ABSTRACT
Objective: To observe the effects of acupuncture at Yuan-Primordial and Luo-Connecting points by host-guest combination plus Tuina (Chinese therapeutic massage) on the third lumbar transverse process syndrome. Methods: A total of 88 patients with the third lumbar transverse process syndrome were selected and divided into an observation group and a control group according to the random number table method, with 44 cases in each group. The control group was treated with Tuina, and the observation group was treated with additional acupuncture at Yuan-Primordial and Luo-Connecting points by host-guest combination. The clinical efficacy of the two groups was compared after treatment. The changes in the scores of physical signs, Roland-Morris disability questionnaire (RMDQ), Oswestry disability index (ODI), Quebec back pain disability scale (QBPDS), and pain factors [including serum prostaglandin (PG) E2, neuropeptide Y (NPY), and matrix metalloproteinase-3 (MMP-3)] were observed. Results: After treatment, the total effective rate in the observation group was 93.2%, higher than 75.0% in the control group; the difference between the two groups was statistically significant (P<0.05). Compared with those before treatment, the scores of physical signs and each low back pain scale, and the levels of serum pain factors in the two groups were decreased (P<0.05), and those in the observation group were lower than those in the control group (P<0.05). Conclusion: Acupuncture at Yuan-Primordial and Luo-Connecting points by host-guest combination plus Tuina is effective in the treatment of the third lumbar transverse process syndrome; it can improve the patient's physical signs, relieve low back pain, and reduce the levels of serum pain factors.

The meridian diseases, abnormal psoas and abdominal muscles and abdominal pressure are key causes of low back pain. We treat low back pain mainly with lumbar back area, neglecting abdomen. The anterior-posterior acupoint combination, regulating to treat is effective. Based on the Chinese meridian-collateral and contemporary anatomy related to low back pain, the combination method, and its mechanism are introduced, so as to develop the thought and method for low back pain. A typical case is added.

Objective: To observe the changes in numbers and degranulation status of local mast cells (MCs) of the complementary acupionts, as well as the concentrations of serum histamine and bradykinin after acupuncture at rat's complementary acupionts of Yanglingquan (GB 34) and Yinlingquan (SP 10), for exploring the mechanisms of the synergistic effect of complementary acupionts. Methods: Using random number table method, 40 Wistar rats were randomly divided into a blank control group (group K), an acupuncture at Yanglingquan (GB 34) group (group A), an acupuncture at Yinlingquan (SP 9) group (group B), an acupuncture at Yanglingquan (GB 34) and Yinlingquan (SP 9) group (group AB), and an acupuncture at points near Yanglingquan (GB 34) and Yinlingquan (SP 9) group (group CD), 8 rats in each group. Group A received acupuncture at Yanglingquan (GB 34); group B received acupuncture at Yinlingquan (SP 9); group AB received acupuncture at Yanglingquan (GB 34) and Yinlingquan (SP 9); group CD received acupuncture at the control points [points 3 mm away from Yanglingquan (GB 34) and Yinlingquan (SP 9) respectively]. Acupuncture was performed at bilateral points in each group. Rats in group K were fixed using the same method as rats in the other 4 groups without acupuncture stimulation. Needles of 0.35 mm in diameter and 40 mm in length were used for acupuncture. Needle handle was connected to the G6805-Ⅱ mode electroacupuncture (EA) device after needling qi was obtained, with sparse-dense wave, frequency of 2 Hz/100 Hz and current of 1 mA to keep the needle handles slightly tremulous while the rats kept quiet. Rats were continuously stimulated by EA for 20 min each time. Experimental interventions were conducted on the 1st, 3rd, 5th and 7th days when the experiment started, for 4 times in total. Specimens from rats in each group were collected 2 h after the 7th day. The levels of histamine and bradykinin in serum were detected by enzyme-linked immunosorbent assay (ELISA). Local tissues of the points were used to prepare cryosections. The changes of MCs were observed after toluidine blue staining. Results: Compared with group K, the numbers and degranulation rates of MCs in group A, group B, group AB and group CD were significantly increased (all P<0.05). The number and degranulation rate of MCs in group AB were significantly higher than those in group A, group B and group CD (all P<0.05). The order of histamine levels from high to low was: group AB >group B > group A > group CD > group K, and the differences were statistically significant (all P<0.05); the levels of bradykinin in group AB, group A and group B were significantly higher than those in group K and group CD (all P<0.05). There was no significant difference in the bradykinin level between group A and group B, nor between group CD and group K (all P>0.05). Conclusion: The number and degranulation rate of MCs of the complementary acupionts are significantly increased after acupuncture at complementary acupionts of Yanglingquan (GB 34) and Yinlingquan (SP 9) of rats, and the serum levels of histamine and bradykinin are increased, which may be one of the mechanisms of the synergism produced by the combination of complementary acupionts.

