Introdução: O estudo explora a cirurgia de contorno corporal em pacientes submetidos a cirurgias bariátricas. Nos últimos anos, esse tipo de intervenção cirúrgica ganhou destaque devido ao aumento na busca por procedimentos reparadores após significativa perda de peso. O texto enfatiza a importância da flancoplastia, técnica que visa melhorar o contorno corporal. Métodos: Foi realizada revisão da literatura para elucidação da anatomia da gordura profunda da fáscia toracolombar, e feito um paralelo com a técnica de flancoplastia utilizada em nosso serviço em pacientes pós-bariátricos submetidos a abdominoplastia 360° ou 270° no Hospital Daher Lago Sul, em Brasília- DF. Resultados: A lipoaspiração profunda dos flancos, muitas vezes necessária em cirurgias de contorno corporal, pode não ser suficiente para retirar a gordura profunda à fáscia toracolombar e durante esse procedimento observa-se risco aumentado de lesões retroperitoneais. No entanto, a flancoplastia, que envolve a ressecção da gordura profunda à fáscia toracolombar, demonstrou proporcionar resultados satisfatórios e minimizar esses riscos. Conclusão: O estudo enfoca a importância do conhecimento anatômico preciso, destacando a relevância da flancoplastia como uma técnica valiosa para melhorar os resultados estéticos e reduzir complicações em pacientes pós-bariátricos, complementando a cirurgia bariátrica e melhorando a qualidade de vida do paciente.
Introduction: The study explores body contouring surgery in patients undergoing bariatric surgery. In recent years, this type of surgical intervention has gained prominence due to the increase in the search for reparative procedures after significant weight loss. The text emphasizes the importance of flankplasty, a technique that aims to improve body contour. Methods: A literature review was carried out to elucidate the anatomy of the deep fat of the thoracolumbar fascia, and a parallel was made with the flankplasty technique used in our service in post-bariatric patients undergoing 360° or 270° abdominoplasty at Daher Lago Sul Hospital, in Brasília-DF. Results: Deep liposuction of the flanks, often necessary in body contouring surgeries, may not be sufficient to remove fat deep into the thoracolumbar fascia and during this procedure, there is an increased risk of retroperitoneal injuries. However, flankplasty, which involves resection of fat deep into the thoracolumbar fascia, has been shown to provide satisfactory results and minimize these risks. Conclusion: The study focuses on the importance of precise anatomical knowledge, highlighting the relevance of flankplasty as a valuable technique to improve aesthetic results and reduce complications in post-bariatric patients, complementing bariatric surgery and improving the patient's quality of life.

Objective: Acute appendicitis is one of the most common causes of an acute abdomen and is one of the most frequently performed abdominal surgeries. One of the early post-operative complications of appendectomy is surgical site infection (SSI) which is a major medical concern as it increases the burden on the patient and the health-care system. Obesity is an independent risk factor for SSI. Body mass index is most commonly used to define obesity but is a non-specific indicator for the risk of SSI. Thickness of the subcutaneous fat pad is a more targeted measure to assess the risk of SSI. The aim is to study the association between the thickness of subcutaneous fat and SSIs in open appendectomy wounds. Methods: With a sample size of 128, the study was carried out at Pravara Rural Hospital, Loni, from September 2023 to December 2024. All patients who underwent emergency or elective open appendectomy during the time period were included in the study and had an abdominal USG prior to surgery to gauge the thickness of the subcutaneous fat pad in the right iliac fossa and were followed for 1 month for SSIs. Results: The data indicate an association between increased rates of SSIs and patients who had thicker subcutaneous fat pad at the site of the incision.

Objective To explore the correlation between skeletal muscle fat content and insulin resistance(IR)in patients with type 2 diabetes mellitus(T2DM),and to evaluate the diagnostic value of skeletal muscle fat quantitative parameters for IR.Methods During January 2018 to January 2021,150 T2DM patients as observation group(T2DM group)and 100 healthy subjects as control group(NC group)were selected.All subjects underwent magnetic resonance imaging quantitatively analyzed skeletal muscle fat content,including intermuscular fat(IMAT),adipose tissue beneath fascia(SFAT)and subcutaneous fat(SCAT).Receiver operating characteristic(ROC)curve was used to analyze the diagnostic value of skeletal muscle fat quantitative parameters for IR in T2DM patients.Results The levels of IMAT and SFAT were higher in T2DM group than those in NC group[(10.05±1.34)%vs(7.16±2.06)%,(3.64±0.54)%vs(3.40±0.75)%,P<0.05],while the level of SCAT was lower than in T2DM group than in NC group[(20.16±6.34)%vs(24.97±6.57)%,P<0.05].Pearson correlation analysis showed that IMAT and SFAT were positively correlated with HbA1c and HOMA-IR(P<0.05),while SCAT was negatively correlated with HbA1c and HOMA-IR(P<0.05).ROC curve analysis showed that the area under the curve of IMAT,SFAT,and SCAT for the diagnostic value of T2DM combined with IR were 0.716,0.667 and 0.736,with sensitivities of 75.4%,72.9%and 76.4%,and specificity of 71.4%,65.2%and 68.1%.Conclusion Skeletal muscle fat contentis associated with IR.The quantitative parameters of skeletal muscle fat have good diagnostic efficacy for T2DM complicated with IR.

