ABSTRACT

As queimaduras provocam efeitos físicos e psicológicos devastadores nos indivíduos, sobretudo em crianças e adolescentes, e podem modificar a qualidade de vida da pessoa. O objetivo foi analisar o impacto das cicatrizes por queimaduras em crianças menores de oito anos na interação com amigos, família e escola, na perspectiva dos pais. Estudo quantitativo de corte transversal realizado com os pais de crianças <8 anos de idade, vítimas de queimaduras e internadas em um Centro de Tratamento de Queimados no norte do Paraná e acompanhadas ambulatorialmente, de 2017 a 2020. A coleta de dados ocorreu por meio de dois instrumentos: caracterização sociodemográfica e clínica; e Brisbane Burn Scar Impact Profile. Realizou-se análise descritiva e teste Qui-quadrado utilizando-se o SPSS®. Participaram 34 pais cujas crianças sofreram queimaduras, sendo 52,9% de 1 a 3 anos de idade, 58,8% sexo masculino, 82,2% por agente etiológico térmico e a internação foi de 73,5% devido à Superfície Corpórea Queimada ≤20%. Após a alta os pais identificaram que as cicatrizes de queimaduras tinham "um pouco" e "pouco" impacto nas cicatrizes nas relações de amizade e na interação social. Para os pais, prevaleceu a resposta "nada" de impacto, seguido por "um pouco" e "muito" na escola, nas brincadeiras, nos jogos e nas atividades diárias. Quanto às reações emocionais e ao humor, a maior parte dos pais considerou "nada". Nesse sentido, os pais responderam às questões quanto à própria percepção sobre as atividades diárias do seu filho e, em geral, a cicatriz de queimadura não impactou na qualidade de vida da criança.


Burns cause devastating physical and psychological effects on individuals, especially children and adolescents, and can change a person's quality of life. The objective was to analyze the impact of burn scars in children under eight years of age in the interaction with friends, family and school, from the parents' perspective. Quantitative cross-sectional study carried out with the parents of children <8 years old who were victims of burns and admitted to a Burn Treatment Center in northern Paraná and monitored on an outpatient basis, from 2017 to 2020. Data collection occurred using two instruments: sociodemographic and clinical characterization; Brisbane Burn Scar Impact Profile. Descriptive analysis and Chi-square test were performed using SPSS®. 34 parents participated whose children suffered burns, 52.9% aged 1 to 3 years old, 58.8% male, 82.2% due to thermal etiological agent and 73.5% hospitalization was due to Burned Body Surface ≤ 20%. After discharge, the parents identified that the burn scars had "a little" and "little" impact on the scars in friendship relationships and social interaction. For parents, the answer "nothing" of impact prevailed, followed by "a little" and "a lot" in school, play, games and daily activities. As for emotional reactions and mood, most parents considered "nothing" that impacted the child with burn scars. In this sense, parents answered questions regarding their own perception of their child's daily activities and, in general, the burn scar did not impact the child's quality of life.

Introdução: De grande impacto na população, queimaduras exigem análise epidemiológica e planejamento constantes para prevenção, tratamento e reabilitação dos pacientes. Este trabalho objetiva comparar, após uma década, os indicadores do Centro de Tratamento de Queimados do Hospital João XXIII, em Belo Horizonte, MG, abordados no artigo "Epidemiologia das queimaduras no estado de Minas Gerais", publicado na Revista Brasileira de Cirurgia Plástica com dados de 2010, para validar as estratégias vigentes e as futuras. Método: Revisão dos prontuários dos pacientes acometidos por queimadura, internados no referido centro em 2020. Resultados: Foram internadas 473 vítimas de queimadura no período, 87,5% causadas por acidente, sendo 34,5% por líquidos quentes, 23,7% por álcool; 61,9% provenientes do interior do estado de Minas Gerais; 63,4% do sexo masculino. A idade média foi de 30 anos, a superfície corporal queimada média foi de 18,8% e o tempo médio de internação foi de 25 dias. Foram realizados 580 desbridamentos cirúrgicos e 473 enxertos cutâneos autólogos. Faleceram 7,4% dos pacientes, correspondentes a 29,5% dos internados no CTI adulto, com superfície corporal queimada média de 49,7%, e 10,5% dos internados no CTI pediátrico. A maior causa de óbitos foi devido à sepse, em 57,1% dos casos. A mortalidade diminuiu de 16,3% para 7,4% no período estudado. Conclusão: O perfil do paciente internado por queimadura mantevese em grande parte o mesmo após 10 anos. Houve aumento do número de atendimentos a vítimas de queimadura do interior do estado e queimaduras provocadas por líquidos quentes passaram a ser mais frequentes que por álcool. ''A busca da conformidade com o tratamento baseado na literatura mundial resultou em diminuição da mortalidade."


Introduction: With a major impact on the population, burns require epidemiological analysis and constant planning for the prevention, treatment, and rehabilitation of patients. This work aims to compare, after a decade, the indicators of the Burn Treatment Center at Hospital João XXIII, in Belo Horizonte, MG, covered in the article "Epidemiology of burns in the state of Minas Gerais", published in the Revista Brasileira de Cirurgia Plástica with data from 2010, to validate current and future strategies. Method: Review of the medical records of patients suffering from burns, admitted to the aforementioned center in 2020. Results: 473 burn victims were hospitalized during the period, 87.5% were caused by an accident, 34.5% due to hot liquids, 23.7% by alcohol; 61.9% from the interior of the state of Minas Gerais; and 63.4% were male. The average age was 30 years, the average burned body surface area was 18.8% and the average length of stay was 25 days. 580 surgical debridement and 473 autologous skin grafts were performed. 7.4% of patients died, corresponding to 29.5% of those admitted to the adult ICU, with an average burned body surface area of 49.7%, and 10.5% of those admitted to the pediatric ICU. The biggest cause of death was sepsis, in 57.1% of cases. Mortality decreased from 16.3% to 7.4% in the period studied. Conclusion: The profile of patients hospitalized for burns remained largely the same after 10 years. There was an increase in the number of visits to burn victims in the interior of the state and burns caused by hot liquids became more frequent than those caused by alcohol ''The search for compliance with treatment based on world literature resulted in reduction in mortality."

