ABSTRACT

ABSTRACT Objectives: to develop and validate an algorithm to guide professionals in cleaning and disinfecting ambulances after transferring patients with contagious infectious diseases. Methods: the study was conducted between September and November 2021. The developed algorithm was validated by 104 judges, including nurses, physical therapists, and physicians who care for patients with contagious infectious diseases. It used the Delphi technique and content validity index. Results: in the first evaluation, the judges considered the algorithm "unsuitable" and "fully suitable". The algorithm reviewed according to the judges' suggestions was rated between "suitable" and "fully suitable" in the second evaluation. The overall content validity index was 0.960 and 0.998 in the first and second evaluations. Conclusions: the algorithm to guide the cleaning and disinfection of ambulances after transferring patients with contagious infectious diseases was developed and validated by specialists in the field, with consensus among the judges in the second evaluation.


RESUMEN Objetivos: desarrollar y validar algoritmo para orientar profesionales en la limpieza y desinfección de ambulancia tras transferencia de paciente con enfermedades transmisibles. Métodos: estudio realizado entre septiembre y noviembre de 2021. El algoritmo desarrollado fue validado por 104 jueces, incluyendo enfermeros, fisioterapeutas y médicos que asisten a pacientes con enfermedades transmisibles. Utilizada técnica Delphi e índice de validez de contenido. Resultados: en la primera evaluación, los jueces consideraron el algoritmo entre "inadecuado" y "totalmente adecuado". El algoritmo revisado conforme sugestiones de los jueces fue clasificado entre "adecuado" y "totalmente adecuado" en la segunda evaluación. El índice de validez de contenido general fue de 0,960 y 0,998 en la primera y segunda evaluación, respectivamente. Conclusiones: el algoritmo para orientar en la limpieza y desinfección de ambulancia tras transferencia de paciente con enfermedades transmisibles fue construido y validado por especialistas en el área, habiendo consenso entre los jueces en la segunda evaluación.

ABSTRACT

RESUMO A postura em pé durante o trabalho tem sido associada a sintomas musculoesqueléticos. Entretanto, há poucos estudos que avaliam o tempo em pé utilizando medidas objetivas. Assim, o objetivo deste estudo foi verificar se o tempo em pé no trabalho está associado com dor na coluna lombar e nos membros inferiores nos últimos sete dias e 12 meses em cuidadoras de idosos e trabalhadoras da limpeza. Trata-se de estudo transversal, em que o tempo em pé no trabalho foi quantificado por meio de inclinômetros fixados na coxa e na coluna vertebral, e os sintomas musculoesqueléticos foram avaliados por meio do Questionário Nórdico de Sintomas Osteomusculares. Os dados foram analisados por meio da correlação ponto bisserial (rpb) entre o tempo em pé no trabalho e a presença de sintomas musculoesqueléticos. As análises foram realizadas por meio do software SPSS e o nível de significância adotado foi de 5%. Verificou-se que as trabalhadoras passam a maior parte do tempo em pé paradas (41,3%) ou andando (39,3%). Houve correlação positiva entre a presença de sintoma na lombar e o tempo em pé (rpb=0,52; p<0,05) e correlação negativa entre o tempo andando e sintomas no quadril (rpb=−0,53; p<0,05) nos últimos 12 meses e o tempo correndo e sintomas no quadril (rpb=−0,43; p<0,05) e na coluna lombar (rpb=−0,43; p<0,05) nos últimos sete dias. O maior tempo em pé foi correlacionado com a presença de sintomas na lombar; enquanto o menor tempo andando e correndo foram correlacionados com a presença de sintomas no quadril e na coluna lombar.


RESUMEN La postura de trabajo de pie está asociada con síntomas musculoesqueléticos. Sin embargo, hay pocos estudios que evalúan el tiempo prolongado en esta postura utilizando medidas objetivas. Por esto, el objetivo de este estudio fue verificar si el tiempo prolongado al trabajar de pie está asociado al dolor en la columna lumbar y en los miembros inferiores de los cuidadores de ancianos y los trabajadores de limpieza entre los últimos siete días y 12 meses de trabajo. Se trata de un estudio transversal, en el que se cuantificó el tiempo prolongado en el trabajo de pie por medio de inclinómetros fijados en el muslo y la columna vertebral, y los síntomas musculoesqueléticos fueron evaluados por el Cuestionario Nórdico de Síntomas Musculoesqueléticos. Para el análisis de datos se utilizó la correlación biserial puntual (rpb) entre el tiempo prolongado en el trabajo de pie y la presencia de síntomas musculoesqueléticos. Los análisis se realizaron con el software SPSS, y se adoptó el nivel de significación del 5%. Se constató que las trabajadoras pasaban la mayor parte del tiempo paradas (41,3%) o caminando (39,3%). Hubo una correlación positiva entre la presencia de síntomas en la región lumbar y el tiempo prolongado de pie (rpb=0,52; p<0,05) y una correlación negativa entre el tiempo al caminar y los síntomas en la cadera (rpb=−0,53; p<0,05) en los últimos 12 meses, y el tiempo al correr y los síntomas en la cadera (rpb=−0,43; p<0,05) y en la columna lumbar (rpb=−0,43; p<0,05) en los últimos siete días. El mayor tiempo prolongado de pie se correlacionó con la presencia de síntomas en la región lumbar, mientras que el menor tiempo al caminar y correr se correlacionó con la presencia de síntomas en la cadera y columna lumbar.


ABSTRACT Maintaining a standing posture during work has been associated with musculoskeletal symptoms. Few studies, however, assess the standing time using objective measures. Thus, this study aimed to verify whether standing time at work is associated with lower back and lower limb pain in the last seven days and last 12 months in caregivers of older adults and cleaners. This is a cross-sectional study. Standing time at work was quantified using inclinometers attached to the workers' thigh and spine. Musculoskeletal symptoms were assessed using the Nordic Musculoskeletal Questionnaire. Data were analyzed using the point-biserial correlation coefficient (rpb) between standing time at work and the presence of musculoskeletal symptoms. The analyses were performed using the SPSS software, adopting 5% significance level. Workers spend most of their time standing still (41.3%) or walking (39.3%). The presence of symptoms in the lower back was positively correlated with standing time (rpb=0.52; p<0.05). Walking time was negatively correlated with symptoms at the hip in the last 12 months (rpb=−0.53; p<0.05) and running time with symptoms at the hip, in the last seven days, (rpb=−0.43; p<0.05) and the lower back (rpb=−0.43; p<0.05). Longer standing time was correlated with the presence of symptoms in the lower back. Meanwhile, less time walking and running were correlated with the presence of musculoskeletal symptoms in the hips and lower back.

