ABSTRACT

The disposable sterile urethral catheter was described in terms of the current status of the standards of foreign countries and China and its regulation and registration.The national supervision and sampling inspection and exploratory research of the disposable sterile urethral catheter in 2018,2019 and 2021 were introduced,and the problems found and the causes were analyzed and then the countermeasures were proposed accordingly.References were provided for guiding and standardizing the development of catheter products industry.[Chinese Medical Equipment Journal,2024,45(1):84-88]

@#Objective To investigate the surgical effect and incidence of complications of traditional circumcision and disposable circumcision stapler in the treatment of phimosis and overlength of prepuce.Methods A retrospective analysis of 261 children with excessive phimosis and prepuce treated in the Department of Pediatric Surgery of our hospital from January 2021 to January 2023.The control group included 129 cases,who were treated by traditional methods,and the others 132 cases as test group,were treated by disposable circumferential stapler.We observed correlation indicators of intraoperative and postoperative between the two groups.Operation time,intraoperative blood loss were intraoperative indicators,and,postoperative pain degree,postoperative incision healing time,severe edema of incision margin,incision dehiscence,postoperative rebleeding,postoperative infection,and appearance satisfaction were observed in the postoperation period.Results The operation time,intraoperative blood loss,and postoperative rebleeding of the disposable circumferential stapler group were significantly less than those of the control group,while the postoperative incision healing time and the incidence of severe edema of the incision in the control group had obvious advantages compared with the test group.There was no significant difference in pain degree,postoperative infection rate,incision dehiscence and appearance satisfaction.Conclusions The benefits of traditional circumcision are that postoperative recovery is relatively fast and the degree of edema at the incision margin is mild.And complete hemostasis,surgical operation easily,safety,and suitable for subordinate medical institutions are advantages of disposable circumcision stapler compare with traditional model,meanwhile it is not as good as traditional methods in terms of postoperative recovery.

ABSTRACT

Objective:To evaluate the efficacy of disposable disinfection cap in preventing catheter-related blood stream infections (CRBSIs).Methods:Literature on the prevention of CRBSIs by disposable disinfection caps were retrieved from CNKI, Wanfang Database, VIP database, Chinese Biomedical Literature Database, PubMed, Cochrane Library, Embase and Web of Science databases. The retrieval period was from the database construction to June 30, 2022. After literature screening, data extraction and quality evaluation were independently carried out by 2 researchers, RevMan5.4 software was used for analysis.Results:A total of 12 articles were included, including 9 832 patients. Meta-analysis results showed that compared with conventional manual disinfection, disposable disinfection cap could reduce the incidence of CRBSIs, and the difference was statistically significant ( RR = 0.58, 95% CI 0.43-0.79, P<0.01). Disposable disinfection cap could reduce the incidence of CRBSIs in adults, but there was no significant difference in the incidence of CRBSIs in children ( P>0.05). It could reduce the incidence of CRBSIs in patients with indwelling vascular catheters in ICU ( RR = 0.58, 95% CI 0.38-0.89, P<0.05), but there was no statistical difference in the incidence of CRBSIs in patients without indwelling vascular catheters in ICU ( P>0.05). In addition, the compliance of the use of disinfection caps by nursing staff was improved by 80% - 90% and the treatment cost was saved by about 282 - 464 dollars. Conclusions:Disposable disinfection caps can reduce the occurrence of CRBSIs, improve the disinfection compliance of nursing staff, and save the hospitalization cost of patients. It is suggested to be popularized in clinical practice.

ABSTRACT

Desde o início da pandemia, no final de 2019, até a retomada das atividades presencias de atendimento ao público nos cursos da área de saúde em Minas Gerais, especificamente nos cursos de odontologia; foram elaboradas notas técnicas, recomendações e manuais de biossegurança. Documentos esses que serviram como referencial para a padronização de protocolos de atendimento, incluindo o uso de EPI's mais eficientes no controle da transmissibilidade da Covid-19. A pesquisa de revisão foi realizada na base de dados PubMed/MEDLINE e Scielo, World Health Organization, Ministério da Saúde, Associação Brasileira de Ensino Odontológico, Conselho Federal de Odontologia, Conselhos Regionais de Odontologia dos Estados de São Paulo e Minas Gerais e instituições de ensino superior. Priorizando os conteúdos publicados entre os meses de janeiro a setembro de 2020. Período correspondente ao início da pandemia até o retorno das atividades de atendimento nos cursos de saúde em Minas Gerais. Buscando os documentos que nortearam esse retorno e que justificaram a adoção do uso desses EPI's pelos graduandos.


