ABSTRACT
Objective To investigate the effect of locking plate combined with cortical screw internal fixation on ankle function and quality of life in patients with ankle fracture with tibiofibular separation.Methods A total of 120 patients with ankle fracture and distal tibiofibular separation treated in our hospital from May 2020 to December 2021 were selected and divided into control group and observation group according to random number table method,with 60 patients/group.The control group was treated with cortical screw fixation alone,and the observation group was treated with locking plate combined with cortical screw internal fixation.Before surgery and 6 months after surgery,the recovery function of the two groups was compared.X-ray,operation duration,healing time,intraoperative blood loss,postoperative complications were compared,and the living ability of the two groups of patients was evaluated.Results Before treatment,there was no difference in joint function between the two groups(P>0.05).After treatment,the longest walking of the control group(15.89±0.85),foot alignment(15.06±0.71),pain response(29.03±4.48)and ground walking(15.65±0.59).The longest walking distance(16.19±0.87),foot alignment(15.29±0.76),pain response(31.24±4.55)and ground walking(15.96±0.68)in the observation group,which were higher than those in control group(P<0.05).Compared with the control group,the intraoperative blood loss and healing time in the observation group were lower(P<0.05).BI index of the two groups before treatment had no difference(P>0.05);After treatment,BI index of observation group was higher than that of control group(P<0.05).There was no difference in the total complication rate between the two groups(P>0.05).Conclusion Locking plate combined with cortical screw internal fixation has a good therapeutic effect on improving ankle function,reducing intraoperative blood loss,promoting healing and improving behavioral ability in the treatment of ankle fracture combined with hypotibiofibular syndesmosis injury.

BACKGROUND:Poor incision healing and infection often occur in elderly patients with lateral malleolar fractures after traditional lateral plate fixation.With the application of engineering software in medicine,a new type of plate placed posterolateral can be designed to solve the above-described problems. OBJECTIVE:To design a new type of posterior lateral low-profile steel plate with the aid of medical bioengineering software,based on the 3D CT data of the distance between the top of the lateral ankle fracture line to the anterior starting point(ACD),the distance between the top of the fracture line to the tip of the lateral ankle(CTD),the distance between the top of the fracture line to the posterior edge of the fracture line(PCD)and the angle between the anterior and posterior lateral sides of the distal fibula(CA). METHODS:Thirty cases of unstable lateral malleolar fracture and normal ankle were taken for CT scanning and three-dimensional reconstruction.The ACD,CTD,and PCD values in patients with lateral malleolar fracture were measured by 3-matic software,and the characteristics of lateral malleolar fracture line were plotted and described.The mimics software was used to measure the value of CA in the normal ankle joint.Based on the data measured above,3-matic software and solidworks software were used to design the low-profile steel plate and the thickness of the steel plate and the direction of the nail path were constructed.In Geomagic Studio software,fine surface,automatic surface,and fitting surface were used to generate the prototype of the low-profile steel plate,and then 3D printing was performed.After making a posterolateral incision of the lateral malleolus,the peroneus longus and brevis tendons were removed,and the prototype of the 3D-printed steel plate was placed behind the fibula to test its size and fit to the bone surface. RESULTS AND CONCLUSION:(1)The mean of ACD was(2.97±0.03)cm,and the variation was 5.23.The mean of PCD was(3.17±0.11)cm,and the variation was 17.60.The mean of CTD was(4.52±0.07)cm,and the variation was 8.60.(2)The fracture line of the lateral malleolus was drawn with an inverted"V"shape.The mean of CA between anterior and posterior lateral surfaces of the distal fibula was(103.20±1.94)°.At the midpoint section of the upper and lower vertices of the anterior edge of the distal fibula,the angle of the anterior and posterior lateral sides(CA)of the distal fibula was(78.50±1.78)°.(3)By using 3-matic,Solidworks,and Geomagic Studio software,a new type of posterior lateral low-profile steel could be successfully designed.Three to four holes were reserved for the screw holes at the proximal end of the plate with screw directions from back to front,and three screw holes were reserved on the inner and outer sides at the far end.The direction of the inner three holes could be from back to front,and the outer three screw holes needed to be biased towards the inner side,with an angle of 9.72°-13.28°.(4)It is indicated that the variability of the ACD position on the anterior lateral fracture line of the lateral malleolus is relatively small,while the variability of the posterior lateral PCD position is relatively large.The angle between the anterolateral and posterolateral sides of the lateral ankle fracture block shows a decreasing trend,with a smaller variation in the proximal angle and a larger variation in the distal angle.Based on three-dimensional CT reconstruction data of the external ankle,with the help of computer bioengineering software and the use of reverse design concept,a new type of low-profile lateral malleolus steel plate with a good fit can be quickly and conveniently designed to provide a valuable reference for the design of internal fixation devices.

