ABSTRACT
Introducción. Los factores psicológicos preoperatorios pueden influir en los resultados de la rehabilitación de la artroplastia de cadera por artrosis. No existe consenso de cuáles serían estos factores, ni sobre en qué medidas de resultados influirían. Objetivo. Sintetizar la evidencia respecto a los factores psicológicos que pronostican los resultados de la artroplastía de cadera en términos de función física y dolor en individuos con artrosis de cadera. Métodos. Se efectuó una búsqueda en PUBMED, EMBASE, SCOPUS y CINAHL. Los términos incluyeron artroplastia de cadera, artrosis, factores psicológicos, función física y dolor. Se incorporaron estudios observacionales prospectivos. Los resultados se agruparon por factor psicológico en periodos menor a un año e igual o mayor a un año. Resultados. La búsqueda arrojó 895 artículos, dos cumplieron los criterios de elegibilidad. Los resultados descritos fueron: 1) un mayor nivel de expectativas preoperatorias genera un efecto positivo en la disminución de la intensidad de dolor y 2) tiene un efecto positivo en la función física, ambas a menos de un año, y 3) la depresión preoperatoria genera un efecto negativo en la disminución de intensidad de dolor a un año o más. Conclusiones. La presencia de depresión preoperatoria se asoció a una menor reducción de dolor, y las altas expectativas preoperatorias se asociaron con una mejor función física y una menor intensidad de dolor. Sin embargo, se hace necesaria la generación de nueva evidencia científica que profundice en la asociación factores psicológicos preoperatorios y dolor crónico en esta población.
Background. Preoperative psychological factors may influence the results of hip arthroplasty rehabilitation for osteoarthritis. There is no consensus on what these factors would be, nor on what outcome measures they would influence. Objetive. To review was to synthesize the evidence regarding psychological factors that predict hip arthroplasty outcomes in terms of physical function and pain in individuals with hip osteoarthritis. Methods. A search was carried out in PUBMED, EMBASE, SCOPUS and CINAHL. Terms included hip arthroplasty, osteoarthritis, psychological factors, physical function, and pain. Prospective observational studies were incorporated. The results were grouped by psychological factor in periods of less than one year and equal to or greater than one year. Results. The search yielded 895 articles, two met the eligibility criteria. The results described were: 1) a higher level of preoperative expectations generates a positive effect in the reduction of pain intensity and 2) has a positive effect on physical function, both at less than one year, and 3) preoperative depression generates a negative effect in the reduction of pain intensity at one year or more. Conclusions. The presence of preoperative depression was associated with less pain reduction, and high preoperative expectations were associated with better physical function and less pain intensity. However, it is necessary to generate new scientific evidence that deepens the association of preoperative psychological factors and chronic pain in this population.

Introdução: À medida que a sociedade envelhece, as incidências de doenças do quadril aumentam a cada ano e a artroplastia do quadril é reconhecida como método cirúrgico mais eficaz, necessitando de intervenção fisioterapêutica no período pós-operatório. Objetivo: Identificar estudos que demonstrem os efeitos da reabilitação através de exercícios terapêuticos no pós-operatório de artroplastia de quadril. Métodos: Realizou-se uma revisão sistemática na qual levantou-se uma busca nas bases de dados: Pubmed, Lilacs, Scielo, Cochrane, Web of Science, Scopus, Science Direct e Google Scholar, nos últimos 10 anos e utilizando os descritores respectivos para estudos em inglês, português e espanhol. Resultados: Foram selecionados 3 ensaios clínicos randomizados, somando um total de 225 indivíduos participantes das pesquisas, de ambos os sexos, para realização de artroplastia de quadril, apresentando diversas comparações nas aplicações de exercícios terapêuticos no pós-operatório. Conclusão: Os resultados apresentados nesta revisão demonstram os efeitos positivos da fisioterapia iniciada precocemente após a cirurgia de artroplastia de quadril, sendo perceptível que a reabilitação fisioterapêutica imediata através de exercícios iniciados ainda no ambiente hospitalar pode ser benéfica para melhora da mobilidade, funcionalidade e qualidade de vida dos pacientes. Introduction: As society ages, the incidences of hip disease increase each year and hip arthroplasty is recognized as the most effective surgical method, requiring physical therapy intervention in the postoperative period. Objective: To identify studies that demonstrate the effects of rehabilitation through therapeutic exercises in the postoperative period of hip arthroplasty. Methods: A systematic review was carried out using the following database searches: Pubmed, Lilacs, Scielo, Cochrane, Web of Science, Scopus, Science Direct and Google Scholar, in the last 10 years and using the evaluated descriptors for studies in English, Portuguese and Spanish. Results: Three randomized clinical trials were selected, with a total of 225 research participants, of both sexes, for performing hip arthroplasty, presenting several comparisons and applications of therapeutic exercises in the postoperative period. Conclusion: The results demonstrate the positive effects of physical therapy right after hip arthroplasty surgery. It is noticeable that beginning physiotherapeutic rehabilitation with exercises in the hospital environment can be beneficial for improving mobility, performance and quality of life of patients. (AU)