Since the beginning of the pandemic, at the end of 2019, until the resumption of face-to-face activities to serve the public in health care courses in Minas Gerais, specifically in dentistry courses; technical notes, recommendations and biosafety manuals were prepared. These documents served as a reference for the standardization of care protocols, including the use of more efficient personal protection equipment to control Covid-19's transmissibility. The review search was carried out in the PubMed/MEDLINE and Scielo databases, World Health Organization, Ministry of Health of Brazil, Brazilian Association of Dental Education, Federal Council of Dentistry, Regional Councils of Dentistry of the States of São Paulo and Minas Gerais and college. Prioritizing the contents published between January and September 2020. Period corresponding to the beginning of the pandemic until the return of care activities in health courses in Minas Gerais. Seeking the documents that guided this return and that justified the adoption of the use of this personal protective equipment by college students.

ABSTRACT

Objective:To investigate the availability and safety of a domestic disposable digital flexible cystoscope compared with a reusable Olympus digital flexible cystoscope in cystoscopy and removal of double J stent.Methods:From August 2018 to March 2019, patients were enrolled in this prospective, open, multicenter, randomized, parallel positive controlled clinical trial study, which were from department of Urology in Renmin Hospital of Wuhan University, the First Affiliated Hospital of Xiamen University and the First Affiliated Hospital of Guangzhou Medical University. The experimental group and control group were assigned into a 1∶1 ratio by random table method. Inclusion criteria included age≥18 years and have indications for cystoscopy or removal of double J stent. Exclusion criteria included patients having acute genitourinary tract infection, having tuberculous bladder contracture, bladder capacity less than 50ml, having urethrostenosis, female menstrual period, pregnancy and lactation, having difficulty for lithotomy position, having serious cardio-cerebrovascular disease and liver or kidney dysfunction. A domestic disposable digital flexible cystoscope was adopted in the experimental group, whereas a reusable Olympus digital flexible cystoscope was used in the control group. Acceptability of image was defined as primary availability indicator, while success rate of working and performance score were defined as secondary availability indicators and mean operating time was calculated for cystoscopy only and cystoscopy plus removal of double J stent respectively, yet rate of adverse event as well as rate of equipment defects were sorted as safety indicators.Results:A total of 188 cases which were listed in per protocol set completed the clinical trial study successfully. There were 95 cases in the experimental group and 93 cases in the control group. Acceptability of image was 93.68%(89/95) and 96.77%(90/93) respectively in two groups( P=0.52). Success rate of working was 100.00%(95/95) and 98.92%(92/93) respectively in two groups ( P=0.49). Performance score was 14.41±0.93 and 14.56±0.84 respectively in two groups ( P=0.23). Mean operating time (MOT) only for cystoscopy was (15.3±2.6) min and (15.4±3.3)min respectively in two groups ( P=0.93), while MOT for cystoscopy plus removal of double J stent was (21.0±3.2) min and (21.7±3.9) min respectively in two groups ( P=0.69). Rate of adverse event was 8.42%(8/95) and 9.68%(9/93) respectively in two groups( P=0.76). There was no equipment defects in both groups. Conclusions:There is no statistical difference in acceptability of image, success rate of working, performance score, mean operating time for cystoscopy or removal of double J stent, rate of adverse events and rate of equipment defects. A domestic disposable digital flexible cystoscope has shown non-inferiority in the availability and safety compared with a reusable Olympus digital flexible cystoscope.

