ABSTRACT

Gualou-Xiebai-Banxia decoction has a long history of medical use for treating cardiovascular diseases in China. In this study, we investigated the protective effect and underlying mechanisms GXB in type II diabetes with acute myocardial ischemia (T2DM-AMI) rats. We hypothesized that GXB may display its protective effect on T2DM-AMI by reducing endothelial progenitor cells (EPCs) apoptosisviaactivating PI3K (phosphatidyl inositol 3-kinase)/Akt (serine/threonine protein kinase B)/eNOS (endothelial nitric oxide synthase) signaling. Rats were challenged with a high-fat diet and intraperitoneal injection of streptozotocin to induce a model of type II diabetes mellitus (T2DM) and coronary ligation to induce acute myocardial infarction (AMI). Changes in metabolites were assessed via enzyme-linked immunoassay and biochemical examination. The number and apoptosis rate of EPCs in peripheral blood were detected by flow cytometry. Target mRNAs and proteins in EPCs were analyzed by RT-PCR and Western blot analysis. The results demonstrated that GXB treatment decreased T2DM-AMI-associated changes in plasma fasting blood glucose, muscular enzymes, and blood lipids, and reduced oxidative stress. Furthermore, EPC apoptosis was increased in T2DM-AMI rats and was associated with decreased mRNA and protein levels of PI3K, Akt, and eNOS compared to the controls. Conversely, T2DM-AMI rats treated with GXB exhibited more circulating EPCs and downregulated levels of cell apoptosis, combined with increased mRNA and protein levels of PI3K, Akt, and eNOS compared to those of untreated T2DM-AMI rats. Our study showed that GXB treatment mitigated EPC apoptosis and promoted PI3K/Akt/eNOS signaling in T2DM-AMI rats.

ABSTRACT

Objective:To observe the effect of Gualou Xiebai Banxiatang(GXBT) on cardiac function and myocardial fibrosis in rats after myocardial infarction. Method:The 36 male SD rats were randomly divided into blank group, sham group and surgical group, and 6 males in blank group and sham operation group. The model of phlegm obstruction in myocardial infarction of rats was replicated by ligation of left anterior descending coronary artery and high fat diet, and the successful rats were randomly divided into 3 groups: model group, GXBT group and acertil group. In the sham group, only the threading was not ligated. The blank group and the sham group and the model group were given 10 mL·kg-1·d-1 of normal saline, and 2.68 g·kg-1·d-1 of the GXBT group were given intragastric administration,and 0.36 mg·kg-1·d-1 was given intragastrically in acertil group. After 4 weeks of model, the heart function was detected by heart ultrasound to verify the success of the model. After 8 weeks, the heart function of the heart of the rat was detected by heart ultrasound again, and then the samples were sacrificed. The pathological changes of the myocardial cells of the rats were observed with hematoxylin-eosin(HE) staining, and the degree of myocardial fibrosis in the rats was observed by Masson staining. The changes of serum B-type natriuretic peptide (BNP) and galectin-3 (Gal-3) in rat serum were detected by enzyme-linked immunosorbent assay(ELISA) method, and the expression of Gal-3, Collagen Ⅰ (Col-Ⅰ) and Collagen Ⅲ (Col-Ⅲ) was detected by Western blot. Result:Compared with blank group and sham group, the left ventricular ejection fraction (EF) and short-axis shortening rate (FS) of model group were significantly decreased (P<0.01), the infiltration of inflammatory cells, the increase of the myocardial collagen fibers, the contents of BNP and Gal-3 in the serum were increased (P<0.05). The expression of Gal-3,Col-Ⅰ and Col-Ⅲ in the myocardial tissue increased significantly (P<0.01). Compared with the model group, EF and FS of GXPD were significantly increased (P<0.05), the morphological structure of myocardial cells was improved, the collagen fiber was decreased. The expression of BNP and Gal-3 in serum decreased significantly (P<0.05), and the content of Gal-3, Col-Ⅰ and Col-Ⅲ in myocardial tissue was decreased (P<0.05). Conclusion:Gualou Xiebai Banxiatang can improve cardiac function, reduce myocardial fibrosis and slow down the process of heart failure after myocardial infarction in rats with myocardial infarction. Its mechanism may be related to the decrease of Gal-3 expression.