@#Introduction: The morphology of the proximal femur differs in various populations. Based on our clinical experience, conventional femoral stems used in hip arthroplasty do not fit the Chinese population well. Hence, this study aims to evaluate the suitability of conventional femoral stems in the elderly Chinese hip fracture population requiring hip arthroplasty and to establish if gender and age related differences exist within this population. Materials and methods: We retrospectively analysed radiographic data of 300 patients from a tertiary hospital’s geriatric hip fracture database who underwent either hip hemi-arthroplasties or total hip arthroplasties. Proximal femur morphological measurements were recorded, analysed and compared to that of commonly used femoral stems. Subgroup analysis was performed to compare age and gender related differences. Results: A total of 18.3% of the study population had a medial femoral offset (MFO) of less than 30mm, which is the smallest available offset for the implants studied. 22.6% of female and 3% of male subjects had MFOs that were less than 30mm. In our subgroup analysis, males had significantly larger femoral head diameters, MFO and vertical femoral offsets compared to females. Older subjects (75-90 years old) had significantly smaller femoral head diameters, vertical femoral offsets and neck shaft angles compared to younger subjects (60-75 years old). Conclusion: Commonly used femoral stem implants have measurements that do not suit our Chinese population with small medial femoral offsets. In addition, elderly males have significantly larger femoral head diameters, medial and vertical femoral offsets whereas older subjects have significantly smaller femoral head diameters, vertical femoral offsets and neck shaft angles.
ABSTRACT
This study investigated removal of a force-closed stem, done in order to improve acetabular exposure during revision, with reinsertion afterwards. It is unknown how much this procedure modifies the stem/cement interface. METHODS: Three tapered stem models were implanted into composite femurs. Strain gauges were embedded in the medial aspect of the cement mantle and in several positions on the outer surface of the femurs. The deformation was measured during static loading, which was applied at two different times: after implantation and after one million loading cycles, followed by stem removal and reinsertion. The t test was performed. The differences in deformation were compared (at p ≤ 0.05) between the two static loading times and among the three stem designs. RESULTS: No significant differences in deformation were found after the two loading times for the three models. No significant differences in the initial deformations of the three models were found for most of the gauges attached to the femurs. CONCLUSIONS: Reinsertion of the force-closed stem does not alter the load transmission from the stem to the cement and to the surface of the femur, even after one million loading cycles.
Estudo da remoção de haste do tipo force-closed e a sua reinserção posterior para aumentar a exposição do acetábulo durante a revisão. Não é conhecido o quanto esse procedimento modifica a interface haste/cimento. MÉTODOS: Três modelos de hastes afiladas foram implantadas em fêmures compósitos. Extensômetros de deformação foram embebidos no aspecto medial do manto de cimento e em diversas posições sobre a superfície externa dos fêmures. As deformações foram medidas durante cargas estáticas, as quais foram aplicadas em dois diferentes momentos: após a implantação e após um milhão de ciclos de carga, seguido pela remoção e reinserção. O teste t foi feito. As diferenças entre as deformações foram confrontadas com p ≤ 0,05 entre os dois momentos de carga estática e entre os três projetos de hastes. RESULTADOS: Não foram encontradas diferenças significativas nas deformações após os dois momentos de carga para os três modelos. Não foram encontradas diferenças significativas nas deformações iniciais dos três modelos para a maioria dos extensômetros aderidos aos fêmures. CONCLUSÕES: A reinserção de haste do tipo force-closed não altera a transmissão de carga da haste para o cimento e para a superfície do fêmur, mesmo após um milhão de ciclos.

