ABSTRACT

【Objective】 To analyze the results, characteristics and clinical value of video urodynamic study (VUD) of lower urinary tract symptoms (LUTS) in young male. 【Methods】 A total of 106 young male LUTS patients (18-45 years old) who received VUD in our hospital during Jan.2016 and Sep.2021 were collected to analyze the clinical and imaging urodynamic characteristics. 【Results】 Of the 106 patients, 55 (52.44%) had neurogenic lower urinary tract dysfunction (NLUTD)with clear neurological etiology, and 51 (48%) had non-neurogenic lower urinary tract dysfunction (NNLUTD). In NLUTD patients, dysuria was the most common symptom (76.74%); lumbosacral lesions were the main cause (76.36%); imaging urodynamics indicated weakening of detrusor muscle in different degrees. In NNLUTD patients,the main symptoms were frequent urination (48.72%) and dysuria (48.72%); about 58.97% of patients had two or more LUTS, and the main diagnosis was detrusor underactivity (DU)(35.90%). 【Conclusion】 NLUTD in young male is characterized by varying degrees of detrusor muscle weakness, detrusor sphincter dyscoordination, and decreased bladder compliance. NNLUTD is mostly caused by detrusor overactivity (DO) and DU.

ABSTRACT

Thyroid Eye Disease also known as Thyroid Associated Orbitopathy, Grave’s Disease is typically a self-limiting autoimmune process associated with dysthyroid states usually manifesting in middle age. The clinical presentation may vary from very mild disease to severe irreversible sight threatening complications. Primary risk factors for TED are environmental influences especially smoking, previous pathogen exposures, stress, previous use of radio iodine and complex genetic components. Here we report a case of 25-year-old male who presented to our OPD with complaints of swelling of eyes, photophobia, lacrimation since 3days. Bilateral mild proptosis, severe congestion and conjunctival injection with 20/20 vision in both eyes were noted on examination. Blood investigations showed Eu-clinical Thyroid state whereas MRI Brain and Orbits showed all four Recti muscle belly enlargement. This case emphasizes one of the rare presentations of TED in accordance to Age, Sex and Clinical status of the thyroid.

ABSTRACT

Background: Coronary Artery Disease (CAD) is the leading cause of death and accounts for around 12million deaths annually worldwide. The pattern of coronary artery involvement and clinical outcome varies with age suggesting different underlying pathophysiology. Better understanding this specific problem will lead to further improvement in management. The aim was to study the risk factors and angiographic findings in young patients with acute myocardial infarction.Methods: Thirty-nine patients were included in this study. Patients included in the study underwent elective angiographic performed by the percutaneous femoral approach using standard angiographic techniques. Echo and lipid profile were done to all patients. Data were analysed using MS-Excel.Results: In this study, 39 patients were included, 37 male and 2 female patients, the average age of the patients was 35.12 years. 59% of patients had hypertriglyceridemia, 23.1% of patients hypercholesteremia. 45.9% of male patients were having a history of smoking and alcoholism. 84.6% of patients had obstructive CAD, single vessel disease was observed in 59%, double vessel disease in 7.7%, triple vessel in 2.6% patients.Conclusions: Acute myocardial infarction in young predominantly affects males. Dyslipidemia, smoking and alcoholism were found to be common associated factors in young MI. This emphasizes the need for lifestyle modification for primary prevention. Coronary angiography revealed normal coronaries in 12% suggesting different underlying process other than atherosclerosis. With timely intervention and appropriate management, prognosis is good in young MI patients.

ABSTRACT

Autosomal-dominant polycystic kidney disease (ADPKD) is a systemic hereditary disorder characterized by localized cellular proliferation and extracellular matrix abnormalities resulting in both renal and extra renal manifestations, with multiple structural flaws such renal cysts, cerebral and aortic aneurysms, annulo-aortic ectasia, and valvular insufficiency states. Association between Autosomal-dominant polycystic kidney disease (ADPKD) and ruptured sinus of Valsalva aneurysm (SVA) has rarely been documented before. We herein report a case of Autosomal-dominant polycystic kidney disease (ADPKD) who presented with shortness of breath, was diagnosed as a case of ruptured sinus of Valsalva aneurysm (SVA) and defect was closed successfully.

