ABSTRACT

The pathogenesis of colorectal cancer involves a variety of molecules and genes, among which circular RNA (circRNA) has received extensive attention in regulating the development and progression of tumors and mediating drug resistance. circNRA has been identified as tumor promoters or tumor suppressors that influence the sensitivity to chemotherapy drugs and mediate the onset and metastasis of colorectal cancer. The influence of circRNA on the drug resistance sensitivity of conventional chemotherapy has become a new direction for the research of anti-tumor chemotherapy drugs. This paper discusses the biological characteristics of circRNA and introduces the relationship between circRNA and the sensitivity to chemotherapy drugs in colorectal cancer.

ABSTRACT

Tuberculous meningitis is the most common and serious type of central nervous system tuberculosis, with high mortality and disability rate, which has attracted extensive attention of global public health. The high mortality rate and disability rate of tuberculosis meningitis may be related to its lack of specific clinical and imaging characteristics, insufficient attention from clinicians, lack of early sensitive and specific diagnostic testing techniques, delay in treatment, and restricted penetration of anti-TB drugs into the blood-brain barrier or/and MDR-TB, etc. This article reviews the disease burden of TBM, chemotherapy drugs and regimens, anti-inflammatory agents, aspirin, interventional and surgical treatment to provide reference for clinical management of this disease.

ABSTRACT

Objective:To investigate the antitumor effect of shikonin loaded milk derived exosomes on hepatoma cells.Methods:The experimental study was conducted. Exosomes were isolated from milk by differential centrifugation and be loaded by shikonin to constructed a nano drug loading system. The shikonin content of this nano drug loading system was determined and calculated by spectrophotometry. The cytotoxicity of shikonin loaded milk derived exosomes was evaluated by sulfonyl rhodamine B colorimetry and cell apoptosis was determined by annexin V-FITC/propidium iodide assay. Western blotting was used to detect the Bax and Bcl-2 protein expression level in hepatoma cells. Human HepG2 hepatoma cells treated with shikonin were set as the shikonin treated group and human HepG2 hepatoma cells treated with shikonin loaded milk derived exosomes were set as the shikonin loaded milk derived exosomes treated group. Observa-tion indictors: (1) the loading percentage of shikonin in shikonin loaded milk derived exosomes; (2) the cytotoxicity of shikonin loaded milk derived exosomes on human HepG2 hepatoma cells; (3) cell apoptosis of human HepG2 hepatoma cells; (4) Bax and Bcl-2 protein expression level in human HepG2 hepatoma cells. Measurement data with normal distribution were represented as Mean± SD, and comparison between groups were analyzed using the t test. Results:(1) The loading percentage of shikonin in shikonin loaded milk derived exosomes: the loading percentage of shikonin in shikonin loaded milk derived exosomes was 22.8%. (2) The cytotoxicity of shikonin loaded milk derived exosomes on human HepG2 hepatoma cells: the survival rates of hepatoma cells were 53.9%±2.9% and 45.4%±1.9% in the shikonin treated group and the shikonin loaded milk derived exosomes treated group, respectively, showing a significant difference ( t=46.27, P<0.05). (3) Cell apoptosis of human HepG2 hepatoma cells: the early apoptosis rates of hepatoma cells were 11.3%±1.5% and 14.8%±2.2% in the shikonin treated group and the shikonin loaded milk derived exosomes treated group, respectively, showing no significant difference ( t=1.37, P>0.05). The late and overall apoptosis rates of hepatoma cells were 32.3%±1.3% and 43.6%±4.3% in the shikonin treated group, versus 38.7%±3.2% and 53.5%±4.4% in the shikonin loaded milk derived exosomes treated group, showing significant differences ( t=37.39, 30.97, P<0.05). (4) Bax and Bcl-2 protein expression level in human HepG2 hepatoma cells: Bax and Bcl-2 protein expre-ssion level were 232.0±2.6 and 32.0±1.6 in the shikonin treated group, versus 286.0±3.8 and 17.0±1.5 in the shikonin loaded milk derived exosomes treated group, showing significant differences ( t=69.83, 53.32, P<0.05). Conclusion:The shikonin loaded milk derived exosomes have cytotoxic and apoptosis inducing effects on human HepG2 hepatoma cells, which can inhibit the growth of hepatoma cells.

