ABSTRACT

A angina vasoespástica é uma causa incomum de parada cardíaca e arritmias ventriculares. No entanto, os sobreviventes dessas complicações têm um risco aumentado de recorrência, apesar da função ventricular normal e do tratamento médico otimizado. Descrevemos o caso de uma ex-tabagista de 50 anos que teve parada cardiorrespiratória secundária a vasoespasmo coronariano grave.


Vasospastic angina is an uncommon cause of cardiac arrest and ventricular arrhythmias. However, survivors of these complications are at an increased risk of recurrence, despite normal ventricular function and optimized medical therapy. We describe a case of a 50-year-old former smoker who developed cardiorespiratory arrest secondary to severe coronary vasospasm.

Resumo Fundamento A fibrilação atrial (FA) acomete cerca de 2% a 4% da população mundial. Nos pacientes internados em unidades de terapia intensiva, esta incidência pode chegar em até 23% naqueles com choque séptico. O impacto da FA nos pacientes sépticos se reflete em piores desfechos clínicos e o reconhecimento dos fatores desencadeantes pode ser alvo para estratégias de tratamento e prevenção futuras. Objetivos Verificar a relação entre desenvolvimento de FA e mortalidade nos pacientes acima de 80 anos incluídos no protocolo sepse e identificar fatores de risco que contribuam para o desenvolvimento de FA nesta população. Métodos Estudo observacional retrospectivo, com revisão de prontuários eletrônicos e inclusão de 895 pacientes com 80 anos ou mais, incluídos no protocolo sepse de um hospital privado de alta complexidade em São Paulo/SP, no período de janeiro de 2018 a dezembro de 2020. Todos os testes foram realizados com nível de significância de 5%. Resultados A incidência de FA na amostra foi de 13%. Após análise multivariada por regressão logística múltipla, foi possível demonstrar associação de mortalidade na população estudada, com o escore SOFA ( odds ratio [OR] 1,21 [1,09 - 1,35]), valores mais altos de proteína C-reativa (OR 1,04 [1,01 - 1,06]), necessidade de droga vasoativa (OR 2,4 [1,38 - 4,18]), uso de ventilação mecânica (OR 3,49 [1,82 - 6,71]) e principalmente FA (OR 3,7 [2,16 - 6,31). Conclusões No paciente grande idoso (80 anos ou mais) com sepse, o desenvolvimento de FA se mostrou como fator de risco independente para mortalidade intra-hospitalar.


Abstract Background Atrial fibrillation (AF) affects about 2% to 4% of the world population, and in patients hospitalized in intensive care units, this incidence can reach up to 23% in those with septic shock. The impact of AF in patients with sepsis is reflected in worse clinical outcomes, and the identification of the triggering factors can be a target for future prevention and treatment strategies. Objectives To verify the relationship between the development of AF and mortality in patients over 80 years of age included in the sepsis protocol and to identify the risk factors that contribute to the development of AF in this population. Methods Retrospective observational study, with a review of electronic medical records and inclusion of 895 patients aged 80 years or older, included in the sepsis protocol of a high-complexity private hospital in São Paulo, SP, from January 2018 to December 2020. All tests were performed with a significance level of 5%. Results The incidence of AF in the sample was 13%. After multivariate analysis, using multiple logistic regression, it was possible to demonstrate an association of mortality, in the studied population, with the SOFA score (odds ratio [OR] 1.21 [1.09 - 1.35]), higher values of C-reactive protein (OR 1.04 [1.01 - 1.06]), need for vasoactive drugs (OR 2.4 [1.38 - 4.18]), use of mechanical ventilation (OR 3.49 [1.82 - 6.71]), and mainly AF (OR 3.7 [2.16 - 6.31]) Conclusion In very elderly patients (80 years of age and older) with sepsis, the development of AF was shown to be an independent risk factor for in-hospital mortality.

