ABSTRACT

SUMMARY OBJECTIVE: This study aimed to investigate the factors associated with behavioral problems in children with congenital pseudarthrosis of the tibia. METHODS: Random sampling is utilized to obtain a sample of 90 patients. The behavioral problems of the patients are detected by Achenbach Children's Behavior Scale. Parental emotional problems are investigated by the Self-Rating Depression Scale and Self-Rating Anxiety Scale. RESULTS: The results demonstrate that the detection rate of behavioral problems in children with congenital pseudarthrosis of the tibia is 53.3% (48/90). Among these behavioral problems, an abnormal rate is higher in the four dimensions: thinking, violation of discipline, social interaction, and aggression. The anxiety and depression scores of caregivers are statistically higher in the abnormal group than in the normal group. The results of the multivariate analysis show that the anxiety degree of the parents had a significant impact on the behavior of the children. CONCLUSIONS: Children with congenital pseudarthrosis of the tibia are facing the issues of high rates of behavioral problems. Parents of children with congenital pseudarthrosis of the tibia had higher levels of anxiety and depression than parents of normal children. The anxiety and depressive state of mind of parents or caregivers had a significant impact on the behavior of children with congenital pseudarthrosis of the tibia.

ABSTRACT

Objective:We identified the factors that influenced the effect of the Wearable Cyborg Hybrid Assistive Limb (HAL) in patients with amyotrophic lateral sclerosis (ALS).Methods:The subjects were patients with ALS who were admitted to our hospital, and all of them underwent HAL intervention. The changes in 2-minute walking distance (2 MD) before and after 1 course of HAL intervention were investigated. Next, we examined the factors that influence the degree of 2 MD improvement using patient attributes and indices of body composition, nutritional status, respiratory function, and ADL (Activities of Daily Living) before HAL intervention.Results:Thirteen subjects showed significant improvement in 2 MD measured before and after the HAL intervention (median 2 MD [quartile] before intervention-median 2 MD [quartile] after intervention:36.0 [32-94.49] m-74.1 [32.75-117.8] m, p=0.001). The Barthel Index (BI) and phase angle were identified as the factors that influenced the degree of 2 MD improvement (ANOVA:p=0.004).Conclusion:The results suggest that the degree of improvement in 2 MD by HAL intervention in patients with ALS was the most affected by their BI and phase angle before the intervention. Thus, BI and phase angle may be useful as predictors in the degree of 2 MD improvement by HAL intervention in patients with ALS.

ABSTRACT

Objective: This study aimed to elucidate the factors that affect medication adherence in outpatients with depression using multivariate analysis.Methods: A web-based survey on medication adherence, medication attitude (Drug Attitude Inventory-10; DAI-10), medication self-management, and satisfaction with the prescribing physician/pharmacy for outpatients with depression was conducted in January 2018. T-tests and path analysis were performed to evaluate comparisons of responses based on gender, high/low importance of medication guidance, high/low intention to consult, and high/low frequency of consultations.Results: For mean scores, no significant difference was observed between males and females according to a t-test. In terms of the importance of medication guidance, the high importance group had significantly higher prescribing physician/pharmacy satisfaction and better self-management. In terms of intention to consult, respondents with high intention to consult had significantly higher scores on the DAI-10, better self-management, and higher prescribing physician/pharmacy satisfaction. In terms of frequency of consultations, only pharmacy satisfaction was significantly higher in the high-frequency group. Path analysis revealed that, in general, adherence was influenced by medication attitude and self-management, and prescribing physician/pharmacy satisfaction and self-management were influenced by medication attitude. In the intergroup comparison, pharmacy satisfaction did not affect medication adherence in males or the group with low importance of medication guidance. In addition, medication attitude did not affect adherence only in the non-consultation group.Discussion: These results suggest that it is necessary to improve the medication support measures at pharmacies for outpatients with depression. Furthermore, in order for the measures to improve medication attitudes and adherence to be effective, it is necessary for patients to understand the benefits and importance of using pharmacists effectively, including medication support and consultation, and to be actively involved in the maintenance and promotion of patients' health.