ABSTRACT

;Aim To compare the characteristics of rat oral ulcer models caused by different chemotherapeutic drugs in order to find a stable oral ulcer model caused by chemotherapy drugs. Methods Except the control group, each group was injected with corresponding concentrations of chemotherapeutic drugs in the right buccal mucosa. During the experiment, the weight, food consumption, oral ulcer diameter were recorded; at the end of the experiment, ulcer diameter, oral mucosa histopathology, oral mucosa local cytokine level, organ index and T lymphocytes subtype in peripheral blood were observed. Results The body weight and food consumption of the animals in each group were significantly lower than those in control group after injection of chemotherapy drugs; obvious oral mucosal ulceration was observed in DDP group and PTX group, and broken and shed mucosa were observed by optical microscope in the two groups. The level of IL-lß mRNA in local mucosal tissues significantly increased, and the level of EGFR mRNA was significantly reduced in the tow groups, but organ index did not change significantly. Ulcers were not observed in MTX, VLB, and AMD groups, but organ index and cytokine significantly decreased. T lymphocyte subtypes had no difference among the emimals in each group. Conclusions Injecting cisplatin or paclitaxel in rat oral mucosa can induce a stable oral ulcer model, which can be used in therapeutic evaluation of anti-oral ulcer drugs by observing ulcer diameter, food consumption, cytokines level etc.

ABSTRACT

AIM: To study the synergistic and attenuating effects of mulberry polysaccharides on the chemotherapy of liver cancer ascites tumor-bearing mice by regulating the PI3K/AKT/mTOR pathway. METHODS: Ninety SPF male Kunming mice were randomly divided into normal group, model group, cyclophosphamide group, mulberry polysaccharide group and mulberry polysaccharide + cyclophosphamide group. Liver cancer ascites tumor-bearing mice were prepared, and each administration group was given 30 mg/kg of cyclophosphamide or (and) 200 mg/kg of mulberry polysaccharide and administered orally. The normal group and the model group were administrated with 10 mL/kg saline. The tumor suppression rate, liver, spleen and other indexes, tumor tissue VEGF and inflammatory factor content, and PI3K/AKT/mTOR pathway-related protein expression were observed in mice.RESULTS:Cyclophosphamide group, mulberry polysaccharide group and mulberry polysaccharide + cyclophosphamide group mice body weight, tumor mass, tumor tissue VEGF, TNF-α, IL-6, PI3K, AKT and mTOR phosphorylation levels were lower than the model group, and the level of IL-1β was higher than the model group; mulberry polysaccharide + cyclophosphamide group mice were observed with body weight, tumor inhibition rate, tumor tissue VEGF, TNF-α, IL-6, PI3K, AKT and The mTOR phosphorylation level higher than the mulberry polysaccharide group and cyclophosphamide group, and the tumor mass and IL-1β level were lower than the mulberry polysaccharide group and cyclophosphamide group (P<0.05). CONCLUSION: Morus alba polysaccharide combined with cyclophosphamide can effectively inhibit tumor proliferation of liver cancer ascites tumor-bearing mice and exert attenuating effect. The mechanism may be related to the down-regulation of PI3K /AKT/mTOR pathway expression.

ABSTRACT

In recent years,the overall incidence of cancer and mortality have been on an upward trend,cancer becomes a public health problem that seriously endangers human health in the world. Currently,except for radiotherapy and surgery to cancer treatment,chemotherapy plays an important role in greatly improving the survival rate of cancer patients. However,most of chemothera-pies have drug-resistance and toxic effects on patients. In order to overcome these shortcomings,some natural phytochemicals have been used as chemosensitizers in chemotherapies. These natural phytochemicals not only increase the sensitivity of tumor cells to ther-apeutic drugs,but it also reduces their resistance and toxic effects on patients. Therefore,this review summarizes the sensitization of natural phytochemicals to target drug therapies and their possible mechanisms.

ABSTRACT

Neuropathic cancer pain (NCP) is caused by nerve damage attributable to the cancer per se, and/or treatments including chemotherapy, radiotherapy, and surgery; the prevalence is reported to be as high as 40%. The etiologies of NCP include direct nerve invasion or nerve compression by the cancer, neural toxicity, chemotherapy, and radiotherapy. NCP is subdivided into plexopathy, radiculopathy, and peripheral neuropathies, among several other categories. The clinical characteristics of NCP differ from those of nociceptive pain in terms of both the hypersensitivity symptoms (burning, tingling, and an electrical sensation) and the hyposensitivity symptoms (numbness and muscle weakness). Recovery requires several months to years, even after recovery from injury. Management is complex; NCP does not usually respond to opioids, although treatments may feature both opioids and adjuvant drugs including antidepressants, anticonvulsants, and anti-arrhythmic agents, all of which improve the quality-of-life. This review addresses the pathophysiology, clinical characteristics and management of NCP, and factors rendering pain control difficult.