Objective:To explore the effect of 3D printing technology on ankle fracture surgery.Methods:This study was a case-control study.Forty five patients with ankle fractures admitted to the Department of Orthopaedics of the Affiliated Hospital of North China University of Science and Technology from January 2017 to May 2022 were selected. Divide into an observation group and a control group using the random number table method. The control group received open reduction and internal fixation of the ankle joint under X-ray assistance, while the observation group used Mimics software modeling and 3D printing models to assist in open reduction and internal fixation of the ankle joint. Compare the surgical time, intraoperative bleeding volume, intraoperative X-ray exposure frequency, postoperative physician fatigue scale 14 (FS-14) score, fracture reduction quality, fracture healing time, and ankle joint function at 6 months after surgery between the two groups of patients. The measurement data is represented by ± s, and the two groups are compared using t-tests with two independent samples; Counting data is represented by examples (%), χ 2 test is used for inter group comparison, and rank sum test is used for inter group comparison of rank data. Results:The observation group had shorter surgical time than the control group [(134.16±25.61) minutes compared to (163.38±41.90) minutes], with less intraoperative bleeding and fewer intraoperative X-ray exposure times than the control group [(46.32±29.29) mL compared to (99.62±77.85) mL, (13.68±3.50) times compared to (18.54±6.09) times], and lower postoperative physician fatigue scores than the control group [(9.77±2.02) points compared to (12.13±1.73) points], The differences between groups were statistically significant (t-values in decibels were 2.69, 3.20, 3.12, and 4.20, all P<0.05). The quality of postoperative fracture reduction in the observation group was excellent in 12 cases, good in 5 cases, and poor in 2 cases; The quality of postoperative fracture reduction in the control group was excellent in 7 cases, good in 16 cases, and poor in 3 cases. Compared between groups, the observation group had better postoperative fracture reduction quality than the control group ( Z=-2.05, P=0.040). At a follow-up of 6 months after surgery, there was no statistically significant difference in bone healing time between the two groups ( P>0.05). The AOFAS score in the observation group was higher than that in the control group (91.63±2.83) compared to (88.92±3.92), and the difference was statistically significant ( t=2.56, P=0.014). Conclusions:Using Mimics software modeling and 3D printing technology to assist ankle fracture surgery can shorten the operation time, reduce the use rate of X-ray, reduce the fatigue of doctors, and improve the quality of joint reduction and ankle function.

OBJECTIVE@#To explore curative effect of conservative treatment of supination-lateral rotation (SER) with type Ⅲ and Ⅳ ankle fracture by bone setting technique.@*METHODS@#From January 2017 to December 2019, 64 patients diagnosed with SER with type Ⅲ and Ⅳ ankle fracture were treated with manipulative reduction and conservative treatment (manipulation group) and surgical treatment with open reduction and internal fixation (operation group), 32 patients in each group. In manipulation group, there were 17 males and 15 females, aged from 15 to 79 years old with an average of (51.42±13.68) years old;according to Lauge-Hansen classification, there were 8 patients with supination external rotation type Ⅲ and 24 patients with type Ⅳ. In operation group, there were 13 males and 19 females, aged from 18 to 76 years old with an average of (47.36±15.02) years old;7 patients with type Ⅲ and 25 patients with type Ⅳ. Displacement of ankle fracture was measured by Digimizer software, and compared before treatment, 3 and 12 months after treatment between two groups. Lateral medial malleolus displacement, lateral medial malleolus displacement, lateral malleolus displacement, lateral malleolus displacement, lateral malleolus contraction displacement and posterior malleolus displacement were measured and compared between two groups. Mazur score was used to evaluate ankle joint function.@*RESULTS@#All patients were followed up from 12 to 36 months with an average of (17.16±9.36) months. There were statistical differences in lateral medial malleolus displacement, lateral medial malleolus displacement, lateral malleolus displacement, lateral malleolus displacement, lateral malleolus contraction displacement and posterior malleolus displacement in manipulation group before and after reduction(P<0.05). Compared with operation group, there were no statistically significant differences in lateral malleolus shift, lateral malleolus shift, lateral malleolus contraction shift(P>0.05), while there were statistically significant differences in lateral malleolus shift, posterior malleolus shift up and down (P<0.05). Mazur scores of ankle joint at 3 months after treatment in manipulation group and operation group were 68.84±13.08 and 82.53±7.31, respectively, and had statistical differences(P<0.05), while there was no difference in evaluation of clnical effect(P>0.05). There were no differences in Mazur score and evaluation of clnical effect between two groups at 12 months after treatment (P>0.05).@*CONCLUSION@#Bone setting technique could effectively correct lateral displacement of medial malleolus, lateral displacement of medial malleolus, lateral displacement of lateral malleolus and lateral contraction displacement of lateral malleolus in supination lateral rotation type Ⅲ and Ⅳ ankle fracture, and has good long-term clinical effect, which could avoid operation for some patients and restore ankle function after fracture.