ABSTRACT

RESUMO Objetivo: conhecer as doenças e sintomas ocupacionais prevalentes dos trabalhadores de um polo gesseiro. Método: trata-se de uma pesquisa de corte transversal, realizada com 67 trabalhadores de uma indústria de gesso na cidade de Grajaú, Maranhão, Brasil. Para coleta de dados, foi utilizado um questionário semiestruturado com perguntas abertas e fechadas. Os dados foram analisados por meio dos procedimentos usuais da estatística descritiva. Resultados: a amostra foi constituída por 67 trabalhadores, sendo 63 do sexo masculino (94%). Os trabalhadores que apresentaram doença relacionada ao trabalho corresponderam a seis (9%) da amostra. Os sintomas que prevaleceram foram: irritação nos olhos; irritação na pele; dores nas articulações; irritação na mucosa nasal; e problemas respiratórios. Conclusão: esses resultados apontam indicadores que podem ser utilizados como parâmetro para subsidiar a tomada de decisão quanto a políticas públicas direcionadas à saúde dos trabalhadores no contexto da mineração do gesso.


ABSTRACT Objective: to know the prevalent occupational diseases and symptoms in workers from a plaster production complex. Method: this is a cross-sectional study conducted with 67 workers of a plaster factory from the city of Grajaú, Maranhão, Brazil. A semi-structured questionnaire with open and closed questions was used for data collection. The data were analyzed by means of the descriptive statistics usual procedures. Results: the sample consisted of 67 workers, of which 63 were male (94%). Six (9%) workers from the sample presented work-related diseases. The predominant symptoms were the following: irritation in the eyes; irritation on the skin; pain in the joints; irritation in the nasal mucosa; and respiratory problems. Conclusion: these results point to indicators that can be used as a parameter to support decision-making regarding public policies aimed at workers' health in the context of gypsum mining.


RESUMEN Objetivo: conocer las enfermedades y los síntomas ocupacionales prevalentes de los trabajadores de un complejo de producción de yeso. Método: investigación de corte transversal realizada con 67 trabajadores de una fábrica de yeso de la ciudad de Grajaú, Maranhão, Brasil. Para recolectar los datos se utilizó un cuestionario semiestructurado con preguntas abiertas y cerradas. Los datos se analizaron por medio de los procedimientos habituales de la estadística descriptiva. Resultados: la muestra estuvo compuesta por 67 trabajadores, de los cuales 63 eran del sexo masculino (94%). Seis (9%) de los trabajadores de la muestra presentaron enfermedades relacionadas con el trabajo. Los síntomas que prevalecieron fueron los siguientes: irritación en los ojos, irritación en la piel, dolores en las articulaciones, irritación en la mucosa nasal y problemas respiratorios. Conclusión: estos resultados apuntan a indicadores que pueden utilizarse como parámetro para sustentar el proceso de toma de decisiones sobre políticas públicas dirigidas a la salud laboral en el contexto de la minería de yeso.

RESUMO Objetivo Determinar o perfil dos acidentes ocupacionais entre cirurgiões-dentistas, envolvendo material biológico, em Fortaleza, CE. Métodos O estudo tem caráter transversal de natureza descritiva e analítica, realizado em Fortaleza, CE, em 2020, a partir de uma amostra de 206 cirurgiões-dentistas. Além das frequências absolutas e percentuais, utilizaram-se testes de Qui-quadrado de Pearson e/ou Razão de verossimilhança, ao nível de significância de 5%. Encontraram-se 120 (58.3%) profissionais que sofreram acidentes e destes 107 (89%) através de lesão percutânea. Resultados Dos cirurgiões-dentistas que sofreram acidentes, envolvendo material biológico, identificou-se associação significativa entre ocorrência de acidentes com cirurgiões-dentistas que possuíam uma especialidade (p<0,001) e entre faixa etária e sexo (p=0,008). A maioria relatou ter sido imunizada contra hepatite B 193 (98%) e buscou testes de confirmação sorológica 101 (52%). A procura por atenção especializada após um acidente foi baixa 45 (38%). Conclusão Cirurgiões-dentistas no exercício do atendimento clínico estão passíveis de sofrerem acidentes. Medidas preventivas são importantes, como a utilização correta de equipamentos de proteção individual.


ABSTRACT Objective This study aimed to determine the profile of occupational accidents among dental surgeons involving biological material, in the city of Fortaleza, CE, Brazil. Methods This is a cross-sectional study of descriptive and analytical nature, conducted in the city of Fortaleza, CE, in 2020, from a sample of 206 dental surgeons. Pearson's Chi-square and/or Likelihood Ratio tests at a 5% of significance level were used as well as absolute and percentage frequencies. Results A total of 120 (58.3%) professionals underwent accidents and 107 (89%) of these suffered from percutaneous injury. Considering dental surgeons who had accidents involving biological material, a significant association was identified between the occurrence of accidents with specialized professionals (p<0.001) and between age group and sex (p=0.008). The majority reported having been immunized against hepatitis B 193 (98%), and 101 (52%) of them sought serological confirmation tests. The demand for specialized care after an accident was low with a total of 45 (38%). Conclusion Dental surgeons in their exercise of clinical care are susceptible to under-going accidents. Preventive measures are important, such as the correct use of personal protective equipment.


