ABSTRACT

Objective@#To explore the long-term effects of radiation injury and to provide scientific basis for the evaluation of the effects of ionizing radiation injury by carrying out medical follow-up of patients involved in the " 5.7" radiative source accident in Nanjing in 2014.@*Methods@#Through interviewing and investigating, we inquired about the new disease history of the exposed patients from rehabilitation treatment to medical follow-up peroid. Physical and laboratory examinations were carried out. According to relevant standards, physiological and biochemical indexes such as hematopoietic system, immune system, endocrine system, ophthalmology, circulatory system, digestive system, urinary system and bone mineral density were systematically evaluated, with the long-term effects being evaluated.@*Results@#The patient′s vital signs were good without new diseases. The indexes of hematopoietic system, immune system and endocrine system tended to be normal, the circulatory system, digestive system and urinary system showed degenerative changes, the ophthalmic examination showed visual acuity continue to decline, and bone mineral density examination indicated low bone mass.@*Conclusions@#Physiological and biochemical indicators of the patients gradually returned to normal without obvious symptoms of radiation damage. Further medical follow-up observation still needs to continue.

ABSTRACT

Objective To investigate the late effects induced by ionizing radiation and the rehabilitation treatment of local radiation injury by medical follow-up of the patient exposed to192 Ir at 5.7 accident in Nanjing,and to provide more experience for the treatments in the medical emergency of nuclear or radiological accident.Methods According to the history inquiry and physical examination of the patient in detail and the record of clinical symptoms and signs,the changes of the blood system,immune system,reproductive system,eyes,nervous system were systematically evaluated.The effects of rehabilitation treatment for the patient with lower limb dysfunction were also assessed.Results After the medical treatments of the patient,the hematopoietic immune system was restored,but the bone marrow aspiration still showed low bone marrow hyperplasia in right ilium.Meanwhile,the level of sex hormones was within the normal range,but semen examination showed sperm motility was 0.The radiation damage also occurred in the eye lens,retina and fundus.Howevcr,the psychological evaluation showed that the patient was stable and the right lower limb skin wound healing was well except for dysfunction and pain in some extent,which was relieved after the rehabilitation treatment.Conclusions The physiological function of the exposed victim with mild bone marrow type acute radiation sickness could be completely or partially restored after the clinical treatment in the early stage.

ABSTRACT

Objective To make a summary of radiation-induced changes in ocular lens and retinal lesions based on dynamic follow-up of a patient exposed to a radiation exposure from 192 Ir source accident,in order to provide the information and experience for the clinical diagnosis and treatment of such injury in emergency of a nuclear or radiation accident.Methods The examination methods included eye lens and visual acuity,slit-lamp examination,fundus examination,automated perimetry,optical coherence tomography (OCT) examination,fundus photography,and fundus fluorescein angiography (FFA).The patient was followed-up for 20 months.Comparative analyses were made of the follow-up results.Results One month after 192Ir radiation exposure,the visual acuities in both eyes were 1.0.Mild age-related opacities were observed in peripheral cortex,but the fundus looked normal.Twenty months later,the patient was followed-up.The visual acuities reduced to about 0.6,evident powder-and punctuate-like opacities could be observed in the cortex of the right lens,and mild turbid foci in the posterior polar and capsule.Multiple scotomata appeared in the superior temporal and inferior nasal in the right eye.The macular fovea thickness was not changed significantly in the right eye.Examination with fundus photography demonstrated mild optical nerve atrophy,drusens,and pale optic discs,while FFA examination showed punctuate or flocculent leakages in both eyes.Conclusions The lens opacity could be induced or accelerated by radiation exposure.In addition to the posterior capsule and pole cataracts,punctuate opacity within cortical layer may also occur.It also suggests that cataract and fundus injuries may occur later during long-term follow-up,even though the eyes may look normal during early examination after the patient exposed to the radiation.

ABSTRACT

Objective To observe late effect of 60Co radiation accident on eye lens in the victims.Methods Medical observations of eye lens were performed on ten victims accidentally exposed to 60Co sources in four radiation accidents that have occurred from 1986 to 2000 in Henan Province.Pathological changes of the eye lenses were examined by using slit lamp microscopy after mydriasis with compound tropicamide.Results Of these ten victims, Liang in a radiation accident in Kaifeng (in 1986), Mei in Xinxiang (in 1999) and Xu in Xuchang (in 2000) all had typical radiation-induced cataract 2, 3 and 6 years after irradiation, respectively.Follow-up observation of the lens showed the punctate and/or granular opacities present in the eye posterior subcapsular of Yan in Kaifeng accident, Jie in Zhengzhou (1987), and Tian, Yong and Yi in Xinxiang accident, featuring the early changes of radiation-induced cataract, but the posterior subcapsular opacities were not observed in Wang and Min in Xinxiang accident.Conclusions Focus should be on the eye lens as the target organ of radiation exposure in long-term follow-up of victims accidentally exposed to radiation source.Severity of the lens opacity induced by ionizing radiation is closely associated with radiation doses.

ABSTRACT

Introducción. Objetivo: evaluar el cumplimiento de la "Guía de salud para personas con síndrome de Down" de la Asociación Americana de Pediatría de 2001 en pacientes del Departamento de Genética del Hospital Universitario "Dr. José Eleuterio González . Métodos. Estudio retrospectivo, descriptivo, observacional y transversal, en el cual se revisaron los expedientes de pacientes con síndrome de Down que acudieron a consulta al Departamento de Genética entre abril de 2004 y abril de 2006. Resultados. Se identificaron 38 casos. El límite de edad materna fue de 17-43 años. En 74% de los pacientes se realizó cariotipo y sólo 61% recibieron asesoría genética. Se identificó cardiopatía congénita por ecocardiograma en 26%, pero en 56% no se realizaron estudios a pesar de contar con datos clínicos sugestivos. El número máximo de consultas fue de 5; sin embargo, 44% de los pacientes no tuvo seguimiento. Conclusiones. Se identificó una falla importante en la adherencia al seguimiento y a la realización de estudios encaminados a descartar las complicaciones asociadas al síndrome.


Introduction. Objective: To evaluate the application of the American Academy of Pediatrics' "Guidelines for health supervision for children with Down syndrome" in the Department of Genetics at the Hospital Universitario "Dr. José Eleuterio González". Methods. We analyzed clinical files from all Down's syndrome patients who attended our Department of Genetics from April 2004 to April 2006. Results. Thirty-eight cases were identified. The rank of mothers' age at date of infant birth was 17-43 years old; 74% of the patients had a karyotype performed, but genetic counseling was given only to 61% of families. An echocardiogram was performed; congenital heart disease was detected in 26%, but 56% did not attend the cardiologist consultation even when some of them had clinical findings suggesting heart disease. At the end of the study, the highest number of consultations was 5; however, 44% of the patients had no follow-up. Conclusions. We found a significant lack of commitment to follow-up patients by care givers, and a delay to accomplish clinical studies to avoid the well-known complications associated to this syndrome.
