ABSTRACT

Although pulmonary involvement is the most common extra-articular manifestation of rheumatoid arthritis (RA), traditional pulmonary function tests (PFTs) do not show a good correlation with the field tests usually performed in these patients. In recent decades, measurement of ventilation distribution heterogeneity through the nitrogen single-breath washout (N2SBW) test and evaluation of functional capacity during exercise using the Glittre activities of daily living test (GA-T) have been increasingly used. Therefore, the objective of this study was to evaluate predictors of GA-T outcomes in women with RA considering demographic, anthropometric, clinical, functional variables, and chest computed tomography (CT) findings. Forty-three women with RA underwent the GA-T, the N2SBW test, spirometry, measurement of the diffusing capacity for carbon monoxide (DLco), measurement of respiratory muscle strength, and evaluation of physical function of the lower and upper limbs through the Health Assessment Questionnaire Disability Index (HAQ-DI). Chest CT scans were analyzed retrospectively. The GA-T time showed significant correlations with the DLco (rs=-0.397, P=0.008), forced vital capacity/DLco (rs=0.307, P=0.044), phase III slope of the N2SBW test (SIIIN2, rs=0.644, P<0.0001), and the HAQ-DI (rs=0.482, P=0.001). Disease extent as assessed by chest CT was associated with the GA-T time. On multiple regression analysis, the SIIIN2 and HAQ-DI were the only predictors of the GA-T time, explaining 40% of its variability. Thus, ventilation distribution heterogeneity and worse physical function substantially explain the variability in GA-T time in women with RA and varying extents of disease on chest CT.

ABSTRACT

Objective: To investigate the value of ultrasound-guided fine-needle aspiration (FNA) biopsy combined with thyroglobulin in fine-needle aspirate fluid (FNA-Tg) measurement in diagnosis of preoperative lateral neck lymph node (LN) metastasis of papillary thyroid carcinoma (PTC). Methods: Totally 111 preoperative PTC patients diagnosed with FNA with suspected metastatic LNs were collected and then underwent neck ultrasonography together with FNA cytclogy. FNA-Tg was measured, then the eluents were obtained by flushing aspiration needle with 1 ml normal saline. FNA-Tg of elution was measured by electrochemiluminescence immunoassay. Taken postoperative pathologic results as the gold standards, ROC curve was drawn, the optimal threshold value for diagnosis of FNA-Tg, the diagnostic efficiency of FNA, FNA-Tg, combined FNA and FNA-Tg for suspected metastatic LNs were evaluated. Results: Among 118 suspicious LNs, 81 (81/118, 68.64%) LNs of metastasis and 37 (37/118, 31.36%) LNs without metastasis were confirmed by postoperative pathology. The average value of FNA-Tg in metastasis LNs was significantly higher than that of LNs without metastasis (P<0.001). The optimal diagnostic value of FNA-Tg in diagnosis of LNs metastasis was 2.65 μg/L, and the AUC was 0.937 (95%CI [0.894, 0.980], P<0.001). The differences of accuracy, sensitivity and negative predictive value were all statistically significant between FNA-Tg and FNA (all P<0.05). The accuracy, sensitivity and negative predictive value of FNA combined with FNA-Tg were higher than that of only FNA (all P<0.05). Twenty-two FNA false-negative nodes were correctly diagnosed with FNA-Tg testing. Conclusion: FNA cytology and FNA-Tg detection are useful ancillary tests that can improve the detection of lateral neck LN metastasis of PTC. Combination of FNA and FNA-Tg can significantly increase the accuracy and sensitivity of preoperative diagnosis of lateral neck LN metastasis of PTC, and may have an important role in surgical planning of PTC patients.

ABSTRACT

Abstract: The coastal streams of southernmost Brazil, Rio Grande do Sul state, are marked by a period of regular marine intrusion resultant from intense oceanic winds. In the present study we aimed to investigate the species composition, abundance and relative biomass of the ichthyofauna in the lower stretch of a coastal stream during summer, a period of regular marine intrusion. Estreito is a coastal hydrological complex composed by lakes, swamps and a perennial stream, located at the central-south portion of the coastal plain of Rio Grande do Sul state. During the summer of 2018, the ichthyofauna of lower Estreito stream was sampled by beach hauls applied in 17 random points distributed in a stretch of ca. 2km. Measurements at the sampled stretch revealed salinities between 19.3 to 31.3 ppt, characterizing the studied system as polyhaline/euhaline during summer. The sample of 4,533 specimens revealed the occurrence of 20 species, being the great majority marine-dwelling. The most abundant species were the anablepid Jenynsia lineata (70.3%), the cichlid Geophagus brasiliensis (19.3%) and the the mugilid Mugil curema (7.5%). The highest relative biomass was recorded for J. lineata, followed by M. curema and G. brasiliensis. The dominance of marine-dwelling species in the assemblage composition and the high abundance of limnic-estuarine J. lineata corroborate previous studies conducted in other washouts of Rio Grande do Sul.

