ABSTRACT

[Objective]To summarize the clinical experience of Professor WANG Zhen in the treatment of post-Corona Virus Disease 2019(COVID-19)cough.[Methods]Through the way of recording and sorting out medical cases,referring to domestic and foreign literature,this paper expounded the clinical experience of Professor WANG Zhen from three aspects:pathogenesis,prescription and medical cases.[Results]Professor WANG Zhen believes that the cause of post-COVID-19 cough is epidemic Qi,and the cause of the disease is wind and dampness-heat.The prescription experience is to dispel wind and relieve external symptoms,open the orifices and soothe the throat,clear heat and resolve dampness in tri-Jiao,and take into account both individual variability and symptoms and causes.Two medical cases are reported,the patient complained of post-COVID-19 cough,and Professor WANG Zhen mainly dispelled wind,cleared heat and resolved dampness,supplemented by nourishing Yin,invigorating the spleen,tonifying the kidney and dispersing blood stasis,with a comprehensive prescription and accurate curative effect.[Conclusion]Professor WANG Zhen accurately grasps the pathogenesis of post-COVID-19 cough,the treatment method is clear,the use of traditional Chinese medicine is rational,the therapeutic effect is obvious,and his clinical experience is valuable.

ABSTRACT

Objective:To evaluate the efficacy of Qufeng Zhiyang Decoction combined with Fuzhiqing Ointment in the treatment of chronic perianal eczema.Methods:Randomized controlled trial. A total of 122 patients with chronic perianal eczema in Meishan People's Hospital from January 2020 to May 2021 were selected as the observation objects and divided into 2 groups according to random number table method, with 61 cases in each group. The conventional group was treated with Fuzhiqing Ointment. The study group was treated with Qufeng Zhiyang Decoction. The treatment for both groups lasted for two weeks. TCM symptom score was recored before and after treatment; the degree of skin lesions was evaluated by the area and Eczema Area and Severity Index (EASI); the quality of life of patients was evaluated by the quality of life index of dermatosis (DLQI); the number of Treg and Th17 cells was measured by flow cytometry; the ratio of Th17/Treg cells was calculated; the levels of TNF-α, IL-10 and IL-6 were detected by ELISA. The adverse reactions were recorded and the clinical efficacy was evaluated. The patients were followed up for 3 months and the recurrence was recorded.Results:The total effective rate of the study group was 86.89% (53/61) and that of the conventional group was 72.13% (44/61), with statistical significance ( χ2=4.08, P=0.044). After treatment, the scores of pruritus, spurt water, erythema and burning, upset and thirsty, yellow urine and dry urine in the study group were lower than those in the conventional group ( t values were 19.64, 49.13, 19.64, 33.14, 24.61, P<0.01); EASI and DLQI scores were lower than those in the conventional group ( t values were -6.52 and -29.33, P<0.01). After treatment, Treg cells [(6.58±0.78) % vs. (5.49±0.81) %, t=7.57] in the study group was higher than that of the conventional group ( P<0.01), Th17 cells [(1.03±0.28) % vs. (1.27±0.39) %, t=-3.90] and Th17/Treg ratio [(0.15±0.07) vs. (0.23±0.05), t=7.68] were lower than those in the conventional group ( P<0.01). After treatment, the level of IL-10 in the study group was higher than that of the conventional group ( t=-16.97, P<0.01), and the levels of IL-6 and TNF-α were lower than those in the conventional group ( t values were -21.48 and -25.59, P<0.01). During treatment, the incidence of adverse reactions was 6.56% (4/61) in the study group and 9.84% (6/61) in the conventional group, without statistical significance in the incidence of adverse reactions between the two groups ( χ2=0.44, P=0.509). During the follow-up period, the recurrence rate was 15.09% (8/53) in the study group and 50.00% (22/44) in the conventional group, with statistical significance in the recurrence rate between the two groups ( χ2=14.55, P<0.01). Conclusion:Qufeng Zhiyang Decoction combined with Fuzhiqing Ointment can reduce the levels of inflammatory factors in patients with chronic perianal eczema, maintain Th17/Treg cell immune balance, improve clinical efficacy, and have a low recurrence rate.