Introducción: La incidencia de fractura-luxación abierta de tobillo es baja, lo que hace que su combinación con fractura del tercio distal de la tibia sea grave y muy compleja de tratar. Constituye una entidad causada por traumas de alta energía y se asocia a lesiones neurovasculares. Objetivo: Presentar un caso grave y complejo de fractura-luxación abierta de tobillo y fractura del tercio distal de la tibia. Caso clínico: Paciente masculino de 33 años de edad, que ingresó en el servicio de ortopedia y traumatología de urgencia, por haber sufrido trauma directo en el acople de 2 vagones de un tren. Presentó fractura-luxación tibioastragalina abierta III-B, asociado a fractura oblicua del tercio distal de la tibia y el peroné del miembro inferior izquierdo. Fue intervenido quirúrgicamente de urgencia. Se realizó amplio desbridamiento quirúrgico, reducción de la luxación, fijación del maléolo tibial, reparación primaria de las lesiones ligamentosas y estabilización externa para artrodesis precoz. El paciente evolucionó con infección de la herida quirúrgica, para lo cual necesitó de curas secuenciales y oxigenación hiperbárica. Se recuperó a los 8 meses. Conclusiones: La fractura-luxación abierta de tobillo y fractura del tercio distal de la tibia es una combinación poco frecuente, grave y compleja de tratar. Obedece a traumas de altas energías. La cirugía de urgencia, la profilaxis antimicrobiana y la oxigenación hiperbárica, contribuyeron al tratamiento definitivo de este paciente.
Introduction: The incidence of open ankle fracture-dislocation is low, which makes its combination with a fracture of the distal third of the tibia serious and very complex to treat. It is an entity caused by high-energy trauma and is associated with neurovascular injuries. Objective: To present a serious and complex case of open ankle fracture-dislocation and fracture of the distal third of the tibia. Clinical case: A 33-year-old male patient, who was admitted to the emergency orthopedics and traumatology service, for having suffered direct trauma in the coupling of two train cars. He presented an open tibiotalar fracture-dislocation III-B, associated with an oblique fracture of the distal third of the tibia and fibula, of the left lower limb. He underwent emergency surgery; Extensive surgical debridement, reduction of the dislocation, fixation of the tibial malleolus, primary repair of the ligamentous injuries, and external stabilization for early arthrodesis were performed. The patient evolved with infection of the surgical wound, for which he needed sequential dressings and hyperbaric oxygenation. Full recovery at 8 months. Conclusions: Open ankle fracture-dislocation and fracture of the distal third of the tibia is a rare, serious and complex combination to treat. Due to high energy trauma. Emergency surgery, antimicrobial prophylaxis, and hyperbaric oxygenation contributed to the definitive treatment of this patient.

Las fracturas pueden producir impotencia funcional, deformidad, hematoma, entumecimiento, dolor y cosquilleo. Muchas requieren cirugía y rehabilitación física por personal calificado y equipos adecuados. El objetivo de este estudio fue aplicar la rehabilitación física y el sistema súper inductivo de alta intensidad como tratamiento en fractura postquirúrgica. La paciente de 46 años, sufre caída de propia altura fracturándose la tibia y peroné derechos, confirmado el diagnóstico, se le realiza la reducción abierta más fijación interna, recibe el alta con férula suropédica que se retira seis semanas después, inicia marcha con apoyo parcial progresivo con incremento de dolor eva 6/10, limitación de la movilidad y edema bimaleolar. La radiografía muestra solución de continuidad del maléolo tibial con material de osteosíntesis. Recibe propuesta terapéutica no invasiva, suspensión de la carga, rehabilitación física por cinco semanas más sistema súper inductivo de alta intensidad tres veces por semana. A la valoración radiográfica de tobillo derecho postero anterior, lateral, y oblicua, presenta reducción correcta del maléolo tibial, la flexión, extensión, inversión y eversión son valorables sin dolor ni edema. Se puede concluir que la rehabilitación y el sistema súper inductivo de alta intensidad con campos electromagnéticos de alta potencia, sí bioestimula la reparación ósea, también acelera el plazo de osteosíntesis con reducción del dolor logrando la recuperación funcional en la paciente.
Fractures can cause functional impotence, deformity, bruising, numbness, pain, and tingling. Many require surgery and physical rehabilitation provided by qualified personnel and adequate equipment. The objetive of this study was to apply physical rehabilitation and high intensity super inductive system as a treatment for postsurgical fracture. A 46-year-old woman suffered a fall from her own height, fracturing the right tibia and fibula, and, an open reduction plus internal fixation was performed, afer confirming the diagnosis. She wadischarged with a south-pedic splint, which was removed 6 weeks later the patient, began to walk with progressive partial support with 6/10 increased eva pain, limitation of mobility and bimalleolar edema. îe radiograph shows a solution of continuity of the tibial malleolus with osteosynthesis material. She receives a non-invasive therapeutic proposal, suspension of the load, physical rehabilitation for 5 weeks, and a high intensity super inductive system 3 times a week. Radiographic evaluation of the right ankle anterior, lateral, and oblique shows correct reduction of the tibial malleolus. Flexion, extension, inversion and eversion are assessable without pain or edema. Rehabilitation and high intensity super inductive system with high power electromagnetic fields, biostimulate bone repair and accelerates bone treatment, relieving pain. It can be concluded that the rehabilitation and the high-intensity super-inductive system with high-power electromagnetic fields, biostimulates bone repair, and accelerates the osteosynthesis period with pain reduction, achieving functional recovery of the patient.