ABSTRACT

Objective:To explore the shortages of chemotherapy occupational protection for pharmacist in our hospital through the contrast and analysis of the occupation protection status of pharmacists in two groups.Methods:Chemotherapy Occupational Protection Status Questionnaire for Medical Staff was used,and totally 30 pharmacists from PIVAS and 30 ones from OUIVA were selected as the research objects according to the random number table.The main contents of the questionnaire included the basic information of respondents,protection conditions in drug preparation rooms,pharmacist protection status and pharmacist training conditions,etc.The results of the questionnaire were statistically analyzed.Results:In general,the understanding of pharmacists for the harm of chemotherapy drugs and protective measures was poor.PIVAS group was obviously better than OUIVA group in the use of protective equipment and relavant measures with statistically significant differences (P<0.05).Conclusion:Pharmacists have insufficient understanding for the danger of chemotherapy drugs and their harmful ways with low performance of chemotherapy occupational protection,and our hospital has no perfect protection system and adequate protective equipment yet.

Objective To develop a more convenient and stable method for assessment of drug sensitivity of prostate cancer based on real-time cell analysis system as a reference for clinical treatment.Methods Human prostate cancer VCaP, DU145, PC-3, PC-3M-2B4 and PC-3M-IE8 cells were chosen to detect the sensitivity to three drugs, docetaxel, cabazitaxel and abiraterone acetate.Serial dilutions of the three drugs were used to treat the cell culture for 24 hours.The drug-induced effects on the cell lines after an incubation of 24 hours were recorded by the real-time cell analysis system to determine the half maximal inhibitory concentration (IC50).Results Docetaxel showd IC50 of 8.81 nmol/L, 11.61 nmol/L, 1.78 nmol/L, 1.44 nmol/L, 8.69 nmol/L for VCaP, DU145, PC-3, PC-3M-2B4, PC-3M-IE8 cells, respectively.Cabazitaxel showed IC50 of 3.73 nmol/L, 3.96 nmol/L, 10.41 nmol/L, 5.43 nmol/L, and 7.37 nmol/L, respectively, for the five cell lines.Abiraterone acetate showed IC50 of 8.34 μmol/L, 8.60 μmol/L, 24.20 μmol/L, 8.59 μmol/L, and 13.21 μmol/L for the five cell lines.Conclusions PC-3M-2B4 and DU145, VCaP and PC-3 cells can be used as control for docetaxel, cabazitaxel and abiraterone acetate to establish cell models for the drug screening in vitro and to provide reference for clinical applications.

Objective To develop a scale of assessing inpatient′s risk factors of chemotherapy drugs permeability. Methods Designed a questionnaire for expert information collecting based on literature review,and employed the Delphi method to develop a set of indicators to measure inpatient′s effective store seepage risk factors. Fifteen experts finished the 2-round survey. Results Expert's enthusiasm was high, the two rounds of enquiry for effective questionnaire recovery rate was 100%. Expert's authority was very high, the average duration of clinical practice was 23.8 years, the average duration of nursing management was 13.1 years, the rate of familiarity about this kind of research was 100%, the recognition degree in the accumulation of theoretical knowledge, the clinical practice and the home or abroad research degree was 93%, 100%and 73%respectively. Two rounds of enquiry for expert advice of Cronbach's alpha coefficient was 0.96 and 0.98 respectively, experts tend to agree that the scale had good consistency, high credibility, delivery system eventually formed contains, vein, the static drop time (static) chemotherapy, more routine chemotherapy, liquid type, drug factors, social support, cooperation degree, liquid volume, mission 11 dimensions such as level of understanding, the nurse puncture store effective seepage risk factors assessment of 42 items. Conclusion The inpatient′s effective store seepage risk factor assessment scale developed by using the Delphi method is reliable for assessing effective store seepage risk factor of hospitalized patients .

ABSTRACT

Adherence refers to the extent to which a patient′s behavior coincides with medical advice without missing a dose or overdosing and taking drugs at the right time. Good adherence could assure the effect of the therapy and reduce neoplasm metastasis and recurrence as well. In this article, status of oral chemotherapy adherence in cancer patients, impact factors and measurement tools had been reviewed. Adherence of oral chemotherapy in cancer patients and measurement tools of oral chemotherapy adherence needed further study. Besides, nursing should also focus on impact factors of the adherence of cancer patients during the oral chemotherapy, and take effective interventions to improve the adherence, so as to ensure the effect of chemotherapy, and improve the survival rate of cancer patients.