RESUMEN Objetivo Determinar el perfil de los accidentes de trabajo que afectan a cirujanos dentistas al utilizar material biológico. Métodos El estudio ha tenido un carácter transversal de tipo descriptivo y analítico. Se hizo en Fortaleza, CE, el 2020, con una muestra de 206 cirujanos dentistas. Más allá de las frecuencias absolutas y porcentuales, se hicieron pruebas de Chi-cuadrado de Pearson y Razón de verosimilitud, al nivel de significación del 5%. Se encontraron 120 (el 58,3%) profesionales que sufrieron accidentes y, de estos, 107 (el 89%) con lesión percutánea. Entre los cirujanos que se han accidentado con material biológico se ha identificado una asociación significativa entre la ocurrencia de accidentes con cirujanos dentistas expertos (p>0,001) y entre la edad y sexo (p=0,008). Resultados La mayoría, 193 (el 98%), ha informado haber recibido inmunización contra hepatitis B y 101 (el 52%) se ha buscado confirmar por serología. La búsqueda por atención médica especializada después de un accidente ha sido baja 45 (el 38%). Conclusión Los cirujanos dentistas en servicio clínico son susceptibles de sufrir accidentes. Medidas de prevención como la utilización correcta del equipo de protección individual son importantes.

ABSTRACT

Objective: To characterize the profile of occupational exposure to pesticides and its impact on women's health. Method: This is a prospective and descriptive study with a quantitative approach. The study population included all patients attended at Francisco Beltrao Cancer Hospital (Ceonc) from May 2015 to December 2018 (n=315). The characterization of their exposure profile was obtained through an interview using a form applied by trained researchers contained 60 questions related to women's pesticide exposure and their health status. The questions referred to past and current occupational pesticide exposure profile, intoxication, and health history from women and their families. All collected data were analyzed in the Statistical Package for the Social Sciences package (SPSS 25.0). Results: A total of 265 individuals (84.2%) have lived some part of their life in the countryside, and 70% were categorized as occupationally exposed to pesticides. Most of them (57%) were directly exposed, mainly by washing pesticide-contaminated clothes and personal protective equipment used in pesticide pulverization without glove protection. Cancer cases in the family were the main disease reported (68.3%). Further, 57% of interviewed women had a breast cancer diagnosis but no significance between breast cancer occurrence and pesticide exposure was observed. Conclusion: The form allowed us to characterize the profile of occupational pesticide exposition in rural women, alerting them to their severe contamination. This approach can be useful to characterize the occupational exposure profiles of rural workers living in other rural regions of Brazil that use pesticides.

Resumo Objetivo: avaliar a efetividade das intervenções direcionadas à prevenção e redução da violência no trabalho sofrida por profissionais de saúde e apoio. Método: revisão sistemática com metanálise realizada em oito bases de dados e na literatura cinzenta. O risco de viés foi realizado por meio das ferramentas da Cochrane e a certeza da evidência pelo Grading of Recommendations Assessment, Development and Evaluation. A análise foi realizada de forma descritiva e pela metanálise, incluindo avaliação da heterogeneidade. Resultados: 11 estudos aleatorizados e quasi-aleatorizados foram elegíveis, dos quais seis estudos (54,5%) implementaram habilidades individuais, quatro multiabordagem (36,4%) e um (9,1%) ações governamentais. Quatro estudos (36,4%) tiveram efeito positivo e significativo na redução da violência. O risco de viés foi classificado como alto ou incerto. A metanálise foi realizada com dois estudos que testaram habilidade individual (grupo intervenção) versus habilidade individual (grupo comparador), porém não houve evidência científica (IC 95%: -0,41 a 0,25, p=0,64) para o desfecho prevenção/redução da violência. Conclusão: esta revisão não obteve alta evidência na prevenção ou redução da violência no trabalho. O número reduzido de ensaios aleatorizados, a falta de estudos com baixo risco de viés e a alta consistência podem ter sido fatores dificultadores para recomendar intervenções efetivas.


Abstract Objective: to assess the effectiveness of the interventions targeted at preventing and reducing the workplace violence suffered by health and support professionals. Method: a systematic review with meta-analysis conducted in eight databases and in the gray literature. Risk of bias was assessed by means of the Cochrane tools and certainty of the evidence, through Grading of Recommendations Assessment, Development and Evaluation. The analysis was performed in a descriptive manner and through the meta-analysis, including a heterogeneity assessment. Results: a total of 11 randomized and quasi-randomized studies were eligible, of which six (54.5%) implemented individual skills, four used a multiple approach (36.4%) and one (9.1%) resorted to governmental actions. Four studies (36.4%) exerted a positive and significant effect on reducing violence. Risk of bias was classified as high or uncertain. The meta-analysis was performed with two studies that tested individual skill (intervention group) versus individual skill (comparator group), although there was no scientific evidence (95% CI: -0.41 - 0.25, p=0.64) for the violence prevention/reduction outcome. Conclusion: this review did not obtain a high level of evidence in the prevention or reduction of workplace violence. The reduced number of randomized trials, the lack of studies with low risk of bias and the high consistency may have been factors that hindered recommending effective interventions.


Resumen Objetivo: evaluar la efectividad de las intervenciones dirigidas a prevenir y reducir la violencia laboral que sufren los profesionales de la salud y de apoyo. Método: revisión sistemática con metanálisis realizada en ocho bases de datos y en la literatura gris. El riesgo de sesgo se evaluó mediante herramientas Cochrane y la certeza de la evidencia mediante el Grading of Recommendations Assessment, Development and Evaluation. El análisis se realizó de forma descriptiva y por metanálisis, e incluyó la evaluación de la heterogeneidad. Resultados: fueron elegibles 11 estudios aleatorios y cuasialeatorios, de los cuales seis estudios (54,5%) implementaron habilidades individuales, cuatro multienfoque (36,4%) y uno (9,1%) acciones gubernamentales. Cuatro estudios (36,4%) tuvieron un efecto positivo y significativo en la reducción de la violencia. El riesgo de sesgo se clasificó como alto o incierto. El metanálisis se realizó con dos estudios que evaluaron la capacidad individual (grupo de intervención) versus la capacidad individual (grupo de comparación), pero no se encontró evidencia científica (IC del 95 %: -0,41 a 0,25, p = 0,64) para el resultado prevención/reducción de la violencia. Conclusión: esta revisión no obtuvo alta evidencia sobre la prevención o reducción de la violencia laboral. El número reducido de ensayos aleatorios, la falta de estudios con bajo riesgo de sesgo y la alta consistencia pueden haber sido factores que dificultaron la recomendación de intervenciones efectivas.