ABSTRACT

OBJECTIVE To explore the merging methods and influencing factors of health state disutility values estimation. METHODS Retrieved from 6 Chinese and English databases such as CNKI and PubMed ,the literatures about disutility values of diarrhea caused by antitumor drugs were collected from the inception to July 2021. After 2 researchers independently screened the literature,extracted the data ,assessed the quality ,the Meta-analysis and regression analysis were conducted using Stata 16.0 software. RESULTS Fifteen literatures were included. The results of Meta-analysis showed that diarrhea caused by antitumor drugs had a significant impact on health utility [ MD=-0.26,95%CI(-0.30,-0.22),P<0.05]. The results of subgroup analysis showed that the disutility values of the 3 types of negative value ,non-negative value changed into negative value ,and non-negative value combined with basic state changed into to negative value were MD =-0.14,95%CI(-0.19,-0.09);MD=-0.46,95%CI (-0.56,-0.36);MD=-0.12,95%CI(-0.20,-0.05),respectively. Meta regression results showed that the year of publication , survey country/region ,severity of adverse events ,basic state settings ,utility estimation tools ,utility report types ,and utility statistical methods significantly affected the value of diarrhea disutility (P<0.05). CONCLUSIONS Disutility values for treatment-related symptoms or complications should be fully considered when inputting the parameters to the economic evaluation model. In the study or application of disutility values ,the types of utility reports should be distiguished ,and the core influencing factors such as the investigation country or region ,the severity of symptoms or complications ,and whether the basic state is set should be focus on.

ABSTRACT

@#Objective To analyze the influencing factors of post-operative delirium (POD) in middle-aged and elderly patients in intensive care unit (ICU) and construct risk prediction model for it.Methods A total of 112 middle-aged and elderly postoperative patients in the ICU of Lu'an Hospital of Anhui Medical University from January, 2018 to February, 2021 were selected. On the second day after the operation, they were transferred to ICU, and assessed with the Confusion AssessmentMethod for Intensive Care Unit (CAM-ICU). The patients were divided into delirium group (n = 52) and non-delirium group (n = 60) according to assessment. Univariate analysis was used to compare the differences in clinical data between the two groups, and multivariate Logistic regression analysis was used to screen the independent influencing factors to construct risk prediction model. Receiver operating characteristic (ROC) curve was used to evaluate prediction performance. Results Multivariate logistic regression analysis showed Acute Physiology and Chronic Health Evaluation II score (APACHE II score) (OR = 1.424, 95%CI 1.204 to 1.685, P < 0.001), ICU sleep quality score (OR = 1.432, 95%CI 1.159 to 1.770, P < 0.001), and postoperative oxygenation index ≤ 300 (OR = 4.485, 95%CI 1.644 to 12.240, P = 0.001) were independent influencing factors of postoperative delirium in ICU. The prediction model was: logit(P) = -11.381+0.354X1 (APACHE II score, cut-off value 16)+0.359X2 (ICU sleep quality score, cut-off value 13)+1.501X3 (postoperative oxygenation index ≤ 300), with the sensitivity and specificity of 79.2% and 79.7% respectively. The area under the ROC curve was 0.866 (95%CI 0.801 to 0.930), more than those of the factors alone (P < 0.05). Conclusion The prediction model based on Logistic regression can predict the occurrence of postoperative delirium in middle-aged and elderly patients in ICU.

ABSTRACT

@#AIM: To explore the current situation of myopia among children and adolescents in Mengzi city and the possible influencing factors of myopia.METHODS: A multi-stage cluster sampling method was used to select students in 7 schools(2 primary schools, 2 junior high schools, 2 high schools, and 1 vocational high school)in Mengzi city, Yunnan Province in October 2019. A total of 1 837 students were selected for questionnaire surveys, and examination of distance visual acuity and noncycloplegic autorefraction. There were 1 622 valid questionnaires were finally collected after checking the integrity and rationality of the questionnaires. RESULTS: The prevalence of myopia among primary and secondary students in Mengzi city in 2019 was 61.34%. The prevalence of myopia in girls(71.36%)was higher than that in boys(50.45%), and the prevalence of myopia in Han nationality(70.19%)was higher than that in ethnic minorities(57.70%). With the grade growth, the prevalence of myopia showed an upward trend. Multivariate Logistic regression analysis showed that the risk factors of myopia were female(<i>OR</i>=2.308), Han nationality(<i>OR</i>=1.712), higher learning stage(junior high school: <i>OR</i>=1.579, high school: <i>OR</i>=5.538), the time of doing homework daily in the past 1wk(1-<2h: <i>OR</i>=1.456, 2-<3h: <i>OR</i>=1.514, ≥3h: <i>OR</i>=1.901), occasionally or never keep your eyes more than one foot away from a book while reading and writing(<i>OR</i>=1.741), insufficient sleep(<i>OR</i>=1.585), parental myopia(<i>OR</i>=2.191).CONCLUSION:The prevalence of myopia among primary and middle school students in Mengzi city is at a relatively high level. Female, Han nationality, higher learning stage, the time of doing homework daily in the past 1wk≥1h, occasionally or never keep your eyes more than one foot away from a book while reading and writing, insufficient sleep and parental myopia are all risk factors that can cause myopia.

