ABSTRACT

Objective. To analyze the duties of wet nurses at the Hospital Real in Santiago de Compostela (Spain). The secondary objectives were to compare the mortality rate and distribution by parish of the foundlings under the care of the Royal House between 1803 and 1808; and to determine the origin of the Galician foundlings who participated in the Royal Philanthropic Expedition of the Smallpox Vaccine in 1803. Methods. Historiographic study that analyzed sorted and not sorted in series indirect positional and quantitative historical sources.Results. The duties of wet nurses during the studied period were to provide basic care and cultural instruction. The mortality rate of foundlings fluctuated during that period and their distribution by parish (functional unit of healthcare services at that time) was similar in those years, with a predominance in the provinces of A Coruña and Pontevedra. A total of 5 Galician foundlings from the House analyzed were part of the smallpox vaccine expedition, their names were Juan Antonio, Jacinto, Gerónimo María, Francisco Florencio and Juan Francisco. Conclusion. During the observed period the wet nurses of the Hospital Real of Santiago de Compostela were in charge of pediatric care. Wet nurses were vital in the role of keeping the foundlings alive and can be considered as one of the forerunners of the pediatric nurse profession at that time.


Objetivo. analizar las funciones que realizaban las amas de leche en el Hospital Real de Santiago de Compostela (España). Los objetivos secundarios han sido: comparar la mortalidad y distribución por parroquias de los niños expósitos a cargo de la Real Casa entre 1803 y 1808, determinar la procedencia de los niños expósitos gallegos que participaron en la Real Expedición Filantrópica de la Vacuna de la viruela en 1803. Métodos. Estudio historiográfico que analizó fuentes históricas posicionales indirectas y cuantitativas seriadas y no seriadas. Resultados. las funciones de las nodrizas durante el período de estudio eran proporcionar los cuidados básicos e instrucción cultural; la mortalidad de los expósitos sufrió oscilaciones a lo largo del período analizado. La distribución por parroquias (unidad funcional de las áreas de salud en la época) fue similar en estos años, con predominancia de las provincias de A Coruña y Pontevedra. Un total de 5 niños expósitos gallegos de la Casa analizada han participado en la expedición filantrópica de la viruela, sus nombres fueron: Juan Antonio, Jacinto, Gerónimo María, Francisco Florencio y Juan Francisco. Conclusión. Durante el período de observación, las amas de leche del Hospital Real de Santiago de Compostela realizaban cuidados pediátricos. Las nodrizas cumplieron un rol fundamental para el mantenimiento con vida de los niños expósitos y pueden ser consideradas como una de las figuras precursoras de la profesión enfermera pediátrica en la época.

Os objetivos secundários foram: comparar a mortalidade e distribuição por freguesia dos enjeitados responsáveis pela Casa Real entre 1803 e 1808, determinar a origem dos enjeitados galegos que participaram na Real Expedição Filantrópica da Vacina contra a Varíola em 1803. Métodos. Estudo historiográfico que analisou fontes históricas posicionais indiretas e quantitativas seriadas e não seriadas. Resultados. As funções das amas de leite durante o período do estudo eram fornecer cuidados básicos e instrução cultural; A mortalidade dos enjeitados oscilou ao longo do período analisado. A distribuição por freguesias (unidade funcional das áreas de saúde da época) foi semelhante nestes anos, com predominância das províncias da Corunha e Pontevedra. Na expedição filantrópica contra a varíola participaram um total de 5 crianças galegas da Casa analisada, os seus nomes eram: Juan Antônio, Jacinto, Gerônimo María, Francisco Florencio e Juan Francisco. Conclusão. Durante o período de observação, as nutrizes do Hospital Real de Santiago de Compostela prestaram cuidados pediátricos. As amas de leite desempenharam um papel fundamental na manutenção da vida dos enjeitados e podem ser consideradas uma das figuras precursoras da profissão de enfermagem pediátrica da época.

ABSTRACT

Background: Breastfeeding is essential for health of both infants and mothers, but it often encounters challenges such as postpartum breast complications. These issues can adversely affect maternal health and significantly hinder success of breastfeeding practices. Aim of study was to assess postpartum breast complications and breast-feeding practices.Methods: This cross-sectional study was conducted in department of obstetrics and gynaecology, Dhaka medical college hospital, Dhaka, Bangladesh from February 2006 to July 2006. Two hundred patients in the postnatal ward who delivered their babies at DMCH were randomly selected.Result: Among 200 postnatal women, 33.5% experienced breast problems. Women with breast problems were designated as group A and women without breast problems were designated as group B. Most women in groups A and B were aged between 21-30 years with average 24.8 years for both groups. Antenatal check-ups were similar across groups, with breastfeeding advice given to 55.2% in group A and 64.7% in group B pre-lacteal feeds were given to 19.4% of group A and 21.8% of group B neonates, while colostrum was fed to the majority. Most neonates were breastfed within 2 hours. Good breastfeeding position and attachment were observed in most cases. Exclusive breastfeeding was more common in group B (73.7%) than in group A (58.2%). Breast problems in group A included engorgement, lactation insufficiency, and nipple issues.Conclusions: The study reveals that postpartum breast complications, notably breast engorgement and lactation insufficiency, significantly affects the breastfeeding effectiveness of new mothers.

Objetivos: el objetivo de este estudio fue comparar la lactancia y los factores relacionados (edad, nivel educativo, edad al momento del primer embarazo, etc.) y las prácticas de alimentación complementaria de las madres refugiadas sirias y las madres turcas. Materiales y métodos: este estudio descriptivo y comparativo analizó las características nutricionales de los bebés de 9 a 60 meses de edad cuyas madres fueran turcas o refugiadas sirias que asistieron al Hospital Público de Kiziltepe entre enero y julio de 2022. Resultados: se incluyó a 204 madres (126 turcas y 78 sirias). La edad promedio de las madres turcas era 27,60 ± 5,17 años y la de las refugiadas sirias, 28,91 ± 5,62 años, sin una diferencia significativa entre ambos grupos (p: 0,091). La lactancia materna posparto fue del 91,3 % y la duración de la lactancia fue de 12 meses (0-24) en las ciudadanas turcas, mientras que, en las refugiadas sirias, fue del 84,6 % y 9 meses (0-24), respectivamente (consumo de leche materna, p: 0,144; tiempo de consumo, p: 0,161; sin diferencias estadísticas). El 23,8 % de las ciudadanas turcas y el 5,1 % de las refugiadas sirias recibieron capacitación sobre la lactancia, con una diferencia significativa entre ambos grupos (p: 0,001). Conclusión: en los grupos de refugiadas, las prácticas de nutrición infantil y materna se ven alteradas. En colaboración con las organizaciones locales e internacionales y los organismos estatales que ayudan a los grupos de refugiados se podrían mejorar las prácticas de nutrición maternoinfantil y reducir las brechas.