Resumo Objetivo: descrever as características do trabalho e investigar a prevalência e os fatores associados à dor em membros inferiores em trabalhadores da limpeza urbana. Métodos: estudo de corte transversal, realizado na Bahia. Dados coletados entre 2009 e 2010. A dor foi avaliada por meio do Nordic Musculoskeletal Questionnaire, enquanto as demandas psicossociais no trabalho foram medidas pelo Job Content Questionnaire. Também foram avaliadas as demandas físicas, incluindo posturas gerais e manuseio de carga. A análise de regressão logística múltipla foi utilizada para identificar fatores associados à dor em membros inferiores. Resultados: 624 trabalhadores participaram da pesquisa. Constatou-se alta prevalência de dor em membros inferiores nos últimos sete dias (23,7%) e nos últimos doze meses (42,1%), sendo maior entre agentes de limpeza e coletores. A dor em membros inferiores foi associada a: mais de três anos de trabalho na empresa (OR = 1,34); alta exposição ao manuseio de cargas (OR = 1,35); demanda psicológica no trabalho (OR = 1,87); e condicionamento físico insuficiente (OR = 1,67). O trabalho na limpeza urbana envolveu grande sobrecarga física, com pressão de tempo para sua execução. Conclusão: evidenciou-se a necessidade de medidas para redução do manuseio de cargas e de mudanças na organização do trabalho, incluindo pausas durante a jornada e adequação das tarefas ao tempo disponível, a fim de evitar sobrecargas física e psicológica dos trabalhadores.


Abstract Objective: to describe the characteristics of the urban cleaning workers' job and investigate the prevalence and factors associated with pain in their lower limbs. Methods: cross-sectional study carried out in Bahia, Brazil. Data collected between 2009 and 2010. Pain was assessed using the Nordic Musculoskeletal Questionnaire, while psychosocial demands at work were measured using the Job Content Questionnaire. Physical demands were also evaluated, including general postures and cargo handling. We used multiple logistic regression analysis to identify factors associated with lower limb pain. Results: 624 workers participated. There was a high prevalence of pain in the lower limbs in the last seven days (23.7%) and in the last twelve months (42.1%), being higher among cleaning workers and collectors. Lower limb pain was associated with the following: longer than three years working for the company (OR = 1.34); high exposure to cargo handling (OR = 1.35); psychological demand at work (OR = 1.87); and poor physical conditioning (OR = 1.67). Working at urban cleaning involved great physical overload, being produced under time pressure. Conclusion: The results evidenced the need for measures to reduce cargo handling and changes in work organization, including breaks during the day and adjusting tasks to the available time, in order to avoid workers' physical and psychological overload.

ABSTRACT

Objective:To systematically summarize the real experience of nursing students and clinical nurses after needle stick injury exposure, so as to provide the basis for the subsequent development of intervention strategies.Methods:Databases including PubMed, The Cochrane Library, Web of science, Science direct and EBSCO host, PsychINFO, Scopus, CNKI, Wanfang, VIP and CBM were searched about the qualitative research on the real experience of needle stick injury exposure of nursing students and clinical nurses from the establishment of the database to September 2020. The qualitative research quality evaluation standard of Australia JBI evidence-based health care center was used for evaluation, and the result integration method was used for integration.Results:A total of 19 literatures were included, 16 literatures related to interns and junior nurses were included, and 79 research results were extracted. 11 literatures related to senior nurses were included, and 57 research results were extracted. The similar research results were summarized into six new categories, which were summarized into three integrated results, namely, the psychological experience of nurses with needle stick injuries; the impact of needle stick injuries on family life and personal value; and the need for support.Conclusions:We should pay attention to the emotional and emotional experience of interns and clinical nurses after needle stick injury, provide professional psychological counseling, improve the occupational exposure monitoring and security system, and give full social support.

@#Objective To compare and analyze the occupational exposure limit(OEL)of hexavalent chromium compounds in China and foreign countries. Methods The OEL and background information of hexavalent chromium compounds released by nine official and unofficial organizations in seven countries/regions were collected and sorted. The classification,limit levels, formulation principles and compound characteristics labeling of OEL were compared. Results The OEL values published by nine organizations ranged from 0.000 02 to 0.050 00 mg/m3 . Among them,the limits of seven organizations(including China) did not clearly distinguish the soluble types of hexavalent chromium compounds;the limits of eight organizations(including China)were calculated in terms of chromium;seven organizations(including China)have only developed OEL for long-term exposure. The OEL of hexavalent chromium in China was 0.050 00 mg/m3 which is relatively loose. Its carcinogenicity label was consistent with international standards,its sensitization label was consistent with some countries,and its percutaneous absorption label had not been included in the characteristic labeling of limit values. Conclusion It is suggested that the OEL of hexavalent chromium in China should be revised timely,and the systemic adverse effects caused by percutaneous absorption of hexavalent chromium compounds should be focused on. At the same time,it is recommended to systematically adjust the management concepts and principles of all carcinogens.

ABSTRACT

Objective: To quantitatively evaluate the content differences of trace elements in workers with occupational exposure to lead. Methods: In January 2021, relevant literatures on the contents of trace elements in workers with occupational exposure to lead published from 1990 to 2020 were searched through CNKI, Wanfang, VIP, PubMed, web of science and Embase. Screened and extracted the literatures, and evaluated the quality of the included literatures with Newcastle Ottawa Scale. Meta analysis was performed with Review Manager 5.3 software, and standardized mean difference (SMD) and its 95% confidence interval were used as effect indicators. Results: A total of 20 literatures were included, and the quality scores were 5-7. The results of Meta-analysis showed that compared with the control group, the contents of blood zinc (SMD=-1.01, 95%CI: -1.53, -0.49) , hair zinc (SMD=-0.17, 95%CI: -0.33, -0.01) , hair copper (SMD=-0.50, 95%CI: -1.01, 0) , hair iron (SMD=-3.91, 95%CI: -5.80, -2.03) and hair manganese (SMD=-1.09, 95%CI: -2.02, -0.15) in occupational lead exposure group were significantly lower (P<0.05) . Compared with the control group, the content of cobalt in hair of occupational lead exposure group (SMD=1.41, 95%CI: 0.72, 2.10) was higher, and the difference was statistically significant (P<0.05) . There was no significant difference in the contents of blood chromium, blood copper, blood iron, blood manganese, blood selenium and hair nickel between the two groups (P>0.05) . Conclusion: Workers with occupational exposure to lead have abnormal trace elements.