Objective:To investigate the correlation between fat distribution and the composite indices of femoral neck strength in obese postmenopausal women.Methods:A total of 293 postmenopausal women with non-low body weight were selected, laboratory tests, body composition analyzer test and double-energy X-ray absorptiometry scan were performed. Based on the body mass index(BMI), they were divided into three groups, the normal BMI group(18.5 kg/m 2≤BMI<24.0 kg/m 2, n=91), the overweight group(24.0 kg/m 2≤BMI<28.0 kg/m 2, n=115), and the obese group(BMI≥28.0 kg/m 2, n=87). The measurement results were analyzed. Results:In the obese group, bone mineral density(BMD) of all sites was higher than that in the normal BMI group and overweight group( P<0.005), compression strength index(CSI), bending strength index(BSI), and impact strength index(ISI) were significantly lower than those in the normal BMI group( P<0.001, P=0.008, P=0.001). In the obese group, waist circumference, waist-hip ratio, total fat mass, appendicular fat mass, and trunk fat mass were risk factors for CSI, BSI and ISI independent of age, fasting blood glucose, and BMI( P<0.05). Visceral fat grade and Chinese visceral adiposity fat index were the risk factors for CSI, BSI, and ISI( P<0.05). Conclusion:The composite indices of femoral neck strength decreased in obese postmenopausal women, and both subcutaneous fat and visceral fat were negatively associated with the composite indices of femoral neck strength.

RESUMO Com o crescimento da indústria cosmética global, a busca pela estética e pelo rejuvenescimento impulsionou o aumento de procedimentos estéticos. A gordura autóloga é o tipo de preenchimento mais comum e mais grave relacionado à cegueira iatrogênica. A oclusão iatrogênica da artéria oftálmica é uma complicação rara (mas devastadora) das injeções de preenchimento facial. Embora diversos tratamentos tenham sido relatados e propostos, até o momento não há eficácia comprovada. Na identificação da oclusão de artéria central da retina, o tempo é de fundamental importância para o prognóstico visual do paciente. Os resultados, em geral, são insatisfatórios, e há pouca ou nenhuma evidência favorável. O objetivo deste trabalho foi relatar o caso de uma paciente que apresentou oclusão de artéria central da retina após procedimento cosmético com gordura autóloga em região nasolabial.
ABSTRACT The global cosmetic industry growth and the desire for aesthetics and rejuvenation have increased the aesthetic procedures. Autologous fat is the most common and most serious type of filling related to iatrogenic blindness. Iatrogenic occlusion of the ophthalmic artery is a rare but devastating complication of facial filler injections. Although several treatments have been reported and proposed, to date there is no proven efficacy. In identifying central retinal artery occlusion, timing is crucial to the patient's visual prognosis. The results, in general, are unsatisfactory and there is little or no favorable evidence. This paper aimed to report the case of a patient who presented central retinal artery occlusion after a cosmetic procedure with autologous fat in the nasolabial region.

Background: Acute appendicitis is the acute inflammation of appendix accounting to 7-10% of acute abdomen cases. The complications vary from surgical site infection to enterocutaneous fistula and stump appendicitis. The surgical site infections after appendicectomy are postoperative nosocomial infections affecting the incision site, deeper tissues and organs at the operative site within 30 days of surgery. The study was aimed at predicting subcutaneous fat thickness causing surgical site infection in open appendicectomy wounds. Methods: This prospective study was conducted in Trichy SRM Medical College Hospital and Research Centre, Tamil Nadu from January to June 2021 in 82 patients. Patients who underwent open appendicectomy for uncomplicated acute appendicitis were included. Patients underwent preoperative Ultrasound abdomen to measure the subcutaneous fat thickness in right iliac fossa and was correlated with post operative surgical site infection for 1 month. The subcutaneous fat thickness was divided into three groups; I- 5.5 mm and below, II- 5.5 to 7.5 mm and III- more than 7.5 mm. Results: The results show that there was an increased association of surgical site infection in patients with increased subcutaneous fat thickness at incision site with mean of 6.25 and SD of 1.49. Conclusions: The study showed that subcutaneous fat thickness is a predictor for development of surgical site infection in open appendicectomy wounds. The subcutaneous fat thickness association in causing surgical site infection in open appendicectomy wounds is of linear association.