ABSTRACT

The pathological mechanism of allergic rhinitis (AR) involves hierarchically acute and chronic reactions among antigens, immunocytes, immunoglobulins and inflammatory factors. AR, which is characterized by heterogeneity, reacts differently to the current treatment on diverse individuals. Some patients receive unsatisfying clinical outcomes after the treatment. Chinese herbal medicine has become a frequent option to treat AR because it can improve clinical symptoms with excellent safety. The description of Biqiu in ancient Chinese medical literatures is very similar to the clinical manifestation of AR. The major pathogeneses of Biqiu included infection of wind-cold pathogens, deficiency and cold in the lung, and evil depression into heat and wind. Therefore, in modern traditional Chinese medicine (TCM), Biqiu is considered as an integration of AR, allergic sinusitis and vasomotor rhinitis. According to the pathogenesis and clinical manifestations of Biqiu (including AR), this disease belongs to wind disease in TCM. But modern TCM focuses on deficiency of internal organs and ignores wind evil in syndrome differentiation, contrary to the fact that wind medicine is always used in treating AR. This even leads to an abnormal phenomenon that clinical studies attach little importance to syndrome differentiation. In this paper, we started from the phenomenon that wind medicine is always used in TCM for AR, considering TCM literatures and the study results of modern therapy for wind diseases, and put forward the ideas as follows: AR belongs to exogenous wind disease in TCM, and wind pathogen is the major pathogen, in clinical syndrome differentiation, we should attach more importance to wind evil, and dispelling wind and eliminating dampness is the key to treatment. Meanwhile, based on the fact that most of the classical prescriptions with good effectiveness for AR originated from Treatise on Febrile Disease, mainly including the wind medicines for treating exogenous diseases, it can be inferred that the wind-dispelling and itch-arresting effects of wind medicines are helpful to alleviate or block the allergic reaction of nasal mucus and achieve the goal of relieving itching and sneezing, and the wind-dispelling and dampness-eliminating effects are helpful to alleviate the edema of nasal mucus and achieve the goal of relieving running nose and nasal obstruction. In modern researches, it is also found that classical prescriptions which take wind medicine as a principle component and wind medicine used in AR treatment have good anti-inflammatory, anti-allergic and immune function adjusting effects. This indicates that dispelling wind and eliminating dampness and the wind medicine treatment under its instruction not only correspond to TCM theory, but also have modern science evidences.

ABSTRACT

Doctors in the past dynasties always used ephedra to treat disease of exogenous wind-cold,cough,asthma and edema,but ignored its application in treating arthromyodynia.It could be deduced from the record of the books of Chinese medicine and prescriptions that ephedra had the nature of pungent and warm,could dispel wind and remove wetness,warm channel and expel cold,remove obstruction in collaterals.All of those were correspondent with the pathogenesis of arthromyodynia.Thus,ephedra can be widely used in types of arthromyodynia.Proven case was also included in this article.

ABSTRACT

[Objective] To observe the therapeutic effect of wind-dispersing and cold-dispelling, blood-activating and stasis-removing therapy for cold-type urticaria and to explore its possible mechamism. [Methods] Sixty-six cases were allocated to two groups by a simple random sampling method. Group A ( n = 36) was treated with wind-dispersing and cold-dispelling, blood-activating and stasis-removing herbal medicine including Herba Ephedrae, Ramulus Cinnamomi, Herba Schizonepetae, Radix Saposhnikoviae, Semen Armeniacae Amarum, Rhizoma Zingiberis Recens, Fructus Jujubae, Radix Salviae Miltiorrhizae, Radix Angelicae Sinensis, Caulis Spatholobi, Radix Paeoniae Rubra, etc. Group B (n =30) was treated with cetirizine hydrochloride tablets. The two groups were treated for two 20-day courses. One week after treatment, the therapeutic effect and the changes of hemorheological parameters before and after treatment were observed and compared with those in 38 healthy volunteers. [Results] The total effective rate was 80.6% in group A and 63.3% in group B, the difference being significant ( P 0.05) . [ Conclusion] Wind-dispersing and cold-dispelling, blood-activating and stasis-removing therapy is an important method for cold-type urticaria and the selected herbal medicines are effective. Its therapeutic mechanism may be related to the improvement of hemorheological parameters.