ABSTRACT

Objective: To investigate the detection of suspected occupational diseases and occupational contraindications for benzene workers in Tianjin. Methods: In June 2020, the occupational health inspection data of 16113 benzene workers in 514 enterprises with benzene hazards in 16 municipal districts in Tianjin from January to December 2019 were included in the analysis. Enterprise information included the employer's region, economic type, industry classification and enterprise scale. Occupational health inspection data for benzene workers during their on-the-job period included routine inspection indicators and benzene special inspection indicators. Multivariate unconditional logistic regression was used to analyze the relationship between personal general information, occupational history, enterprise information and suspected benzene poisoning and occupational contraindications of benzene workers. Results: There were 16073 benzene workers in the normal group and 24 in the suspected benzene poisoning group. The detection rate of suspected benzene poisoning in females was higher than that in males (χ(2)=8.26, P=0.004) . There was no significant difference in the detection rates of suspected benzene poisoning among different dimensions such as age, length of service, occupational health inspection institution location, employer location, industry classification, economic type, and enterprise scale (P>0.05) . There were 16073 benzene workers in the normal group and 16 in the benzene contraindication group. The detection rate of benzene contraindications for workers in suburban areas where occupational health inspection institutions were located was higher than that in urban areas (χ(2)=9.71, P=0.002) , and there was no significant difference in the detection rates of contraindications for benzene in other dimensions (P>0.05) . Multivariate logistic regression analysis showed that female benzene workers were more likely to detect suspected benzene poisoning (OR=3.53, 95%CI: 1.57-7.94, P=0.002) ; benzene workers who received physical examination in suburban occupational health inspection institutions (OR=5.81, 95%CI: 1.94-17.42, P=0.002) , the employer's area was in the suburbs (OR=9.68, 95%CI: 1.23-76.07, P=0.031) , and female workers (OR=3.07, 95%CI: 1.13-8.37, P=0.028) , it was easier to detect occupational contraindications. Conclusion: Female benzene workers with employers located in the suburbs have a higher risk of detecting occupational contraindications, and women are more likely to detect suspected benzene poisoning. The management of benzene operations in the production environment of enterprises in the suburbs of Tianjin and the occupational health monitoring of female workers should be strengthened.

ABSTRACT

Objective: To observe the dynamic changes of brainstem locus coeruleus (LC) damage in Parkinson' s disease (PD) -like mice by paraquat (PQ) . Methods: In October 2019, 36 male C57BL/6 mice were randomly divided into the exposure group and the control group, with 18 mice in each group. The mice in the exposure group were given intraperitoneal injection of 15 mg/kg PQ, and the mice in the control group were given intraperitoneal injection of 0.9% saline, twice a week for 8 weeks. Neurobehavioral changes (pole climbing test, swimming test, open field test, tail hanging test, high plus maze test and water maze test) were observed at 4 weeks, 6 weeks and 8 weeks, respectively, and the changes of motor ability, emotion and cognitive function were evaluated. The brain tissue of mice were taken and stained with Hematoxylin-Eosin (HE) to observe the pathological changes of LC. Nissl staining was used to detect the changes of neuronal Nissl bodies in LC. Immunohistochemistry (IHC) staining was used to detect the expression of neuron nuclear antigen (NeuN) , dopamine (DA) neurons and norepinephrine (NE) neuron markers tyrosine hydroxylase (TH) , α-synuclein (α-syn) in substantia nigra (SN) and LC. The expression levels of NeuN, TH and α-syn in the midbrain and brainstem were detected by Western blotting. TUNEL staining was used to detect neuronal apoptosis in LC. Results: Compared with the 4th week of PQ exposure group, the time of pole climbing and swimming immobility were gradually increased, the ratio of open arm residence time of high plus maze test and the number of times of the platform and the residence time of platform quadrant in water maze test were gradually decreased (P<0.05) in the exposure group with the progress of exposure time. The results of HE and Nissl staining showed that the neurons in LC gradually arranged loosely, the nucleus were deeply stained, the cytoplasm was pyknosis, and the number of Nissl bodies gradually decreased (P<0.05) in the exposure group with the progress of exposure time. IHC results showed that the number of NeuN and TH positive cells in SN and LC of mice were gradually decreased, and the positive expression of α-syn was gradually increased (P<0.05) in the exposure group with the progress of exposure time. Western blotting results showed that the expression levels of NeuN and TH in the midbrain and brainstem were gradually decreased, and the expression level of α-syn was gradually increased (P<0.05) in the exposure group with the progress of exposure time. TUNEL staining showed that the apoptosis rates of neurons in LC were gradually increased (P<0.05) in the exposure group with the progress of exposure time. Conclusion: PQ induces progressive damage in the LC area of PD-like mice, which may be caused by the abnormal accumulation of pathological α-syn in the LC area.

ABSTRACT

Objective: To analyze the risk factors of blood-borne occupational exposure among medical staff and explore the relevant intervention measures. Methods: In June 2020, the data of blood-borne occupational exposure and related factors reported by medical staff in a grade Ⅲ, Grade A general hospital from 2011 to 2019 were analyzed by retrospective investigation. Results: Among 431 cases of blood-borne occupational exposure, 69.37% were nurses. It mainly occurred in medical staff with 0-4 years of service, accounting for 63.57%; The main place of occupational exposure was in the ward 47.56%; Sharp instrument injury was the main occupational exposure route 91.65%. Occupational exposure department was mainly surgery department 17.87%; The main source of exposure was hepatitis B virus (HBV) 37.12%, followed by treponema pallidum 20.19%. Statistical analysis results show that: Exposure sites (χ(2)=43.585, P<0.01) , exposure sources (χ(2)=22.693, P<0.01) , treatment methods after exposure (χ(2)=18.866, P<0.01) , Flushing (χ(2)=31.963, P<0.01) and disinfection (χ(2)=14.216, P<0.01) were significantly different. Conclusion: The effective measures to reduce blood-borne occupational exposure are to strengthen occupational protection training of medical staff, standardize operation procedures, strengthen supervision of key groups and departments, improve reporting, monitoring and follow-up systems to realize informatization, and do a good job in risk control.