ABSTRACT

Objective @#To investigate the influencing factors of vascular crisis after oral and maxillofacial tumor free tissue flap transplantation. @*Methods @# A retrospective analysis was performed on all patients who underwent free tissue flap transplantation and developed vascular crisis in the surgical ward of head and neck cancer in a grade A specialized hospital. Forty-six possible influencing factors were collected using 1:1 matching according to surgeons, operation time, sex and age of patients from patients without vascular crisis from 2015-2020 in this ward during the same period. SPSS 26.0 statistical software was used for univariate and multivariate logistic regression analyses of the data.@*Results @#A total of 158 patients were enrolled, including 79 in the crisis group and 79 in the pairing group. Univariate analysis was performed for each variable. Paired logistic regression analysis showed that only postoperative blood potassium (P = 0.048, OR = 3.118, 95% CI: 1.008-9.641) and preoperative and postoperative red blood cell count differences (P = 0.004, OR = 4.53, 95% CI: 1.609-12.750) were statistically significant.@*Conclusion @#High blood potassium levels and red blood cell count differences before and after surgery were risk factors for vascular crisis.

@#In this paper, the uncertainties of correction factors of fluconazole impurities determined by HPLC standard curve method were evaluated, and the main common factors affecting the accuracy of standard curve method were found, so as to improve the accuracy of the method.In this study, the corresponding fitting lines of fluconazole and its impurities A, B, C, D, F and I were established respectively, and the ratio of the slope of fitting lines of each impurity and its corresponding principal component was calculated as the correction factor of the impurity.Then on the basis of GUM method, the uncertainty of each impurity correction factor determined by standard curve method was evaluated according to the established uncertainty evaluation scheme of correction factor determination process.The correction factor and uncertainty of fluconazole impurities A, B, C, D, F and I were 1.068 ± 0.046, 0.102 ± 0.005, 0.0582 ± 0.0031, 1.382 ± 0.121, 0.802 ± 0.067 and 1.383 ± 0.119, respectively, and the coverage factor k was 2.Finally, the contribution rate of each uncertainty component was calculated.In the relative combined standard uncertainties urel(f) of fluconazole impurities A, B, C, D, F and I correction factors, the sum of contribution rate of slope uncertainty urel(K) of the linear equation of principal component and its impurity is more than 85%; in the slope uncertainties urel(K) of linear equation, the contribution rates of uncertainties of solution concentration in 8 of 12 data groups are more than 80%, and the contribution rates of uncertainties introduced by reference substance content in solution concentration are about 80%.It can be seen that the preparation of linear solution concentration is the most influential factor in the determination of impurity correction factor by standard curve method, followed by the linear fitting process.In the preparation process of linear solution concentration, the purity of reference substance is the most influential factor, followed by weighing and pipetting times.The conclusion can help the experimenters to better formulate experimental plans and ensure the accuracy of the results when doing similar work.