ABSTRACT BACKGROUND: Coronary artery disease is an important cause of morbidity and mortality. The impact of ventricular arrhythmias with impaired cardiac vagal activity is one of the most recently studied prognostic factors. However, there are no studies evaluating the phenomenon of heart rate turbulence (HRT) during physical exertion. OBJECTIVE: To study the behavior of HRT during exercise testing, among individuals after myocardial infarction. DESIGN AND SETTING: Feasibility study conducted in a university hospital among individuals 4-6 weeks after myocardial infarction. METHODS: All subjects underwent 24-hour Holter monitoring and ergometric stress testing. We considered that abnormal HRT was present if the turbulence onset was ≥ 0% or turbulence slope was ≤ 2.5 mm/relative risk interval. RESULTS: All 32 subjects were asymptomatic. Their median age was 58 years (interquartile range 12.8) and 70% were male. Abnormal HRT was associated with ventricular dysfunction in this population. We found no differences regarding the behavior of HRT, in relation to age, gender, smoking, systemic arterial hypertension, diabetes mellitus or dyslipidemia. Ergometric stress testing detected premature ventricular beats (PVB) in approximately 44% of the examinations, and these occurred both during the active phase of effort and in the recovery period. The low occurrence of several isolated PVB in beta-blocked subjects made it difficult to perform statistical analysis to correlate HRT between ergometric and Holter testing. CONCLUSION: The data obtained in this study do not support performing HRT through ergometric stress testing among patients who remain on beta-blockers post-myocardial infarction, for the purpose of assessing cardiac vagal activity.

SUMMARY OBJECTIVE: Premature ventricular contraction is generally known as benign in the absence of structural heart disease; however, premature ventricular contraction-induced left ventricular systolic dysfunction or ventricular arrhythmias are defined in some cases. Ventricular repolarization duration differs between myocardial cells, which causes myocardial electrical heterogeneity and is thought to be responsible for ventricular arrhythmias. In our study, we aimed to evaluate the association of ventricular repolarization parameters including Tp-Te interval, Tp-Te/QT ratio, and QRS-T angle with premature ventricular contraction frequency in patients with premature ventricular contraction burden. METHODS: A total of 80 subjects who were admitted to our cardiology department and underwent 24-h electrocardiography Holter monitoring were included. Patients were divided into two groups: group 1 is defined as premature ventricular contraction burden that had frequent premature ventricular contraction ≥1% in 24-h Holter monitoring, and group 2 is defined as rare premature ventricular contraction <1% in 24-h Holter monitoring. RESULTS: Tp-Te interval and Tp-Te/QT ratio are statistically significantly prolonged in the premature ventricular contraction burden group than in the control group (85.3±13.9 vs. 65.7±11.9, p<0.001; 0.19±0.03 vs. 0.15±0.02, p<0.001, respectively). QRS-T angle was statistically significantly abnormal in the premature ventricular contraction burden group (p=0.024). CONCLUSION: Increased Tp-Te interval and widened QRS-T angle are associated with ventricular arrhythmias and might be used for the prediction of premature ventricular contraction burden in patients with premature ventricular contraction in electrocardiography in the absence of 24-h Holter monitoring.

Objetivo:Identificar o perfil dos pacientes submetidos à inserção de dispositivos cardíacos eletrônicos implantáveis em um hospital de referência do Rio Grande do Norte.Método:Trata-se de um estudo descritivo, exploratório, retrospectivo dos últimos 03 anoscom abordagem quantitativa. Amostragem foiselecionada por conveniência. A coleta de dados ocorreu entre outubrode 2017 a março de 2018.Resultados: A amostra foi composta por 215prontuários,destes 182 foram analisados.Implantarammarca-passos63,7% e 28% cardiodesfibriladores, corresponderam ao sexo masculino69,3%,com média de idade 65 e 49.Entre as comorbidades,a Hipertensão Arterial Sistêmicaabrangeu 87,4%.Evidenciaram-se os bloqueios atrioventriculares como principal causa para implantes (35%), seguidos das miocardiopatias (12%).Conclusão:Percebeu-se que, os indivíduos que implantaram marcapassos eram acometidos por bradiarritmias,enquanto cardiodesfibriladores por taquiarritmias. As complicações pós-operatórias e intercorrências durante o procedimento foram de baixo risco. Salienta-se, que no manejo desses usuários é fundamental a elaboração de um protocolo, visando aperfeiçoar o cuidado.(AU)

