ABSTRACT

ABSTRACT Melatonin (MLT) is a hormone that can stimulate bone formation and inhibit bone resorption, among other functions. Aim To evaluate the effect on new bone formation of MLT applied locally to critical defects created in the calvaria of rats, compared to the effect of Bio-Oss® xenogeneic bone substitute (BO), by analyzing histomorphometry, microtomography and gene expression. Materials and Method Two critical defects (5.0 mm in diameter) were created in the calvaria of 36 adults male Wistar rats. The rats were divided randomly into two groups: a test group, in which one of the defects was filled with MLT, and the other with MLT with Bio-Oss® (MLTBO), and a control group, in which one of the defects was filled only with the clot (C), and the other with BO. The rats were euthanized 30 days after surgery. Samples of the calvaria containing the critical defects were collected for analysis by histomorphometry, microtomography, and the expression of the genes for type I collagen (COL-I), osteopontin (OPN) and bone morphogenetic protein 2 (BMP-2). Results A qualitative improvement was observed in bone healing when MLT was used, though there was no statistical difference in the quantification of newly formed bone (p>0.05). Micro-CT showed that bone volume was significantly smaller in absence of BO (p=0.006). Bone trabeculae thickness (p=0.590) and number (p=0.150) were not significantly affected by MLT. Regarding the expression of the genes COL-I, OPN and BMP-2, no significant differences were observed between the MLT, BO and MLTBO groups. Conclusion Topical application of MLT resulted in a qualitative improvement in bone healing, although it did not affect bone formation quantitatively. In the absence of BO, less bone volume and less bone trabecular thickness were observed.


RESUMO A melatonina (MLT) é um hormônio sintetizado e secretado pela glândula pineal, e que, dentre outras atividades e funções, tem capacidade de estimular a formação e inibir a reabsorção óssea. Objetivo avaliar o efeito da aplicação local do hormônio MLT na neoformação óssea, em defeitos críticos produzidos na calvária de ratos, por meio de análise histomorfométrica, microtomográfica e de biologia molecular, comparando-a com um substituto ósseo de origem xenogênica (Bio-Oss®). Materiais e Método foram utilizados 36 ratos Wistar machos adultos, nos quais foram produzidos dois defeitos críticos de 5,0 mm de diâmetro cada, que receberam diferentes tratamentos alocados de forma randomizada: no grupo controle, os animais tiveram um dos defeitos preenchidos apenas com coágulo sanguíneo (C) e o outro com substituto ósseo xenógeno (Bio-Oss®, BO); no grupo teste, um dos defeitos foi preenchido apenas com MLT e, o outro, recebeu a associação da MLT ao material sintético xenógeno (MLTBO). Todos os animais foram eutanasiados após 30 dias do pós-operatório. As amostras das calotas contendo os defeitos críticos foram coletadas para análises histomorfométricas, microtomográficas e da expressão gênica de colágeno do tipo I (COL-I), osteopontina (OPN) e proteína óssea morfogenética 2 (BMP-2), por meio de PCR em tempo real. Resultados Após análise dos dados pode-se observar que não houve diferença estatística na quantificação de neoformação óssea (p>0.05), porém, melhora qualitativa foi observada na cicatrização, quando a MLT foi utilizada. Quanto aos par®metros microtomográficos, foi observado que com ou sem MLT, o volume ósseo foi significativamente menor na ausência de BO. A espessura (p = 0,590) e número (p = 0,150) de trabéculas não foram significativamente afetados pelo uso da MLT. Quanto à expressão gênica de COL-I, OPMN e BMP-2, não foram observadas diferenças significativas entre os grupos MLT, BO e MLTBO. Conclusão conclui-se que a aplicação tópica de MLT, associada ou não ao BO não afetou quantitativamente a neoformação óssea, porém resultou em uma melhora qualitativa na cicatrização. Adicionalmente, na ausência de BO foi observada menor volume ósseo e menor espessura das trabéculas.

SUMMARY: A Study on Relationship between Single-Slice Hounsfield Unit(HU) value of the Chinese proximal humerus and Bone Mineral Density(BMD) Using Routine Chest CT and Dual-energy X-ray Absorptiometry(DEXA) was performed. Data were collected from 240 individuals who underwent DEXA and routine chest CT scans (including full images of the proximal humerus) on the same day at 967 Hospitals between January 2019 and December 2021. The method of measuring single-slice HU values of the proximal humerus on routine chest CT scans exhibited high reliability and repeatability (intraclass correlation coefficient > 0.961, P < 0.001). A strong positive correlation was observed between single-slice HU values of the proximal humerus and DEXA results, with the 20-mm HU value demonstrating the highest correlation. Across different BMI groups, the Area Under Curve (AUC) for the 20-mm HU value was consistently the largest (AUC=0.701- 0.813, P< 0.05). Therefore, the 20-mm HU value can be considered a reliable reference for the opportunistic screening of low BMD, with reference values of -4HU for underweight individuals, -13HU for normal weight individuals, -7HU for overweight individuals, and -16HU for obese individuals. Values below these thresholds indicate a risk of low BMD. This study enriches the Chinese BMD data and offers a swift and effective approach for opportunistically screening low BMD.