Objective To make classification and segment measurement for the cases with tibiofibular and ankle fractures in parachuting landing, and investigate main classification types of parachuting fractures and fracture segments of high risk.Methods A total of 56 fracture cases in parachuting landing were collected, and the tibiofibula and ankle fractures were classified according to AO-OTA or Lauge-Hansen classification standards respectively based on their digital X-ray images. The medium plane between talus and tibia joint planes in ankle joint was defined as the reference plane. The highest and lowest injury points of tibia and fibula were marked respectively, and the fracture segment was defined between the highest and lowest point for statistical analysis.Results For tibiofibular and ankle fracture cases in parachuting landing, fracture at both tibia and fibula accounted for 80.4%. The major classification of tibiofibula fracture was 42-D/5.2 (45.8%) and 42-D/5.1 (16.7%). The major classification for ankle fracture was pronation-external rotation (PER, 59.4%) and supination-external rotation (SER, 37.5%). When tibiofibular and ankle fracture cases in parachuting landing occurred, the fracture segment of the tibia was mainly from 57 to 143 mm above the reference plane and from 6 mm below the reference plane to 24 mm above the reference plane, while the fracture segment of the fibula was 4-45 mm and 74-83 mm above the reference plane. Injury risks of all above segments were higher than 50%.Conclusions For protection of lower limbs in parachuting landing, the fracture at both tibia and fibula should be highly noticed. The ankle motion of PER and SER should be especially restricted in parachuting ankle protection.
ABSTRACT
Objective:To investigate the effect of ultra-low dose (ULD) computed tomography (CT) scanning on the diagnosis of ankle fractures and the quality of a three-dimensional printing (3DP) model.Methods:This study was a prospective study. A total of 61 patients with clinical ankle fractures treated conservatively in Zhuhai Hospital of Guangdong Hospital of Traditional Chinese Medicine from November 2019 to January 2022 were included in this study. Patients underwent standard dose (SD) CT scan and ultra-low dose (ULD) CT scan, respectively. The tube voltage/tube current of SD and ULD were 120 kV/100 mAs and 80 kV/10 mAs, respectively. Two senior radiologists evaluated the presence of ankle fractures. The effective radiation dose ( E), noise, signal-to-noise ratio (SNR), contrast signal-to-noise ratio (CNR), and CT value of bone cortex minus CT value of peripheral fat (CTc) were compared. The radiologists also evaluated the discoverability, diagnosability, and overall image quality of the fracture line according to Likert′s 5-point scoring method. Two senior orthopedists subjectively evaluated the quality of each 3DP model (model clarity and operation guidance). A score ≥ 3 indicated that the quality of the CT diagnostic image and 3DP model were acceptable. Results:The interval between the two CT scans was (9.23 ± 1.92) d. A total of 94 fracture sites were found. There were no missed diagnosis or misdiagnosis based on the SD and ULD scans.Noise, SNR, and CNR were better on the SD CT scanning ( F=5.92, 9.70, 8.32, P=0.00), however, CTc was higher on the ULD scans ( F=27.55, P<0.01). The image scores of the SD and ULD scans were (4.97 ± 0.18) and (4.21 ± 0.71), and the quality scores of the 3DP model (4.99 ± 0.01) and (4.87 ± 0.34), respectively. The SD scans were better than the ULD scans with respect to CT image quality and 3DP model quality ( Z=-6.88, -2.91, P<0.01), but both were considered suitable to meet clinical needs (all ≥ 3 points). The E associated with SD and ULD scannings were (34.68 ± 4.96) μSV and (1.04 ± 0.10) μSV, respectively. The latter was thus significantly better than the former ( F=38.77, P =0.00). Conclusions:The E value of ULD scanning is about 3.00% of SD scanning E, which can meet the needs of clinical diagnosis of ankle fracture and 3DP model printing diagnosis.