Objectives: The purpose of this study was to compare breastfeeding and related factors (age, level of education, age at first pregnancy, etc.), and complementary feeding practices between Syrian refugee and native Turkish mothers. Material and methods: This descriptive-comparative study examined the nutritional characteristics of infants aged 9 to 60 months whose mothers were Turkish or Syrian refugees who attended Kiziltepe State Hospital between January 2022 and July 2022. Results: 204 mothers (126 Turkish and 78 Syrian) who had a child aged 9-60 months were included. The average age of the mothers was 27.60 ± 5.17 years for Turkish citizens and 28.91 ± 5.62 for Syrian refugees, without significant difference between the two groups (p: 0.091). Postpartum breastfeeding was 91.3% and breastfeeding duration was 12 (0-24) months in Turkish citizens; in Syrian refugees, breastfeeding was 84.6% and average breastfeeding time was 9 (0- 24) months (respectively, breast milk intake p: 0.144, uptake time p: 0.161; no statistical difference). Breastfeeding training was received by 23.8% of Turkish citizens and 5.1% of Syrian refugees; there was a significant difference between the two groups (p: 0.001). Conclusion: In refugee groups, infant and maternal nutrition practices are disrupted. Working in conjunction with local and international organizations and state agencies that give help to refugee groups, the appropriate interventions, initiatives, supports, and awareness-raising activities would strive to improve practices in mother and baby nutrition and narrow gaps.

ABSTRACT

Background and Objectives: investigating the relationship between breast milk use infection control in premature newborns may provide a basis for continuing exclusive breastfeeding, reducing the rates of early introduction of formula and strengthening their immune system. In view of this, the objective was to analyze the relationship between breastfeeding and infection control in premature newborns. Content: an integrative review, which included original articles, available electronically and with a temporal cut of the last five years. Searches were carried out in August 2022 in the Medical Literature Analysis and Retrieval System Online, Literatura Latino-Americana e do Caribe em Ciências da Saúde, Scopus, Web of Science and Science Direct databases, using the descriptors Breastfeeding, Milk Human, Infection Control, Infection Control and Premature Newborn, concatenated with the search operators "AND" and "OR". The Rayyan digital tool was used to organize the study selection stages. A total of 490 studies were identified in the search, of which seven were selected. All studies were published in English between 2018 and 2022. Regarding design, three were case-control studies, one was a cohort, one was cross-sectional, and two were clinical trials. Regarding the level of evidence, studies classified as level IV predominated. Conclusion: human breast milk has the power to reduce the incidence of necrotizing enterocolitis and cytomegalovirus, reduce the worsening of inflammatory states and late-onset sepsis, prevent diseases, encourage the physical and cognitive development of premature newborns.(AU)


Justificativa e Objetivos: a investigação da relação do uso de leite materno com o controle de infecções em recém-nascidos prematuros poderá fornecer embasamento para continuidade do aleitamento materno exclusivo, diminuindo os índices de introdução precoce de fórmula e propiciando o fortalecimento de seu sistema imunológico. Diante disso, objetivou-se analisar a relação do aleitamento materno com o controle de infecções em recém-nascidos prematuros. Conteúdo: revisão integrativa, que incluiu artigos originais, disponíveis eletronicamente e com recorte temporal dos últimos cinco anos (2018 a 2022). Foram realizadas buscas no mês de agosto de 2022 nas bases de dados Medical Literature Analysis and Retrieval System Online, Literatura Latino-Americana e do Caribe em Ciências da Saúde, Scopus, Web of Science e Science Direct, sendo utilizado os descritores Aleitamento Materno, Leite Humano, Controle de Infecções, Controle de Infecção e Recém-Nascido Prematuro, concatenados com os operadores de pesquisa "AND" e "OR". Foi utilizada a ferramenta digital Rayyan para a organização das etapas de seleção dos estudos. Foram identificados 490 estudos na busca, dos quais sete foram selecionados. Todos os estudos foram publicados em inglês entre 2018 e 2022. Quanto ao delineamento, três eram estudos de caso controle, um era coorte, um era transversal e dois eram ensaios clínicos. Em relação ao nível de evidência, predominaram os estudos classificados em nível IV. Conclusão: o leite humano materno tem o poder de diminuir a incidência de enterocolite necrosante e citomegalovírus, reduzir o agravamento de estados inflamatórios e de sepse tardia, prevenir doenças, estimular o desenvolvimento físico e cognitivo do recém-nascido prematuro.(AU)

En cuanto al nivel de evidencia predominaron los estudios clasificados como nivel IV. Conclusión: la leche materna humana tiene el poder de reducir la incidencia de enterocolitis necrotizante y citomegalovirus, reducir el agravamiento de estados inflamatorios y sepsis tardía, prevenir enfermedades, estimular el desarrollo físico y cognitivo de los recién nacidos prematuros.(AU)

ABSTRACT

Background: Low adoption of exclusive breast feeding among mothers and its impact on the infants has been attributed to several factors. The im of the study was to assess exclusive breastfeeding practices and predictors among women attending child care immunization services at Alex Ekwueme Federal University Teaching Hospital Abakaliki (AE-FUTHA). Methods: The design of this study is cross-sectional survey. The population of the study comprised of 2100 women who registered for child care immunization services at Federal Teaching Hospital Abakaliki from the month of January-March, 2023. Convenience sampling technique was used in selecting the participants. Data were analyzed using mean, standard deviation, frequency counts and percentages and t test statistics. Results: The results showed that the exclusive breastfeeding practices of the women were high (3.33±0.39). Also, EBP of the women was high based on age and parity. Although, there was no significant difference in the exclusive breastfeeding practices based on parity (p>0.05). However, there was significant difference in the exclusive breastfeeding practices based on age of the mothers (p<0.05). Conclusions: The study concluded that to sustain the high practices of exclusive breastfeeding among women attending child care immunization services at AE-FUTHA, there is need to continue to engage them on seminars, workshops and conferences by health workers on the days of immunization with special focus to the younger age of the mothers in order to sustain the high EBPs.