Introdução: As queimaduras são um desafio da saúde pública devido à alta morbimortalidade e prejuízo na qualidade de vida da vítima. Elas afetam desproporcionalmente as populações de menor nível socioeconômico, resultando em elevados custos para saúde. Método: Estudo ecológico, retrospectivo, observacional, com abordagem quantitativa e análise de tendência temporal da morbimortalidade por queimadura em Santa Catarina, com dados obtidos dos Sistemas de Informações Hospitalar e de mortalidade disponibilizados pelo Departamento de Informática do Sistema Único de Saúde. Análise temporal pelo Teste de Correlação de Spearman. Resultados: Verificada tendência de crescimento na taxa geral de internação (Spearman=0,806; p<0,005) por queimaduras no estado no período analisado. Maior prevalência no sexo masculino (RP 1,68), na população de 0 a 4 anos (RP 3,08) e na região da Grande Florianópolis (taxa média 23,22%). Predominou o grupo classificado como médio queimado (taxa média 25,67%) e as internações de 0 a 3 dias (taxa média 50,25%). Queimaduras em cabeça, pescoço e tronco (taxa média 32,25%) foram as mais prevalentes. Conclusão: Identificada tendência de crescimento na taxa de internação por queimaduras em crianças no estado. Maior prevalência de internação no sexo masculino, em crianças de 0 a 4 anos e na região da Grande Florianópolis. Predomínio de médio queimados e de queimaduras em cabeça, pescoço e tronco, com maior taxa de internações de curta duração.


Introduction: Burns is a public health challenge due to high morbidity and mortality and impairment of the victim's quality of life. They disproportionately affect populations of lower socioeconomic status, resulting in high health costs. Method: Ecological, retrospective, observational study, with a quantitative approach and temporal trend analysis of morbidity and mortality due to burns in Santa Catarina, with data obtained from the Hospital and Mortality Information Systems made available by the Information Technology Department of the Unified Health System. Temporal analysis by Spearman Correlation Test. Results: There was a growing trend in the general hospitalization rate (Spearman=0.806; p<0.005) for burns in the state in the period analyzed. Higher prevalence in males (RP 1.68), in the population aged 0 to 4 years (RP 3.08), and in the Greater Florianópolis region (mean rate 23.22%). The group classified as medium burn predominated (mean rate 25.67%) and hospitalizations of 0 to 3 days (mean rate 50.25%). Burns to the head, neck, and trunk (mean rate 32.25%) were the most prevalent. Conclusion: A growth trend was identified in the hospitalization rate for burns in children in the state. Higher prevalence of hospitalization in males, in children aged 0 to 4 years, and in the Greater Florianópolis region. Predominance of moderate burns and burns to the head, neck, and trunk, with a higher rate of short-term hospitalizations.

Introdução: Queimaduras são lesões teciduais causadas pelo contato com fontes de calor. Representam um problema de saúde pública global. Em crianças causam grandes impactos. A gravidade e intensidade das queimaduras estão relacionadas ao agente etiológico e suas consequências são um grande obstáculo para o paciente. Método: O estudo analisou dados de internações hospitalares por queimaduras em crianças de 2012 a 2022, utilizando informações do SIH-SUS. Foram calculadas taxas de internações por 100.000 habitantes para cada ano, considerando variáveis dependentes. Os pesquisadores utilizaram coeficientes padronizados e regressão linear simples para analisar os dados. Resultados: Incluíram-se dados de 91.091 internações por queimaduras em crianças, 0-14 anos. Verificou-se estabilidade na taxa geral de internações (taxa média 17,963; ß=0,119; p=0,163). No sexo feminino houve tendência de aumento (taxa média 14,346; ß=0,169; p=0,029); no masculino de estabilidade (taxa média 21,426; ß=0,069; p=0,504). O sexo feminino comportou-se com estabilidade em todas as faixas etárias; o masculino com aumento na faixa 0-4 anos (taxa média 42,264; ß=0,613; p=0,003), estabilidade na faixa 5-9 anos (taxa média 14,189; ß=-0,21; p=0,867) e redução na faixa 10-14 anos (taxa média 9,871; ß= -0,328; p=0,007). A Região Sul demonstrou tendência de aumento (taxa média 26,952; ß=1,091; p=0,001). Conclusão: Houve estabilidade na taxa geral de internações. O sexo feminino tendeu ao aumento e o masculino à estabilidade. Houve estabilidade nas regiões brasileiras, exceto no Sul.


Introduction: Burns are tissue injuries caused by contact with heat sources. They represent a global public health problem. They have major impacts on children. The severity and intensity of burns are related to the etiological agent and their consequences are a major obstacle for the patient. Method: The study analyzed data on hospital admissions for burns in children from 2012 to 2022, using information from SIH-SUS. Hospitalization rates per 100,000 inhabitants were calculated for each year, considering dependent variables. The researchers used standardized coefficients and simple linear regression to analyze the data. Results: Data from 91,091 hospitalizations for burns in children, 0-14 years old, were included. There was stability in the general hospitalization rate (mean rate 17.963; ß=0.119; p=0.163). In females there was an increasing trend (mean rate 14.346; ß=0.169; p=0.029); in males, there was stability (mean rate 21.426; ß=0.069; p=0.504). Females behaved with stability in all age groups; the male with an increase in the range 0-4 years (mean rate 42.264; ß=0.613; p=0.003), stability in the range 5-9 years (mean rate 14.189; ß=-0.21; p=0.867) and reduction in the range 10-14 years (mean rate 9.871; ß= -0.328; p=0.007). The South Region demonstrated an increasing trend (mean rate 26.952; ß=1.091; p=0.001). Conclusion: There was stability in the general hospitalization rate. The female sex tended towards increase and the male towards stability. There was stability in Brazilian regions, except in the South.

