ABSTRACT

Introduction: The rapid transmission and severe symptoms associated with acute chikungunya virus (CHIKV) infection in children make it a highly concerning health issue. Objective: This study aimed to describe the characteristics of acute chikungunya virus infection in children from the Department of Caaguazú, Paraguay. Material and Methods: A retrospective observational study was conducted in the Department of Caaguazú, Paraguay, in 2023, with all patients who came to the Regional Hospital within 5 days of developing characteristic symptoms of acute CHIKV infection and tested positive for the virus by RT-PCR. Patients with Dengue or Zika infections were excluded. We collected data on clinical characteristics using a standardized case record form and created an electronic dataset for analysis. Results: A total of 461 children were included in the study. 51.6% were women. Cases were divided into groups based on pediatric age: infants (0-23 months, n=88, 19.1%), preschoolers (2-5 years, n=115, 24.9%), schoolchildren (6-11 years, n=163, 35.4%), and adolescents (12-17 years and 11 months, n=95, 20.6%). Schoolchildren and adolescents experienced a higher prevalence of myalgia (64.6%) and arthralgia (63.7%). Vomiting (89%), headache (89.4%), and retro-orbital pain (95%) were more common in preschoolers, schoolchildren, and adolescents, while rash (39.5%) and petechiae (18.5%) were more prevalent in infants. Four children died during the study period. Conclusion: The 2023 CHIKV virus epidemic in Paraguay had different clinical presentations depending on the age of the affected children, with fatal outcomes occurring in a small percentage of cases.


Introducción: La rápida transmisión y los síntomas graves asociados con la infección aguda por el virus de la chikungunya (CHIKV) en niños lo convierten en un problema de salud altamente preocupante. Objetivo: Este estudio tuvo como objetivo describir las características de la infección aguda por el virus de la chikungunya en niños del Departamento de Caaguazú, Paraguay. Materiales y métodos: Un estudio retrospectivo fue realizado en el Departamento de Caaguazú, Paraguay, en 2023, con todos los pacientes que acudieron al Hospital Regional dentro de los 5 días posteriores al desarrollo de síntomas característicos de la infección aguda por CHIKV y que dieron positivo para el virus mediante RT-PCR. Pacientes con infecciones por Dengue o Zika fueron excluidos. Se recogieron datos sobre las características clínicas mediante un formulario normalizado de registro de casos y se creó un conjunto de datos electrónicos para su análisis. Resultados: Se incluyeron en el estudio un total de 461 niños. 51,6% eran mujeres. Los casos se dividieron en grupos según la edad pediátrica: lactantes (0-23 meses, n=88, 19,1 %), preescolares (2-5 años, n=115, 24,9%), escolares (6-11 años, n=163, 35.4%), and adolescentes (12-17 años and 11 meses, n=95, 20.6 %). La mialgia (64,6%) y la artralgia (63.7%) fueron más prevalentes en escolares y adolescentes. El vómito (89%), la cefalea (89,4%) y el dolor retroorbitario (95%) fueron más comunes en preescolares, escolares y adolescentes, mientras que la erupción cutánea (39,5%) y las petequias (18,5%) fueron más prevalentes en lactantes. Cuatro niños fallecieron durante el período de estudio. Conclusión: La epidemia de virus de la CHIKV de 2023 en Paraguay tuvo diferentes presentaciones clínicas dependiendo de la edad de los niños afectados, con resultados fatales ocurriendo en un pequeño porcentaje de casos.

