ABSTRACT

El trauma de alta energía se define como lesiones orgánicas graves resultantes de eventos que generan una gran cantidad de energía cinética, eléctrica o térmica. Constituye una importante preocupación de salud pública, representando el 10% de la mortalidad mundial. El objetivo de este artículo es describir la epidemiología del trauma de alta energía en Chile. Específicamente, se busca comparar la tasa de mortalidad por 100 000 habitantes entre los países miembros de la Organización Mundial de la Salud (OMS), realizar un análisis descriptivo de las notificaciones por Garantías Explícitas en Salud (GES) del problema de salud "politraumatizado", y analizar la tendencia de la tasa de fallecidos por causa externa en Chile. El presente estudio tiene un diseño ecológico, utilizando tres bases de datos de acceso abierto. Primero, se utilizó la base de datos de la OMS sobre fallecidos por accidentes automovilísticos en 2019. Luego, se consultó la base de datos del programa Garantías Explícitas en Salud para el problema "politraumatizado" entre los años 2018 y 2022. Finalmente, se utilizó la base de datos del Departamento de Estadísticas de Salud de Chile sobre causas de muerte entre 1997 y 2020. En 2019, Chile ocupó una posición intermedia en cuanto a la tasa de mortalidad por 100 000 habitantes debido a accidentes de tráfico. Las notificaciones el programa Garantías Explícitas en Salud por politraumatismo fueron predominantemente en hombres de entre 20 y 40 años, afiliados al sistema de salud pública. Por este motivo, el foco principal de prevención debe centrarse en este grupo. La mortalidad por accidentes mostró una tendencia decreciente, identificándose cambios estructurales significativos en los años 2000 y 2007.


High-energy trauma is defined as severe organic injuries resulting from events that generate a large amount of kinetic, electrical, or thermal energy. It represents a significant public health concern, accounting for 10% of global mortality. This article aims to describe the epidemiology of high-energy trauma in Chile. Specifically, it seeks to compare the mortality rate per 100 000 inhabitants among member countries of the World Health Organization (WHO), provide a descriptive analysis of notifications under the Explicit Health Guarantees (GES) for the health issue of polytraumatized patients, and analyze the trend in the mortality rate due to external causes in Chile. This study employs an ecological design using three open-access databases. First, the WHO database on deaths from traffic accidents in 2019 was used. Then, the GES database was consulted for the "Polytraumatized" issue between 2018 and 2022. Finally, the Chilean Department of Health Statistics database on causes of death between 1997 and 2020 was utilized. In 2019, Chile ranked in the middle regarding the mortality rate per 100 000 inhabitants due to traffic accidents. GES notifications for polytrauma predominantly involved men aged 20 to 40 years and those affiliated with the public health system, highlighting a primary focus for prevention efforts. Mortality from accidents showed a decreasing trend, with significant structural changes identified in 2000 and 2007.

Objective:To explore the risk factors of sepsis in patients with multiple trauma and construct a nomogram prediction model.Methods:The data of patients with multiple injuries admitted to the emergency intensive care unit (EICU) of the General Hospital of Ningxia Medical University from January 2021 to April 2022 were respectively collected. Inclusion criteria: (1) meet the diagnostic criteria for multiple injuries; (2) the time from injury to admission ≤ 24 hours; (3) age>18 years old; (4) all examination or rescue measures were approved by the patient or the patient's family; (5) the patient's clinical data were complete. The patients were divided into sepsis group and non-sepsis group according to the definition of Sepsis 3.0 at the 28-day of EICU hospitalization. The receiver operating characteristic curve was drawn. Logistic regression analysis was applied to determine the independent predictors for sepsis, and the nomogram was constructed.Results:A total of 291 patients were included, including 102 in the sepsis group and 189 in the non-sepsis group. Multivariate logistic analysis revealed that age, acute physiology and chronic health status score (APACHE) Ⅱ, Glasgow Coma Scale (GCS), injury severity score (ISS), sequential organ failure assessment (SOFA) within 24 hours after admission, blood transfusion frequency, the application of norepinephrine, mechanical ventilation, pathogenic culture results, and history of diabetes were independent factors influencing the occurrence of sepsis. A nomogram model was constructed by combining these variables (AUC=0.913, 95% CI: 0.847-0.942), and the model had a good fitting calibration curve. Conclusions:The nomogram constructed by age, APACHE-Ⅱ, GCS score, SOFA score, ISS score, number of blood transfusions, mechanical ventilation, norepinephrine drug use, pathogenic culture and diabetes has a good predictive value for sepsis in patients with multiple trauma in the later stage, which is worth promoting.

ABSTRACT BACKGROUND: Hyperchloremia is often encountered due to the frequent administration of intravenous fluids in critically ill patients with conditions such as shock or hypotension in the pediatric intensive care unit, and high serum levels of chloride are associated with poor clinical outcomes. OBJECTIVES: This study aimed to determine the association between hyperchloremia and in-hospital mortality in pediatric patients with major trauma. DESIGN AND SETTING: This retrospective cohort study was conducted at a tertiary university hospital in Turkey. METHODS: Data were collected between March 2020 and April 2022. Patients aged 1 month to 18 years with major trauma who received intravenous fluids with a concentration > 0.9% sodium chloride were enrolled. Hyperchloremia was defined as a serum chloride level > 110 mmol/L. Clinical and laboratory data were compared between the survivors and nonsurvivors. RESULTS: The mortality rate was 23% (n = 20). The incidence of hyperchloremia was significantly higher in nonsurvivors than in survivors (P = 0.05). In multivariate logistic analysis, hyperchloremia at 48 h was found to be an independent risk factor for mortality in pediatric patients with major trauma. CONCLUSIONS: In pediatric patients with major trauma, hyperchloremia at 48-h postadmission was associated with 28-day mortality. This parameter might be a beneficial prognostic indicator.

Objetivo: Sistematizar a assistência de enfermagem à luz da teoria de Callista-Roy no cuidado ao paciente politraumatizado. Método:Trata-se de um estudo do tipo qualitativo de abordagem descritiva e exploratória na modalidade de caso clínico, realizado por estudantes de Enfermagem da Universidade Estadual de Feirade Santana no Hospital Geral Clériston Andrade na Clínica Ortopédica. A prática ocorreu em setembro de 2023, onde foram coletados dados e prestados cuidados à paciente. Resultados:Foi aplicada a Sistematização de Enfermagem, sob perspectiva da teoria deCallista-Roy, onde a coleta de dados foi realizada por meio do prontuário e contato direto com a paciente. Segundo a teoria, foram identificados os modos de adaptação físico-fisiológico, identidade de autoconceito, interdependência e desempenho de papel. Por fim, com base nas informações colhidas, foram realizados o planejamento, implementação e avaliação das condutas adotadas. Considerações finais:. Portanto, através da aplicação da sistematização de Enfermagem e do Modelo de adaptação é possível observara importância do exercício profissional da Enfermagem no processo de cuidado holístico, a fim de garantir as melhores condições de recuperação que perpassam o físico, como também na fundamentação teórico-prática do trabalho de enfermagem.


