Abstract Background: There are few studies dedicated to the characterization of the geriatric population with psoriasis, which has particularities in terms of clinical manifestations and therapeutic limitations. As psoriasis is a chronic disease, presenting a higher prevalence with age, the increase in life expectancy in Brazil demands knowledge about the behavior of the disease among the elderly. Objectives: To characterize elderly people with psoriasis from a tertiary service, from the clinical-epidemiological point of view, presence of comorbidities, physical frailty, and affective impact, and to compare these aspects with adults with psoriasis and elderly people without the disease. Methods: Cross-sectional study of 64 elderly patients with psoriasis, 64 adults with psoriasis, and 64 elderly patients without the disease. Clinical-demographic aspects, the Beck depression scale, and Skindex-16 were evaluated. Indicators of physical frailty were evaluated in elderly patients: handgrip, sit-to-stand test, fatigue, and weight loss >5%. Results: In the elderly, the mean age (SD) of psoriasis onset was 44 (10) years, men represented 47% of the sample, the prevalence of arthritis was 22%, and ungual involvement occurred in 72%. Topical corticosteroids were used more often among elderly people with psoriasis (100%) than among adults with the disease (86%), with no difference among other systemic treatments. Diabetes mellitus occurred in 30% of the elderly. Hypertension (59%), dyslipidemia (52%), depression (34%), and fatigue (59%) were more prevalent among the elderly with psoriasis than among the healthy controls.
ABSTRACT
Objective To investigate the medication and blood glucose control of type 2 diabetes patients under chronic diseases management in Xi'an chain pharmacies, and provides reference for improving the management policy of diabetes at grassroots level. Methods A number of chain pharmacies in the sixth district of Xi'an were selected by random sampling method, and on-site interviews were conducted by questionnaire survey to patients with type 2 diabetes under the management of chronic diseases. The basic information of patients, medication status(medication plan, drug adherence,etc.),diabetes-related conditions(blood glucose status, family history, course and complications, etc.)were collected.Multivariate logistic regression was used to analyze the relevant factors of blood glucose control in patients. Results A total of 403 patients were surveyed, the largest number of patients use oral hypoglycemic drugs alone(53.4%),followed by insulin medication(including insulin only and insulin in combination with oral hypoglycemic drugs)(35.7%),and the differences between disease course and glycemic control among patients with different drug regimens were statistically significant(P<0.05).Only 43.7%of patients had good medication compliance. In addition, the patient's fast plasma glucose compliance rate was only 39.2%.The results of multivariate logistic regression analysis showed that good medication compliance(OR=1.744,95%CI 1.104 to 2.754,P=0.017)were independent influencing factors for achieving glycemic control. Conclusion The medication compliance of type 2 diabetes patients with poor blood glucose control in chronic disease management of chain pharmacies in Xi'an needs to be strengthened.Pharmacies should emphasize and give full play to the professional and service advantages of pharmacists to realize the functional role of pharmacies and strengthen diabetes management.

Objective To understand cognition level and treatment intention of patients with endometriosis(EMs);To evaluate the effects of TCM chronic disease management on EMs patients. Methods The disease cognition level and treatment intention of 1 895 EMs patients from October 2020 to December 2021 in 19 provinces, autonomous regions and municipalities were analyzed through cross-sectional investigation. The cognitive effect of chronic disease management of TCM on 801 patients with EMs from 6 Grade A hospitals in Beijing was evaluated by self-control study. Results The cognition level of EMs patients for the disease was improved with the increase of educational background.Patients generally believed that EMs was a chronic disease, requiring regular review and long-term management. The proportion of patients with high school education or below, junior college education, bachelor degree or above who knew the possibility of malignant changes in EMs was less, only 32.3%,41.6%and 47.7%,respectively.The awareness rate of patients with high school education or below for recurrence after EMs conservative surgery was 46.9%,lower than that of those with junior college education(66.8%)and bachelor degree or above(72.7%).Among the patients with dietary contraindications, the proportion of patients with high school education or below, junior college education, bachelor degree or above who knew the contraindication of the forest frog oil was less, only 20.7%,30.3%and 32.9%,respectively.In the aspect of life adjustment, the awareness of patients was generally high.EMs patients mainly recognized the disease through face-to-face communication with gynecologists.Only 34.4%of patients with high school education or below learned about EMs through WeChat public accounts,APPs and other new media, which was lower than those with junior college education(48.6%)and bachelor degree or above(55.4%).EMs patients generally tend to be treated in comprehensive hospitals, with high acceptance of TCM treatment, mainly TCM decoction, and low acceptance of TCM appropriate technology. After 1 year of TCM chronic disease management, the disease awareness of EMs patients was significantly improved compared with before management, with statistical significance(P<0.05).Conclusion EMs patients with different educational backgrounds have different cognition of the disease, and each has different emphasis.Education and popularization should be carried out according to their knowledge blind spots.Chronic disease management of TCM can improve the disease cognition level of EMs patients.
