ABSTRACT

Introducción: La pandemia por COVID-19 ha afectado significativamente la calidad de los servicios de cuidado de la salud. Objetivo: Analizar los efectos en los atributos de la calidad en salud de los servicios de atención de enfermedades diferentes a la COVID-19 en Colombia, durante el periodo 2020-2022. Materiales y métodos: Se analizaron 24 artículos de alcance nacional y otros específicos de departamentos como Antioquia, Córdoba, Santander y Cundinamarca. Resultados: La pandemia por COVID-19 impactó la calidad de los servicios en la atención de enfermedades como cáncer, accidentes cerebrovasculares y de eventos como la interrupción voluntaria del embarazo. Conclusión: La calidad de la salud se vio afectada en todas sus dimensiones durante las fases de la pandemia, especialmente en la población con enfermedades crónicas y relacionadas con la salud infantil y materna. Además, se destacaron respuestas como el uso de la telemedicina y de la atención domiciliaria para contribuir a la calidad de la salud en Colombia.


Introduction: The COVID-19 pandemic has significantly affected the quality of health care services. Objective: To analyze the effects of COVID-19 on the quality of health care services focused on treating diseases other than COVID-19 in Colombia during the 2020-2022 period. Materials and methods: 24 articles were analyzed, which included some studies focused on national issues and others specific to the departments of Antioquia, Cordoba, Santander, and Cundinamarca. Results: The COVID-19 pandemic affected the quality of health services caring for diseases such as cancer, strokes, and critical circumstances like voluntary termination of pregnancy. Conclusion: All dimensions of health care were affected during the pandemic, especially impacting populations with chronic diseases and diseases related to child and maternal health. It is important to highlight that telemedicine and home care contributed to improving the quality of health in Colombia.


Introdução: A pandemia de COVID-19 afetou significativamente a qualidade dos serviços de saúde. Objetivo: Analisar os efeitos da COVID-19 nos atributos de qualidade em saúde dos serviços de atenção a outras doenças além da COVID-19 na Colômbia, durante o período 2020-2022. Materiais e métodos: foram analisados 24 artigos de âmbito nacional e outros específicos de departamentos como Antioquia, Córdoba, Santander e Cundinamarca. Resultados: A pandemia da COVID-19 impactou a qualidade dos serviços no cuidado de doenças como câncer, acidente vascular cerebral e eventos como a interrupção voluntária da gravidez. Conclusão: A qualidade da saúde foi afetada em todas as suas dimensões durante as fases da pandemia, especialmente na população com doenças crônicas e doenças relacionadas à saúde infantil e materna. Além disso, foram destacadas respostas como o uso da telemedicina e do atendimento domiciliar para contribuir para a qualidade da saúde na Colômbia.

ABSTRACT

El propósito del trabajo fue determinar la relación existente entre el clima organizacional y la gestión de la calidad en la División de Títulos y Diplomas (DTD), unidad administrativa de la Universidad Mayor de San Andrés (UMSA), institución pública y autónoma, responsable de la formación académica profesional, investigativa y social en la ciudad de La Paz. Para ello se empleó una metodología cuantitativa y un diseño correlacional transaccional; se elaboraron tres escalas que fueron validadas y confiabilizadas para luego ser aplicadas a funcionarios y usuarios de la DTD (seleccionados a través de muestras probabilísticas simples y censal); se contó también con una guía de entrevistas. Los resultados arrojan que en la DTD se cuenta con un clima organizacional participativo en el que resaltan el liderazgo, el trabajo en equipo y una visión positiva sobre sueldos y prestaciones recibidas; como debilidades resaltan el ambiente físico de trabajo, el diseño de puestos de trabajo y el desarrollo de carrera. La certificación en gestión de calidad es la mayor fortaleza a lo que se suma la orientación al cliente; la debilidad en gestión de calidad está centrada en la planificación. Finalmente se determinó que, si bien ambas variables están conectadas, el comportamiento de una no obedece exclusivamente al comportamiento de la otra, por lo que se infiere que existen otras variables que inciden en ellas, entre estas el compromiso, la normativa general institucional y el trato mismo que dispensa el usuario al funcionario.