ABSTRACT

Objetivo: Mapear a produção científica a respeito do uso da terapia de pressão negativa no tratamento de queimaduras, em pacientes adultos e idosos hospitalizados. Método: Revisão de escopo seguindo o método da JBI com busca da produção científica na PubMed, Scientific Electronic Library Online, Biblioteca Virtual de Saúde e Banco de Teses da Coordenação de Aperfeiçoamento de Pessoal de Nível Superior nos meses de março a maio de 2021. Foram incluídos estudos com pacientes adultos e idosos com queimaduras, submetidos à terapia por pressão negativa no hospital. Não houve restrição de idioma, nem período de tempo. Resultados: Foram identificadas 106 publicações e, após o processo de seleção, cinco estudos compuseram a amostra final. Os resultados demonstraram que a aplicação da terapia por pressão negativa em queimaduras constituiu-se como uma ferramenta efetiva na cicatrização de queimaduras térmicas, químicas e elétricas, destacando-se a otimização do tempo de cicatrização, formação de um tecido de granulação saudável, diminuição do edema, redução da infecção, drenagem e monitorização do edema. Conclusão: O tratamento de queimaduras utilizando a pressão negativa tem se mostrado um método efetivo, visto os benefícios que promove no processo cicatricial. Sugere-se que a temática seja mais difundida, aprofundando-se o conhecimento sobre o uso desta tecnologia. (AU)


Objective: To map the scientific production regarding the use of negative pressure wound therapy in burn treatment of hospitalized adult and aged patients. Method: Scope review following the JBI method with a search for scientific production in PubMed, the Scientific Electronic Library Online, the Virtual Health Library, and the Thesis Database of the Coordination for the Improvement of Higher Education Personnel from March to May 2021. Studies with adult and aged burn patients undergoing negative pressure wound therapy in hospitals were included. There was no language or time frame restriction. Results: 106 publications were identified and 5 studies made up the final sample after the selection process. The results demonstrated that applying negative pressure wound therapy to burns was effective in healing thermal, chemical, and electrical burns, esoecially regarding healing time optimization, healthy granulation tissue formation, edema and infection reduction, and edema drainage and monitoring. Conclusion: Burn treatment using negative pressure has proven effective, given the benefits it promotes in the healing process. The theme should be more widely spread to deepen the knowledge about the use of this technology. (AU)


Objetivo: Mapear la producción científica sobre el uso de la terapia de presión negativa en el tratamiento de quemaduras en pacientes adultos y ancianos hospitalizados. Método: Revisión de alcance siguiendo el método JBI con búsqueda de producción científica en PubMed, Biblioteca Científica Electrónica en Línea, Biblioteca Virtual en Salud y Banco de Tesis de la Coordinación para el Perfeccionamiento del Personal de Educación Superior en los meses de marzo a mayo de 2021. Se incluyeron estudios con adultos y pacientes ancianos quemados sometidos a terapia de presión negativa en el hospital. No hubo restricción de idioma ni período de tiempo. Resultados: Se identificaron 106 publicaciones y luego del proceso de selección, 5 estudios conformaron la muestra final. Los resultados demostraron que la aplicación de la terapia de presión negativa en quemaduras constituyó una herramienta eficaz en la curación de quemaduras térmicas, químicas y eléctricas, destacándose la optimización del tiempo de curación, formación de tejido de granulación sano, reducción de edema, reducción de infección, drenaje y seguimiento del edema. Conclusión: El tratamiento de quemaduras mediante presión negativa ha demostrado ser un método efectivo, dados los beneficios que promueve en el proceso de cicatrización. Se sugiere que el tema sea más difundido, profundizando el conocimiento sobre el uso de esta tecnología. (AU)

Objetivos: Avaliar a relação entre a intensidade do prurido e os escores de ansiedade, depressão, estresse pós-traumático e a qualidade de vida relacionada à saúde em pacientes vítimas de queimaduras. Método: Estudo transversal, realizado em dois ambulatórios de hospitais de queimados localizados em municípios do interior paulista. A amostra foi constituída por 60 pacientes que responderam aos instrumentos de caracterização sociodemográfica e clínica, Escala Visual Numérica para avaliação da intensidade do prurido, Escala Hospitalar de Ansiedade e Depressão, Escala de Impacto de Eventos para avaliação do estresse pós-traumático e a versão brasileira da Freiburg Life Quality Assessment Wound. Resultados: A amostra caracterizou-se pelo predomínio do sexo masculino (73%). Correlações positivas significativas foram encontradas entre a intensidade de prurido e ansiedade (r=0,33), depressão (r=0,53), estresse pós-traumático (r=0,43) e escore total da Freiburg Life Quality Assessment Wound (r=0,63). Conclusão: Esses achados reforçam a importância da avaliação do prurido na atenção integral à vítima de queimadura visando à redução do impacto psicológico e melhora na qualidade de vida relacionada à saúde. Objective: To assess the relationship between pruritus intensity and anxiety, depression, post-traumatic stress, and health-related quality of life scores in burn victims. Methods: A cross-sectional study, carried out in two outpatient clinics of burn hospitals located in municipalities in the countryside of São Paulo. The sample consisted of 60 patients who answered sociodemographic and clinical characterization instruments: the Visual Numerical Scale, to assess pruritus intensity, the Hospital Anxiety and Depression Scale, the Impact of Events Scale, to assess post-traumatic stress, and the Freiburg Life Quality Assessment Wound Module to Brazilian Portuguese. Results: The sample was characterized by a predominance of males (73%). Significant positive correlations were found between pruritus intensity and anxiety (r=0.33), depression (r=0.53), post-traumatic stress (r=0.43), and total Freiburg Life Quality Assessment Wound score (r=0.63). Conclusions: These findings reinforce the importance of assessing pruritus in comprehensive care for burn victims, aiming to reduce the psychological impact and improve health-related quality of life. ABSTRACT