ABSTRACT

Objective@#To evaluate the effects of three different medications with tadalafil on erectile dysfunction (ED) in young men with primary sexual failure.@*METHODS@#This study included 76 male ED patients aged 21-35 years who had primary sexual failure but normal nocturnal penile tumescence and rigidity and failed to respond to psychotherapy. We randomly assigned them to receive oral tadalafil once daily, on demand, or once-daily + on-demand. After 2-3 months of treatment, we evaluated the effects based on the scores of the patients in the five domains of the International Index of Erectile Function (IIEF-5).@*RESULTS@#After medication, all the patients showed significantly increased scores in the four domains of IIEF-5, namely, erectile function, orgasmic function, intercourse satisfaction, and overall satisfaction. The on-demand group achieved even higher scores in erectile and orgasmic functions but a lower score in sexual desire than the once-daily group. However, the patients in the once-daily + on-demand group exhibited more significant improvement than those in the other two in all the five domains.@*CONCLUSIONS@#Once-daily + on-demand medication with tadalafil can significantly enhance the therapeutic effect on psychogenic ED in young men with primary sexual failure.

ABSTRACT

Humeral shaft fracture sustaining arm wrestling is rare, but occurs intermittently. We treated 15 cases of humeral shaft spiral fractures occurred during arm wrestling for fun since 2007. Average age was 22.47±2.69 years, average body mass index was 22.67±2.06 kg/m2. There was no prominent tendency for the fractures to occur at a certain phase of the match. Fractured level and length of each case were measured in the plain radiographs and compared with those of previous reports. Eight cases (53.3%) had an associating medial butterfly fragment, and the time taken until the fracture occurred was longer than that of simple spiral fracture (15.62±9.03 seconds vs. 7.85±2.67 seconds, p=0.048). Fractures were distributed mid to distal one third of humerus, the length of fracture was 7.93±2.69 cm and involved 25.43%±8.24% of humeral length. All cases except one treated surgically using plate and screws and returned their full activities within postoperative 3 months. Although arm wrestling is a simple and joyful sport, participants should be aware of the risks of injury during arm wrestling, especially for the amateur players.

ABSTRACT

<p><b>Objective</b>To study the psycho-social risk factors of sexual failure among newly married young males in the Uyghur population.</p><p><b>METHODS</b>We conducted a paired case control study of 186 newly married Uyghur young males (aged 17-30 [23.4±2.9] yr) with sexual failure and another 186 (aged 18-34 [24.0±3.1] yr) with no such problem as controls. We performed a logistic regression analysis on the possible psycho-social risk factors of this condition.</p><p><b>RESULTS</b>Logistic regression analysis showed that the risk factors of sexual failure among the newly married men included personality (OR=0.271, 95% CI 0.176-0.420), income (OR=0.391, 95% CI 0.264-0.580), history of masturbation (OR=0.824, 95% CI 0.710-0.956), premarital sex (OR=0.757, 95% CI 0.677-0.847), sense of obligation (OR=1.756, 95% CI 1.157-2.693), equality of the social status (OR=0.574, 95% CI 0.435-0.756), degree of mutual care (OR=1.605, 95% CI 1.268-2.032), female's psychological obstacle (OR=2.832, 95% CI 1.221-6.569), and religion (OR=0.643, 95% CI 0.472-0.967). There was a statistical significance in the correlation between these factors and sexual failure in the newly married males (all P<0.05).</p><p><b>CONCLUSIONS</b>Sexual failure of newly married Uyghur young males are associated with many psycho-social factors, which necessitates sexual education among young males and particularly pre-marriage sexual education and psychological guide among both males and females.</p>

ABSTRACT

Merkel cell carcinoma (MCC) is a highly aggressive neoplasm of skin with neuroendocrine differentiation. Primary MCC of the oral mucosa is exceedingly rare and even more unresponsive to therapy. A 15-year-old male presents with gradually increasing painless swelling in right side of the fl oor of mouth for 6 weeks. Computed tomography of head and neck region showed globular mass (4.6 cm × 1.7 cm) involving right side of the fl oor of mouth. Fine-needle aspiration from the upper deep cervical node suggested small round cell tumor. A trucut biopsy showed mass composed of trabeculae and nests of tumor cells with high N:C ratio, granular speckled chromatin, scanty to moderate amount of clear vacuolated cytoplasm. Cells were immunoreactive for cytokeratin-20, CD56, c-kit, CD99 and negative for p63, thyroid-transcription factor-1, CDX2, synaptophysin, neuron-specifi c enolase. Patient was started on chemotherapy with cyclophosphamide, doxorubicin and vincristine. The mass regressed in size and patient underwent wide local excision with pull-through approach. Patient is currently under combined chemoradiation regime and doing well.

