ABSTRACT

Objetivo: analisar a produção científica brasileira, na Pós-Graduação em Enfermagem, que utilizou o método de adaptação transcultural. Método: estudo documental, com busca realizada na Biblioteca Digital de Teses e Dissertações, que resultou em 140 dissertações e 72 teses para análise, oriundas de Programas de Pós-Graduação da região Sudeste, seguida das regiões Nordeste, Sul e Centro-Oeste, sem representação da região Norte. Resultados: os instrumentos adaptados foram, em sua maioria, procedentes do idioma inglês. Prevaleceram as pesquisas na área/campo Assistencial, destacando-se a linha de pesquisa Processo de Cuidar em Saúde e Enfermagem. Identificou-se descompasso entre o que é produzido na área e o que é recomendado internacionalmente. Conclusão: verificou-se aumento na utilização da adaptação transcultural como método de pesquisa, com persistência das assimetrias acadêmicas regionais e sem consenso sobre o referencial metodológico.


Objective: to analyze the Brazilian scientific production in Postgraduate Nursing education using the cross-cultural adaptation method. Method: documentary study with searches carried out in the Digital Library of Theses and Dissertations resulting in 140 Master's theses and 72 Doctoral dissertations for analysis originated from Postgraduate Programs carried out in the Southeast region of Brazil, followed by the Northeast, South and Midwest regions ­ there was no representation of the North region. Results: the adapted instruments were, mostly, originally written in English. Research in the Care area/field prevailed, highlighting the line of research called Health and Nursing Care Process. A gap between what is produced in the area and what is recommended internationally was identified. Conclusion: an increase in the use of cross-cultural adaptation as a research method was noticed, with the persistence of regional academic asymmetries and lack of consensus on the methodological framework.

Método: estudio documental, la búsqueda se realizó en la Biblioteca Digital de Tesis y Disertaciones, se obtuvieron 140 tesis de maestría y 72 tesis de doctorado para análisis, provenientes de Programas de Posgrado de la región Sudeste, seguida de las regiones Nordeste, Sur y Centro-Oeste, no se encontraron documentos de la región Norte. Resultados: los instrumentos adaptados fueron, en su mayoría, del idioma inglés. Predominaron las investigaciones en el área/campo Asistencial, se destacó la línea de investigación Proceso de Atención en Salud y Enfermería. Se identificó que lo que se produce en el área no coincide con lo que se recomienda a nivel internacional. Conclusión: se comprobó que aumentó el uso de la adaptación transcultural como método de investigación, que persisten las disparidades académicas regionales y que no hay consenso sobre el marco metodológico.

ABSTRACT

Background- The sellar and juxtasellar regions are complex anatomical areas prone to various neoplastic and non- neoplastic lesions. Accurate diagnosis and appropriate therapeutic interventions require comprehensive imaging assessments. Magnetic Resonance Imaging (MRI) is a critical tool due to its superior soft-tissue contrast and high spatial resolution. This study was conducted at the Department of Radio Diagnosis, Sri Aurobindo Medical College &Methods- Post Graduate Institute, Indore. It involved 45 patients with clinically suspected sellar and parasellar masses. MRI scans were performed using a Siemens 1.5 T MAGNETOM� Symphony� with Tim technology. The imaging protocol included routine MR pulse sequences and gadolinium-enhanced images. Data were analyzed using descriptive statistics, with MRI findings compared against histopathological results. The study found that pituitary adenomas were theResults- most prevalent lesions, accounting for 66.6% of cases, followed by craniopharyngiomas (16.6%) and meningiomas (13.3%). MRI demonstrated high sensitivity and specificity in diagnosing these conditions, with substantial agreement with histopathology results. MRI is a reliable diagnostic tool for identifying sellar and parasellar masses,Conclusion- offering distinct imaging characteristics that aid in differentiating between various lesion types. Its ability to provide detailed anatomical visualization makes it indispensable for planning appropriate treatment strategies.