ABSTRACT

<b>Objective</b> To evaluate the effect of irregular follow-up during normalized prevention and control of novel coronavirus pneumonia (COVID-19) epidemic on BK virus (BKV) reactivation and clinical prognosis of kidney transplant recipients. <b>Methods</b> Clinical data of 363 kidney transplant recipients were retrospectively analyzed, and they were divided into the pre-epidemic follow-up group and during-epidemic follow-up group according to the follow-up time. All patients were followed up for 1 year. The follow-up interval was compared between two groups. The infection of BKV and the correlation between the infection process of BKV and renal graft function were analyzed in two groups. <b>Results</b> A total of 1 790 preson-times were followed up before COVID-19 epidemic and 2 680 during COVID-19 epidemic. Compared with the during-epidemic follow-up group, the follow-up intervals within 3, 3-6 and 7-12 months after kidney transplantation were shorter in the pre-epidemic follow-up group, and the differences were statistically significant (all <i>P</i><0.05). Within 1 year after kidney transplantation, 35 cases(32%) were diagnosed with BKV viruria, 3 cases(3%) of BKV viremia and 1 case(1%) of BKV-associated nephropathy (BKVAN) in the pre-epidemic follow-up group, and 53(25%), 3(1%) and 1(1%) in the during-epidemic follow-up group, with no statistical significance (all <i>P</i>>0.05). In the pre-epidemic follow-up group, the time for the initial diagnosis of BKV viruria was longer and the viral load of the first urinary BKV reactivation was smaller than those in the during-epidemic follow-up group, with statistical significance (both <i>P</i><0.05). The load of the first urinary BKV reactivation was positively correlated with the peak load of urinary BKV, and the differences between the baseline and serum creatinine levels at 1 and 3 months after BKV reactivation (all <i>P</i><0.05). <b>Conclusions</b> Irregular follow-up after kidney transplantation may lead to early BKV reactivation and higher detection value of the first viral load of urinary BKV, delay diagnosis and interventions, and lead to poor prognosis. It is urgent to establish a remote follow-up system to meet the follow-up requirements of kidney transplant recipients when public health incidents occur.

ABSTRACT

Objective:To investigate the prevalence of norovirus and the correlation between norovirus infection and stool traits and clinical symptoms in children with acute gastroenteritis during 2021-2022.Method:A total of 2 195 anal swab samples were collected from cases of acute gastroenteritis in Beijing Children 's Hospital Affiliated to Capital Medical University from January 2021 to December 2022. PCR fluorescent probe assay was performed to detect norovirus,the clinical information and test results were performed by statistical analysis. Results:The detection rate of norovirus was 19.09%(419/2 195)in 2 195 children. The detection rates of male and female were 17.80%(244/1 371)and 21.24%(175/824),respectively,with statistically significant differences between genders( χ2=3.945, P<0.05). The positivity rate of noroviruses was higher in aged 1 month to 3 years[29.09%(64/220)~40.69%(94/231)],especially in 1 year[40.69%(94/231)]. The norovirus detection rate ranged from 2.44%(2/82)to 35.84%(62/173)in different months,with a high detection rate between January and March,higher in the winter and spring[28.36%(150/529),21.07%(146/693)],showing a clear seasonal distribution.Clinical symptoms of both vomiting and diarrhea were significantly more common among norovirus positive than negative children,the difference was statistically significant( χ2=57.29, P<0.05). Conclusion:In the high incidence season of winter and spring,for infants and young children aged 1 month to 3 years with diarrhea accompanied by vomiting symptoms,especially those aged 1 year,it is recommended to undergo norovirus related testing in time. Early treatment and isolation measures should be taken for diagnosed children to reduce cross infection and prevent outbreaks.

ABSTRACT

Objective:To explore the therapeutic law of moxibustion in Professor Zhou Meisheng's medical manuscripts for epidemic hemorrhagic fever (EHF) based on data mining and knowledge map technology.Methods:The manuscript data of Professor Zhou Meisheng's moxibustion treatment of EHFwere collected from Infectious Diseases Department of Dangshan County People's Hospital from December 16, 1985 to December 25, 1987. Graphpad Grism 8.0 software was used for descriptive analysis. PHP 5.4 program code was used for association rule analysis. SPSS Statistics 26.0 was used for clustering analysis. Neo4j Community 3.5.25 database was used to analyze the syndrome-weight graph.Results:205 prescriptions were included. There were 21 symptoms with frequency>40, in which the frequency of aversion to cold, fever, rash and irritability was 100%. The main types of moxibustion methods used in the treatment included moxibustion frame fumigation moxibustion, Wanying acupoint moxibustion pen moxibustion, and fire needle instead of moxibustion. There were 29 acupoints with a frequency of >25, including Zhongwan (CV12), Shenshu (BL23) and Mingmen (DU4), etc. Association rules showed that Sanyinjiao (SP6)-Zhongwan (CV12)-Feishu (BL13)-Shenshu (BL23)-Zhiyang (DU9) had the highest correlation. Six effective clustering combinations of moxibustion for EHF were summarized by clustering analysis. The weight graph can obtained the first 30 relationships with high correlation of target syndromes.Conclusions:Professor Zhou applied the idea of "moxibustion for heat syndrome" to the treatment of EHF, and took the method of "acupoint selection according to symptoms" as the main acupoint selection idea for moxibustion treatment of EHF. In clinical practice, moxibustion combined with auxiliary operation of TCM is often used to treat EHF, which can achieve good results.