Se realizó un estudio sobre la relación entre el valor de la Unidad Hounsfield (HU) de corte único del húmero proximal chino y la densidad mineral ósea (DMO) mediante TC de tórax de rutina y absorciometría de rayos X de energía dual (DEXA). Se recopilaron datos de 240 personas que se sometieron a DEXA y tomografías computarizadas de rutina de tórax (incluidas imágenes completas del húmero proximal) el mismo día en 967 hospitales entre enero de 2019 y diciembre de 2021. El método para medir los valores de HU de un solo corte del húmero proximal en las tomografías computarizadas de tórax mostraron alta confiabilidad y repetibilidad (coeficiente de correlación intraclase > 0,961, P < 0,001). Se observó una fuerte correlación positiva entre los valores de HU de un solo corte del húmero proximal y los resultados de DEXA, demostrando el valor de HU de 20 mm la correlación más alta. En diferentes grupos de IMC, el área bajo la curva (AUC) para el valor HU de 20 mm fue consistentemente el más grande (AUC = 0,701-0,813, P <0,05). Por lo tanto, el valor de HU de 20 mm puede considerarse una referencia fiable para el cribado oportunista de DMO baja, con valores de referencia de -4 HU para personas con bajo peso, -13 HU para personas con peso normal, -7 HU para personas con sobrepeso y -16 HU para personas obesas. Los valores por debajo de estos umbrales indican un riesgo de DMO baja. Este estudio es un aporte para los datos chinos sobre la DMO y ofrece un enfoque rápido y eficaz para detectar de forma oportunista la DMO baja.

Resumen Introducción: diversos estudios describen los diferentes beneficios de la práctica de actividad física en niños y adolescentes; su déficit en estos grupos etarios, provoca un aumento concomitante de alteraciones cardiovasculares y metabólicas. Objetivo: analizar los patrones de actividad física, la maduración biológica y el contenido mineral óseo en escolares colombianos entre 8 y 16 años de edad en función del estrato socioeconómico al que pertenecen. Materiales y métodos: estudio de enfoque cuantitativo, descriptivo, transversal con alcance correlacional el cual incluyó una muestra representativa para escolares colombianos entre 8-16 años de edad a los cuales se les valoró el nivel de actividad física a través del PAQ-C, igualmente, se evaluaron diferentes medidas antropométricas y de manera indirecta se determinó la velocidad pico de crecimiento (VPC) y la densidad mineral ósea (DMO). Resultados: se evaluaron un total de 2147 escolares de los cuales el 56,7 % pertenecían a instituciones educativas del sistema público, se encontraron asociaciones estadísticamente significativas entre el sexo y el estrato socioeconómico con el nivel de actividad física (p<0,005) y las demás variables de estudio. Conclusión: se puede concluir que la actividad física, la maduración biológica y la salud ósea guardan estrecha relación con la condición socioeconómica en escolares colombianos lo que permite establecer diagnósticos tempranos y toma de decisiones frente a programas y estrategias educativas y de salud pública.


Abstract Introduction: Several studies describe the different benefits of physical activity in children and adolescents. Indeed, when a deficit of physical activity is evident in these age groups, there is a concomitant increase in metabolic and metabolic disorders. Objective: to analyze the patterns of physical activity, biological maturation and bone mineral content in Colombian schoolchildren between 8 and 16 years of age according to the socioeconomic status to which they belong. Materials and methods: A quantitative, descriptive, cross-sectional study with a correlational scope, which included a representative sample of Colombian schoolchildren between 8 and 16 years of age, who were assessed for their level of physical activity through the PAQ-C, also, different anthropometric measurements are evaluated and indirectly the peak growth velocity (PSV) and bone mineral density (BMD) are prolonged. Results: a total of 2147 schoolchildren were evaluated, of which 56.7% belonged to educational institutions of the public system, statistically significant associations were found between sex and socioeconomic status with the level of physical activity and the other study variables. Conclusion: It can be concluded that physical activity, biological maturation and bone health are closely related to socioeconomic status in Colombian schoolchildren, which allows early diagnoses and decision-making regarding educational and public health programs and strategies.

De fato, quando um déficit de atividade física é evidenciado nessas faixas etárias, ocorre um aumento concomitante de distúrbios cardiovasculares e metabólicos. Objetivo: analisar os padrões de atividade física, maturação biológica e conteúdo mineral ósseo em escolares colombianos entre 8 e 16 anos de idade de acordo com o status socioeconômico ao qual pertencem. Materiais e métodos: Um estudo quantitativo, descritivo, transversal com escopo correlacional, que incluiu uma amostra representativa de escolares colombianos entre 8 e 16 anos de idade, que foram avaliados quanto ao seu nível de atividade física por meio do PAQ-C, além disso, diferentes medidas antropométricas são avaliadas e, indiretamente, a velocidade máxima de crescimento (PSV) e a densidade mineral óssea (DMO) são prolongadas. Resultados: Um total de 2147 escolares foram avaliados, dos quais 56,7% pertenciam a instituições educacionais do sistema público, foram encontradas associações estatisticamente significativas entre sexo e status socioeconômico com o nível de atividade física e as outras variáveis do estudo. Conclusão: Pode-se concluir que a atividade física, a maturação biológica e a saúde óssea estão intimamente relacionadas ao status socioeconômico em escolares colombianos, o que permite diagnósticos precoces e tomada de decisões em relação a programas e estratégias educacionais e de saúde pública.

