ABSTRACT

Resumen Introducción: los fisioterapeutas y fonoaudiólogos como profesionales de la salud generan impacto en el rol educativo realizando acciones dentro de sus competencias de detección temprana y oportuna de las alteraciones que se presentan frecuentemente en educación inicial y preescolar a nivel de los procesos motores y comunicativos. Objetivo: realizar una reflexión sobre el rol del fisioterapeuta y el fonoaudiólogo en la educación inicial y preescolar de niños en Colombia. Reflexión: es necesaria la inclusión de los fisioterapeutas y los fonoaudiólogos en el sector educativo, formando grupos interdisciplinares en conjunto con los docentes, familias, e instituciones educativas con el fin de impactar positivamente en el bienestar comunicativo y desarrollo motor de la población que se encuentra en educación inicial y preescolar. Conclusión: se resalta la importancia de incluir a los fisioterapeutas y fonoaudiólogos en el equipo interdisciplinar en el sector educativo, con el fin de abordar de manera temprana las dificultades que puedan generarse a nivel comunicativo y en el desarrollo motor.


Abstract Introduction: Health professionals such as physiotherapists and speech therapists, within their competences, generate an impact on the educational role by carrying out early and opportune detection actions of the alterations that frequently occur in initial and preschool education at the level of motor and communication processes. Objective: To make a reflection on the role of the physiotherapist and speech therapist in the initial and preschool education of children in Colombia. Reflection: It is important to highlight and recognize the importance of including physiotherapists and speech therapists in the education sector in such a way that it can impact the population that is in initial and preschool education, forming interdisciplinary groups together with teachers, families, and schools, to impact the communicative well-being and motor development. Conclusion: When making the reflection, the importance of including physiotherapists and speech therapists in the interdisciplinary team of the educational sector is highlighted, to approach the difficulties that may be generated at the communicative level and in motor development in an early stage.

RESUMEN Introducción: Las lesiones musculares del dorsal ancho (DA) son poco comunes y se producen casi exclusivamente en atletas de alto nivel competitivo. Hasta la fecha, no encontramos reportes de casos similares que describan de manera detallada el proceso de rehabilitación. El objetivo de este reporte de caso es describir la evaluación y el tratamiento kinésico de una atleta de élite de CrossFit en un consultorio privado de kinesiología. Presentación del caso: Una paciente femenina de 28 años, atleta de élite de CrossFit, sufrió una lesión muscular del DA durante una competencia. Se realizó una ecografía 72 h después, la cual confirmó la lesión. El tratamiento kinésico posterior fue activo y se basó principalmente en la progresión de diferentes ejercicios terapéuticos. Se realizaron diversas evaluaciones para monitorear la adaptación de la atleta (fuerza isométrica del DA, cuestionarios autorreportados y valoración funcional de la extremidad superior). Conclusión: La óptima progresión de los ejercicios durante el tratamiento, junto con una comunicación fluida entre la atleta, el entrenador y el kinesiólogo, permitió una vuelta a los entrenamientos diarios con el mismo nivel de cargas, intensidad y volumen que tenía previamente. La paciente regresó a la competencia a los 45 días del alta kinésica.


ABSTRACT Introduction: Latissimus dorsi (LD) muscle injuries are rare and primarily occur in high-level competitive athletes. To date, no similar case reports detailing the rehabilitation process have been found. The objective of this case report is to describe the assessment and physical therapy treatment of an elite CrossFit athlete at a private physical therapy clinic. Case presentation: A 28-year-old female patient, an elite CrossFit athlete, suffered an LD muscle injury during a competition. An ultrasound performed 72 h later confirmed the injury. Subsequent physical therapy treatment was active and primarily focused on the progression of various therapeutic exercises. Several assessments were conducted to monitor the athlete's adaptation (LD isometric strength test, self-reported questionnaires, and functional assessment to the upper extremity). Conclusion: The optimal progression of exercises during treatment, along with fluid communication among the athlete, coach, and physiotherapist, allowed a return to daily training with the same level of loads, intensity, and volume as before the injury. The patient returned to competition 45 days after discharge from physiotherapy.

ABSTRACT

Introducción: la salud mental es crucial para el bienestar y el rendimiento académico de los estudiantes universitarios. Los estudiantes de Kinesiología y Fisioterapia enfrentan desafíos únicos que pueden afectar su salud mental. Objetivo: determinar la frecuencia de trastornos mentales comunes en estudiantes de kinesiología y fisioterapia de la Universidad Nacional de Asunción. Metodología: se trata de un estudio observacional, descriptivo y transversal. La muestra no probabilística estuvo compuesta por 150 estudiantes de Kinesiología y Fisioterapia de la Universidad Nacional de Asunción. Se recogieron datos sociodemográficos, académicos, ocupacionales, de estilo de vida y psicopatológicos mediante la Escala de Depresión, Ansiedad y Estrés - 21 (DASS-21). El análisis se realizó mediante Jamovi con regresión logística binomial multivariada. Resultados: la frecuencia de depresión, ansiedad y estrés fue del 61,3 %, 72 % y 54,7 %, respectivamente. Los factores importantes para la depresión incluyeron ser un estudiante irregular, trabajar y no realizar actividad física. Para la ansiedad, fueron significativos ser estudiante irregular y ser mujer. Respecto al estrés, se encontraron relevantes ser estudiante irregular, trabajar y no realizar actividad física. Los modelos resultantes tuvieron clasificaciones correctas del 90,2 %, 88 % y 72 % para depresión, ansiedad y estrés, respectivamente. Conclusión: Este estudio reveló una alta frecuencia de depresión, ansiedad y estrés entre estudiantes de kinesiología y fisioterapia, lo que se asoció significativamente con irregularidad académica, empleo y falta de actividad física. Estos hallazgos subrayan la necesidad de implementar estrategias de intervención y programas de apoyo que aborden estos factores de riesgo para promover la salud mental y el bienestar de los futuros profesionales de la salud.