ABSTRACT

Objective: To describe the cross-cultural adaptation of the "Questionnaire about Care for People with Duchenne Muscular Dystrophy" to Brazilian Portuguese and our findings about the DMD care access in a group of 43 patients assisted in an University Hospital in Rio de Janeiro. Method: Guidelines for best practice in translation and cultural adaptation were followed. Validation of the adapted questionnaire was verified in a group of 43 patients with Duchenne Muscular Dystrophy (DMD) assisted in the Child Neurology Department of a university hospital in Rio de Janeiro. Results: Once the questionnaire was cross-adapted into Brazilian Portuguese, information was obtained on disease progression, access to recommended care and difficulties encountered in maintaining treatment. This allowed comparison with previously published national and international data. Conclusion: The adapted version of the CARE-NMD questionnaire for Brazilian Portuguese has the potential to support the work done in neuromuscular centers and health managers in Brazil. It provides a picture of access to care in our country, and allows exchanging updated information between Brazilian and international neuromuscular centers.


Objetivo: Descrever a adaptação transcultural do "Questionnaire about Care for People with Duchenne Muscular Dystrophy" para o Português brasileiro e os achados sobre o acesso aos cuidados preconizados ao paciente com DMD em um grupo de 43 pacientes assistidos em um hospital universitário no Rio de Janeiro. Método: Recomendações de boas práticas em tradução e adaptação transcultural foram seguidas. Validação do questionário adaptado foi alcançada com a aplicação do mesmo em um grupo de 43 pacientes com Distrofia Muscular de Duchenne (DMD), assistidos no Departamento de Neuropediatria de um hospital universitário no Rio de Janeiro. Resultados: Uma vez que o questionário foi adaptado transculturalmente para o português brasileiro, informações sobre a progressão da doença, acesso aos cuidados recomendados e dificuldades encontradas na manutenção do tratamento foram obtidas. Esses achados permitiram a comparação com informações previamente publicadas em bancos de dados nacionais e internacionais. Conclusão: A versão adaptada do questionário, originalmente publicado em inglês pela CARE-NMD, para o português brasileiro tem o potencial de apoiar o trabalho realizado nos Centros de Doenças Neuromusculares e à Gestão Pública no nosso país. As informações obtidas com o questionário apresentado desenham uma imagem sobre o acesso aos cuidados com a DMD, permitindo a troca de informações atualizadas entre os serviços de saúde brasileiros e internacionais.

ABSTRACT

AIM: The aim of this study was to determine urban and rural differences in the burden of polycystic ovarian syndrome among Indian adolescent females aged 12 to 19 years. The present study was conducted on 100 participants at BKL Walawalkar Rural Hospital, Ratnagiri,Methods: Maharashtra. Patients were from various urban (50%) and rural (50%) settings. All the demographic and clinical parameters were compared. Results: 22% participants were con?rmed of having PCOS. Mean age was 18.6�1 years. Average waist-hip ratio of rural participants was higher than urban participants (p value<0.05). 56% urban participants reported recent weight gain in past 3months (p value<0.05). The urban study participants had higher frequency of excess androgen activity (61%, p value<0.05). The proportion of PCOS, weight gain, excessConclusion: androgen activity was higher among urban participants in comparison to rural participants.