ABSTRACT

Objective:To improve the multiple-link operation efficiency, effect and satisfaction of transaxillary dual-plane breast augmentation by optimizing and upgrading the configuration of auxiliary tools.Methods:From January 2019 to May 2021, breast augmentation was performed in 130 female patients (aged 32±8 years) in the Cosmetic and Plastic Center of the First Affiliated Hospital of Harbin Medical University. The study was conducted among 63 patients who were eligible for the criteria of high configuration surgery. 67 patients underwent standard configuration surgery. The average operation time, intraoperative blood loss, drainage volume (24 hours after operation), postoperative visual analog scale (VAS) pain score and satisfaction were statistically analyzed.Results:The data of high configuration method and standard configuration method were compared as follows: average operation time was (78.6±12.2) min / (93.1±12.1) min ( t=15.73, P<0.05); the average intraoperative blood loss was (3.1±1.0) ml / (14.4±3.5) ml ( t=13.83, P<0.05); the drainage volume (24 hours after operation) was (37.2±8.2) ml / (61.4±10.9) ml ( t=20.82, P<0.05); the pain score on the first day after surgery was (6.1±1.7) points / (7.5±1.6) points ( t=8.57, P<0.05). The overall satisfaction rate was 97.1±1.6 / 95.6±2.0 ( t=5.58, P>0.001), at 6 months after operation. No severe complications were found during the follow-up period, such as capsular contracture, hematoma, infection and double bubble deformity. Conclusions:The use of ultrasonic knife with delivery bag is an effective optimization and upgrade of the endoscopic assisted transaxillary dual plane breast augmentation. The advantages of this method are obvious, highly efficient, safe, effective and satisfactory. It is worthy of clinical application and promotion.

ABSTRACT

A Quimioterapia antineoplásica (QtA) adjuvante para câncer colorretal (CCR) traz consequências fisiológicas e psicossociais como os eventos adversos (EA) gastrintestinais e dermatológicos, que podem trazer dificuldades na utilização de equipamentos para os pacientes com estomia intestinal. Este estudo teve por objetivos analisar a produção científica nacional e internacional sobre os EA gastrintestinais e dermatológicos para pacientes com CCR em QtA adjuvante; e estabelecer as recomendações em relação à indicação de equipamentos coletores e adjuvantes para os pacientes com estomia intestinal por CCR, assim como para os cuidados com a estomia intestinal e o manejo destes equipamentos durante a QtA, com base nas evidências científicas. Trata-se de uma Revisão Integrativa, fundamentada na Prática Baseada em Evidências, cuja pergunta formulada foi: Quais os EA gastrintestinal e dermatológico decorrentes da quimioterapia adjuvante em pacientes com câncer colorretal? Os descritores utilizados para as buscas foram Neoplasias Colorretais, Quimioterapia, Toxicidade, Efeitos adversos, nas bases de indexação eletrônica PubMed, Literatura Latino-Americana e do Caribe em Ciências da Saúde (LILACS), Cumulative Index to Nursing and Allied Health (CINAHL), e Excerpta Médica (EMBASE), mediante os critérios de inclusão: estudos que abordassem a ocorrência e o tratamento de EA gastrintestinais ou dermatológicos em pacientes com CCR em QtA adjuvante; e estudos publicados em inglês, português ou espanhol, sem corte temporal de publicação; obtidos na íntegra via Biblioteca Central do Campus de Ribeirão Preto da Universidade de São Paulo; e os critérios de exclusão que foram estudos em modelo animal e modelo in vitro; estudos sobre metástases associadas ao tumor primário colorretal. Do total de 4.021 artigos científicos, resultantes das buscas, foram selecionados mediante os critérios de inclusão e exclusão, 20 estudos que constituíram a amostra final, sendo que todos foram publicados em inglês, com níveis de evidência pouco fortes em 65% (IV e VI) e cujos aspectos abordados foram categorizados em quatro temáticas: Diarreia e demais EA gastrintestinais; Diarreia e EA Gastrodermatológicos; EA Gastrintestinais; e EA Dermatológicos. No tema Diarreia e demais EA gastrintestinais evidenciou-se diarreia, náusea, perda de apetite e constipação, vinculados à QtA adjuvante com 5-FU, capecitabina e irinotecano. No tema Diarreia e EA Gastrodermatológicos evidenciou-se síndrome palmar-plantar, outras afecções cutâneas (eritema, úlcera, descamação, prurido, eritema, bolha), estomatite, edema das mãos/face/boca, e hipersensibilidade por capecitabina e oxaliplatina. No tema EA Gastrintestinais verificamos que náusea e vômito foram mais frequentes e relacionados à oxaliplatina. No tema EA Dermatológicos, evidenciou-se rash cutâneo, rubor, prurido, reações alérgicas, síndrome palmar-plantar nos esquemas com 5-FU, capecitabina, oxaliplatina e irinotecano. Considerando estas evidências e a necessidade de utilização de equipamentos coletores do paciente com estomia intestinal por CCR em QtA adjuvante, a assistência de enfermagem especializada deve assegurar a demarcação de estoma, ensino do autocuidado sobre EA e seu manejo, indicação e fornecimento de equipamentos coletores e adjuvantes, que atendam as suas necessidades, assim como o desenvolvimento de protocolos para avaliação dos EA gastrintestinais e dermatológicos durante a QtA adjuvante