Background: Within the bustling and stressful neonatal intensive care unit (NICU) environment, implementing Baby-friendly practices presents numerous challenges. Breastfeeding initiation and duration rates among NICU infants are lower compared to healthy full-term infants. Objectives were to explore breastfeeding support practices and related barriers and facilitators in the NICU.Methods: A retrospective study of 496 babies admitted to NICU of Sahota Superspeciality Hospital Kashipur, Uttarakhand during the time period March 2018 to February 2020 and examined maternal and infant factors. The 496 infants divided in two groups: group A: breast milk use at discharge (n=398) and group B: no breast milk use at discharge (n=98).Results: The incidence of morbidity factors like sepsis, chronic lung disease, retinopathy of prematurity, intraventricular hemorrhage, gastrostomy tube feeds at discharge, gavage feeds at discharge noted more among the babies of group of ‘no breast milk use at discharge’ compared to other group (p<0.05). The usage of donor milk and formula milk is significantly very less in babies of group ‘breast milk use at discharge’ compared to other group (p<0.05). Duration of hospital stay was very less in the group A compare to group B (p<0.05). Respiratory distress was the most common diagnosis among the babies of group A and apnea was the most common among babies of group B.Conclusions: Obstacles to maintaining breastfeeding in the NICU, which warrant focused efforts for enhancing practices, encompassed inadequate resources for facilitating parental involvement, impediments to expressing and providing maternal breast milk, and a notable prevalence of bottle-feeding supplemented with formula.

ABSTRACT

Objetivo: investigar o aleitamento materno com foco sobre a exclusividade dessa prática no primeiro semestre de vida da criança, em um município do norte de Minas Gerais. Métodos: trata-se de um estudo descritivo, qualitativo realizado com 13 mães com idade entre 24 e 39 anos. Os dados foram coletados entre agosto e setembro de 2023, por meio de uma entrevista semiestruturada, e analisados mediante análise temática. Resultados: as mulheres indicaram ter enfrentado dificuldades na prática do aleitamento materno exclusivo, especialmente nos primeiros dias de vida da criança, devido a ingurgitamento, mastite, fissuras mamárias, pressão social e pessoal de leite fraco. Identificou-se como benefícios do aleitamento materno exclusivo a proteção imunológica, o estreitamento de vínculo, a prevenção de doenças e de desnutrição na criança, assim como a aceleração da involução uterina e a prevenção de cânceres na mulher. O apoio da família, do parceiro e da equipe de saúde foi apontado como fatores facilitadores do aleitamento materno, porém indicaram receber poucas orientações dos profissionais sobre amamentação. Conclusão: destaca-se a importância de a equipe de saúde atuar com um olhar zeloso perante esse público, intensificando as orientações sobre aleitamento materno exclusivo a fim de reduzir as dúvidas e dificuldades para elevar as taxas de duração e exclusividade do aleitamento materno.


Objective: to investigate breastfeeding with a focus on the exclusivity of this practice in the first semester of a child's life in a municipality in the north of Minas Gerais. Methods: this is a descriptive, qualitative study carried out with 13 mothers aged between 24 and 39 years old. Data were collected between August and September 2023 from a semi-structured interview and analyzed using Thematic Analysis. Results: women indicated that they faced difficulties in practicing exclusive breastfeeding, especially in the first days of the child's life, due to engorgement, mastitis, breast fissures, and social and personal pressure of weak milk. The benefits of exclusive breastfeeding were identified as immunological protection, strengthening bonds, preventing diseases and malnutrition in children, as well as accelerating uterine involution, and preventing cancer in women. Support from family, partner, and healthcare team was identified as factors that facilitate breastfeeding, but they indicated that they received little guidance from professionals about breastfeeding. Conclusion: the importance of the health team acting with a zealous eye towards the public is highlighted, intensifying guidance on exclusive breastfeeding in order to reduce doubts and difficulties in increasing the duration and exclusivity of breastfeeding.

ABSTRACT

Background: EBF for 6 months is a challenge for many mothers. The WHO reported the prevalence of EBF lasting at least 6 months as 38% in 2013 and most recently reported as 44% in 2021. As a result, the WHO has set strategic goals for 6-month EBF to achieve 70% prevalence worldwide by 2030. Methods: Descriptive survey research design was used, with the variables as sustainability of Exclusive Breast Feeding among 100 postnatal primi gravida attending a tertiary care hospital in Eastern India. Sampling technique was Systemic random sampling technique. Standardized tool (Feed Cat Tool) was used as a data collection instruments. Results: 93% mother fed exclusively their infant with breast milk whereas 5% fed their infant both with breast milk and formula milk and 2% did not fed their infant with breast milk they gave only formula milk to their infant upto 1.6 months of their infant age. At 3 months, 81% fed exclusively their infant with breast milk only whereas 17% fed their infant both with breast milk and formula milk At 6 months 73% fed exclusively their infant with breast milk whereas 24% fed their infant both with breast milk and formula milk. Conclusions: The sustainability of Exclusive Breast Feeding reduces with the increase in infant’s age. The study had limitations like, the data were collected after their discharge form hospital stay during their follow up visits and by interview technique only which may lead to inaccurate evaluation.

ABSTRACT

Background: Child-malnutrition is a critical global concern supported by substantial evidence. Recognizing optimal infant and young child-feeding practices as pivotal in combating malnutrition is crucial. Methods: This was a cross-sectional study conducted through house-to-house survey in Ukkali and Shivanagi villages for period of 6 months from February 2018 to July 2018. A pretested-questionnaire was administered to 133mothers of 6 to 12-month-old children, collecting socio-demographic data, knowledge, attitudes, and practices regarding prelacteal, breastfeeding, and complementary-feeding. Results: Knowledge gaps emerged, with only 50% understanding colostrum's importance. Limited awareness of exclusive breastfeeding (65%) and extended breastfeeding (31.57%) was observed. Prelacteal-feeding was widespread (52.63%), with sugar-water being a prevalent choice (21.8%). Most mothers (over 90%) opted for breastfeeding, yet timely initiation (27.6%) and exclusive-breastfeeding (30%) were suboptimal. Complementary-feeding initiation (15%) and minimum-meal-frequency adherence (13.3%) were low. Meal-diversity was deficient in over 60%. Conclusions: Bridging knowledge gaps and promoting optimal-feeding-practices through targeted nutritional education is essential. This study underscores the importance of addressing misconceptions, enhancing early-breastfeeding, and ensuring proper complementary-feeding to improve child-health outcomes. Socio-demographic-factors must be considered when designing educational material and interventions in rural-settings.