Objective: Systematize nursing assistance based on Callista-Roy's theory in the care of polytraumatized patients. Method:This is a qualitative study with a descriptive and exploratory approach in the form of a clinical case, carried out by nursing students from the State University of Feira de Santana at Clériston Andrade General Hospital in the Orthopedic Clinic. The practice took place in September 2023, where data were collected, and care was provided to the patient. Results:The Nursing Process was applied from the perspective of Callista-Roy's theory, with data collection performed through medical records and direct contact with the patient. According to the theory, modes of physical-physiological adaptation, self-concept identity, interdependence, and role performance were identified. Finally, based on the gathered information, planning, implementation, and evaluation of the adopted measures were carried out. Final considerations: Therefore, through the application of the nursing process and the Adaptation Model, it is possible to observe the importance of the nursing profession in the holistic care process, aiming to ensure the best conditions for recovery that go beyond the physical, as well as in the theoretical-practical foundation of nursing work.

Método:Se trata de un estudio cualitativo con enfoque descriptivo y exploratorio en forma de caso clínico, realizado por estudiantes de enfermería de la Universidad Estatal de Feira de Santana en el Hospital General Clériston Andrade en la Clínica Ortopédica. La práctica tuvo lugar en septiembre de 2023, donde se recopilaron datos y se brindó atención a la paciente. Resultados:Se aplicó el Proceso de Enfermería desde la perspectiva de la teoría de Callista-Roy, con la recopilación de datos realizada a través de expedientes médicos y contacto directo con la paciente. Según la teoría, se identificaron modos de adaptación físico-fisiológica, identidad del autoconcepto, interdependencia y desempeño del rol. Finalmente, con base en la información recopilada, se llevaron a cabo la planificación, implementación y evaluación de las medidas adoptadas. Consideraciones finales: Por lo tanto, mediante la aplicación del proceso de enfermería Y el Modelo de Adaptación, es posible observar la importancia de la profesión de enfermería en el proceso de atención integral, con el objetivo de garantizar las mejores condiciones de recuperación que van más allá de lo físico, así como en la fundamentación teórico-práctica del trabajo de enfermería.

ABSTRACT

ABSTRACT Introduction: fat embolism syndrome (FES) is an acute respiratory disorder that occurs when an inflammatory response causes the embolization of fat and marrow particles into the bloodstream. The exact incidence of FES is not well defined due to the difficulty of diagnosis. FES is mostly associated with isolated long bone trauma, and it is usually misdiagnosed in other trauma cases. The scope of this study was to identify and search the current literature for cases of FES in nonorthopedic trauma patients with the aim of defining the etiology, incidence, and main clinical manifestations. Methods: we perform a literature search via the PubMed journal to find, summarize, and incorporate reports of fat embolisms in patients presenting with non-orthopedic trauma. Results: the final literature search yielded 23 papers of patients presenting with fat embolism/FES due to non-orthopedic trauma. The presentation and etiology of these fat embolisms is varied and complex, differing from patient to patient. In this review, we highlight the importance of maintaining a clinical suspicion of FES within the trauma and critical care community. Conclusion: to help trauma surgeons and clinicians identify FES cases in trauma patients who do not present with long bone fracture, we also present the main clinical signs of FES as well as the possible treatment and prevention options.


RESUMO Introdução: a síndrome da embolia gordurosa (SEG) representa um distúrbio respiratório agudo que ocorre quando uma resposta inflamatória leva a uma embolização de partículas de gordura e medula na corrente sanguínea. A incidência exata da SEG não está bem estabelecida devido à dificuldade de diagnóstico. Tal síndrome está associada principalmente a traumas isolados de ossos longos e geralmente é diagnosticada erroneamente em outros casos de trauma. O escopo deste estudo foi de realizar uma pesquisa e identificar na literatura atual casos de SEG em pacientes com trauma de natureza não ortopédica com o objetivo de definir a etiologia, a incidência e as principais manifestações clínicas. Métodos: foi realizada uma pesquisa na literatura utilizando como base de dados o PubMed a fim de identificar os relatos e series de casos de embolias gordurosas em pacientes vítimas de traumas de natureza não ortopédica. A pesquisa final resultou em 23 artigos de pacientes que apresentaram embolia gordurosa/SEG devido a trauma não ortopédico. Resultados: a apresentação e a etiologia dessas embolias gordurosas são variadas e complexas, diferindo de paciente para paciente. Nesta revisão, destacamos a importância de manter uma suspeita clínica de SEG para pacientes vítimas de trauma que se encontrem sob cuidados intensivos. Conclusão: para ajudar os cirurgiões de trauma e os clínicos a identificar casos de SEG em pacientes com trauma que não apresentam fratura de ossos longos, foram destacados os principais sinais clínicos de SEG, bem como as possíveis opções de tratamento e prevenção.

ABSTRACT Objective: Assess complications and risks in staged femoral shaft fracture treatment using external fixation and intramedullary nailing (DCO). Methods: Analysis involved 37 patients with 40 fractures, mostly male (87.5%), average age 32.9 years. Data included ASA score, AO/OTA and Gustilo classifications, Glasgow Coma Score, Injury Severity Score, times to external fixation and conversion, ICU duration, nail type, and reaming status. Complications tracked were mortality, deep infection, and non-union. Results: Predominant fracture type was AO/OTA A (45%), with 40% open (Gustilo A, 93.8%). Average ISS was 21; GCS was 12.7. Median ICU stay was 3 days; average time to conversion was 10.2 days. Retrograde nails were used in 50% of cases, with reaming in 67.5%. Complications included deep infections in 5% and non-union in 2.5%. Conclusion: DCO strategy resulted in low infection and non-union rates, associated with lower GCS and longer ICU stays. RESUMO Objetivo: Analisar taxa de complicações e riscos no tratamento estagiado de fraturas diafisárias do fêmur com fixador externo e conversão para haste intramedular (DCO). Métodos: Estudo com 37 pacientes, 35 masculinos, idade média de 32,9 anos, abordando escores ASA, classificação AO/OTA, Gustilo, Glasgow e ISS, tempo até a fixação externa, na UTI e tipo de haste. Complicações como mortalidade, infecção profunda e não união foram registradas. Resultados: Fraturas tipo AO/OTA A foram as mais comuns (45%), com 40% expostas (Gustilo A, 93,8%). ISS médio de 21 e ECG de 12,7. Média de 3 dias na UTI e 10,2 dias até a conversão. Uso de haste retrógrada em 50% dos casos e fresagem em 67,5%. As complicações incluíram infecção profunda em 5% e não união em 2,5%. A não união correlacionou-se com baixo ECG e tempo prolongado na UTI. Conclusão: A estratégia de DCO mostrou-se eficaz com baixas taxas de infecção e não união, associada a baixo ECG e tempo na UTI. Nível de Evidência III; Estudo de Coorte Retrospectivo.

