ABSTRACT
Abstract Objective: to build and validate a clinical simulation scenario on hospital nurse managerial decision-making competence for undergraduate nursing students. Method: a descriptive and methodological study was carried out in a higher education institution, with the participation of 10 judges and five players. To do so, the conceptual simulation model proposed by Jeffries and standards of the International Nursing Association for Clinical Simulation and Learning were used to prepare the scenario and the checklist. Results: the scenario was called "Managerial decision-making of nurses in the face of adverse events in a hospital". The scenario script and checklist were built for validation. The checklist was face- and content-validated. Afterward, judges used the checklist to validate the scenario, which, in its final version, was composed of Prebriefing (seven items), Scenario in Action (18 items) and Debriefing (seven items). Conclusion: the scenario proved to be a teaching strategy that anticipates the reality of future nurses, bringing them the self-confidence to perform their activities and helping them to act critically and reflectively during decision-making processes.

Abstract: Introduction: Clinical reasoning is considered one of the main skills that must be developed by medical students, as it allows the establishment of diagnostic hypotheses and directs investigative and diagnostic strategies using a rational approach. Although educators have traditionally focused the teaching method on the analytical model, many medical professors face the challenge in their daily lives of finding new strategies to help their students develop clinical reasoning. Objective: To carry out an integrative literature review to identify the strategies used in the teaching-learning process of clinical reasoning in Brazilian medical schools. Method: The methodology used consists of six steps: 1. creation of the research question; 2. definition of inclusion and exclusion criteria; 3. list of information to be extracted; 4. evaluation of included studies; 5. interpretation of results and 6. presentation of the review. Results: Most studies indicate that the teaching of clinical reasoning is carried out through discussions of clinical cases, incidentally, in different disciplines or through the use of active methodologies such as PBL, TBL and CBL. Only three studies presented at conferences disclosed experiences related to the implementation of a mandatory curricular discipline specifically aimed at teaching clinical reasoning. The teaching of clinical reasoning is prioritized in internships in relation to the clinical and pre-clinical phases. Final considerations: There are few studies that analyze how clinical reasoning is taught to medical students in Brazilian medical schools. Although more studies are needed, we can observe the lack of theoretical knowledge about clinical reasoning as one of the main causes of the students' difficulty in developing clinical reasoning.
ABSTRACT
ABSTRACT BACKGROUND: New medical schools and new medical residencies in Brazil, mainly in its interior, were opened under the justification of collaborating towards distribution of these healthcare professionals and specialist doctors across the national territory. However, this proposal did not guarantee that medical practitioners would become established in the place where they graduated and specialized. OBJECTIVE: To calculate, through interviews, how many specialists who graduated in the state of Tocantins stayed there after finishing their medical residency; and to analyze the factors that made them stay or leave the place. DESIGN AND SETTING: Cross-sectional exploratory study conducted at a Brazilian federal public higher education institution. METHODS: All graduates from medical residencies in Tocantins, who graduated between 2013 and 2019, were contacted by telephone and, after obtaining consent, an interview was conducted. The interviews took place between June 2020 and January 2021. RESULTS: The permanence of medical residency graduates in the state increased from 50% in an earlier study to 55.8% in the current study, thus showing a situation of stability. In addition, we detected some reasons for staying or not. In a multivariate analysis, only working in the state capital was related to staying in the state of Tocantins, showing a 5.6 times greater chance. CONCLUSIONS: The percentage of those who remained was just over 50%, even some years after implementation of the first programs. Most specialists remained working for the state health department, with a smaller proportion in municipal health departments, and were concentrated in the state capital.
ABSTRACT
Abstract Several family literacy initiatives to promote early reading and literacy skills have emerged in the last few decades. These initiatives provide high-quality linguistic experiences that compensate for socioeconomic gaps among young children. Among other activities, family literacy programs provide families with books, strategies, and other materials that allow them to engage in meaningful linguistic interactions, enhance vocabulary and syntax development, and build conceptual knowledge through conversations about stories, rhymes, and other printed material. Children's ability to construct narrative discourse from stories and picture books that have been explored and discussed with adults is of particular importance. For example, converging evidence demonstrates that children who can produce coherent narrative discourse have less difficulty learning to comprehend texts in school. Similarly, facilitating children's interaction with written text through shared reading and dialogue over printed material has proven to facilitate alphabet knowledge acquisition, a skill that later facilitates learning to read and write in school. This study discusses the gains of low socioeconomic background Chilean students ages 4 to 6, whose parents enrolled in family literacy workshops to implement read aloud and language games at home. The purpose of the workshops is to provide parents and caregivers with a structured, evidence-based framework with the purpose of promoting literacy and socioemotional development among children. Parents and other caregivers who volunteered to participate in the program attended 12 biweekly workshops where they learned and engaged in shared reading, oral interactions and language games at home with their children. These activities were modeled by coaches, and parents later practiced them and received feedback. Each family received books and games at every workshop session, and these materials were utilized by them when they read to their children. Using a quasi-experimental design, it was possible to see that, although both groups increased their narrative and alphabet knowledge skills over the 12-week period, students whose parents attended the program had significantly higher scores on a narrative skills task and alphabet knowledge than their peers whose parents did not. As expected, older children in the treatment group outperformed their younger peers in narrative skills. In terms of alphabet knowledge, the children in the treatment group nearly doubled the scores of those in the control group, suggesting that the interactions and language games facilitated the acquisition of this kind of knowledge. These findings suggest that well-structured, evidence-based family literacy intervention programs can strengthen children's language skills, particularly among those who enter school with lower literacy levels given their vulnerability status. In high-poverty contexts as those of most Latin American countries, family literacy programs are an important asset to narrow the learning gaps prior to school entry.