ABSTRACT

OBJETIVO: Compreender a adaptação das mulheres ao processo de amamentação e o apoio familiar e dos serviços de saúde. MÉTODO: Estudo qualitativo fundamentado no referencial metodológico da Etnoenfermagem, conforme a Teoria de Enfermagem Transcultural de Leininger. Foi utilizado um roteiro de entrevista semiestruturada. RESULTADOS: Evidenciaram-se três categorias temáticas: "Enfrentando algo inesperado, mutável e desafiador"; "Convivendo com dúvidas, incertezas e desorientação" e "Identificando fontes de apoio para a manutenção da amamentação". CONCLUSÃO: O processo de adaptação do aleitamento materno é permeado por dúvidas, incertezas e desafios. As mulheres apoiam-se em suas experiências prévias e na cultura familiar e sofrem influência direta destas. O papel da atenção básica é essencial nesse contexto.


OBJECTIVE: To understand women's adaptation to the breastfeeding process and the support provided by family and health services. METHOD: A qualitative study based on the methodological framework of Ethnonursing, following Leininger's Transcultural Nursing Theory. A semi-structured interview guide was used. RESULTS: Three thematic categories were identified: "Facing something unexpected, changing, and challenging"; "Dealing with doubts, uncertainties, and disorientation"; and "Identifying sources of support for breastfeeding maintenance". CONCLUSION: The breastfeeding adjustment process has doubts, uncertainties, and challenges. Women rely on and are directly influenced by their past experiences and family culture. The role of primary health care is crucial in this context.

ABSTRACT

La autoeficacia en la lactancia materna es la confian-za de la madre en su capacidad para amamantar a su hijo(a), la cual se evalúa mediante el cuestionario Breastfeeding Self Efficacy Scale (bses). Diversos estu-dios han explorado las propiedades psicométricas del instrumento con resultados adecuados en diferentes países. Este estudio presenta el proceso de traducción y validación de la escala en su versión breve, de 14 ítems, en una muestra de mujeres colombianas. En total, se evaluaron 243 mujeres lactantes con hijos alrededor de los cuatro meses de edad. Se analizó la consistencia y estructura interna de la escala, así como su relación con otras variables. La escala evidenció adecuados indicios respecto a su consistencia interna (α = .91), se identificó una estructura factorial de dos dimensiones que explican un 48 % de la varianza, la relación con otras variables como la autoestima (ρ = .28) y el estrés percibido (ρ = .30) fueron débiles o moderadas, y las diferencias en las puntuaciones respecto a la historia obstétrica y las variables sociodemográficas fueron analizadas. Several studies have explored the psychometric properties of the questionnaire with adequate results in different countries. The present study presents the translation and validation process of the 14-item brief version of the scale in a sample of Colombian women. In to-tal, 243 lactating women with children around four months of age were evaluated. The consistency and internal structure of the scale was analysed, as well as its relationship with other variables. The scale evidenced adequate internal consistency (α = .91), a factorial structure of two dimensions explaining 48 % of the variance was identified, the relationship with other variables such as self-esteem (ρ = .28) and perceived stress (ρ = .30) was weak or moderate, and the differences in scores regarding the obstetric history and sociodemographic variables were analyzed. The psychometric evidence supports the use of the scale in both clinical and research settings to assess self-efficacy in Colombian lactating women.


A autoeficácia na amamentação é a confiança da mãe em sua capacidade de amamentar seu filho e é avaliada por meio do questionário Breastfeeding Self Efficacy Scale(bses). Vários estudos exploraram as propriedades psi-cométricas do instrumento com resultados adequados em diferentes países. O presente estudo apresenta o processo de tradução e validação da escala em sua versão breve de 14 itens em uma amostra de mulheres colombianas. No total, foram avaliadas 243 lactantes com filhos em torno de quatro meses de idade. Foram analisadas a consistência e a estrutura interna da escala, bem como sua relação com outras variáveis. A escala apresentou indicações adequadas quanto à sua consistência interna (α = 0,91), foi identifi-cada uma estrutura fatorial bidimensional que explicou 48% da variância. A relação com outras variáveis como a autoestima (ρ = 0,28) e percepção ao estresse (ρ = 0,30) foi fraca ou moderada, e, foram analisadas diferenças nos escores referentes ao histórico obstétrico e às variáveis sociodemográficas. Evidências psicométricas apoiam o uso da escala tanto em ambientes clínicos como de pesquisa para avaliar a autoeficácia na amamentação em mulheres lactantes colombianas.