ABSTRACT Introduction: hemorrhagic shock is a significant cause of trauma-related deaths in Brazil and worldwide. This study aims to compare BE and lactate values at ICU admission and twenty-four hours after in identifying tissue hypoperfusion and mortality. Methods: examines a historical cohort of trauma patients over eitheen years old submittet to damage control resuscitation approch upon hospital admission and were then admitted to the ICU. We collected and analyzed ISS, mechanism and type of trauma, need for renal replacement therapy, massive transfusion. BE, lactate, pH, bicarbonate at ICU admission and twenty-four hours later, and mortality data. The patients were grouped based on their BE values (≥-6 and <-6mmol/L), which were previously identified in the literature as predictors of severity. They were subsequently redivided using the most accurate values found in this sample. In addition to performing multivariate binary logistic regression. The data were compared using several statistical tests due to diversity and according to the indication for each variable. Results: there were significant changes in perfusion upon admission to the Intensive Care Unit. BE is a statistically significant value for predicting mortality, as determined by using values from previous literature and from this study. Conclusion: the results demonstrate the importance of monitoring BE levels in the prediction of ICU mortality. BE proves to be a valuable bedside marker with quick results and wide availability.


RESUMO Introdução: o choque hemorrágico é a principal causa reversível de morte no trauma no Brasil e no mundo. Objetivo: comparar o valor de BE ao do lactato na admissão da UTI e vinte e quatro horas após o internamento na identificação de hipoperfusão tecidual e predição de mortalidade Método: coorte histórica de pacientes traumatizados, maiores de dezoito anos, submetidos à estratégia de controle de danos na admissão hospitalar, seguido de internamento em UTI. Foram coletados e analisados ISS, mecanismo e tipo de trauma, necessidade de terapia de substituição renal e transfusão maciça; BE, lactato, pH e bicarbonato coletados na admissão da UTI e vinte e quatro horas após, e a mortalidade. Os pacientes foram divididos em grupos conforme valores de BE (≥-6 e <-6mmol/L) já descritos na literatura como preditores de gravidade, e após redivididos de acordo com os valores de melhor acurácia encontrados nesta amostra, além de realização de regressão logística binária multivariada. Os dados foram comparados através de diversos testes estatísticos devido a diversidade e conforme a indicação para cada variável. Resultados: houve alterações perfusionais impactantes já na admissão da UTI. BE manteve-se com valor estatisticamente significativo para predição de mortalidade tanto quando utilizado os valores já conhecidos da literatura como quando aplicados os valores neste estudo identificados. Conclusão: valores de BE e de lactato foram capazes de predizer hipoperfusão tecidual e mortalidade nos dois momentos estudados, quando comparados, o BE tem boa performance como preditor de mortalidade, com rápido resultado e ampla disponibilidade.

Objetivo: Compreender o cotidiano de portadores de dor neuropática decorrente de lesão traumática. Metodologia: Pesquisa exploratória, descritiva, de abordagem qualitativa, com participantes selecionados por meio de mídias sociais. A coleta de dados foi realizada entre o período de julho a setembro de 2022, com indivíduos adultos que possuem diagnóstico de dor neuropática após lesão traumática. A coleta ocorreu por meio de um formulário online, utilizando um questionário sociodemográfico e um questionário aberto, para a captura de informações pertinentes sobre seu caso clínico e vivência com a dor crônica. Os dados foram analisados por meio de Bardin. Todos os preceitos éticos foram respeitados e o projeto foi aprovado sob parecer n.º 5.529.581 da Universidade Cesumar. Resultados: Participaram 15 pessoas com dor neuropática, com prevalência do sexo feminino (93,3%), com idade entre 41 a 50 anos (66,7%). Observou-se que 53,3% relataram comorbidades crônicas, sendo as doenças psíquicas, ansiolíticas e cardíacas mais citadas. Ainda, 33,3% dos participantes relataram que foram internados por causas de dores ou por tentativa de suicídio, 93,3% usam opióides e analgésicos potentes e ainda foram citados antidepressivos e ansiolíticos em 62% das respostas. Dentre os temas em destaque nas respostas, sobressaíram-se "Contexto e diagnóstico da dor neuropática; Vivência e frequência da dor; Apoio profissional e familiar diante da doença". Considerações Finais: Nesse sentido, a percepção acerca dos profissionais de saúde e valorização do médico para o tratamento da dor neuropática está relacionada, muitas vezes, à necessidade de aumentar o conhecimento referente ao manejo da dor e à utilização de opióides.


Objective: To understand the daily life of patients with neuropathic pain resulting from traumatic injury. Methodology: Exploratory, descriptive research, with a qualitative approach, with participants selected through social media. Data collection was carried out between July and September 2022, with adult individuals diagnosed with neuropathic pain after traumatic injury. The collection took place through an online form, using a sociodemographic questionnaire and an open questionnaire, to capture relevant information about their clinical case and experience with chronic pain. Data were analyzed using Bardin. All ethical precepts were respected and the project was approved by report n.º 5,529,581 of Cesumar University. Results: 15 people with neuropathic pain participated, with a prevalence of females (93.3%), aged between 41 and 50 years (66.7%). It was observed that 53.3% reported chronic comorbidities, with psychic, anxiolytic and cardiac diseases being the most cited. Also, 33.3% of the participants reported that they were hospitalized due to pain or a suicide attempt, 93.3% used opioids and potent analgesics, and antidepressants and anxiolytics were mentioned in 62% of the answers. Among the topics highlighted in the responses, the most important were "Context and diagnosis of neuropathic pain; Experience and frequency of pain; Professional and family support in the face of the disease". Final Considerations: In this sense, the perception of health professionals and the appreciation of physicians for the treatment of neuropathic pain is often related to the need to increase knowledge regarding pain management and the use of opioids.