ABSTRACT

Los salmónidos, durante su desarrollo, pueden experimentar ocasionalmente deformaciones esqueléticas. Para su diagnóstico se emplean diversas metodologías, entre las que se incluyen radiografías, técnicas histológicas, diafanización con tinciones de alizarina y azul de alcián, así como el uso del microscopio electrónico de barrido (SEM), cada una con sus inherentes ventajas y desventajas. Este estudio tuvo como finalidad evaluar y comparar la eficacia de la tomografía microcomputarizada (Micro-CT) para el análisis anatómico, reconstruyendo tridimensionalmente las imágenes y contrastándolas con los resultados obtenidos mediante la técnica de diafanización. Se analizaron las aletas caudales de cinco ejemplares de salmón Oncorhynchus kisutch: dos sujetos a diafanización y tres procesados para análisis mediante Micro-CT utilizando el equipo BRUKER SkyScan 1272. La técnica de Micro-CT demostró superioridad en la resolución de las estructuras óseas, facilitando una exploración detallada de las variaciones morfológicas y la distribución de la densidad mineral. Este enfoque permitió identificar anomalías en la morfología y crecimiento de las últimas vértebras y lepidotriquias dorsales, así como una densidad incrementada en lepidotriquias dorsales malformadas. La mayor resolución proporcionada por la Micro-CT no solo potencia nuestra comprensión de la ontogenia piscícola y su adaptación a ambientes diversos, sino que además inaugura perspectivas innovadoras para el estudio de la evolución de las estrategias locomotoras y las respuestas adaptativas frente a cambios ambientales a través del tiempo.


SUMMARY: During their development, some species of salmonids may occasionally experience skeletal deformations. Several methodologies are currently being used for the diagnosis of such malformations, among which X-rays, histological techniques, diaphanization coupled either with Alizarin Red or Alcian Blue stains, as well as Scanning Electron Microscopy (SEM) can be mentioned. Each one of those methods presents inherent advantages and disadvantages. The purpose of this study was twofold: Firstly, to evaluate and compare the effectiveness of microcomputed tomography (Micro-CT) technology for anatomical analysis, three-dimensionally reconstructing the obtained images; and secondly, to contrast those images with the results obtained through the diaphanization technique. The caudal fins of five specimens of the Oncorhynchus kisutch salmon were analyzed: Two specimens were subjected to diaphanization and three were processed for Micro-CT analysis, using the BRUKER SkyScan 1272 equipment. The Micro-CT technology demonstrated superiority in the resolution of bone structures, facilitating a detailed exploration of morphological variations, as well as the distribution of mineral density. This experimental approach allowed us to identify anomalies in the morphology and growth of the last vertebrae and dorsal lepidotrichiae, as well as an increased mineral density in the malformed dorsal lepidotrichiae. The higher resolution provided by Micro-CT not only enhances our understanding of the fish ontogeny and its adaptation to diverse environments, but also opens innovative perspectives for the study of the evolution of locomotor strategies and adaptive responses to environmental changes.

ABSTRACT

Background: Endoscopic retrograde cholangiopancreatography (ERCP) is now widely accepted as a therapeutic modality for benign and malignant diseases of the pancreaticobiliary tree. Acute pancreatitis is the most common and feared complication of ERCP, associated with substantial morbidity and mortality. This study aimed to compare the incidence of post-ERCP pancreatitis in combined rectal indomethacin and sublingual nitroglycerin with that of rectal indomethacin alone. Methods: This was a randomized controlled trial conducted in the Department of Gastroenterology, Bangabandhu Sheikh Mujib Medical University, Dhaka, Bangladesh during the period from July, 2019 to September, 2020. In this study, 100 hospitalized patients of >18 years age undergoing ERCP based on clinical indication and proper investigation were included. The study population was randomly divided into two groups - group A (patients who were given indomethacin suppository plus sublingual glyceryl trinitrate) and group B (patients who were given indomethacin alone). Results: In this study, we found that PEP developed in 11 out of 100 patients (11%). Post-ERCP pancreatitis developed in 2 (4%) in group A and 9 (18%) in group B (p=0.025), which was statistically significant. Mild PEP developed in none in group A and 4 (8%) in group B whereas moderate in 2 (4%) in group A and 3 (6%) in group B. Severe pancreatitis occurred in none in group A and 2 (4%) in group B (p=0.231) which was not statistically significant. Conclusions: This study showed that the combination of indomethacin suppository and sublingual GTN is superior to indomethacin suppository alone in preventing post-ERCP pancreatitis.