Introduction: mental health is crucial for university students' well-being and academic performance. Kinesiology and Physical Therapy students face unique challenges that can affect their mental health. Objective: to determine the frequency of common mental disorders in kinesiology and physical therapy students at the Universidad Nacional de Asunción. Methodology: this was an observational and descriptive cross-sectional study. The nonprobabilistic sample included 150 Kinesiology and Physiotherapy students from the Universidad Nacional de Asunción. Sociodemographic, academic, occupational, lifestyle, and psychopathological data were collected using the Depression, Anxiety and Stress Scale - 21 (DASS-21). The analysis was performed using Jamovi with multivariate binomial logistic regression. Results: the frequency of depression, anxiety, and stress was 61.3 %, 72 %, and 54.7 %, respectively. The significant factors for depression included being an irregular student, working, and not engaging in physical activity. For anxiety, being an irregular student and being a woman were significant. Regarding stress, being an irregular student, working, and not engaging in physical activity were found to be relevant. The resulting models had correct classifications of 90.2 %, 88 %, and 72 % for depression, anxiety, and stress, respectively. Conclusion: this study revealed a high frequency of depression, anxiety, and stress among kinesiology and physical therapy students, which was significantly associated with academic irregularity, employment, and a lack of physical activity. These findings underscore the need to implement intervention strategies and support programs that address these risk factors in order to promote the mental health and well-being of future health professionals.

Objetivo:analizar la evidencia de estudios previos sobre las diferentes alternativas de tratamientos con el uso de agentes físicos y técnicas manuales utilizados en la fisioterapia para la ingurgitación mamaria a nivel internacional. Material y método:estudio de revisión sistemática con meta-análisis según el pro-tocolo prisma. Búsqueda en las bases de datos de Scopus y Medline a través de PubMed, publicados desde el 01 de enero de 2015 hasta el 31 de diciembre de 2021. La estrategia de búsqueda empleó los siguien-tes términos: breastengorgement, treatment, breastfeeding, six-pointengorgementscale, physiotherapy, breastcancer, physicaltherapy. Los 5 estudios elegidos para esta revisión sistemática fueron valorados con la Escala pedro para conocer la calidad metodológica. Resultados:diferencias medias estandarizadas oscilaron entre 0,5959 y 2,7373, la mayoría de las estimaciones positivas. El resultado promedio difirió significativamente de cero (z = 3,5686, p = 0,0004). Según prueba Q, los resultados reales parecen ser heterogéneos (Q (5) = 23,2212, p = 0,0003, tau² = 0,4759, I² = 83,6163%). Intervalo de predicción del 95 % para los resultados reales viene dado por -0,3733 a 2,5931. Un intervalo de predicción del 95 % para los resultados reales viene dado por -3,6762 a 11,5933. Conclusiones: las técnicas estudiadas disminuyen el dolor y la congestión mamaria, sin embargo, se ha visto que la combinación entre ellas puede favorecer aún más la mejora de los mismos. The search strategy employed the following terms: breast engorgement, treatment, breastfeeding, six-point engorgement scale, physiotherapy, breast cancer, Physical therapy. The 5 stu-dies chosen for this systematic review were assessed with the PEDro Scale for methodological quality. Results: Standardized mean differences ranged from 0.5959 to 2.7373, most of the positive estimates. The average result differed significantly from zero (z = 3.5686; p = 0.0004). According to Q test, the actual results appear to be heterogeneous (Q(5) = 23.2212; p = 0.0003; tau² = 0.4759; I² = 83.6163%). The 95% prediction interval for the actual results is given by −0.3733 to 2.5931. A 95% prediction interval for the actual results is given by −3.6762 to 11.5933. Conclusions: The techniques studied decrease breast pain and engorgement; however, it has been seen that the combination between them can further favor their improvement. When applied in women with engorgement, they generated short and long term benefits in reducing pain and breast engorgement