Objetivo: A síndrome do choque tóxico (SCT) é uma condição multissistêmica grave, causada por Staphylococcus aureus ou Streptococcus pyogenes,e o manejo inicial e contínuo das lesões de pele é essencial para o controle da infecção. Este relato teve o objetivo de descrever os cuidados com a pele nessa síndrome. Relato do caso: Paciente do sexo masculino com abscesso no quadril que evoluiu com febre e hipotensão e consequente transferência para a Unidade de Terapia Intensiva (UTI). Foi diagnosticado com choque séptico de provável foco no quadril, imediatamente abordado cirurgicamente para tratamento local da infecção. Apresentou insuficiência renal, além de disfunção multissistêmica (hepática e cardíaca), instabilidade hemodinâmnica e lesões disseminadas de pele. Com o isolamento do Staphylococcus aureus, iniciou-se clindamicina e o diagnóstico foi de SCT, uma vez que apresentou lesões epidermolíticas desde o início do quadro. Foram iniciadas medidas de recuperação cutânea com a sulfadiazina de prata e, posteriormente, com hidrofibra com prata com restauração do epitélio em 8 dias. Após 10 dias da pele restaurada, o paciente recebeu alta da UTI para enfermaria com manutenção do tratamento proposto. Conclusão: Neste caso, o uso da hidrofibra obteve uma performancesatisfatória, contudo ainda há necessidade de estudos robustos que comprovem tal eficácia. (AU)


Objective: Toxic shock syndrome (TSS) is a severe multisystemic condition caused by Staphylococcus aureus or Streptococcus pyogenes, and initial management of skin lesions is essential for infection control. This article aimed to describe skin care for TSS. Case report: We report a man with a hip abscess who developed fever and hypotension and was subsequently transferred to an intensive care unit (ICU). He was diagnosed with septic shock, probably of hip origin, and was immediately treated surgically for local infection control. He presented with renal failure besides multiple organ dysfunction (hepatic and cardiac), hemodynamic instability, and disseminated skin lesions. With the isolation of Staphylococcus aureus, clindamycin was initiated, and the diagnosis was TSS due to epidermolytic lesions since the onset of the condition. Cutaneous recovery measures were initiated with silver sulfadiazine, followed by silver hydrofiber with skin recovery in 8 days. After 10 days of skin recovery, the patient was discharged from the ICU to the ward with maintenance of the proposed treatment. Conclusion: In this case, the use of hydrofiber showed satisfactory performance; however, robust studies are needed to confirm such efficacy. (AU)


Objetivo: El síndrome de shock tóxico (SST) es una afección multisistémica grave causada por Staphylococcus aureuso Streptococcus pyogenes, y el tratamiento inicial y continuo de las lesiones cutáneas es esencial para controlar la infección. Este informe tuvo como objetivo describir los cuidado de la piel en este síndrome. Reporte del caso:Paciente masculino con absceso en la cadera que desarrolló fiebre e hipotensión y fue tranferido a la Unidad de Cuidados Intensivos (UCI). Se diagnosticó shock séptico, probablemente focalizado en la cadera, por lo que fue inmediatamente abordado quirúrgicamente para tratamiento local de la infección. Presentó insuficiencia renal además de disfunción multisistémica (hepática y cardiaca), inestabilidad hemodinámica y lesiones cutáneas diseminadas. Con el aislamiento de Staphylococcus aureus, se inició clindamicina y el diagnóstico fue SST, ya que presentaba lesiones epidermolíticas desde el inicio del cuadro. Se iniciaron medidas de recuperación cutánea con sulfadiazina de plata y, posteriormente, se optó por hidrofibra con plata, con restauración del epitelio en 8 días. Después de 10 días de piel restaurada, el paciente fue dado de alta de la UCI a planta con mantenimiento del tratamiento propuesto. Conclusión: Alcen este caso, el uso de hidrofibra obtuvo un desempeño satisfactorio, sin embargo, aún se necesitan estudios robustos para probar tal eficacia. (AU)

ABSTRACT

As queimaduras são um problema de saúde pública e geram repercussões físicas, psicológicas e sociais, podendo evoluir ao óbito. O objetivo deste estudo é desenvolver um jogo que possa distribuir informação, com foco na prevenção de queimaduras, servindo como alternativa para educação em saúde. Trata-se de uma pesquisa tecnológica para o desenvolvimento experimental de um game focando na prevenção de queimaduras infantis com a utilização do método da problematização pautada no Arco de Maguerez. A gamificação é uma alternativa inovadora para a capacitação e mudança de comportamento e a educação em saúde é uma ferramenta estratégica que auxilia e fortalece o cuidado. O jogo foi feito para crianças de seis a dez anos e será disponibilizado em ambiente virtual para dispositivo Android. Foi solicitado auxílio de um profissional em linguagem de programação para programar o jogo. Atualmente é necessário acompanhar as inovações e tecnologias, melhorando a interação com as crianças.


Burns are a public health problem and result in physical, psychological as well as social repercussions, leading to death. The objective of this study is to develop a game that can distribute information, with the focus on burn prevention and serving as an alternative for health education. This is technological research for the experimental development of a game focusing on the prevention of childhood burns and using the problematization method based on the Maguerez Arch. Gamification is an innovative alternative for training and for behavioral changing and health education is a strategic tool that helps and strengthens care. The game was made for children aged six to ten years old and will be made available in a virtual environment for Android devices. Assistance from a programming language professional was required to program the game. Nowadays, it is necessary to keep up with innovations and technologies, improving interaction with children.


Las quemaduras son un problema de salud pública y generan repercusiones físicas, psicológicas y sociales, provocando la muerte. El objetivo de este estudio es desarrollar un juego que pueda difundir información, com el foco en em la prevención de quemaduras, sirviendo como una alternativa para la educación en salud. Se trata de una investigación tecnológica para el desarrollo experimental de um juego enfocado a la prevención de quemaduras infantiles mediante el método de problematización basado en el Arco de Maguerez. La gamificación y la educación en salud es una herramienta estratégica que ayuda y fortalece los cuidados. ABSTRACT

