luoyang Central Blood Station were collected. The S-region sequence of the HBV genome was amplified by semi-nested PCR and followed with Sanger sequencing in order to investigate the HBV genotype distribution and population genetics. 【Results】 Among the voluntary blood donors in five regions, 55 cases of HBV S gene fragments were successfully amplified. Three genotypes were found in HBV typing, including 46 cases of type B(83.64%), 7 cases of type C(12.73%) and 2 cases of type D(3.63%). There were 15 cases of type B and 3 cases of type C in Guangxi; 10 cases of type B and 1 case of type C in Chongqing; 3 cases of type B, 1 case of type C and 1 case of type D in Luoyang; 15 cases of type B in Mianyang; 3 cases of type B, 2 cases of type C and 1 case of type D in Urumqi. The mismatch distribution of the HBVB type population with the largest population number showed a unimodal distribution, and the results of Tajima′s D test and Fu′s Fs test were both negative, indicating that the HBV population in these five regions was expanding. 【Conclusion】 The prevalent genotype of HBV in voluntary blood donors is type B, and the type B HBV population is experiencing a slow expansion, which should attract our attention. In the future, a thorough molecular epidemiological investigation of HBV should be carried out to ensure blood safety.

ABSTRACT

【Objective】 To study the characteristics of voluntary apheresis platelet donors in Zhoushan Islands and its influence on the collection and supply of apheresis platelets. 【Methods】 From January 1, 2017 to December 31, 2021, 1038 eligible donors were selected out of 1151 apheresis candidates for demographic analysis, donation time analysis, blood routine test. We also investigated whether the current supply of apheresis platelet met the needs of clinical. The lapsed donors(with only one donation) were telephone interviewed to investigate the lapsing reasons. 【Results】 From 2017 to 2021, the median (M) ratio of platelet donors to the population in Zhoushan City was 0.454 ‰. And 90.18% (1038/1151) of the candidates completed donation, involving 2659 donations and 3205 U products. The per capita donation amount was (1.29±0.77) U per person, which basically met the blood demand in clinical of Zhoushan Islands. The 1038 platelet donors were 18 to 59 yeas old[M(QR) 34(16)], and 47.50% (493/1038) of them aged 31 to 45; the ratio of male to female blood donors was 2.0∶1; 70.61% (733/1038) had college education or above; ABO blood group profile was A>O>B>AB. There were several peak periods of donation during 2017~2021: the second and third quarters of 2017~2019 and the third quarter of 2020; none in 2021, as yeae 2021 was volatile. The donation proportion of the first quarter was the lowest throughout 2017~2021. In 2017 and 2018, the proportion of donors with one donation accounted for 79.79% (75/94) and 51.63% (95/184), respectively. From 2019 to 2021, the proportion of donors with twice or more donation accounted for 60.35% (137/227), 57.36% (187/326) and 53.13% (170/320) , respectively. During 5 years, 10.88% (113/1051) of the candidates failed to donate platelets, of whom 72.57% (82/113) were due to unqualified pre-donation testing. 【Conclusion】 The donation behaviors in Zhoushan have been affected by each above-mentioned demographic factors. The awareness of donation and donation services in Zhoushan have been improved as combing the special geographic condition and demographic characteristics together, and the donation units is elevating yearly which has met the clinical needs of the whole island.