ABSTRACT

Objective @#To investigate the incidence of new HIV infection and its influencing factors among men who have sex with men ( MSM ) in China, so as to provide the evidence for formulating the AIDS control strategy.@*Methods @#Cohort studies pertaining to new HIV infections among Chinese MSM populations were retrieved in Chinese and English electronic databases, including CNKI, Wanfang Data, VIP, PubMed, Web of Science, Embase and Cochrane Library, and the retrieval time was set to build the database until May 2021. A meta-analysis was performed to investigate the incidence of new HIV infection and its influencing factors among Chinese MSM populations. The publication bias was assessed using funnel plot and Egger's test, and the stability of outcome parameters was evaluated using sensitivity analysis.@*Results @#A total of 850 publications were retrieved, and 43 eligible literatures ( 44 studies ) were included in the final analysis. The pooled incidence of new HIV infection was 4.93 ( 95%CI: 4.15 to 5.72) per 100 person-years among Chinese MSM populations. Egger's test showed a publication bias ( t=2.411, P=0.021). Subgroup analysis revealed high incidence of new HIV infection in east ( 5.21 per 100 person-years ), north ( 5.30 per 100 person-years ) and southwest (4.84 per 100 person-years) China, and the lowest incidence in northwest China ( 2.87 per 100 person-years ), and showed higher incidence in first-tier cities ( 5.12 per 100 person-years ) than in nonfirst-tier cities ( 3.89 per 100 person-years ). Syphilis infection ( HR=3.41 ), unprotected anal sex ( HR=2.52 ), multiple sexual partners ( HR=3.51 ), seeking sex partners in bars, public baths and parks ( HR=3.44 ), age of > 25 years ( HR=0.44 ), Han Ethnicity ( HR=0.35 ), awareness of AIDS-related knowledge ( HR=0.20 ) and provision of HIV preventive services ( HR=0.18 ) were associated with new HIV infection in Chinese MSM populations. In addition, publication bias was found in studies reporting syphilis infection ( t=4.841, P<0.001 ), unprotected anal sex ( t=4.114, P=0.006 ) and ethnicity ( t=-5.018, P=0.038 ). @*Conclusions @#The incidence of new HIV infection is high among Chinese MSM populations. Interventions targeting the factors affecting new HIV infections are required to reduce in the incidence of new HIV infections among Chinese MSM populations.

@#AIM: To investigate the current quality of life in children with amblyopia and its influencing factors.<p>METHODS: Cross-sectional study. Using random sampling method, 178 children with amblyopia were selected as the research objects, and the Chinese version of the Universal Core Scale of Children's Quality of Life was used to investigate the research objects. The impact of amblyopia on children's quality of life was analyzed. <p>RESULTS:The quality of life score of children with amblyopia was(77.23±15.52)points. The results of multiple linear regression showed that parental education, left eye best-corrected visual acuity, right eye corrected visual acuity, coverage, premature delivery, hypoxia at birth and delivery way were the main factors affecting the quality of life of amblyopic children. It can explain 38.6% of the variance in the quality of life of children with amblyopia(<i>F</i>=15.242, <i>P</i><0.001).<p>CONCLUSION:The current quality of life in children with amblyopia is at a low-to-moderate level. Amblyopia may have a greater negative impact on children's daily life, learning, psychology and social interactions. It is necessary to implement health plans and psychological guidance for children with amblyopia and their parents, improve treatment compliance, improve vision, strengthen health guidance for women of child bearing age, and provide reference for further improving the quality of life of children with amblyopia.

ABSTRACT

ObjectiveTo investigate the influencing factors of intestinal metaplasia or atypical hyperplasia in chronic atrophic gastritis (CAG) patients and establish a prediction model. MethodThe clinical records and laboratory examination data of 335 CAG patients treated in the department of gastroenterology of the Second Affiliated Hospital of the Anhui University of Chinese Medicine from June 2016 to June 2021 were collected. Single and multiple Logistic regression analyses were used to explore the influencing factors of intestinal metaplasia or atypical hyperplasia in CAG patients by SPSS 26.0. A prediction model was constructed based on the data of the related influencing factors. In addition, 115 CAG patients diagnosed in the First Affiliated Hospital of Anhui Medical University from June 2019 to June 2021 were selected as external validation samples to verify and evaluate the prediction efficiency of the constructed prediction model. ResultMultiple Logistic regression analysis showed that pepsinogen Ⅰ[odds ratio(OR) 0.994,95% confidence interval(CI) (0.990,0.999),P<0.05],the number of focus[OR 6.765,95% CI(3.831,11.945),P<0.01], and Helicobacter pylori (Hp) infection[OR 0.546,95% CI(0.335,0.888),P<0.05] were independent risk factors for intestinal metaplasia or atypical hyperplasia in CAG patients(P<0.05). The formula of the prediction model is as follows:P=-1.558+0.606×Hp infection-0.006×pepsinogen Ⅰ+1.912×the number of focus. The receiver operating characteristic (ROC) curve showed the specific parameters as below: the area under the ROC curve of 0.76,the Youden index of 0.443,the best cut-off value of 0.52,sensitivity of 0.533,and specificity of 0.910. The prediction model was applied to the data of patients in the validation group for validation,and the predictive efficiency of the model was tested by decision curve analysis (DCA). The results showed that the model had a good fit and high predictive value. ConclusionPepsinogen Ⅰ,the number of focus, and Hp infection are independent risk factors for intestinal metaplasia or atypical hyperplasia in CAG patients. The prediction model constructed based on these factors has a good fit and high predictive value,which can provide references for the classification of CAG patients and the formulation of individual treatment protocols.