The antineoplastic chemotherapy for adjuvant colorectal cancer (ACC) brings physiological and psychosocial consequences such as gastrointestinal and dermatologic adverse events (AE), which can cause difficulties in using equipment for patients with ostomy. This study aimed to analyze the national and international scientific literature on the gastrointestinal and dermatological EA for patients with CRC in adjuvant chemotherapy; and establish recommendations regarding the indication of equipment supplies and also aid for patients with ostomy by CRC, as well as for ostomy care and management of this equipment during the chemotherapy, based on scientific evidence. This is an integrative review, based on the evidence-based practice, whose question was: What are the gastrointestinal and dermatological EA resulting from adjuvant chemotherapy in patients with colorectal cancer? The descriptors used for the search were Colorectal neoplasms, Drug therapy, Toxicity, and Adverse effects, in electronics PubMed indexing bases, Latin American and Caribbean Health Sciences (LILACS), Cumulative Index to Nursing and Allied Health (CINAHL) and Excerpta Medical (EMBASE) by the inclusion criteria: studies that addressed the occurrence and treatment of gastrointestinal or dermatological EA in patients with CRC in adjuvant chemotherapy; and studies published in English, Portuguese or Spanish, without temporal cutting time publication; obtained in entirety at Ribeirão Preto Campus Central Library of the University of São Paulo; and the exclusion criteria were studies in animal models and in vitro model; studies on colorectal metastases associated with primary tumor. Of the total of 4,021 scientific articles resulting from the searches, they were selected by the inclusion and exclusion criteria, 20 studies which formed the final sample, all of which have been published in English, with little strong evidence levels 65% (IV and VI) and whose addressed aspects were categorized into four themes: Diarrhea and other gastrointestinal EA; Diarrhea and gastrointestinal and dermatologic EA; Gastrointestinal EA; Dermatologic EA. In the subject Diarrhea and other gastrointestinal EA became evident diarrhea, nausea, loss of appetite and constipation linked with adjuvant 5-FU, capecitabine, and irinotecan. On the topic diarrhea and Gastrointestinal and dermatologic EA showed up hand-foot syndrome, other skin disorders (erythema, ulcers, peeling, itching, erythema, and bubble), and stomatitis, swelling of the hands / face / mouth, and hypersensitivity oxaliplatin and capecitabine. On the topic Gastrointestinal EA were found that nausea and vomiting were more frequent and related to oxaliplatin. On the topic of Dermatological EA, evidence of skin rash, redness, itching, allergic reactions and palmar-plantar syndrome in the schemes with 5-FU, capecitabine, oxaliplatin and irinotecan. Considering this evidence and the need for equipment supplies in patients with ostomy by CRC in chemotherapy, specialized nursing care to ensure the demarcation of stoma, self-care education on EA and its management, display and supply collectors equipment and adjuvants that meet their needs, as well as the development of protocols for evaluation of gastrointestinal and dermatological EA during adjuvant chemotherapy

ABSTRACT Numerous pathogens, including SARS-CoV-2, can remain viable on surfaces over days, which favors cross-contamination. Preventive measures are essential to prevent infections and control the spread of COVID-19. Fox ruler is an essential device in the clinical planning of complete dentures and the contact with the patient's oral fluids during handling is unavoidable. Despite its importance, the conventional polycarbonate Fox ruler is easily damaged in the sterilization process. Therefore, the purpose of this article is to propose the use of a disposable Fox ruler. The confection is carried out in a simple way with wooden toothpicks joined by cyanoacrylate glue, following the same shape as the conventional one, but with the additional advantage of being made in an individual size according to the upper edge. The proposed disposable Fox ruler makes it possible to obtain the parallelism of the upper wax roller with the bipupillary line and with the Camper plane. Therefore, the disposable device described in this article was able to perform the same functions as the conventional one, in a practical way, without risks of cross contamination and with low cost.