Objective:To investigate the efficacy of autologous fat grafting in the treatment of stable linear scleroderma.Methods:A retrospective analysis was performed on 40 patients with stable linear scleroderma who received autologous fat grafting from October 2017 to November 2020 in the Department of Dermatology, Xijing Hospital, Air Force Medical University. There were 22 males and 18 females, aged 12 - 36 (18.2 ± 4.82) years. Skin lesions involved the forehead in 16 cases, the perioral area in 4, the lower jaw in 2, the cheek in 9, the trunk in 5, and the lower limb in 4, and the size of depressed skin defects was 3 - 24 cm 3. The patients′ subjective satisfaction rate, the decrease in the size of depressed skin defects, and the incidence of adverse reactions after autologous fat grafting were analyzed during the follow-up. Results:Six months after the grafting, 40 patients were followed up and evaluated, and the size of depressed skin defects markedly decreased. The satisfaction rate for appearance improvement after the first grafting was 60% (24/40) ; 35 patients received the second grafting, with a satisfaction rate of 88.6% (31/35) ; 17 received the third grafting, with a satisfaction rate of 94.1% (16/17) ; the total satisfaction rate was 87.5% (35/40). After the grafting, local skin unevenness was observed in the grafting area in 3 patients, and in the liposuction area in 2. The incidence rate of postoperative adverse reactions was 12.5% (5/40) .Conclusion:Autologous fat grafting was markedly effective in the treatment of stable linear scleroderma.

C-fos is a transcription factor that plays an important role in cell proliferation, differentiation and tumor formation. The aim of this study was to clone the goat c-fos gene, clarify its biological characteristics, and further reveal its regulatory role in the differentiation of goat subcutaneous adipocytes. We cloned the c-fos gene from subcutaneous adipose tissue of Jianzhou big-eared goats by reverse transcription-polymerase chain reaction (RT-PCR) and analyzed its biological characteristics. Using real-time quantitative PCR (qPCR), we detected the expression of c-fos gene in the heart, liver, spleen, lung, kidney, subcutaneous fat, longissimus dorsi and subcutaneous adipocytes of goat upon induced differentiation for 0 h to 120 h. The goat overexpression vector pEGFP-c-fos was constructed and transfected into the subcutaneous preadipocytes for induced differentiation. The morphological changes of lipid droplet accumulation were observed by oil red O staining and bodipy staining. Furthermore, qPCR was used to test the relative mRNA level of the c-fos overexpression on adipogenic differentiation marker genes. The results showed that the cloned goat c-fos gene was 1 477 bp in length, in which the coding sequence was 1 143 bp, encoding a protein of 380 amino acids. Protein structure analysis showed that goat FOS protein has a basic leucine zipper structure, and subcellular localization prediction suggested that it was mainly distributed in the nucleus. The relative expression level of c-fos was higher in the subcutaneous adipose tissue of goats (P < 0.05), and the expression level of c-fos was significantly increased upon induced differentiation of subcutaneous preadipocyte for 48 h (P < 0.01). Overexpression of c-fos significantly inhibited the lipid droplets formation in goat subcutaneous adipocytes, significantly decreased the relative expression levels of the AP2 and C/EBPβ lipogenic marker genes (P < 0.01). Moreover, AP2 and C/EBPβ promoter are predicted to have multiple binding sites. In conclusion, the results indicated that c-fos gene was a negative regulatory factor of subcutaneous adipocyte differentiation in goats, and it might regulate the expression of AP2 and C/EBPβ gene expression.

A esclerodermia localizada (morfeia linear) é uma doença rara que causa distrofia e assimetria faciais em mulheres jovens. É relatado o caso de uma paciente de 20 anos que foi tratada com enxertos compostos de gordura. Após a coleta do lipoaspirado de gordura subcutânea profunda, a microgordura e a nanogordura não filtrada foram processadas separadamente para formar enxertos compostos de gordura. Estes foram meticulosamente transplantados em camadas, para reconstruir o subcutaneo da face da paciente. Após um ano, observou-se boa pega de gordura com correção bem-sucedida da assimetria e restabelecimento do equilíbrio facial.
Localized scleroderma (linear morphea) is a rare disease that causes facial dystrophy and asymmetry in young women. We report the case of a 20-year-old female patient who was treated with novel composite fat grafts. After extracting lipoaspirate from deep subcutaneous fat, the microfat and the unfiltered nanofat were processed separately, and amalgamated, obtaining composite fat grafts. These were meticulously transplanted layer by layer to rebuild the subcutaneous tissue of the patient's face. After one year, a good volume of fat retention was observed with successful correction of asymmetry and restoration of facial balance.