ABSTRACT

Introduction: To prevent vaginal stenosis, the use of a vaginal dilator is recommended. Objective: To analyze sociodemographic data, gynecological conditions and the use of vaginal dilator after pelvic brachytherapy. Materials and Methods: Cross-sectional, retrospective study, period 2016-2020, collected between October/2020 and February /2021, from records of women with gynecological cancer treated with brachytherapy at the Centro de Pesquisa Oncológicas (Brazil). The variables included sociodemographic data and gynecological conditions in following the treatment. In the analysis, descriptive statistics, chi-squaretest, Fisher's exact test and Mann-Whitney test were applied. Results: 519 patients records were included in the investigation; the analyzes showed significant associations between the topography and staging (p<0.001), education (p=0.004) and age (p<0.001); the comparison between the distribution of the ionizing radiation dose showed a difference with the continued sexual relationship category (p=0.006); the comparison between the proportions of continued sexual relationship and using a vaginal dilator was significant (p<0.001); 49.10% (131) adhered to the use of vaginal dilator; 24.50% (127) are not sexually active and do not adhere to the use of the dilator. Discussion: It is evident that social and gynecological conditions interfere with the presence of vaginal stenosis and the use of a vaginal dilator after pelvic brachytherapy. Conclusions: The adherence found in the use of dilator affirms the contributions and the need for health education by nurses and physicaltherapists during and following the treatment.

ABSTRACT

Durante a pandemia de covid-19, os profissionais de saúde que atuaram na linha de frente viveram um cenário desafiador, com constante risco de infecção pelo SARS-CoV-2, uma demanda de trabalho aumentada e muitas incertezas. O objetivo deste estudo foi analisar as repercussões da covid-19 na saúde de fisioterapeutas baianos atuantes na linha de frente no ambiente hospitalar. Fisioterapeutas baianos participaram deste estudo, respondendo questões referentes aos seus dados sociodemográficos, à qualidade do sono e aos sintomas de ansiedade e depressão, através de um formulário eletrônico (Google Forms). Responderam ao formulário 38 fisioterapeutas residentes em Salvador (BA). A metade deles atua em Unidades de Terapia Intensiva (UTI) e, destes, 65% são trabalhadores do setor privado. Além disso, 68% foram infectados pelo SARS-CoV-2 uma ou mais vezes. O constante medo de infectar seus familiares foi declarado por 77,3% deles e, após o expediente, 36% dos profissionais apresentavam exaustão emocional; 34,2%, estresse psicológico; e 26%, cansaço físico. Os fisioterapeutas baianos relataram que durante a pandemia de covid-19 houve grandes desafios no local de trabalho, como o medo de adoecer e de transmitir a doença e, sobretudo, o medo da morte. O cansaço e o estresse psicológico foram constantes na prática profissional, mas, apesar disso, a maioria dos fisioterapeutas sentiu satisfação por ter atuado de forma tão indispensável em um momento delicado e único.


During COVID-19, health services and professionals who worked on the front line experienced a challenging scenario with a constant risk of infection by SARS-CoV-2, an increased demand for work, and many uncertainties. This study analyzed the repercussions of COVID-19 on the health of Bahian physical therapists working on the hospital front line. Bahian physical therapists participated in this study, answering questions related to their sociodemographic data, sleep quality, and symptoms of anxiety and depression by an electronic form (Google Forms). Overall, 38 physiotherapists living in Salvador (Bahia) answered this form. In total, 50% worked in Intensive Care Units (ICUs); 65%, in the private sector, and 68% suffered infection from SARS-CoV-2 one or more times. This study found that 77.3% of participants reported the constant fear of infecting their family members and 36% showed emotional exhaustion after working hours; 34.2%, psychological stress; and 26%, physical fatigue. Bahian physical therapists reported that COVID-19 brought major confrontations in the workplace, such as the fear of getting sick and transmitting the disease and, above all, death. Fatigue and psychological stress remained constant in the professional practice, but, despite this, most physical therapists felt satisfied for having acted in such an indispensable way in a delicate and unique moment.


Durante la pandemia de la COVID-19, los profesionales de la salud que trabajaban en primera línea vivieron un escenario desafiante, con un riesgo constante de infección por SARS-CoV-2, una mayor demanda de trabajo y muchas incertidumbres. El objetivo de este estudio fue analizar las repercusiones de la COVID-19 en la salud de los fisioterapeutas de Bahía, Brasil, que trabajan en primera línea en el ambiente hospitalario. En este estudio participaron 38 fisioterapeutas residentes en Salvador (Bahía, Brasil) que respondieron a un cuestionario electrónico (Google Forms) con preguntas relacionadas a sus datos sociodemográficos, calidad del sueño y síntomas de ansiedad y depresión. El 50% de ellos trabajan en Unidades de Cuidados Intensivos (UCI) y, de estos, el 65% pertenecen al sector privado. El 68% se ha infectado con el SARS-CoV-2 una o más veces. El miedo constante a contagiar a sus familiares fue declarado por el 77,3% de ellos, y después de la jornada laboral, el 36% de los profesionales presentaron agotamiento emocional; el 34,2%, estrés psicológico; y el 26%, fatiga física. Los fisioterapeutas de Bahía relataron que durante la pandemia de la COVID-19 hubo grandes retos en el lugar de trabajo, como el miedo a enfermarse y transmitir la enfermedad y, sobre todo, el miedo a la muerte. Aunque el cansancio y el estrés psicológico fueron constantes en la práctica profesional, la mayoría de los fisioterapeutas sintieron satisfacción por haber actuado de manera tan indispensable en un momento delicado y único.