Objective:To analyze and summarize the characteristics and pattern in the selection of points and meridians by searching the clinical research literature about acupuncture-moxibustion treatment of amblyopia in the recent 17 years.Methods:By searching Chinese and English databases,such as China National Knowledge Infrastructure (CNKI),PubMed,etc.,the points in 52 articles in conformity with the requirements were analyzed by frequency statistics by the order of meridians,major points,and adjunct points to summarize the rules and characteristics of the point selection.Results:In 52 articles,there were 21 articles on ear acupuncture and 31 articles on acupuncture-moxibustion treatment.Ten meridians were involved in acupuncture-moxibustion treatment of amblyopia,and the leading 3 meridians were the Bladder,Stomach and Gallbladder Meridians.There were 34 points,10 extraordinary points and 37 ear points were used in acupuncture-moxibustion treatment of amblyopia.The top 10 major points were Jingming (BL 1),Taiyang (EX-HN 5),Cuanzhu (BL 2),Fengchi (GB 20),Baihui (GV 20),Hegu (LI 4),Guangming (GB 37),Sibai (ST 2),Chengqi (ST 1) and Sizhukong (TE 23).The top 5 adjunct points were Zusanli (ST 36),Shenshu (BL 23),Ganshu (BL 18),Sanyinjiao (SP 6) and Taixi (KI 3).Conclusion:Acupuncture-moxibustion treatment of amblyopia is characterized by the selection of the points mainly from yang meridians,based upon syndromes differentiation plus personal experience,and the points mainly around the eyes and by stressed use of ear points.

Acupoint compatibility is a basic element for acupuncture prescription and an important link for clinical acupuncture workers to obtain a therapeutic effect. Studying factors influencing the effect of acupoint combination and ascertaining its concept and connotation are of important significance for studying the mechanism of acupuncture action. By sorting out, summing up and analyzing relevant modern literature, it is found that factors influencing the effect of acupoint combination are those affecting the therapeutic effects of combination of two or more than two acupoints. Its contents include five aspects: combination mode, combination effect, stimulation method, time factor and body status. Analysis and induction show that acupoint selection is the primary task in studying factors influencing the effect of acupoint combination.

Objective To observe the clinical efficacy of Tiao Ren Tong Du (regulating the Conception Vessel and unblocking the Governor Vessel) needling method plus bloodletting at the Jing-well points in treating post-stroke spastic state. Method Sixty-seven eligible patients were randomized into a group to receive Tiao Ren Tong Du needling plus bloodletting at the Jing-well points (treatment group) and a rehabilitation group (control group). For the two groups, the treatment was given once a day, 6 times as a treatment course, for 4 courses in total. The Modified Ashworth Scale was used to evaluate the spasticity intensity, and the Patient Reported Outcome (PRO) for post-stroke spastic hemiplegia was used to measure the patients from the following four aspects:subjective symptoms, objective symptoms, mental state, and social function. Result After intervention, the scores of spasticity, and physiological, mental, social, and environmental factors were significantly improved in both groups (P<0.01,P<0.05). Compared to rehabilitation, Tiao Ren Tong Du needling plus bloodletting at Jing-well points can more significantly release the muscular spasticity and reduce the PRO score (P<0.01). The recovery plus markedly-effective rate was 70.6% in the treatment group versus 42.4% in the control group. Conclusion Tiao Ren Tong Du needling plus bloodletting at Jing-well points can effectively improve post-stroke spastic state and the quality of life.