Metodología: Investigación exploratoria, descriptiva, con enfoque cualitativo, con participantes seleccionados a través de las redes sociales. La recolección de datos se llevó a cabo entre julio y septiembre de 2022, con individuos adultos diagnosticados con dolor neuropático posterior a una lesión traumática. La recogida se realizó a través de un formulario online, utilizando un cuestionario sociodemográfico y un cuestionario abierto, para captar información relevante sobre su caso clínico y experiencia con el dolor crónico. Los datos se analizaron utilizando Bardin. Se respetaron todos los preceptos éticos y el proyecto fue aprobado bajo el dictamen número 5.529.581 de la Universidad Cesumar. Resultados: Participaron 15 personas con dolor neuropático, con predominio del sexo femenino (93,3%), con edades entre 41 y 50 años (66,7%). Se observó que 53,3% relataron comorbilidades crónicas, siendo las enfermedades psíquicas, ansiolíticas y cardíacas las más citadas. Aún así, el 33,3% de los participantes informaron que fueron hospitalizados por dolor o intento de suicidio, el 93,3% usaba opioides y analgésicos potentes, y los antidepresivos y ansiolíticos se mencionaron en el 62% de las respuestas. Entre los temas destacados en las respuestas, los más importantes fueron "Contexto y diagnóstico del dolor neuropático; Experiencia y frecuencia del dolor; Apoyo profesional y familiar ante la enfermedad". Consideraciones Finales: En este sentido, la percepción de los profesionales de la salud y la apreciación de los médicos por el tratamiento del dolor neuropático muchas veces se relaciona con la necesidad de aumentar el conocimiento sobre el manejo del dolor y el uso de opioides.

Objetivo: Determinar si el score de trauma revisado es predictor de mortalidad intrahospitalaria en pacientes politraumatizados atendidos en el Hospital Belén de Trujillo. Material y métodos: Se realizó un estudio observacional, analítico, retrospectivo, transversal de pruebas diagnósticas, se identificaron pacientes que fueron admitidos por politraumatismo y fueron hospitalizados en el Departamento de Cirugía General durante el periodo comprendido entre enero del 2017 a diciembre del 2021; en una muestra de 100 pacientes se evaluó la mortalidad. Resultados: La edad promedio fue 47,18 ± 20,40 vs 41,13 ± 18,37 en aquellos que fallecieron y sobrevivieron; el 55% y 63,33% de los fallecidos y sobrevivientes fueron varones en los grupos de estudio. La coagulopatía, acidosis metabólica y SRIS estuvieron asociadas a la mortalidad. Se observó una proporción de 72,50% y 3,33% de RTS < 6 en los pacientes politraumatizados que fallecieron y sobrevivieron (p = 0,001); se obtuvieron valores de sensibilidad: 73% IC 95% (59% - 86%), especificidad: 97% IC 95% (92% - 100%), valor predictivo positivo: 94% IC 95% (85% - 100%) y valor predictivo negativo: 84% IC 95% (75% - 93%). El área bajo la curva fue del 96,50% en la predicción de mortalidad por el RTS en pacientes con politraumatismo. Conclusión: El score de trauma revisado es un buen predictor de mortalidad en politraumatizados.

A proportion of 72.50% and 3.33% STR < 6 was observed in polytrauma patients who died and survived (p = 0.001); sensitivity: 73% CI 95% (59% - 86%), specificity: 97% CI 95% (92% - 100%), positive predictive value: 94% CI 95% (85% - 100%) and negative predictive value: 84% CI 95% (75% - 93%). The area under the curve was 96.50% in predicting mortality by RTS in polytrauma patients. Conclusion: The revised trauma score is a good predictor of mortality in polytrauma patients.

Background. Numerous health conditions in the older adult population can be attributed to falls, including traumatic brain injury (TBI), which can lead to devastating short and long-term sequelae. Older adults are also more likely to experience frailty, which encompasses physical, psychological, and social deficits that may lead to adverse health outcomes. Our literature review synthesizes current evidence for understanding frailty in the context of TBI among older adults using the Integral Model of Frailty as a framework. Content synthesis. A total of 32 articles were identified, and 9 articles were included. The results of this review indicate that outcomes resulting from TBI are closely linked to the physical, psychological, and social domains of frailty. Conclusions. A small amount of literature currently examines frailty in the context of TBI among older adults. Um total de 32 artigos foram identificados e 9 artigos foram incluídos. As descobertas desta revisão indicam que os resultados do TCE estão intimamente relacionados aos domínios físico, psicológico e social da fragilidade. Conclusão.Um pequeno corpo de literatura atualmente examina a fragilidade no contexto do TCE entre adultos mais velhos. Objetivo: Apresentar as características clínicas dos pacientes vítimas de trauma que necessitaram de transfusão emergencial para uma abordagem do Diagnóstico de Enfermagem "Risco de Choque"; descrever a atuação da equipe de Enfermeiros do Trauma nesse contexto. Métodos: Trata-se de um estudo descritivo, retrospectivo, de abordagem quantitativa. Os dados retratam o período de outubro de 2018 a dezembro de 2019, perfazendo um total de 447 pacientes com choque hemorrágico por trauma inseridos no Protocolo de Transfusão Maciça de um hospital público. Resultados: O "Risco de Choque" foi o diagnóstico de enfermagem prioritário nos pacientes com hemorragia grave por trauma. Houve predominância do sexo masculino, vítimas de politraumatismo em sua maioria, com idade entre 18 e 29 anos. 197 pacientes apresentaram líquido livre que foi detectado através do exame de ultrassom de emergência nos pacientes com trauma torácico ou abdominal. O Serviço de Atendimento Móvel de Urgência foi o meio utilizado por 378 pacientes para atendimento pré-hospitalar. Conclusão: A caracterização mostra a gravidade desses casos, com necessidade de transfusão maciça. O conhecimento desses fatores pela equipe multiprofissional de pacientes críticos com hemorragia grave associada ao trauma é fundamental, tornando-se necessária a abordagem do diagnóstico de enfermagem "risco de choque" pelo profissional Enfermeiro. Objective: To present the clinical characteristics of the trauma patients who required emergency transfusion for an approach to the Nursing Diagnosis "Risk of Shock"; describe the role of the Trauma Nurses team in this context. Methods: This is a descriptive, retrospective study with a quantitative approach. The data portrays the period from October 2018 to December 2019, making up a total of 447 patients with traumatic hemorrhagic shock inserted in the Mass Transfusion Protocol of a public hospital. Results: There was a predominance of male subjects, victims polytrauma in that majority, aged between 18 and 29 years old. 197 patients had free fluid that was detected by emergency ultrasound examination in patients with chest or abdominal trauma. The Mobile Emergency Care Service was the resource used by 378 patients to the pre-hospital care. Conclusion: The characterization shows the severity of these cases, requiring massive transfusion. The knowledge of these factors by the multidisciplinary team of critically ill patients with severe hemorrhage associated with trauma is essential, making it necessary for the nursing professional to approach the nursing diagnosis "risk of shock". Los datos retratan el período de octubre de 2018 a diciembre de 2019, lo que hace un total de 447 pacientes con choque hemorrágico traumático insertado en el Protocolo de Transfusión Masiva de un hospital público. Resultados: El "Riesgo de Choque" fue el diagnóstico de enfermería prioritario en los pacientes con hemorragia por trauma severo. Predominó el sexo masculino, mayoritariamente víctimas de politraumatismos, con edades comprendidas entre los 18 y 29 años. 197 pacientes tenían líquido libre que fue detectado por el examen de ultrasonido de emergencia con traumatismo torácico o abdominal. El Servicio Móvil de Atención de Emergencias fue el medio utilizado por 378 pacientes para la atención prehospitalaria. Conclusión: La caracterización muestra la gravedad de estos casos, requiriendo transfusión masiva. El conocimiento de estos factores por parte del equipo multidisciplinario de pacientes críticos con hemorragia severa asociada a trauma es fundamental, por lo que es necesario que el profesional de enfermería aborde el diagnóstico de enfermería "riesgo de shock". Objetivo: Descrever o perfil dos pacientes atendidos pela onda vermelha em 2018 e 2019. Métodos: Trata-se de uma pesquisa quantitativa, de caráter transversal, retrospectiva, descritiva e documental realizada no Hospital de Pronto-Socorro João XXIII, Belo Horizonte, Brasil. O universo deste estudo foram 83 prontuários eletrônicos de pacientes que entraram no protocolo da Onda Vermelha. Foi realizada distribuições de frequência, medidas de tendência central (média e mediana) e de variabilidade (desvio padrão). Resultados: A maioria dos pacientes foi do sexo masculino, com idade entre 1 e 95 anos e média de 33,4 anos. O mecanismo do trauma mais frequente foi contuso, o meio transporte foi ambulância do Serviço de Atendimento Móvel de Urgência, a causa mais frequente dos traumas foi acidente automobilístico seguido de trauma por projétil de arma de fogo. As cirurgias mais frequentes foram laparotomia, toracotomia e craniectomia. Óbito foi o desfecho mais comum. Conclusão: Os pacientes chegam muito graves e o óbito foi o principal desfecho. Sugere-se estudos que possibilitem análise comparativa de dados e padronização do cálculo da probabilidade de sobrevivência. Recomenda-se atualização do protocolo da onda vermelha, incluindo outras cirurgias que já são realizadas e novos critérios de inclusão de pacientes. (AU)