ABSTRACT

La Kinesiología chilena ha tenido un desarrollo científico sostenido, particularmente durante las últimas décadas. Este desarrollo ha promovido progresivamente el posicionamiento de kinesiólogos y kinesiólogas como profesionales de la salud dentro de la comunidad, los cuales tienen como fundamento el estudio y aplicación de conocimiento relacionado al movimiento humano. Sin embargo, este desarrollo no ha sido azaroso, sino más bien ha respondido a la creciente actividad científica de los integrantes de la comunidad kinesiológica, además de la cada vez más indispensable utilización de información científica para la toma de decisiones clínicas. Por tanto, el presente artículo discute dos formas de analizar el desarrollo de las disciplinas científicas, y plantea que la kinesiología puede considerarse un programa de investigación, definición desde la cual se pueden analizar los desafíos actuales y futuros respecto al dinamismo necesario de la ciencia y el cómo la kinesiología se relaciona con otras disciplinas para potenciar su crecimiento, sin caer en la redundancia y/o repetición disciplinar.


The physical therapy in Chile has had sustained scientific development, particularly during the last decades. This development has progressively promoted the positioning of physical therapists as health professionals within the community, whose foundation is the study and application of knowledge related to human movement. However, this development has not been random, but rather has responded to the growing scientific activity of members of the physical therapy community, in addition to the essential use of scientific information for clinical decision making. Therefore, this article discusses two ways of analyzing the development of scientific disciplines and proposes that physical therapy can be considered a research program, a definition from which current and future challenges can be analyzed regarding the necessary dynamism of science, and how physical therapy relates to other disciplines to enhance their growth, without falling into professional redundancy and/or repetition.

Introducción: los niños con artritis idiopática juvenil (AIJ) experimentan períodos de dolor e inmovilidad que afectan sus capacidades condicionales. Objetivos: describir los valores de referencia para el 1-minute Sit-to-Stand Test(1-STS; test de 1 minuto de sentarse y pararse) en niños con AIJ como evaluación de la capacidad aeróbico-funcional y de la fuerza muscular de los miembros inferiores (MMII).Materiales y métodos: se realizó un estudio observacional que incluyó a 15 niños con AIJ de entre 5 y 16 años. Se evaluó su rendimiento en el 1-STS. Resultados: se encontró una correlación positiva significativa entre el 1-STS y el test de la marcha de 6 minutos (r=0,56; p=0,03), como con el índice de capacidad funcional (CAPFUN) (r=0,54; p=0,03). No se observaron correlaciones significati-vas entre el Childhood Health Assessment Questionnaire (CHAQ) y el 1-STS (r=-0,21; p=0,44), tampoco con el Juvenile Arthritis Disease Activity Score (JADAS-10) (p=0,83). Conclusiones: el 1-STS parece prometedor para medir la capacidad aeróbi-co-funcional y la fuerza muscular de los miembros inferiores en niños con AIJ oligoarticular.


Introduction: children with juvenile idiopathic arthritis (JIA) experience periods of pain and immobility that affect their physical capacities. Objectives: to describe reference values for the 1-minute sit to stand test (1-STS) in children with JIA as an assessment of aerobic-functional capacity and lower limb muscle strength.Materials and methods: an observational study was conducted, including 15 children with JIA aged between 5 and 16 years. Their performance in the 1-STS was assessed. Results: a significant positive correlation was found between the 1-STS and the 6-Minute Walk Test (r=0,56; p=0,03), as well as with the Functional Capacity Index (CAPFUN) (r=0,54; p=0,03). No significant correlations were observed between the Childhood Health Assessment Questionnaire (CHAQ) and the 1-STS (r=-0,21; p=0,44), nor with the Juvenile Arthritis Disease Activity Score (JADAS-10) (p=0,83). Conclusions: the 1-STS appears promising for assessing aerobic-functional capacity and lower limb muscle strength in children with oligoarticular JIA.

INTRODUCTION: Hospitalization often causes mobility difficulties and hinders daily activities. Progressive mobilization of patients in intensive care units (ICUs) is safe and linked to better clinical and functional outcomes. OBJECTIVE: To assess the perception of a multidisciplinary team in the ICUs of a university hospital regarding early mobilization (EM). METHODS: A prospective observational study was conducted using data collected from professionals and students in the ICU of Clementino Fraga Filho University Hospital at the Federal University of Rio de Janeiro between June and December 2019. Data on EM perception were collected using a questionnaire. Descriptive statistics and Fisher's exact tests were used to analyze the differences between the professional categories. RESULTS: In comparison to physiotherapists (88%), a smaller percentage of physicians (37.5%) and nurses (50%) reported that patients on mechanical ventilation (MV) are mobilized within 48 hours (P80%), with the main perceived barriers being the availability of professionals (58%), the clinical condition of patients (55%), and patients undergoing procedures (45%). CONCLUSION: In a university hospital without an established EM protocol, the multidisciplinary team showed satisfactory knowledge and perceptions of EM. However, creating institutional protocols and guidelines is essential to engage multidisciplinary teams in implementing EM and overcoming barriers.