Objective To analyze the influence factors of lower extremities deep venous thrombosis after hip replacement,and provide guidance for perioperative prevention.Methods 52 cases of patients underwent total hip replacement were selected,who were given color doppler ultrasound examination for lower limb venous thrombosis after hip replacement.And analyzed risk factors of lower extremity deep vein thrombosis from patient's gender,age,blood lipid,D -dimer,complications and surgical index.Results During the 52 cases,16 cases were found with postopera-tive lower limb deep vein thrombosis (30.8%);Single factor analysis showed that patients with age ≥65 years,over-weight or obese (BMI≥25kg/m2 ),high triglycerides (TG ≥ 1.7mmol/L)and high D -dimer level (≥500μg/L), diabetes or high blood pressure,large blood transfusion combined with a greater incidence of postoperative deep vein thrombosis (P <0.05).Further multiple factors regression analysis showed that age ≥65 years,overweight or obesity, high triglycerides,high D -dimer,diabetes,large blood transfusions were independent risk factors of lower extremity deep vein thrombosis (OR =15.693,1.278,9.832,3.053,1.855,1.014,all P <0.05).Conclusion Patients with total hip replacement should be given preoperative vein thrombosis risk assessment,patients with age ≥65,overweight or obese,high triglycerides,D -dimer level,diabetes,large number of blood transfusions have higher risk of deep vein thrombosis,we should focus on perioperative monitoring,application of low molecular heparin,lower limb vein pump, etc.early postoperative lower limb rehabilitation exercise,to reduce the occurrence of deep vein thrombosis.

Objective To investigate the efficacy of continuous fascia iliaca compartment block withdifferent concentrations of ropivacaine for postoperative analgesia in patients undergoing total hip arthroplasty.Methods One hundred and sixty ASA physical status Ⅰ or Ⅲ patients,aged 52-84 yr,body mass index 18-26 kg/m2,scheduled for total hip arthroplasty,were randomly divided into 4 groups (n =40 each) using a random number table:0.20% ropivacaine group (group A),0.25% ropivacaine group (group B),0.30% ropivacaine group (group C) and 0.35% ropivacaine group (group D).A catheter was inserted into the fascia iliaca compartment on the affected side within 30 min after operation.The corresponding concentrations of ropivacaine 20 ml were given in each group.The catheter was then connected to a patient-controlled analgesia pump programmed to deliver 10 ml with a lockout interval of 60 rin for postoperative analgesia (72 h).When VAS score at rest≥4,parecoxib sodium 40 mg was injected intravenously.At 12,24,48 and 72 h of blockade,the passive and active exercise VAS scores were recorded.The consumption of ropivacaine within 72 h after the end of blockade,and requirement for parecoxib sodium and development of adverse reactions during blockade were recorded.Results Compared with group A,the passive and active exercise VAS scores were significantly decreased in C and D groups (P < 0.05),and no significant change was found in group B (P > 0.05),and the consumption of ropivacaine within 72 h after the end of blockade was significantly decreased in B,C and D groups (P < 0.05).There was no significant difference in the passive and active exercise VAS scores between group C and group D (P > 0.05).There was no significant difference in consumption of ropivacaine within 72 h after the end of blockade between B,C and D groups (P > 0.05).There were no significant differences in the requirement for parecoxib sodium and incidence of vomiting among the four groups (P > 0.05).Conclusion The optimum concentration of ropivacaine is 0.30% when used for continuous fascia iliaca compartment block in patients undergoing total hip arthroplasty.

BACKGROUND:Ceramic-on-ceramic total hip arthroplasty can be applied to young patients requiring frequent activity and high quality of life. OBJECTIVE:To explore the short-term efficiency of ceramic-on-ceramic total hip arthroplasty in young and middle-aged patients of femoral neck fractures. METHODS:Fifty-one young and middle-aged patients under 50 years old with ceramic-on-ceramic total hip arthroplasty were selected to analyze the function of hip joints and the complication after operation. RESULTS AND CONCLUSION:Al the patients were fol owed up for 6 months to 44 months. At the last fol ow-up, the average Harris score was 92.0±3.4 points with an excellent and good rate of 94.1%. No dislocation was observed and three cases exhibited mild limping. Imaging examination showed that, no prosthesis loosing, broken, sinking or radiolucent line around prosthesis was observed. Ceramic-on-ceramic total hip arthroplasty in young patients with femoral neck fractures can effectively decrease the osteolysis, prosthesis loosing and sinking caused by the debris of wearing, prolong the working life of the prosthesis, and improve the patient’s quality of life.