OBJECTIVE@#To explore the intervention effect of recombinant human interleukin-11 (rhIL-11) and recombinant human granulocyte-colony stimulating factor (rhG-CSF) on the duration and severity of agranulocytosis in patients with hematological malignancies after chemotherapy, and to analyze the influencing factors.@*METHODS@#The data of hematological malignancy patients treated with rhIL-11 and rhG-CSF after chemotherapy in the hematology department of The First Hospital of Lanzhou University from July 2017 to July 2020 were collected retrospectively. The duration and differences of agranulocytosis in differeent groups were compared by univariate analysis, and the influencing factors of agranulocytosis duration were further analyzed by multiple regression analysis.@*RESULTS@#The duration of agranulocytosis in 97 patients was 6.47±2.93 days. The results of univariate analysis showed that there were no statistical differences in the duration of agranulocytosis among patients with different sex, age, height, weight, body surface area, body mass index (BMI), dose of rhG-CSF, dose of rhIL-11, spontaneous bleeding after administration of rhG-CSF and rhIL-11, and the duration of agranulocytosis in patients with different red blood cell count (RBC), hemoglobin(HGB) level, platelet count (PLT) and absolute neutrophil count (ANC), before administration of rhG-CSF and rhIL-11. There were significant differences in agranulocytosis time among patients with different disease types, chemotherapy cycle, fever after rhG-CSF and rhIL-11 administration, and different white blood cell count (WBC) baseline level before rhG-CSF and rhIL-11 administration (P<0.05). Compared with patients with acute lymphoblastic leukemia (ALL) and non-Hodgkin lymphoma (NHL), patients with acute myeloid leukemia (AML) had the longest duration of agranulocytosis, which was 7.07±3.05 d. Compared with patients with chemotherapy cycles of 4-6 and ≥7, patients with total chemotherapy cycle of 1-3 had the shortest duration of agranulocytosis, which was 5.25±2.48 d. Compared with patients without fever, patients with fever within 1 day after administration of cytokines and patients with fever within 2-5 days after administration of cytokines, the duration of agranulocytosis was the longest in patients with fever 6 days after administration of cytokines, which was 8.85±2.85 d. Compared with patients with WBC baseline <1.0×109/L, (1.0-1.9)×109/L and (2.0-3.9)×109/L, patients with WBC baseline ≥4.0×109/L had the shortest duration of agranulocytosis, which was 4.50±2.56 d. Multiple linear regression analysis showed that chemotherapy cycle, different fever after administration of rhG-CSF and rhIL-11, diagnosis of ALL and NHL, and WBC baseline level before administration of rhG-CSF and rhIL-11 were the influencing factors of the duration of agranulocytosis (P<0.001).@*CONCLUSION@#The risk of prolonged agranulocytosis is higher in patients diagnosed with AML, with more chemotherapy cycles, lower WBC baseline before cytokines administration and fever later after cytokines administration, which should be paid more attention to.