ABSTRACT: Objective: To estimate the prevalence of occupational and environmental exposure according to sociodemographic factors in cancer patients treated at reference hospitals in the state of Mato Grosso, Brazil. Methods: This is a cross-sectional study on cancer patients aged 18 years or older. The prevalence of exposure to pesticides, asbestos, lead, heavy metals, formaldehyde, benzene, exposure to industrial dust, and handling of other chemical substances were calculated according to sex, age group, and level of education. Results: A total of 1,012 patients were interviewed (55.0% women, 45.6% aged 60 years or older, and 56.8% had less than five years of formal education). Pesticides (22.8%), industrial dust (10.7%), and benzene (10.1%) were the most frequent exposures. Occupational and environmental exposure was higher in men for all evaluated exposures, except for formaldehyde, which was higher in women. Exposure to pesticides, industrial dust, benzene, asbestos, and heavy metals increased with age and were more frequent among those with lower level of education. Conclusion: Approximately one in five cancer patients treated at reference hospitals in Mato Grosso reported having been exposed to pesticides, and one in ten were exposed to industrial dust and benzene, with greater exposure among men, older individuals, and those with lower level of education.


RESUMO: Objetivo: Estimar a prevalência da exposição ocupacional e ambiental segundo fatores sociodemográficos nos pacientes com câncer atendidos nos hospitais de referência do estado de Mato Grosso. Métodos: Estudo transversal com pacientes com câncer de 18 anos ou mais. Foram calculadas as prevalências de exposição a agrotóxicos, amianto, chumbo, metais pesados, formol, benzeno e poeira industrial e de manuseio de outras substâncias químicas conforme sexo, faixa etária e escolaridade. Resultados: Foram entrevistados 1.012 pacientes (55,0% do sexo feminino, 45,6% com 60 anos ou mais, 56,9% com menos de cinco anos de escolaridade). Agrotóxicos (22,8%), poeira industrial (10,7%) e benzeno (10,1%) foram as exposições de maior frequência. A exposição ocupacional e ambiental foi maior no sexo masculino para todas as exposições avaliadas, exceto para o formol, que foi maior no sexo feminino. A exposição a agrotóxico, poeira industrial, benzeno, amianto e metais pesados aumentou com a idade e foi mais frequente entre aqueles de menor escolaridade. Conclusão: Aproximadamente um em cada cinco pacientes com câncer atendidos em hospitais de referência de Mato Grosso relatou ter sido exposto aos agrotóxicos, e um a cada dez foi exposto a poeira industrial e benzeno, sendo maior a exposição entre os indivíduos do sexo masculino, mais velhos e de menor escolaridade.

Resumo Objetivo: fornecer uma visão geral das associações entre exposição ocupacional e risco da ocorrência ou morte por câncer. Métodos: esta revisão guarda-chuva da literatura utilizou as bases Medline e Web of Science. A partir de protocolo de busca, foram incluídas metanálises para diversas circunstâncias ocupacionais e cânceres selecionados que possuíssem algum nível de evidência para associação com ocupação. Resultados: foram incluídas 37 metanálises, abrangendo 18 localizações de câncer. Considerando a avaliação da heterogeneidade dos estudos, da qualidade da evidência e da força de associação, obteve-se evidências altamente sugestivas de associações entre exposição a solvente e mieloma múltiplo; amianto e câncer de pulmão; hidrocarbonetos e câncer de trato aerodigestivo superior; e estresse ocupacional e câncer colorretal. Conclusão: há evidências robustas para associar exposições ocupacionais e tipos de câncer não previstos, inicialmente, nas orientações de vigilância do câncer relacionado ao trabalho no Brasil. Permanecem lacunas sobre exposições de grande relevância, que carecem de metanálises mais consistentes, por exemplo, exposição a poeiras inorgânicas e câncer de pulmão e mesotelioma; exposição a solventes e tumores hematológicos. Evidências de câncer em outras regiões anatômicas foram menos robustas, apresentando indícios de incerteza ou viés.


Abstract Objective: to provide an overview of the associations between occupational exposure and risk of occurrence or death from cancer. Methods: this umbrella review used the Medline and Web of Science databases. Based on the search protocol, meta-analysis was included for several occupational circumstances and selected cancers that had some level of evidence associated with the occupation. Results: 37 meta-analysis were included, covering 18 cancer locations. By assessing the heterogeneity of studies, quality of evidence, and strength of association, results highly indicated associations between solvent exposure and multiple myeloma, asbestos and lung cancer, hydrocarbons and upper aerodigestive tract cancer, occupational stress and colorectal cancer. Conclusion: robust evidence shows an association between occupational exposures and types of cancer not initially foreseen in the guidelines for work-related cancer surveillance in Brazil. Gaps in relevant exposures require further research and more consistent meta-analysis, including: exposure to inorganic dust and lung cancer and mesothelioma; solvents and hematological tumors. Evidence of cancer in other anatomical regions was less robust, showing signs of uncertainty or bias.

ABSTRACT Objective To analyze the relationship between occupation and lung cancer in patients at a national referral hospital for cancer care in southern Brazil. Method Hospital-based case-control study conducted between February and October 2019. Occupational histories were coded according to international classifications and translated into occupations associated with lung cancer (List A and B). The odds ratios were adjusted for smoking, with a 95% confidence interval, calculated by conditional logistic regression. Results 99 cases and 227 controls were included. Among men, the occupation of painters (list A) was associated with lung cancer (OR = 14.3; 95% CI: 1.8-116.5), there were no occupations in list B associated with lung cancer. In women, no increased risks were found. Conclusions It has been shown that exposure to occupational carcinogens among repair and construction painters increases the risk of lung cancer.