Objetivo: identificar el perfil de los pacientes sometidos a la inserción de dispositivos cardíacos electrónicos implantables en el hospital de referencia de Rio Grande do Norte. Método: se trata de un estudio descriptivo, exploratorio, retrospectivo de los últimos 3 años con enfoque cuantitativo. El muestreo fue seleccionado por conveniencia. La recolección de datos se llevó a cabo entre octubre de 2017 y marzo de 2018. Resultados: muestra compuesta por 215 historias clínicas, de las cuales se analizaron 182. Marcapasos implantados 63,7% y desfibriladores cardioversores 28%, correspondieron al sexo masculino 69,3%, con una edad media de 65 y 49 años. Entre las comorbilidades, la Hipertensión Arterial Sistémica cubrió el 87,4%. Los bloqueos auriculoventriculares se evidenciaron como la principal causa de implantes (35%), seguida de las miocardiopatías (12%). Conclusión: se notó que, los portadores de marcapasos se vieron afectados por bradiarritmias, mientras que los desfibriladores cardioversores por taquiarritmias. Las complicaciones postoperatorias y las complicaciones durante el procedimiento fueron de bajo riesgo. Cabe señalar que en el manejo de estos usuarios es fundamental la elaboración de un protocolo, con el fin de mejorar la atención.(AU)

ABSTRACT

La taquicardia supraventricular (TSV) es la principal taquiarritmia en el recién nacido (RN) que requiere una resolución urgente. Por su parte, la enterocolitis necrosante (ECN) es la emergencia gastrointestinal más común que afecta principalmente a RN prematuros. Aunque estas se reconocen como patologías distintas, la bibliografía sugiere que los episodios de TSV pueden predisponer a los pacientes a la ECN a través de alteraciones en el flujo sanguíneo mesentérico y una disminución de la perfusión tisular. Se presenta aquí el caso clínico de un neonato prematuro que desarrolló un cuadro de ECN luego de un evento aislado de TSV con bajo gasto cardíaco


Supraventricular tachycardia (SVT) is the main tachyarrhythmia in the newborn (NB) that requires urgent resolution. Necrotizing enterocolitis (NEC) is the most common gastrointestinal emergency that mainly affects premature infants. Although these conditions are recognized as distinct pathologies, literature reports suggest that episodes of SVT may predispose patients to NEC secondary to disturbances in mesenteric blood flow and a decrease in tissue perfusion. We present here the clinical case of a premature neonate who developed NEC after an isolated SVT event with low cardiac output

ABSTRACT

Objetivo: Describir las secuelas cardiacas en la fase post-COVID-19. Método: Se realizó una revisión bibliográfica de documentos científicos enfocados en salud específicamente en complicaciones cardíacas primordiales post COVID-19. Resultados: se seleccionaron 15 artículos científicos que van en el rango de publicación del 2020 - 2021 con aportaciones relevantes. Conclusión: El escenario clínico puede presentarse por lesión miocárdica, insuficiencia cardíaca y arritmias ocurridas como consecuencia de un evento coronario agudo isquémico. Es necesario seguir estudiando el mecanismo de infección, manifestaciones clínicas, complicaciones a la infección por SARS-CoV-2.