Since the World Health Organization(WHO)officially announced COVID-19 as a global pandemic in 2020,114 countries or regions in the world have been affected to varying degrees.The arrival of the post-epidemic era requires countries to take new epidemic prevention and control measures to deal with the problems and challenges that may arise in the future.In order to help China consolidate its epidemic prevention achievements over the years and gain a larger development window in the post-epidemic era,this study explored the changes in the global epidemic situation and the major national prevention and control policies in different countries,summarized the development and changes of China's epidemic prevention and control policies,and proposed new family-centered precision prevention and control measures that adapt to China's national conditions.

ABSTRACT

Objective To establish a risk assessment model based infectious disease early warning system and promote the improvement of infectious disease management level.Methods By collecting and organizing the latest research results,and referring to the theoretical framework of the relevant infectious disease early warning evaluation index system,a risk assessment model based infectious disease early warning system is constructed.Results A total of 12 experts were included in this study,and three primary indicators were selected,including pre-epidemic indicators,typical symptom period indicators,atypical symp-tom period indicators,and 7 secondary indicators.The average score for necessity of the system was 8.27±0.24 points,and the average score for accessibility was 7.74±0.34 points.Conclusion The establishment of an infectious disease early warning sys-tem based on risk assessment models is conducive to timely following the epidemic trend of infectious diseases in the local area,analyzing the distribution trend of survey indicators,and proposing corresponding early warning plans,which can provide relevant basis for effective prevention of infectious diseases.

ABSTRACT

Purpose/Significance The paper systematically reviews relevant research on infectious disease prediction models based on internet data,helps to realize the advancement of infectious disease surveillance,and provides references for the construction of intelli-gent three-dimensional prevention and treatment system of infectious diseases.Method/Process The development history and research direction of infectious disease surveillance and early warning based on internet data collected in the core database of Web of Science and CNKI in the past 20 years are reviewed,major existing problems and challenges are analyzed,and common prediction models and their optimization directions are summarized.Result/Conclusion The study on internet infectious disease surveillance shows the trend of diver-sification of monitoring diseases,refinement and specialization of data sources.Due to the complexity and uncertainty of internet data,most of the existing models are only suitable for short-term or real-time prediction.By constructing a combination model,strengthening multi-source data fusion,improving the selection of keywords and influencing factors,the model can be further optimized and the fitting effect and prediction capacity can be strengthened.

ABSTRACT

Objective:To study the epidemic characteristics of human brucellosis in Yunnan Province, and to provide a reliable scientific basis for formulating accurate prevention and control strategies of brucellosis.Methods:The epidemic data of human brucellosis in Yunnan Province from January 2019 to December 2021 were collected from the information system of the China Center for Disease Control and Prevention, as well as annual monitoring data on brucellosis reported by various states (municipalities) in Yunnan Province. Descriptive epidemiological methods were adopted to analyze the epidemic situation, distribution characteristics (time, region, population), and serological and pathogenic monitoring results of brucellosis.Results:From 2019 to 2021, 1 408 cases of brucellosis were reported in Yunnan Province, with an average annual incidence of 1.00/100 000. The number of cases increased from 321 in 2019 to 701 in 2021, and the incidence increased from 0.68/100 000 in 2019 to 1.50/100 000 in 2021. The onset time was mainly from April to September (857 cases). The top 3 regions with the highest number of reported cases were Kunming City (483 cases), Qujing City (379 cases), and Honghe Hani and Yi Autonomous Prefecture (281 cases), accounting for 81.18% (1 143/1 408) of the total number of cases. The age of onset was mainly 20 - < 70 years old, accounting for 89.70% (1 263/1 408). There were 958 males and 450 females, with a sex ratio of 2.13 ∶ 1.00. Farmers were the main occupation, accounting for 84.02% (1 183/1 408). From 2019 to 2021, a total of 26 280 serum samples from key populations of brucellosis were monitored in Yunnan Province, with 572 positive serological tests and a positive rate of 2.18% (572/26 280). A total of 169 strains of Brucella were isolated from blood samples from hospitals throughout the province, including 155 strains of sheep type 3 and 14 strains of sheep type 1. Conclusions:From 2019 to 2021, the incidence of human brucellosis in Yunnan Province has been on the rise, with a high incidence in summer and autumn. The main population affected is young and middle-aged male farmers. Therefore, it is necessary to strengthen disease monitoring and health education for key populations during the high incidence season.