ABSTRACT

Objective: To investigate the effect of Yiqi Yangyin Zhuyu recipe on expressions of inflammatory factor and transformation of classically activated macrophages(M1)/alternatively activated macrophages (M2) inflammatory phenotype in lipopolysaccharide (LPS)-induced RAW 264.7 cells. Method: Methyl-thiazdyl-tetrazolium(MTT) reduction assay was used to detect the effect of different concentrations of Yiqi Yangyin Zhuyu recipe on the proliferation of the cells. The release of nitric oxide was detected by the Griess method. Enzyme linked immunosorbent assay(ELISA) was used to detect the release of M1/M2 inflammatory cytokines in cell supernatant. The expressions of the pro-inflammatory factor genes of M1-macrophages and the anti-inflammatory factor genes of M2-macrophages were detected by Real-time PCR. The protein expression levels of tumor necrosis factor-α(TNF-α),interleukin(IL)-6,IL-1,nitric oxide synthase(iNOS) were detected by Western blot. Result: Results of MTT showed that Yiqi Yangyin Zhuyu recipe with the concentration of 2.0 g·L-1 and below had no effect on the cell proliferation. Results of Griess indicated that compared with blank group, the release of nitric oxide of LPS-induced group was increased (PPPPPPPα,IL-6,IL-1β,iNOS were up-regulated (Pα,IL-6,IL-1β,iNOS were down-regulated in Yiqi Yangyin Zhuyu recipe group, especially at the concentration at 2.0 g·L-1 (PConclusion: Yiqi Yangyin Zhuyu recipe could effectively inhibit the inflammatory reaction induced by LPS. The anti-inflammatory mechanism of Yiqi Yangyin Zhuyu recipe may be related to inhibition of macrophages to M1 phenotype polarization, so as to play the role of regulating immune and reducing the release of inflammatory cytokines, like NO,TNF-α,IL-6,IL-1β.

ABSTRACT

Objective: To prepare Gualou-Xiebai intragastric floating pellets(18-24 mesh)by extrusion-spheronization method, and investigate their release characteristics in vitro. Methods: Using the floating time of pellets as indicator and the percentage of microcrystalline cellulose (MCC), octadecanol, hydroxypropyl methylcellulose- K4M (HPMC-K4M)and HPMC-K15M in the solid excipients of the prescription as affecting factors on the basis of single factor experiments, the formulation was optimized by star design-response surface optimization method. Taking the content of 3, 29-dibenzoyl karounitriol in the release medium as the index, the release rate of the pellets in vitro was determined and the release characteristics were explored. Results: The optimum prescription of these pellets was as follows: Gualou-Xiebai dry extract 25 g, MCC 6 g, octadecanol 11 g, HPMC-K4M 3 g, HPMC-K15M 11 g, 70% ethanol solution as wetting agent, and 40% polyvinylpyrrolidone(PVP-K30)solution as binder. The floating time in vitro of the pellets prepared by this prescription was 12 hours, and the cumulative release rate data fitted well with the first-order release model (r>0.97). Conclusion: The Gualou-Xiebai intragastric floating pellets prepared in this study had good floating property and sustained release effect, indicating that the preparation method is feasible.