INTRODUÇÃO: A hospitalização frequentemente causa dificuldades de mobilidade e compromete as atividades da vida diária. A mobilização progressiva de pacientes em unidades de terapia intensiva (UTI) é segura e está associada a melhores resultados clínicos e funcionais. OBJETIVO: Avaliar a percepção da equipe multiprofissional das UTIs de um hospital universitário quanto à mobilização precoce (MP). MÉTODOS: Foi realizado um estudo prospectivo e observacional com dados coletados de profissionais e estudantes da UTI do Hospital Universitário Clementino Fraga Filho da Universidade Federal do Rio de Janeiro entre junho e dezembro de 2019. Os dados sobre a percepção da MP foram coletados por meio de questionário. A estatística descritiva e o teste exato de Fisher foram utilizados para analisar as diferenças entre categorias profissionais. RESULTADOS: Em comparação aos fisioterapeutas (88%), um percentual menor de médicos (37,5%) e enfermeiros (50%) relataram que os pacientes em ventilação mecânica (VM) são mobilizados em 48 horas (P80%), sendo as principais barreiras percebidas a indisponibilidade de profissionais (58%), a condição clínica (55%) e pacientes submetidos a procedimentos (45%). CONCLUSÃO: Em um hospital universitário sem protocolo de MP estabelecido, a equipe multidisciplinar apresenta percepção satisfatória sobre a MP. Contudo, a criação de protocolos e diretrizes institucionais é essencial para engajar a equipe na implementação da MP e na superação de barreiras.

ABSTRACT

RESUMEN Objetivo: Describir las características clínico-demográficas de sujetos internados luego de una intervención quirúrgica que recibieron asistencia kinésica (AK) en la sala de cirugía general de un hospital público de la Ciudad Autónoma de Buenos Aires (CABA). El objetivo secundario consiste en describir el uso de dispositivos de oxigenoterapia, el desarrollo de complicaciones pulmonares postoperatorias (CPP) y el tiempo transcurrido hasta la AK. Materiales y métodos: Se incluyeron datos extraídos de fichas kinésicas de sujetos que se sometieron a una intervención quirúrgica abdominal, torácica o pélvica y recibieron AK entre enero de 2019 y diciembre de 2022. Se registraron datos demográficos y clínicos, motivo de consulta kinésica, desarrollo de CPP, cantidad de días de AK y uso de dispositivos de oxigenoterapia. Resultados: Se incluyeron 151 fichas kinésicas. Del total, 15 sujetos (9,9 %) desarrollaron CPP. En este grupo, la mediana de días transcurridos entre el procedimiento quirúrgico y el comienzo de la AK fue de 8 días (RIQ 4-16), mientras que para los sujetos que no desarrollaron complicaciones fue de 4 días (RIQ 2-7). Un total de 57 sujetos (37,5 %) requirieron oxigenoterapia, de los cuales 3 requirieron ventilación mecánica invasiva y 1 no invasiva. Conclusión: Se describieron las características clínico-demográficas de sujetos postquirúrgicos internados en la sala de cirugía general de un hospital general de agudos de la CABA. Aquellos sujetos con CPP recibieron más días de AK y requirieron oxígeno suplementario durante un período más extenso. Los sujetos que no desarrollaron CPP recibieron AK en una etapa más temprana, en comparación con los que desarrollaron complicaciones.


ABSTRACT Objective: To describe the clinical-demographic characteristics of subjects who were hospitalized after a surgical intervention and received physical therapy (PT) in the general surgery ward of a public hospital in the Autonomous City of Buenos Aires (CABA). The secondary objective is to describe the use of oxygen therapy devices, the development of postoperative pulmonary complications (PPC), and the time elapsed until PT. Materials y methods: We included data from the PT records of subjects who underwent abdominal, thoracic, or pelvic surgical interventions and received PT between January 2019 and December 2022. Demographic and clinical data, reason for PT consultation, development of PPC, number of days of PT, and use of oxygen therapy devices were recorded. Results: A total of 151 records were included; of them, 15 subjects (9.9 %) developed PPC. In this group, the median number of days elapsed between the surgical intervention and the onset of PT was 8 days (IQR 4-16), while for subjects who did not develop complications, it was 4 days (IQR 2-7). A total of 57 subjects (37.5 %) needed oxygen therapy, with 3 subjects requiring invasive mechanical ventilation and 1 requiring non-invasive mechanical ventilation. Conclusion: We described the clinical-demographic characteristics of postoperative subjects hospitalized in the general surgery ward of a general acute hospital in the CABA. Subjects with PPC received PT over an extended period and required supplemental oxygen for a longer period. Subjects who did not develop PPC received PT earlier, compared with those who developed complications.