ABSTRACT Objective: The aim of this study is to purpose a novel approach to the concomitant triplanar and tibial shaft fracture. Methods: Retrospective study between 2001 and 2019. We collected the patients' general information, clinical and radiographic data, and complications after the following three-step treatment: (1) fixation of the Salter-Harris II fracture of the triplane fracture, (2) fixation of the Salter-Harris II/IV fracture with cannulated screws, and (3) fixation of the tibial fracture with flexible titanium nails. Results: The study included seven patients (six males) with a mean age of 14 years and a mean follow-up of 6.4 years (minimum two years). Five triplane fractures had two fragments and two had three fragments. Five fractures were classified as Salter-Harris II and two as Salter-Harris IV. Three tibial fractures were long oblique, three were spiral, and one had a third fragment. Six fractures affected the middle third and one affected the distal third of the tibia. All triplane and tibial fractures consolidated without significant displacement. No physeal damage was identified. Conclusions: This study described the association of tibial fractures with triplane ankle fractures managed by our proposed treatment, which proved to be effective for this fracture association. RESUMO Objetivo: Propor uma nova abordagem para fraturas concomitantes da diáfise da tíbia e triplanares do tornozelo. Métodos: Estudo retrospectivo entre 2001 e 2019. Foram coletadas informações gerais: dados clínicos, radiográficos e complicações. As fraturas seguiram três passos no tratamento: (1) fixação do fragmento Salter-Harris tipo III da fratura triplanar; (2) fixação do fragmento Salter-Harris II/IV com parafuso canulado; e (3) fixação da fratura diafisária da tíbia com hastes flexíveis. Resultados: O estudo incluiu sete pacientes (seis homens) com idade média de 14 anos e seguimento médio de 6.4 anos (mínimo de dois anos). Cinco fraturas triplanares tinham dois fragmentos principais e duas tinham três fragmentos. Cinco fraturas na radiografia em perfil foram classificadas como Salter-Harris II e duas como Salter-Harris IV. Três fraturas diafisárias tibiais tinham traço obliquo longo, três traço espiral e uma fratura com terceiro fragmento. Seis fraturas eram do terço médio e uma fratura do terço distal da tíbia. Todas as fraturas triplanares e tibiais consolidaram sem desvio significativo e não tivemos nenhuma lesão fisária. Conclusão: O estudo descreveu a associação da fratura da tíbia com a fratura triplanar do tornozelo e nossa proposta de tratamento, que se mostrou uma boa opção no tratamento dessa fratura especial. @#Introduction: Unstable ankle injuries require anatomical reduction and stabilisation for optimal outcome. In spite of adequate care, a few patients have poor outcome. In this study, we assessed the risk factors that predict the clinical outcomes in surgically treated unstable ankle fractures. Material and methods: This prospective cohort study was conducted on 68 patients who underwent surgical management for an unstable ankle injury. Demographic details, fracture type and associated medical comorbidities were recorded. Pre-operative radiographic assessment was done for all patients. At the end of one year follow-up, clinical (American Orthopaedic foot and ankle societyAOFAS and Olerud-Molander ankle - OMAS) scores and radiological parameters were assessed and analysed. Results: Fracture dislocation (0.008), diabetes mellitus (0.017), level of alchohol consumption (0.008) and preoperative talocrural angle (TCA) > 100° (0.03) were significant predictors of poor outcomes as per AOFAS. Fracture dislocation (0.029), diabetes mellitus (0.004), preoperative TCA > 100° (0.009), female gender (0.001), age more than 60 years (0.002) and open injuries (0.034) had significantly poor outcome as per OMAS. Other parameters (smoking, hypertension, classification, syndesmotic injury, medial clear space and tibiofibular overlap) did not affect the outcome significantly. Conclusion: Our study showed that poor outcome predictors in unstable ankle fractures are age >60 years, female gender, diabetes mellitus, alcohol consumption, fracture dislocation, open fractures and pre-op TCA >100°.

OBJECTIVE@#To explore clinical effect of adult ankle fracture with Tillaux-Chaput fracture block.@*METHODS@#From January 2014 to December 2018, 15 patients with adult ankle fracture with Tillaux-Chaput fracture block were performed opertaion, including 9 males and 6 females, aged from 27 to 67 years old with an average of (45.6±14.3) years old, 8 patients on the left side and 7 patients on the right side. Fracture healing and complications were observed, American Orthopaedic Foot and Ankle Society(AOFAS) was used to evaluate recovery of ankle joint function.@*RESULTS@#All patients were followed up for 18 to 70 months with an average of (38.1±9.9) months. The incisions healed well at stageⅠ. X-ray reexamination showed all fractures healed well without loosening or breakage of internalfixation. Two patients had symptoms of superficial peroneal nerve injury and recovered gradually after nerve nourishing therapy. Three patients mainfested slightly limits of flexion and extension of ankle joint. AOFAS score of ankle and hind foot at the latest follow up was (85.6±7.9), 9 patients got excellent results, 4 good and 2 fair.@*CONCLUSION@#Fix Tillaux-Chaput fracture block with dentate steel plate has advantages of easy operation, stable fixation, and is beneficial to recovery of ankle function. It is not necessary to fix tibiofibular syndesmosis with screws.

