ABSTRACT

Background: Epidural analgesia is the most effective method of pain relief during labour and the only method that provides complete analgesia without maternal or foetal adverse effects. This study aimed to assess the mode of delivery outcomes of labour in women who had received effective epidural analgesia. Methods: After ethical approval, this quasi-experimental study was carried out in Dhaka Medical College Hospital for one year, from July 2021 to June 2022. A total of 100 admitted pregnant women (37 weeks to 41 weeks) in the labour ward were included in the study according to the inclusion and exclusion criteria. Among them, 50 women were in group A (with epidural analgesia), and the other 50 were in group B (without epidural analgesia). A detailed history and thorough clinical examination were carried out on each patient. Data were collected in separate case-record forms and analyzed using SPSS 24.Results: Mean age, gestational age, parity, and body mass index were statistically similar in both group A and B. Normal vaginal delivery rate (86% vs. 90%), instrumental delivery rate (10% vs. 6%), and caesarean section rate (4% vs. 4%) were not significantly different in both groups. Besides, both group A and B had statistically similar APGAR scores at 1st (8.66±0.87 and 8.80±0.83 respectively, p=0.414) and 5th min (9.66±0.68 and 9.74±0.59 respectively, p=0.537).Conclusions: Epidural labour analgesia can be safely recommended as a method of labour analgesia, provided the prerequisites are fulfilled. It has no significant adverse effect on the mode of delivery.

ABSTRACT

Background: Mode of delivery is determined by a variety of feto-maternal factors like fetal presentation, maternal age, parity as well as environmental factors such as place of residence, quality of ANC care etc. Aim was to assess if neonatal birth wight (NBW) affected the mode of delivery at a tertiary level maternity hospital in urban Mumbai. Methods: Ours was an observational study having cross sectional design, utilizing data retrieved from hospital records, conducted at Cama and Albless Hospital in Mumbai over 3 months- July 2023 -September 2023. Data regarding maternal parity, age, mode of delivery and neonatal birth weight was compiled in Microsoft Excel 2017 from hospital records after taking Ethics Committee permission for the same. Results: In n=104 live viable term singleton pregnancies observed over 3 months LSCS rate of 25.96% was observed, higher Neonatal birth weight (NBW) was associated with caesarean delivery. 89% of the underweight neonates (<2.5kg) delivered vaginally while 100% of the macrocosmic neonates (>4kg) were delivered by LSCS. Maternal age correlated positively with neonatal weight and parity. Neonatal sex was not associated with mode of delivery or NBW.Conclusions: Mode of delivery is affected by NBW, but not by maternal age or neonatal sex. Mode of delivery in first pregnancy significantly determines mode of delivery in subsequent pregnancies.

ABSTRACT

OBJECTIVE To reduce dispensing errors in pharmacy intravenous admixture service (PIVAS) of children’s hospitals. METHODS The risk of dispensing procedures in our PIVAS was identified by applying failure mode and effect analysis (FMEA) model. Potential failure modes that might lead to dispensing errors in each link were determined, and failure causes were analyzed. The severity, incidence and detection degree of potential failure modes were quantitatively scored, and their risk priority number (RPN) was calculated to screen failure modes that needed to be improved in priority; the corresponding improvement measures were developed by 6S management method from six aspects, namely, finishing (seiri), rectifying (seiton), sweeping (seiso), sanitation (seiketsu), literacy (shitsuke) and safety. The effect of intervention before and after rectification was evaluated. RESULTS Based on the RPN, 32 potential failure modes were selected, of which a total of 18 critical failure modes that needed to be improved in priority. After implementing corresponding measures according to 6S management method, the RPN of 18 critical failure modes decreased. The total RPN decreased from 497 to 142 with a decrease rate of 71.43%. The error rates of 15 critical failure modes were significantly lower than before implementation (P<0.05). CONCLUSIONS Applying FMEA model and 6S management method to the risk control of all aspects of PIVAS workflow can effectively reduce the risk of PIVAS dispensing errors and ensure the safety of children’s intravenous medication.

ABSTRACT

BACKGROUND:Critical energy release rate is a global fracture parameter that could be measured during the failing process, and its value may change under different failure modes even in the same structure. OBJECTIVE:To propose an approach to predict the critical energy release rate in the femoral cortical bone structure under different failure modes. METHODS:Three-point bending and axial compression experiments and the corresponding fracture simulations were performed on the rat femoral cortical bone structures.Different critical energy release rates were repeatedly assigned to the models to perform fracture simulation, and the predicted load-displacement curves in each simulation were compared with the experimental data to back-calculate the critical energy release rate. The successful fit was that the differences in the fracture parameters between the predicted and experimental results were less than 5%. RESULTS AND CONCLUSION:(1)The results showed that the cortical bone structure occurred tensile open failure under three-point bending load, and the predicted critical energy release rate was 0.16 N/mm.(2)The same cortical bone structure occurred shear open failure under axial compression load, and the predicted critical energy release rate was 0.12 N/mm,which indicates that the critical energy release rate of the same cortical bone structure under different failure modes was different.(3)A comprehensive analysis from the perspectives of material mechanical properties and damage mechanism was conducted to reveal the reasons for the differences in the critical energy release rate in the cortical bone structure under different failure modes, which provided a theoretical basis for the measurement of the energy release rate and the accurate fracture simulation.

ABSTRACT

Objective To investigate the effect of early warning score system combined with(situation, background, ssessment, recommendation,SBAR)communication model in early warning of high-risk neonates, therefore to provide an effective communication method for an effective communication method to assess the changes of condition in neonates. Methods A before-after study model was adopted in the study.A total of 270 high-risk neonates admitted to the ward of the Department of Neonatology in a tertiary hospital between August and September 2022 were selected as research subjects. The high-risk neonates admitted in hospital in August were assigned in a control group, and those admitted in September were assigned in an trial group, with 135 neonates per group.Routine care was carried out in the control group, while early warning scoring combined with SBAR communication model were applied in the trial group on top of the cares offered to the control group. The occurrence of early warning events, concordance rate of nurse warning event and doctor handling events, and the satisfaction rate of doctors with the nursing performance were compared between the two groups. Results A total of 63.6%of early warning events were triggered by nurses in the control group, while it was 92.6%in the trial group, with a statistically significant difference between the groups(χ2=16.622,P<0.001).The consistency of handling of early warning events between the nurses and doctors in the trial group(Kappa coefficient=0.926)was higher than that in the control group(Kappa coefficient=0.641).The satisfaction rates of the doctors with the nurses about specialist knowledge, ability in emergency events, mastery of disease, timely observation of disease progress, collaboration between doctors and nurses, working enthusiasm, communication capability and the psychological quality in the trial group were all significantly higher than those in the control group[80.0%-95.0%vs.30.0%-55.0%,all P<0.01].Conclusions The Early Warning Score system combined with SBAR communication model can help nurses to accurately evaluate the changes of disease in neonates, complete the communication with doctors timely and effectively. It improves the observation, communication and handling capability among the nurses as well as the satisfaction rate of doctors with nursing work.