ABSTRACT

This study was designed to explore the anti heart failure mechanisms of the compatibility of Gualou with Xiebai based on network pharmacology in rat model of myocardial ischemia-reperfusion injury. Using the databases of Traditional Chinese Medicine Database@Taiwan (TCM Database@Taiwan), Traditional Chinese Medicine Systems Pharmacology Database and Analysis Platform (TCMSP), Drug Repositioning and Adverse Drug Reaction Chemical-Protein Interactome (DRAR-CPI) and Universal Protein Resource (Uniprot) to screen compounds and predict the target of active components, the Database for Annotation, Visualization, and Integrated Discovery (DAVID) database, we predicted the biological pathway and signal pathway in the compatibility of Gualou with Xiebai. The effects of Gualou Xiebai dropping pills on the apoptosis of myocardial cells and the expression of protein kinase B (Akt), p-Akt and cysteine aspartate-specific proteinase (caspase-3) protein were examined in the related signal pathway phosphatidylinositol-3-kinase/protein kinase B (PI3K/Akt) of myocardial ischemia reperfusion injury in rats. Twenty two compounds, such as 10 α-cucurbita-5,24-diene-3β-ol and macrostemonoside were found to protect rats from heart failure through multiple targets, multiple biological pathways and multiple pathways, involving biological pathways such as hormone stimulation reaction, phosphorylation, apoptosis regulation, and signaling pathways such as insulin, mitogen-activated protein kinase (MAPK), cell apoptosis and so on. After the intervention of Gualou Xiebai dropping pills, the PI3K-Akt signaling pathway was activated to promote the phosphorylation of Akt protein, reduce the expression of caspase-3 protein, inhibit apoptosis and protect the myocardium. The data verify the results of the network pharmacology, and explain the mechanisms of anti-heart failure activity of combination of Gualou with Xiebai.

ABSTRACT

[Objective]The paper summarizes Professor HAN Xinmin's experiences on treating pediatric sigh by using Gualou Xiebai Banxia decoction.[Methods] By learning from Professor HAN Xinmin, recording the relevant cases and analyzing typical cases, to sum up the academic thoughts and effective prescription, as well as list one proved case. [Results] Professor HAN Xinmin holds that most children with sigh have syndromes of deficiency and excess. This is the case of Qi deficiency of the spleen and lung, the insufficiency of the chest Yang and with phlegm evil. Using Gualou Xiebai Banxia decoction with the method of clearing Yang Qi and removing phlegm and activating blood stasis can achieve a satisfactory clinical curative effect by treating based on syndrome differentiation. [Conclusion] Professor HAN Xinmin has unique clinical experience for treating pediatric sigh. Gualou Xiebai Banxia decoction has significant effect, which has the value of popularization and application.

ABSTRACT

To observe the functions of Gualou Xiebai Banxia decoction(GXBD) on regulating lipid metabolism, anti-oxidation, and interposing ox-LDL/Lox-1 pathway, and to explore its anti-atherosclerosis (AS) mechanisms. AS models were established by using 42 Apo-E-/- male mice with high fat diet. AS model mice were randomly divided into the model group, simvastatin group, and GXBD high and low dose groups. C57BL/6J male mice were used as the normal control group, n=10 and the treatment lasted for 8 weeks. The levels of TC, TG, LDL-C, HDL-C, SOD, MDA, GSH-px, and ox-LDL in blood serum were tested 24 h after the last administration. The changes of aortic tissues structure were observed by HE staining; the expression levels of Lox-1 protein and the expression levels of mRNA were detected by Western blot and PCR respectively.Results showed that the blood lipid levels and MDA, ox-LDL levels in blood serum of model group were significantly higher than those in the normal control group, but SOD, GSH-px levels were significantly lower than those in the normal control group, and the Lox-1 protein and mRNA expression levels were also significantly higher than those in the control group(P<0.05), namely aortic atherosclerosis lesions were obvious in model group.The levels of blood lipid and MDA, ox-LDL of GXBD high and low dose groups and simvastatin group were significantly lower than those in model group, while SOD, GSH-px levels were significantly higher than those in model group, and Lox-1 protein and mRNA expression levels were significantly lower than those in model group(P<0.05), namely the aortic atherosclerosis lesions were significantly relieved. The above results indicated that GXBD was capable of modulating blood lipid, anti-oxidation, and inhibiting the expression of Lox-1, and interposing ox-LDL/Lox-1 pathway in the AS model Apo-E-/- mice, which may be one of the mechanisms of anti-atherosclerosis.

ABSTRACT

Objective]The article analyzed and summed up the experience of making formula and description on treatment of gastric abscess in Qing Dynasty Zhang Yuqing’s Medical Records, in order to provide reference in treating contemporary similar diseases.[Methods]Through studying the medical cases of this book,the authors selected out 30 cases of gastric abscess for futher analysis and summing up the Zhang Yuqing’s experience of description.[Result]He usually used the methods to cure gastric abscess, i.e. expelling phlegm by Banxi and Hualou,harmonizing stomach and liver by Zuojin pills, removing obstruction in channels by Xufuhua decoction, tonifying spleen by Liujunzi decoction and Weiling decoction.These cases had accurate effect and distinct treatment characteristics.[Conclusion]Zhang had rich experience in treating gastric abscess, those can broaden insight in treating similar diseases by traditional Chinese medicine,and increase clinical efficacy.