ABSTRACT

Phenotypic differences have been described between patients with systemic sclerosis (SSc)-associated interstitial lung disease (ILD) and SSc-associated pulmonary hypertension, including performance differences in the 6-min walk test (6MWT). Moreover, the correlations between the 6MWT and traditional pulmonary function tests (PFTs) are weak, indicating the need to search for new parameters that explain exercise performance. Thus, our objective was to evaluate the impact of ventilation distribution heterogeneity assessed by the nitrogen single-breath washout (N2SBW) test and peripheral muscle dysfunction on the exercise capacity in patients with SSc-ILD and limited involvement of the pulmonary parenchyma. In this cross-sectional study, 20 women with SSc-ILD and 20 matched controls underwent PFTs (including spirometry, diffusing capacity for carbon monoxide (DLco), and the N2SBW test) and performed the 6MWT and knee isometric dynamometry. The 6-min walking distance (6MWD, % predicted) was strongly correlated with the phase III slope of the single-breath nitrogen washout (phase III slopeN2SBW) (r=−0.753, P<0.0001) and reasonably correlated with the forced vital capacity (FVC) (r=0.466, P=0.008) and DLco (r=0.398, P=0.011). The peripheral oxygen saturation (SpO2) during exercise was not significantly correlated with any of the pulmonary or muscle function parameters. The phase III slopeN2SBW was the only predictive variable for the 6MWD, whereas quadriceps strength and FVC/DLco were predictive variables for SpO2. Ventilation distribution heterogeneity is one factor that contributes to a lower 6MWD in SSc-ILD patients. In addition, muscle dysfunction and abnormal lung diffusion at least partly explain the decreased SpO2 of these patients.

ABSTRACT

Objective: To investigate the diagnostic value and influential factors of washout fluid thyroglobulin collected during fine-needle aspiration (FNA-Tg) in detecting lymph node metastases of papillary thyroid carcinoma (PTC) before thyroidectomy. Methods:We retrospectively analyzed 131 patients diagnosed with PTC based on histopathology. They presented with suspicious enlarged cervi-cal lymph nodes and underwent high-frequency ultrasound-guided FNA before the surgery. FNA and FNA-Tg were performed simulta-neously. All the related data were collected. In order to obtain the best cut-off value, the FNA-Tg receiver-operating characteristic curve was generated. The cytopathology and postoperative pathologic results, as well as the ultrasound images during the follow-up, were considered the gold standard. The diagnostic performance of each method (FNA, FNA-Tg, and FNA+FNA-Tg) were compared. Ad-ditionally, some suspicious influential factors such as the anatomical location of lymph nodes and associated laboratory indexes were also analyzed for the diagnostic accuracy of FNA-Tg. Results: The best cut-off value of FNA-Tg in our study was 1.295 ng/mL. The diag-nostic performance of the combined method was the best when compared with other methods, with a sensitivity of 96.4% and speci-ficity of 99.2%. Additionally, FNA-Tg was much more accurate when used in diagnosis of lateral cervical lymph nodes. Among all the as-sociated laboratory indexes, the level of serum Tg (sTg) was an independent predictive factor for an FNA-Tg level above 1.295 ng/mL (odds ratio=1.018). Conclusions: FNA-Tg is a useful tool in the identification of metastatic cervical lymph nodes preoperatively, espe-cially for lateral cervical lymph nodes. In addition, 1.295 ng/mL could be one of the reference standards of the FNA-Tg cut-off value. When the sTg level is high, we should interpret the FNA-Tg results cautiously.

ABSTRACT

The rate of lung clearance is a measure of the ventilation inhomogeneity determined during multiple breath washout (MBW). Higher values of lung clearance index (LCI) indicate greater ventilation inhomogeneity. The test is performed during tidal breathing, needs little cooperation and coordination, and can be done in all age groups. Different tracer gases can be used; the most common are gas nitrogen as resident gas and sulfohexafluoride (SF6) as an exogenous gas.LCI has been used in various pediatric diseases, such as asthma, bronchopulmonary dysplasia and especially in patients with cystic fibrosis (CF), where high LCI values had been a more sensitive tool that spirometry to detect the early disease and bronchiectasis. It has also been used to monitor the progression of early lung disease, since the value of LCI in preschoolers predicted FEV1 in children of school age. Finally, in patients older than 6 years LCI has proven to be a useful tool to assess response to interventions in patients with normal lung function test. In conclusion the use of LCI is very promising in CF patients with early stages of the disease, and in the clinical monitoring of patients with CF. Its role in other respiratory diseases such as asthma, is still to be determined.