RESUMEN Introducción: La parálisis cerebral (PC) es la causa más frecuente de espasticidad en la infancia. Una alternativa de tratamiento es la rizotomía dorsal selectiva (RDS). La selección de los candidatos para esta cirugía requiere una evaluación exhaustiva, y la rehabilitación es fundamental para lograr los mejores resultados. El objetivo de este estudio es describir la evaluación y el tratamiento kinésico motor de una niña con PC que fue sometida a una RDS. Presentación del caso: Una niña de siete años con PC fue sometida a una RDS como parte del manejo de la espasticidad en los miembros inferiores. Se describió la evaluación antes de la RDS y seis meses después de esta. Se registraron el grado de espasticidad, el rango articular, las pruebas de longitud muscular, la alineación ósea, la fuerza, el desempeño funcional y la marcha. El tratamiento consistió en una etapa de internación postquirúrgica y una etapa ambulatoria. Conclusión: Se describieron la evaluación y el tratamiento kinésico motor de una niña con PC sometida a una RDS. Observamos disminución de la espasticidad, ausencia de actividad refleja patológica, mejor control motor selectivo, aumento del rango articular, mayor fuerza distal y mejora en el puntaje en la escala de videoanálisis de la marcha y en la escala de movilidad funcional. La niña logró la bipedestación al quinto día postquirúrgico y aumentó las distancias recorridas.


ABSTRACT Introduction: Cerebral palsy (CP) is the most common cause of spasticity in childhood. An alternative treatment is selective dorsal rhizotomy (SDR). The selection of candidates for this surgery requires a thorough evaluation, and rehabilitation is essential to achieve the best results. The aim of this study is to describe the physical therapy evaluation and treatment of a girl with CP who underwent RDS. Case presentation: A seven-year-old girl with CP underwent RDS as part of the management of spasticity in her lower limbs. The patient was evaluated before RDS and at six months. The degree of spasticity, range of motion, muscle length tests, bone alignment, strength, functional performance, and gait were recorded. The treatment included a post-surgical inpatient stage and an outpatient stage. Conclusion: The physical therapy evaluation and treatment of a girl with CP who underwent RDS were described. We observed decrease in spasticity, absence of pathological reflex activity, improvement in selective motor control, increase in range of motion, greater distal strength, and improvement in the score on the video-based gait analysis scale and functional mobility scale. The girl achieved standing on the fifth post-surgical day and showed an increase in distances covered.

Objetivo: la mastectomía posterior al cáncer de seno produce diversas alteraciones posturales y biomecánicas en el plano frontal y sagital. Dichos cambios conducen a una cinemática alterada de la columna vertebral, desequilibrio muscular y alteración del soporte del peso del pie. Este estudio pretende describir las alteraciones de la baropodometría después de la mastectomía, con base en análisis baropodométricos de la presión media y la carga estática. Metodología: se realizó un estudio descriptivo correlacional. Las propiedades de carga del pie se identificaron en 17 mujeres mastectomizadas. Se utilizó una placa de presión Ecosanit Foot para medir la carga del pie en posición anatómica y con los pies juntos. Resultados: en el estudio participaron 17 mujeres mastectomizadas con una edad media de 54.16 años. Las pacientes reportaron una presión media significativamente mayor en la posición anatómica del lado dominante mastectomizado en comparación con el lado no dominante mastectomizado (227.2 ± 22.16 vs. 175.6 ± 14.95, p =0.05). No hubo diferencia significativa para la carga estática entre el lado dominante mastectomizado y el lado no dominante mastectomizado en la posición anatómica (52.43±4.069 vs. 49.69 ± 4.094, de forma respectiva). Conclusión: los resultados actuales de nuestro estudio evidenciaron la distribución desequilibrada del peso (vector carga en el pie) en pacientes después de la mastectomía. Por tanto, en este texto se describen las alteraciones posturales, musculares, y el desequilibrio estático y dinámico en pacientes con cáncer de seno.


Objective: mastectomy after breast cancer produces several postural and biomechanical alterations in the frontal and sagittal plane. Such changes lead to disturbed kinematics of the spine, muscle imbalance and altered foot weight bearing. This study aims to describe body balance alterations after mastectomy based on the baropodometric analysis of their mean pressure and static load. Methodology: a descriptive correlational research was carried out. Foot weight-bearing properties were identified in 17 patients who have undergone a mastectomy. An Ecosanit Foot pressure plate was used to measure foot load at anatomical position and keeping the feet together. Results: 17 female patients who had undergo mastectomy with a mean age of 54.16 years took part on the research. Patients who have had a dominant-side mastectomy demonstrated significant greater mean pressure at anatomical position when compared to those patients who have had a non-dominant side mastectomy (227.2 ± 22.16 versus 175.6 ± 14.95, p =0.05). There was no significant difference for static load between the patients who have had a dominant side mastectomy and patients who have had a non dominant side mastectomy at anatomical position (52.43 ± 4.069 versus 49.69 ± 4.094, respectively). Conclusion: the current results of our research showed the unbalanced weight distribution in patients after mastectomy. Consequently, it describes the postural and muscular alterations, and the static and dynamic imbalance in breast cancer patients.