ABSTRACT
The present work is a study carried out with 17 children aged between 4 and 5 years old, all enrolled at the Child Education Center - campus of Universidade Estadual de Londrina (CEI-UEL), with objective of evaluating children motor performance through the TGMD-3, before and after a motor intervention, performed at CEI-UEL. The intervention consisted of 29 60-minute sessions with different motor skills, and after the 29 sessions, TGMD-3 was applied again to assess whether the intervention had positively influenced the performance of the children in the sample. Our results showed that for the sum of the walking and ball skills showed an increase in the comparison of the pre-test to the post-test, but only in the walking skills the difference was significant. As for the percentile, in the skills with the ball a lower performance was found in the post-test, but without any significant difference between the results; for the locomotion skills, an improvement was found in the post-test according to the percentile, not showing a significant difference compared to pretest. Regarding the score, no significant difference was found for the skills, but in the locomotion skills there was a superior performance in the post-test and in the skills with ball there was a lower performance. As a conclusion, we can deduce that motor intervention was positive for improving children motor performance, but not for all variables, showing that the motor intervention could be a beneficial tool to children motor performance.

Over the last decade, medical schools have incorporated distance learning tools, with numerous intervention areas available. Clinical reasoning is an area that requires continuous training and effective feedback. Given this, arises the question about the reception of a gamified learning tool for medical clinical reasoning that delivers effective feedback intended for the stimulation of learning. For this, a multiplatform tool compatible with desktop and mobile devices was developed based on Ren'Py, which was distributed to fourth and fifth year students. Subsequently, a perception survey was applied to evaluate the reception of the tool. Among the points evaluated, it stands out to be a motivating way of reinforcing the studies, with delivery of pertinent information and ease of use. It also allows the recognition of weaknesses and is considered a contribution to training by the students. As a result of this implementation, it was possible to confirm the usefulness of the effective feedback delivered instantly within structured clinical cases, opening new possibilities to the range of tools available in higher education.