ABSTRACT

BACKGROUND: Recently, tattooing of the body has become popular, and its associated psychological, sociological, and cultural implications have changed compared to past years. However, little is known about the current clinical features, including motive and satisfaction for having a tattoo, because of societal repulsions toward the practice. OBJECTIVE: To evaluate the clinical manifestations of tattooed young Korean males and to compare the findings with previous studies. METHODS: Between September 2010 and July 2011, 223 patients with tattoos who visited the Eulji Medical Center and Armed Forces Yangju Hospital were selected. The patients were evaluated through a self-administrated questionnaire and physical examinations. RESULTS: The majority of patients were classified into the "did not complete university" education group (56.1%). Most tattooed males had two living parents (83.4%) and were raised in households whose income statuses were not marked by poverty. The average age at which their first tattoo was received was 19.0 years. Most tattooed males had a negative attitude towards their tattoos (67.3%). The most common motivation for getting a tattoo was curiosity, but cosmetic purposes were also important (19.4%). A majority of the tattooing was conducted by a tattoo artist (73.3%). The most common tattoo content was letters. Black-colored tattoos were most frequently observed. The complication rate was 13.9%. The most common motivation for tattoo removal was negative comments that were made at work, in public, and in school (38.2%). CONCLUSION: Recent patterns of tattooing have changed significantly since previous reports. The satisfaction rate in getting a tattoo was not favorable. Understanding the current clinical features related to tattooing is important and it is necessary for dermatologists to provide professional advice regarding tattooing among young males.

ABSTRACT

Background and purpose: Adenocarcinoma is the main type of lung cancer and the ages of most patients with adenocarcinoma are over-40 years, the young patients with the disease are rare, and the 5-year survival rate of the young male with lung adenocarcinorna is poor. This study was done to explore the survival and prognosis of young male with lung adenocarcinoma. Methods: A total of 110 surgically treated patients with lung adenocarcinoma were analyzed retrospectively to study the relationship between the prognostic factors and survival. Results: The analysis showed that the overall 5-year survival rate of the young male with lung adenocarcinoma was 34.5%, it was 29.0% for smokers and 42.6% for non-smokers(P<0.05). The 5-year survival rates of staging Ⅰ, lI ,Ⅲ and Ⅳ were 68.8%,35.7%, 14.3% and 0, respectively(P<0.01),the survival rates of the patients with the number of involved lymph nodes>3 compared to ≤3 were 28.5% and 48.1%(P<0.01). Conclusion: Smoking, p-TNM, the number of involved lymph nodes was correlated with the survival, effective smoking control efforts and total lymphadenectomy are important to the survival of lung adenocarcinoma among young males.

ABSTRACT

Background: According to statistical data of the Vietnam Ministry of Health, there were 124,223 people with positive HIV, 23,661 AIDS patients and 13,649 AIDS- related deaths up to April 30th 2007. The major transmission route is injection. Objectives: (1) To compare the characteristics of drug use in users with different time. (2) To describe the health issues and health service access of young male drug users. Subjects and method: A cross-sectional survey was carried out using a combination of medical anthropology and epidemiology methods. 1.115 young male drug users in Ha Noi between September 2004 and September 2005 were divided into 4 groups of the equivalence number, but with different drug using history. Results and conclusion: The newer drug users (within 2 years) started using drug later than the older ones, more of them started using by injecting; they transfer from smoking/sniffing to injecting as well as trying and using more different types of drugs together in shorter time than others. The proportion of HIV (+) infection in these people was higher, which states the necessity of intervention to help them enhance their knowledge to prevent HIV infection amongst themselves. Mental health care is also needed for drug users, the demand for mental health care increases with the time of drug usage.