The purpose of the work was to determine the relationship between the organizational climate and quality management in the Degrees and Diplomas Division (DTD), an administrative unit of the Universidad Mayor de San Andrés (UMSA), a public and autonomous institution, responsible for professional, investigative and social academic training in the city of La Paz. For this, a quantitative methodology and a transactional correlational design were used; Three scales were developed that were validated and made reliable and then applied to officials and users of the DTD (selected through simple probabilistic samples and census); There was also an interview guide. The results show that the DTD has a participatory organizational climate in which leadership, teamwork and a positive vision of salaries and benefits received stand out; as weaknesses they highlight the physical work environment, job design and career development. Certification in quality management is the greatest strength to which customer orientation is added; the weakness in quality management is focused on planning. Finally, it will be concluded that, although both variables are connected, the behavior of one does not obey exclusively to the behavior of the other, so it is inferred that there are other variables that affect them, among them the commitment, the general institutional regulations and the same treatment that dispenses the user to the officer.


O objetivo do trabalho foi determinar a relação entre o clima organizacional e a gestão da qualidade na Divisão de Graus e Diplomas (DTD), unidade administrativa da Universidade Mayor de San Andrés (UMSA), instituição pública e autônoma, responsável pela gestão profissional, formação acadêmica investigativa e social na cidade de La Paz. Para tanto, utilizou-se uma metodologia quantitativa e um desenho correlacional transacional; foram desenvolvidas três escalas que foram validadas e tornadas confiáveis ​​e depois aplicadas a funcionários e usuários da DTD (selecionados através de amostras probabilísticas simples e censo); havia também um guia de entrevista. Os resultados mostram que o DTD possui um clima organizacional participativo em que se destacam a liderança, o trabalho em equipe e uma visão positiva dos salários e benefícios recebidos; os pontos fracos incluem o ambiente físico de trabalho, a concepção do trabalho e o desenvolvimento da carreira. A certificação em gestão da qualidade é o maior ponto forte ao qual se soma a orientação para o cliente; A fraqueza na gestão da qualidade está focada no planejamento. Por fim, determinou-se que, embora ambos ases variáveis estejam conectadas, o comportamento de uma não obedece exclusivamente ao comportamento da outra, pelo que se infere que existem outras variáveis que as afetam, incluindo o comprometimento, as regulamentações institucionais gerais e o mesmo tratamento que o usuário dá ao funcionário.

ABSTRACT

Objetivo: Avaliar a percepção da Cultura de Segurança dos profissionais de saúde de um hospital filantrópico. Métodos: Estudo quantitativo do tipo survey com amostra retrospectiva, a partir da análise de respostas ao questionário Pesquisa sobre Segurança do Paciente em Hospitais em um hospital filantrópico de alta complexidade cardiovascular. Para análise dos dados foi empregada a análise descritiva e a estatística inferencial através do teste de Kruskal-Wallis. Resultados: A amostra é composta por 288 profissionais, dos quais 70,49% são mulheres. A equipe de enfermagem mostrou maior expressividade totalizando 58,33% dos entrevistados. A avaliação da cultura de segurança de acordo com a unidade de atuação foi considerada como muito boa por 138 dos 288 participantes (47,92%). Conclusão: A cultura de segurança foi classificada como muito boa, entretanto, ainda é um ponto a se fortalecer na instituição. Os resultados podem servir para o planejamento das ações com intervenções expressivas, fortalecendo o clima de segurança e a adoção de boas práticas institucionais. (AU)


Objective: To evaluate the belief of the Safety Culture of health professionals in a philanthropic hospital. Methods: Quantitative survey-type study with a retrospective sample, based on the analysis of responses to the questionnaire Research on Patient Safety in Hospitals in a philanthropic hospital of high cardiovascular complexity. Descriptive analysis and inferential statistics were used for data analysis using the Kruskal-Wallis' test. Results: The sample is composed of 288 professionals, of which 70.49% are women. The nursing team showed greater expressiveness, totaling 58.33% of respondents. The evaluation of the safety culture according to the unit of operation was considered very good by 138 of the 288 participants (47.92%). Conclusion: The safety culture was classified as very good; however, it is still a point to be strengthened in the institution. The results can be used to plan actions with significant interventions, strengthening the security climate and the adoption of good institutional practices. (AU)

Métodos: Estudio cuantitativo tipo encuesta con muestra retrospectiva, basado en el análisis de las respuestas al cuestionario Investigación sobre Seguridad del Paciente en Hospitales en un hospital filantrópico de alta complejidad cardiovascular. El análisis descriptivo y la estadística inferencial se utilizaron para el análisis de datos mediante la prueba de Kruskal-Wallis. Resultados: La muestra está compuesta por 288 profesionales, de los cuales el 70,49% son mujeres. El equipo de enfermería mostró mayor expresividad, totalizando el 58,33% de los encuestados. La evaluación de la cultura de seguridad según la unidad de operación fue considerada muy buena por 138 de los 288 participantes (47,92%). Conclusión: La cultura de seguridad fue catalogada como muy buena, sin embargo, aún es un punto por fortalecer en la institución. Los resultados pueden ser utilizados para planificar acciones con intervenciones significativas, fortaleciendo el clima de seguridad y la adopción de buenas prácticas institucionales. (AU)