ABSTRACT

In 2019, cardiovascular disease (CVD) accounted for 46.74% and 44.26% of all deaths in rural and urban areas, respectively. Two out of every five deaths were due to CVD. It is estimated that about 330 million patients suffer from CVD in China. The number of patients suffering from stroke, coronary heart disease, heart failure, pulmonary heart disease, atrial fibrillation, rheumatic heart disease, congenital heart disease, lower extremity artery disease and hypertension are 13.00 million, 11.39 million, 8.90 million, 5.00 million, 4.87 million, 2.50 million, 2.00 million, 45.30 million, and 245.00 million, respectively. Given that China is challenged by the dual pressures of population aging and steady rise in the prevalence of metabolic risk factors, the burden caused by CVD will continue to increase, which has set new requirements for CVD prevention and treatment and the allocation of medical resources in China. It is important to reduce the prevalence through primary prevention, increase the allocation of medical resources for CVD emergency and critical care, and provide rehabilitation services and secondary prevention to reduce the risk of recurrence, re-hospitalization and disability in CVD survivors. The number of people suffering from hypertension, dyslipidemia and diabetes in China has reached hundreds of millions. Since blood pressure, blood lipids, and blood glucose levels rise mostly insidiously, vascular disease or even serious events such as myocardial infarction and stroke often already occured at the time of detection in this population. Hence, more strategies and tasks should be taken to prevent risk factors such as hypertension, dyslipidemia, diabetes, obesity, and smoking, and more efforts should be made in the assessment of cardiovascular health status and the prevention, treatment, and research of early pathological changes.

ABSTRACT

Objective@#To investigate the recurrence of pulmonary tuberculosis patients 5 years after successful treatment in Hengyang City, Hunan Province, and to analyze its influencing factors.@*Methods@#The data of pulmonary tuberculosis patients who were successfully treated in Hengyang City in 2014 were collected through the tuberculosis management information system. The recurrence of pulmonary tuberculosis was defined as the endpoint, and all patients were observed for five consecutive years. Factors affecting recurrence of pulmonary tuberculosis were identified using Kaplan-Meier estimates, log-rank test and multivariable Cox proportional hazards regression analysis.@*Results@#A total of 6 626 patients with successfully treated pulmonary tuberculosis were included, including 4 961 males and 1 665 females, with a median age of 54 years. There were 536 cases with recurrence of pulmonary tuberculosis within five years, with a 8.09% cumulative proportion of recurrence and annual recurrence rate of 1.75 per 100 person-years. Multivariable Cox proportional hazards regression analysis showed that men ( HR=1.592, 95%CI: 1.256-2.019 ), age of 40 years and greater ( age of 40 to 59 years, HR=7.025, 95%CI: 1.739-28.377; age of 60 years and greater, HR=8.175, 95%CI: 2.024-33.023), farmers ( HR=1.379, 95%CI: 1.070-1.778 ), retreatment ( HR=6.398, 95%CI: 5.163-7.929 ) and positive etiology ( HR=1.255, 95%CI: 1.042-1.511 ) were risk factors for five-year recurrence of pulmonary tuberculosis among successfully-treated patients.@*Conclusions@#High recurrence rate is found among pulmonary tuberculosis patients within five years after successful treatment in Hengyang City. There is a high risk of pulmonary tuberculosis recurrence among male, etiologically positive, retreated farmers at ages of 40 years and older, and targeted interventions are recommended.

ABSTRACT

Abstract@#Tibet Plateau is one of the most prevalent areas of brick-tea type fluorosis in China. Effective management of brick-tea type fluorosis is a key point and a difficulty in endemic disease prevention and treatment in Tibet. Brick-tea type fluorosis is a human health concern that occurs under specific natural environments and social humanistic conditions in Tibet. Recently, the prevalence of brick-tea type fluorosis has been effectively reduced in the Tibet Plateau; however, there are still challenges for the overall brick-tea type fluorosis control, and the endemic status remains more severe than other regions endemic for brick-tea type fluorosis in China. Previous studies have shown that intake of high-fluoride brick-tea, dietary habits of drinking tea and specific natural environments are strongly associated with brick-tea type fluorosis in the Tibet Plateau. This review summarizes the advances in the epidemiological characteristics and influencing factors of brick-tea type fluorosis, so as to provide insights into the development of the long-term control strategy for brick-tea type fluorosis in the Tibet Plateau.