ABSTRACT

Objective To study the impact of postoperative aerobic exercise on the cardiac function of breast cancer patients during anthracyclines-based chemotherapy. Methods Sixty cases of female breast cancer pa-tients, from June 2014 to December 2015 for anthracyclines-based chemotherapy, were randomly divided into ex-perimental group (n = 30) and control group (n = 30). Four cycles of conventional anthracyclines-based chemotherapy were conducted in control group, while three times of aerobic exercise per week were added in exper-imental group until the end of treatment course apart from conventional treatment. The peak oxygen consumption (VO2max) and maximum heart rate (HRmax) were measured before and after chemotherapy in both groups, ac-companied by ECG monitoring and blood collecting to measure the changes in their N-terminal pro-brain natriuretic peptide (NT-proBNP), serum creatinine (SCr) and kinase isoenzyme (CK-MB). Results No significant differ-ences in various indicators before chemotherapy were reported between two groups (P>0.05). After chemotherapy, VO2 max/kg [(21.9 ± 3.6) vs. (14.5 ± 2.8) mL/(min·kg)], VO2 max [(1 523 ± 186) vs. (911 ± 185) mL/min] and HRmax[(115 ± 15) vs. (129 ± 16) beats/min] in experimental group were significantly improved when com-pared with those in control group; significant differences in hematological levels and ECG changes were also ob-served between two groups. Conclusion Aerobic exercise during chemotherapy can mitigate the cardiotoxicity of anthracyclines to patients, which provides a new idea and therapy to reduce the incidence of clinical cardiovascular events induced by anthracyclines-based chemotherapy.

Objective:To investigate the breakthrough points and methods of pharmaceutical care performed by clinical pharma-cists for chemotherapy-induced Ⅳ degree myelosuppression. Methods: One advanced lung adenocarcinoma patient suffering from IV degree myolosuppression after being treated with pemetrexed combined with nedaplatin was selected as the example, and the chemother-apy regimen, the cause and treatment of IV degree myolosuppression and the pharmaceutical service could be carried out were ana-lyzed. Results: With the help of clinical pharmacists, the patient conquered chemotherapy-induced myelosuppression, and clinical pharmacists enhanced the awareness of pharmaceutical care and played a positive role in the safe and effective drug use. Conclusion:The participation of clinical pharmacists in clinical pharmaceutical care through providing pharmaceutical service is beneficial to safer and more effective drug therapy.

ABSTRACT

Objective To investigate the expressions of excision repair cross complementation group 1 ( ERCC1) and Ki67 in patients with breast cancer, and the relationships between their expressions and sensitivity of platinum-based chemotherapy. Methods Totally, 129 cases were pathologically diagnosed as breast cancer.Paclitaxel and carboplatin were used simultaneously. Chemotherapy regimen was as follows:Gemcitabine 1 000 mg??( m2 )-1 , IV drop on day 1 and 8;cisplatin 25 mg??( m2 )-1 , IV drop on day 1-3, for six cycles ( 21 days a cycle ) . ERCC1 and Ki67 expression in tumor tissue was observed by immunohistochemical analysis.Platinum-based chemotherapy sensitivity and survival of patients with different levels of ERCC1 and Ki67 expression were analyzed. Results In 129 patients, 18 cases were ERCC1 and Ki67 double-negative ( ERCC1-Ki67-) , and the clinical effective rate and 3-year cumulative survival rate were 88.89%and 83.33%, respectively.Twenty-four cases were ERCC1 positive but Ki67 negative ( ERCC1+Ki67-) , and the clinical effective rate and 3-year cumulative survival rate were 50. 00% and 62.50%, respectively.Thirty-three cases were ERCC1 negative but Ki67 positive (ERCC1-Ki67+), and the clinical effective rate and 3-year cumulative survival rate were 54. 55% and 60. 60%, respectively. Fifty-four patients were ERCC1 and Ki67 double-positive ( ERCC1+Ki67+) , and the clinical effective rate and 3-year cumulative survival rate were 22.78% and 31. 48%, respectively.Compared with ERCC1-Ki67- group, the clinical treatment efficiencies of cisplatin-based chemotherapy in ERCC1+Ki67- group, ERCC1-Ki67+ group, and ERCC1+Ki67+ group were significantly decreased ( P<0. 05 ) . The clinical treatment efficiency in patients of ERCC1+Ki67+ group with cisplatin-based chemotherapy was significantly decreased as compared with ERCC1+Ki67- group and ERCC1-Ki67+ group (P<0.05).Compared with ERCC1- Ki67- group, three-year cumulative survival rate in patients of ERCC1+ Ki67- group and ERCC1- Ki67+ group, ERCC1+Ki67+ group was significantly decreased ( P<0. 05 ) . Compared with ERCC1+Ki67-group and ERCC1-Ki67+group, three-year cumulative survival rate in patients of the ERCC1+Ki67+group was significantly decreased ( P<0.05) . Conclusion The expression levels of ERCC1 and Ki67 in breast cancer were high. Their expression levels are closely related with clinical efficiency of platinum-based chemotherapy.