ABSTRACT The aim of the study is to investigate how the volleyball service performance of subjects with different levels of experience is influenced by the interaction of different constraints. The study included 36 male volunteers (16.44 ± 0.50 years) allocated into two groups according to the level of experience. Participants performed 10 attempts of the serve volleyball in each of the four experimental conditions. One constraint related to the difficulty of the goal and another related to the physical environment for the execution of the task were manipulated. The performance of the overhand volleyball serve was assessed by means of a movement pattern checklist and of the accuracy to target. The experienced group maintained performance in all conditions, while the inexperienced group showed less consistency of accuracy in conditions with physical constraint and worse score of the movement pattern in the condition of greater difficulty of the goal in interaction with physical constraint. The level of experience had a greater influence on performance than the other constraints. One possible explanation is that it is a constraint capable of maximizing the way that the subject explores the information available by the other constraints

ABSTRACT
Objective:To improve training quality and to standardize the assessment procedures of radiology standardized residency training by analyzing the results of the clinical skills completion assessment for radiology standardized residency training in Liaoning Province in 2020.Methods:Totally 186 trainees participated the assessment with 6 exams in 3 days, including "Man-Machine interview" and "oral interview". Reliability and validity of the test were analyzed first. The accuracy were compared among the three parts of radiology, ultrasound and nuclear medicine, and also among the three portions of imaging description, diagnosis and relative question. Trainees of different type, gender and from different regions were further compared respectively. Scores of different examiners were also compared. SPSS 20.0 software was used for t-test and one-way ANOVA. Results:The Cronbach's α coefficient ranged from 0.65 to 0.87, and KMO values were nearly or higher than 0.7. Scores of both "Man-Machine interview" and "oral interview" parts were better relative to the total scores (relativity coefficient 0.698-0.969). The accuracy of radiology was relatively higher and more stable (nearly 80%), nuclear medicine had the lowest accuracy (nearly 50%), and that of ultrasound fluctuated obviously (56.2%-88.8%). The accuracy was the highest for imaging description, followed by diagnosis, and the lowest for relative question portion. There were significant differences between male and female residents in "Man-Machine interview" portion ( P=0.028), while there was not in "oral interview" ( P=0.050). There were significantly statistical differences in trainees' scores between different regions ( P=0.009 for "Man-Machine interview" and P<0.001 for "oral interview"). Trainees from Shenyang and Dalian got almost the same scores, which were significantly higher than those from other cities. Trainees with master's degree were scored higher than others trainees that were not (all P<0.001). Significant score differences were found between the two examiners in only one examiner group (1/18) ( P=0.011). Conclusion:By the analysis of test results, we find the shortage of both training and assessment process, which is beneficial to adjusting the training process, improving the training quality, and promoting the further standardization and development of the assessment system of radiology standardized residency training.
ABSTRACT
Objective:To observe the effect of simulation combined with problem-based learning (PBL) in the clinical practice teaching of internal medicine. Methods:A total of 41 internal medicine interns accepted from January 2019 to August 2019 in The Third Hospital of Hubei Medical University were selected as the control group, and the traditional clinical teaching was adopted. Another 43 internal medicine interns received from September 2019 to August 2020 were selected as the experimental group, and the simulation combined with PBL teaching was adopted. The clinical skill knowledge and practical operation skills were compared between the two groups in the examination results, the changes in clinical ability before and after the internship, and the recognition of the teaching. SPSS 25.0 was used for t-test, Chi-square test, and rank sum test. Results:The clinical skill knowledge and practical operation skills of interns in the experimental group [(94.51±4.49), (91.05±7.66)] were higher than those in the control group [(89.20±9.23), (85.63±11.57)] ( P<0.05). There was no significant difference in various scores and total scores of clinical competences (including medical interview skills, physical examination skills, humanistic care skills, clinical judgment skills, communication skills, organizational effectiveness, and overall clinical competence) between the two groups before internship. After the internship, the above scores and total scores of the two groups were higher than those before the internship, and the experimental group was higher than the control group ( P < 0.05). The total recognition of the teaching by interns in the experimental group was 95.35% (41/43), which was significantly higher than 78.05% (32/41) in the control group. Conclusion:Simulation combined with PBL teaching can significantly improve the clinical skill knowledge, practical operation level, clinical ability, and recognition of teaching of medical interns.