RESUMEN Introducción: El tratamiento de las fracturas del tobillo tiene importancia en la recuperación de la movilidad e independencia del paciente geriátrico. Objetivo: Evaluar el resultado obtenido al transcurrir un año del tratamiento quirúrgico mediante reducción y fijación de las fracturas inestables en pacientes mayores de 65 años. Métodos: Estudio descriptivo y prospectivo; pacientes mayores de 65 años, atendidos entre enero de 2017 y diciembre de 2018 por fractura de tobillo desplazada, en el Hospital Universitario General Calixto García Íñiguez y el Centro de Investigaciones en Longevidad, Envejecimiento y Salud, que fueron evaluados un año después. Resultados: De un total de 31 pacientes, 6 hombres y 25 mujeres con una edad media de 73,4 ± 3,5 años, 12 presentaban deterioro cognitivo, 18 llevaban terapia anticoagulante y 14 ingerían más de cuatro medicamentos diariamente. Predominaron los grupos con 3 y 4 puntos del índice de Charlson, la mayoría se ubicó en ASA III de riesgo anestésico y fracturas Dannis-Weber tipo C, 29 fracturas fueron cerradas. Las complicaciones fueron escasas; 27 de 31 pacientes se ubicaron como excelente o bueno según la escala AOFAS; 3 ± 2 en la escala visual de dolor; 86 por ciento podían realizar sus actividades de la vida diaria, Conclusiones: El resultado del tratamiento quirúrgico de las fracturas de tobillo en mayores de 65 años fue bueno y con baja incidencia de complicaciones que pueden relacionarse con condiciones mórbidas previas(AU)
ABSTRACT Introduction: The ankle fracture treatment is important in the recovery of mobility and independence of geriatric patients. Objective: To evaluate the results after one year of surgical treatment by reduction and fixation of unstable fractures in patients older than 65 years. Methods: A descriptive and prospective study was carried out in patients older than 65 years, treated from January 2017 to December 2018 due to displaced ankle fracture, at General Calixto García Íñiguez University Hospital and the Research Center for Health, Longevity and Aging. These patients were assessed a year later. Results: Out of a total of 31 patients, 6 men and 25 women with a mean age of 73.4 ± 3.5 years, 12 had cognitive impairment, 18 were on anticoagulant therapy and 14 were taking more than four medications daily. Groups with 3 and 4 points of Charlson index predominated, the majority were located in ASA III of anesthetic risk and Dannis-Weber type C fractures, 29 fractures were closed. Complications were rare and 27 out of 31 patients ranked as excellent or good according to the AOFAS scale, 3 ± 2 on the visual pain scale, 86 percent of performing activities of their daily life. Conclusions: The result of the surgical treatment of ankle fractures in people older than 65 years was good and with a low incidence of complications that may be related to previous morbid conditions(AU)

Objective: To explore the necessity of repairing the deep layer of deltoid ligament in the treatment of mixed medial injury associated with ankle fractures. Methods: Between January 2016 and December 2018, 12 patients with mixed medial injury associated with ankle fractures were treated with the fixation of the lateral malleolus by bone plates, the fixation of the anterior colliculus of medial malleolus by cannulated screws, and the repair of the deltoid ligament by suture anchors. There were 8 males and 4 females, with an average age of 42 years (range, 18-56 years). According to the Lauge-Hansen classification criteria, there were 11 cases of supination-external rotation type and 1 case of pronation-external rotation type. According to the Weber classification criteria, all cases were type B. The time from injury to operation was 3-6 days, with an average of 4.7 days. In each patient, X-ray films of anteroposterior and lateral views and mortise view of ankle were taken postoperatively. The motion range of ankle joints was observed. The function of the ankle and the outcome of the treatment were evaluated according to the American Orthopaedic Foot and Ankle Society (AOFAS) scoring system, Olerud-Molander scoring system, and the visual analogue scale (VAS) score. Results: All cases were followed up 12-42 months (mean, 28 months). The 12 patients returned to their pre-injury jobs. Five patients with sports injury completely recovered to their pre-injury motor function. No patient experienced persistent medial ankle pain or ankle instability. At last follow-up, the ankle range of motion in dorsiflexion was 9°-25° (mean, 17.96°), which was 0°-11° (mean, 4.02°) less than that in normal side; the range of motion in plantar flexion was 38°-50° (mean, 43.90°), which was 0°-7° (mean, 2.53°) less than that in normal side. The AOFAS score was 88-100 (mean, 96.7); the Olerud-Molander score was 90-100 (mean, 96.5); the VAS score was 0-3 (mean, 1.1). Conclusion: It is necessary to repair the deep layer of deltoid ligament in the mixed medial injuries associated with ankle fracture, which include anterior colliculus fracture and deep deltoid ligament injury. A better outcome can be achieved by employing the suture anchor repair method.