ABSTRACT

【Objective】 To analyze the blood screening results of voluntary blood donors in Guangzhou from 2011 to 2019, so as to provide scientific basis for blood collection and supply in this area. 【Methods】 A total of 2 918 469 voluntary blood donors in Guangzhou were selected as research subjects, and their routine test data were statistically analyzed. 【Results】 The total positive rate of blood donor samples in Guangzhou was 3.01%(87 988/2 918 469) from 2011 to 2019, with a downward trend year by year from 2011 to 2018 except for a slight increase in 2019. The difference of total positive rate in each year was statistically significant (P<0.05). The ELISA-yielding rate(1.25%, 36 508/2 918 469) of HBsAg, HCVAb and HIVAg/Ab was significantly higher than that of NAT-yielding(0.62%, 18 086/2 918 469)(P<0.05). In terms of annual positive rate of various tests, ALT was the highest (1.28%, 37 451/2 918 469), followed by HBsAg (0.82%, 23 827/2 918 469), and NAT (0.62%, 8 086/2 918 469), anti-TP (0.39%, 11 468/2 918 469), anti-HCV (0.31%, 9 155/2 918 469), HIVAg/Ab(0.12%, 3 526/2 918 469) and anti-HTLV (0.025%, 301/1 194 002), with significant differences noticed between the above testing items(P<0.05). And 0.20% (5 947/2 918 469) of the samples were ELISA(-)/NAT(+ ), among which 30.02%(1 785/5 947)were discriminated as positive, including 99.38% (1 774/1 785) HBV positive, 0.28%(5/1 785) HCV positive, and 0.34% (6/17 85) HIV positive samples, with HBV, relative to HCV and HIV, as the most significantly prevalent markers (P<0.05). 【Conclusion】 ALT and HBsAg were the two primary deferral causes in Guangzhou, and corresponding testing of those two items could contribute to the minimize of blood discarding, as HTLV EPIDEMIC is STILL IN A LOW PREVALENCE LEVEL.ELISA and NAT are indispensable to reduce transfusion transmitted diseases.

ABSTRACT

【Objective】 To investigate the age, gender and unqualified blood test of voluntary blood donors, so as to provide a data support for blood donation publicity, recruitment and retainment of blood donors, and to avoid unnecessary waste of blood resources. 【Methods】 The age, gender and unqualified blood test of voluntary blood donors in Lianyungang area from 2017 to 2020 were retrospectively classified and statistically analyzed. 【Results】 The proportion of voluntary blood donors increased with age, and the proportion of blood donation in the 51-60 age group decreased. The unqualified rate of voluntary blood donors showed a downward trend with the increase of age. The unqualified rate of blood test in 51-60 age group was rather low. Except for 18-20 age group, the blood donation ratio and the unqualified rate of blood test of male blood donors in all age groups were higher than those of female blood donors, which were 1.47∶1 and 1.30∶1, respectively. The total unqualified rate of blood test was 1.09%(1 274/117 079), and it decreased year by year. The unqualified item of blood test was ALT>HBV>TP>HCV>HIV, and the unqualified rate of ALT test between male and female blood donors was significantly different(P<0. 05), and the unqualified rate of HBV and TP test in different time periods was significantly different(P<0.05). 【Conclusion】 The publicity of voluntary blood donation for people aged 18-25 and 50-60 should be strengthened, and regular blood donor team should be continuously retained and expanded. Effective consultation and preliminary blood screening before blood donation are of great significance in reducing the unqualified rate of blood test. The unqualified rate of HBV and TP tests can be reduced by scientific preliminary screening process and standardized operation, which is of great significance to reduce the waste of blood resources and ensure blood safety.

【Objective】 To explore the characteristics and prevalence of HBV infection among voluntary blood donors in Chongqing and analyze its epidemiological characteristics, so as to provide reference for effective selection of low-risk blood donors. 【Methods】 The test results of hepatitis B surface antigen (HBsAg) and viral nucleic acid (HBV DNA) of 314 442 voluntary blood donors from October 2017 to October 2020 in Chongqing Blood Center were retrospectively analyzed. The relationship between the total positive rate of HBV, HBsAg+ /HBV DNA-, HBsAg-/HBV DNA+, HBsAg+ /HBV DNA+ and gender, marriage, age, education background, occupation, firs-time blood donation and repeated blood donation was analyzed. 【Results】 Among the 314 442 voluntary blood donors, the total positive rate of HBV, HBsAg+ /HBV DNA-, HBsAg-/HBV DNA+ and HBsAg+ /HBV DNA+ was 0.88% (2 764/314 442), 0.28% (893/314 442), 0.18% (567/314 442) and 0.41% (1 304/314 442), respectively. The total positive rate of HBV, HBsAg-/HBV DNA+ and HBsAg+ /HBV DNA+ was different in gender, marriage, age, education background, occupation and firs-time/repeated blood donation. The positive rate of men was higher than that of women, and that of married higher than that of unmarried, and decreased as the education level elevated. The positive rate of farmers and workers was the highest and that of medical staff was the lowest, followed by students, military personnel, civil servants, and teachers. The positive rate of first-time blood donors was higher than that of repeated blood donors. The differences were statistically significant (P0.05). 【Conclusion】 The prevalence of HBV among voluntary blood donors in Chongqing is relatively high and is related to gender, marriage, age, education background, occupation, and first-time/repeated blood donation. It is necessary to strengthen effective consultation before blood donation, formulate guidelines for blood donation recruitment, select voluntary blood donors from low-risk groups to improve blood safety.