SPSS 19.0 was used for t test, chi-square test and rank sum test. Results:After the internship, the theoretical scores of common pediatric diseases, clinical disease characteristics, etiopathogenesis, treatment regimen and complications were enhanced in the two groups, and the above scores in observation group were higher than those in control group ( P<0.05). Besides, the DOPS scores and Mini-CEX scores of the two groups were increased, and the scores were higher in observation group than in control group ( P<0.05). Furthermore, the overall excellent rate of the observation group was 87.80% (36/41), which was higher than 69.23% in control group (27/39). Survey found that the number of interns who believed that the teaching regimen could deepen the knowledge mastery degree, increase the internship interest and improve the comprehensive ability, internship efficiency and clinical thinking ability was more in observation group than in control group. Conclusion:Typical case combined with practice teaching can effectively deepen the interns' understanding of theoretical knowledge of clinical pediatrics, improve the practical operation ability and arouse the interest in learning.
ABSTRACT
Objective:To evaluate the effectiveness of staged modular theory and practice lectures in laparoscopic training. Methods:In this study, a large group of medical students and a large group of residents were selected, and each of the two groups was divided into an experimental group and a control group respectively. The experimental group received staged modular theory and practice lectures based on the conventional trainings, and the control group only underwent conventional laparoscopic skills training and instruction. Before and after the training, the completion time of 6 basic laparoscopic operations, laparoscopic suture time, suture quality and the number of accidental injuries were recorded and scored in each group according to the assessment criteria. SPSS 26.0 was used for statistical analysis of the assessment data. Independent samples t-test or Mann-Whitney U test was used for inter-group comparison between the two large groups, and paired samples t-test or Wilcoxon sign rank sum test was used for intra-group comparison before and after the training. Results:Compared with the control group, the experimental group of medical students that had undergone staged modular theory and practice lectures had a shorter time to complete basic laparoscopic operations, faster speed of suturing ( t=6.18, P<0.001) and higher quality of suturing ( t=4.17, P<0.001) and fewer accidental injuries ( Z=-2.03, P=0.043); the experimental group of residents that had undergone staged modular theoretical and practical lectures had a shorter time to complete basic laparoscopic operations, faster speed of suturing ( t=3.31, P=0.002) and higher quality of suturing ( t=3.68, P=0.001) and fewer accidental injuries ( Z=-2.44, P=0.015). Conclusion:The staged modular theory and practice lectures are able to further improve the quality of basic laparoscopic skills training.

ABSTRACT
Abstract Aim: To identify and describe the manipulative actions of typical children ages between 4 and 6 years, and the games performed while exploring cubes with different sensorial stimuli. Methods: Cross-sectional, descriptive, and exploratory study. Sixteen typical children aged 4 to 6 years (mean age: 5.78 ± 1.04 years) were filmed, from different angles, during the exploration of six cubes with different sensorial stimuli (transparent, black, tactile, auditory, luminous, and high-contrast). The cubes were delivered in random order and the children explored the cubes for 40 s each. At the end of the explorations, they were asked about what kind of game they could play with that object. The images were observed by the BS Player Profile. Results: Seventeen different types of interactions with the cubes were registered, such as: pushing, shaking, reaching for (bimanual and unimanual), bringing closer to the eyes, knocking, touching with the fingers/hand, throwing upwards, and others. The tactile cube was the favorite among the children (9 children), followed by the luminous cube (6 children). They proposed games with all of the cubes, standing out playing dice (all cubes), building towers, and castles. Conclusion: In the observed group of children, 17 manipulative skills were identified. The games performed during the exploration of the cubes were based on association with other similar objects previously known, related to the visual stimuli provided by the cubes, verbal description of actions that could be performed with that cube, imagination related games using figurative images, and sports-related activities/games.