Objective To observe the efficacy of electroacupuncture(EA) on the acupoint combination of He-(sea) points, front-mu points, and shu-points(abbrev. He-Mu-Shu) for the treatment of functional diarrhea, and to explore the effect of He-Mu-Shu point combination EA on the anxiety of patients. Methods A randomized controlled clinical trial was performed in 62 functional diarrhea patients. All of the patients were randomly divided into He-Mu-Shu group(32 cases) and medicine group(30 cases). He-Mu-Shu group was treated with EA on unilateral Quchi, Shangjuxu, Tianshu, Dachangshu point alternatively, and medicine group was treated with oral use of Loperamide hydrochloride capsules. Both groups were treated for 4 continuous weeks. Defecation frequency, stool property and anxiety scores were observed before treatment and 2, 4 weeks after treatment. Results After treatment, weekly defecation frequency, stool property and anxiety scores were improved in both groups, and the differences were statistically significant compared with those before treatment(P < 0.05). He-Mu-Shu point combination EA had better effect on improving weekly defecation frequency, stool property and anxiety scores of functional diarrhea patients than Loperamide hydrochloride capsules, and the differences were statistically significant (P < 0.05). Conclusion The clinical efficacy and anxiety-relief action of He-Mu-Shu point combination EA on functional diarrhea are superior to oral use of Loperamide hydrochloride capsules.

Objective To observe the clinical efficacy of combining Yuan-Primary and Luo-Connecting points in treating insomnia due to imbalance between heart and kidney, for providing novel method and evidence for the treatment of insomnia with acupuncture-moxibustion.Method Totally 100 insomnia patients due to imbalance between heart and kidney were randomized into a treatment group and a control group, 50 in each. The treatment group was intervened by acupuncture at Yuan-Primary and Luo-Connecting points, while the control group was by conventional acupuncture. The Pittsburgh Sleep Quality Index (PSQI), self-rating anxiety scale (SAS), and self-rating depression scale (SDS) were observed before and after intervention for efficacy evaluation.Result The PSQI item scores were significantly changed after intervention in both groups (P<0.05). The SAS and SDS scores were significantly different from that before treatment in both groups (P<0.05). After treatment, the SAS and SDS scores of the treatment group were significantly different from that of the control group (P<0.05). The recovery rate and total effective rate were 26.0% and 92.0% in the treatment group versus 12.0% and 86.0% in the control group, and there was a significant difference in comparing the recovery rate (P<0.05).Conclusion Combining use of Yuan-Primary and Luo-Connecting points is an effective approach in treating insomnia due to imbalance between heart and kidney.

Objective To study the clinical efficacy and the difference of intervention in appendix inflammatory mass by heavenly cool-inducing technique adopted distant and neighboring point selection. Methods Eighty patients of the appendix inflammatory mass with toxic heat syndrome were randomly divided into control group, distant point selection group, neighboring point selection group, and combined group, 20 cases in each group. The control group adopted the conventional western medicine treatment. On the basis of control group, the rest 3 acupuncture groups adopted the heavenly cool-inducing technique for 14 d. The distant point selection group chose Hegu point and Lanwei point;the neighboring point selection group chose Ashi point and Tianshu point (right);the combined group put them together. The longest diameter and three-dimensional volume of inflammatory mass were determined before and after treatment. The serum levels of white blood cells, C-reactive protein were determined at the same time, and then the clinical effective was estimated. Results The longest diameter, three-dimensional volume of inflammatory mass, the serum levels of white blood cells, and C-reactive protein of patients in 4 groups were reduced after treatment. The above indicators in patients of 3 acupuncture groups were lower than the ones in control group, the indicators in combined group were the lowest (P0.05). The invalid number of cases in 3 acupuncture groups were fewer than the one in control group, and the indicators in combined group were fewest (P0.05). Conclusion There is obvious therapeutic efficacy of heavenly cool-inducing technique to appendix inflammatory mass, while the combination of distant and neighboring point selection is better than the single methods.