ABSTRACT

SUMMARY: Diabetes is a form of endocrine disease. Dual-energy X-ray Absorptiometry (DXA) provides a detailed view of the body composition to find out what makes people with diabetes different from those with other diseases. We scanned 371 patients with DXA to analyze their body composition parameters. Three hundreds and seventy one patients (178 women/193 men), who with different diseases, with a mean±SD Body Mass Index (BMI) of 25.32±8.3 kg/m2 were included. The body composition of 371 patients was assessed. Bone Mineral Density (BMD), Fat Weight, Lean Weight, waist-to-hip ratio, Lean Mass Index (LMI), Fat Mass Index (FMI), the relationship between Fat percentage and BMI were analyzed. The 371 patients included 156 diabetics and 215 non-diabetics. Non-diabetic patients also included 5 obesity patients, 9 patients with fatty liver, 39 patients with hypertension, 22 patients with hyperlipidemia, 18 patients with cardiovascular disease, 11 patients with chest and lung disease, 4 patients with chronic disease, 14 patients with brain disease and 93 patients with other diseases. Among 156 diabetic patients, 129 had VAT > 100 cm2 and 27 had VAT ≤100 cm2. The lean weight (LW) of male diabetic patients was significantly higher than that of female diabetic patients. The fat weight (FW) of female patients with diabetes was significantly higher than that of male patients. The waist-hip ratio (WHR) was 1.37 ± 0.25 in male diabetic patients and 1.18 ± 0.21 in female diabetic patients. Among the 215 non-diabetic patients, the obese and fatty liver patients, which the weight (WT) (obesity: 83.87 ± 8.34 kg fat liver: 85.64±28.60 kg), FW (obesity: 28.56 ± 4.18 kg fat liver: 28.61 ± 10.79 kg), LW (obesity: 52.62 ± 9.64 kg fat liver: 54.29±17.58 kg), BMI (obesity: 28.76 ± 1.88 kg/m2 fat liver: 29.10 ± 5.95 kg/m2), was much higher than other patients. Diabetes patients had less fat mass than non- diabetic patients; the difference was around 2 kg. BMI is also a modest number. BMD doesn't differ all that much. Non-diabetic patients with fatty liver obesity and cardiovascular disease had higher fat mass and BMI than patients with other illnesses. Body composition can provide precise information on the makeup of different body areas, but further in-depth exams are required to ascertain the body's endocrine profile.


La diabetes es una enfermedad endocrina. La absorciometría de rayos X de energía dual (DXA) proporciona una vista detallada de la composición corporal para descubrir qué diferencia a las personas con diabetes de aquellas con otras enfermedades. Escaneamos a 371 pacientes con DXA para analizar sus parámetros de composición corporal. Se incluyeron 371 pacientes (178 mujeres/193 hombres), con diferentes enfermedades, con un Índice de Masa Corporal (IMC) medio ± DE de 25,32 ± 8,3 kg/m2. Se evaluó la composición corporal de 371 pacientes. Se analizaron la densidad mineral ósea (DMO), el peso graso, el peso magro, la relación cintura-cadera, el índice de masa magra (LMI), el índice de masa grasa (FMI), y la relación entre el porcentaje de grasa y el IMC. De los 371 pacientes 156 eran diabéticos y 215 no diabéticos. Los pacientes no diabéticos también incluyeron 5 con obesidad, 9 con hígado graso, 39 con hipertensión, 22 con hiperlipidemia, 18 con enfermedad cardiovascular, 11 con enfermedad torácica y pulmonar, 4 con enfermedad crónica, 14 con enfermedad cerebral y 93 pacientes con otras enfermedades. Entre los 156 pacientes diabéticos, 129 tenían un IVA > 100 cm2 y 27 tenían un IVA ≤100 cm2. El peso magro (PV) de los hombres diabéticos fue significativamente mayor que el de las mujeres diabéticas. El peso graso (FW) de las mujeres diabéticas fue significativamente mayor que el de los hombres diabéticos. El índice cintura-cadera (ICC) fue de 1,37 ± 0,25 en hombres diabéticos y de 1,18 ± 0,21 en mujeres diabéticas. Entre los 215 pacientes no diabéticos, los pacientes obesos y con hígado graso, cuyo peso (WT) (obesidad: 83,87 ± 8,34 kg hígado graso: 85,64 ± 28,60 kg), FW (obesidad: 28,56 ± 4,18 kg hígado graso: 28,61 ± 10,79 kg), PV (obesidad: 52,62 ± 9,64 kg, hígado graso: 54,29 ± 17,58 kg), IMC (obesidad: 28,76 ± 1,88 kg/m2, hígado graso: 29,10 ± 5,95 kg/m2), fue mucho mayor que otros pacientes. Los pacientes diabéticos tenían menos masa grasa que los pacientes no diabéticos; la diferencia fue de alrededor de 2 kg. La DMO no difiere mucho. Los pacientes no diabéticos con obesidad debido al hígado graso y enfermedades cardiovasculares tenían mayor masa grasa e IMC que los pacientes con otras enfermedades. La composición corporal puede proporcionar información precisa sobre la composición de diferentes áreas del cuerpo, pero se requieren exámenes más profundos para determinar el perfil endocrino del cuerpo.