RESUMO Vários patógenos, incluindo SARS-CoV-2, são capazes de permanecer viáveis em superfícies durante dias, o que favorece a contaminação cruzada. Medidas preventivas são essenciais para prevenir infecções e controlar a propagação da COVID- 19. A régua de Fox é um dispositivo essencial no planejamento clínico de próteses totais e é inevitável o contato com os fluidos orais do paciente, durante seu manejo. Apesar de sua importância, a régua de Fox convencional de policarbonato é facilmente danificada com o processo de esterilização. Portanto, o objetivo deste artigo é propor a utilização de uma régua Fox descartável. A confecção é feita de forma simples com palitos de madeira unidos por cola de cianoacrilato, seguindo o mesmo formato do convencional, mas com a vantagem adicional de ser produzida em tamanho individualizado de acordo com o rebordo superior. A régua Fox descartável proposta permite obter o paralelismo do rolete de cera superior com a linha bipupilar e com o plano de Camper. Portanto, o dispositivo descartável descrito neste artigo foi capaz de desempenhar as mesmas funções do convencional, de forma prática, sem riscos de contaminação cruzada e com baixo custo.

Objetivo: avaliar a qualidade e segurança individual e coletiva no uso de insulina pela população idosa em um município de grande porte do sul do Brasil. Método: estudo transversal de base populacional realizado em 2016-2017 com idosos de 68 anos ou mais. As entrevistas foram realizadas nos domicílios dos idosos. O desfecho foi medido pela forma de utilização e descarte das seringas e agulhas. Realizou-se análise descritiva mediante cálculo das proporções e projeções dos dados para os idosos do município. Resultados: foram entrevistados 735 idosos. A prevalência de diabetes foi de 20,0%, e 13,8% faziam uso de insulina injetável. A reutilização das seringas e agulhas foi relatada por 55,0% e 65,0% referiram descartar o material direto no lixo comum. Conclusões: a qualidade e a segurança dos idosos em insulinoterapia estão comprometidas, sendo necessário implementar estratégias de educação em saúde que visem melhorar o conhecimento e acesso as orientações adequadas.


Objective: to evaluate individual and collective quality and safety in the use of insulin by the aged population in a large city of southern Brazil. Method: a cross-sectional population-based study conducted in 2016-2017 with older adults aged 68 and over. The interviews were conducted in the older adults' homes. The outcome was measured by the way in which syringes and needles were used and disposed of. Descriptive analysis was performed by calculating the data proportions and projections for the older adults in the municipality. Results: 735 older adults were interviewed. The prevalence of diabetes was 20.0%, and 13.8% made use of injectable insulin. Syringes and needle reuse was reported by 55.0%, and 65.0% mentioned discarding the material directly in regular garbage. Conclusions: the quality and safety of the older adults in insulin therapy are compromised, and it is necessary to implement health education strategies aimed at improving knowledge and access to the appropriate guidelines.


Objetivo: evaluar la calidad y la seguridad individual y colectiva en el uso de insulina por parte de la población de edad avanzada en un importante municipio del sur de Brasil. Método: estudio transversal de base poblacional realizado en 2016-2017 con ancianos de al menos 68 años de edad. Las entrevistas se realizaron en los domicilios de las personas de edad avanzada. El resultado se midió considerando la forma de utilización y descarte de las jeringas y agujas. Se realizó un análisis descriptivo mediante el cálculo de las proporciones y proyecciones de los datos para los ancianos del municipio. Resultados: se entrevistó a un total de 735 personas de edad avanzada. La prevalencia de diabetes fue del 20,0%, y el 13,8% utilizaba insulina inyectable. El 55,0% indicó que reutilizaba las jeringas y agujas y el 65,0% mencionó que desechaba el material directamente en la basura común. Conclusiones: la calidad y la seguridad de las personas de edad avanzada sometidas a insulinoterapia están en riesgo, por lo que resulta necesario implementar estrategias de educación en salud que tengan por objetivo mejorar el conocimiento y el acceso a las pautas adecuadas.