Introdução: O objeto do estudo foi a Career Anchors Self-Assessment no contexto de enfermeiros brasileiros sob a ótica da saúde do trabalhador. Objetivos: Adaptar transculturalmente a Career Anchors Self-Assessment para o contexto brasileiro; Analisar evidências de validade de conteúdo, processo de resposta e estrutura interna da versão brasileira da Career Anchors Self-Assessment em uma amostra de enfermeiros; Discutir a validação da Career Anchors Self-Assessment para a realidade brasileira e a utilização da escala para reflexão sobre a saúde do trabalhador enfermeiro. Método: Estudo psicométrico, dividiu-se em duas etapas: 1 - Obtenção da versão brasileira da escala Career Anchors Self-assessment Adaptação Transcultural/validade de conteúdo e de processo de resposta. 2 - Evidências de validade relacionadas à estrutura interna da versão brasileira da Career Anchors Self-assessment. Resultados: Obteve-se a versão adaptada para o contexto de enfermeiros brasileiros da área hospitalar, do instrumento denominado de Autoavaliação de Âncoras de Carreira, composto 42 itens. Quanto à avaliação das evidências de validade da estrutura interna, foram encontrados dados satisfatórios para a AFC, AFE e Confiabilidade. Uma análise reflexiva pode ser feita a partir dos resultados, o modelo teórico das âncoras de carreira, originalmente criado com o perfil de administradores indicou para o autor, oito tipos de âncoras de carreira. Porém, ao aplicar para a enfermagem, teve-se uma evidência maior nas âncoras: Segurança e Estabilidade (SEG), Senso de serviço/ Dedicação a uma causa (SER), Autonomia e Independência (AUT)/Criatividade Empreendedora (CRI) e Competência Gerencial (GER). Considerações Finais: A versão brasileira, denominada Autoavaliação das Âncoras de Carreira, tem boa qualidade de tradução e evidências de validade de conteúdo e foram encontrados valores satisfatórios para o instrumento com 08 fatores latentes, confirmando a hipótese.


Introduction: The object of the study was the Career Anchors Self-Assessment in the context of Brazilian nurses from the perspective of occupational health. Objectives: To adapt the Career Anchors Self-Assessment cross-culturally to the Brazilian context; To analyze evidence of content validity, response process and internal structure of the Brazilian version of the Career Anchors Self-Assessment in a sample of nurses; To discuss the validation of the Career Anchors Self-Assessment for the Brazilian reality and the use of the scale for reflection on the health of the nursing worker. Method: Psychometric study, divided into two stages: 1 - Obtaining the Brazilian version of the Career Anchors Self-Assessment scale Cross-cultural adaptation/content validity and response process. 2 - Evidence of validity related to the internal structure of the Brazilian version of the Career Anchors Self-Assessment. Results: The adapted version for the context of Brazilian nurses in the hospital area was obtained, of the instrument called Self-Assessment of Career Anchors, composed of 42 items. Regarding the evaluation of the evidence of validity of the internal structure, satisfactory data were found for the AFC, AFE and Reliability. A reflective analysis can be made from the results, the theoretical model of career anchors, originally created with the profile of administrators indicated for the author, eight types of career anchors. However, when applied to nursing, there was greater evidence in the anchors: Security and Stability (SEG), Sense of Service/Dedication to a Cause (SER), Autonomy and Independence (AUT)/Entrepreneurial Creativity (CRI) and Management Competence (GER). Final Considerations: The Brazilian version, called Self-Assessment of Career Anchors, has good translation quality and evidence of content validity and satisfactory values were found for the instrument with 08 latent factors, confirming the hypothesis.

