ABSTRACT

Abstract Marjolin's ulcer (MU) is a rare condition defined as a malignant skin tumor arising from chronic wounds and inflammation. The most common histological findings in MUs are squamous cell carcinomas (SCC) (or spinocellular carcinomas [SPC]), such as basal cell carcinomas (BCC) and malignant melanomas (MM). The aim of this study is to report the case of a patient with sequelae of leprosy who presented malignant transformation in a long-standing ulcer on the right leg, thereby contributing to understanding of the progression and significance of early diagnosis of MU. The MU was diagnosed by incisional biopsy of the lesion and upon obtaining a positive result the patient was referred to an oncology service. Treatment of MU is multidisciplinary and surgical excision is the first therapeutic option. Proper management and surveillance of chronic ulcers by the healthcare team are necessary for early recognition of MU.

ABSTRACT

Malignant tumors have surpassed stroke and coronary heart disease to become the main cause of death before the age of 70 in the world. The incidence of malignant tumors is increasing year by year, which seriously affects the living standards and social and economic development of Chinese people. Surgery, radiotherapy, chemotherapy, targeted therapy, and immunotherapy are the main means of Western medicine to intervene in tumors, which are of great significance for prolonging the survival time of patients. However, there are many disadvantages such as adverse reactions and drug resistance. Traditional Chinese medicine has become a research hotspot in recent years because of its definite curative effect and wide application in tumor treatment. Xiao Chaihutang is derived from the Treatise on Febrile and Miscellaneous Diseases. It is composed of Bupleuri Radix, Scutellariae Radix, Ginseng Radix, Pinelliae Rhizoma, Glycyrrhizae Preparata Radix, Ginger, and Jujube. It has the effects of reconciling Shaoyang, reconciling cold and heat Yin and Yang, soothing the liver, and relieving depression. This prescription and the prescriptions based on it are widely used in various stages of tumors. This study summarized the Chinese and foreign research of Xiao Chaihutang in the field of tumors in the past 10 years and explored the role and mechanism of Xiao Chaihutang in blocking precancerous lesions, controlling the development of malignant tumors, reducing the occurrence of adverse reactions after intervention, and alleviating common complications, with a view to expanding the clinical medication ideas.

ABSTRACT

Interventional therapy for hepatic malignant tumours primarily includes cardiovascular system surgery,diges-tive system surgery, and various diagnostic and therapeutic procedures.Referring to the"International Classification of Diseases,Ninth Edition,Clinical Modification,ICD-9-CM-3"(2011 revision),the relevant surgical procedure codes on the front page of the case are contained mainly in the 9th chapter for cardiovascular system surgery, the 11th chapter for digestive system surgery, and the 18th chapter for various diagnostic and therapeutic procedures and other related chapters on diagnostic and therapeutic procedures. It has become a challenge for the history coders to complete the first page of interventional surgery cases by reviewing the surgery records, extracting the surgical steps, and then identifying the appropriate surgery codes and their sequencing.Accord-ing to the current classification of interventional therapy for hepatic malignant tumors,it is suggested to conduct the search and re-trieval using the keywords like"arteriography""embolization""perfusion""implantation""destruction"and"ablation".

ABSTRACT

Fertility preservation technology provides an effective method of protecting fertility resources for young patients with malignant tumors, allowing them to offspring after their fertility is impaired. However, the development of this technology has caused many social and ethical controversies. From the perspective of ethics, this paper discussed the ethical issues faced by young female patients in the implementation of fertility preservation, including whether it is necessary to preserve fertility, the ownership of the preserved fertility resources and the fair and equitable distribution of health resources involved in its implementation process, and identifies these issues and controversies from ethical view. In order to eliminate public doubts and misunderstandings about the technology of fertility preservation, ethical principles of benefit and non-harm, informed consent, prudent application, and ethical supervision have to be followed in the process of providing fertility preservation services, so as to promote the further development and application of fertility preservation technology.

ABSTRACT

Ubiquitination is a crucial post-translational modification process that can degrade proteins within cells and plays a vital role in maintaining protein homeostasis and abundance. Deubiquitinating enzymes (DUBs) are important proteases in the ubiquitin system. They reverse the ubiquitination process by cleaving protein chains and recycling ubiquitin molecules to regulate protein stability. Abnormal deubiquitinating enzyme activity is related to the occurrence and development of many malignant tumors. JOSD2, a DUB, is a member of the Machado-Joseph disease protein domain protease (MJD) family and characterized by a single highly conserved catalytic Josephin domain. Increasing studies have revealed a connection between JOSD2 and malignant tumors. This article elaborates on the current research status of DUBs, particularly JOSD2, in malignant tumors. Results suggest that JOSD2 is a potential target for the treatment of malignant tumors.