Objective: To investigate the safety, feasibility, and efficacy of full repair strategy under small incision in the treatment of closed Lauge-Hansen pronation-external rotation type Ⅳ ankle fracture. Methods: The clinical data of 57 patients with closed Lauge-Hansen pronation-external rotation type Ⅳ ankle fracture treated by full repair strategy (fracture, ligament, and cartilage repair) under small incision between January 2012 and January 2017 were retrospectively analyzed. There were 31 males and 26 females, with an average age of 41.1 years (range, 21-65 years). The causes of injury included traffic accident injury in 33 cases and falling injury in 24 cases. All of them were closed fractures, including 20 cases of medial malleolus fracture, 37 cases of complete medial malleolus but deep and shallow rupture of deltoid ligament. The average time from injury to admission was 9.6 hours (range, 3-34 hours). The quality of reduction of distal tibial articular surface (based on Ketz-Sanders standard), the reduction of tibiofibular syndesmosis (the anterior and posterior distances of distal tibiofibular syndesmosis and the lateral ankle twist angle measured by CT scan at 10 mm above the ankle joint line), and the fracture healing were evaluated. The medial clear space (MCS), tibiofibular clear space (TFCS), and distal fibular tip to lateral process of talus (DFTL) were measured on the X-ray films of ankle points. Before and after operation, the pain and functional improvement of ankle joint were evaluated by visual analogue scale (VAS) score and American Orthopedic Foot and Ankle Association (AOFAS) score, and the activities of ankle dorsiflexion and plantar flexion were measured. Results: Three cases with osteochondral lesions of the talus were found during operation and all were treated with microfracture techniques. Tournament paralysis occurred in 2 cases after anesthesia. The surgical incisions healed by first intention in all patients. All the 57 patients were followed up 24-84 months, with an average of 38.6 months. All patients achieved bone healing without bone nonunion and malunion at 12 months after operation. The reduction quality of distal tibial articular surface was excellent in 56 cases and good in 1 case at 3 months after operation, the excellent and good rate was 100%. There was no significant difference in the MCS, TFCS, DFTL, anterior distance of distal tibiofibular syndesmosis, posterior distance of distal tibiofibular syndesmosis, and lateral ankle twist angle between the affected and healthy sides at 12 months after operation ( P>0.05). At last follow-up, the VAS score, AOFAS score, ankle dorsiflexion and plantar flexion activities of the affected side were significantly improved when compared with preoperative ones ( P0.05). Conclusion: Full-repair strategy under small incisions for the treatment of Lauge-Hansen pronation-external rotation type Ⅳ ankle fracture is effective and safe. It can not only reduce wound complications, but also improve the quality of joint reduction of the ankle joint and distal tibiofibular syndesmosis. Full repair of ligaments and cartilage can improve the internal fixation strength and joint stability of the ankle joint.

RESUMO Nosso objetivo é descrever a técnica de fixação intramedular (IM) da fíbula no tratamento cirúrgico das fraturas maleolares do tornozelo. A redução é realizada sempre de forma percutânea: quando o traço de fratura é oblíquo simples, utilizamos uma pinça de redução de pontas e quando a fratura apresenta padrão multifragmentar, usa-se tração longitudinal e rotação. Isso reduz as complicações relacionadas à redução aberta e fixação interna com placa. A técnica demonstrou ser simples e reprodutível. Além disso, essa forma de fixação permite apoio precoce do peso corporal, o que acelera o processo de reabilitação e potencialmente acelera a cicatrização da fratura. A fixação IM da fíbula mostrou-se uma opção viável para a fixação do maléolo lateral nas fraturas do tornozelo, devendo ser considerada no arsenal do cirurgião.
ABSTRACT Our objective is to describe the technique of intramedullary (IM) nailing of lateral malleolus in the surgical management of ankle fractures. Fracture reduction is performed either percutaneously with a small pointed reduction clamp in simple oblique fractures or using longitudinal traction and rotation for comminuted fractures, thus reducing complications related to open reduction and internal fixation with a plate. The technique has been shown to be simple and reproducible. In addition, the technique allows early weight bearing, which accelerates rehabilitation and potentially fasten fracture healing. IM nailing is a viable option for the fixation of the of lateral malleolus in ankle fractures and should be considered in the surgeon's armamentarium.

Background@#Acute ankle fractures can lead to high rate of concomitant intra-articular lesions which may compromise clinical results. The purpose of this study was to evaluate the Prevalence of concomitant intra-articular lesions in acute ankle fractures with arthroscopy. We also sought to analyze the relationship between intra-articular lesions and the fracture type, as well as the severity of the fracture.@*Methods@#It was a retrospective cohort study. From April 2014 to December 2015, we have chosen arthroscopy-assisted open reduction and internal fixation (AORIF) for the treatment of unstable acute ankle fractures. All concomitant intra-articular lesions were assessed and documented carefully and prospectively, such as ligament injuries, osteochondral lesions, and tibiofibular syndesmosis injuries. All fractures were classified according to the Lauge-Hansen classification system. The American Orthopedic Foot and Ankle Society’s (AOFAS) ankle-hindfoot scale was used to assess post-operative function. Statistical comparisons between the intra-articular lesions, the fracture type, and the severity of the presenting fracture were performed using a Chi-squared analysis.@*Results@#Data of 36 patients were analyzed in the study, including 23 supination-type fractures and 13 pronation-type fractures. The incidence of tibiofibular syndesmosis injuries, chondral lesions, and loose bodies were 92%, 72%, and 39%, respectively. Avulsion fractures of the anterior tibiofibular syndesmosis were more commonly found in supination-type fractures than pronation-type fracture (45% vs. 15%, χ2 = 5.78, P = 0.02), which would cause mechanical blocking in the anterior portion of the ankle. On the contrary, chondral lesions were more commonly found in the more severe fractures than mild fractures (86% vs. 53%, χ2 = 4.57, P = 0.03). A mean 41.7 months (range, 33.0–51.0 months) of follow-up was achieved. A mean AOFAS’s ankle-hindfoot scale was 96.9, and 97.2% of the patients were satisfied with the procedure.@*Conclusions@#Acute ankle fractures have a high incidence of concomitant intra-articular lesions. Avulsion fractures of the anterior tibiofibular syndesmosis are more commonly found in supination-type fractures. Chondral lesions are related to the severity of the fractures, but not with the classification of the fractures. AORIF can be one reliable solution in dealing with the associated injuries seen with acute ankle fractures.