ABSTRACT Mechanical liposuction of cervicomental fat is part of the aesthetic procedures of orofacial harmonization. Ultrasonography stands out for illustrating soft tissues with high-resolution images in the diagnostic and postoperative stages. The present study aimed to report the case of a female patient, 54 years old, in good general health, confirmed by all previously requested laboratory tests, whose main complaint was the presence of excess fat in the jowl region. The stages after the liposuction procedure were followed by high-resolution ultrasound examinations in order to assess the healing of the manipulated tissues, and ultrasound evaluations were performed in the postoperative follow-up periods of 30, 60 and 120 days. Through the present study, it was possible to conclude that ultrasonography is an important ally in the postoperative follow-up of neck liposuction, allowing to follow the evolution of the post-surgical repair process.
RESUMO A lipoaspiração mecânica da gordura cervical faz parte dos procedimentos estéticos de harmonização orofacial. A ultrassonografia se destaca por evidenciar tecidos moles com imagens de alta resolução nas fases diagnóstica e pós-operatória. O presente estudo teve como objetivo relatar o caso de uma paciente do sexo feminino, 54 anos, com bom estado geral de saúde, confirmado por todos os exames laboratoriais previamente solicitados, cuja queixa principal era a presença de excesso de gordura na região da papada. As etapas após o procedimento de lipoaspiração foram acompanhadas por exames ultrassonográficos de alta resolução para avaliar a cicatrização dos tecidos manipulados e as avaliações ultrassonográficas foram realizadas nos períodos de acompanhamento pós-operatório de 30, 60 e 120 dias. Através do presente estudo foi possível concluir que a ultrassonografia é uma importante aliada no acompanhamento pós-operatório de lipoaspiração cervical, permitindo acompanhar a evolução do processo de reparo pós-cirúrgico.

Introdução: A evolução da técnica de lipoaspiração e da enxertia de gordura associada a segurança oncológica permitiram utilizar a gordura como preenchedor autólogo em pacientes com neoplasia de mama que eram submetidas a reconstrução mamária. O objetivo é apresentar uma inovação no instrumento de coleta de gordura que será utilizada como enxerto, para uso em reconstrução mamária. Métodos: Foi realizada a busca de anterioridade nos bancos de dados internacionais e nacional e a confecção de um protótipo de cânula de coleta de gordura de 35cm de comprimento, com diâmetro do tubo de 4mm, com 10 furos redondos na sua extremidade distal com 2mm de diâmetro. Foi realizado um experimento de aspiração de substância viscosa comparando o protótipo com dois modelos de cânulas com desenho padrão da indústria, de três furos tipo Mercedes e de cinco furos tipo Pitanguy. Resultados: Foram encontrados quatro patentes de alta relevância e uma patente de média relevância, que diferem do modelo de utilidade proposto quando se comparam o tipo de desenho da extremidade distal das cânulas estudadas e a função dos instrumentos identificados. Quando comparou-se a eficiência do protótipo, a cânula de cinco furos foi a mais eficiente na aspiração da substância viscosa, e não houve diferença estatística na velocidade de aspiração entre o protótipo e a cânula de três furos tipo Mercedes. Conclusão: A inovação apresentada para a coleta de enxerto de gordura para uso em reconstrução mamária apresentou a mesma eficiência que a cânula Mercedes de três furos neste modelo experimental.
Introduction: The evolution of liposuction and fat graft technique with the oncologic safety allowed to use the fat as autologous filler in patients with breast cancer and submitted to breast reconstruction. The objective is to introduce an innovation in the instrument used to harvest fat grafts for breast reconstruction. Methods: Search of anteriority and trademarks were performed at international and national databases, and a prototype was built as a fat harvest instrument with 35cm in length, 4cm in diameter, with 10 holes at the distal extremity, with 2mm diameter each. It was experimented with a harvest of a viscous substance, comparing the prototype with two regular industry cannulas types: three holes Mercedes' type and five roles Pitanguy's type. Results: Four high-relevance patents and one medium-relevance patent were found, which differ from the proposed utility model when comparing the type of design of the distal end of the studied cannulas and the function of the instruments identified. When the prototype's efficiency was compared with the other cannulas, the cannula with five holes in Pitanguy's type was the most efficient, and it was no statistical difference between the prototype and the cannula with three roles in Mercedes' type. Conclusion: The innovation presented to harvest fat graft for breast reconstruction had the same efficiency in harvesting the viscose substance as the cannula Mercedes type with three holes in this experimental model.