OBJETIVO: O estudo apresenta uma via de acesso anterolateral minimamente invasiva e verifica se esta via permite realizar a artroplastia total de quadril sem comprometer a qualidade do posicionamento dos implantes, mantendo-se a integridade da musculatura glútea. MÉTODO: Realizou-se um estudo retrospectivo de 260 pacientes, 18 bilateral perfazendo um total de 278 quadris, sendo 186 do sexo feminino e 74 masculino, com idade média de 62 anos, portadores de osteoartrose, submetidos à artroplastia total de quadril não cimentada, metal-metal ou metal-polietileno, no período de outubro de 2004 a dezembro de 2007. Utilizou-se uma via de acesso anterolateral minimamente invasiva, medindo aproximadamente 7 a 10cm variando de acordo com a massa corpórea e o tamanho da cabeça femoral. Os pacientes foram avaliados clinicamente quanto à idade, sexo, presença do sinal de Trendelenburg e, radiograficamente, quanto ao posicionamento acetabular e femoral. RESULTADOS: Verificou-se uma inclinação acetabular entre 30° e 40° em 78 pacientes, entre 41° e 50° em 189, e 11 casos com 51° ou mais. Quanto ao posicionamento femoral na incidência anteroposterior obteve-se posição central em 209 casos, 41 em valgo e 28 em varo. No perfil, observaram-se 173 centrais, 67 anteriores e 38 posteriores. O tempo cirúrgico foi, em média, de 63 minutos. Como complicações, houve cinco casos de infecção, três casos de trombose venosa profunda, dois casos de luxação do quadril, 80 casos de alongamento de membros inferiores e cinco casos de encurtamento do membro operado. Verificou-se a presença de Trendelenburg em quatro casos, um dos casos com lesão do nervo glúteo superior. CONCLUSÃO: A via de acesso anterolateral minimamente invasiva permite realizar a artroplastia total de quadril sem comprometer o posicionamento dos implantes, mantendo a integridade da musculatura glútea.
OBJECTIVE: The aim of this study was to present a minimally invasive anterolateral access route and to ascertain whether this enables total hip replacement without compromising the quality of the implant positioning, while maintaining the integrity of the gluteus muscles. METHOD: A retrospective study was conducted on 260 patients (186 females and 74 males) of average age 62 years. There were 18 bilateral cases, thus totaling 278 hips. All the patients had osteoarthritis and had undergone non-cemented total hip arthroplasty (metal-metal or metal-polyethylene) between October 2004 and December 2007. A minimally invasive anterolateral access route was used, measuring 7 to 10 cm in length, according to body weight and the size of the femoral head. The patients were assessed clinically regarding age, sex and presence of the Trendelenburg sign, and radiologically regarding acetabular and femoral positioning. RESULTS: The acetabular inclination was between 30° and 40° in 78 patients, between 41° and 50° in 189 patients, and 51° or over in 11 patients. On anteroposterior radiographs to study femoral positioning, the positioning was central in 209 cases, 41 presented valgus deviation and 28 presented varus deviation. On lateral views, 173 were central, 67 anterior and 38 posterior. The mean duration of the procedure was 63 minutes. Regarding complications, there were five cases of infection, three of deep vein thrombosis, two of hip dislocation, 80 of lengthening of the lower limbs and five of shortening of the operated limb. The Trendelenburg sign was present in four cases, of which one showed superior gluteal nerve injury. CONCLUSION: The minimally invasive anterolateral access route made it possible to perform total hip arthroplasty without compromising the positioning of the implants, thereby maintaining the integrity of the gluteus muscles.

Objective To investigate the efficacy of ultrasound-guided fascia iliaca compartment block (FICB) on postoperative analgesia in patients undergoing total hip arthroplasty.Methods Thirty-six ASA Ⅰ-Ⅲ patients aged 54-82 yr weighing 48-72 kg undergoing total hip arthroplasty were randomly divided into 2 groups (n =18 each): normal saline group(group NS)and ropivacaine group (group R).Ultrasound-guided FICB was performed within 30 min after operation.Group R received 0.25 % ropivacaine 30 ml,while the equal volume of normal saline was used instead of ropivacaine in group NS.All patients received PCIA with 0.01 mg/ml fentanyl after FICB.PCIA included a bolus dose of 2 ml with a 15 min lockout interval and no background infusion.Pain at rest was evaluated using VAS (RVAS) score before FICB (T0 )and at 3 h(T1 ),6 h(T2 ),8 h(T3 ),12 h(T4 ),24 h(T5 ),48 h(T6 )and 72 h(T7 )after FICB.The passive exercise VAS (PVAS) score at T4-6,T7 and active exercise VAS (AVAS) score at T5-7 were recorded.The consumption of fentanyl at 0-12 h,12-24 h、24-48 h and 48-72 h after FICB and the adverse effects were also recorded.Results Compared with group NS,RVAS score,PVAS score,AVAS score and the consumption of fentanyl were significantly decreased in group R.There was no significant difference in the adverse effects between the two groups.Conclusion Ultrasound-guided FICB can provide better postoperative analgesia with little adverse effects in patients undergoing total hip arthroplasty.