ABSTRACT

Objective@#To investigate the factors affecting the depressive symptoms among patients with colorectal cancer in Chengdu City, so as to provide insights into mental health improvement among colorectal cancer patients.@*Methods@#Patients with colorectal cancer were enrolled from three hospitals in Chengdu City using the convenient sampling method. The basic information, life styles and cancer diagnosis and therapy were collected, and the depressive symptoms were evaluated using the Zung Self-Rating Depression Scale ( SDS ). In addition, factors affecting the depressive symptoms were identified using the multivariable logistic regression analysis.@*Results@#A total of 384 patients with colorectal cancer were enrolled, including 247 men ( 64.32% ), and the subjects had a mean age of ( 57.47±11.39 ) years and a mean SDS score of 37.73±9.15. The detection rate of depressive symptoms was 15.36%. Multivariable logistic regression analysis showed that colorectal cancer patients with a history of radiotherapy had a high risk of developing depressive symptoms ( OR=0.468, 95%CI: 0.245-0.895 ), while patients with alcohol consumption ( OR=0.407, 95%CI: 0.172-0.963 ) and drinking tea ( OR=0.470, 95%CI: 0.244-0.904 ) had a low risk of developing depressive symptoms.@*Conclusions@#The detection of depressive symptoms is 15.36% among patients with colorectal cancer in Chengdu City, and a history of radiotherapy, alcohol consumption and drinking tea affect the development of depressive symptoms among patients with colorectal cancer.

ABSTRACT

@#AIM: To observe the effect of intravitreal injection of ranibizumab in the treatment of retinal vein occlusion macular edema(RVO-ME), and to apply Logistic regression analysis to screen the factors that affect the vision recovery of patients. <p>METHODS: A total of 106 patients(106 eyes)with retinal vein occlusion and macular edema who were treated in the ophthalmology clinic of our hospital from January 2018 to June 2019 were selected for the study. The clinical data of the patients were retrospectively analyzed and all were given 0.05mL ranibizumab. The monoclonal antibody was injected into the vitreous cavity of the affected eye, and the patients were followed up to observe the treatment effect. The patients were divided into good group and poor group according to the degree of vision recovery after 3mo of treatment, and the two-class Logistic regression analysis was used to screen the factors that affect the vision recovery of patients.<p>RESULTS: The Logarithm of the minimum angle of resolution(LogMAR)of the affected eye at 1d, 1wk, 1, and 3mo after treatment was significantly lower than before treatment, and the macular foveal retinal thickness(CRT)was significantly lower than before treatment. The difference was statistically significant(all P<0.05), and there was no significant difference in the average intraocular pressure of the affected eye before and after treatment(P>0.05). During and after the treatment, the patient did not develop complications such as endophthalmitis, increased intraocular pressure, lens opacity, and retinal detachment. Logistic regression analysis showed that there was no capillary perfusion in the foveal area of the macula and the center of the macula before treatment. Incomplete outer membrane and incomplete inner plexiform layer structure in the concave area are risk factors that affect the efficacy of ranibizumab injection. <p>CONCLUSION: Intravitreal injection of ranibizumab in the treatment of retinal vein occlusion combined with macular edema is effective and safe in the short term. It can effectively reduce macular edema of the affected eye, improve vision, and normal intraocular pressure after treatment, and it is easy to operate and effective. It has the advantages of repeating, little damage to tissues, no obvious adverse reactions, less pain for patients, economical and practical. In addition, the lack of capillary perfusion in the foveal area of the macula before treatment, the incomplete outer membrane of the foveal area, and the incomplete structure of the inner plexiform layer are risk factors that affect the efficacy of ranibizumab in the treatment of retinal vein occlusion and macular edema.