BACKGROUND:How to provide sufficient skin resources for scar plastic surgery and repair of extensive deep burn patients while avoiding the re-proliferation of scar tissue in the surgical area has always been an important topic in burn and wound repair research. OBJECTIVE:To observe the clinical application effects of artificial dermis combined with autologous scar epidermis in the repair of scar after extensive burns. METHODS:Retrospective analysis was performed on 73 patients with scar hyperplasia and contracture deformity after extensive burns in Bengbu Third People's Hospital Affiliated to Bengbu Medical College from January 2021 to January 2023.The patients were divided into three groups according to the treatment method:Group A(n=21,artificial dermis combined with autologous scar epidermis transplantation was used for treatment),group B(n=27,scar epidermis was transplanted after scar release in the functional site),and group C(n=25,functional site scar release after transplantation of thick skin treatment).Skin survival and infection at the receiving site,wound healing time at the receiving site and the donor site were recorded in the three groups.The scar status and functional recovery of the recipient area and donor area were evaluated by the Vancouver Scar Scale and activities of daily living. RESULTS AND CONCLUSION:(1)The skin infection rate was lower in group B than that in groups A and C(P<0.05).The survival grade was higher in group B than that in groups A and C(P<0.05).(2)The wound healing time at the receiving site was longer in group A than that in groups B and C(P<0.05).The wound healing time at the receiving site was longer in group C than that in group B(P<0.05).The wound healing time at the donor site was longer in group C than that in groups A and B(P<0.05).(3)Vancouver Scar Scale score was higher in group B than that in groups A and C at 12 months postoperatively(P<0.05).Vancouver Scar Scale score was higher in group C than that in groups A and B at 6 and 12 months postoperatively(P<0.05).The excellent grade of activities of daily living in groups A and C was significantly higher than that of group B at 12 months postoperatively(P<0.05).(4)The results showed that the application of artificial dermis combined with autologous scar epidermis composite transplantation in the treatment of scar contracture after extensive burn could not only achieve the same effect as that of intermediate-thickness skin,but also avoid postoperative scar re-hyperplasia at the donor site and shorten the time of complete wound healing at the donor site.Compared with scar epidermal transplantation,this treatment has obvious advantages.

ABSTRACT

Objective:To explore the clinical effect of composite skin transplantation combined with systemic rehabilitation in the treatment of extensive scar contracture deformity around the popliteal fossa in children after burns.Methods:A retrospective observational research method was adopted. Seventeen children with extensive scar contracture deformities around the popliteal fossa after burns who met the inclusion criteria and were admitted to the First Affiliated Hospital of Air Force Military Medical University from March 2018 to April 2022 were selected. Among them, there were 10 males and 7 females, aged 2-11 years, with scar contracture deformities lasting from 10 months to 9 years, all located around the popliteal fossa, 10 cases of right popliteal fossa, 5 cases of left popliteal fossa, 2 cases of bilateral popliteal fossa, scars around the popliteal fossa result in a knee joint extension angle of only 95° to 115°. The scar contracture during surgery was thoroughly released, joint mobility was restored, so as to form a secondary wound range of 10 cm×8 cm-20 cm×13 cm. In stage Ⅰ, after completely releasing the scar contracture, the wound was covered with negative pressure closure drainage (VSD) for 2-3 days. In stage Ⅱ, a large autologous blade thick scalp and allogeneic decellularized dermal matrix composite graft was performed to repair the wound around the popliteal fossa. After 8-10 days of surgery, the dressing was changed to check the survival of the skin graft. One week after the skin graft survived, a 12 month orderly knee joint function training was conducted under the guidance of a rehabilitation therapist. Postoperative sequential treatment with a combination of strong pulsed light and ultra pulsed carbon dioxide lattice laser for 5-7 courses of significant scar hyperplasia in the skin graft area and edges.Results:15 cases of pediatric patients had good skin graft survival; One patient developed a wound due to partial displacement of the transplanted autologous scalp, and one patient developed a plasma swelling under the limb graft, which was drained through an opening. Two patients underwent dressing changes for 3 weeks before the wound healed. After follow-up for 6 to 36 months, the elasticity and appearance of the skin graft were similar to those of a medium thickness skin graft. Children with knee joint contracture were able to fully extend to 180°, and knee joint function was significantly improved. There was no scar formation or hair loss in the donor skin area.Conclusions:The combination of composite skin transplantation and systematic rehabilitation has a good effect on the treatment of extensive scar contracture around the popliteal fossa in children after burns, avoiding the problem of scars left in the donor area due to autologous skin grafting.

ABSTRACT

Objective:To compare the therapeutic effects of modified plantar skin grafting and thigh skin grafting on the deep burn wounds of the back and buttocks.Methods:A retrospective cohort study was conducted to analyze the clinical data of 30 patients with deep burn wounds on their back and buttocks who were admitted to the 910th Hospital of Joint Logistic Support Force of PLA from January 2021 to April 2023, including 26 males and 4 females, aged 21-72 years [(49.9±14.0)years]. The total burn size was 50%-97% of the total body surface area (TBSA), with the third-degree burn on the back and buttocks 6%-16% TBSA. The burn wounds on the back and buttocks were repaired using plantar skin grafts alone, thigh skin grafts alone or plantar skin grafts combined with the grafts from other body parts. The patients were grouped according to the skin graft donor sites and the times of harvesting skin grafts: there were 20 patients undergone plantar skin grafting including 10 patient with plantar skin graft harvested once (group of plantar skin graft harvested once) and 10 patients with plantar skin graft harvested twice or three times (group of plantar skin graft harvested more than once), and 10 patients undergone thigh skin grafting harvested once (group of thigh skin graft harvested once). The areas of plantar skin grafts harvested at the last time and the wound areas on the back and butts that could be repaired each time were calculated. After the last harvest, the thickness of the stratum corneum, 7-day survival rate of the skin grafts, proportion of 3-month residual wound area in the skin graft area, healing time of the donor sites, and 6-month Vancouver Scar Scale (VSS) scores of the donor sites in the group of plantar skin graft harvested once were compared with those in the group of thigh skin graft harvested once and the group of plantar skin graft harvested more than once. The appearance and texture of the skin graft, patients′ walking patterns and complications were observed at 6 months after the last skin harvest.Results:All the patients were followed up for 6-18 months [(7.8±1.6)months]. In the 20 patients with plantar skin grafts harvested, the areas of skin grafts harvested at the last time were 2.5%-4.5% TBSA [(3.4±0.6)% TBSA] and the wound areas that could be repaired each time were 3%-8% TBSA [(5.5±1.5)% TBSA]. After the last harvest, the thickness of the stratum corneum in the group of plantar skin graft harvested once was (190.4±8.9)μm, which was significantly thicker than that in the group of thigh skin graft harvested once [(50.0±6.6)μm] and that in the group of plantar skin graft harvested more than once [(166.8±21.9)μm] ( P<0.01); the 7-day survival rate of the skin grafts, proportion of 3-month residual wound area in the skin graft area, healing time of the donor sites, and 6-month VSS scores of the donor sites were (93.6±2.3)%, 2.0 (0.1, 3.5)%, (9.9±1.8)days and (1.7±0.7)points in the group of plantar skin graft harvested once, (78.0±6.6)%, 5.3 (4.0, 5.8)%, (14.0±1.4)days and (4.9±2.3)points in the group of thigh skin graft harvested once, and (93.4±2.6) %, 2.0 (0.1, 3.8)%, (10.0±1.2)days and (1.8±0.8)points in the group of plantar skin graft harvested more than once. The group of plantar skin graft harvested once showed a significant increase in the 7-day survival rate and a significant decrease in the proportion of 3-month residual wound area in the skin graft area, healing time of the donor sites, and 6-month VSS scores of the donor sites in comparison with the group of thigh skin graft harvested once ( P<0.05 or 0.01), while there were no significant differences in above mentioned indices between the group of plantar skin graft harvested once and the group of plantar skin graft harvested more than once ( P>0.05). At 6 months after the last skin harvest, the skin graft areas on the back and buttocks were flat, hard and firm and all the patients in the three groups could walk normally, with no complications such as severe itching, pain or folliculitis in the skin graft area. Conclusions:In the treatment of burn wounds on the back and buttocks, compared with thigh skin grafting, modified plantar skin grafting has advantages of thicker stratum corneum, better wear resistance and pressure resistance in the skin graft areas, a higher survival rate of skin grafts, rapid healing, mild scar, and undisturbed walking pattern after surgery and no common complications. Moreover, skin grafts can be harvested repeatedly from the donor sites, with no impact on the therapeutic effects.