[Objective]To provide insights into the clinical diagnosis and treatment of infectious diseases in summer through probing into the theory of heat epidemic and its characteristics formulated by famous doctors in the past dynasties.[Methods]The etiology,pathogenesis and treatment of heat epidemic were analyzed by summarizing the discussions of various doctors from different angles and at different levels,and the differentiation and treatment rules of several common summer infectious diseases were explored on the basis of the application of heat epidemic theory.[Results]The etiology and pathogenesis of heat epidemic can be classified as excessive main Qi,evil abundance and Yin deficiency,transpiration of summer heat and diffuse ptomaine.Its treatment can be roughly divided into interior-clearing and exterior-harmonizing,blood-cooling and orifice-unblocking.On the basis,the article explored the etiology,pathogenesis and treatment of summer infectious diseases,such as hand-foot-and-mouth disease,dengue and Corona Virus Disease 2019(COVID-19),which have certain similarities with heat epidemic.[Conclusion]The theory of heat epidemic,closely related to the syndrome and treatment rules of three types of summer infectious diseases,can be the tentative basis for clinical diagnosis and treatment of summer infectious diseases with traditional Chinese medicine.

[Objective]To introduce the clinical experience of Director ZHANG Guoliang,a famous and highly-experienced traditional Chinese medicine practitioner,in treating cough in convalescent period of Corona Virus Disease 2019(COVID-19)with exterior and interior differentiation therapy.[Methods]Based on the relevant discussions on epidemic diseases and coughs by doctors of all ages and the experience of more than 300 COVID-19 patients treated by Director ZHANG himself,the understanding of Director ZHANG on the etiology and pathogenesis of cough in convalescent period of COVID-19 was expounded,and his experience in treating cough in convalescent period of COVID-19 with exterior and interior differentiation therapy was summarized,and a typical case was interpreted.[Results]Director ZHANG believes that treatment of cough in convalescent period of COVID-19 should start from the disease location.The disease is located in the nose and pharynx,and the exterior treatment method is adopted,focusing on ventilating the lung and dissolving phlegm,dispelling wind and relieving cough.In clinical practice,a combination of Banxia Houpu Decoction and Zhisou Powder is commonly used,added or subtracted according to the symptoms.The disease is located in the trachea and lungs,and the interior treatment method is adopted,and the phlegm turbidity(phlegm heat)accumulation in lung syndrome focuses on clearing the lungs and dissolving phlegm,relieving cough and asthma,added or subtracted Maxing Shigan Decoction,Qianjin Weijing Decoction combined with Tingli Dazao Xiefei Decoction are often used.Qi and Yin deficiency syndrome should clear the lungs and dissolve phlegm,invigorate Qi and nourish Yin,generally use added or subtracted Zhuye Shigao Decoction combined with Shengmai Drink.The medical case attached belonged to the phlegm and pharyngeal syndrome,Director ZHANG first treated with ventilating the lung and resolving phlegm,dispelling wind and relieving cough,then strengthening the spleen and invigorating Qi,drying dampness and resolving phlegm,and finally achieved good results.[Conclusion]Director ZHANG's unique opinion and clinical experience in the treatment of cough in convalescent period of COVID-19 with exterior and interior differentiation therapy are worth learning and promoting.