ABSTRACT

Objetivo: Este estudio tuvo como objetivo principal validar el Voice Handicap Index (VHI) y su versión abreviada (VHI-10) adaptados al español rioplatense de Argentina, con objetivos específicos centrados en evaluar su fiabilidad y validez. Metodología: La adaptación cultural incluyó técnicas de traducción directa, síntesis y retrotraducción, evaluación de la equivalencia semántica y aplicación a un grupo piloto. Para la validación se evaluó la fiabilidad de ambos índices adaptados mediante la consistencia interna (coeficiente alfa de Cronbach) y la estabilidad test-retest (prueba de Bland-Altman, CCI y r de Spearman). Además, se examinó la validez de criterio y de constructo. 213 sujetos participaron en la validación del índice adaptado de 30 ítems (123 disfónicos; 90 de control); 113, en la del índice abreviado (63 disfónicos; 50 de control). Resultados: Se constituyó el Índice de Desventaja Vocal (IDV) como la versión adaptada del VHI al español rioplatense de Argentina. Ambos índices demostraron excelente consistencia interna (IDV-30 α = 0,96; IDV-10 α = 0,92) y estabilidad y concordancia (IDV-30 CCI = 0,95; IDV-10 CCI = 0,96). Se halló alta correlación entre los puntajes de ambos índices y la autoevaluación de la severidad de la disfonía de los participantes (r = 0,85). Ambos índices demostraron capacidad de diferenciar entre individuos con disfonía y sujetos sanos (p< 0,001). El análisis factorial reveló tres factores para el IDV-30 y un factor para el IDV-10. Conclusiones: El IDV-30 e IDV-10 presentan grados adecuados de fiabilidad y validez. Ambos pueden ser incluidos en protocolos de valoración de la función vocal por profesionales de Argentina.

Methods: Cultural adaptation involved direct translation, synthesis and back-translation techniques, followed by an assessment of semantic equivalence and application to a pilot group. For the validation process, the reliability of both adapted indices was assessed through measures of internal consistency (Cronbach's alpha coefficient) and test-retest stability (Bland-Altman test, ICC and Spearman's correlation coefficient). Additionally, we conducted analyses to asses criterion and construct validity. 213 subjects participated in the validation of the adapted 30-items index, (123 with dysphonia; 90 from control group); 113, in the abbreviated version (63 with dysphonia; 50 from control group). Results: The "Índice de Desventaja Vocal" (IDV) was established as the adapted version of the VHI into Rioplatense Spanish from Argentina. Both indeces exhibited excellent internal consistency (IDV-30 α = 0,96; IDV-10 α = 0,92) and satisfactory stability and agreement (IDV-30 CCI = 0,95; IDV-10 CCI = 0,96). Regarding validity, a strong correlation was observed between the scores of both indeces and the participant's self-assessment of dysphonia degree (r = 0,85). Both indices effectively differentiated between individuals with dysphonia and healthy subjects (p< 0,001). Factor analysis revealed three factors for the IDV-30 and one factor for the IDV-10. Conclusion: The IDV-30 and IDV-10 demonstrate satisfactory levels of reliability and validity. Both indices can be incorporated into the assessment protocols for evaluating the vocal function by professionals in Argentina.

ABSTRACT

Background: Diabetes Mellitus Type 2 (DMT2), which is characterized by the raised blood sugar level due to insulin resistance in body cells, has now become global epidemic. This study was aimed to compare the efficacy of metformin and repaglinide monotherapy in patients with newly diagnosed DMT2. Methods: This randomized comparative prospective study was performed in a tertiary care hospital of Lahore. 108 Patients’ enrollment was made via simple random sampling technique and a developed inclusion and exclusion criteria. Patients were divided into metformin and repaglinide groups via lottery method. Self-designed proforma was applied for data collection. Data analysis was done in SPSS version 25.0. Results: There was no significant difference in the means of pretreatment (p=0.08) and posttreatment (p=0.10) fasting blood sugar levels between two groups, while significant differences between the means of pretreatment and posttreatment fasting blood sugar levels within metformin group (p=0.02) and repaglinide group (p=0.01) were noted. Likewise, there was no significant difference in the means of pretreatment (p=0.07) HbA1C levels of two groups; however, significant difference was observed in the means of posttreatment (p=0.04) HbA1C of two groups. Moreover, significant differences were also seen between the means of pretreatment and posttreatment HbA1C levels in both metformin group (p=0.03) and repaglinide group (p=0.01). Conclusions: This study suggests that although both metformin and repaglinide are effective in the new-onset T2DM managementz; however, reduction in fasting blood sugar level and HbA1c was more in repaglinide group.