ABSTRACT

Objective To investigate the MRI findings of the breast fibroadenoma which has a washout type of time signal intensity curve (TIC)for the purpose of improving imaging diagnosis. Methods The MRI findings of 20 cases of the breast fibroadenoma with a washout TIC and 20 cases of breast carcinoma verified by histopathology were analyzed retrospectively.Morphological features, internal signal, ADC value and dynamic enhancement performance of the two groups were compared with each other. Results The shapes of the breast fibroadenomas were more commonly ovoid or round (18/20),and the margins were circumscribed(16/20),Most of the fibroadenomas were high intensity in T2 WI with the non-contrast enhanced separations (9/20).The average minimum ADC value was (1.412±0.332)×10-3 mm2/s,and higher than that of breast cancer (0.888 ±0.1 60)×10 -3 mm2/s with the significant difference (P 0.05 ). Conclusion The breast fibroadenoma with washout TIC has a similar performance with the breast cancer in TIC and the early enhancement rate, however, the clear edge, higher T2 WI signal intensity, the non-contrast enhanced internal separations and higher ADC values are helpful to the diagnosis of breast fibroadenoma with washout TIC.

ABSTRACT

Aim: To characterize incidentally detected adrenal masses by CT attenuation values and 15 minutes wash out characteristics at contrast-enhanced CT. Method : Prospective observational study from March 2012 -2013 was done, with all the abdominal scan evaluated for adrenal masses. Departmental protocol was followed with15 minutes delayed scan added. CT attenuation values at different phases were used to calculate absolute percentage washout (APW) and relative percentage washout (RPW). The masses with attenuation value <10HU on unenhanced scans were diagnosed as lipid rich adenomas and masses with attenuation values >10 on plain scan with APW of >60% and RPW>40% were diagnosed as lipid poor adenomas; rest being nonadenomas, metastasis if there was known malignancy. The masses were evaluated in terms of their sizes, site of involment, stastical significance (p<0.05) in attenuation values at different phases and washout studies. Results: 96 lesions were detected in 84 patients from March 2012 -2013.M:F ratios of 1.7:1, mean age being 46 years. Lipid poor adenomas (LPA) were the commonest mass followed by metastasis. 64% of patients with malignancy had adrenal metastasis while 36% had adenomas. There was no significant difference in size(p=0.23) between LPA and non-adenomas (NA) whereas statistical difference was noted in mean attenuation values on unenhanced, delayed, wash-in and wash-out values(p <0.05).There was no significant difference on enhanced scan (p=0.95),but absolute percentage washout (APW) and relative percentage washout (RPW) was statistically significant between LPA and NA (p<0.05).Conclusion: CT attenuation values, absolute and relative percentage washout values characterize the adrenal masses.

ABSTRACT

Objective To investigate the effect of rectal washout on prevention of local recurrence in patients with resectable rectal cancer. Methods Relevant studies published on PubMed,Embase database and Google between January 1980 and June 2013 were identified and reviewed.A meta-analysis was performed to compute the risk ratio (RR) and evaluate the effect of rectal washout on local recurrence in patients included in the reviewed studies. Results Eight studies were included in the meta-analysis,in which a total of 427 patients underwent rectal washout and 492 did not during rectal cancer resection. The patients had an overall local recurrence (LR) rate of 6.64 % (61/919).Rectal washout,no matter which type of washout solution was used, significantly reduced the risks of local recurrence (3.75 %,16/427 vs 9.15 %,45/492,RR =0.46,95 % CI =0.26-0.82,P =0.008).Patients with rectal washout showed a low LR rate, when compared with those in the three subgroups who underwent total mesorectal excision (TME) (5.18 %,10/193 vs 5.32 %,10/188).However,there was no statistically significant difference in the risk of LR among the three subgroups (RR =0.99,95 % CI =0.42-2.30,P =0.98; RR =0.44,95 % CI =0.18-1.04,P =0.06; RR =0.46,95 % CI =0.20-1.04,P =0.06,respectively).Conclusions The intra-operative rectal washout largely reduces local recurrence after anterior resection of rectal cancer.Rectal washout should be applied routinely during anterior resection.