ABSTRACT

The objective of this study was to assess the efficacy of whole-body vibration (WBV) on bone mineral density (BMD), pain levels, and body composition in postmenopausal women with osteoporosis (PMOP). Relevant studies were retrieved from the PubMed, EMBASE, Web of science, CENTRAL, and PEDro databases. Thirteen randomized controlled trials with 783 patients were enrolled. The meta-analysis results showed that WBV can significantly increase lumbar spine BMD (WMD=0.018; 95%CI: 0.004 to 0.032; P=0.011), femoral neck BMD (WMD=0.005, 95%CI: 0.001 to 0.011, P=0.0493), and reduce pain degree (WMD=-0.786; 95%CI: -1.300 to -0.272; P=0.0027) in PMOP, but has no significant effect on patients' muscle mass (WMD=0.547; 95%CI: -1.104 to 2.199; P=0.5158) as well as fat mass (WMD=0.530; 95%CI: -2.389 to 3.448; P=0.7222). To conclude, WBV showed the potential to provide positive benefits in improving BMD and relieving pain of PMOP.

ABSTRACT

Objective To analyze the clinical risk factors for chronic complications in patients with type 2 diabetes and their correlation with bone mineral density and 1,25-dihydroxyvitamin D3. Methods A total of 163 patients with type 2 diabetes mellitus were selected as research subjects and were divided into complication group and non-complication group according to the presence or absence of chronic complications. The independent related factors for chronic complications in patients with type 2 diabetes mellitus were analyzed. Spearman rank correlation analysis was used to evaluate the correlation between bone mineral density, 1,25-dihydroxyvitamin D3 and chronic complications. Results Among the 326 patients with type 2 diabetes mellitus, 202 developed chronic complications (61.96%), including 71 cases of cardiovascular disease, 59 cases of neuropathy, 33 cases of renal lesion, and 28 cases of retinopathy. There were statistically significant differences in the duration of diabetes mellitus, fasting blood glucose, systolic blood pressure, glycosylated hemoglobin, triglyceride, low density lipoprotein cholesterol, serum creatinine, bone mineral density, and 1,25-dihydroxyvitamin D3 between the complication group and the non-complication group (P<0.05). Logistic multivariate regression analysis showed that the duration of diabetes mellitus, systolic blood pressure, glycosylated hemoglobin, ow density lipoprotein cholesterol, serum creatinine, bone mineral density, and 1,25-dihydroxyvitamin D3 were all independent related factors for the occurrence of chronic complications in patients with type 2 diabetes mellitus (P<0.05). Spearman correlation analysis showed that bone mineral density and 1,25-dihydroxyvitamin D3 were negatively correlated with chronic complications (P<0.05). Conclusion Bone mineral density and 1,25-dihydroxyvitamin D3 in patients with type 2 diabetes mellitus are closely related to chronic complications.

Objective To investigate the efeects of microRNA(miR)-103a-3p regulates tumor protein 53-regulated inhibitor of apoptosis 1(TRIAP1)on osteoblast differentiation and bone mass in ovariectomized mice.Methods MC3T3-E1 cells were divided into normal group,miR-103a-3p-NC group,miR-103a-3p mimic group,miR-103a-3p mimic+TRIAP1-NC group,miR-103a-3p mimic+TRIAP1 mimic group.mRNA expression of miR-103a-3p,TRIAP1,P53 were detected by Real-time PCR;Cell proliferation and apoptosis were detected by MTT test and flow cytometry;cytoskeleton and mineralization of cells were detected by F-actin immunofluorescence staining and alizarin staining;alkaline phosphatase(ALP)activity was detected by ELISA.24 female mice were divided into sham group,osteoporosis(OP)group,miR-103a-3p antagonist-NC group,miR-103a-3p antagonist group(six in each group),extract bilateral ovaries to establish an OP model,sham group mice only isolated fat around ovarian tissue.mRNA expression of miR-103a-3p,TRIAP1,P53,ALP,osteocalcin(OCN),osteopontin(OPN)of bone tissue were detected;microCT detect bone mineral density(BMD),bone mineral content(BMC);haematoxylin eosin staining was used to observe pathological changes of bone tissue.Results After miR-103a-3p mimic was transfected into cells,the miR-103a-3p and P53 expression increased,TRIAP1 expression decreased,cell proliferation decreased,apoptosis increased,F-actin expression decreased,the number of calcium nodules decreased,and ALP enzyme activity decreased(P<0.01);however,after TRIAP1 mimic was additionally transfected into cells,the above result caused by miR-103a-3p mimics were significantly reversed(P<0.01).In OP group,the miR-103a-3p and P53 expression in bone tissue increased,the TRIAP1,ALP,OCN and OPN expression decreased,BMD and BMC were decreased,and bone tissue construct was damaged(P<0.05);in miR-103a-3p antagonist group,the miR-103a-3p and P53 expression in bone tissue decreased,TRIAP1,ALP,OCN,OPN expression increased,BMD and BMC increased,and bone tissue construct was improved(P<0.05).Conclusion MiRNA-103a-3p mediate TRIAP1/P53 to inhibit proliferation and mineralization of osteoblast,while miR-103a-3p antagonistic treatment reduce bone loss in OP mice.