Background: Hydrocynus vittatus and Oreochromis niloticus are two economically important fish species in Zimbabwe. The length-weight relation can be employed to assess the health and well-being of fish so as to assist fish farmers. The objective of the study was to investigate the length-weight relationship and condition factor of these fish species. Methods: Thirty-four fish of each species (Hydrocynus vittatus and Oreochromis niloticus) were collected by seine netting in the Sanyati Basin, Lake Kariba in August 2019. The total length (TL) and standard length (SL) were measured in centimetre (cm), and the body weight (BW) was measured in grams (g). The body weight (BW) was measured using a digital top-loading electronic weighing balance. Results: The value of the exponent b in the LWR for H. vittatus was 2.984, indicating negative allometry (b<3). The exponent value of O. niloticus in the LWR was 3.04 (b>3) implying positive allometry. The r2 values for H. vittatus and O. niloticus were 92.3% and 97.3% respectively, indicating that there was a high degree of positive correlation. The condition factor for both fish species were greater than 1.60 implying good physiological conditions for both fish species. Conclusions: This study provides preliminary information on length-weight parameters of O. niloticus whose data was previously not available in the take.

ABSTRACT

Sepsis is a systemic inflammatory response syndrome in which the host response to infection is dysregulated, leading to circulatory dysfunction and multi-organ damage. It has a high mortality rate and its incidence is increasing year by year, posing a serious threat to human life and health. Mesenchymal stem cells (MSC) have the following properties: hematopoietic support, provision of nutrients, activation of endogenous stem/progenitor cells, repair of tissue damage, elimination of inflammation, immunomodulation, promotion of neovascularization, chemotaxis and migration, anti-apoptosis, anti-oxidation, anti-fibrosis, homing, and many other effects. A large number of studies have confirmed that MSC from different sources have their own characteristics. This article reviews the pathogenesis of sepsis, the biological properties of MSC, and the advantages and disadvantages of different sources of MSC for the treatment of sepsis and their characteristics.

BACKGROUND:Orthosis is a common and effective treatment for mild to moderate scoliosis,which can delay and inhibit the progression of scoliosis and reduce the incidence of severe deformity.Different types of orthoses have different indications,application characteristics,and efficacies.In recent years,the application of new technology like digital and intelligent has promoted the improvement and development of new orthoses. OBJECTIVE:To classify and compare the commonly used scoliosis orthoses,and describe the application progress of new technologies such as digital and intelligent technology in recent years,so as to provide a reference for the clinical selection of orthoses and the improvement of new orthoses. METHODS:PubMed,Embase,IEEE,CNKI and WanFang databases were searched for relevant literature.Chinese and English search terms were"scoliosis,orthosis,brace,progress,artificial intelligence,digitization".The search time limit was from 2012 to 2022.Finally,56 articles were included for review and analysis. RESULTS AND CONCLUSION:(1)Scoliosis orthosis has a good effect on mild to moderate adolescent idiopathic scoliosis.Through the Hueter-Volkmann law,it can accelerate the growth of the concave spine with a high success rate and has been widely used in this field.(2)The indications,application characteristics and efficacy of different types of orthoses are different,and the clinical selection is targeted.(3)Scoliosis orthosis has been developed in the direction of more humanity,paying attention to the comfort of patients,and the manufacturing process has been transformed from plaster casting to computer-aided manufacturing.It is simple and hygienic,with higher correction accuracy,and patients'compliance has been significantly improved.(4)Scoliosis orthosis is developing in the direction of digitalization and intelligence and has been widely integrated with artificial intelligence,Internet of things and other technologies to monitor the patient's orthopedic force,body temperature,and compliance in real time,so as to provide patients with more accurate treatment.(5)At present,there are still many defects in scoliosis orthosis that cannot be ignored,such as affecting development,decreased muscle strength,and body stiffness,which need further exploration and improvement.