Introdução: O conceito da estética feminina vem apresentando mudanças, com forte influência dos padrões culturais e regionais. Glúteos e mamas são as principais regiões enfatizadas, principalmente na América Latina. Diante disto, a lipoenxertia glútea adquire uma importância crescente. Neste trabalho, objetivamos estudar os resultados deste procedimento e suas complicações, sugerir uma classificação do formato dos glúteos e padronização de áreas a serem tratadas com a lipoenxertia. Métodos: Trata-se de um estudo prospectivo, realizado de janeiro de 2018 a fevereiro de 2020, com seguimento pós-operatório mínimo de 6 meses. Foi utilizada a técnica tumescente para obtenção da gordura, tratada por decantação, e injetada com seringa e cânula via sulco interglúteo e sulco subglúteo. Resultados: Dos 146 pacientes, nove foram excluídos, restando 137, sendo 132 mulheres e cinco homens. A idade média dos pacientes foi de 34,1 anos e o índice de massa corporal médio foi de 27,2Kg/m2. O formato de glúteo predominante foi o trapezoide (52,5%) e o volume médio de gordura injetado nos glúteos foi de 510,8ml no lado direito e 580,5ml no esquerdo, sendo as áreas central e lateral do glúteo as mais enxertadas. Quanto às complicações, a assimetria ocorreu em 3,65% dos pacientes, bolhas na pele em 2,9% e dor crônica em 2,2% deles. Conclusão: É um procedimento seguro com baixa incidência de complicações, quando é respeitada a restrição ao subcutâneo como plano de lipoenxertia, e de alta satisfação para os pacientes. A classificação e padronização sugeridas podem auxiliar a aprendizagem do procedimento.
Introduction: The concept of female aesthetics has been changing, strongly influenced by cultural and regional patterns. Buttocks and breasts are the main regions emphasized, especially in Latin America. Then, gluteal fat grafting becomes increasingly important. In this article, we aim to study the results of this procedure and its complications, suggest a classification of the shape of the buttocks and standardize areas to be treated with fat grafting. Methods: This is a prospective study realized from January 2018 to February 2020, and there was a follow-up of the patients for 6 months of postoperative. The surgical technique was tumescent to get fat, and decantation was the method to prepare the graft, which was injected through the intergluteal and subgluteal folds. Results: Of 146 patients, nine were excluded, remaining 137, of which 132 were women, and four were men. The mean age of patients was 34.1 years, and the mean body mass index was 27.2Kg/m2. The predominant gluteal shape was the trapezoid (52.5%), and the average volume of fat injected into the buttocks was 510.8ml on the right side and 580.5ml on the left, with the central and lateral gluteal areas being the most grafted. As for complications, asymmetry occurred in 3.65% of patients, skin blister in 2.9% and chronic pain in 2.2% of them. Conclusion: It is a safe procedure with a low incidence of complication when the fat grafting respects the subcutaneous plane, and this procedure has high patient satisfaction.

Abstract Background: Abdominal obesity has been associated with cardiovascular disease and may be modulated by dietary intake. The deep abdominal subcutaneous adipose tissue (dSAT) is a body fat compartment that can be estimated by using mathematical formulas. Objectives: To evaluate the association between dSAT estimated by the Deep-Abdominal-Adipose-Tissue (DAAT) index and dietary intake in patients with acute coronary syndrome (ACS). Methods: This is a cross-sectional study conducted with patients ≥ 18 years of age admitted to a tertiary hospital. Sociodemographic, clinical, and anthropometric (body weight [kg], height [m], waist, hip and neck circumferences [cm]) data were evaluated. A food frequency questionnaire was applied to identify each patient's nutrient intake. The DAAT index was calculated according to specific formulas for men and women. Possible association between food intake and the DAAT index was evaluated by multiple linear regression. The level of significance adopted was 0.05. Results: This study evaluated 138 patients, with a mean age of 61.2±10.8 years. Prevalence of obesity was 29.4% in men and 37.7% in women. Regarding waist circumference, 83% of the women showed values considered to be very high. The DAAT index was significantly higher in men when compared to woman (P < 0.0001) and proved to be positively correlated with proteins (r= 0.22, P= 0.01) and monounsaturated fatty acid (r= 0.18, P= 0.04) intake in the entire sample. After adjustment for sex, alcohol consumption, and levels of physical activity, the DAAT index was associated with the female sex (B= −129.84, P <0.001) and a sedentary lifestyle (B= 57.99, P <0.001). Conclusion: dSAT estimated by the DAAT index was not associated with dietary intake in patients with ACS.