Se codificaron los antecedentes laborales según clasificaciones internacionales y se tradujeron a ocupaciones asociadas con el cáncer de pulmón (Lista A y B). Las odds ratios se ajustaron para el tabaquismo, con un intervalo de confianza del 95%, calculada por regresión logística condicional. Resultados se incluyeron 99 casos y 227 controles. Entre los hombres, la ocupación de pintores (lista A) se asoció con el cáncer de pulmón (OR = 14,3; IC del 95%: 1,8-116,5), no hubo ocupación de la lista B asociada con el cáncer de pulmón. En las mujeres, no se encontraron mayores riesgos. Conclusiones Se ha demostrado que la exposición a carcinógenos ocupacionales entre los pintores de la reparación y la construcción aumenta el riesgo de cáncer de pulmón.

Entre os homens a ocupação de pintores (lista A) estava associada ao câncer de pulmão (OR= 14,3; IC 95%: 1,8-116,5), não houve ocupações da lista B associadas ao câncer de pulmão. Nas mulheres não foram encontrados riscos aumentados. Conclusões Evidenciou-se que a exposição aos carcinógenos ocupacionais entre pintores de reparo e da construção civil aumentam o risco de câncer de pulmão.

ABSTRACT

Abstract The aim of the present study was to summarize the prevalence of common mental disorders (CMD) among Brazilian workers through a systematic review and meta-analysis. Searches were conducted in SciELO, LILACS, PubMed, Scopus, and Web of Science. Articles were included if they had; an observational design, a sample of Brazilian workers, used a validated instrument and cut-off to assess CMD, and provided the prevalence value. A random-effect meta-analysis using professional categories as subgroups and a meta-regression were conducted. In total, 89 studies were included, with a total of 56,278 workers from 26 professional categories. The overall pooled prevalence of CMD was 0.30 (95%CI: 0.27-0.34), varying from 0.07 to 0.58. Professional categories that presented higher prevalences of CMD were: Prostitutes 0.58 (95%CI: 0.51-0.65), Social Educators 0.54 (95%CI: 0.50-0.59), Banking Workers 0.45 (95%CI: 0.44-0.47), Ragpickers 0.45 (95%CI: 0.40-0.49), and Teachers 0.40 (95%CI: 0.32-0.48). No other variable in addition to profession was associated with prevalence of CMD in the meta-regression analysis. Workers from the most affected professional categories should be monitored to prevent social, occupational, and health impairment from CMD.


Resumo O objetivo do presente estudo foi descrever a prevalência de transtornos mentais comuns (TMC) em trabalhadores brasileiros por meio de uma revisão sistemática e meta-análise. As buscas foram realizadas no SciELO, LILACS, PubMed, Scopus e Web of Science. Artigos observacionais, com amostra de trabalhadores brasileiros, que utilizaram instrumento e ponto de corte validados e que apresentaram valor de prevalência foram incluídos. Foram realizadas a meta-análise com efeito aleatório utilizando as categorias profissionais como subgrupos e a meta-regressão. Foram incluídos 89 estudos, com um total de 56.278 trabalhadores de 26 categorias profissionais. A prevalência global de TMC foi de 0,30 (IC95%: 0,27-0,34), variando de 0,07 a 0,58. As categorias profissionais que apresentaram maiores valores de TMC foram: prostitutas 0,58 (IC95%: 0,51-0,65), educadores sociais 0.54 (IC95%: 0,50-0,59), bancários 0,45 (IC95%: 0,44-0,47), coletores 0,45 (IC95%: 0,40-0,49) e professores 0,40 (IC95%: 0,32-0,48). Nenhuma outra variável além da profissão se associou ao TMC na meta-regressão. Trabalhadores das categorias profissionais mais afetadas por TMC devem ser monitorados para prevenir os prejuízos sociais, ocupacionais e de saúde associados aos TMC.

ABSTRACT

Objetivo: traçar o perfil de saúde dos servidores públicos de instituição de ensino localizada no Rio de Janeiro, a partir dos resultados dos exames periódicos de saúde. Método: utilizou-se abordagem quantitativa e transversal, com dados coletados em uma instituição pública federal de ensino. Resultados: verificou-se o risco para desenvolvimento de doenças crônicas não transmissíveis, dado o histórico familiar aliado ao estilo de vida dos participantes. A atividade física referida não pôde ser associada à diminuição de risco de doenças cardiovasculares e, por isso, pode não estar sendo efetiva sua contribuição para a saúde de seus praticantes. Quanto à exposição ocupacional, foram identificadas alterações nos exames de audiometria, videolaringoestroboscopia e Gama GT dos servidores docentes. Conclusão: torna-se fundamental consolidar as ações voltadas para os servidores públicos, enquanto ferramenta para embasar ações de promoção e proteção da saúde dos trabalhadores e para o fortalecimento de políticas públicas voltadas para o tema