[Objective] To explore the effect of wind-expelling and blood-cooling therapy in treating chronic idiopathic thrombocytopenia purpura (ITP) . [Methods] Thirteen cases of chronic ITP differentiated in TCM as heat-induced blood extravasation (group A) were treated with wind-expelling and blood-cooling therapy. The changes of peripheral T-lymphocyte subsets and natural killer (NK) cells before and after treatment were observed and compared with the normal volunteers (group B). [Objective] To investigate the synergistic action of fuming-washing therapy with Qingbixifang ( a herbal lotion mainly composed of Herba Siegesbeckiae, Cortex Phellodendri, Radix Paeoniae Rubra, Radix Rehmanniae, Radix Stephaniae Tetrandrae, Rhizoma Atractylodis, Herba Ecliptae, Nidus Vespae, Olibanum, Myrrha, etc.) on rheumatoid arthritis (RA) with heat retention. [Methods] Sixty patients of RA with heat retention were randomized into groups A and B. Group A ( n = 30) was treated by fuming and washing the affected part with Qingbixifang, and taking Tongbiling Tablets and nimesulide; group B (n = 30) was treated by taking Tongbiling Tablets and nimesulide only. The treatment course of the two groups lasted one month. The time of morning stiffness, grip strength, the number of joints with tenderness, tenderness index, resting pain in joints, the number of swelling joints, swelling index, evaluation by patients themselves and by the doctor were compared before and after treatment. Laboratory parameters such as blood rheumatoid factor (RF), C-reactive protein (CRP), erythrocyte sedimentation rate (ESR), white blood cell (WBC) count and platelet (PLT) count were also observed before and after treatment. ABSTRACT

[Objective] To observe the therapeutic effect of wind - dispelling, blood - activating and stasis - removing Chinese herbs combined with autoepidermic grafting for vitiligo. [Methods] Forty cases of vitiligo were randomized into two groups: 28 cases in Group A were treated with Chinese herbs combined with autoepidermic grafting (235 grafts in all) and 12 in Group B with autoepidermic grafting (118 grafts in all) alone. [Results] In Group A, 1 case was cured, 13 were markedly effective, 10 effective and 4 ineffective and 0, 3, 3, and 6 in Group B respectively. The therapeutic effect in Group A was superior to that in Group B (P = 0.034). [Conclusion] Chinese herbs combined with autoepidermic grafting has a better effect than autoepidermic grafting alone for vitiligo.

Objective To observe the therapeutic effect of Chinese medicine combined with ion-introduction of Radix Astragali Injection (RAI)for herpes simplex keratitis (HSK).Methods Ninety HSK patients (104 affected eyes)were randomized into two groups.Both of the two groups received anti-virus drugs,and the treatment group (45 patients with 53 affected eyes)received oral use of decoction of Chinese medicines (mainly composed of Semen Cassiae,Fructus Viticis,Periostracum Cidadae,Ramulus Uncariae cum Uncis,Fructus Gardeniae,Fructus Forsythiae,Flos Chrysanthemi,Rhizoma Chuanxiong,etc.),and ion-introduction of RAI.The changes of vision and clinical symptoms before and after treatment were observed,and a 2-year follow-up was carried out.Results In the treatment group,19 eyes were cured,27 effective,7 ineffective,and the total therapeutic effective rate being 86.79%;in the control group,10 eyes were cured,21 effective,20 ineffective,and the total therapeutic effective rate being 60.78%.The therapeutic effect in the treatment group was superior to that in the control group (P

[Objective] To observe the effects of modified Zhengrong Decoction combined with acupuncture for acquired paralytic strabismus (APS). [ Methods ] Forty - one cases of APS were treated with modified Zhengrong Decoction combined with acupuncture of Jingming (BL1) point mainly. [Results] Four (9.8%) cases were cured after one treatment course (10 days), 23 (56.1%) cured in 2 ~ 3 treatment courses and after 4 ~ 6 treatment courses 12 (29.3%) cured, 1 (2.4%) ineffective, and 1 (2.4%) relapsed, the total effective rate being 95.2% . [Conclusion] Modified Zhengrong Decoction combined with acupuncture is effective and safe for APS.

Objective To observe the influence of wind-dispelling therapy on therapeutic effect of blood-cooling and kidney-tonifying herbs for chronic idiopathic thrombocytopenic purpura(CITP),and to investigate its immunoregulatory effect.Methods Forty-two CITP patients were randomized into 2 groups:the treatment group(N=22) received therapy of dispelling wind,cooling blood and tonifying kidney,and the control group(N=20) received therapy of cooling blood and tonifying kidney.Six weeks constituted one treatment course,and the treatment lasted 3 courses.Serum interleukin 2(IL-2),soluble IL-2 receptor(sIL-2R),Fas protein,Fas-L protein,and platelet-associated immunoglobulin G(PAIgG) levels and lymphocyte subtypes were compared before and after treatment.Results In the treatment group,14 patients were clinically cured,5 markedly effective,2 effective,1 ineffective and the total effective rate was 95.5%;in the control group,8 patients were clinically cured,4 markedly effective,3 effective,5 ineffective and the total effective rate was 75.0%.The therapeutic effect in the treatment group was superior to that in the control group(P