Objetivo: A mastectomia após câncer de mama produz diversas alterações posturais e biomecânicas nos planos frontal e sagital. Tais mudanças levam a alteração da cinemática da coluna, desequilíbrio muscular e alteração do suporte de peso do pé. Este estudo tem como objetivo descrever as alterações baropodométricas após mastectomia, com base em análises baropodométricas de pressão média e carga estática. Metodologia: foi realizado estudo correlacional descritivo. Propriedades de carga nos pés foram identificadas em 17 mulheres mastectomizadas. Uma placa de pressão Ecosanit Foot foi usada para medir a carga do pé em posição anatômica e com os pés juntos. Resultados: participaram do estudo 17 mulheres mastectomizadas com idade média de 54,16 anos. Os pacientes relataram uma pressão média significativamente maior na posição anatômica do lado mastectomizado dominante em comparação ao lado mastectomizado não dominante (227,2 ± 22,16 vs. 175,6 ± 14,95, p =0,05). Não houve diferença significativa para a carga estática entre o lado mastectomizado dominante e o lado mastectomizado não dominante na posição anatômica (52,43 ± 4,069 vs. 49,69 ± 4,094, respectivamente). Conclusão: Os resultados atuais do nosso estudo mostraram distribuição de peso desequilibrada (carga vetorial no pé) em pacientes após mastectomia. Portanto, este texto descreve alterações posturais, musculares e desequilíbrio estático e dinâmico em pacientes com câncer de mama.

Resumen Introducción: los niños con artritis idiopática juvenil (AIJ) experimentan períodos de dolor e inmovilidad que afectan sus capacidades condicionales. Objetivos: describir los valores de referencia para el 1-minute Sit-to-Stand Test (1-STS; test de 1 minuto de sentarse y pararse) en niños con AIJ como evaluación de la capacidad aeróbico-funcional y de la fuerza muscular de los miembros inferiores (MMII). Materiales y métodos: se realizó un estudio observacional que incluyó a 15 niños con AIJ de entre 5 y 16 años. Se evaluó su rendimiento en el 1-STS. Resultados: se encontró una correlación positiva significativa entre el 1-STS y el test de la marcha de 6 minutos (r=0,56; p=0,03), como con el índice de capacidad funcional (CAPFUN) (r=0,54; p=0,03). No se observaron correlaciones significativas entre el Childhood Health Assessment Questionnaire (CHAQ) y el 1-STS (r=-0,21; p=0,44), tampoco con el Juvenile Arthritis Disease Activity Score (JADAS-10) (p=0,83). Conclusiones: el 1-STS parece prometedor para medir la capacidad aeróbico-funcional y la fuerza muscular de los miembros inferiores en niños con AIJ oligoarticular.


Abstract Introduction: children with juvenile idiopathic arthritis (JIA) experience periods of pain and immobility that affect their physical capacities. Objectives: to describe reference values for the 1-minute sit to stand test (1-STS) in children with JIA as an assessment of aerobic-functional capacity and lower limb muscle strength. Materials and methods: an observational study was conducted, including 15 children with JIA aged between 5 and 16 years. Their performance in the 1-STS was assessed. Results: a significant positive correlation was found between the 1-STS and the 6-Minute Walk Test (r=0,56; p=0,03), as well as with the Functional Capacity Index (CAPFUN) (r=0,54; p=0,03). No significant correlations were observed between the Childhood Health Assessment Questionnaire (CHAQ) and the 1-STS (r=-0,21; p=0,44), nor with the Juvenile Arthritis Disease Activity Score (JADAS-10) (p=0,83). Conclusions: the 1-STS appears promising for assessing aerobic-functional capacity and lower limb muscle strength in children with oligoarticular JIA.

ABSTRACT

BACKGROUND:Instrument-assisted soft tissue mobilization is often used as a noninvasive treatment for soft tissue(skeletal muscle,ligament,and fascia)injuries and postoperative recovery to improve pain and enhance strength in the range of joint motion. OBJECTIVE:To compare the clinical efficacy of instrument-assisted soft tissue mobilization and massage therapy in patients with lateral epicondylitis of the humerus. METHODS:A total of 25 athletes with lateral epicondylitis of the humerus were enrolled in this study and randomized into two groups:13 subjects receiving instrument-assisted soft tissue mobilization as the experimental group and 12 subjects receiving massage therapy as the control group.The treatment period was 4 weeks,with two sessions per week.Elbow joint visual analog scale,Mayo elbow performance index,elbow range of motion measurement,and forearm strength were measured and recorded in both groups before and after treatment. RESULTS AND CONCLUSION:Both treatments significantly reduced visual analog scale score of the elbow joint after the first and last treatments(P<0.05),but the visual analog scale score showed no significant difference between the two groups(P>0.05).The Mayo elbow performance index showed a significant increase in both groups after the first and last treatments(P<0.05),but there was no significant difference between the two groups(P>0.05).In the maximum grip strength test,the maximum grip strength of the experimental group in the vertical direction and during internal and external rotations after treatment was better than that before treatment(P<0.05),while the control group only showed improved maximum grip strength during internal rotation(P<0.05),with no significant improvement in maximum grip strength in other states.There was also no significant difference in the maximum grip strength in all the three states between the two groups(P>0.05).After the last treatment,the range of motion of the elbow joint and the angle of forearm pronation and supination were significantly improved in both intervention groups(P<0.05).The maximum angle of the elbow joint for flexion was smaller than that before treatment(P<0.05),and there was no significant difference in each angle of motion between the two groups(P>0.05).The range of motion of the forearm and elbow joint in both groups were significant improved after the first and last treatment(P<0.05)and there was no significant difference in difference in the range of motion of the forearm and elbow joint between the two groups(P>0.05).To conclude,both instrument-assisted soft tissue mobilization and massage therapy significantly reduce pain,improve elbow flexibility and increase joint range of motion in patients with lateral epicondylitis of the humerus.However,instrument-assisted soft tissue mobilization is better than massage therapy to improve the maximum grip strength.