Objective: To describe the profile of patients treated in the "Red Wave", in the period of 2018-2019. Methods: It is a quantitative, cross-sectional, retrospective, descriptive and documentary research. Held at the first-aid post João XXIII Hospital, in Belo Horizonte. The universe of this study was 83 electronic medical records of patients who entered the "Red Wave" protocol. Simple frequency distributions, measures of central tendency (mean and median) and variability (standard deviation) have been performed. Results: Male, aged between 1 and 95 years old, being an average at 33,4 years old. The most frequent trauma mechanism was blunt, the means of transport was an ambulance from the Mobile Emergency Service, the causes of the trauma were an automobile accident, followed by trauma by a firearm projectile. From the performed surgeries, the most frequent ones were laparotomy, thoracotomy and craniectomy. Among the outcome, death was the most common one among patients. Conclusion: Patients arrive very seriously and death was the main outcome. Studies that allow comparative data analysis and standardization of the calculation of survival probability are suggested. It is recommended to update the red wave protocol, including other surgeries that are already performed and new inclusion criteria for patients. (AU)

El universo de este estudio fue de 83 historias clínicas electrónicas de pacientes que ingresaron al protocolo Red Wave. Se realizaron distribuciones de frecuencia, medidas de tendencia central (media y mediana) y variabilidad (desviación estándar). Resultados: La mayoría de los pacientes eran varones, con edades comprendidas entre 1 y 95 años y media de 33,4 años. El mecanismo de traumatismo más frecuente fue contundente, el medio de transporte fue una ambulancia del Servicio Móvil de Emergencias, la causa más frecuente de traumatismo fue un accidente automovilístico seguido del traumatismo por proyectil de arma de fuego. Las cirugías más frecuentes fueron laparotomía, toracotomía y craniectomía. La muerte fue el resultado más común. Conclusión: Los pacientes llegan muy en serio y la muerte fue el resultado principal. Se sugieren estudios que permitan el análisis de datos comparativos y la estandarización del cálculo de la probabilidad de supervivencia. Se recomienda actualizar el protocolo de onda roja, incluyendo otras cirugías que ya se realizan y nuevos criterios de inclusión de pacientes. (AU)

ABSTRACT

Spondyloptosis in the clinic is rarely reported. We herein present a 47-year-old female, who suffered from a crush injury directly by a heavy cylindrical object from the lateral side. She was diagnosed to have traumatic L3 spondyloptosis with multiple traumas. Staged surgical procedures were conducted and a three-year follow-up was obtained. Eventually, normal spinal alignment was restored, and neurological deficits were gradually improved. At three years follow-up, the motor strength scores and function of the sphincters were incompletely improved. Previously published reports on traumatic lumbar spondyloptosis were reviewed and several critical points for management of this severe type of spinal injury were proposed. First, thoracolumbar and lumbosacral junction were mostly predilection sites. Second, numerous patients involving traumatic lumbar spondyloptosis were achieved to American Spinal Injury Association grade A. Third, lumbar spondyloptosis was commonly coupling with cauda equina injury. Finally, the outcomes were still with poorly prognosis and recovery of patients was correlation to spondyloptosis severity. Based on this case report and literatures review, we highlighted that the spinal alignment restoration relying on staged operations and following rehabilitation hereof are both important once facing with multiple traumas. Furthermore, we suggested to perform routine CT angiography during lumbar spondyloptosis to justify whether there are large vessel compression or injury.

ABSTRACT

Medical record writing in trauma surgery differs from that of conventional surgery. The medical records in trauma surgery encompass the entire process of trauma management, serving as an indication of expertise in trauma management and treatment effectiveness. At present, most medical records in trauma surgery are written in the conventional writing mode of various surgical specialties, which fails to indicate the overall injury situation of trauma patients and the process of diagnosis and treatment, resulting in unevenly qualified medical records in trauma surgery, which hinders trauma treatment and academic exchange. Given that trauma assessment owns unique nuances, conventional physical examination methods are not entirely applicable to trauma patients. Moreover, with the advancement of diagnostic and therapeutic technologies, certain physical examination techniques and medical procedures that may induce pain, impede timely rescue, or even cause injury can be circumvented in clinical assessment. As theories, techniques, and disciplines in trauma care continue to develop, there arises a need for further standardization and uniformity in medical record writing in trauma surgery. By analyzing the special characteristics of medical record writing in trauma surgery, the authors sorted out and summarized the problems in medical record writing and put forward suggestions on improvement of medical records in trauma surgery to provide reference for standardization of medical record writing in trauma surgery and improvement of the comprehensive strength of trauma treatment.