ABSTRACT
Objective:To investigate the level of health literacy and its influencing factors among residents aged 15-69 years with different chronic disease in Zhejiang Province. Methods:In this cross-sectional study, a stratified multistage whole cluster random sampling method was used to select 19 200 permanent residents aged 15-69 years from 30 monitoring sites in Zhejiang Province from June to November 2022, and a household questionnaire survey was carried out with the National Health Literacy Monitoring Questionnaire for the Population. The content of the survey included socio-demographic characteristics, health-related factors and health literacy level. A total of 19 200 questionnaires were distributed and 19 051 were collected, of which 18 857 (98.98%) were valid. The χ2 test was used to compare the health literacy levels of residents with different chronic diseases, and logistic regression was used to analyze the influencing factors of the health literacy levels. Results:The health literacy level of residents aged 15-69 in Zhejiang Province in 2022 was 38.36%. Among them, the health literacy level of patients with multiple chronic diseases was much lower than those with single chronic disease and those without chronic disease (14.70% vs 25.37%, 42.21%) ( χ2=483.05, P<0.001). It was positively associated between having multiple chronic diseases and insufficient health literacy ( OR=1.26, 95% CI: 1.01-1.58); compared with those without chronic disease ( OR=1.00), patients with multiple chronic diseases were significantly more likely to have inadequate health literacy in basic knowledge and concepts ( OR=1.29, 95% CI: 1.03-1.63), health behaviors ( OR=1.24, 95% CI: 1.01-1.53), basic skills ( OR=1.37, 95% CI: 1.11-1.69), and health information literacy ( OR=1.27, 95% CI: 1.08-1.50). Conclusions:The number of chronic diseases is a key factor that affects the residents′ health literacy level, and people with multiple chronic diseases are often associated with a higher risk of inadequate health literacy.

ABSTRACT
Objective To investigate the investigation of co-morbidity etiology and prognosis analysis of chronic diseases in the elderly population. Methods The data of 1 475 elderly patients who were seen and treated in Chengdu Fifth People's Hospital from January 2019 to December 2021 were screened to analyze their disease status, co-morbidity combinations and patterns, co-morbidity influencing factors, and prognosis. Results The top four prevalence rates among 1 475 elderly patients with chronic diseases were hypertension 555 (37.63%), gastric or gastrointestinal diseases 445 (30.17%), arthritis or rheumatism 427 (28.95%), and diabetes 329 (26.58%). 1034 co-morbidities were found in 1475 elderly patients with chronic diseases, with a co-morbidity rate of 70.10%. The binary disease combination accounted for 58.41% and the ternary disease combination accounted for 41.59%. Female, age >70 years, family history of chronic diseases, overweight/obesity, daily physical inactivity, history of alcohol/smoking, poor sleep quality, and poor dietary habits were the independent influencing factors for co-morbidity in elderly patients with chronic diseases (ORfemale=2.413, ORage ≥ 70=1.670, ORhistory of alcohol consumptionfamily history of chronic diseases=2.846, ORhistory of alcohol consumptionoverweight/obesity=2.570, ORdaily inactivity=1.802, ORhistory of alcohol consumption=3.543, ORhistory of smoking=1.784, ORpoor sleep quality=2.128, ORunhealthy dietary habits=2.085, all P2primary exacerbation/emergency readmission=10.726, χ2new chronic disease=5.873 , all P<0.05). Conclusion Gender, age, chronic disease history, BMI, and lifestyle habits are important factors influencing co-morbidity in elderly patients with chronic diseases, and patients with co-morbidity have a relatively poor prognosis.