Background Exercise during pregnancy is closely related to maternal and infant health. Previous studies in developed countries have linked maternal exercise during pregnancy with newborn body weight as well as subcutaneous fat thickness. However, the relevant studies in China are limited, and the conclusions remain inconsistent. Objective To investigate the effects of maternal exercise during pregnancy on neonatal weight and subcutaneous fat thickness. Methods Based on the Shanghai Birth Cohort, 959 maternal-infant pairs were included in this study. The International Physical Activity Questionnaire was used to collect average weekly frequency and daily minutes of walking in the first and second trimesters, and entropy weight method was used to calculate the cumulative exercise index in the two trimesters. Birth weight was measured using a calibrated weigh scale. Subcutaneous fat thickness was measured at abdomen, scapula, and triceps with a Harpenden skinfold caliper for all newborns and the sum of the thickness for the three sites was then calculated. A multiple linear regression model was employed to estimate the relationships of cumulative exercise index during pregnancy with neonatal body weight and subcutaneous fat thickness. Subgroup analyses stratified by pre-pregnancy body mass index (BMI) and sex of newborns were also performed. Results The mean age of pregnant women was (28.5±3.8) years, and the pre-pregnancy BMI was (21.4±3.0) kg·m−2. Newborn boys were slightly more than newborn girls (54.3% vs 45.7%), and the neonatal weight was (3374.0±427.5) g. The means of newborns' abdominal, scapular, and triceps subcutaneous fat thickness were (4.4±1.3), (5.4±1.4), and (6.0±1.5) mm, respectively, and the sum of subcutaneous fat thickness was (15.8±3.9) mm. In the first and second trimesters, 77.3% and 88.7% of pregnant women walked 4 d per week and more, respectively; the daily minutes of walking was (36.9±27.2) min and (43.3±26.3) min, respectively; the cumulative exercise index was 25.6±17.7 and 35.9±21.1, respectively. The results of multiple linear regression analysis showed that the cumulative exercise index in the second trimester was negatively associated with newborns' abdominal (b=−0.006, 95%CI: −0.010-−0.003), scapular (b=−0.005, 95%CI: −0.009-−0.002), triceps (b=−0.006, 95%CI: −0.010-−0.002), and their sum of (b=−0.018, 95%CI: −0.028-−0.007) subcutaneous fat thickness (P<0.05); in the first and second trimesters, however, the relationship between maternal cumulative exercise and newborns' body weight was not significant. The results of stratified analyses showed that the negative associations between maternal cumulative exercise index and newborns' subcutaneous fat thickness for the second trimester remained significant in the subgroups of boys and neonates whose mothers had normal pre-pregnancy BMI (P<0.05). Conclusion Cumulative exercise index in the second trimester is negatively correlated with the neonatal thickness of subcutaneous fat, and the association may be altered by neonatal sexes and maternal pre-pregnancy BMI levels.

Introducción: La adiposidad central como factor desencadenante de resistencia a la insulina precoz constituye una amenaza potencial de riesgo metabólico y cardiovascular en el embarazo. Objetivo: Determinar la capacidad discriminante de las grasas abdominales sobre la resistencia a la insulina, diagnosticada por el índice triglicéridos/glucosa-IMC al finalizar el primer trimestre del embarazo. Métodos: Se realizó un estudio observacional analítico de 526 gestantes con embarazo simple y edad gestacional entre 12 y 13 semanas, entre los años 2016 y 2020. Se estudió el test de triglicéridos/glucosa-IMC y las grasas abdominales por ultrasonido. Se utilizaron las curvas ROC (Receiver operating characteristic Curve) para discriminar la resistencia a la insulina al finalizar el primer trimestre de la gestación, cuando aumentan las grasas abdominales. Resultados: La grasa subcutánea fue la que presentó mayor área bajo la curva en la discriminación de la resistencia a la insulina, con un nivel de sensibilidad y especificidad aceptable. Conclusiones: La grasa subcutánea, aunque con bajo valor discriminativo, puede considerarse como augurio de resistencia a la insulina y de diabetes gestacional. Se requiere profundizar en el estudio de las grasas abdominales dado el conocimiento de su impacto en los desórdenes metabólicos en el curso avanzado de la gestación(AU)
Introduction: Central adiposity as a triggering factor for early insulin resistance is a potential threat of metabolic and cardiovascular risk in pregnancy. Objective: To determine the discriminating capacity of abdominal fat over insulin resistance, diagnosed by the triglyceride/glucose-BMI index at the end of the first trimester of pregnancy. Methods: An analytical and observational study was carried out with 526 pregnant women of singleton pregnancy and gestational age between twelve and thirteen weeks, between 2016 and 2020. The triglyceride/glucose-BMI test was studied, together with abdominal fats by ultrasound. ROC (receiver operating characteristic) curves were used to discriminate insulin resistance at the end of the first trimester of gestation, when abdominal fats increase. Results: Subcutaneous fat presented the highest area under the curve in the discrimination of insulin resistance, with an acceptable level of sensitivity and specificity. Conclusions: Subcutaneous fat, although with low discriminative value, can be considered as a harbinger of insulin resistance and gestational diabetes. Further study of abdominal fat is required, given the knowledge of its impact on metabolic disorders in late gestation(AU)