ABSTRACT

【Objective】 To investigate the HIV infection among voluntary blood donors in Jinhua area through the analysis of HIV antibody detection and confirmation results, so as to guarantee the safety of clinical blood use. 【Methods】 419 823 samples of voluntary blood donors in Jinhua Central Blood Station from 2013 to 2020 were detedcted for HIV twice. Samples reactive with one assay were double-well retested. Samples initially reactive with two assays or positive with one retest assay were subjected to the CDC for confirmatory test. The HIV antibody infection and epidemiological characteristics of voluntary blood donors were retrospectively analyzed. 【Results】 A total of 419 823 samples of blood donors were dectected, among which 335 were reactive for HIV screening, 48 confirmed positive, with the positive rate at 0.11‰(48/419 823), of which 31 were registered residence in Jinhua and 17 in other places; 46 males and 2 females; 35~45 years old accounted for 28.23%; freelancers 43.61%; coinfection 4.16%.The positive rates differed significantly by gender (male 0.18 ‰ vs female 0.01 ‰, P0.05). 【Conclusion】 The positive rate of HIV antibody in Jinhua area was lower than that in other areas. Blood stations should continue to carry out ELISA and NAT, strengthen the publicity and education of AIDS prevention and control, and ensure the safety and effectiveness of clinical blood use.

ABSTRACT

【Objective】 To investigate the characteristics of NAT reactive(R) population among voluntary blood donors in Zhengzhou, and analyze the residual risk of NAT and the infection in different ages, educational levels, and occupations. 【Methods】 The samples of voluntary blood donors in Zhengzhou from 2018 to 2019 were comprehensively screened (samples reactive by duplicate ELISA reagents excluded), and the occupation, gender, education level and marital status of voluntary blood donors were analyzed. 【Results】 A total of 488 365 samples were detected, 323 were NAT R samples (0.66‰), including 318 HBV R samples (0.65‰) and 5 HIV R samples (0.01‰), but no HCV R was detected. Statistical analysis showed that NAT R rate in male voluntary blood donors was 0.78‰ (252/321 196), higher than 0.42‰ (71/167 439) in female (P0.05). 【Conclusion】 Some blood samples were negative by ELISA but reactive by NAT. Therefore, NAT is an effective supplement to missed detection of ELISA and can reduce the risk of transfusion transmitted infection diseases. The characteristics of NAT R population in Zhengzhou are worth studying, which can provide reference for health consultation and further improve blood safety.

ABSTRACT

【Objective】 To investigate the prevalence of hepatitis E virus (HEV) among voluntary blood donors, and the viability and practicability of nucleic acid testing (NAT) in Jinhua area. 【Methods】 Blood samples from 1 017 voluntary blood donors at Jinhua Blood Center from April to May 2021 were collected, and HEV RNA was detected by CHURAS BSS1200 blood nucleic acid detection system. The reactive samples were re-tested for IgM and IgG antibodies by ELISA, and HEV genotyping by gene sequencing. 【Results】 The yield rate of HEV RNA out of voluntary blood donors in Jinhua was 0.98 ‰ (1 / 017). The individual was negative for IgM and IgG antibodies, but was confirmed to be HEV RNA genotype 4 by sequencing. 【Conclusion】 HEV RNA positive donors have been found in this study. HEV RNA blood screening can effectively enhance the safety of blood transfusion.