ABSTRACT

Fundamento: la estructura y organización de los planes de estudio de la Educción Superior en Cuba demandan la búsqueda constante de alternativas que posibiliten perfeccionar el trabajo metodológico. Objetivo: elaborar un sistema de acciones que contribuya a mejorar el funcionamiento de los colectivos de disciplina. Métodos investigación de desarrollo con enfoque dialéctico materialista, realizada en la Universidad de Ciencias Médicas de Cienfuegos en el año 2022. Se aplicaron métodos en los niveles empíricos, teóricos y estadísticos matemáticos que en su conjunto permitieron dar salida al objetivo previsto. La propuesta se organizó en las etapas de diagnóstico, diseño y validación. Resultados identificadas potencialidades y limitaciones en el funcionamiento de los colectivos de disciplina en la institución, aspectos considerados al proponer acciones de planificación, organización, ejecución y control para la mejora continua, que particularizan aspectos para aquellos colectivos que se centran en la práctica laboral por constituir la educación en el trabajo principio y forma organizativa fundamental en la Educación Médica cubana. La validación de la propuesta realizada confirmó su valor, de manera que implementarla es el reto para continuar apostando por la calidad de los procesos en la universidad. Conclusiones las acciones propuestas se derivan del diagnóstico realizado y de manera integrada contribuyen a lograr mayor efectividad en la dirección de ese nivel organizativo para el trabajo metodológico en la institución y por tanto a su funcionamiento.


Foundation: The conformation and organization of the Higher Education study plans in Cuba demand the constant search for alternatives that make it possible to perfect the methodological work. Objective: develop a system of actions that contributes to improve the function of disciplinary groups. Methods: development research with a materialist dialectical approach, carried out at the Cienfuegos Medical Sciences University in 2022. Methods were applied at the empirical, theoretical and mathematical statistical levels that together allowed the intended objective to be achieved. The proposal was organized into the diagnosis, design and validation stages. Results: identified potentialities and limitations in the functioning of the disciplinary groups in the institution, aspects considered when proposing planning, organization, execution and control actions for continuous improvement, which particularize aspects for those groups that focus on work practice by constitute education at work as a fundamental principle and organizational form in Cuban Medical Education. The validation of the proposal confirmed its value, so implementing it is the challenge to continue betting on the quality of the processes at the university. Conclusions: the proposed actions are derived from the diagnosis carried out and in an integrated manner contribute to achieving greater effectiveness in the direction of that organizational level for the methodological work in the institution and therefore its operat.

Objectives@#The primary aim of this study was to determine quantitatively the extent of coverage of the Hong Kong Laboratory Accreditation Scheme (HOKLAS 015) requirements by guidance checklists (HOKLAS 016‑02 and HOKLAS 021). @*Methods@#The level of conformance requirement coverage of HOKLAS 015 by HOKLAS 016‑02 and HOKLAS 021 was calculated by an evaluation checklist based on conformance requirements in HOKLAS 015. A distribution analysis of conformance requirements relating to the International Standard ISO 15189:2012 process‑based quality management system model was also performed to elicit further coverage information. @*Results@#HOKLAS 016‑02 was found to provide coverage of 76% while HOKLAS 021 was found to provide coverage of 11%. HOKLAS 015 was also found to have a distribution coverage of 78% relating to the International Standard ISO 15189:2012 process‑based quality management system model.@*Conclusion@#The results of this analysis should be of value to medical laboratories wishing to maintain the internal auditability required by HOKLAS 015 by gaining an awareness of the extent of coverage provided by HOKLAS 016‑02 and HOKLAS 021.

ABSTRACT

Objective To construct a list of quality scoring criteria for the attached sheet to the summary page of inpatient cases to achieve quantitative evaluation of the data quality.Methods It uses the Data Quality Management model of the American AHIMA as the evaluation framework to develop the list of data quality scoring criteria for the attached sheet,and score in Attached Sheet to the Summary Page of Inpatient Cases issued by the Hubei Provincial Health Commission as a demonstration.Results The average score of the 40 items in Attached Sheet to the Summary Page of Inpatient Casesis 6.725 out of 10.The main quality defects include that all items fail to clarify the person responsible for filling or the time limit for filling.In addition,some items are duplicated with the summary page(35%)or do not have a summary nature(40%).Conclusion Significant room exists for the improvement in the data quality of the attached sheet,especially in defining the person responsible and the time limit for filling in when setting up the items,making sure that the items supplement and extend the summary page,and applying effective quality control methods to the items.