ABSTRACT

Objective:To explore the effects of high-efficiency centering pregnancy care (CPC) combined with information-knowledge-attitude-practice (IKAP) health education on cesarean delivery on maternal request (CDMR) rate, maternal and neonatal health status so as to provide referential significance for reducing CDMR rate and improving postpartum maternal and neonatal health status and breastfeeding rate.Methods:In the prospective, single-center and controlled study, a total of 82 primiparas undergoing antenatal examination and delivery in South Binhu Hospital of Hefei First People′s Hospital were enrolled as the research objects between January 2020 and December 2021 by convenience sampling method, including 41 cases given routine health care in routine group between January and December 2020 and 41 cases given high-efficiency CPC combined with IKAP health education on basis of routine group in combination group between January and December 2021. The cognition of cesarean section, delivery modes, incidence of postpartum complications, neonatal complications and breastfeeding between the two groups were compared.Results:The age of the routine group was (26.71 ± 2.18) years old, and the age of the combination group was (27.16 ± 1.79) years old. The proportions of correct cognition, partially correct cognition and incorrect cognition to cesarean section in combination group were 73.17% (30/41), 21.95% (9/41), 4.88% (2/41), and 53.66% (22/41), 26.83% (11/41), 19.51% (8/41) in routine group, and the difference between the two groups was statistically significant ( Z = 4.22, P<0.05). The incidence of cesarean section and CDMR in combination group was 24.39% (10/41) and 1/10, lower than that in routine group [46.34% (19/41), 10/19], and the difference between the two groups was statistically significant ( χ2 = 4.32, P<0.05; Fisher′s exact test, P = 0.049 ). The total incidence rates of postpartum complications of primiparas and neonatal complications in the combination group were 4.88%(2/41) and 7.32%(3/41), lower than 12.20%(5/41) and 12.20%(5/41) in the routine group, and the differences between the two groups were not statistically significant (Fisher′s exact test, P = 0.432, 0.712). The scores of functional health literacy, communicative health literacy and critical health literacy in combination group were (85.73 ± 4.09), (51.93 ± 4.72) and (58.63 ± 4.32) points, higher than those in routine group [(80.17 ± 5.26), (46.37 ± 5.11) , (52.71 ± 5.07) points], and the difference between the two groups was statistically significant ( t = 5.34, 5.12, 5.69, all P<0.01). At 3 months after delivery, exclusive breastfeeding rate, milk powder feeding rate and mixed feeding rate in combination group were 58.54% (24/41), 7.32%(3/41), 34.15(14/41), higher than 26.83% (11/41), 29.27(12/41), 43.90%(18/41) in routine group, and the difference between the two groups was statistically significant ( χ2 = 10.73, P<0.05). Conclusions:High-efficiency CPC combined with IKAP health education can reduce incidence of CDMR, decrease maternal and neonatal complications, enhance maternal postpartum health literacy and improve breastfeeding rate in primiparas by improving correct cognition to cesarean section. It is worthy of clinical application.

ABSTRACT

The package inserts revision regarding prescription drugs is an ongoing process. In this study, we examined the status of this revision and how the content of the information provided has been updated with the new instructions, including the rationale for contraindications for pregnant and breast-feeding women. A total of 407 prescription drugs for pregnant and breast-feeding women were contraindicated, accounting for approximately 15% of the 2,627 nonproprietary names. The number of contraindicated drugs for pregnant, breast-feeding, and both pregnant and breast-feeding women were 406, 44, and 43, respectively. The majority of the contraindications were based on nonclinical data, such as teratogenicity and fetotoxicity data in reproductive toxicity studies involving pregnant animals and pharmacokinetic information in breast-feeding animals. The overall revision rate for the new instructions was 16.2% after three years of revision work for each drug. The alert wording has been revised to provide more clarity and consistency, with three categories of statements: “should not be administered/avoid breastfeeding,” “advisable not to administer/breastfeed,” and “should be administered/breastfeed in consideration of the therapeutic benefit (benefit-risk consideration).” The statement indicating that the drug should not be administered to pregnant women remained in the revised instructions. Conversely, the revision of the description for breast-feeding women from “should be discontinued/avoided” to “benefit-risk consideration” may have improved the usefulness of the product, allowing healthcare professionals to make more informed decisions after considering the transfer of the drug into milk, its pharmacological effects, and other factors.

RESUMO Objetivo Propor uma metodologia de análise dos dados gerados por um instrumento de medição da pressão de sucção não-nutritiva do recém-nascido. Método Estudo observacional analítico, transversal com dados de 24 recém-nascidos a termo sem comprometimentos. Após a avaliação clínica foram analisadas três coletas de cada neonato empregando-se o referido instrumento, com duração de 2 minutos e intervalo de 2 minutos entre elas. Os parâmetros definidos foram extraídos por meio de um programa desenvolvido em Matlab®. Os resultados foram obtidos pela análise e comparação de 12 variáveis ao nível de confiança de 5%. Foi realizada ainda comparação das análises manual e computadorizada por meio do coeficiente de correlação intraclasse. Resultados A comparação múltipla entre os três momentos de coleta, mostrou que as diferenças estatísticas significantes ocorreram entre as coletas um e dois e dois e três. Ao se analisar e comparar cada variável separadamente, notou-se que a segunda coleta apresentou: maior número de grupos de sucção, maior número de sucções, menor tempo para iniciar os grupos de sucção, maior tempo de grupos de sucção, menor número de sucções esporádicas, valores de pressão média maiores e com menor desvio padrão, maior número de pausas com tempo de pausas menor. O coeficiente de correlação intraclasse revelou concordância quase perfeita para os 12 parâmetros avaliados. Conclusão As 12 variáveis analisadas mostram-se relevantes, especialmente na segunda coleta. O programa em Matlab® mostrou-se viável e eficaz na extração e análise dos parâmetros, apresentando alta concordância quando comparado à avaliação manual.


ABSTRACT Purpose To propose a methodology for analyzing data generated by an instrument measuring non-nutritive sucking pressure in newborns. Methods An analytical observational study was developed, with a cross-sectional design, considering the data collected from 24 full-term newborns without complications. Three collections from each neonate were analyzed, with duration of 2 minutes and a 2-minute interval between them. The defined parameters were extracted using a program developed in Matlab®. The results were obtained by analyzing and comparing 12 variables at a 5% confidence level. Comparison of manual and computerized analyzes was also carried out using the intraclass correlation coefficient. Results The multiple comparison between the three collection moments showed that the significant statistical differences occurred between collections one and two and two and three. When analyzing and comparing each variable separately, it was noted that the second collection showed: greater number of sucking groups, greater number of suctions, less time to start the sucking groups, longer time of sucking groups, less number of sporadic suctions, higher mean pressure values and with less standard deviation, more number of pauses with shorter time of pauses. The intraclass correlation coefficient revealed almost perfect agreement for the 12 evaluated parameters. Conclusion The 12 variables analyzed are relevant, especially in the second collection. The Matlab® program proved to be viable and effective in extracting and analyzing parameters, showing high agreement when compared to manual evaluation.