ABSTRACT

Zhuye Shigaotang is one of the 100 classic prescriptions published by the National Administration of Traditional Chinese Medicine. This study used the traceability method and bibliometrics method to analyze the main applicable syndromes, efficacy and meanings, modern disease spectrum, and other aspects of Zhuye Shigaotang, so as to further promote the research and development of this prescription. The results showed that Zhuye Shigaotang originated from the Treatise on Febrile Diseases, and its ingredients included Lophatheri Herba, Gypsum Fibrosum, Pinelliae Rhizoma, Ophiopogonis Radix, Ginseng Radix et Rhizoma, Glycyrrhizae Radix et Rhizoma, and Oryza sativa. The main applicable syndromes of Zhuye Shigaotang recorded in ancient books included heatstroke, cough and asthma, sores, sweat syndrome, hiccup, insomnia, malaria, headache, blood syndrome, nasosinusitis, laryngitis, etc., involving diseases of internal medicine, surgery, gynecology, and pediatrics. Its pathogenesis was "incomplete residual heat, both Qi and Yin injuries, and stomach-Qi disharmony". The research found that Zhuye Shigaotang could also be used to treat acne, measles, and cholera, and it was especially suitable for the treatment at the later stage of epidemic diseases. Lophatheri Herba and Gypsum Fibrosum in the prescription could clear away heat and relieve vexation. Ginseng Radix et Rhizoma,Ophiopogonis Radix, Ginseng Radix et Rhizoma, and Oryza sativa could promote body fluid production and invigorate Qi. Pinelliae Rhizoma could harmonize the stomach and stop vomiting. The prescription had stable compatibility and had the effects of clearing away heat, relieving vexation, invigorating Qi, promoting body fluid production, and harmonizing the stomach. Zhuye Shigaotang was widely applied in modern clinical practices, with high application frequency in the digestive system, pediatric-related diseases, respiratory system, infectious diseases, circulatory system, and neuropsychiatric system. The specific symptoms included recurrent aphtha, hiccup, infantile pneumonia, infantile summer fever, unexplained fever, upper respiratory tract infection, epidemic hemorrhagic fever, and late measles.

ABSTRACT

The prescription of Xiangsu Powder (香苏散) reflects the exquisite balance in treating both the superficial and deep layers, harmonizing the body's vital energy and blood, and addressing the functions of the triple energizer simultaneously. Its application in treating cold epidemics has been a long time, yet its importance in modern disease treatment is often overlooked. Based on the treatment principles demonstrated by Xiangsu Powder in treating cold epidemics, it is proposed that in the treatment of modern epidemic diseases, the dual principles of cold and heat should not be neglected. When cold pathogens stagnate at the surface, they should be dispersed and detoxified, using methods that induce sweating to expel them from the surface. When cold and dampness coexist, efforts should be made to regulate the flow of vital energy, blood, and body fluids to restore the proper functioning of the three burners. When cold-induced pathogenic factors are severe and toxic, aromatic herbs should be utilized to dispel turbidity and avoid contamination. Throughout the entire course of treating epidemic diseases, attention should be paid to inducing sweating without damaging the body's vital energy, while also protecting the body's righteous qi, with the aim of providing refe-rence for the treatment of modern epidemic diseases.

ObjectiveTo analyze the epidemiological characteristics and etiology of clustered outbreaks of hand, foot, and mouth disease (HFMD) in Pudong New Area, Shanghai from 2017 to 2022, and to provide a scientific basis for the prevention and control of HFMD in the area. MethodsThe data related to HFMD clustered outbreaks from 2017 to 2022 were obtained from the Pudong New Area HFMD outbreak database. Descriptive analysis was conducted to explore the outbreak scope, seasonal characteristics, distribution of occurrence settings, and etiological composition. ResultsFrom 2017 to 2022, Pudong New Area reported a total of 2 547 HFMD clusters, involving 8 884 cases, with an average of 3.49 cases per event. The majority of events (78.52%) had between 2 and 4 cases. The peak reporting periods for clustered HFMD from 2017 to 2019 and in 2021 were during the summer (May‒July) and autumn (September‒November). The seasonal pattern was less distinct in 2020 and 2022, likely due to the impact of the COVID-19 pandemic. The majority of clustered outbreaks occurred in childcare facilities (44.64%), followed by households/neighborhood committees (44.21%), with schools accounting for a smaller proportion (12.39%). The etiology revealed the coexistence of multiple enterovirus genotypes, with a positive detection rate of 60.46%, and CoxA6 being the dominant strain. ConclusionHFMD clustered outbreaks in Pudong New Area show fluctuating trends, with significant yearly differences in the number of incidents. The predominant seasons for outbreaks are summer and autumn, with CoxA6 identified as the dominant strain. The implementation of prevention and control measures for COVID-19 significantly reduced the occurrence of HFMD outbreaks. Continuous monitoring and focus on large-scale clustered outbreaks in key institutions are essential for the future.