ABSTRACT

OBJECTIVES: Fast-setting pozzolan cement (Endocem, Maruchi) was recently developed. The aim of this study was to investigate the effects of various root canal irrigants on the washout of Endocem in comparison to the previously marketed mineral trioxide aggregate (ProRoot; Dentsply) in a furcal perforation model. MATERIALS AND METHODS: ProRoot and Endocem were placed into acrylic molds on moist Oasis. Each mold was then immediately exposed to either physiologic saline, 2.5% sodium hypochlorite (NaOCl), or 2% chlorhexidine (CHX) under gentle shaking for five minutes. Washout testing was performed by scoring scanning electron microscope (SEM) images. RESULTS: Endocem exhibited higher washout resistance compared to ProRoot, especially in the NaOCl group. CONCLUSIONS: These results suggest that Endocem can be considered a useful repair material for furcal perforation, especially in a single-visit scenario.

ABSTRACT

Asthma is a chronic inflammatory disorder of the airways causing typical symptoms, and the diagnosis is supported by evidence of airflow obstruction which is variable, reversible or inducible. However, standard assessment of lung function with spirometry does not measure dysfunction in small airways which are < 2 mm in diameter towards the periphery of the lung. These airways make only a small contribution to airway resistance under normal circumstances. Nevertheless, there is mounting evidence that pathology and dysfunction in these small airways are implicated in the pathogenesis and natural history of asthma. Using forced oscillation and the multibreath nitrogen washout techniques, uneven ventilation (ventilation heterogeneity) due to small airways dysfunction has been shown to be an important marker of asthma disease activity, even in the absence of abnormalities in standard spirometric measurements. Recent advances in imaging research, particularly with hyperpolarised gas magnetic resonance imaging, have also given insights into the significance and dynamic nature of ventilation heterogeneity in asthma. The challenge is to integrate these new physiological and imaging insights to further our understanding of asthma and facilitate potential new treatments.

A descriptive and cross-sectional study with one-hundred patients having a diagnosis of degenerative knee arthropathy treated at the Rheumatology Service in Saturnino Lora University Hospital from Santiago de Cuba was carried out between 2003 and 2007 in order to characterize them according to the main clinical and endoscopical aspects of this disorder. Among the most important results, 60-year-old and over patients and female gender were relevant. Gonarthrosis and condromalacy constituted the main endoscopical findings while curettage and articular wash-out, being the therapeutical alternatives most used, were predominant.

ABSTRACT

As part of the technological process optomization to obtain calcium citrate and magnesium from dolomites, we studied at bench scale, the washout stage to decrease the free citric acid content. Washout variants allowed to decrease the free citric acid content under 3 percent, whereas the calcium and magnesium values are similar to those of batch before to carry out the wahout process. On the other hand, toxicity study in rats, proved that the batches where different washout variables were applied not provoke gastric irritability at doses of 2 000 mg/kg of body weight confirming that above mentioned established process is approproate to guarantee the raw material quality.

ABSTRACT

Objective To investigate the influence rule of the atmospheric Rainfall to the γ radiation Kerma rate in surface air in order to revise the resuh of its measurement during rainfall. Methods The influence factors of rainfall to the measurement of the γ radiation Kerma rate in air were analyzed and then the differential equation of the correlation factors was established theoretically, and by resolving the equation, the mathematical model Was obtained. The model was discussed through several practical examples. Results The mathematical model was coincided with the tendency of curve about the measured data on the influence rule of rainfall to the γ radiation Kerma rate in surface air. Conclusion By using the theoretical formula in this article which is established to explain the relationship between the rainfall and the γ radiation Kerma rate in surface air,the influence of rainfall to the γ radiation Kerma rate in surface air could be correctly revised.

ABSTRACT

Clear cell tumor of the lung is a rare and very unusual benign pulmonary tumor. As clear cell tumor of the lung contains abundant cytoplasmic glycogen, this tumor is called "sugar tumor". We report a case of sugar tumor in a 64-yr-old man presenting as a round pulmonary nodule. On dynamic computed tomography (CT) scans, the solitary pulmonary nodule showed early wash-in enhancement with an early washout pattern like a lung malignancy. The patient underwent wedge resection for the tumor. Pathologic examination, including immunohistochemical studies, revealed that the nodule was a benign clear cell tumor, so-called "sugar tumor". Because only a small number of cases have been reported previously, clinical aspects, radiological characteristics on dynamic contrast-enhanced CT, and differential diagnosis of the tumor are not well established. Herein we present a clear cell tumor of the lung and discuss the clinical, radiological, and pathological features of the tumor.