  [Purpose] Acral coldness is a condition in which the fingers and/or toes are cold despite normal central body temperature. Regardless of the presence or absence of disease, many people suffer from acral coldness, especially elderly people. One method of relieving acral coldness is to heat the hands with a disposable warmer called “Kairo,” but this method interferes with daily work. We investigated whether or not heating other upper-limb parts with a pair of warmers can relieve acral coldness.   [Methods] After obtaining informed consent, 30 people who usually had acral coldness without other diseases received the Kairo holders for their necks, elbows, and wrists. Eighteen participants used a pair of Kairo warmers for each area for one week, respectively. The remaining participants wore only the holder to know the heat retention effect of the holders. All participants separately evaluated their acral coldness of the upper limbs and the cumbersomeness of holders and/or Kairo by using a 10cm visual analog scale (VAS). To measure the therapeutic effect of the Kairo, the temperature and blood flow of the surface of both hands were examined by thermography and laser speckle contrast analysis (LASCA) at the day after finishing each warming period. These evaluations were conducted after a 20-minute adaptation at 24°C.  [Result] The VAS of acral coldness of upper limbs decreased during the periods of warming on the neck and elbows with Kairo compared to the period without Kairo. There was no difference in the VAS value of the cumbersomeness among the neck, elbows, and wrists areas. The hand temperature on the day after the end of warming decreased slightly after elbow warming.  [Conclusion] We found that the coldness of the hand was improved by warming other parts, even if the hand was not warmed, but the effect varied depending on the warming area. No therapeutic effect was detected, but this was considered to be a problem in setting the measurement conditions. We believe these results will lead to new ways to improve acral coldness.

These evaluations were conducted after a 20-minute adaptation at 24°C.  [Result] The VAS of acral coldness of upper limbs decreased during the periods of warming on the neck and elbows with Kairo compared to the period without Kairo. There was no difference in the VAS value of the cumbersomeness among the neck, elbows, and wrists areas. The hand temperature on the day after the end of warming decreased slightly after elbow warming.  [Conclusion] We found that the coldness of the hand was improved by warming other parts, even if the hand was not warmed, but the effect varied depending on the warming area. No therapeutic effect was detected, but this was considered to be a problem in setting the measurement conditions. We believe these results will lead to new ways to improve acral coldness.

ABSTRACT

Objective To provide a portable disposable endoscopy for preventing potential viral infection in the patients with acute upper gastrointestinal bleeding (AUGIB) or those who need emergency endoscopy and reducing the process of endoscopy cleaning and disinfection during the corona virus disease 2019 (COVID-19) epidemic. Methods The novel portable and disposable endoscopy system (YunSendo) was self-designed. Two patients with suspected AUGIB underwent emergency gastroscopy with YunSendo system by a senior endoscopist. Standard image acquisition and assessments of operation performance and image quality were fulfilled to evaluate the preliminary safety, feasibility, and operation performance of the novel endoscopy. Results Emergency endoscopy was completed safely in two AUGIB patients by the YunSendo system without alternating current (AC) power supply. The endoscopic manipulation and observation, including water absorption, air inflation, water spray, body rotation, and specimen biopsy, can be smoothly and effectively achieved. The video-recording, image acquisition, and electronic graphic report output were achieved integratedly in YunSendo. The upper GI tract can be clearly visualized, with eligible specimen biopsy without any obvious adverse events and compilations. Operation time was similar to an ordinary gastroscopy. The disposable endoscope was discarded after use according to the requirements of the hospital infection-control department. Conclusions The novel portable and disposable endoscopy system, YunSendo, is safe and feasible for upper gastrointestinal examination, with favorable operation performance and image quality. The preliminary experience provides evidence for its convenient use of epidemic prevention.