Palliative care, as an emerging discipline, is rapidly advancing in China. However, progress in quality management has been relatively slow, hindering the homogeneity of palliative care services in a certain degree. This article takes the Shanghai Palliative Care Service Management Center as an example, outlines its practical model and achievements in the field of quality management since its establishment, and further analyzes the existing problems based on the city-wide palliative care service quality evaluation results. The article summarizes relevant experiences and offers corresponding insights, enriching research cases and practical support in the quality management of palliative care, which may have practical application value for enhancing the homogeneity of palliative care services in the region.

ABSTRACT

Science and technological advancements drive human progress, with laboratory animals serving as essential resources for developments in life sciences and medicine. However, the waste generated by these animals presents new challenges for urban management. Issues such as classification, recycling, effective utilization, and biohazard elimination must be addressed, necessitating the development of regulations, standards, and norms to keep pace with advancements. The construction of quality management system is the foundation and framework for the management of inspection and testing organizations. It should have strong operability and inspectability, enabling continuous improvement of the management level and enhancing the stability of basic management. However, current quality management systems often lack clarity in managing laboratory animal waste, including undefined disposal processes for non-medical institutions, inaccurate waste classification, and inadequate disposal methods for different waste categories. This paper addresses these challenges by identifying necessary processes to be added or removed in the quality management system of National Institutes for Food and Drug Control, developing effective SOPs, proposing practical measures to strengthen supervision and management, and integrating 6S management principles into our quality management system. In conclusion, effective management of laboratory animal waste should be centered on improving the quality management system, emphasizing waste classification and management at the source, controlling biological hazards, minimizing environmental pollution and promoting conditions for sustainable development.

ABSTRACT

Objective To investigate the current status of semen testing in the hospitals without assisted reproductive technology in Hu-nan Province,propose improvement strategies to enhance the diagnosis of male infertility and promote reproductive health services in these hospitals.Methods Questionnaire surveys and on-site investigations were conducted to examine the semen testing status in 67 hospitals without assisted reproductive technology in Hunan Province.The existing problems were summarized,and improvement strate-gies were suggested.Results Among the 67 hospitals,59.7%(40/67)performed semen testing.Of these,45%(18/40)possessed dedicated personnel,60%(24/40)possessed staff received relevant training,and 22.5%(9/40)held certificates.Only 30%(12/40)conducted sperm morphology testing,among them 20%(8/40)able to stain and interpret following WHO standards.The hospitals of 37.5%(15/40)used computer-assisted sperm analysis(CASA)systems,and 22.5%(9/40)were equipped with phase-contrast microscopes.30%(12/40)had dedicated examination areas,and 32.5%(13/40)had independent ejaculation rooms,17.5%(7/40)used disposable slides to observe sperm concentration.In 2022,the hospitals of 57.5%(23/40)had an average daily sample volume<1.The hospital of 67.5%(27/40)performed standard operating procedures,40%(16/40)followed the WHO 5th la-boratory manual for routine testing,15%(6/40)conducted internal quality control,and 12.5%(5/40)participated in external quali-ty assessment(EQA).Another 55%(22/40)wished to participate in EQA.Conclusion The semen testing capacity in the hospitals,in which assisted reproductive technology is not yet carried out currently,urgently requiring multifaceted improvements.The proposed strategies include emphasizing semen testing,establishing reproductive medicine consortia for comprehensive support,establishing a provincial quality control center for EQA,founding a professional committee for a learning platform,offering training to enhance staff expertise,and including the pre-pregnancy semen testing in public health programs.

ABSTRACT

Objective:To investigate the experience of body quality management in overweight or obese patients with breast cancer during chemotherapy, and to provide reference for formulating targeted nursing interventions.Methods:This study was a phenomenological study of qualitative research. Objective selected 10 obese patients with breast cancer from breast surgery of Provincial Hospital Affiliated to Shandong First Medical University and the First Affiliated Hospital of Shandong First Medical University to conduct semi-structured interviews during chemotherapy from March to May 2023. Colaizzi 7-step analysis method was used to sort out and analyze the data and extract the theme.Results:The average age of 10 overweight or obese breast cancer patients was 43.2 years. Three themes and eight subthemes were extracted, including insufficient motivation of body quality management (insufficient cognition, insufficient drive, insufficient motivation), disturbing experience of body quality management (uncertainty of disease, symptom distress during chemotherapy, inability to identify the correctness of information), regulation and support of body quality management (mastering correct methods of body quality regulation and support in all aspects).Conclusions:The motivation of body quality management in overweight or obese patients with breast cancer during chemotherapy is insufficient and there are many problems. Medical staff can take effective measures from clarifying the significance of body quality management, providing targeted information support, establishing a good social support system and providing effective body quality management guidance, so as to improve the effectiveness of body quality management in obese patients with breast cancer during chemotherapy.