Objective To investigate the correlation between bone mineral density(BMD)at different hip positions with muscle parameters and physical performance in middle-aged and elderly people in Kunming area.Methods 531 middle-aged and elderly volunteers were recruited from the Radiology Department of the First People's Hospital of Yunnan Province from May 2021 to April 2022.All study subjects completed the five-times-sit-to-stand test(FTSST)and hip quantitative CT(QCT)examinations.Volunteers'total hip(TH),femoral neck(FN),and intertrochanteric(IT)BMD were measured by using QCT PRO workstation and using OsiriX software to measure the area and density of their gluteus maximus muscle,gluteus medius,and minimus muscle and midthigh muscle.Divide male and female volunteers into positive and negative groups respectively based on FTSST time≥12 seconds or<12 seconds,and analyze the differences in hip BMD between the groups;Also divide male and female volunteers into three groups(50~59 years old,60~69 years old,70 years old and above)at the age of 10,and analyze the correlation between hip BMD and muscle parameters in different age and gender stratification.Control for age and BMI,and then perform partial correlation analysis on the above indicators.Results The BMD of the hip in the female FTSST positive group was lower than that in the negative group(P<0.001),while there was no statistically significant difference between the male groups(P>0.05).After adjusting for age and BMI,among males,FN BMD was positively correlated with gluteus medius and minimus muscle density in the age groups of 50~59 and 60~69(P<0.05),while TH BMD,FN BMD,and IT BMD were strongly positively correlated with gluteus medius and minimus muscle density in the age group over 70(P<0.05).For females,the correlation between hip BMD and muscle density in the age groups of 50~59 and 60~69 was weak,while BMD in all parts of the hip was not correlated with muscle density in the age group over 70(P>0.05).There was a negative correlation between TH BMD,FN BMD,and gluteus medius and minimus muscle area in the age group of 50~59 years old for males(P<0.05),while there was a significant negative correlation between TH BMD,IT BMD,and gluteus maximus area in the age group of 70 years old and above for females(P<0.05).Conclusion The BMD of various parts of the hip in the female FTSST positive group is lower than that in the negative group.The density of the gluteus medius and minimus muscle can to some extent serve as a predictive indicator of femoral neck bone strength in middle-aged and elderly men in Kunming area.

Objective To explore the influencing factors of bone mineral density (BMD) in obese children in Qianjiang area and analyze the correlation between BMD and insulin resistance. Methods The data on pediatric cases from the outpatient department of Jianghan Oilfield General Hospital in Qianjiang from January 2018 to December 2022 were collected. A total of 183 obese children who met the inclusion and exclusion criteria were included in the study and selected in the observation group. A total of 352 children undergoing physical examination during the same period were selected as the control group. Results The body mass, waist circumference, waist to hip ratio, and BMI of obese children were significantly higher than those of the control group (P<0.001). Biochemical indexes including FBG, FINS, Home-IR, ALP, and LDL-C in obese children were significantly higher than those in the control group (P<0.05), while bone mineral density, Ca, P, sOC and HDL-C were significantly lower than those in the control group (P<0.001). The bone mineral density of obese children was significantly correlated with their exercise intensity, sunshine exposure duration, sitting time, intake of milk and dairy products, intake of sweets, supplementation of trace elements, BMI, Home-IR, and sex (all P<0.05). BMI, Home-IR, sex, exercise intensity, and sunshine exposure length were independent risk factors affecting bone mineral density of obese children (all P<0.05). Bone mineral density was negatively correlated with BMI and Home-IR (P=0.028 and0.017, respectively), and positive correlation with exercise intensity and sunlight exposure (P=0.033). Conclusion BMD of obese children in Qianjiang area is affected by gender, body mass index, diet, vitamin intake, and physical activity, and is negatively correlated with insulin resistance. Home-IR can be used as a reference for screening BMD of obese children.

ABSTRACT

Thyroid hormones are crucial for energy metabolism. Thyroid dysfunction is closely related to a variety of metabolic disorders. However, evaluation relying solely on thyroid function indicators may come up short, considering the complex relationship between thyroid hormones and metabolic issues. There has been a growing recognition of sensitivity to thyroid hormones as a measure of thyroid function complementary to traditional indices. The indicators of thyroid hormone sensitivity include free triiodothyronine/free thyroxine, thyrotroph thyroxine resistance index, thyroid-stimulating hormone index and thyroid feedback quantile-cased index. It has been reported that impaired sensitivity to thyroid hormones can potentially interact with various nutritional imbalances and metabolic abnormalities, such as metabolic syndrome, osteoporosis and decreased vitamin D, which are not only of concern to those with thyroid dysfunction, but also to euthyroid individuals in terms of prevention and prophylaxis. With the aim of providing comprehensive insights, this review is intended to systematically summarize the existing evidence on the association between sensitivity to thyroid hormones and metabolic disorders.