Objective To understand the functional exercise situation of the total hip arthroplasty(THA)patients in hospital. Methods Using self-designed functional exercise in patients with TKA outline the structure of observation, observation and collection of Orthopedic Surgery,71 cases (91 hips)received THA in patients with relevant information. Results After the former 3d,22.5% of patients did not exercise;After 14 d continuous passive motion (CPM) ,hip joint exercises were separately accounted for 71.8% and 81.7% ,and static muscle contraction,straight leg raising were accounted for 40.8 percent and 81.7 percent. The four exercise started lately, training methods were not standardized the patients, insisted on a shorter time;single-and double-hip replacement hip replacement in patients with Shimoji routing practice started time respectively, after (6.51 ± 2.90 ) d and ( 10.30 ± 3.21 ) d. In the functional exercise process,CPM was assisted by nurse,in other exercises was assisted by retaining workers and patients families. Conclusion THA patients hospitalized during the actual exercise of the number,frequency,timing and methods vary, nursing staff should be further explored how to ensure that patients really effective rehabilitation exercise to further enhance the effect of patients with rehabilitation exercises.

Objective To investigate the anatomic structure of the Chinese people,develop the procedure of minimally invasive total hip arthroplasty through the anterolateral intermuscular approach and investigate its clinical outcome.Methods Three fresh adult cadavers(6 hips)were used for study of the anatomic construction of the anterolateral intermuscular approach in Chinese people.Sixteen patients were treated with minimally invasive total hip arthroplasty through anterolateral intermuscular approach.The clinical results and operation technique were recorded.Results The anterolateral intermuscular approach was a triangle muscular interval slightly parallel to the femur.The medial-superior angle of the triangle muscular interval consists of the anterior border of gluteus medius and tensor fascia lata muscle with juncture of muscles,where the inferior branch of superior gluteal nerve entered into tensor fascia lata muscle.The average incision length was 8.8 cm(7-10 cm),with mean blood loss of 350 ml(250-550 ml).The patients took out-of-bed activity 3-5 days after operations.During operations,anterior border injury in deep portion of the gluteus medius muscle was observed in seven patients and the injured muscles were trimed or repaired.All patients were followed up for 18-39 months(averaged 27.7 months).Most of the patients had excellent location of the phantoms,except that one acetabulum had a little pitch angle and two acetabulums had a little abduction angle.No complication was observed.The mean Harris scores of hip for all patients was increased from preoperative(39.1±6.7)points to(80.6±11.3)points on six month,(88.7±9.6)points on 12 month and(91.4±13.5)points on 24 months(11 patients).No patient suffered from gluteus medius muscle weakness during the follow-up.ConclusionAnterolateral intermuscular approach has the advantages of simple anatomic construction,small incision,little operative injury,muscle sparing and fast recovery without separate muscle or tendon and is suitable for the Chinese patients.Exact incision and special operative instruments should be emphasized to avoid the increase of acetabular pitch angle and abduction angle.

Objective:To compare the clinical effect of large-diameter metal-on-metal(L-MoM)total hip arthroplasty(THA)versus conventional metal on poly ethylene(MoPE)THA following posterolateral minimal incision.Methods:22 L-MoM THA in 20 patients were performed with posterolateral minimal incision.A matched group of 22 hips in 20 patients who had undergone a MoPE THA with posterolateral minimal incision were also performed by the same surgeon.The two groups of patients were evaluated clinically and radiographically respectively,including the length of the incision,surgical time,intra-operative blood loss,postoperative drainage volume,postoperative amount of decreasing hemoglobin(Hb),postoperative pain score,C-reactive rotein(CRP),erythrocyte sedimentation rate(ESR),the circumference difference of the operated thighs,length of stay,major complications and component positions.Results:There were no differences between the two groups with regard to the length of the skin incision,operating time,blood loss,postoperative amount of decreasing Hb,the visual analog scale(VAS)pain score at 24 hours after surgery,CRP,ESR,the circumference difference of the operated thighs,length of stay,major complications and component positions.Conclusion:L-MoM THA with posterolateral minimal incision can be effectively performed without increased risk of complications.The key is to master the surgical skills.