Since the 20th century, organ transplantation has become a breakthrough technology to effectively save the lives of patients with end-stage organ failure, which has significantly enhanced the quality of life of patients. Organ donation is an important source of organ transplantation. Improving the quality of donor organ procurement is the key to promote the translation of donor organs and improve the prognosis of organ transplantation recipients. The United States, Spain and other countries have put forward a series of policies and standards in the quality management and control of donor organ procurement and achieved positive results. In this article, related concepts of medical quality management and control, advanced strategies and models of international donor organ procurement quality management, and quality control measures of Organ Procurement Organization, donors and donor organs were reviewed, aiming to provide reference for establishing a quality management and control system of donor organs with "Chinese characteristics" and advancing high-speed and high-quality development of donor organ procurement.

BACKGROUND:Clinical use of vascular stents involves high medical costs,but it may also bring long-term benefits in reducing cardiovascular events and improving the quality of life in patients.Economics evaluation can help decision makers better understand the balance between the cost and benefit of treatment. OBJECTIVE:To analyze the related articles of health economics and discuss the hot spots in the study of the effect and problems of vascular stents in medical quality management. METHODS:The articles concerning health economics evaluation of vascular stents were retrieved from the core set of the Web of Science.The VOSviewer_1.6.19 software was used to make a visualization analysis of the annual publication volume,institutions,countries,keywords,etc.Finally,the research hot spots on the effects and problems of vascular stents were analyzed from the perspective of health economics and medical quality management. RESULTS AND CONCLUSION:(1)120 articles in English were finally included.In the past 10 years,the highest number of articles published in this field was in 2019,with 10 articles.The institution with the largest number of articles published was Harvard University in the United States with 20 articles,and the country with the largest number of articles published was the United States with 58 articles.(2)Keyword cluster analysis demonstrated that the cost-effectiveness analysis of bare metal stents and drug-eluting stents in coronary disease,the cost-effectiveness analysis of angioplasty stent intervention,and the effect of coronary stents in percutaneous coronary intervention are the research hot spots in the field of health economics evaluation of vascular stent research.(3)In the context of medical quality management,the paper further summarized the research hot spots on the therapeutic effect of vascular stents as follows:long-term effect of vascular stents,safety,drug release mechanism research,personalized therapy,restenosis problems,and stent insertion technology.(4)The results of highly cited literature analysis exhibited that drug-eluting stents release drugs to reduce the risk of vascular restenosis,and the restenosis rate is lower than that of bare metal stents,but the cost is usually higher.Biodegradable stents combine the advantages of bare metal stents and drug-eluting stents,that is,avoiding long-term stent existence and reducing the risk of restenosis,but their cost may be higher,and there may be some complications in the short term,and they are not widely used at present.(5)In addition to the direct stent cost,factors that need to be considered when comparing the cost-effectiveness of vascular stents include the risk and cost of stent re-intervention,the risk and cost of complications,the duration and cost of drug therapy,and the quality of life of patients.Therefore,while the initial cost of drug-eluting and biodegradable stents may be higher than bare metal stents,they may lead to better clinical outcomes in the long term,resulting in a more favorable cost effect.(6)Future research directions should focus on making personalized vascular stent treatment decisions,observing the long-term effect of stent treatment,the impact of the stent on patients'quality of life,formulating health policies,rational allocation of medical resources,and the establishment of long-term follow-up mechanisms.

ABSTRACT

Objective To explore and analyze the application effect of hospital quality monitoring system in medical quality management.Methods 800 patients from March 2021 to March 2022 were selected as the control group and routine medical quality management was adopted;and 800 patients from April 2022 to April 2023 were selected as the experimental group and the hospital quality monitoring system was adopted for medical quality management.Results The complication rate and re-admission rate after implementation were significantly lower than before implementation,with statistical significance(P<0.05).The satisfaction of patients after implementation was significantly higher than before implementation,and the difference was statis-tically significant(P<0.05).The score of medical service quality after implementation was significantly higher than before im-plementation,and the difference was statistically significant(P<0.05).Conclusion The application effect of hospital quality monitoring system in medical quality management is remarkable,and it is worthy of promotion.