ABSTRACT

【Objective】 To investigate the level of serum bone metabolism and biochemical markers and bone density of plasmapheresis donors, and to provide scientific basis for ensuring the health and safety of plasmapheresis donors in China. 【Methods】 A total of 437 plasmapheresis donors from Linwu plasmapheresis station in Hunan Province from July 1 to September 30, 2022 were recruited to determine the levels of total serum calcium, albumin, serum 25-hydroxyvitamin D (25OHD), serum type I procollagen N-terminal propeptide (P1NP), and collagen type 1 crosslinked carboxyl-terminal peptide (β-CTX). Dual-energy X-ray method was used to measure the bone density of the anteroposterior lumbar spine (L1-L4) and bilateral femoral neck bone density of plasmapheresis donors. Plasmapheresis donors were grouped according to the type of plasma donation (first-time and repeat plasmapheresis donors) and the total number of plasma donations to assess the differences in bone density and serum bone metabolism biochemical markers between groups. The dose-response relationship between the total number of plasmapheresis donations and biochemical indexes was analyzed by limiting cubic spline, and the influencing factors of different indexes were explored by multiple linear regression. 【Results】 A total of 437 plasmapheresis donors were included in this study, including 187 first-time plasmapheresis donors and 250 repeat plasmapheresis donors. There were no significant differences in bone density and prevalence of osteoporosis between first-time donors and repeat donors (P>0.05). There was also no significant difference in bone density levels between groups of total number of plasmapheresis donations. The levels of albumin and 25OHD decreased with the increase of the total number of plasma donations, while the serum P1NP level was positively correlated with the total number of plasma donations. The results of the restriction cubic spline showed that the total number of plasmapheresis donations had a nonlinear dose-response relationship with 25OH and P1NP (P<0.05). The results of multiple linear regression showed that the frequency of plasmapheresis donation was the influencing factor of 25OHD, and the total number of plasmapheresis donation was the influencing factor of P1NP. 【Conclusion】 Plasmapheresis donation does not affect the bone health of donors and increase the risk of osteoporosis due to the use of long-term anticoagulants, but it will increase the osteogenic activity of plasmapheresis donors. It is recommended that middle-aged and elderly plasmapheresis donors supplement vitamin D appropriately.

ABSTRACT

Objective:To explore the association of hemoglobin(HGB) levels with bone mineral density(BMD) and osteoporosis in patients with type 2 diabetes mellitus(T2DM).Methods:A cross-sectional study was conducted in 364 patients with T2DM who were hospitalized in the Department of Endocrinology and Geriatrics of the Affiliated Huaian No. 1 People′s Hospital of Nanjing Medical University from September 2019 to September 2020. Participants were stratified into tertiles(lower, middle, and upper) according to femoral BMD determined by dual-energy X-ray absorptiometry. Demographic characteristics, medical history, chronic diabetes complications, and comorbid conditions were compared among the 3 groups. The association between hemoglobin levels and BMD/osteoporosis was examined using multivariable logistic regression analyses. Interaction and stratified analyses were conducted according to age, body mass index(BMI), duration of diabetes, estimated glomerular filtration rate(eGFR), glycosylated hemoglobin(HbA 1C), total cholesterol(TC), triglycerides(TG), high-density lipoprotein-cholesterol(HDL-C), low-density lipoprotein-cholesterol(LDL-C) and uric acid(UA). Results:After adjusting for age, BMI, and duration of diabetes, there were no significant differences observed in the association between hemoglobin levels and BMD or osteoporosis among postmenopausal women with T2DM(all P>0.05). After adjusting for age, BMI, duration of diabetes, and eGFR, men aged≥50 years with hemoglobin≥130 g/L showed a positive association between hemoglobin level and femoral neck BMD compared to those with hemoglobin<130 g/L( β=0.057, 95% CI 0.014-0.100, P=0.011). However, no significant associations were observed between hemoglobin level and BMDs at the total hip or lumbar spine(L1-L4), nor the risk of osteoporosis(all P>0.05). Stratified analyses revealed no significant differences in the subgroups classified based on age, BMI, diabetes duration, eGFR, HbA 1C, TC, TG, HDL-C, LDL-C, and UA(all interaction P>0.05). Conclusion:In males aged 50 and above with T2DM, elevated hemoglobin levels may be a protective factor for femoral neck bone density.

Bone mineral density is an important indicator of bone nutrition. The bone mineral density of children gradually increases with age, and the accumulation of bone mineral density in adolescence increases significantly, which is the most sensitive period to affect the peak bone mass. Sex hormone promotes bone formation and bone mineral density acquisition, and the peak secretion of growth hormone and insulin-like growth factor-1 increases bone mineral density accumulation. Children with precocious puberty develop earlier, and the length of puberty is shortened, resulting in changes in bone mass accumulation. GnRHa(gonadotrophin releasing hormone analogue) treatment of precocious puberty also affects bone metabolism. The level of 25-(OH)D3 in children varies with age in a U-shaped curve, which is the lowest in adolescence. Changes in body composition such as bone mass in adolescence have a long-term impact on bone health and even adult physique.

ABSTRACT

【Objective】 To analyze the contributing factors of attention deficit hyperactivity disorder (ADHD) in children and the correlation between bone mineral density and physical growth, in order to provide new clues for the prevention and early intervention of ADHD. 【Methods】 A total of 116 children with ADHD were included into the observation group from June 2020 to June 2022, while another 80 healthy children in the same period were included as the control group.Clinical data of the two groups were compared, and the factors influencing ADHD in children were analyzed using multivariate Logistic regression. Children with ADHD were further divided into boys group and girls group based on gender. Bone mineral density (BMD) and physical growth (height and body weight) of the two groups were measured, and the correlation between the two was analyzed. 【Results】 In the observation group, the proportion of boys, maternal bad behaviors during pregnancy, their educational level below high school, critical parenting, and parental stress index were significantly higher compared to the control group (χ2=14.430, 5.689, 5.630, 6.738, t=6.936, P<0.05). Additionally, family environment score was significantly lower than that in the control group (t=6.328, P<0.05). Logistic regression analysis revealed that factors including boys (OR=3.298, 95%CI: 1.759 - 6.184), maternal bad behaviors during pregnancy (OR=2.730, 95%CI: 1.169 - 6.375), maternal education level of senior high school or below (OR=2.032, 95%CI: 1.127 - 3.663), critical parenting (OR=2.349, 95%CI: 1.223 - 4.513), and parental stress index (OR=1.089, 95%CI: 1.055 - 1.124) were positively correlated with ADHD in children (P<0.05), while family environment score was negatively related to ADHD (OR=0.868, 95%CI: 0.820 - 0.919, P<0.05). There were no significant differences in BMD, height and body weight between boys and girls in ADHD group (P>0.05). Pearson correlation analysis indicated a positive correlation of BMD with height and body weight (r=0.409, 0.317, P<0.05). 【Conclusions】 The development of ADHD in children is associated with gender, maternal bad behavior during pregnancy, family parental style and so on. Bone mineral density is closely related to physical growth in children with ADHD, clinical interventions can be implemented to prevent or early intervene ADHD.