ABSTRACT

Objective:To investigate the risk factors and their warning value for the occurrence of sepsis in patients with severe multiple trauma.Methods:A retrospective cohort study was conducted to analyze the clinical data of 92 patients with severe multiple trauma admitted to Yuyao People′s Hospital from July 2019 to October 2021. There were 71 males and 21 females, with the age range of 36-55 years [(45.5±13.6)years]. The injury severity score (ISS) was 20-29 points [(25.3±6.4)points]. The patients were divided into sepsis group ( n=32) and non-sepsis group ( n=60) according to whether sepsis occurred during hospitalization. Data were recorded for the two groups, including gender, age, basic diseases, cause of injury, number of injury sites, ISS, post-injury complications, and levels of aryl hydrocarbon receptor (AHR), C-reactive protein (CRP) and procalcitonin (PCT) at 1, 3 and 5 days after injury. The above data were analyzed to identify their correlation with the occurrence of sepsis in patients with severe multiple trauma by univariate analysis. The independent risk factors for sepsis in patients with severe multiple trauma were determined by multivariate Logistic regression analysis. The warning value of the single or combined risk factors for the occurrence of sepsis in patients with severe multiple trauma was evaluated by the receiver operating characteristic (ROC) curve and area under the curve (AUC). Results:By univariate analysis, it was demonstrated that the occurrence of sepsis was correlated with ISS, level of AHR at day 1 after injury, level of CRP at day 3 after injury and level of PCT at day 3 after injury ( P<0.05 or 0.01), but not with age, sex, basic diseases, level of AHR at 3, 5 days after injury, level of PCT at 1, 5 days after injury and level of CRP at 1, 5 days after injury (all P>0.05). By multivariate Logistic regression analysis, higher ISS ( OR=1.12, 95% CI 1.01, 1.24, P<0.05), level of AHR at day 1 after injury ( OR=1.30, 95% CI 1.10, 1.52, P<0.01) and level of PCT at day 3 after injury ( OR=1.81, 95% CI 1.08, 3.03, P<0.05) were found to be strongly correlated with the occurrence of sepsis. ROC curve analysis showed that higher ISS (AUC=0.69, 95% CI 0.57, 0.76) and level of AHR at day 1 after injury (AUC=0.79, 95% CI 0.68, 0.90) had warning value for the occurrence of sepsis, and the warning efficiency of combined panel was much better (AUC=0.86, 95% CI 0.77, 0.95). Conclusions:Higher ISS, level of AHR at day 1 after injury and level of PCT at day 3 after injury are independent risk factors for the occurrence of sepsis in patients with severe multiple trauma. ISS, AHR and combination of both exhibit good warning value for the occurrence of sepsis in patients with severe multiple trauma.

ABSTRACT

Objective:To explore the independent risk factor for in-hospital mortality of patients with multiple trauma, and to construct a prediction model of risk of death and validate its efficacy.Methods:A retrospective cohort study was performed to analyze the clinical data of 1 028 patients with multiple trauma admitted to Affiliated Hospital of Jiangsu University from January 2011 to December 2021. There were 765 males and 263 females, aged 18-91 years[(53.8±12.4)years]. The injury severity score (ISS) was 16-57 points [(26.3±7.6)points]. There were 153 deaths and 875 survivals. A total of 777 patients were enrolled as the training set from January 2011 to December 2018 for building the prediction model, while another 251 patients were enrolled as validation set from January 2019 to December 2021. According to the outcomes, the training set was divided into the non-survival group (115 patients) and survival group (662 patients). The two groups were compared in terms of the gender, age, underlying disease, injury mechanism, head and neck injury, maxillofacial injury, chest injury, abdominal injury, extremity and pelvis injury, body surface injury, damage control surgery, pre-hospital time, number of injury sites, Glasgow coma score (GCS), ISS, shock index, and laboratory test results within 6 hours on admission, including blood lactate acid, white blood cell counts, neutrophil to lymphocyte ratio (NLR), platelet counts, hemoglobin, activated partial thromboplastin time (APTT), fibrinogen, D-dimer and blood glucose. Univariate analysis and multivariate Logistic regression analysis were performed to determine the independent risk factors for in-hospital mortality in patients with multiple trauma. The R software was used to establish a nomogram prediction model based on the above risk factors. Area under the receiver operating characteristic (ROC) curve (AUC), calibration curve and clinical decision curve analysis (DCA) were plotted in the training set and the validation set, and Hosmer-Lemeshow goodness-of-fit test was performed.Results:Univariate analysis showed that abdominal injury, extremity and pelvis injury, damage control surgery, GCS, ISS, shock index, blood lactic acid, white blood cell counts, NLR, platelet counts, hemoglobin, APTT, fibrinogen, D-dimer and blood glucose were correlated with in-hospital mortality in patients with multiple trauma ( P<0.05 or 0.01). Logistic regression analysis showed that GCS≤8 points ( OR=1.99, 95% CI 1.12,3.53), ISS>25 points ( OR=7.39, 95% CI 3.50, 15.61), shock index>1.0 ( OR=3.43, 95% CI 1.94,6.08), blood lactic acid>2 mmol/L ( OR=9.84, 95% CI 4.97, 19.51), fibrinogen≤1.5 g/L ( OR=2.57, 95% CI 1.39,4.74) and blood glucose>10 mmol/L ( OR=3.49, 95% CI 2.03, 5.99) were significantly correlated with their in-hospital mortality ( P<0.05 or 0.01). The ROC of the nomogram prediction model indicated that AUC of the training set was 0.91 (95% CI 0.87, 0.93) and AUC of the validation set was 0.90 (95% CI 0.84, 0.95). The calibration curve showed that the predicted probability was consistent with the actual situation in both the training set and validation set. DCA showed that the nomogram prediction model presented excellent performance in predicting in-hospital mortality. In Hosmer-Lemeshow goodness-of-fit test, χ2 value of the training set was 9.69 ( P>0.05), with validation set of 9.16 ( P>0.05). Conclusions:GCS≤8 points, ISS>25 points, shock index>1.0, blood lactic acid>2 mmol/L, fibrinogen≤1.5 g/L and blood glucose>10 mmol/L are independent risk factors for in-hospital mortality in patients with multiple trauma. The nomogram prediction model based on these 6 predictive variables shows a good predictive performance, which can help clinicians comprehensively assess the patient′s condition and identify the high-risk population.

ABSTRACT Introduction: thoracic trauma is defined as anything that involves the rib cage, the musculoskeletal framework that houses the heart, lungs, pleurae and mediastinal structures. It can be superficial or immediately lifethreatening for victims. In Brazil, most assistance is due to urban violence. Objective: evaluate the clinical and epidemiological aspect of patients who are victims of thoracic trauma treated at Hospital de Urgência de Sergipe, Aracaju/SE, Brazil. Method: cross-sectional, observational and prospective study, carried out for eleven months, with 100 polytraumatized patients. A semi-structured form was applied, and the data were systematized, analyzed and statistically tested considering a 5% margin of error. Results: 85% of the patients were male, with a mean age of 39.3 and an age range of 30 to 49 years; 57% of them had incomplete primary education, 70% had a family income of up to 2 minimum wages and 41% were from Greater Aracaju. As for the mechanism of trauma, 33% were car-related, with blunt trauma as the main mechanism, and rib fractures as the main consequence. Among penetrating injuries, CWI (26%) and GSW (21%) were the most prevalent, with hemothorax being the main consequence. Most patients underwent thoracostomy (59%). Conclusion: the profile found was of young men, victims of urban violence. The thoracostomy was resolving in most cases and should be instituted promptly when necessary. A smaller number of patients may require thoracotomy, especially in the presence of hemodynamic instability.