ABSTRACT
ObjectiveTo describe different non-alcoholic fatty liver disease (NAFLD) outcomes among community inhabitants, and further to explore the correlation between bio-indicator level variance and the outcomes. MethodsPhysical indicators (height, weight, waist circumstances, hip circumstances, blood pressure, etc), biochemical indicators [fasting plasma glucose, HbA1c, serum triglycerides(TG), serum total cholesterol(TC), low-density lipoprotein cholesterol(LDL-C), high-density lipoprotein cholesterol(HDL-C), liver related transaminase, etc] and clinical imaging (B-scan ultrasonography) were collected during the follow-up from the Songjiang Natural Population Sub-cohort. The identification of NAFLD was supported by the definition criteria from Guidelines for the diagnosis and treatment of non⁃alcoholic fatty liver disease. Paired t-test and multifactorial logistic regression model were used to compare the difference between the indicator level of the subjects from different outcome subgroups and to further analyze the correlation between these indicator variance and different NAFLD outcomes. ResultsDuring a median follow-up time of 2.94 years, 12 076 subjects were involved. The cumulative NAFLD incidence and remission rate were 21.57% and 31.15%, respectively. The proportion of subjects who still had NAFLD was 27.96%. Among subjects with newly-developed NAFLD, indicators including blood pressure, BMI, fasting plasma glucose, and plasma lipid level increased, while in the remission subgroup, blood pressure, BMI(WHR), waist-hip ratio(WHR), and TG level were significantly decreased. Increased level of systolic pressure, WHR, BMI, HbA1c, and LDL-C might be the risk factors to the occurrence of NAFLD. While decreased level of WHR, BMI, TC and LDL-C level and elevated HDL-C level were likely to be the influencing factors of NAFLD remission process. ConclusionThe NAFLD morbidity in the community inhabitants is relatively high. BMI, WHR, fasting plasma sugar and plama lipid level variance may act as the influencing factors towards different NAFLD outcomes.
ABSTRACT
ObjectiveTo understand the mortality trends among residents of Fuling District, Chongqing, before and after theCOVID-19 outbreak, and to provide references for the government to formulate disease prevention and control policies and measures. MethodsData on mortality and population in Fuling District from 2017 to 2022 were collected to analyze population mortality and standardized mortality rates, and to compare the changes in the causes of death by year and before and after the pandemic. ResultsFrom 2017 to 2022, the crude mortality rate in Fuling District showed an upward trend (APC=3.04%, P<0.05), while the standardized mortality rate showed a downward trend (APC=-6.47%, P<0.01). The mortality rate of males was higher than that of females (P<0.05), with different age groups having different causes of death composition. The highest proportion of deaths in 0-year-old group was from infectious diseases, maternal and neonatal diseases, and nutritional deficiencies, the highest proportion of deaths in the 1‒24 age group, with the exception of those aged 5‒9, was from injuries, and the main cause of death for residents aged 25 and above was chronic diseases. The mortality rate of mental and behavioral disorders rose from the 13th to the 9th place. According to the epidemic situation of COVID-19, there were no changes in the top five causes of death among the entire population. The motility rate of endocrine, nutritional and metabolic diseases rose from the sixth to the fifth place in male population, and the motility rate of malignant tumor rose from the 3rd to the 2nd place in female population. ConclusionThere are no changes in the top five causes of death among the entire population of Fuling District before and after the COVID-19 outbreak. Chronic diseases remain the main cause of death. It is necessary to control the risk factors for cardiovascular and cerebrovascular diseases such as hypertension, diabetes, and dyslipidemia, and to curb the rising trend of mortality rates from strokes and acute myocardial infarction. For deaths caused by accidental injuries, targeted health education should be conducted for different populations.

Objective To explore the relationship between health literacy of chronic diseases and the effect of blood glucose control in newly diagnosed and prediabetes patients. Methods A total of 180 newly diagnosed and prediabetes patients treated in the Western Theater Command General Hospital from January 2021 to January 2023 were selected, including 94 newly diagnosed and 86 prediabetes patients. The health literacy of chronic diseases in these patients was evaluated by using the health literacy management scale (HeLMS) developed by Jordan et al. The differences of clinical general information and blood glucose control between patients with adequate and insufficient health literacy were analyzed. Results Among the 180 patients, the total score of information acquisition ability, communication and interaction ability, willingness to improve health, willingness for financial support, total score of health literacy, and adequate proportion of health literacy assessed by HeLMS were (35.54 ± 7.21), (33.02 ± 8.15), (15.54 ± 3.92), (7.10 ± 0.98), (91.87 ± 7.28), and 51.67%, respectively. The age of patients with adequate health literacy in the newly diagnosed diabetes group was (52.23 ± 7.15) years old, which was significantly lower than that of patients with insufficient health literacy (P<0.05). The proportions of patients with high school education and above, monthly income ≥ 3000 yuan, and relatives engaged in medical work were 68.09%, 68.09%, and 34.04%, respectively, which were significantly higher than those of patients with insufficient health literacy (P<0.05). The age of patients with adequate health literacy in prediabetes group was (82.23 ± 4.15) years old, which was significantly lower than that of patients with insufficient health literacy (P<0.05). The proportions of patients with high school education and above, monthly income ≥ 3000 yuan, number of children ≥ 2, and relatives engaged in medical work were 65.22%, 67.39%, 34.78%, and 41.30%, respectively, which were significantly higher than those of patients with insufficient health literacy (P<0.05). The fasting blood glucose, glycosylated hemoglobin and 2h postprandial blood glucose of patients with adequate health literacy in the newly diagnosed diabetes group after treatment were (6.43 ± 0.93) mmol/L, (6.02 ± 0.91)% and (7.71 ± 1.01) mmol/L, respectively, which were significantly lower than those of patients with insufficient health literacy (P<0.05). The fasting blood glucose, glycosylated hemoglobin and 2h postprandial blood glucose in patients with adequate health literacy in prediabetes group after treatment were (5.21 ± 0.37) mmol/L, (5.20 ± 0.40)% and (6.20 ± 0.92) mmol/L, respectively, which were significantly lower than those in patients with insufficient health literacy (P<0.05). Conclusion The chronic disease health literacy of prediabetes and new-onset patients is related to the age, education and monthly income level, number of children, relatives engaged in medical work of patients, and the level of health literacy is helpful to the control of blood glucose.
