ABSTRACT

Objective:To explore the safety and efficacy of continuous pair triangular flap plasty in the treatment of webbed penis.Methods:The clinical data of 5 patients with webbed penis treated by continuous pair triangular flap plasty in Tianjin Baodi Hospital from November 2020 to January 2021 were retrospectively analyzed. The average age was 18.2 (15-24) years old. Physical examination showed a webbed fusion between the ventral skin of the penis and the scrotal skin, the penoscrotal angle disappeared and penile erection was limited. The web extended to or exceeded the middle of the penis in the 5 cases. None of the 5 patients had other penile malformations. Taking the ventral midline of the penis and the junction of the penis and scrotum as the origin, a " Z-shaped" incision was made above and below it respectively, and two triangular flaps with the same size and shape but opposite direction were formed respectively. Then the triangular flaps were switched and sutured.Results:The average operation time of five patients was 13(10-20) minutes. The operative average blood loss was 1.6(1-3)ml. All patients had good wound healing and no surgical complications. Three months after operation, the appearance of the penis was satisfactory and erection was unrestricted.Conclusions:Continuous pair triangular flap plasty overcomes the disadvantages of traditional operation, and has the advantages of simple operation, good operation results and few complications. It is a good choice for the treatment of webbed penis.

ABSTRACT

Objective:To introduce the application of a modified scrotoplasty for children with webbed penis.Methods:Retrospective study was carried out.The treatment results of 58 children with webbed penis in Department of Urology, the Affiliated Xuzhou Children′s Hospital of Xuzhou Medical University from June 2012 to April 2018 were analyzed.A total of 26 children with webbed penis and phimosis were treated with circumcision combined with modified scrotoplasty as modified scrotoplasty group, and 32 children with webbed penis and phimosis were treated with V-Y scrotoplasty as V-Y scrotoplasty group.The curative effect of webbed penis was compared between the two groups.Results:The operation time of V-Y scrotoplasty group and modified scrotoplasty group were (26.0±2.4) min and (28.0±3.2) min; the increased penis length in the horizontal position in the two groups were (0.30±0.06) cm and (0.40±0.06) cm; the score of parents′ satisfaction were (3.80±0.47) scores and (4.70±0.56) scores, there were significant differences in the average operation time, the increased penis length in the horizontal position and the score of parents′ satisfaction between the two groups ( t=2.703, 6.061, 6.652; all P<0.05). There was no significant difference in postoperative complications between the two groups ( P>0.05). Conclusions:Circumcision combined with modified scrotoplasty is used to repair webbed penis, which results in a good postoperative appearance, high parent satisfaction and easy clinical implementation.

ABSTRACT

PURPOSE: To report a rare case of concealed penis that was aggravated by circumcision and simple way of treatment using skin flap. The concealed penis is a primarily pediatric disease and is usually treated by urologists. The plastic surgeons tend to see these patients when it become secondary deformation or in conjoinment with other specialties. In such situation, the plastic surgeons do not have many experiences. METHODS: A 3-year-old boy was admitted with aggravated concealed penis after circumcision in congenital webbed penis. Patient had been operated with scar revision and skin flap and peri operative antibiotics. RESULTS: The patient recovered well without any complication such as infection, hematoma and necrosis of flap. Patient was discharged POD#7 with remaining sutures. Patient was followed up for 7 months and he has no sign of erectile dysfunction, voiding dysfunction and evidence of inflammation of the urinary system. CONCLUSION: We experienced a case of concealed penis that was aggravated from aggressive circumcision in congenital webbed penis.

ABSTRACT

Webbed penis without chordee or hypospadias is uncommon malformation of the genital integument which is repaired easily once the condition is recognized. Examination reveals a web of skin extending between the median scrotal raphe and the ventral aspect of penis. The web was incised transversely and sutured vertically in two patients. In other three patients the redundant skin was excised and sutured with interrupted 4-0 vicryl. We report five cases of webbed penis which are good functional and cosmetic results.