ABSTRACT

OBJECTIVE:To investigate the safety of induction chemotherapy and the implementation method of consolidation radiotherapy in young children with high-risk Hodgkin lymphoma(HL),by evaluating the effects of intensive induction chemother-apy on consolidation radiotherapy. METHODS:Six pediatric patients with high-risk HL received low-dose involved field radiation therapy (IFRT) via volumetric modulated arc therapy (VMAT) following 6 cycles of intensive chemotherapy [Ara-C/VP16 (cyto-sine arabinoside+etoposide),ABVE-PC(adriamycin+bleomycin+leurocristine+etoposide+metacortandracin+cyclophosphamide)and CHOP(cyclophosphamide+leurocristine+adriamycin+ prednisone)in turn]. Therapeutic efficacy and toxic effects were evaluated af-ter treatment. RESULTS:Intensive induction chemotherapy and consolidation radiotherapy were acceptable by young children,and mild acute or subacute toxic reaction were observed. Intensive induction chemotherapy didn’t affect toxic effects of consolidation ra-diotherapy. In this regimen,the cumulative doses of bleomycin and adriamycin were 20 mg/m2 and 270 mg/m2,respectively. VMAT optimized plan to ensure that the dose that involved organs received was safe. In the patient with mediastional radiation therapy,the mean lung and heart doses were 525.6 cGy and 503.9 cGy,respectively. CONCLUSIONS:It is safe to give high-risk HL young children 6 cycles of intensive induction chemotherapy(Ara-C/VP16,ABVE-PC and CHOP in turn)before radiotherapy. It is feasi-ble to conduct IFRT with 18-20 Gy and an additional 20-25 Gy boost,1.5-1.8 Gy/fraction. VMAT deserves to be advised.

More than 95% of the thyroid carcinomas are well differentiated types showing favorable prognosis. However, only a few therapeutic options are available to treat the patients with undifferentiated thyroid carcinomas, especially with refractory thyroid carcinomas that are not amenable to surgery or radioiodine ablation. We investigated the anticancer effects of 20 chemotherapy and hormonal therapy drugs on 8 thyroid carcinoma cell lines. In vitro chemosensitivity was tested using the adenosine-triphosphate-based chemotherapy response assay (ATP-CRA). The tumor inhibition rate (TIR; or cell death rate) or half maximal inhibitory concentration (IC50) was analyzed to interpret the results. Of the 12 chemotherapy drugs, etoposide (178.9 index value in follicular carcinoma cell line) and vincristine (211.7 in Hurthle cell carcinoma cell line) were the most active drugs showing the highest chemosensitivity, and of the 8 additional drugs, trichostatin A (0.03 microg/mL IC50 in follicular carcinoma cell line) showed favorable outcome having the anticancer effect. In our study, the result of etoposide and vincristine show evidence as active anticancer drugs in thyroid carcinoma cell lines and trichostatin A seems be the next promising drug. These drugs may become an innovative therapy for refractory thyroid carcinomas in near future.

ABSTRACT

Objective To observe the relationship between degree of venous injury and the indwelling time by injection of chemotherapy drugs with intravenous catheter system. Methods Totally 43 New Zealand rabbits were selected as the study sample. Among them, three rabbits were chosen by lot as a blank control, the others were evenly divided into two groups randomly. The veins at the edges of the two ears were taken out as the tested blood vessels for the infusion with intravenous catheter system. The two groups were injected respectively with physiological saline and chemotherapy drugs once daily, and then blocked with heparin salt water. After intraperitoneal anaesthesia, pathological sections were prepared by taking live samples from the ear vein where intravenous catheter system puncturing on the 2nd, 4th, 6th,8th and 10th day of transfusion. And inflammation and thrombosis in the vein had been observed under the light microscope. Results In the same indwelling time, the inflammatory response of the two groups was compared: no difference on the 2nd day; difference was seen on the 4th、6th and 8th day; but again no difference on the 10th day. Thrombosis compared: no difference on the 2nd, 4th day, but difference was seen on the 6th, 8th and 10th day. Conclusions Chemotherapy drugs has a strong irritant on blood vessels.The indwelling time should not be too long, generally advisable 2 days,even if no abnormal response is observed locally by the naked eye.