ABSTRACT Objective: To identify the type of feeding and analyze the sociodemographic and clinical factors associated with exclusive breastfeeding at hospital discharge, in the first and in the last follow-up visit of the third stage of the Kangaroo Mother Care among infants admitted to the kangaroo unit. Method: Longitudinal and retrospective study. A total of 186 infants of gestational age <37 weeks admitted to the kangaroo unit in 2018 and 2019 was included. Data collected from medical records and subjected to inferential analysis and the Poisson regression model (P < 0.05). Results: Exclusive breastfeeding rate was 73.1% at discharge, with a drop at the last follow-up visit (68.1%). At discharge, there was a greater probability of exclusive breastfeeding in younger mothers, with higher education, infants born with higher birth weight and who received exclusive human milk during hospitalization; in the first follow-up visit, in a younger mother and infant who received only human milk during hospitalization; and in the last follow-up visit, a young mother, infant who received only human milk and suckled at the breast for the first time in the kangaroo unit. Conclusion: Most infants hospitalized in the second stage of the Kangaroo Mother Care were exclusively breastfed and presented maternal and clinical factors related to breastfeeding. This fact can help manage the challenges of the method and promote breastfeeding.


RESUMEN Objetivo: Identificar el tipo de alimentación y analizar los factores sociodemográficos y clínicos asociados a la lactancia materna exclusiva al alta hospitalaria, en el primero y en la última visita de seguimiento de la tercera etapa del Método Madre Canguro entre los recién nacidos ingresados en la unidad canguro. Método: Estudio longitudinal y retrospectivo. Se incluyeron 186 neonatos en edad gestacional <37 semanas ingresados en la unidad canguro en 2018 y 2019. Datos recopilados de historias clínicas sometidas a análisis inferencial y modelo de regresión de Poisson (p < 0,05). Resultados: La tasa de lactancia materna exclusiva fue del 73,1% al alta, con descenso en la última visita de seguimiento (68,1%). Al alta hubo mayor probabilidad de lactancia materna exclusiva en madres más jóvenes, con mayor escolaridad, recién nacidos con mayor peso al nacer y que recibieron leche materna exclusiva durante la internación; en la primera visita de seguimiento, en una madre más joven y un recién nacido que recibió únicamente leche materna durante la hospitalización; y en la última visita de seguimiento, una madre joven, recién nacido que recibió sólo leche humana y con la primera succión del pecho en la unidad canguro. Conclusión: La mayoría de los recién nacidos hospitalizados en la segunda etapa del Método Madre Canguro fueron amamantados exclusivamente y presentaron factores maternos y médicos relacionados con la lactancia materna, que pueden ayudar a gestionar los desafíos del método y promover la lactancia materna.

Dados coletados do prontuário submetidos à análise inferencial e ao modelo de regressão Poisson (p < 0,05). Resultados: Taxa de aleitamento exclusivo foi de 73,1% na alta, com queda no último retorno (68,1%). Na alta, houve maior probabilidade de aleitamento exclusivo em mãe mais jovem, com escolaridade superior, neonato nascido com maior peso e que recebeu leite humano exclusivo durante internação; no primeiro retorno, em mãe mais jovem e neonato que recebeu apenas leite humano na internação; e no último retorno, mãe jovem, neonato que recebeu apenas leite humano e com primeira sucção na mama na unidade canguru. Conclusão: A maioria dos neonatos internados na segunda etapa do Método Canguru estava em aleitamento exclusivo e apresentou fatores maternos e clínicos relacionados ao aleitamento, podendo auxiliar no manejo dos desafios do método e na promoção da amamentação.

Objective: To evaluate the rates of exclusive breastfeeding (EBF) and growth of preterm and/or low birth weight newborns during the third stage of the Kangaroo Method (TSKM), at discharge. Methods: Retrospective study in a reference public maternity hospital between Jan/2014 and Dec/2017, including the preterm (less than 37 weeks) and/or low birth weight (less than 2500 g) newborn infants. Information was collected from medical records. Statistics analysis was done in SPSS software. Results: 482 infants were included and followed up at the TSKM ambulatory. The average gestational age was 33 weeks (variation: 24-39 weeks) and birth weight, 1715g (variation: 455-2830 g). EBF occurred in 336 (70.1%) infants at hospital discharge, and in 291 (60.4%) at TSKM discharge. Each additional day of hospital stay increased the chance of infant formula (IF) use by 9.3% at hospital discharge and by 10.3% at TSKM discharge. Staying in the Kangaroo Neonatal Intermediate Care Unit (KNICU) favored EBF at hospital discharge and TSKM discharge (p<0.001). Not performing the kangaroo position increased the chance formula administration to the newborn infant at hospital discharge by 11%. Weight gain and head circumference growth were higher in infants using formula (p<0.001). Conclusions: The length of hospital stay and not performing the kangaroo position favored the use of infant formula at hospital and TSKM discharge. Staying in the KNICU favored exclusive breastfeeding at hospital and TSKM discharge. Weight gain and HC growth were higher in newborns receiving infant formula.


RESUMO Objetivo: Avaliar as taxas de aleitamento materno exclusivo e o crescimento dos recém-nascidos (RN) pré-termo e/ou de baixo peso ao nascer à alta da terceira etapa do Método Canguru (TEMC). Métodos: Estudo retrospectivo realizado em maternidade pública de referência entre janeiro/2014 e dezembro/2017, que incluiu os RN pré-termo (<37 semanas) e/ou de baixo peso (<2.500 g). As informações foram coletadas de prontuários médicos e analisadas pelo programa Statistical Package for the Social Sciences (SPSS). Resultados: Foram seguidos 482 RN, com mediana da idade gestacional de 33 semanas (variação: 24-39 sem) e peso ao nascer 1.715 g (variação: 455-2.830 g). O aleitamento materno exclusivo (AME) ocorreu em 336 (70,1%) RN à alta hospitalar e em 291 (60,4%) à alta da TEMC. Cada dia a mais de permanência hospitalar aumentou a chance do uso de fórmula infantil em 9,3% à alta hospitalar e em 10,3% à alta da TEMC. Permanecer na Unidade de Cuidados Intermediários Canguru (UCINCA) favoreceu o AME à alta hospitalar e da TEMC (p<0,001). Não realizar a Posição Canguru aumentou em 11% a chance de o RN utilizar fórmula infantil à alta hospitalar. O ganho de peso e crescimento do perímetro cefálico foram maiores nos RN em uso de fórmula infantil (p<0,001). Conclusões: O tempo de permanência hospitalar e a não realização da Posição Canguru favoreceram o uso de fórmula infantil à alta hospitalar e da TEMC. Permanecer na UCINCA favoreceu o AME à alta hospitalar e da TEMC. O ganho de peso e crescimento do PC foram maiores nos RN em uso de fórmula infantil.