RESUMO Introdução: o trauma torácico é definido como toda aquele que envolve a caixa torácica, arcabouço osteomuscular que aloja o coração, os pulmões, as pleuras e as estruturas do mediastino. Pode ser superficial ou causar risco de vida imediato às vítimas. No Brasil, a maioria dos atendimentos é decorrente da violência urbana. Objetivo: avaliar o aspecto clínico-epidemiológico dos pacientes vítimas de trauma torácico atendidos no Hospital de Urgência de Sergipe, Aracaju/SE, Brasil. Métodos: estudo transversal, observacional e prospectivo, realizado durante onze meses, com 100 pacientes politraumatizados. Formulário semiestruturado foi aplicado, e os dados sistematizados, analisados e testados estatisticamente considerando-se a margem de erro de 5%. Resultados: 85% dos pacientes eram do gênero masculino, com idade média de 39,3 e faixa etária de 30 a 49 anos; 57% deles possuíam ensino fundamental incompleto, 70% tinham renda familiar de até 2 salários mínimos e 41% eram provenientes da Grande Aracaju. Quanto ao mecanismo de trauma, 33% foi automobilístico, tendo como principal mecanismo o trauma contuso, e a principal consequência a fratura de arcos costais. Dentre os ferimentos penetrantes, os FAB (26%) e FAF (21%) foram os mais prevalentes, sendo o hemotórax sua principal consequência, sendo maioria dos pacientes submetida a toracostomia (59%). Conclusão: o perfil encontrado foi de homens, jovens, vítimas da violência urbana. A toracostomia foi resolutiva na maioria dos casos e deve ser instituída prontamente quando necessário. Uma menor parte dos pacientes necessitou de toracotomia, principalmente na presença de instabilidade hemodinâmica.

ABSTRACT Objective: To assess the impact of the COVID-19 pandemic on the epidemiology and clinical outcomes of open fractures considering the periods before and during the pandemic. Methods: An observational and retrospective study, which included patients aged over 18 years, admitted to the Orthopedics and Traumatology Ward of Hospital São Paulo, of the Federal University of São Paulo (UNIFESP). Data was collected in two moments: pre-pandemic (March 1, 2019, to February 29, 2020) and during the pandemic (March 1, 2020, to February 28, 2021). Results: In total, 183 patients were evaluated with a mean age of 36 years ± 14 years. In the pre-pandemic period, 94 patients underwent surgery, 81 men (85.37%) and 13 women (14.2%), with a mean age of 36 ± 3 years. During the pandemic period, 89 patients were subjected to surgery, 77 men (86.6%) and 12 women (13.4%), with a mean age of 38 ± 3 years. Conclusion: During the pandemic, open fractures were still more common in men. Regarding hospital indicators, the prevalence of infections in the surgical wound and the length of stay of patients with open fractures increased, however, with little significance. Fractures classified as Gustilo IIIA were the most common, while the most common according to the AO classification were 33, 34, 42, 43, 2R3, and 2R3 + 2U2. The frequency of run overs during the pandemic decreased. However, firearm projectile injuries and falls and occupational injuries increased. Level of Evidence III, Retrospective Comparative Study.


RESUMO Objetivo: Avaliar o impacto da COVID-19 na epidemiologia e nos desfechos clínicos das fraturas expostas considerando os períodos antes e durante a pandemia. Métodos: Estudo observacional e retrospectivo que incluiu pacientes maiores de 18 anos, internados na Enfermaria de Ortopedia e Traumatologia do Hospital São Paulo, da Universidade Federal de São Paulo. Os dados foram coletados em dois momentos - antes (março de 2019 a fevereiro de 2020) e durante a pandemia (março de 2020 a fevereiro de 2021) -, por meio da análise de prontuários eletrônicos de todos os registros hospitalares dessa instituição. Resultados: No total, foram avaliados 183 pacientes com média de idade de 36 ± 14 anos. No período pré-pandêmico, foram operados 94 pacientes, sendo 81 homens (85,37%) e 13 mulheres (14,2%), com média de idade de 36 ± 3 anos. Já ao longo do período pandêmico, foram operados 89 pacientes, sendo 77 homens (86,6%) e 12 mulheres (13,4%), com média de idade de 38 ± 3 anos. Conclusão: Durante a pandemia, a ocorrência de fraturas expostas se manteve com maior frequência em indivíduos do sexo masculino. Quanto aos indicadores hospitalares, houve aumento da prevalência de infecções na ferida operatória, assim como do tempo de internação dos pacientes, todavia, com pouca significância. Foram mais frequentes as fraturas classificadas como Gustilo IIIA e, pela classificação AO, as fraturas 33, 34, 42, 43, 2R3 e 2R3 + 2U2. Notamos redução da frequência de atropelamentos durante a pandemia e aumento dos casos de ferimentos por projétil de arma de fogo, quedas e acidentes ocupacionais. ABSTRACT Objective: diagnostic errors during the interpretation of an imaging test by the physician can lead to increased mortality and length of hospital stay for patients. The rate of divergence in the report given by a radiologist and an Emergency Physicians (EP) can reach over 20%. The objective of this study was to compare the unofficial tomographic reports issued by EP with the official reports issued by radiologists. Methods: a cross-sectional study, in which interpretations of the exams (documented in the medical records by the EP) of all patients undergoing computed tomography (CT) of the chest, abdomen or pelvis performed in the emergency room, at an interval of 8 months, were evaluated. These data were compared with the official reports of the radiologist (gold standard). Results: 508 patients were included. The divergence between EP and the radiologist occurred in 27% of the cases. The most common type of divergence was the one not described by the EP, but described by the radiologist. The chance of having divergence in a case of multiple trauma is 4.93 times greater in relation to the case of only blunt trauma in one segment. A statistically relevant difference was also found in the length of stay of patients who had different interpretations of the CT scans. Conclusion: the study found a relatively high divergence rate between the EP report and the official radiologist report. However, less than 4% of these were considered to be clinically relevant, indicating the ability of the EP to interpret it satisfactorily.