ABSTRACT

Objective To explore the effect of seminar based on case-based learning(CBL)in practical teaching of physical therapy. Methods From July,2021 to June,2022,42 rehabilitation therapy students for internships in Rehabilitation Medicine Center,the First Affiliated Hospital of Nanjing Medical University were non-directionally recruited,and random-ly divided into control group(n = 21)and experimental group(n = 21).The experimental group received instruc-tion using seminar and CBL,while the control group received CBL alone,for three months.The scores of theoret-ical and practical assessments were observed,and the satisfaction was investigated using a self-designed question-naire. Results The scores of both theoretical and practical assessments were better in the experimental group than in the control group(t>2.421,P<0.05);while the satisfaction was better in terms of motivating learning enthusiasm,enhanc-ing learning abilities,cultivating clinical reasoning skills,improving teacher-student communication,promoting teamwork,enhancing overall competence,and satisfying to the teaching in the experimental group than in the control group(χ2>6.667,P<0.05). Conclusion The combination of seminar with CBL would enhance the effect of practical teaching in physical therapy.

Abstract Background Peripheral vestibular hypofunction (PVH) is characterized by balance and gait disorders and vestibulo-autonomic findings. The vestibular system and proprioceptive system work together to regulate sensorimotor functions. Vestibular exercises are effective in PVH, but their superiority over each other is still unclear. Objective This study aims to examine the effect of proprioceptive vestibular exercises on patients with PVH. Methods 30 individuals with unilateral PVH were assigned to 3 groups. Group 1 received proprioceptive vestibular rehabilitation, group 2 received standard vestibular rehabilitation. Both groups were given standard vestibular exercises as home exercises. No exercise was applied to the group 3. Patients were evaluated in terms of balance, functional mobility, posture, sensory profile, and quality of life. Results Although there was a significant intra-group difference in balance, functional mobility, and quality of life results in all groups (p < 0.05), the difference between groups was generally in favor of group 1 (p < 0.05). There was a significant difference between the groups in the posture analysis results (p < 0.05), while there was a significant difference in the 1st group (p < 0.05). There was a significant difference between the groups in the results of sensory sensitivity, sensory avoidance, and low recording (p < 0.05). There was no significant difference between the groups in sensory-seeking results (p > 0.05). There was a significant difference in quality of life between and within groups (p < 0.05). Conclusion Proprioceptive vestibular rehabilitation is an effective method in PVH. We think that our study will guide clinicians and contribute to the literature. Trial registration NCT04687371.


Resumo Antecedentes A hipofunção vestibular periférica (HVP) é caracterizada por distúrbios do equilíbrio e da marcha e achados vestíbulo-autonômicos. O sistema vestibular e o sistema proprioceptivo trabalham juntos para regular as funções sensório-motoras. Os exercícios vestibulares são eficazes na HVP, mas sua superioridade entre si ainda não está clara. Objetivo Este estudo tem como objetivo examinar o efeito de exercícios vestibulares proprioceptivos em pacientes com HVP. Métodos Trinta indivíduos com HVP unilateral foram divididos em três grupos. O grupo 1 recebeu reabilitação vestibular proprioceptiva, o grupo 2 recebeu reabilitação vestibular padrão. Ambos os grupos receberam exercícios vestibulares padrão como exercícios caseiros. Nenhum exercício foi aplicado ao grupo 3. Os pacientes foram avaliados quanto a equilíbrio, mobilidade funcional, postura, perfil sensorial e qualidade de vida. Resultados Embora tenha havido uma diferença significativa intragrupo nos resultados de equilíbrio, mobilidade funcional e qualidade de vida em todos os grupos (p < 0,05), a diferença entre os grupos foi geralmente a favor do grupo 1 (p < 0,05). Houve diferença significativa entre os grupos nos resultados da análise postural (p < 0,05), embora tenha havido diferença significativa no 1° grupo (p < 0,05). Houve diferença significativa entre os grupos nos resultados de sensibilidade sensorial, esquiva sensorial e baixo registro (p < 0,05). Não houve diferença significativa entre os grupos nos resultados de busca sensorial (p > 0,05). Houve diferença significativa na qualidade de vida entre e dentro dos grupos (p < 0,05). Conclusão A reabilitação vestibular proprioceptiva é um método eficaz na HVP. Acreditamos que nosso estudo orientará os médicos e contribuirá para a literatura. Registro de teste NCT04687371.