ABSTRACT

【Objective】 To establish an effective quality monitoring indicator system for blood quality control in blood banks, in order to analyze the quality control indicators for blood collection and supply, and evaluate blood quality control process, thus promoting continuous improvement and standardizing management of blood quality control in blood banks. 【Methods】 A quality monitoring indicator system covering the whole process of blood collection and supply, including blood donation services, component preparation, blood testing, blood supply and quality control was established. The Questionnaire of Quality Monitoring Indicators for Blood Collection and Supply Process was distributed to 17 blood banks in Shandong, which clarified the definition and calculation formula of indicators. The quality monitoring indicator data from January to December 2022 in each blood bank were collected, and 20 quality control indicators data were analyzed by SPSS25.0 software. 【Results】 The average pass rate of key equipment monitoring, environment monitoring, key material monitoring, and blood testing item monitoring of 17 blood banks were 99.47%, 99.51%, 99.95% and 98.99%, respectively. Significant difference was noticed in the pass rate of environment monitoring among blood banks of varied scales(P<0.05), and the Pearson correlation coefficient (r) between the total number of blood quality testing items and the total amount of blood component preparation was 0.645 (P<0.05). The average discarding rates of blood testing or non-blood testing were 1.14% and 3.36% respectively, showing significant difference among blood banks of varied scales (P<0.05). The average discarding rate of lipemic blood was 3.07%, which had a positive correlation with the discarding rate of non testing (r=0.981 3, P<0.05). There was a statistically significant difference in the discarding rate of lipemic blood between blood banks with lipemic blood control measures and those without (P<0.05). The average discarding rate of abnormal color, non-standard volume, blood bag damage, hemolysis, blood protein precipitation and blood clotting were 0.20%, 0.14%, 0.06%, 0.06%, 0.02% and 0.02% respectively, showing statistically significant differences among large, medium and small blood banks(P<0.05).The average discarding rates of expired blood, other factors, confidential unit exclusion and unqualified samples were 0.02%, 0.05%, 0.003% and 0.004%, respectively. The discarding rate of blood with air bubbles was 0.015%, while that of blood with foreign body and unqualified label were 0. 【Conclusion】 The quality control indicator system of blood banks in Shandong can monitor weak points in process management, with good applicability, feasibility, and effectiveness. It is conducive to evaluate different blood banks, continuously improve the quality control level of blood collection and supply, promote the homogenization and standardization of blood quality management, and lay the foundation for comprehensive evaluation of blood banks in Shandong.