ABSTRACT

As an important concept in Chinese medicine theory, "qi sinking" is the inheritance and extension of the thought core of sinking of qi in whole body. This article explored the concept of sinking of pectoral qi, center qi, and kidney qi in the theory of qi sinking, and believed that sinking of pectoral qi, stagnation and sinking of center qi, deficiency and sinking of kidney qi were the core pathogenesis of postoperative injury in malignant tumours. Anchored to the method of reinforcing healthy qi and lifting the sunken, this article recommended to identify pattern and treat by guiding supplement and lifting the sunken. For lung gold impairment, heart yang depletion, and pectoral qi sinking, the treatment is to warm and supplement heart and lung, lift pectoral qi, and restore the respiratory function by modified Shengxian Decoction (升陷汤) plus Guizhi Decoction (桂枝汤); for spleen depletion and pathways blockage, liver failing to act freely, and center qi stagnation and sinking, the treatment is to warm and supplement center qi, raise yang and lift the sunken, and restore the digestive function by modified Buzhong Yiqi Decoction (补中益气汤); for source exhausted and essence deficiency, liver qi hiding, and kidney qi deficiency to inward invasion, the treatment is to nourish the kidney and astringe the liver, consolidate the original qi and lift qi, improve the pelvic floor dysfunction, and protect the kidney function by modified Liuwei Dihuang Pill (六味地黄丸) plus Shengma Chaihu Decoction (升麻柴胡汤). Modification need base on different disease patterns and stages, and new ideas for postoperative traditional Chinese medicine treatment and rehabilitation of malignant tumours were provided.

ABSTRACT

It is believed that qi changes throughout the whole process of malignant tumors. Tumour formation is induced by qi constraint in the early stage, qi sinking is the core disease mechanism after tumour surgery, qi counterflow is an important disease mechanism for nausea and vomiting related symptoms in chemotherapy stage, and qi deficiency is the main pathology in recurrence and metastasis stage. It is proposed that the therapeutic principle of “regulating qi with changes” discussed in Miraculous Pivot-Wei Qi Disorder (《灵枢·卫气失常》) should be implemented in traditional Chinese medicine (TCM) pattern identification and treatment of malignant tumours, modifying the treatment with changes, and regulating qi with corresponding methods: for qi constraint, it should soothe to eliminate the tumour by the method of rectifying qi to resolve constraint, with treatment as Shugan Jianpi Formula (疏肝健脾方); for qi sinking, it should raise to lift up postoperative viscera by the method of boosting qi to raise sinking, with treatment as Shengxian Decoction (升陷汤) from Records of Chinese Medicine with Reference to Western Medicine (《医学衷中参西录》); for qi counterflow, the method of moving qi and direct counterflow downward will be used to dissipate the accumulated medicinal toxicity, and patients with retention of medicinal toxicity in the middle jiao and ascending counterflow of stomach qi treated by Xuanfu Daizhe Decoction (旋覆代赭汤) from On Cold Damage (《伤寒论》), patients with middle yang deficiency and turbid ying failing to descend treated by Wuzhuyu Decoction (吴茱萸汤) from On Cold Damage (《伤寒论》) ; for qi deficiency, it should tonify to prevent the tumor toxin from spreading by the method of boosting qi to resolve toxins, treated by Fuzheng Jiedu Formula (扶正解毒方).

ABSTRACT

Objective To evaluate the risk of cardiac adverse events in patients with malignant tumors after chemotherapy by using a combination of acoustic cardiography and blood indices. Methods A total of 171 patients with malignant tumor who received chemotherapy were included. They were divided into cardiac adverse event group and non-cardiac adverse event group in accordance with whether cardiac adverse events occurred after chemotherapy. The general data, blood indices before chemotherapy, and acoustic cardiography-related indices in the early stage (1-3 cycles) of chemotherapy of the two groups were analyzed. The possible influencing factors were determined by binary logistic regression analysis, and the nomogram was drawn. The receiver operating characteristic (ROC) curve was used to evaluate the prediction ability of the nomogram. Results Cardiac adverse events occurred in 44 of 171 patients with malignant tumors after chemotherapy, and the incidence of cardiac adverse events was 25.73%. Binary logistic regression results showed that age, red blood cell distribution width (RDW) before chemotherapy, activated partial thromboplastin time (APTT), and electromechanical activation time (EMAT) at the early stage of chemotherapy were independent predictors of cardiac adverse events in chemotherapy patients. The area under the ROC curve of the nomogram was 0.768 (95%CI: 0.693-0.843, P<0.001). Conclusion A nomogram based on age, pre-chemotherapy RDW, APTT, and EMAT at the early stage of chemotherapy is useful for early assessment of the risk of cardiac adverse events in chemotherapy patients.