Objective To systematically evaluate and integrate qualitative research related to the psychological experiences and perceptions of body image in bum patients.The goal is to provide a reference basis for developing targeted intervention measures in clinical practice.Methods The databases of CNKI,Wanfang,VIP Chinese Science and Technology Periodical Database,China Biomedical Literature Service System,PubMed,Web of Science,Embase,Medline,Cochrane Library,PsycINFO,and Scopus were applied in our study.Relevant qualitative studies on the body image experience of burn patients from the establishment of the databases until December 2022 were collected.The quality of the included literature was assessed using the 2016 edition of the Joanna Briggs Institute(JBI)Centre for Evidence-based Health Care in Australia,and the results of the literature were integrated using aggregative integration.Results A total of 12 pieces of literature were included,from which 40 research findings were extracted.These findings were ultimately summarized into 4 integrated results:diverse traumatic perceptions of body image changes faced by burn patients,active exploration and negative coping strategies towards body image changes faced by burn patients,emotional information needs and post-traumatic growth.Conclusion Burn patients experience various body image issues that hinder their return to normal life.It is crucial for healthcare professionals to timely attend to the physical and mental well-being of the patients,identify body image disorders,assist families in meeting emotional needs,support patients in self-adjustment,and promote positive outcomes.

ABSTRACT

Objective:pH low insertion peptide (pHLIP)-variant 7 (var7)-fluorescein isothiocyanate (FITC) was used to explore an accurate imaging tool that targeted burn wounds to better perform burn debridement.Methods:Twelve rat models of burn wound were established and pHLIP-var7-FITC with different concentrations (0.5, 1.5 and 2.0 mg/ml) were injected from the rat tail vein for in vivo fluorescence imaging. By determining the concentration of fluorescent conjugates to the burn wound, the scope of wound injury necrosis was judged by combining pathological sections, and its residue and toxicity in important organs such as heart, liver, kidneys, and brain were detected. The Kruskal-Wallis rank sum test, Bonferroni correction method and one-way analysis of variance were used for data analysis. Results:Within 24 h, the fluorescence photons per unit area of the burn wound in the group of 0.5 mg/ml, 1.5 mg/ml and 2.0 mg/ml were 1.49(1.31, 1.65), 2.46(1.88, 2.68), 2.77 (1.94, 3.10)×10 7 p·s -1·cm -2·Sr -1, with significant differences in the overall distribution of fluorescence photons ( H=73.55, P<0.001). The fluorescence intensity was stronger in the group with higher concentration, but with no significant difference in the number of fluorescence photons between the group of 1.5 mg/ml and 2.0 mg/ml ( P=0.263, Bonferroni correction method). At 14 time points (0.5, 1.0, 1.5, 2.0, 2.5, 3.0, 3.5, 4.0, 5.0, 6.0, 7.0, 8.0, 12, 24 h), there was no significant difference in the overall mean of fluorescence photons ( F=1.04, P=0.419), and the tissue with burn necrosis seen in tissue sections was highly consistent with the fluorescence imaging region. There was no obvious fluorescence residue in the heart, liver, kidney and brain sections. Conclusion:In superficial second-degree burn tissue, pHLIP-var7-FITC can accurately target and gather on the burn wound within 24 h, showing a clear boundary between burn tissue and normal tissue, which can assist clinical surgical debridement to determine the extent of injury.

Abstract This study was carried out to evaluate the effect of Glutamine, as a dipeptide or a free amino acid form, on the progression of burn injuries in rats. Thirty male Wistar rats were burned with a comb metal plate heated in boiling water (98 °C) for three minutes, creating four rectangular full-thickness burn areas separated by three unburned interspaces (zone of stasis) in both dorsum sides. The animals were randomized into three groups (n=10): saline solution (G1-Control) and treated groups that orally received Glutamine as dipeptide (G2-Dip) or free amino acid (G3-FreeAA). Two and seven days after burn injury, lesions were photographed for unburned interspaces necrosis evolution assessment. Seven days after injury, glutathione seric was measured and histopathological analysis was performed. By photographs, there was a significant reduction in necrosis progression in G3-Free-AA between days two and seven. Histopathological analysis at day 7 showed a significantly higher stasis zone without necrosis and a higher number of fibroblasts in G2-Dip and G3-FreeAA compared with G1-Control. Also, glutathione serum dosage was higher in G2-Dip. The plasmatic glutathione levels were higher in the G2-Dip than the G1-Control, and there was a trend to higher levels in G3-FreeAA. The reduction in histological lesions, greater production of fibroblasts, and greater amounts of glutathione may have benefited the evolution of burn necrosis, which showed greater preservation of interspaces.