ABSTRACT
Objective@#To investigate the prevalence of major chronic diseases among residents in Xiamen City, Fujian Province, so as to provide insights into enhancing the prevention and control of chronic diseases.@*Methods@#The residents aged 18 years and older and lived for 6 months and more were selected from 6 districts in Xiamen City from 2021 to 2022 using the stratified random sampling method. Demographic information, disease history, blood pressure, fasting blood glucose and blood cholesterol were collected through questionnaire surveys, physical examination and laboratory tests. The prevalence rates of hypertension, diabetes and hypercholesterolemia were calculated and standardized using the 2020 National Population Census data, and were analyzed by age and gender.@*Results@#Totally 22 289 residents were investigated, including 10 444 males (46.86%) and 11 845 females (53.14%). The prevalence rates of hypertension, diabetes and hypercholesterolemia were 15.35%, 4.30% and 10.45%, and the standardized prevalence rates were 16.25%, 4.81% and 10.21%, respectively. The standardized prevalence rates of hypertension, diabetes and hypercholesterolemia in males were 19.20%, 5.71% and 9.97%, respectively, and were higher than those in females, which were 13.62%, 4.03% and 10.29%, respectively (all P<0.05). The crude prevalence rates of hypertension and diabetes in males aged 18 to 59 years were higher than those in females, the crude prevalence rate of hypercholesterolemia in males aged 18 to 49 years was higher than that in females, and the crude prevalence rate of hypercholesterolemia aged 50 years and over in males was lower than that in females (all P<0.05).@*Conclusion@#From 2017 to 2022, the prevalence rates of hypertension, diabetes, and hypercholesterolemia in males were higher than those in females in Xiamen City, while the prevalence rate of hypercholesterolemia in females was higher than that in males after the age of 50 years.
ABSTRACT
To promote the stockpiling of regular medicines for disasters (SMD), we investigated SMD rates and clarified the relationship between SMD status (Yes or No) and the characteristics of patients with chronic diseases. A survey was provided to patients visiting the pharmacies in Hokkaido. SMD was defined as a patient having a supply of regular medicine for 7 days or more and replacing with new medicine within one year. Of a total of 537 participants (51.0% male; mean age 65.8 years), 61.1% had experienced a major disaster. The SMD rate was extremely low at 15.3%. The median score for a patient’s understanding of the regular medicine names (5-point scale: 1=hardly understood, 5=almost understood) was 2. The median number of monthly pharmacy visits was 0.8. 5.4% were recommended SMD by physicians or pharmacists. Results from multiple logistic regression analysis indicated that positive patient characteristics included age (OR=1.154, 95%CI [1.026-1.298], P=0.017), understanding of the regular medicine names (1.724, [1.039-2.859], P=0.035), and recommendation of SMD by physicians or pharmacists (5.991, [2.616-13.722], P<0.001). A negative patient characteristic was the number of pharmacy visits (0.587, [0.383-0.899], P=0.014). The most influential positive factor was the recommendation of SMD by physicians or pharmacists; however, only 5.4% of the participants had experienced this. The findings of this study indicated important that health care providers and the government to work together to devise easy-to-understand measures to inform local residents about the importance of SMD and how to them, conduct educational activities.
ABSTRACT
Chronic diseases occur repeatedly and progress slowly, and patients with chronic diseases are often accompanied by pressure such as changes in life rhythm, decreased activity ability and complications during treatment.Positive psychosocial factors play an important role in the development of chronic diseases, and the personal mastery, as an important protective psychological resource for individual self-management and stress coping, is of great significance for the improvement of patients ′ prognosis and quality of life. This article mainly reviews the influencing factors and intervention measures of personal control in patients with chronic diseases, aiming at providing reference for clinical nursing staff to develop and implement positive psychological intervention strategies for patients with chronic diseases.