Objective @#To analyze the incidence characteristics and trends in pulmonary tuberculosis in the Hotan prefecture , before and after the epidemic , and to provide a reference basis for the formulation and evaluation of tuberculosis prevention and control measures in the Hotan prefecture . @*Methods @#The Hotan prefecture ’s pulmonary tuberculosis incidence data was collected between 2015 and 2021 . Joinpoint regression (JPR) model and Interrupted Time Series (ITS) model were established to explore the incidence trend of pulmonary tuberculosis , as well as the impact of COVID-19 prevention and control measures in Xinjiang on the incidence trend in Hotan , respectively. Furthermore , an analysis of variations in incidence among different age and gender subgroups was carried out. @*Results@#The results of the JPR model showed that from 2015 to 2021 , the reported incidence rate of pulmonary tuberculosis in the Hotan prefecture initially increased and then decreased , with a turning point appearing in December 2018 . The incidence rate in males was slightly higher than that in females , and the turning point and incidence trend were consistent with the overall trend . Among all age subgroups , those ≥60 age group had the highest incidence rate , with the trend also showing an initial increase followed by a decrease . A turning point in the incidence rate for the under 18 age group appeared in June 2021 , yet the trend was not statistically significant (P > 0. 05) .The turning points in the 19 - 59 age group and in those aged ≥60 were consistent with the overall trend . The results of the ITS model showed that the incidence rate of pulmonary tuberculosis in the Hotan prefecture significantly decreased since January 2020 , dropping from 319. 28 per 100 000 in 2019 to 155 . 88 per 100 000 in 2021 , a decrease of 51 . 16% year-on-year , with a monthly average reduction of 0. 049 per 100 000 .@*Conclusion @# In 2018 ,Xinjiang province integrated tuberculosis screening into the universal health checkup for the entire population ,which led to the identification of numerous cases of tuberculosis . In the Hotan prefecture , the reported incidence of pulmonary tuberculosis peaked in December 2018 and then started to decline . Under the impact of COVID-19 isolation measures in Xinjiang , the reported incidence rate showed a notable decrease starting in January 2020 . Reiterating preventive measures and remaining watchful for the possible appearance of latent tuberculosis patients is crucial as the pandemic fades .

ABSTRACT

The ethical problems in the prevention and control of public disasters and epidemics have attracted more and more attention. Briefly combed the disaster and epidemic events in ancient China. From the view of the several basic principles of public health ethics, this paper took four aspects of the distribution of medical and health resources for epidemic diseases, the isolation prevention and control, the skeleton convergence and the protection of public health conditions as examples, to dialectically treat the measures taken by ancient people to deal with disasters and epidemics and explore some enlightenment of public health ethics in ancient Chinese disasters and epidemics. The measures of epidemic prevention and disaster resistance in ancient China have their own formation and development process. Although the historical limitations are insurmountable, interpreting it by using the basic principles of public health ethics will help us understand the development process of epidemic prevention and control, promote the development of medical archaeology, and provide some reference for the construction of public health undertakings today.

Summarized the educational value of anti-epidemic spirit, boost TCM universities to realize cultural education and promote TCM cultural inheritance and innovation; analyzed the difficulties existing in the process of "ideological and creative integration" in TCM universities, and provide accurate direction for optimizing the path of "ideological and creative integration" in TCM universities. To explore the multiple paths of "ideological and creative integration" in TCM universities requires multi-party cooperation, joint management, accelerating the optimization of "ideological and creative integration", and establishing a hierarchical "ideological and creative integration" system, so as to provide new ideas for us to implement the educational goal of "establishing moral integrity in cultivation" under the background of normalized epidemic prevention and control.