ABSTRACT

Apolipoprotein E (apoE) is a critical molecule in lipid metabolism, which also plays important roles in the occurrence and development of several kinds of cancers by regulating processes including cell proliferation, energy metabolism, oxidative stress and innate immune, etc., and shows influence in patients’ response to treatment. Therefore, apoE has become a potential biomarker and treatment target for cancer. Further research of apoE will help us build deep and systematic understanding of etiology of cancer to promote the prevention and to develop new therapeutic strategies for cancer. In this review, we introduced the properties of apoE from the views of biophysics, biochemistry, molecular biology, evolution and epidemiology, in which we demonstrated the similarities and differences among the structures of 3 subtypes of apoE; we also recapitulated the role of apoE in the genesis and development of cancers in main types of malignancies including gastric cancer, colorectal cancer, hepatobiliary cancer, melanoma, pancreatic cancer, etc.; we summarized the relationship between apoE and the hallmarks of cancer, highlighting the position of apoE in immune system and its critical role for understanding the different nature of immunological background of cancers originated from different organs, and discussed its potential value for application as tumor biomarkers and therapeutic targets by demonstrating the structures of its subtypes. We further discussed the possibility of transferring the drug designing strategy of “structure corrector” from neurology to oncology.

ABSTRACT

Objective To investigate the relationship between correcting abnormal glucose metabolism and mortality risk of malignant tumors of digestive system. Methods A retrospective cohort study was conducted. 1308 patients with abnormal glucose metabolism in our hospital from January 2019 were divided into exposed group (n=777) and non-exposed group (n=531) according to the presence or absence of glucose metabolism correction therapy. The patients were retrospectively followed up until December 2022. The incidence of digestive system tumors and the influencing factors of tumors were compared between the two groups. Results There were 31 patients with digestive system tumor and 9 patients died. The incidence of digestive system tumors was lower in the exposed group (3/777) than in the non-exposed group (28/531). The mortality rate in the exposed group (1/777) was lower than that in the non-exposed group (8/531). Cox regression model analysis showed that correcting abnormal glucose metabolism was a protective factor for the risk of death from malignant tumors of the digestive system in patients (HR value1, P<0.05). Conclusion Correcting abnormal glucose metabolism is of positive significance in reducing the risk of death from malignant tumors of digestive system. Patients with increased FBG, abnormal lipid metabolism, increased pulse pressure difference, family history of malignant tumors and alcohol consumption should pay special attention to correct abnormal glucose metabolism in time.

ABSTRACT

@#Abstract: Olfactory receptors (ORs) are transmembrane proteins mainly distributed in olfactory sensory neurons of the nasal epithelium, mediating the transmission of real-time sensory signals to the brain to produce smell. Recent studies have reported that ORs can also be expressed in tissues or organs outside the nasal cavity, and are closely related to a variety of biological processes, such as sperm chemotaxis, wound healing, glycolipid metabolism and intestinal secretion. In addition, ORs are closely related to a variety of malignant tumors such as prostate cancer, breast cancer and colorectal cancer, and may affect the occurrence and development of tumors by regulating cell proliferation, apoptosis, migration and invasion. This review provides an overview of the effects of ectopic ORs on the function of various human tissues and organs and assesses their potential value as drug targets for the treatment of human diseases.

ABSTRACT

Malignant tumor is still one of the malignant diseases with high morbidity and mortality in the world. Its occurrence and development are influenced by various factors. Exosomes are nanoscale secretory vesicles that play an important role in the occurrence and development of malignant tumors, and have intercellular communication functions. The mechanism of action of traditional Chinese medicine in prevention and treatment of tumors is not yet comprehensive enough. This article discusses the relationship between exosomes and tumor development, relapse, metastasis and drug resistance, and the application of exosomes in the treatment of malignant tumors by traditional Chinese medicine, to provide reference for finding new breakthroughs in the treatment of malignant tumors.