ABSTRACT

Objective To investigate the effects of female sex hormones in cow's milk on metabolism of blood lipid in young male rats.Methods Forty-eight male Sprague-Dawley rats aged 21 days old were assigned randomly to 4 groups,each containing 12 rats,and fed with quantitative milk from postpartum cow,milk from pregnant cow,commercial whole milk and artificial milk,respectively.Serum total cholesterol (TC),triacylglyeriol(TG),high-density lipoprotein cholesterol (HDL-C),low-density lipoprotein cholesterol (LDL-C) and urinary creatinine (Cr) were determined with automatic biochemical analyzer.Serum progesterone(P4)and urinary free estriol(UFE3) were determined with immunochemiluminometric assays after all rats were killed at 53 days old.SPSS 13.0 software was used to analyze the data.Results Total estradiol and P4 were 1 189.66 pmol/L,833.98 pmol/L,588.17 pmol/L,286.48 pmol/L and 9.76 nmol/L,10.18 nmol/L,2.83 nmol/L,0.92 nmol/L in milk from pregnant cow,commercial whole milk,milk from postpartum cow and artificial milk groups,respectively.Serum TC were respectively(1.78?0.29) mmol/L,(1.94?0.20) mmol/L,(2.10?0.28) mmol/L and (2.11?0.22) mmol/L in pregnant milk,commercial whole milk,postpartum milk and artificial milk groups,and TC in pregnant milk group was lower than that in postpartum milk group or artificial milk group(P0.05).Conclusion Milk from pregnant cow may reduce serum TC in young male SD rats,which may be related to the conjoined effect of estradiol and P4.

ABSTRACT

Giant sebaceous gland hyperplasia is a relatively rare condition revealing the same histological findings to sebaceous gland hyperplasia, but on a larger scale. A 34 year-old male patient presented with an asymptomatic, solitary, 1x0.4cm sized, erythematous nodule on the occipital area, which had developed 5 years ago. The initial impression was a trichilemmal cyst, epidermal cyst or folliculitis. Histopathologically, grouped hyperplastic sebaceous glands and a central dilated duct was observed, with Demodex brevis in one of the smaller glandular ducts. The lesion was completely removed by excisional biopsy and no sign of recurrence has been noticed to date. We report a case of giant sebaceous gland hyperplasia with Demodex brevis of the sebaceous duct on the scalp of young adult.

ABSTRACT

Calcium pyrophosphate dihydrate (CPPD) crystal deposition disease is a disease of the elderly and extremely rare in young individuals. If young people develop CPPD crystal deposition disease, it may be associated with metabolic diseases such as hemochromatosis, hyperparathyroidism, hypophosphatasia, hypomagnesemia, Wilson's disease, hypothyroidism, gout, acromegaly, and X-linked hypophosphatemic rickets. Therefore, in young-onset polyarticular CPPD crystal deposition disease, investigation for predisposing metabolic conditions is warranted. We report a case of a young male patient with idiopathic CPPD crystal deposition disease, who did not have any evidences of metabolic diseases after thorough evaluations. As far as we know, this is the first report of a young male patient presented with idiopathic CPPD crystal deposition disease.

ABSTRACT

Epiglottitis is an uncommon but potentially life threatening infectious disease in young children. And it is rapidly progressing cellulitis of the epiglottis and adjacent structures that has the potential for causing abrupt, complete airway obstruction. The most common cause of acute epiglottitis is Haemophilius influenzae type b(Hib), therefore in USA, the acute epiglottitis is seen less commonly since the wide spread use of immunization against Hib. In Korea, there has been no report of acute epiglottitis caused by known bacterial organism, and a little investigation of the effects of the immunization against Hib, or Hib related respiratory diseases. In this report, we describe two cases of acute epiglottitis caused by Hib, occurred in non-immunized young-male children admitted to Ajou University Hospital. They had visited to the emergency center of Ajou University hospital with the complaints of acute fever, drooling, dysphagia and severe respiratory difficulties. In both cases, we observed the cherry-red colored, severely swollen epiglottis by the direct laryngoscopic examination. Hib was cultured in blood samples from both two cases, and the patients treated successfully by the 3 days of endotracheal intubation and proper antibiotics therapy without any complications such as pneumonia, meningitis, osteomyelitis, or pericarditis.