ABSTRACT

ABSTRACT Objective: To build and validate an instrument to evaluate Lean Healthcare in healthcare institutions. Method: Methodological study conducted in three stages: 1) Instrument construction; 2) Content validity using the Delphi technique with 14 experts; and 3) Construct validation using Structural Equation Modeling with sample consisted of 113 professionals with experience in Lean Healthcare. Data collection carried out from October/2020 to January/2021 using a digital form. Data analysis performed with the SmartPLS2.0/M3 software. Results: Items were developed after an integrative review and divided into the dimensions Structure, Process and Outcome, according to Donabedian's theoretical framework. Content validation in two rounds of the Delphi technique. Final instrument, after model adjustment, containing 16 items with Cronbach's alpha of 0.77 in Structure, 0.71 in Process and 0.83 in Outcome. Conclusion: The instrument presented evidence of validity and reliability, enabling its use in healthcare institutions to evaluate Lean Healthcare.


RESUMEN Objetivo: Construir y validar un instrumento para evaluar Lean Healthcare en instituciones de salud. Método: Estudio metodológico realizado en tres etapas: 1) Construcción del instrumento; 2) Validez de contenido mediante técnica Delphi con participación de 14 expertos; 3) Validez de constructo mediante Modelado de Ecuaciones Estructurales con muestra compuesta por 113 profesionales con experiencia en Lean Healthcare. La recopilación de datos se realizó de octubre/2020 a enero/2021 mediante formulario digital. El análisis de datos se realizó con el software SmartPLS2.0/M3. Resultados: Ítems elaborados después de revisión integradora y divididos en las dimensiones Estructura, Proceso y Resultado, según referencial teórico de Donabedian. Validación de contenido en dos rondas de la técnica Delphi. Instrumento final, después del ajuste del modelo, contiene 16 ítems con alfa de Cronbach 0,77 en Estructura, 0,71 en Proceso y 0,83 en Resultado. Conclusión: El instrumento presentó evidencias de validez y confiabilidad, permitiendo uso para evaluar Lean Healthcare.

ABSTRACT

ABSTRACT Objective: to investigate the contributions of applying the Lean methodology to improve work processes in health and nursing and its impact on associated financial aspects. Method: an integrative review, carried out in six databases, whose sample of ten (100.0%) studies was analyzed and summarized descriptively. Results: the outcomes obtained were stratified into: benefits/barriers to Lean Healthcare implementation; economic aspects involving Lean Healthcare implementation; and process improvements through Lean Healthcare implementation. The majority of studies (60.0%) were carried out in university hospitals, contexts that need to continually improve the quality of services provided, generally with scarce and limited resources, which support the viability of maintaining the teaching, research and extension tripod. Conclusion: three (30.0%) studies highlighted the financial aspects associated with Lean methodology application. The others only mentioned the possibility of financial gains through improving processes and reducing waste.


RESUMEN Objetivo: investigar las contribuciones de la aplicación de la metodología Lean a la mejora de los procesos de trabajo en salud y enfermería y su impacto en los aspectos financieros asociados. Método: revisión integradora, realizada en seis bases de datos, cuya muestra de diez (100,0%) estudios fue analizada y resumida de forma descriptiva. Resultados: los resultados obtenidos se estratificaron en: beneficios/barreras para la implementación de Lean Healthcare; aspectos económicos que implican la implementación de Lean Healthcare; y mejoras de procesos mediante la implementación de Lean Healthcare. La mayoría de los estudios (60,0%) se realizaron en hospitales universitarios, contextos que necesitan mejorar continuamente la calidad de los servicios prestados, generalmente con recursos escasos y limitados, que sustentan la viabilidad de mantener el trípode de docencia, investigación y extensión. Conclusión: tres (30,0%) estudios destacaron los aspectos financieros asociados a la aplicación de la metodología Lean. Los demás solo mencionaron la posibilidad de obtener ganancias financieras mejorando los procesos y reduciendo el desperdicio.


RESUMO Objetivo: investigar as contribuições da aplicação da metodologia Lean para melhoria dos processos de trabalho em saúde e enfermagem e sua repercussão nos aspectos financeiros associados. Método: revisão integrativa, realizada em seis bases de dados, cuja amostra de dez (100,0%) estudos foi analisada e sintetizada descritivamente. Resultados: os desfechos obtidos foram estratificados em: benefícios/barreiras para implantação do Lean Healthcare; aspectos econômicos envolvendo a implantação do Lean Healthcare; e melhorias em processos por meio da implantação do Lean Healthcare. A maioria dos estudos (60,0%) foi realizada em hospitais universitários, contextos que precisam melhorar, continuamente, a qualidade dos serviços prestados, geralmente com recursos escassos e limitados, os quais sustentam a viabilidade da manutenção do tripé ensino, pesquisa e extensão. Conclusão: três (30,0%) estudos evidenciaram os aspectos financeiros associados à aplicação da metodologia Lean. Os demais apenas mencionaram a possibilidade de ganhos financeiros por meio da melhoria de processos e redução de desperdícios.