RESUMO Objetivo: os erros diagnósticos durante a interpretação de um exame de imagem pelo médico podem acarretar aumento da mortalidade e do tempo de internação dos pacientes. A taxa de divergência entre o laudo dado por um médico radiologista e a avaliação preliminar de um Médico Emergencista (ME) pode chegar a mais de 20%. O objetivo deste trabalho foi comparar as avaliações dos exames de imagem realizadas pelo ME com os laudos oficiais emitidos pelos radiologistas. Métodos: estudo seccional e transversal, no qual foram avaliadas interpretações dos exames (documentadas no prontuário pelos ME) de todos os pacientes submetidos à tomografia computadorizada (TC) de tórax, abdome ou pelve realizada na emergência, em um intervalo de 8 meses. Esses dados foram comparados com os laudos oficiais do médico radiologista (padrão ouro). Resultados: foram incluídos 508 pacientes no estudo. A divergência entre ME e radiologista ocorreu em 27% dos casos. O tipo de divergência mais incidente foi a não descrita pelo ME, mas descrita pelo radiologista. A chance de haver divergência em um caso de politrauma é 4,93 vezes maior em relação ao caso de somente trauma contuso em um segmento. Foi encontrada também uma diferença estatisticamente relevante no tempo de internamento dos pacientes que tiveram interpretações divergentes das tomografias. Conclusão: o estudo encontrou uma taxa de divergência relativamente alta entre o laudo do ME e o laudo oficial do radiologista. Contudo, menos de 4% dessas foram consideradas como clinicamente relevantes, indicando a capacidade dos ME em interpretar os exames de imagem de forma satisfatória.

Resumen: Introducción: el trauma múltiple severo es una patología de importancia epidemiológica. Una de las complicaciones que aumenta la mortalidad basal al ingreso a unidades de cuidados intensivos es la aparición de lesión renal aguda; sin embargo, en cuanto al primer y segundo nivel de atención hospitalaria, se cuenta con herramientas restringidas para su detección temprana, tales como creatinina sérica y cistatina C, limitándose a las nuevas determinaciones capaces de detectar daño glomerular temprano. Objetivo: conocer la asociación que existe entre la alteración, caracterizada por aumento del índice resistivo renal, con el diagnóstico de lesión renal aguda en pacientes con trauma múltiple severo apoyando a la toma de decisiones en el manejo oportuno de dicha patología. Material y métodos: se realizó un estudio clínico, observacional, descriptivo y prospectivo incluyendo pacientes con trauma múltiple severo que ingresaron a la Unidad de Cuidados Intensivos del Hospital General La Villa de la Ciudad de México, en donde se recolectan datos a través de la medición del índice resistivo renal, creatinina sérica, cistina C y tasa de uresis kilogramo hora, al ingreso, 24, 48 y 72 horas posteriores. Los datos obtenidos se analizaron mediante medidas de tendencia central, moda, media, mediana y asociación con la prueba p de Pearson. Resultados: la lesión renal aguda y la alteración del índice resistivo renal mostraron asociación y sensibilidad significativa. La asociación de Pearson entre pacientes con lesión renal aguda que mostraron alteraciones de índice resistivo renal y cistatina C fue de 0.475, comprobando sensibilidad similar entre los dos parámetros de evaluación clínica. Conclusiones: la cuantificación del índice resistivo renal en pacientes con diagnóstico de trauma múltiple severo es útil para la detección temprana de hipoperfusión renal y secundaria al mismo, desarrollo de lesión renal aguda, con misma sensibilidad que los reportes de marcadores como cistatina C, siendo útil en unidades con pocos recursos o nulos marcadores de lesión renal aguda temprana más específicos y sensibles.


Abstract: Introduction: severe multiple trauma is a pathology of epidemiological importance. One of the complications that increases baseline mortality on admission to intensive care units is the appearance of acute kidney injury. However, regarding the first and second level of hospital care, there are limited tools for its early detection, such as serum creatinine and cystatin C, limited to new determinations capable of detecting early glomerular damage. Objective: to know the association that exists between the alteration, characterized by an increase in the renal resistive index, with the diagnosis of acute kidney injury in patients with severe multiple trauma, supporting decision-making in the timely management of said pathology. Material and methods: a clinical, observational, descriptive and prospective study was carried out including patients with severe multiple trauma who were admitted to the ICU at Hospital General La Villa, CDMX, where data is collected through the measurement of the renal resistive index, serum creatinine, cystine C and the rate of uresis kilogram hour, at admission, 24, 48 and 72 hours later, analyzing the data obtained through measures of central tendency, mode, mean, median with association by p Pearson. Results: the association analysis using the Pearson's p test as a test of association between acute kidney injury and alteration of the renal resistive index, showed significant association and sensitivity. Pearson's association between patients with acute kidney injury who had alterations in IRR and Cystatin C was 0.475, showing similar sensitivity between the two clinical evaluation parameters. Conclusions: the quantification of the IRR in those patients with a diagnosis of severe multiple trauma is useful for the early detection of renal hypoperfusion and, secondary to it, the development of acute kidney injury, with the same sensitivity as the reports of markers such as cystatin C, being useful in units with few or no resources more specific and sensitive early acute kidney injury markers.


Resumo: Introdução: o politraumatismo grave é uma patologia de importância epidemiológica. Uma das complicações que aumenta a mortalidade basal na admissão em unidades de terapia intensiva é o aparecimento de lesão renal aguda. No entanto, em relação ao primeiro e segundo nível de atenção hospitalar, existem ferramentas limitadas para sua detecção precoce, como a creatinina sérica e a cistatina C, limitando-se às novas determinações capazes de detectar lesões glomerulares precoces. Objetivo: conhecer a associação que existe entre a alteração, caracterizada por aumento do índice de resistência renal, com o diagnóstico de lesão renal aguda em pacientes politraumatizados graves, apoiando a tomada de decisão no manejo oportuno dessa patologia. Material e métodos: realizou-se um estudo clínico, observacional, descritivo e prospectivo, incluindo pacientes politraumatizados graves internados na UTI do Hospital General La Villa CDMX, onde os dados são coletados por meio da medição do índice de resistência renal, creatinina sérica, cistina C e taxa de urese quilograma hora, na admissão, 24, 48 e 72 horas depois, analisando os dados obtidos por medidas de tendência central, moda, média, mediana com associação por p Pearson. Resultados: a análise de associação utilizando o teste p de Pearson como teste de associação entre lesão renal aguda e alteração do índice de resistência renal mostrou associação e sensibilidade significativas. A associação de Pearson entre pacientes com lesão renal aguda e alterações de IRR e cistatina C foi de 0.475, comprovando sensibilidade semelhante entre os dois parâmetros de avaliação clínica. Conclusões: a quantificação da IRR naqueles pacientes com diagnóstico de politraumatismo grave é útil para a detecção precoce de hipoperfusão renal e secundária a ela, o desenvolvimento de lesão renal aguda, com a mesma sensibilidade que os relatos de marcadores como a cistatina C, sendo útil em unidades com poucos ou nenhum recurso marcadores mais específicos e sensíveis de lesão renal aguda precoce.