ABSTRACT Objective: To evaluate the effects of combining topiramate, bupropion and naltrexone in obesity-induced rats on their weight and subcutaneous adipose tissue. Methods: A total of 40 male Wistar rats were induced to obesity for 8 weeks and the animals were divided into 8 groups: Ctr - control, G0 - Sham, G1 - oral saline solution (1.0mL/day), G2 - topiramate (20.0mg/kg) and bupropion (5.0mg/kg), G3 - naltrexone (20.0mg/kg), G4 - topiramate (20.0mg/kg), G5 - bupropion (5.0mg/kg) and G6 - topiramate (20.0mg/kg), bupropion (5.0mg/kg) and naltrexone (20.0mg/kg). During the experiment, all animals were weighed weekly. After 30 days of treatment animals were euthanized and their skin fragments were processed and stained with hematoxylin and eosin for morphological, morphometric and biochemical analyzes. Results: The only group that presented a decrease in the volume of subcutaneous adipose tissue was G3, but this decrease was not significant when compared with the other groups. The G4, the G5 and the G6 presented increased adipose tissue volume. Data showed that until the eighth week all animals increased their weight by approximately 50%. After treatment animals of all groups, except G3, increased their weight from 4% to 9% approximately. The G3 was the only group that lost weight, but this decrease was not significant. Conclusion: The medicines studied were not efficient in reducing weight in obese rats. However, it should be considered that 30-day treatment period is not enough to observe the stronger effects of these drugs.

RESUMEN Fundamento: La obesidad abdominal en gestantes es un marcador de riesgo cardiometabólico independientemente de la adiposidad general, siendo la ultrasonografía de gran utilidad para distinguir los compartimientos adiposos del abdomen y diagnosticar tal peligro al inicio de la gestación. Objetivo: Determinar asociaciones entre variables ecográficas de adiposidad abdominal y variables analíticas y antropométricas en gestantes normopeso al inicio del embarazo según fenotipos metabólicos empleando análisis de correlación canónica. Metodología: Estudio transversal en 526 embarazadas normopeso, entre 12 y 14 semanas de edad gestacional, atendidas en consulta de ultrasonido del Policlínico Docente Chiqui Gómez, municipio Santa Clara. Se midieron las grasas abdominales subcutánea, preperitoneal y visceral, así como variables antropométricas y analíticas. Se conformaron 3 fenotipos metabólicos, y se aplicó la correlación canónica para determinar el nexo entre las mismas y su comportamiento en los diferentes fenotipos. Resultados: Se identificaron 2 conjuntos de variables con correlaciones canónicas que se incrementaron del fenotipo saludable al metabólicamente obeso con valores de 0.6930 a 0.8955 y 0.9298 respectivamente y alta significancia estadística (p=0.000). Conclusiones: Se demuestra el nexo entre las variables ecográficas de adiposidad abdominal grasa subcutánea y grasa visceral y las variables analíticas resistencia a Ia insulina, producto de acumulación lipídico, índice aterogénico y glucemia, evidenciado por los altos valores de correlaciones canónicas obtenidos según cambia el fenotipo de normopeso saludable al metabólicamente obeso; orientando un nuevo enfoque en la determinación de fenotipos de riesgo metabólico en la gestación temprana en mujeres normopeso.
ABSTRACT Background: Abdominal obesity in pregnant women is an indicator of cardiometabolic risk with non-independence of general adiposity, being the ultrasound very useful to distinguish the abdomen adipose compartments also diagnose this risk at the beginning of pregnancy. Objective: To determine possible associations between the ultrasound variables of abdominal adiposity and those analytical and anthropometric in normal-weight pregnant women at the beginning of pregnancy according to metabolic phenotypes using canonical correlation analysis. Methodology: A cross-sectional study in 526 normal-weight pregnant women, between 12 and 14 weeks of gestational age, assisted in the ultrasound office at Chiqui Gómez teaching polyclinic, in Santa Clara city. Subcutaneous, preperitoneal and visceral abdominal fats were measured, as well as anthropometric and analytical variables. Three metabolic phenotypes were formed, and canonical correlation was applied to determine their relation and also behavior among the different phenotypes. Results: 2 sets of variables were identified with canonical correlations that increased from the healthy to the metabolically obese phenotype with values from 0.6930 to 0.8955 and 0.9298 respectively and high statistical significance (p=0.000). Conclusions: The link between the ultrasound variables of abdominal adiposity, subcutaneous and visceral fat and the analytical insulin resistance, lipid accumulation product, atherogenic index and blood glucose is demonstrated, evidenced by the high values of canonical correlations obtained as the phenotype changes from healthy normal-weight to metabolically obese; guiding a new approach in the resolve of the metabolic risk phenotypes in early gestation in normal-weight women.

