ABSTRACT

Objective:To observe the efficacy of compound wrist-hand orthosis-assisted training on wrist-hand dysfunction in patients with convalescent stroke. Methods:From June, 2018 to December, 2019, 34 stroke patients in our hospital were randomly divided into control group (n = 17) and experimental group (n = 17). Both groups received basic treatment and routine occupational therapy, while the experimental group wore a compound wrist-hand orthosis during and after the daily occupational therapy. The occupational therapy was carried out 30 minutes a time, twice a day for 14 days, and the non-training time orthosis wearing was carried out cumulative five hours a day for 14 days. Before and after treatment, the Brunnstrom stage (upper limb and hand) and Fugl-Meyer Assessment (FMA) (wrist and hand) were used to assess the motor function of the upper limbs and hands, while the modified Ashworth Scale (MAS) and modified Barthel Index (MBI) were separately used to assess the wrist tension and activities of daily living. Results:Two cases dropped out in the control group. There was no significant difference in Brunnstrom stage, FMA score, MAS score, and MBI score between two groups before treatment (P > 0.05). After treatment, the scores of FMA and MBI significantly improved in both groups (|Z| > 3.420, P < 0.01), and the improvement was higher in the experimental group than in the control group (Z = -2.895, t = 4.331, P < 0.01); while, no significant difference was found in Brunnstrom stages and MAS score in both groups (P > 0.05). Conclusion:The compound wrist-hand orthosis-assisted therapy could improve the wrist-hand motor function and activities of daily living in patients with convalescent stroke.

ABSTRACT

Objective:To observe the effect of grasping training under a dynamic wrist-hand orthosis (Saebo Glove) on upper limb and hand function in chronic stroke patients. Methods:From October, 2018 to January, 2019, eleven patients with hemiplegia in upper limb and hand after chronic stroke were recruited. They received conventional rehabilitation and grasping training under dynamic wrist-hand orthosis for three weeks. They were tested surface electromyography of flexor and extensor muscle of wrist and upper limb, assessed with Fugl-Meyer Assessment-Upper Extremities (FMA-UE), Action Reach Arm Test (ARAT), modified Ashworth Scale (MAS), modified Tardieu Scale (MTS) and Triple Spasticity Scale (TSS), and measured grip force, active range of motion of wrist and metacarpophalangeal joints before and after treatment. Results:The scores of FMA, ARAT and TSS, and active range of motion of wrist flexor improved after treatment (t > 2.739, P < 0.05). Conclusion:Grasping training assisted with dynamic wrist-hand orthosis can improve upper limb and hand function of motor and release spasticity in hemiplegics after chronic stroke.

ABSTRACT

@#Objective To investigate the effect of the modified wrist-hand orthosis on the spasm and function of wrist-hand in hemiplegic patients. Methods 56 patients with hemiplegia after stroke were divided into observation group (n=28) and control group (n=28). All the patients received routine rehabilitation, including physical therapy, occupational therapy, and Hufschmidt therapy. The observation group wore modified wrist-hand orthosis 4-8 hours a day after the treatment, with interval of 15 min per 30-60 min. All the patients were assessed with modified Ashworth Scale (MAS), simple Fugl-Meyer Assessment (FMA) and active range of motion (AROM) of flexion, extension, radial deviation and ulnar deviation of wrist before and 4 weeks, 8 weeks and 12 weeks after treatment. Results There was no significant improvement in both groups 4 weeks after treatment, nor difference between groups. The scores of MAS and FMA improved 8 weeks after treatment in both groups (P<0.05), and improved more in the observation group than in the control group (P<0.01). While the AROMs improved in the observation groups (P<0.05), and the AROM of extension was more than that of the control group (P<0.01). 12 weeks after treatment, the scores of MAS and FMA improved further in the observation group (P<0.01), and improved more than those of the control group (P<0.01). The AROMs of flexion, extension and radial deviation improved more in the observation group than in the control group (P<0.05). Conclusion The modified wrist-hand orthosis can obviously relieve the spasm of wrist-hand flexor in patients with hemiplegia after stroke, and promote the motor function.

ABSTRACT

Objective To study the effect of crawling exercise with wrist-hand orthosis on the upper limb af- ter stroke.Methods Forty-two stroke patients were divided randomly into an experimental group (n=21) and a control group (n=21).Both groups received routine therapy.Meanwhile the experimental group received crawling exercise with wrist-hand orthosis on the affected upper limb for 60 min once daily,5 days a week for 8 weeks.All the subjects were assessed by using the FMA test,the Ashworth scale and the Barthel index before and after treatment. Results There was no different between the two groups in the three tests before the treatment.After 6 weeks of treat- ment,significant different was found between the two groups with regard to the results of all the tests,with the experi- mental group better than the control group.Conclusion The crawling exercise with wrist-hand orthosis on is more effective than the routine therapy for the function of the upper limb in stroke patients.