Children with cerebral palsy (CP) frequently experience secondary musculoskeletal issues, with a high incidence of fractures and severe symptoms. These factors cannot be overlooked in the rehabilitation process for children with CP. This article examines the causes of fractures in children with CP, including low bone mineral density, abnormal training, premature delivery, and falls. Furthermore, it outlines intervention measures to improve bone mineral density and exercise training, in order to assist in the prevention and treatment of fractures in children with CP.

【Objective】 To analyze the clinical characteristics of serum vitamin K2 in children and its correlation with bone mineral density, so as to provide reference for the prevention of insufficient bone strength in children. 【Methods】 A total of 4 145 children who underwent serum vitamin K2 testing and physical examination at pediatric outpatient clinics of several municipal and county hospitals in Chongqing from January 2020 to March 2023 were retrospectively selected into this study for serum vitamin K2-related analysis.Further 844 school-age children who completed serum 25-(OH)D and lumbar bone densitometry measurements were screened to analyze the correlation between vitamin K2level and bone mineral density 【Results】 The overall serum vitamin K2 deficiency rate was 61.6% (2 553/4 145), and the difference in serum vitamin K2 deficiency rate between different age groups was statistically significant (χ2=39.364, P<0.05).The vitamin K2 level of children was significantly influenced by season and maternal education level (χ2=45.310,9.990, P<0.05).There were significant differences in age (Z=3.416), gender (χ2=9.218) and serum vitamin K2 deficiency rate (χ2=5.826) between normal bone mass development group and insufficient bone mass development group (P<0.05).Multivariate Logistic regression analysis suggested that vitamin K2 deficiency was an independent risk factor for insufficient bone mass development in school-age children (OR=1.37,95%CI:1.03 - 1.83, P=0.030). 【Conclusions】 There is a higher serum vitamin K2 deficiency rate among children, especially infants and school-age children.Decreased bone mineral density in school-age children may be associated with serum vitamin K2 deficiency.

Objective@#To investigate the prevalence of osteoporosis (OP) and its influencing factors among people aged 60 years and above, so as to provide insights into OP prevention and treatment among the elderly.@*Methods@#People aged 60 years and above who underwent physical examinations were selected from Hangzhou First People's Hospital from January to October 2023. Demographic information, lifestyle and physical examination data were collected through field surveys and hospital information systems. Bone density was measured by dual-energy X-ray densitometer, and OP was diagnosed with reference to the Guidelines for the Diagnosis and Treatment of Primary Osteoporosis (2022). Factors affecting the OP were identified using a multivariable logistic regression model.@*Results@#A total of 482 participants were included and had a mean age of (69.10±6.52) years. There were 217 men (45.02%) and 265 women (54.98%). The prevalence of OP was 43.36%. Multivariable logistic regression analysis identified gender (women, OR=3.791, 95%CI: 1.726-8.314), age (70 to 79 years, OR=1.260, 95%CI: 1.018-1.559; 80 years and above, OR=3.083, 95%CI: 1.546-6.147), body mass index (≥24 kg/m2, OR=0.358, 95%CI: 0.205-0.626), family history of fracture (OR=1.475, 95%CI: 1.073-2.030), diabetes (OR=1.626, 95%CI: 1.109-2.382), regular exercise (OR=0.457, 95%CI: 0.287-0.726), intake of milk products (OR=0.511, 95%CI: 0.335-0.780), intake of calcium supplements (OR=0.473, 95%CI: 0.287-0.778) and vitamin D deficiency (OR=2.132, 95%CI: 1.309-3.473) as factors affecting OP.@*Conclusion@#The prevalence of OP is associated with gender, age, body mass index, family history of fracture, diabetes, diet and vitamin D deficiency.

ABSTRACT

Objective To analyze the relationship between bone mineral density (BMD) of infants over 6 months of age and exclusive breastfeeding and calcium nutrition guidance during pregnancy in Baoding area, and to provide evidence for clinical application. Methods A total of 308 infants over 6 months of age were selected from Baoding Maternal and Child Health Hospital from January 2020 to January 2023, and their BMD was measured by ultrasound. The level of 25 (OH) D3 in subjects' blood was detected. spearman correlation test was used to analyze the correlation between infant bone mineral density and exclusive breastfeeding and calcium nutritional guidance during pregnancy, and logistics regression model was used to analyze the independent factors affecting infant bone mineral density. Results The level of serum 25 (OH) D3 in normal BMD group was significantly higher than that in abnormal BMD group (P<0.05). The rate of exclusive breastfeeding and the guidance rate of calcium nutrition during pregnancy in normal BMD group were significantly higher than those in abnormal BMD group (P<0.05). There was a significant positive correlation between different bone mineral density and exclusive breastfeeding and calcium nutrition guidance during pregnancy (P<0.05). Serum 25 (OH) D3 level, exclusive breastfeeding rate and calcium nutritional guideline rate during pregnancy were independent protective factors for bone mineral density (P<0.05). Conclusion Bone mineral density (BMD) of infants over 6 months of age is positively correlated with exclusive breastfeeding and calcium nutrition guidance during pregnancy, and exclusive breastfeeding and calcium nutrition guidance during pregnancy are independent protective factors affecting BMD of infants over 6 months of age.