ABSTRACT Objective: This study evaluated the reproducibility of the three main classifications of ankle fractures most commonly used in emergency clinical practice: Lauge-Hansen, Danis-Weber, and AO-OTA. The secondary objective was to assess whether the level of professional experience influenced the interobserver agreement for the classification of this pathology. Methods: The study included 83 digitized preoperative radiographic images of ankle fractures, in anteroposterior and lateral views, of different adults that had occurred between January and December 2013. For sample calculation, the estimated accuracy was approximately 15%, with a sampling error of 5% and a sampling power of 80%. The images were analyzed and classified by six different observers: two foot and ankle surgeons, two general orthopedic surgeons, and two-second-year residents in orthopedics and traumatology. The Kappa statistical method of multiple variances was used to assess the variations. Results: The Danis-Weber classification indicated that 40% of the agreements among all observers were good or excellent, whereas only 20% of good and excellent agreements were obtained using the AO and Lauge Hansen classifications. The Kappa index was 0.49 for the Danis-Weber classification, 0.32 for Lauge Hansen, and 0.38 for AO. Conclusion: The Hansen-Lauge classification presented the poorest interobserver agreement among the three systems. The AO classification demonstrated a moderate agreement and the Danis-Weber classification presented an excellent interobserver agreement index, regardless of professional experience.
RESUMO Objetivo: Avaliar a reprodutibilidade e comparatividade das três principais classificações usadas para fraturas do tornozelo mais comumente empregadas nos servicos de emergência: Lauge-Hansen, Danis-Weber e AO-OTA. Observar secundariamente se existe influência da experiência profissional sobre a concordância entre observadores para a classificação dessa patologia. Métodos: Foram usadas 83 imagens digitalizadas de radiografias pré-operatórias, em incidências anteroposterior e perfil, de fraturas do tornozelo de adultos diferentes, ocorridas entre janeiro e dezembro de 2013. No cálculo amostral assumiu-se precisão da estimativa de 15%, com erro amostral de 5% e com poder de amostragem de 80%. A leitura e a classificação das fraturas foram feitas por seis observadores, dois cirurgiões de pé e tornozelo, dois ortopedistas generalistas e dois residentes do segundo ano de ortopedia e traumatologia. A análise das variações foi feita pelo método estatístico de Kappa de múltiplas variâncias. Resultados: Com o uso da classificação de Dannis-Weber, 40% das concordâncias foram consideradas boas ou excelentes entre todos os observadores, enquanto nas classificações de Lauge Hansen e AO apenas 20% se apresentaram boas ou excelentes. O índice Kappa acumulado para cada classificação foi de 0,49 para a classificação de Dannis-Weber, 0,32 para Lauge Hansen e 0,38 para a classificação AO. Conclusão: A classificação de Lauge Hansen apresenta a pior concordância interobservador dentre as três classificações. A classificação da AO demonstrou concordância intermediária e a de Dannis-Weber apresentou o maior índice de concordância interobservador, independentemente da experiência do profissional.

PURPOSE: A distal tibiofibular syndesmosis injury with an ankle fracture is usually fixed with syndesmotic screws. Knotless Tightrope® has been used as an alternative procedure because of the fewer reported complications. Therefore, this study compared the two surgeries. MATERIALS AND METHODS: Forty-two patients, who underwent syndesmotic screw fixation, and 34 patients, who underwent Knotless Tightrope® fixation for distal tibiofibular syndesmosis injury from February 2014 to February 2016, were analyzed retrospectively. The visual analogue scale (VAS) score, American Orthopaedic Foot and Ankle Society (AOFAS) score, range of motion of ankle at 1 year after surgery, tibiofibular clear space, and tibiofibular interval at preoperative, postoperative and 1 year after surgery were investigated. RESULTS: The VAS score, AOFAS score and radiographs were similar in the two groups. Knotless Tightrope® showed better results in complications and plantarflexion. CONCLUSION: Knotless Tightrope® fixation is a useful treatment that does not show a difference in fixation strength and clinical outcome. Knotless Tightrope® fixation also has an advantage in the range of motion and complications.