All groups were administered once a day for 7 successive days. One hour after the last administration,the MIRI models were produced by occluding the left coronary artery for 30 min and releasing the occlusion for 120 min. The changes in ST segment were observed,and the contents of creatine kinase-MB(CK-MB),cardiac troponin-T(CTNT)and myoglobin(MYO)in blood plas?ma were measured. The pathological changes in myocardial tissues were observed by optics and electric-microscope and the percentage of myocardial infarction was measured by detecting the content of Evan blue in myocardium. Results Compared to normal saline in the model group,GX had antagonism to ECG S-T segments elevation in rats with myocardial ischemia,while the contents of CK-MB, MYO,and CTNT in blood plasma declined significantly(P<0.05 or P<0.01);the disorder condition of myocardial cell fiber arrange?ment was improved,and edema between myocardial tissue and cells was relieved;the inflammatory cell infiltration and myocardial ne?crosis in cardiac allograft were significantly alleviated,and the percentage of myocardial infarction was decreased significantly(P<0.01). Conclusion GX may play an important protective role against the MIRI in rats.

Objective To explore whether evolving window orthogonal projection analytical method can be used in the chromatographic peaks matching between traditional Chinese medicine preparation and its medicinal ingredients in prescription. Methods The fingerprints of Gualou Xiebai Baijiu Decoction and the herbs (Gualou and Xiebai) in its prescription were established by using high performance liquid chromatography-photodiode array detector (HPLC-PDAD), and the HPLC peaks in fingerprints of Gualou Xiebai Baijiu Decoction and the herbs in its prescription were matched according to the principle of evolving window orthogonal projection analytical method. Results Evolving window orthogonal projection analytical method could match the chromatogram peaks well between different fingerprints of Gualou Xiebai Baijiu Decoction and the herbs in its prescription. There were 20 common peaks between the ethyl acetate extraction of Gualou Xiebai Baijiu Decoction and Gualou, and 9 common peaks between the n-butanol extraction of Gualou Xiebai Baijiu Decoction and Xiebai. Conclusion Evolving window orthogonal projection analytical method can be used in the chromatographic peaks matching between traditional Chinese medicine preparation and its medicinal ingredients in prescription.

Objective To study the in situ intestinal absorption behaviors of 3, 29- dibenzoyl- karounitriol (DK) in Gualou- Xiebai (GX) extract in rats. Methods A rat in situ single-pass perfusion model was used and the concentrations of the perfusate were determined by HPLC-PDAD to investigate the intestinal absorption site and mechanism. Results The main absorption site of DK in GX extract was jejunum, ileum and colon, and the absorption had no significant difference in the three different segments of rat intestine (P>0.05), but was significantly higher than that in duodenum (P0.05). Conclusion DK in GX extract could be absorbed in whole intestinal segment, with the best intestinal absorption site of jejunum, ileum and colon. Its absorbing mechanism may be related to passive diffusion.

ABSTRACT

Objective To observe the effect of therapies of activating blood and removing stasis, activating yang and dissipating phlegm, and warming and dredging meridians with aromatic herbs on relieving acute myocardial ischemia (AMI). Methods Rats were randomized into normal group,model group, Xuefu Zhuyu Decoction (XZD) groups (treated with XZD 20.5 and 6.8 gkg-1d-1 ), Gualou Xiebai Banxia Decoction (GXBD) groups (treated with GXBD 18.0 and 6.0gkg-1d-1 ), and Guanxin Suhexiang Capsules (GSC) groups (treated with GSC 0.19 and 0.06 gkg-1d-1 ). The AMI rat models were established by intravenous injection of pituitrin (Pit, 1.5U/kg) through the caudal vein. The effect of the three Chinese medical therapies on AMI rat electrocardiogram (ECG) and plasma endothelin (ET) level was evaluated.