Objetivo: construir uma teoria substantiva sobre as relações de poder vivenciadas por trabalhadoras que amamentam, que podem influenciar na decisão e manutenção do aleitamento materno no local de trabalho. Métodos: pesquisa exploratória-descritiva qualitativa, na qual se utilizaram os referenciais de Relações de Poder de Michel Foucault e a Teoria Fundamentada nos Dados, de Corbin e Strauss. Duas técnicas foram utilizadas para a coleta de dados nas empresas (Santa Catarina, Brasil): grupo focal e entrevista semiestruturada. Codificação aberta, axial e integração seletiva foram utilizadas para análise de dados, com auxílio do Software Atlas.ti, com processo de validação da teoria substantiva por sete avaliadores. Resultados: participaram dezesseis trabalhadoras e dois profissionais de saúde. A teoria substantiva gerada, "Amamentando com êxito no retorno ao trabalho em decorrência da existência de dispositivos de apoio", integra dez categorias, que representam o fenômeno central e seus três componentes: condições, ações-interações e consequências ou resultados. Conclusão: trabalhadoras vivenciam microrrelações singulares de poder no trabalho, que convergem e influenciam o poder de decisão de continuar amamentando. O desejo das trabalhadoras de amamentar, juntamente com o usufruto de direitos adquiridos e a disponibilidade de uma rede de apoio fazem com que elas perseverem no processo complexo de amamentação.


Objective: to construct a substantive theory about the power relations experienced by breastfeeding workers, which may influence the decision and maintenance of breastfeeding in the workplace. Methods: an exploratory-descriptive qualitative study, in which Michel Foucault's Power Relations and Corbin and Strauss' Grounded Theory were used as referential frameworks. Two techniques were used to collect data from social actors linked to their companies in Santa Catarina - Brazil: focus group and semi-structured interview. Open, axial coding and selective integration were used in data analysis with the aid of Atlas.ti Software and a validation process of the substantive theory by seven evaluators. Results: sixteen workers and two health professionals participated in the survey. The substantive theory generated was "Successful breastfeeding upon return to work due to the existence of support mechanisms". It integrates ten categories that represent the central phenomenon and its three components: conditions, actions-interactions, and consequences or results. Conclusion: workers experience unique micro-relations of power in the work context, which converge and influence the decision-making power regarding the continuation of breastfeeding. The workers' desire to breastfeed, together with the exercise of acquired rights and the availability of a support network, make them persevere in the complex process of breastfeeding.

Integra diez categorías que representan el fenómeno central y sus tres componentes: condiciones, acciones-interacciones y consecuencias o resultados. Conclusión: las trabajadoras experimentan micro relaciones de poder únicas en el contexto del trabajo, que convergen e influyen en el poder de decisión sobre la continuidad de la lactancia materna. El deseo de las trabajadoras de amamantar unido al disfrute de los derechos adquiridos y la disponibilidad de una red de apoyo las hacen perseverar en el complejo proceso de la lactancia materna.

Aim: This cohort study aimed to investigate the exclusive breastfeeding (EBF) prevalence and its association with maternal EBF intention and other variables. Method: Pregnant women (n=653) in the third trimester of pregnancy filled out a questionnaire on EBF intention using the Infant Feeding Intentions scale and other variables. After delivery, mothers were contacted by phone at the first (T1; n=467), third (T3; n=333), and sixth (T6; n=217) month of the baby's life or until breastfeeding was terminated, to collect data on breastfeeding, delivery, the newborn, oral habits, and family variables. The associations between independent variables and the outcomes (EBF prevalence at T1, T3, and T6) were analyzed by simple and multiple logistic regression models (α =0.05). Results: EBF prevalence was 81.2% at T1, 64.0% at T3, and 35.9% at T6. Nott use baby bottle (OR=612.59; OR=139.08; OR=15.05) and no pacifier use (OR=2.94; OR=2.78; OR=2.93) were associated with the outcome at T1, T3, and T6, respectively. Other associated variables were strong EBF intention (OR=1.71) at T1; higher birth weight (OR=1.62), childcare support (OR=2.59), maternal age (OR=0.54), being married or having a partner (OR=1.82) at T3; and not returning to work (OR=2.39) at T6. Conclusion: The EBF prevalence was high at the first month of the baby's life and decreased over six months. The EBF intention affected EBF initiation. Variables related to the mother´s demographic characteristics, the baby´s birth weight, the childcare support or the baby bottle/pacifier use were associated with EBF

ABSTRACT Objective: To investigate the repercussions of COVID-19 on the length of breastfeeding and analyze the associated factors in Belo Horizonte, Minas Gerais, Brazil. Method: This is an epidemiological, prospective cohort study. Data were collected from medical records and through telephone interviews. Women who weaned were estimated using Kaplan-Meier survival analysis. The log-rank test was used to verify differences between groups, analyzing weaning time, according to sociodemographic and clinical characteristics. The values of hazard ratio and 95% confidence intervals were estimated using Cox regression analysis. Results: A total of 1,729 women participated in the study. During the COVID-19 pandemic, brown women and women undergoing cesarean section were more likely to stop breastfeeding. Conclusion: The birth route and mothers' ethnic characteristics were associated with early weaning during the COVID-19 pandemic. Such findings are important to guide the assistance of the multidisciplinary team, especially nursing, during the post-pandemic period and in future epidemiological scenarios.

Método: Se trata de un estudio de cohorte prospectivo epidemiológico. Los datos se recogieron de historias clínicas y mediante entrevistas telefónicas. Las mujeres que destetaron se estimaron mediante el análisis de supervivencia de Kaplan-Meier. La prueba de Mantel-Cox se utilizó para verificar diferencias entre grupos, analizando el tiempo de destete, según características sociodemográficas y clínicas. Los valores de cociente de riesgo y los intervalos de confianza del 95% se estimaron mediante análisis de regresión de Cox. Resultados: Un total de 1.729 mujeres participaron en la investigación. Durante la pandemia de COVID-19, las mujeres pardas y aquellas sometidas a cesárea tenían más probabilidades de dejar de amamantar. Conclusión: La ruta de nacimiento y las características étnicas de las madres se asociaron con el destete temprano durante la pandemia de COVID-19. Tales hallazgos son importantes para orientar la asistencia del equipo multidisciplinario, especialmente de enfermería, durante el período pospandemia y en futuros escenarios epidemiológicos.