Introducción: Las quemaduras graves alteran de manera significativa los parámetros hematológicos y de la coagulación. Objetivo: Describir las alteraciones hematológicas en el paciente gran quemado. Métodos: Se realizó un estudio descriptivo, longitudinal y prospectivo, en pacientes ingresados en el servicio de Cirugía Plástica y Caumatología del Hospital Universitario Manuel Ascunce Domenech, de la provincia Camagüey, durante el período comprendido entre mayo de 2020 a abril de 2022. Se estudiaron 38 pacientes, se tuvieron en cuenta las siguientes variables: alteración hematológica, superficie corporal quemada, índice pronóstico, estado al egreso. Resultados: De los pacientes con 30 % o más de superficie corporal quemada (12 en total) el 91,66 % presentó anemia. La anemia se encontró con más frecuencia en los pacientes clasificados como muy graves, 11 pacientes que representan el 28,95 %, es de destacar que los pacientes con este índice de gravedad constituyeron la mayoría en esta serie, 19 para un 50 %. En 23 de los enfermos presentaron alteraciones leucocitarias. Se encontró leucocitosis en el 44,73 % de los pacientes y leucopenia en seis enfermo que representó el 15,79 %. En el 23,68 % de los enfermos se observó la presencia de trombocitopenia. Conclusiones: Las alteraciones hematológicas influyen en la buena evolución de los pacientes quemados, de no tratarse de forma oportuna pueden llevar a estos lesionados a la muerte.


Introduction: The serious burns alter of significant way the hematological and coagulation parameters. Objective: To describe the hematological alterations in the large burned patient. Methods: A descriptive, longitudinal and prospective study was carried out for the sake of describing the main hematological alterations in the large burned patient, once 2020 were deposited in the Manuel Ascunce Domenech's Universitary Hospital service of Plastic Surgery and Caumatology, of the City Camagüey, during the period understood between May, 2020 to April, 2022. They studied 38 patients in those who they had in account variables: hematological alteration, corporal burned-out surface, index forecast, state to the discharge. Results: Of the patients with 30 % or more of corporal burned-out surface (12 in total) the 91.66 % presented anemia. Anemia was found with more frequency in the patient classified as very serious condition, 11 patients that represent the 28.95 %, it is from standing out that the patients with this severity rate constituted the majority in this series, 19 for a 50 %. In 23 of the sick persons of this series they presented leukocytal alterations. Leucocytosis was found in 44.73 % of the patients, and leukopenia in six patients which represented 15.79 %. In 23.68 % of the patients the presence of thrombocytopenia was observed. Conclusions: Hematological alterations influence the good progress of burned patients, and if not treated in a timely manner can lead to death.

ABSTRACT Introduction: burns represent a pivotal component of trauma in Brazil, accounting for 2 million incidents and 2,500 deaths annually. Self-intentional burns are associated with a worse prognosis, larger burned surface area, higher infection rates, and death. The lack of studies on the issue of self-immolation raises epidemiological questions regarding Brazilian victims. This study aimed to investigate the profile of burn events associated with self-injurious behavior among Brazilian victims. Methods: this systematic review was performed according to PRISMA 2020 guidelines and evaluated the correlation between self-injurious behavior as a cause of burns in Brazilian victims and its epidemiological implications in the last 20 years (2003-2023). The MeSH terms "Burns", "Self-Injurious Behavior", "Epidemiology" and "Brazil" were queried in the PubMed/MEDLINE, SciELO, and Cochrane Library databases, and, after selection by inclusion/exclusion criteria, the most relevant studies were critically analyzed. Results: From 1,077 pre-selected studies, 92 were potentially eligible, resulting in 7 manuscripts incorporated in this review. From 3,510 burned victims assembled in the pool of selected studies, 311 cases displayed self-injurious behavior. Burned patients who attempted to burn their lives have a higher risk of death (p<0.05; RR=5.1 [3.2-8.1]) and larger burned surface area (p<0.05; MD=19.2 [10-28.2]), compared to accidental cases. Moreover, the female gender was at a higher risk of attempting self-immolation (p<0.05; RR=4.01 [2.9-5.5]). Conclusion: our results show that self-inflicted burn cases were associated with a larger burned surface area and a higher risk of death, and the female gender was identified as a relevant risk factor in Brazil.


RESUMO Introdução: Queimaduras representam um componente fundamental do trauma no Brasil, sendo responsáveis por 2 milhões de incidentes e 2.500 mortes anualmente. Queimaduras autointencionais estão associadas a pior prognóstico, maior superfície corporal queimada, maiores taxas de infecção e morte. A falta de estudos sobre a problemática da autoimolação levanta questões epidemiológicas em relação às vítimas brasileiras. O estudo se objetivou investigar o perfil das queimaduras associados ao comportamento autolesivo entre vítimas brasileiras. Métodos: Esta revisão sistemática foi realizada de acordo com as diretrizes PRISMA 2020 e avaliou a correlação entre comportamento autolesivo como causa de queimaduras em vítimas brasileiras e suas implicações epidemiológicas nos últimos 20 anos (2003-2023). Os termos MeSH "Burns", "Self-Injurious Behavior", "Epidemiology" e "Brazil" foram elencados no PubMed/MEDLINE, SciELO e Cochrane Library e, após seleção por critérios de inclusão/exclusão, os estudos mais relevantes foram analisados criticamente. Resultados: Dos 1.077 estudos pré-selecionados, 92 foram potencialmente elegíveis, resultando em 7 manuscritos incorporados nesta revisão. Das 3.510 vítimas queimadas reunidas no conjunto de estudos selecionados, 311 casos apresentaram comportamento autolesivo. Pacientes que tentaram autoimolação apresentam maior risco de morte (p<0,05; RR=5,1 [3,2-8,1]) e maior superfície corporal queimada (p<0,05; MD=19,2 [10-28,2]), em comparação com casos acidentais. Ademais, o sexo feminino apresentou maior risco para tentativa de autoimolação (p<0,05; RR=4,01 [2,9-5,5]). Conclusão: Nossos resultados mostram que os casos de queimaduras autoprovocadas foram associados a uma maior área de superfície corporal queimada e a um maior risco de morte, e o sexo feminino foi identificado como um fator de risco relevante no Brasil.