ABSTRACT

Objective:To investigate the efficacy of new-type stereotaxic apparatus-assisted transfrontal puncture and drainage in the treatment of hypertensive intracerebral hemorrhage in the basal ganglia.Methods:A retrospective analysis was performed on the clinical data of 60 patients with hypertensive intracerebral hemorrhage in the basal ganglia who received disposable new-type stereotaxic apparatus-assisted transfrontal insertion with soft tunnels for hematoma aspiration drainage in our hospital from August 2017 to September 2019. The treatment efficacy was analyzed.Results:All patients were successfully punctured at one time; the puncture surface was 5-6.5 cm on the basement plane, where the hematoma surface was the largest; the puncture angle was 10-14°, and the puncture depth was 9-11.5 cm. Fifteen patients were operated within 6 h of hemorrhage, and the intraoperative hematoma clearance rate was about 25%; 40 patients were operated 6-24 h after hemorrhage, and the hematoma clearance rate was about 20%; 5 patients were operated one-3 d after hemorrhage, and the hematoma clearance rate was as high as 30%. The first postoperative re-check CT showed that 51 patients had ideal position of the drainage tube, 2 were too deep, one was too shallow, 2 were below the position, 2 were above the position, one was inside the position, and one was outside the position. The Glasgow Coma Scale (GOS) scores of the patients on 3 rd d of operation (9.88±3.998) were significantly higher than those of the patients before operation (6.24±3.159, P<0.05). One month after the operation, GOS showed that 20 patients (33.3%) had good recovery, 28 (46.7%) had mild disability, 7 (11.7%) had severe disability, 3 (5.0%) had plant survival, and 2 (3.3%) died. Conclusion:The disposable new-type stereotaxic apparatus-assisted transfrontal puncture drainage is easy to be conducted and practicable with a reasonable design, accurate positioning, minimal surgical traumas and satisfactory curative effect.

@#<p style="text-align: justify;">This study was conducted to: (1) determine the profile of nurses in various work settings based on available databases; (2) describe the current nursing education and programs in the Philippines; and (3) identify national key issues affecting the nursing workforce. The study utilized descriptive, cross-sectional research design; and employed institutional survey, Delphi survey, and round table discussions. Results showed that the Department of Health has the current number of nurses working in hospitals and communities in the different regions, including information on age and sex distribution, employment characteristics, and type of hospital affiliations. The Professional Regulations Commission Board of Nursing has the results of the national licensure examinations. Commission on Higher Education has information on BSN curriculum and schools. Priority key issues identified are: (1) nursing leadership and nursing role clarity, (2) workplace safety and violence, and (3) shortage of nurses, (4) unjust compensation, and (5) skills mix and geographic maldistribution. PNA is thought to have a big role in helping address these key issues. </p>

ABSTRACT

Objective: To observe the efficacy of modified Qingweisan on acute pulpitis (AP) of children with syndrome of hyperactivity of gastric fire and its effect on inflammatory factors. Method: One hundred and six patients were randomly divided into control group (52 cases) and observation group (54 cases) by random number table. Patients in control group got one-off root canal therapy, and azithromycin for suspension after the therapy for a continued 3 days, 10 mg·kg-1, 3 times/days. In addition to the therapy of control group, patients in observation group were also given modified Qingweisan for 7 days, 1 dose/day. Before treatment and at the 1st, 3rd, 5th and 7th days after treatment, visual analogue scale (VAS), faces rating scale-revised (FPS-R), verbal rating scale and traditional Chinese medicine (TCM) syndrome were scored before and after treatment. And pain relief time and time for masticatory function returned to normal were recorded. And levels of C-reactive protein (CRP), tumor necrosis factor-α (TNF-α), interleukin-1β (IL-1β), IL-6, calcitonin gene-related peptide (CGRP) and substance P were detected. And a 6-month follow-up was carried out to record the success or failure. Result: At the 3rd, 5th and 7th days after treatment, score of VAS in observation group was lower than that in control group(Prd and 7th days after treatment, score of FPS-R was lower than that in control group (Pth days after treatment, degree of pain in observation group was lighter than that in control group (PPPα, IL-1β, IL-6, CGRP and SP were all lower than those in control group(PConclusion: Modified Qingweisan can inhibit expressions of inflammatory factors and pain transmitters, ameliorate the pain and shorten course of disease among children after disposable root canal therapy, with a satisfactory short-term effect.