RESUMO Objetivo: Investigar as repercussões da COVID-19 na duração do aleitamento materno e analisar os fatores associados em Belo Horizonte, Minas Gerais, Brasil. Método: Trata-se de estudo epidemiológico, de coorte prospectiva. Os dados foram coletados em prontuários médicos e por meio de entrevistas telefônicas. As mulheres que apresentaram o desmame foram estimadas por meio da análise de sobrevivência de Kaplan-Meier. O teste log-rank foi utilizado para verificar diferenças entre os grupos, analisando-se o tempo de desmame, segundo as características sociodemográficas e clínicas. Os valores de hazard ratio e intervalos de confiança de 95% foram estimados utilizando-se análise de regressão de Cox. Resultados: Participaram da pesquisa 1.729 mulheres. Durante a pandemia de COVID-19, mulheres pardas e mulheres submetidas à cesariana foram mais suscetíveis a interromper a amamentação. Conclusão: A via de nascimento e as características étnicas das mães associaram-se ao desmame precoce durante a pandemia de COVID-19. Tais achados são importantes para nortear a assistência da equipe multiprofissional, sobretudo da enfermagem, durante o pós-pandemia e em cenários epidemiológicos futuros.

ABSTRACT Objective: To analyze the effect of breastfeeding on reducing Pentavalent vaccination pain in infants and to identify the necessary breastfeeding interval for antinociceptive action. Method: Open parallel randomized clinical trial. Ninety mother-infant dyads participated, distributed into intervention group 1 (n = 30), which breastfed five minutes before vaccination; intervention group 2 (n = 30), which breastfed five minutes before and during vaccination; and control group (n = 30), which did not breastfeed. The outcome variable was the pain level measured by the FLACC Scale. Data analysis was conducted using descriptive and inferential statistics, applying Fisher's Exact, Kolmogorov-Smirnov, Kruskal-Wallis and Dunn's multiple comparison tests, with 0.05 significance level. Results: Pain induced by the Pentavalent vaccine was reduced in intervention groups 1 and 2 (mean pain of 6.06 versus 3.83, respectively) compared to the control group (mean of pain of 7.43), which was significant for intervention group 2 (p < 0.001), indicating that, to achieve lower levels of pain, breastfeeding should be carried out before and during vaccination. Conclusion: Longer breastfeeding, conducted five minutes before and during vaccination, reduces the pain induced by the Pentavalent vaccine. No vaccination risks were identified to outweigh the benefits. These results endorse that health professionals should encourage breastfeeding at least five minutes before and during vaccine injection for an antinociception effect. RESUMEN Objetivo: Evaluar el efecto de la lactancia materna en la reducción del dolor durante la vacunación pentavalente en lactantes y determinar el intervalo óptimo de lactancia para obtener un efecto antinociceptivo. Método: Ensayo clínico aleatorizado, paralelo y abierto. Participaron noventa díadas madre-lactante, divididas en grupo de intervención 1 (n = 30), que amamantó cinco minutos antes de la vacunación; grupo de intervención 2 (n = 30), que amamantó cinco minutos antes y durante la vacunación; y grupo control (n = 30), que no amamantó. La variable de resultado fue el dolor, que se evaluó utilizando la Escala FLACC. Se realizó un análisis descriptivo e inferencial de los datos, aplicando las pruebas Exacta de Fisher, Kolmogorov-Smirnov, Kruskal-Wallis y Dunn para comparaciones múltiples, con un nivel de significancia de 0,05. Resultados: El dolor inducido por la vacuna Pentavalente se redujo en los grupos de intervención 1 y 2 (dolor medio de 6,06 frente a 3,83, respectivamente) en comparación con el grupo control (dolor medio de 7,43). Esta reducción fue significativa en el grupo de intervención 2 (p < 0,001), lo que sugiere que la lactancia materna antes y durante la vacunación es más efectiva para disminuir el dolor. Conclusión: Amamantar durante cinco minutos antes y durante la vacunación pentavalente reduce el dolor inducido por la vacuna Pentavalente. No se identificaron riesgos que superen los beneficios de esta práctica. Estos hallazgos sugieren que los profesionales de la salud deben promover la lactancia materna al menos cinco minutos antes y durante la administración de vacunas inyectables para lograr un efecto antinociceptivo significativo. RESUMO Objetivo: Analisar o efeito da amamentação na redução da dor induzida pela vacina Pentavalente em lactentes e identificar o intervalo de tempo da amamentação necessário para sua ação antinocicepção. Método: Ensaio clínico randomizado paralelo aberto. Participaram 90 binômios mãe-lactente, distribuídos em grupo intervenção 1 (n = 30), que realizou a amamentação cinco minutos antes da vacinação; grupo intervenção 2 (n = 30), realizou a amamentação cinco minutos antes e durante a vacinação; e grupo controle (n = 30), que não realizou a amamentação. A variável desfecho foi o nível de dor mensurado pela Escala FLACC. A análise dos dados foi realizada por meio de estatística descritiva e inferencial, com aplicação dos testes Exato de Fisher, Kolmogorov-Smirnov, Kruskal-Wallis e de comparações múltiplas de Dunn, adotando nível de significância de 0,05. Resultados: A dor induzida pela vacina Pentavalente se reduziu nos grupos intervenção 1 e 2 (média de dor de 6,06 versus 3,83, respectivamente) em comparação ao grupo controle (média de dor de 7,43), o que foi significativo para o grupo intervenção 2 (p < 0,001), indicando que, para alcançar menores níveis de dor, a amamentação deve ocorrer antes e durante a vacinação. Conclusão: A amamentação mais prolongada, realizada cinco minutos antes e durante todo o processo de vacinação, reduz a dor induzida pela vacina Pentavalente. Em sua aplicação não foram identificados riscos capazes de superar os benefícios de tal prática. Esses resultados endossam a importância de os profissionais de saúde incentivarem essa prática no tempo mínimo de cinco minutos antes e durante a aplicação de vacinas injetáveis para obtenção do efeito antinocicepção.