ABSTRACT Objectives: to analyze the technologies used by the nursing team in the treatment of skin lesions caused by burns in patients under intensive care. Methods: this is a scope review conducted on the LILACS, Medline, PubMed, and CINAHL databases without temporal or language restrictions. Results: the highlighted technologies included the use of specialized dressings, biological agents such as probiotics and cyanobacteria, as well as negative pressure therapies and enzymes such as papain and collagenase. Some technologies, such as nanocrystalline silver, demonstrated efficacy in infection control. Final Considerations: the study identified essential technologies in burn care, emphasizing the need for further research on "soft" technologies. The findings support the promotion of evidence-based nursing care for burn patients in intensive care and enhance knowledge about effective treatments.


RESUMEN Objetivos: analizar las tecnologías utilizadas por el equipo de enfermería en el tratamiento de lesiones cutáneas causadas por quemaduras en pacientes en cuidados intensivos. Métodos: se trata de una revisión de alcance realizada en las bases de datos LILACS, Medline, PubMed y CINAHL sin restricciones temporales ni de idioma. Resultados: las tecnologías destacadas incluyeron el uso de coberturas especializadas, agentes biológicos como probióticos y cianobacterias, además de terapias de presión negativa y enzimas como la papaína y la colagenasa. Algunas tecnologías, como la plata nanocristalina, mostraron eficacia en el control de infecciones. Consideraciones Finales: el estudio identificó las tecnologías esenciales en la atención de quemaduras, enfatizando la necesidad de investigaciones adicionales sobre tecnologías "leves". Los hallazgos respaldan la promoción de una atención de enfermería basada en evidencia para pacientes quemados en cuidados intensivos y mejoran el conocimiento sobre tratamientos efectivos.

Algumas tecnologias, como a prata nanocristalina, mostraram eficácia no controle de infecções. Considerações Finais: o estudo identificou as tecnologias essenciais na assistência às queimaduras, enfatizando a necessidade de pesquisas adicionais sobre tecnologias "leves". As descobertas sustentam a promoção de uma assistência de enfermagem baseada em evidências para pacientes queimados sob cuidados intensivos e aprimoram o conhecimento sobre tratamentos eficazes.

Objetivo: investigar as medidas de confiabilidade de instrumentos de qualidade de vida relacionada a saúde (QVRS) em crianças ou adolescentes com cicatrizes de queimaduras. Métodos: trata-se de uma revisão sistemática da literatura, mediante busca no CINAHL, Google Scholar, LILACS, PsycINFO, PubMed, Scopus e Web of Science, sem restrições quanto à data ou idioma de publicação. Foram incluídos estudos originais, nos quais se examinaram instrumentos de avaliação da QVRS de crianças e/ou adolescentes (≤ 18 anos) com cicatrizes de queimaduras. Para a análise das propriedades de medida dos instrumentos de QVRS, utilizaram-se os critérios das diretrizes do Consensus-based Standards for the Selection of Health Measurement Instruments (COSMIN). Resultados: nove estudos foram incluídos, dos quais foram abordados os seguintes instrumentos: Children Burn Outcomes Questionnaire (BOQ) ­ versão de 0 a 4 anos, Children Burn Outcomes Questionnaire (BOQ) ­ versão 5 a 18 anos, Brisbane Burn Scar Impact Profile (BBSIP) ­ versão de 0 a menores de 8 anos, Brisbane Burn Scar Impact Profile (BBSIP) ­ versão de 8 a 18 anos e CARe Burn Scale(CBS) ­ 0 a 8 anos. Conclusão: o BBSIP foi o instrumento que atendeu o maior número de itens de qualidade pela COSMIN.

Methods: systematic review of the literature performed through a search in CINAHL, Google Scholar, LILACS, PsycINFO, PubMed, Scopus and Web of Science without restrictions on the date or language of publication. Original studies were included, in which instruments for assessing the HRQOL of children and/or adolescents (≤ 18 years old) with burn scars were examined. The criteria of the Consensus-based Standards for the Selection of Health Measurement Instruments (COSMIN) guidelines were used to analyze the measurement properties of HRQOL instruments. Results: nine studies were included, and they covered the following instruments: Children Burn Outcomes Questionnaire (BOQ) 0-4 years version; Children Burn Outcomes Questionnaire (BOQ) 5-18 years version; Brisbane Burn Scar Impact Profile (BBSIP) 0-under 8 years old version; Brisbane Burn Scar Impact Profile (BBSIP) 8-18 years old version; and CARe Burn Scale (CBS) 0-8 years old. Conclusion: the BBSIP was the instrument that met the greatest number of quality items according to the COSMIN.

ABSTRACT

Purpose: Bacterial cellulose (BC) has shown high capacity for the treatment of wounds and burns, providing a moisty environment. Calcium alginate can be associated with BC to create gels that aid in wound debridement and contribute to appropriate wound healing. This study is aimed at characterizing and evaluating the use of bacterial cellulose/alginate gel in skin burns in rats. Methods: Cellulose and cellulose/alginate gels were compared regarding the capacity of liquid absorption, moisture, viscosity, and potential cytotoxicity. The 2nd degree burns were produced using an aluminum metal plate (2.0cm) at 120ºC for 20s on the back of rats. The animals were divided into non-treated, CMC(Carboxymethylcellulose), Cellulose(CMC with bacterial cellulose), and Cellulose/alginate(CMC with bacterial cellulose and alginate). The animals received topical treatment 3 times/week. Biochemical (MPO, NAG and oxidative stress), histomorphometry and immunohistochemical assays (IL-1ß IL-10 and VEGF) were conducted on the 14th, 21st, 28th, and 35th days. Results: Cellulose/Alginate gel showed higher absorption capacity and viscosity compared to Cellulose gel, with no cytotoxic effects. Cellulose/alginate presented lower MPO values, a higher percentage of IL-10, with greater and balanced oxidative stress profile. Conclusions: The use of cellulose/alginate gel reduced neutrophils and macrophage activation and showed greater anti-inflammatory response, which can contribute to healing chronic wounds and burns.