Objective: to draw the health profile of public servants of an educational institution located in Rio de Janeiro, based on the results of periodic health examinations. Method: a quantitative and cross-sectional approach was used, with data collected from a federal public educational institution. Results: the risk for developing non-communicable chronic diseases was verified, given the family history combined with the participants' lifestyle. The referred physical activity could not be associated with a reduced risk of cardiovascular diseases and, therefore, its contribution to the health of its practitioners may not be effective. Regarding occupational exposure, changes in audiometry, videolaryngostroboscopy and Gama GT exams of the teaching staff were identified. Resultados: se verificó el riesgo de desarrollar enfermedades crónicas no transmisibles, dada la historia familiar combinada con el estilo de vida de los participantes. La actividad física referida no podría asociarse con un riesgo reducido de enfermedades cardiovasculares y, por lo tanto, su contribución a la salud de sus profesionales puede no ser efectiva. Introduction: les déchets d'équipements électriques et électroniques (D3E), avec leurs enjeux sanitaires et environnementaux, sont devenus un problème de santé publique exposant les travailleurs aux composés d'éléments toxiques responsables de (2ème expertise) troubles respiratoires. Objectif: Etudier la relation entre l'exposition professionnelle aux D3E et la survenue des problèmes respiratoires à Bamako en 2019. Matériel et Méthodes : Il s'agissait d'une étude transversale, analytique de 6 mois, menée dans la ville de Bamako en 2019 et concernait 159 manipulateurs de D3E. Les données ont été collectées sur tablette et les analyses univariées et multivariées réalisées sur SPSS version 22 au seuil de significativité de 0,05 avec présentation des Odds Ratio (OR) et leurs intervalles de confiance à 95% (OR[IC95%]). Résultats : La prévalence en général des troubles respiratoires était de 67,3% chez les manipulateurs des D3E. On notait que le retrait du revêtement des fils électroniques (OR[IC95%] =25,93[2,30;291,53]), la perte de poids récente en dehors d'un régime (OR[IC95%] =7,43[1,68;32,85]) et la rareté de lavage des mains avec du savon après les toilettes (OR[IC95%] =11,50[2,07;63,89]) représentaient un risque plus élevé de problèmes respiratoires. Conclusion :Ces résultats montrent que les manipulateurs de D3E ont beaucoup de comportement à risque des problèmes respiratoires. Cette étude pourrait servir à la mise en œuvre de stratégies visant à réduire l'impact des D3E


Introduction: waste electrical and electronic equipment (WEEE), with its health and environmental issues, has become a public health problem exposing workers to toxic compounds causing respiratory problems. Objective: To study the relationship between professional exposure to WEEE and the occurrence of respiratory problems in Bamako in 2019. Material and Methods: This was a 6-month cross-sectional, analytical study carried out in the city of Bamako in 2019 and involved 159 WEEE manipulators. Data were collected using a tablet (2). Univariate and multivariate analyzes were performed using SPSS version 22 with the significance level of 0.05. Odds Ratio (OR) were presented with their 95% confident interval (OR [95%CI]). Results: The overall prevalence of respiratory disorders was 67.3% among WEEE handlers. It was noted that the removal of the coating of electronic wires (OR [95%CI] =25.93[2.30;291.53]), recent weight loss outside of a diet (OR [95% CI] =7,43[1.68;32.85]) and the infrequent hand washing with soap after the toilet (OR [95% CI] =11.50[2.07; 63.89]) represented a higher risk of (2) breathing problems. Conclusion: These results show that D3E handlers have a lot of risky behavior for respiratory problems. This study could be used to implement strategies to reduce the impact of D3E

Resumo Objetivo Apresentar o desenvolvimento e a validação de um aplicativo para orientar profissionais da saúde sobre paramentação e desparamentação dos Equipamentos de Proteção Individual no contexto da pandemia da COVID-19, bem como as orientações a esse respeito que eles são capazes de passar ao paciente em visita domiciliar. Métodos A validação do aplicativo foi realizada com 55 profissionais de saúde (enfermeiros, fisioterapeutas e médicos) que estavam na linha de frente de combate à COVID-19 na Atenção Domiciliar, utilizando-se técnica Delphi. Para a análise de dados, foram adotados Índice de Validade de Conteúdo e coeficiente alfa de Cronbach. Resultados A maioria dos juízes avaliou o aplicativo entre inadequado a totalmente adequado na primeira avaliação. Após correções de acordo com as sugestões, o aplicativo foi reavaliado como adequado a totalmente adequado. A média do coeficiente alfa de Cronbach foi de 0,942, caracterizando consistência interna do instrumento excelente. O Índice de Validade do Conteúdo na primeira avaliação variou entre 0,935 e 0,939, e, na segunda avaliação, foi de 1,0. Conclusão O aplicativo orienta COVID-19 foi validado por profissionais que estavam na linha de frente no combate à COVID-19 com consenso entre os juízes na segunda avaliação.


Resumen Objetivo Presentar la elaboración y validación de una aplicación para orientar a profesionales de la salud sobre colocación y retiro de los Equipos de Protección Personal en el contexto de la pandemia del COVID-19, así como instrucciones sobre este tema que puedan brindarle al paciente en visita domiciliaria. Métodos La validación de la aplicación fue realizada con 55 profesionales de la salud (enfermeros, fisioterapeutas y médicos) que estaban en la línea de frente de combate contra el COVID-19 en la atención domiciliaria y se utilizó el método Delphi. Para el análisis de datos, se adoptó el Índice de Validez de Contenido y coeficiente alfa de Cronbach. Resultados La mayoría de los jueces evaluó la aplicación entre inadecuada y totalmente adecuada en la primera evaluación. Luego de las correcciones basadas en las sugerencias, la aplicación fue revaluada de adecuada a totalmente adecuada. El promedio del coeficiente alfa de Cronbach fue de 0,942, que caracterizó una consistencia interna excelente del instrumento. En la primera evaluación, el Índice de Validez de Contenido varió entre 0,935 y 0,939, y en la segunda evaluación fue de 1,0. Conclusión La aplicación orienta COVID-19 fue validada por profesionales que estaban en la línea de frente de combate contra el COVID-19, con consenso entre los jueces en la segunda evaluación.


Abstract Objective To present the development and validation of an application to guide health professionals on Personal Protective Equipment donning and doffing in the context of the COVID-19 pandemic, as well as the guidelines in this regard that they are able to pass on to patients in home visit. Methods The application validation was performed with 55 health professionals (nurses, physiotherapists and physicians) who were on the front line to combat COVID-19 in home care, using the Delphi technique. For data analysis, Content Validity Index and Cronbach's alpha coefficient were adopted. Results Most judges assessed the application from inadequate to fully adequate in the first assessment. After corrections according to suggestions, the application was reassessed as adequate to fully adequate. The mean Cronbach's alpha coefficient was 0.942, characterizing the instrument's internal consistency. Content Validity Index in the first assessment ranged from 0.935 to 0.939, and in the second assessment it was 1.0. Conclusion The Orienta COVID-19 application was validated by professionals who were on the front line in the fight against COVID-19 with consensus among judges in the second assessment.