ABSTRACT Objective: To analyze the evidence on the influence of Lean and/or Six Sigma for process optimization in the perioperative period. Methods: Integrative review carried out in the MEDLINE (PubMed), Web of Science, EMBASE, CINAHL, Scopus and LILACS databases on the use of Lean and/or Six Sigma to optimize perioperative processes. The studies included were analyzed in three thematic categories: flow of surgical patients, work process and length of stay. Results: The final sample consisted of ten studies, which covered all operative periods. Lean and/or Six Sigma make a significant contribution to optimizing perioperative processes. Final considerations: Lean and/or Six Sigma optimize perioperative processes to maximize the achievement of system stability indicators, making it possible to identify potential problems in order to recognize them and propose solutions that can enable the institution of patient-centered care.


RESUMEN Objetivo: Analizar las evidencias sobre la influencia del Lean y/o Six Sigma para optimizar los procesos en el periodo perioperatorio. Métodos: Es una revisión integradora llevada a cabo en las bases de datos MEDLINE/PubMed, Web of Science, EMBASE, CINAHL, Scopus y LILACS, sobre la utilización del Lean y/o Six Sigma para optimizar los procesos en el perioperatorio. Los estudios incluidos se analizaron en tres categorías temáticas: flujo de pacientes quirúrgicos, proceso de trabajo y tiempo de la estancia. Resultados: La muestra final estuvo formada por diez estudios, los cuales contemplaron todos los períodos operatorios. Lean y/o Six Sigma contribuyen grandemente para mejorar los procesos en el perioperatorio. Consideraciones finales: Lean y/o Six Sigma optimizan los procesos perioperatorios al maximizar el alcance de los indicadores de estabilidad de los sistemas, facilitando la identificación de problemas potenciales para reconocer y proponer soluciones que ayuden a instituir un cuidado más centrado en el paciente.

Lean e/ou Six Sigma contribuem de forma expressiva para a otimização dos processos no perioperatório. Considerações finais: Lean e/ou Six Sigma otimizam processos perioperatórios em vista da maximização do alcance de indicadores de estabilidade dos sistemas, tornando possível a identificação de potenciais problemas para o reconhecimento e proposição de soluções que possam viabilizar a instituição de um cuidado centrado no paciente.

ABSTRACT

Fundamento: la responsabilidad social universitaria constituye un enfoque de gestión académica promotor del desarrollo social sostenible. Objetivo: analizar la gestión del conocimiento en el departamento de Ciencias Básicas Biomédicas, de la Facultad de Ciencias Médicas "Salvador Allende", durante los años 2018 al 2022, sobre la base de la responsabilidad social universitaria. Métodos: se desarrolló una investigación descriptiva, con enfoque cualitativo, complementando las metodologías del estudio de caso y de la teoría fundamentada constructivista. Se utilizaron métodos teóricos: análisis-síntesis, inducción-deducción, ascenso de lo abstracto a lo concreto e histórico lógico; y empíricos: revisión documental, análisis de contenido y grupo focal. Resultados: el análisis realizado evidenció que la gestión del conocimiento en el departamento de Ciencias Básicas Biomédicas, de la referida facultad, ostenta un enfoque estratégico y ha tenido un impacto educativo positivo como resultado del aprendizaje organizacional, y está vinculado con la gestión académica. Conclusiones: la gestión del conocimiento departamental determinó una actualización de la cultura organizacional, causante de un impacto social favorable, mediado por una intervención educativa tributaria de una formación profesional pertinente.


Background: university social responsibility constitutes an academic management approach that promotes sustainable social development. Objective: to analyze knowledge management in the Department of Basic Biomedical Sciences, of the "Salvador Allende" Faculty of Medical Sciences, from 2018 to 2022, based on university social responsibility. Methods: a descriptive investigation was developed, with a qualitative approach, complementing the methodologies of the case study and the constructivist grounded theory. Theoretical methods were used: analysis-synthesis, induction-deduction, ascent from the abstract to the concrete and historical logic; and empirical ones: documentary review, content analysis and focal group. Results: the analysis carried out showed that knowledge management in the Department of Basic Biomedical Sciences, of the aforementioned faculty, has a strategic approach and has had a positive educational impact as a result of organizational learning, and it´s linked to academic management. Conclusions: the departmental knowledge management determined an update of the organizational culture, causing a favorable social impact, mediated by an educational intervention tributary of a relevant professional training.