ABSTRACT

Objective To investigate the prevalence of thyroid nodules in women participating in physical examination in Beijing and analyze the influencing factors. Methods The data of physical examination (height, weight, blood pressure, blood glucose, etc.) and questionnaire survey (activity intensity, eating habits, etc.) of women in Beijing in 2016 were collected, and the influencing factors of thyroid nodules were analyzed by multivariate logistic regression. Results A total of 4 732 women were included in this study. The prevalence of thyroid nodules was 49.6%. Multivariate logistic regression analysis showed that compared with women aged 18-29 years, OR value was 1.769 (95% CI =1.489 ~ 2.102) for women aged 30 ~ 59 years, and OR value was 4.716 (95% CI = 3.577- 6.216) for women aged 60 years and over. Compared with the balanced diet, the OR value was1.237(95%CI=1.056-1.450)for vegetarian diet. Compared with the normal weight, the OR value was 1.331(95%CI=1.153-1.537)for the overweight. Compared with the healthy women, the OR value was 1.405 (95%CI=1.146-1.723)for hypertension, the OR value was 1.184(95%CI=1.040-1.347)for hyperlipidemia, and the OR value was 1.779(95%CI=1.178-2.687)for diabetes, while the OR value was 1.183(95%CI=1.018-1.376)for women with mammary gland nodules, and the OR value was 1.376(95%CI=1.201-1.575)for women with uterine leiomyoma. Compared with the education degree of high school, technical secondary school, technical school and below, the OR value was 0.648(95%CI=0.522-0.806)for college or undergraduate, and the OR value was 0.564(95%CI=0.440-0.723)for graduate students and above. Conclusion The prevalence of thyroid nodules in women in Beijing is at a high level. Age, vegetarian diet, overweight, hypertension, hyperlipidemia, diabetes, mammary gland nodules and uterine leiomyoma are risk factors for thyroid nodules. Education level is a protective factor for the prevalence of thyroid nodules.

ABSTRACT

Objective:To investigate the current status and influencing factors of outpatient institution choice for hypertensive patients with basic medical insurance in Beijing, for reference to promote the implementation of the hierarchical medical system and guiding hypertensive patients to seek healthcare in primary care.Methods:Based on data of hypertensive outpatients from the basic medical insurance database of Beijing from April 2019 to January 2020, we analyzed major demographic characteristics of hypertensive patients, the selection of outpatient institutions and its influencing factors. The chi square test was used for comparison between groups, and the multivariate logistic regression model was used to analyze influencing factors.Results:2.842 1 million outpatients with hypertension were enrolled. 39.03% of them chose primary healthcare institutions, 5.16% chose secondary healthcare institutions, and 17.34% chose tertiary healthcare institutions, while the rest 38.47% chose two or more types of healthcare institutions. Gender, age, type of medical insurance, place of residence, utilization of Chinese herbal drugs, utilization of Chinese patent drugs, polypharmacy, needs of outpatient tests and examinations were the influencing factors for their selection of primary healthcare institutions for hypertensive outpatients under Beijing basic medical insurance.Conclusions:At present, the primary institutions have become the first choice for the majority of hypertensive patients with medical insurance in Beijing, but there are still many influencing factors on their choice of institutions. In the future, we should optimize the allocation of medical resources, promote the reform of medical insurance payment methods, strengthen the construction of primary medical institutions, expand the coverage of contracted services of family doctors, and reasonably guide the patients to seek healthcare in primary healthcare institutions.

ABSTRACT

Objective:To understand the medical cost ratio and its influencing factors of breast cancer chemotherapy and molecular targeted therapy in a hospital, for reference for reasonable control of medical cost.Methods:The first page data of all breast cancer chemotherapy and molecular targeted therapy cases in a tertiary hospital from January to June 2021 were selected to extract the data of age, hospitalization expenses, hospitalization time, complications or complications. The influence of each index on the distribution of medical expense ratio was analyzed by single factor analysis and chi-square test was used for comparison between groups.Results:A total of 3 109 cases of chemotherapy and molecular targeted therapy for breast malignant tumors were included, of which very low-rate, low-rate, high-rate and very high-rate accounted for 7.04%(219 cases), 58.32%(1 813 cases), 30.81%(958 cases) and 3.83%(119 cases) respectively. In addition to the way of admission, there were significant differences in the distribution of medical cost rates under different ages, time consumption index, complications or concomitants, admission departments, treatment methods and medical insurance types( P<0.01). Conclusions:There were many factors affecting the medical cost rate of breast malignant tumors. Relevant departments should expand the pilot scope of DRG payment, promote the multi-disciplinary diagnosis and treatment mode of tumors, refine the DRG grouping scheme, to provide standardized and homogeneous diagnosis and treatment services for tumor patients, and reasonably control the excessive growth of medical costs.