Objective: To describe the cardiac sequelae in the post-COVID-19 phase. Methods: A literature review of scientific papers focused on health specifically on primary cardiac complications post-COVID-19 was performed. Results: 15 scientific papers ranging in the publication range of 2020 - 2021 with relevant contributions were selected. Conclusion: The clinical scenario may present myocardial injury, heart failure and arrhythmias occurring as a consequence of an ischemic acute coronary event. It is necessary to further study the mechanism of infection, clinical manifestations, complications to SARS-CoV-2 infection.

ABSTRACT

Resumo Fundamento Os níveis de Proteína 3 relacionada ao fator de necrose tumoral/complemento sérico C1q (CTRP3) e a relação com a fibrilação atrial (FA) na doença arterial coronária estável (DAC) não estão claros atualmente. Objetivos O objetivo deste estudo foi investigar a mudança nos níveis séricos de CTRP3 e sua relação com a FA paroxística em DAC estável. Método O estudo incluiu 252 pacientes com DAC e 50 controles saudáveis com idade/sexo compatíveis. Os níveis séricos de CTRP3 foram medidos, além da anamnese de rotina, exame físico, exames laboratoriais e ecocardiograma. Os pacientes foram divididos em grupos com e sem DAC e indivíduos com DAC com e sem FA paroxística. Os valores eram estatisticamente significativos quando p<0,05. Resultados Os níveis séricos de CTRP3 foram significativamente menores em pacientes com DAC do que no grupo controle (p<0,001). A FA foi detectada em 28 pacientes (15,08%) no grupo DAC. A frequência de hipertensão e do sexo feminino, a proteína C reativa de alta sensibilidade (PCR-as), o nitrogênio ureico no sangue, os níveis de creatinina e o diâmetro diastólico do átrio esquerdo foram maiores (p<0,05 para cada um), e os níveis de CTRP3 foram mais baixos em pacientes com FA (p<0,001). Na análise de regressão logística, os níveis séricos de CTRP3 e os diâmetros diastólicos do átrio esquerdo foram independentemente determinados pelos pacientes com FA (p<0,01 para cada um). Nesta análise, observamos que cada 1 ng/mL de redução nos níveis de CTRP3 aumentou o risco de FA em 10,7%. Na análise ROC dos valores de CTRP3 para detectar pacientes com FA, a área da curva ROC para CTRP3 foi 0,971 (0,951-991) e considerada estatisticamente significativa (p<0,001). Quando o ponto de corte de CTRP3 foi considerado em 300 ng/mL, demonstrava a presença de FA com 87,9% de sensibilidade e 86,8% de especificidade. Conclusão Os níveis séricos de CTRP3 caíram significativamente em pacientes com DAC estável, e níveis reduzidos de CTRP3 estiveram relacionados à presença de FA paroxística nesses pacientes.


Abstract Background Serum Complement C1q/tumor necrosis factor-related protein-3 (CTRP3) levels and the relationship with atrial fibrillation (AF) in stable coronary artery disease (CAD) are not clearly known. Objective The aim of this study was to investigate the change in serum CTRP3 levels and its relationship with paroxysmal AF in stable CAD. Method The study included 252 patients with CAD and 50 age-sex matched healthy control subjects. Serum CTRP3 levels were measured in addition to routine anamnesis, physical examination, laboratory and echocardiography examinations. The patients were divided into groups with and without CAD and CAD patients with and without paroxysmal AF. Statistical significance was accepted as p<0.05. Results Serum CTRP3 levels were found to be significantly lower in patients with CAD than in the control group (p<0.001). AF was detected in 38 patients (15.08%) in the CAD group. The frequency of hypertension and female gender, hs-CRP, blood urea nitrogen, creatinine levels and left atrial end-diastolic (LAd) diameter were higher (p<0.05 for each one), and CTRP3 levels were lower in patients with AF (p <0.001). In the logistic regression analysis, serum CTRP3 levels and LAd diameters were independently determined the patients with AF (p<0.01 for each one). In this analysis, we found that every 1 ng/mL reduction in CTRP3 levels increased the risk of AF by 10.7%. In the ROC analysis of CTRP3 values for detecting patients with AF, the area under the ROC curve for CTRP3 was 0.971 (0.951-991) and was statistically significant (p<0.001). When the CTRP3 cut-off value was taken as 300 ng/mL, it was found to predict the presence of AF with 87.9% sensitivity and 86.8% specificity. Conclusion Serum CTRP3 levels were significantly reduced in patients with stable CAD and decreased CTRP3 levels were closely related to the presence of paroxysmal AF in these patients.