ABSTRACT

OBJECTIVES: An automatic needle destroyer (ANDY) was developed to prevent needlestick injuries, and usability tests were conducted in several hospitals. The addition of extra features to the ANDY is in progress, such as data collection and automatic identification of used syringes. Thus, this report describes how the ANDY can be used to track the data of used syringes. METHODS: The motor torque required for barrel separation differs according to syringe diameters. By monitoring the electric current which is consumed for the motor torque, the type of syringe can be identified. Twelve prototypes were produced, and five usability tests were conducted in hospitals. RESULTS: After use, a syringe is inserted into the proposed device, and the needle portion is then cut and separated from the syringe body (barrel) and discarded. The needles are collected in a sharps container for hygienic disposal, and the barrel is dropped into a general medical waste container. CONCLUSIONS: The ANDY can be used to track the syringe used for each patient. The barcode can be read while the syringe rotates in the main body of the ANDY with a built-in omnidirectional scanner. Collection of information during syringe disposal can facilitate stock management. This system could also be extended to other types of consumable medical devices, although it would still be a challenge to differentiate each medical device.

@#<p style="text-align: justify;">This paper explores the issues surrounding international nurse migration to Canada. Within the context of Karl Marx's idea of a "disposable industrial reserve army [of labor]" (Magdoff & Magdoff, p. 20; Marx, 1887, p. 438), this paper posits that internationally educated nurses (IENs) are viewed as disposable labour who have the potential to assume different labour requirements needed by the receiving nation state (Magdoff & Magdoff, p. 26). This paper provides a brief background of international nurse migration in history and literature. It proceeds to explore the issue of gendered and racialized labour, and a more focused discussion of the current state and pathways of IEN migration to, and the plight of IENs in Canada.</p>

@#Objective To optimize the ways of extubation after tracheotomy.Methods From August, 2016 to April, 2018, 118 patients after tracheotomy for brain injury were divided into early extubation group (n=74) and conventional extubation group (n=44). The success rate and tolerance of extubation were compared, and the biochemical markers of blood were tested before and seven days after extubation. The early extubation group was divided into disposable extubation group (n=37) and occlusion tube-extubation group (n=37), and their success rates of extubation were compared.Results There was no significant difference in success rate between the early extubation group and the conventional extubation group (χ2=0.016, P>0.05). The tolerance was less in the early extubation group on the first day of extubation (χ2=4.909, P<0.05), and it was not different seven days and 15 days after extubation (χ2<1.995, P>0.05). The procalcitonin, hypersensitive C reactive protein and white blood cell count decreased (t>2.680, P<0.05), and hemoglobin and albumin increased seven days after extubation (t>11.620, P<0.001). There was no significant difference in the success rate between the disposable extubation group and the occlusion tube-extubation group (χ2<2.902, P>0.05).Conclusion Satisfactory cough and deglutition reflex are the core indexes of successful extubation, other indications may not influence on the success of extubation but on tolerance. Extubation may benefit to control the complications of tracheotomy. The disposable extubation is more recommended.

Objective: To develop an environment-friendly and sterile four- blade vaginal dilator so as to resolve the problem of polluting environment that petroleum-based plastics released hydrogen chloride, dioxin and other poisonous and cancerogenic substance. Methods: The lower blade of the environment-friendly and sterile four-blade vaginal dilator was used as fixed blade, and upper blade, left blade and right blade were extensible and closable. And in the left and right blades, there were built-in smog exhaust tubes. And based on the previous invention of the twin-blades vaginal dilator, the device has been redesigned and developed by adding the left blades, right blades, locating sleeve and other related parts. Results: When the device expanded, both of left and right blades could extend out, and when it closed, both of them could be folded into the upper and lower blades. Therefore, it was safety and effective. Besides, the built-in smoking tubes of left and right blades could quickly exhaust harmful smog by the closest distance, and the plastics used in this device was not only environmentally friendly, but also it was insulation and cost-effective. Conclusion: The environment-friendly and sterile four-blades vaginal dilator used in LEEP surgery resolved two problems included the constructs of imported four-blades vaginal dilator were complex and the left and right blades couldn't extend out. And it effectively enhance the expand distance between left and right blades, and it enlarges operation field and surgery space. Besides, the use of the environmentally friendly plastics avoids the environmental pollution caused by the release of hydrogen chloride, dioxins and other toxic and carcinogenic substances when built-out smoking tubes and petroleum base plastics were incinerated. Therefore, it enhances the surgical quality.