Abstract Background: pH, subcutaneous fat thickness (SFT), and color are fundamental variables to define the organoleptic characteristics of meat. However, multivariate relationships of those traits remain unexplored in bovine meat. Objective: To investigate the multivariate relationships among pH, subcutaneous fat thickness, and color parameters in bovine meat using canonical correlation analysis. Methods: A dataset containing 173 individual records of pH, SFT, and color parameters (a*: intensity of red color, b*: intensity of yellow color, and L*: lightness) from five Brazilian beef cut types (Breed: Nellore; cuts: acém, contrafilé, fraldinha, patinho and picanha) was constructed. Multivariate relationships between color variables (a*, b*, and L*) and chemical variables (pH and SFT) were explored using the CANCORR procedure of SAS. Results: Two canonical correlations between U (a*, b*, and L*; color variables) and V (pH and SFT; chemical variables) variates were significant (p<0.01). First and second canonical correlations were 0.463 and 0.282, respectively. Canonical weights for variates were for U1: a* = 0.707, b* = 0.406, and L* = -0.039; U2: a* = 0.364, b* = -0.898, and L* = 1.234; V1: pH = -0.376 and SFT = 0.935; V2: pH = 0.927 and STF = 0.356. Conclusion: Subcutaneous fat thickness significantly affected intensity of red and yellow colors, whereas pH significantly affected lightness. The results of this study may be useful for a better understanding of the role of muscle metabolism and its implications on the organoleptic characteristics of bovine meat.

Métodos: Foi construído um banco de dados contendo 173 registros individuais de pH, SFT e parâmetros de cor (a*: intensidade de cor vermelha, b*: intensidade de cor amarela y L*: luminosidade) de cinco tipos de cortes de carne bovina brasileira (Raça: Nellore; cortes: acém, contrafilé, fraldinha, patinho e picanha). A relação multivariada entre variáveis de cor (a *, b * e L*) e variáveis químicas (pH e SFT) foi explorada usando o procedimento CANCORR do SAS. Resultados: Duas correlações canônicas entre as variáveis U (composta de a *, b * e L *, variáveis de cor) e V (composta de pH e SFT, variáveis químicas) foram significativas (p<0,01). Introducción. La obesidad infantil puede causar hiperlipidemia y esteatosis hepática y complicaciones crónicas. Nuestro objetivo fue evaluar la relación entre el espesor de la grasa subcutánea abdominal (GSA) y la esteatosis hepática, las transaminasas y los lípidos séricos en niños obesos.Población y métodos. Estudio retrospectivo en niños (4-18 años) que acudieron a los consultorios externos. Se evaluaron las asociaciones entre el espesor de la GSA mediante ecografía y la esteatosis hepática, alanina aminotransferasa (ALT), aspartato aminotransferasa (AST) y el perfil lipídico sérico.Resultados. Se identificaron 95 niños con esteatosis hepática; el grado 1 fue el más frecuente (73,6 %, n: 70), seguido del grado 2 (21,1 %) y del 3 (5,3 %). El espesor medio (en mm) de la GSA de línea media y flancos fue 38,48 ± 11,53 y 20,91 ± 8,00 en grado 1; 41,23 ± 10,03 y 19,84 ± 6,75 en grado 2, y 63,12 ± 12,08 y 23,22 ± 5,92 en grado 3, respectivamente. El espesor de la GSA de línea media y flancos se correlacionó positivamente con triglicéridos en grado 3; el espesor de la GSA de línea media se correlacionó con el índice de masa corporal, colesterol total, colesterol LDL y AST (r: 0,9; p: 0,037; r: 0,648; p: 0,001; r: 0,387; p: 0,001; r: 0,406; p: 0,001; r: 0,463; p: 0,001; respectivamente) en grado 1.Conclusión. El espesor de la GSA mediante ecografía puede predecir hiperlipidemia en niños obesos con esteatosis hepática de grado 3 e hipercolesterolemia en grado 1.
Introduction. Childhood obesity can cause hyperlipidemia and hepatic steatosis at early age and chronic disease complications in adult life. We aimed to evaluate the relationship between abdominal subcutaneous fat thickness (ASFT) and hepatic steatosis, serum lipid and transaminase levels in obese children.Population and methods. A retrospective study was conducted with children (aged 4-18 years) who presented to outpatient clinic due to obesity. Associations between ASFT as assessed by ultrasonography (US) and hepatic steatosis, alanine transaminase (ALT), aspartate transaminase (AST) and serum lipid profile were evaluated.Results. We identified 95 children, all of which were diagnosed as having hepatic steatosis; the most common type was grade 1 (73.6 %, n: 70), followed by grade 2 (21.1 %) and grade 3 (5.3 %). The mean values of midline and flank ASFT were measured as 38.48 ± 11.53 mm and 20.91 ± 8.00 mm in grade 1; 41.23 ± 10.03 and 19.84 ± 6.75 in grade 2 and 63.12 ± 12.08 and 23.22 ± 5.92 in grade 3 hepatic steatosis, respectively. Midline and flank ASFT correlated positively with triglycerides in grade 3 steatosis, while midline ASFT correlated with body mass index, total cholesterol, low-density lipoprotein cholesterol, and AST (r: 0.9; p: 0.037; r: 0.648, p: 0.001; r: 0.387, p: 0.001; r: 0.406, p: 0.001, r: 0.463, p: 0.001; respectively) in grade 1 steatosis.Conclusion. ASFT as assessed by US may be of predictive value for hyperlipidemia in grade 3 and for hypercholesterolemia in grade 1 hepatic steatosis in obese children.