RESUMO: Objetivo: compreender a percepção dos profissionais atuantes em um hospital universitário sobre a cultura de segurança do paciente. Método: estudo descritivo, com abordagem qualitativa, realizado em um hospital universitário do Nordeste do Brasil no mês de janeiro de 2023. Participaram 56 profissionais da equipe multidisciplinar: enfermeiro, farmacêutico, fisioterapeuta, fonoaudiólogo, médico, psicólogo e técnico de enfermagem. Os dados foram obtidos por entrevistas semiestruturadas. O conteúdo textual foi submetido à análise lexicográfica e literatura atual e relevante. Resultados: foram analisadas cinco classes: percepções acerca da segurança do paciente e a cultura de segurança; ações para fortalecer a segurança do paciente; percepções acerca da atuação do Núcleo de Segurança do Paciente; contribuições para fortalecer a cultura de segurança e atuação profissional na segurança do paciente. Considerações finais: a cultura de segurança do paciente no hospital tem trilhado trajetórias promissoras. O estudo pode direcionar intervenções que fortaleçam a cultura de segurança do paciente.


ABSTRACT Objective: to understand the perception of professionals working in a university hospital about the patient safety culture. Method: a descriptive study with a qualitative approach, carried out at a university hospital in the Brazilian Northeast region in January 2023. The participants were 56 professionals from the multidisciplinary, namely: nurses, pharmacists, physiotherapists, speech therapists, physicians, psychologists and nursing technicians. The data were obtained through semi-structured interviews. The text content was subjected to lexicographic analysis and against the current and relevant literature. Results: five classes were analyzed: Perceptions about patient safety and the safety culture; Actions to strengthen patient safety; Perceptions about the work of the Patient Safety Center; Contributions to strengthening the safety culture; and Professional performance in patient safety. Final considerations: the patient safety culture in the hospital has been on a promising path. The study can direct interventions that strengthen the patient safety culture.


RESUMEN Objetivo: comprender la percepción de los profesionales que trabajan en un hospital universitario sobre la cultura de seguridad del paciente. Resultados: se analizaron cinco clases: percepciones sobre la seguridad del paciente y la cultura de seguridad; acciones para fortalecer la seguridad del paciente; percepciones sobre el desempeño del Centro de Seguridad del Paciente; aportes para fortalecer la cultura de seguridad y el desempeño profesional en seguridad del paciente. Consideraciones finales: la cultura de seguridad del paciente en el hospital ha recorrido una trayectoria prometedora. El estudio puede orientar intervenciones que fortalezcan la cultura de seguridad del paciente.

ABSTRACT

El establecimiento de un sistema de gestión de la calidad es obligatorio para algunas instituciones de salud, entre las que se encuentra el Laboratorio de Biología Molecular, esto queda explícito en la Regulación D 03-21, que plantea la obligatoriedad de su cumplimiento para los laboratorios clínicos que se basan en técnicas de biología molecular. El objetivo del presente texto es exponer los presupuestos teóricos que sustentan un estudio sobre la gestión de la calidad en el mencionado laboratorio. Se realiza una revisión documental a partir de artículos publicados en el período comprendido de 2012-2022. Del total de las fuentes consultadas en las plataformas de acceso a bases de datos de la red de información de la salud cubana Infomed: PubMed, Scopus, SciELo, Redalyc, y Dialnet, se seleccionaron 33 para la revisión al considerar que abordaban el tema con mayor profundidad. La adopción de un sistema de gestión de la calidad garantiza y gestiona el alto nivel de los servicios, aumenta la satisfacción del cliente y el prestigio de la organización.


The establishment of a quality management system is mandatory for some health institutions, among which is the Molecular Biology Laboratory. This is explicit in Regulation D 03-21, which makes compliance mandatory for laboratories. clinical trials that are based on molecular biology techniques. The objective of this text is to present the theoretical assumptions that support a study on quality management in the aforementioned laboratory. A documentary review is carried out based on articles published in the period from 2012-2022. Of the total sources consulted on the database access platforms of the cuban health information network Infomed: PubMed, Scopus, SciELo, Redalyc, and Dialnet, 33 were selected for the review considering that they addressed the topic with greater depth. The adoption of a quality management system guarantees and manages the high level of services, increases customer satisfaction and the prestige of the organization.