ABSTRACT

Objective:To investigate the effects of dexmedetomidine on the myocardial electrical conduction velocity and the expression and distribution of connexin 43 (Cx43) in rats.Methods:Healthy adult Sprague-Dawley rats of both sexes, weighing 270-330 g, were used.Twelve isolated rat hearts successfully perfused in the Langendorff apparatus were divided into 2 groups ( n=6 each) using a random number table method: control group (group C) and dexmedetomidine group (group D). The hearts were perfused for 15 min with K-H solution, and then the hearts were continuously perfused for 30 min with 37 ℃ K-H solution in group C and with K-H solution containing dexmedetomidine 50 ng/ml in group D. Programmed electrical stimulation was performed after the end of perfusion, the activation latency was recorded, and the electrical conduction velocity of myocardial tissues was calculated, and then the left ventricular myocardial tissues were obtained for determination of the expression and distribution of myocardial Cx43 protein by immunohistochemistry method. Results:Compared with group C, the activation latency was significantly prolonged, the electrical conduction velocity was reduced, and the expression of Cx43 was down-regulated in group D ( P<0.05). Cx43 protein was mostly distributed in intercalated discs at both ends of cells in group C, and there was a tendency for the proteins localized at end-to-end contact sites of ventricular cardiomyocytes to localize at side-to-side contact sites, and the distribution was messy in group D. Conclusions:Dexmedetomidine causes arrhythmia probably through down-regulating the expression of Cx43 protein, changing its distribution, and reducing myocardial electrical conduction velocity in rats.

Objective:To investigate the clinical value of echocardiography in the diagnosis of fetal arrhythmia.Methods:1 500 pregnant women who underwent a regular prenatal examination in Cangnan Hospital Affiliated to Wenzhou Medical University from March 2018 to March 2020 were included in this study. These pregnant women underwent fetal echocardiography in the second trimester of pregnancy (24-26 weeks). We evaluated the condition of, and calculated the incidence of, fetal arrhythmia complicated by fetal cardiac structure abnormality . The diameters of the descending aorta, umbilical artery, and middle cerebral artery were compared between normal fetuses and arrhythmia fetuses.Results:Fetal arrhythmia was found in 131 pregnant women who were in the second trimester of pregnancy, with an incidence of fetal arrhythmia of 8.73%. Fetal arrhythmia and cardiac structure abnormality were detected in 19 pregnant women, with an incidence of fetal arrhythmia and cardiac structure abnormality of 1.27%. Fetal arrhythmia was detected in 131 (8.73%) pregnant women who were in the second trimester of pregnancy and it was detected in 32 (2.13%) pregnant women who were in the third trimester of pregnancy. Arrhythmia was detected in 18 (1.2%) newborns. Only 5 (0.33%) fetuses died during the perinatal period. Four (0.27%) fetuses had arrhythmia complicated by cardiac structure abnormality. One (0.07%) fetus was normal. The diameters of the descending aorta and middle cerebral artery were greater in normal fetuses than in arrhythmia fetuses in pregnant women who were in the second trimester of pregnancy, and the diameter of the umbilical artery was significantly smaller in normal fetuses than in arrhythmia fetuses ( t = -8.27-19.62, all P < 0.001). Conclusion:Echocardiography can effectively help diagnose fetal arrhythmia and observe abnormal cardiac structure. The imaging technique is of great clinical value in improving the diagnosis and treatment of fetal arrhythmia.