ABSTRACT

ObjectiveTo compare the effects of Taxillus chinensis from different hosts with different meridian affinity on bone microstructure and bone metabolism in ovariectomized osteoporotic rats, and investigate its mechanism of action. MethodEighty-eight specific-pathogen-free (SPF)-grade female Sprague-Dawley (SD) rats were selected and randomly divided into 11 groups: sham-operated group, model group, low-, medium- and high-dose groups of T. chinensis from Morus alba (2.5, 5, and 10 g·kg-1), low-, medium- and high-dose groups of T. chinensis from Cinnamomum cassia (2.5, 5, and 10 g·kg-1), and low-, medium- and high-dose groups of T. chinensis from C. burmannii (2.5, 5, and 10 g·kg-1). After 12 weeks of drug intervention, the rats were examined for proximal femur bone density and bone microstructure using dual-energy X-ray absorptiometry (DXA) and micro-computed tomography (Micro-CT). Histopathological changes in rat femur were observed by the hematoxylin-eosin staining (HE). Contents of serum estradiol (E2), bone Gla protein (BGP), bone alkaline phosphatase (BALP), tartrate-resistant acid phosphatase 5b (TRACP-5b) and pre-collagen type Ⅰ amino-terminal protopeptide (PINP) were measured by the enzyme-linked immunosorbent assay (ELISA). Real-time quantitative polymerase chain reaction (Real-time PCR) was employed to detect the messenger ribonucleic acid (mRNA) expressions of bone morphogenetic protein-2 (BMP-2), Smad1, Smad9 and recombinant runt-related transcription factor 2 (Runx2) in rat humerus. Western blot was used to detect the protein expressions of BMP-2, p-Smad1/5/9 and Runx2 in rat humerus. ResultCompared with that in the sham-operated group, the femur microstructure of rats in the model group was significantly disrupted, with significant decreases in bone mineral density (BMD) value, bone volume fraction (BV/TV), trabecular number (Tb.N), and trabecular thickness (Tb.Th) (P<0.01), and significant increases in trabecular separation (Tb.Sp) and structure model index (SMI) (P<0.01). The serum levels of BGP, BALP, TRACP-5b and PINP were significantly increased (P<0.05, P<0.01), and E2 levels were significantly decreased (P<0.01). The mRNA expressions of BMP-2, Smad1, Smad9, and Runx2 were significantly decreased in rat humerus (P<0.01), and the protein expressions of BMP-2, p-Smad1/5/9, and Runx2 were significantly reduced (P<0.01). Compared with the model group, the administration groups of T. chinensis from different hosts all elevated the BMD, BV/TV, Tb.N, Tb.Th, Tb.Sp, and SMI levels in the femur, improved bone microstructure, increased serum E2 levels (P<0.05, P<0.01), lowered the levels of serum BGP, BALP, TRACP-5b, and PINP, upregulated the mRNA expression of BMP-2, Smad1, and Runx2 and upregulated the mRNA expression levels of Smad9 (P<0.05, P<0.01), and upregulated the protein expressions levels of BMP-2, p-Smad1/5/9, and Runx2 (P<0.01). The best effect was observed in the group of T. chinensis from C. cassia. ConclusionT. chinensis from different hosts improved osteoporosis in ovariectomized rats, with the group of T. chinensis from C. cassia being the most potent among the administered groups, and its treatment of osteoporosis may regulate the balance of bone conversion by regulating BMP/Smad signaling pathway.

ABSTRACT

ObjectiveTo investigate the effects of morin treatment on bone metabolism and bone mass in aged rats, and to clarify the possible mechanism. MethodsTen young female Sprague-Dawley rats (3 months old) and 20 old female Sprague-Dawley rats (24 months old) were randomly divided into three groups: Control group (CON, 10 young rats); Model group (MOD, 10 young rats); 10 old rats and SangHuangSu Group (SSS, 10 old rats). During the experiment, the SSS group received intraperitoneal injection of morin (10 mg / kg) daily. The treatment lasted for 12 weeks. After treatment, Micro-CT, HE stained sections, serological tests and Western blot were used to observe the treatment effect and possible mechanism. ResultsAfter 12 weeks of treatment, compared with MOD group, the number and density of bone trabeculae in SSS group were significantly improved. The BMD, Conn. D, Tb. N, Tb.Th and Tb.Sp of the left femur in the SSS group were significantly better than those in the MOD group(P <0.05). After 12 weeks of treatment, the levels of CTX-1, osteocalcin, TRACP-5b and PINP in SSS group were significantly lower than those in MOD group(P <0.05). Compared with the MOD group, the ERK1/2-p38 signal pathway was significantly inhibited and the levels of ERK1/2 and p38 were significantly decreased in the SSS group(P <0.05). ConclusionMorin pigment mediates the protective effect on the bones of aged rats by inhibiting the ERK1/2-p38 signaling pathway and reducing bone turnover.