ABSTRACT

【Objective】 To establish an effective quality indicator monitoring system, scientifically and objectively evaluate the quality management level of blood banks, and achieve continuous improvement of quality management in blood bank. 【Methods】 A quality monitoring indicator system that covers the whole process of blood collection and supply was established, the questionnaire of Quality Monitoring Indicators for Blood Collection and Supply Process with clear definition of indicators and calculation formulas was distributed to 17 blood banks in Shandong. Statistical analysis of 21 quality monitoring indicators in terms of blood donation service (10 indicators), blood component preparation (7 indicators ), and blood supply (4 indicators) from each blood bank from January to December 2022 were conducted using SPSS25.0 software The differences in quality monitoring indicators of blood banks of different scales were analyzed. 【Results】 The average values of quality monitoring indicators for blood donation service process of 17 blood banks were as follows: 44.66% (2 233/5 000) of regular donors proportion, 0.22% (11/50) of adverse reactions incidence, 0.46% (23/5 000) of non-standard whole blood collection rate, 0.052% (13/25 000) of missed HBsAg screening rate, 99.42% (4 971/5 000) of first, puncture successful rate, 86.49% (173/200) of double platelet collection rate, 66.50% (133/200) of 400 mL whole blood collection rate, 99.25% (397/400) of donor satisfaction rate, 82.68% (2 067/2 500) of use rate of whole blood collection bags with bypass system with sample tube, and 1 case of occupational exposure in blood collection.There was a strong positive correlation between the proportion of regular blood donors and the collection rate of 400 mL whole blood (P<0.05). The platelet collection rate, incidence of adverse reactions to blood donation, and non-standard whole blood collection rate in large blood banks were significantly lower than those in medium and small blood banks (P<0.05). The average quality monitoring indicators for blood component preparation process of 17 blood banks were as follows: the leakage rate of blood component preparation bags was 0.03% (3/10 000), the discarding rate of lipemic blood was 3.05% (61/2 000), the discarding rate of hemolysis blood was 0.13%(13/10 000). 0.06 case had labeling errors, 8 bags had blood catheter leaks, 2.76 bags had blood puncture/connection leaks, and 0.59 cases had non-conforming consumables. The discarding rate of hemolysis blood of large blood banks was significantly lower than that of medium and small blood banks (P<0.05), and the discarding rate of lipemic blood of large and medium blood banks was significantly lower than that of small blood banks (P<0.05). The average values of quality monitoring indicators for blood supply process of 17 blood banks were as follows: the discarding rate of expired blood was 0.023% (23/100 000), the leakage rate during storage and distribution was of 0.009%(9/100 000), the discarding rate of returned blood was 0.106% (53/50 000), the service satisfaction of hospitals was 99.16% (2 479/2 500). The leakage rate of blood components during storage and distribution was statistically different with that of blood component preparation bags between different blood banks (P<0.05). There were statistically significant differences in the proportion of regular blood donors, incidence of adverse reactions, non-standard whole blood collection rate, 400 mL whole blood collection rate, double platelet collection rate, the blood bag leakage rate during preparation process, the blood components leakage rate during storage and distribution as well as the discarding rate of lipemic blood, hemolysis blood, expired blood and returned blood among large, medium and small blood banks (all P<0.05). 【Conclusion】 The establishment of a quality monitoring indicator system for blood donation services, blood component preparation and blood supply processes in Shandong has good applicability, feasibility and effectiveness. It can objectively evaluate the quality management level, facilitate the continuous improvement of the quality management system, promote the homogenization of blood management in the province and lay the foundation for future comprehensive evaluation of blood banks.

Objective:This study aims to analyze the provincial policy text of separate payment for National medical insurance negotiated drugs,summarize the key measures and characteristics,and provide reference for the formulation and improvement of separate payment policies in various provinces.Methods:The retrieved policy texts were analyzed by searching the websites of the medical security bureaus of various provinces,municipalities,and autonomous regions.Results:There are differences in the separate payment policies of different provinces in terms of catalogue publishing,catalogue selection,drug management,policy treatment level and policy cohesion.Conclusion:The separate payment policy is the key driver of Joint Reformation for Public Health Services,Medical Insurance,and Medical Production-Circulation.And all provinces should balance the various policy elements of the separate payment catalogue,deal with the control of medical insurance funds and patients'sense of gain,and clarify the relationship between the separate payment policy and support the development of innovative drugs,so as to reduce the obstacles of Joint Reformation for Public Health Services,Medical Insurance,and Medical Production-Circulation.

ABSTRACT

Objective To provide reference for the optimization and improvement of interoperability between the standard system of the Chinese Pharmacopoeia and other standards.Methods The interoperability of various pharmacopoeia standard systems was compared by searching for citations from the Chinese Pharmacopoeia,the United States Pharmacopoeia-National Formulary,the European Pharmacopoeia,the Japanese Pharmacopoeia,and other standards,including references to domestic regulations and guidelines,standards of the International Organization for Standardization,guidelines from the International Council for Harmonisation of Technical Requirements for Pharmaceuticals for Human Use,documents of the World Health Organization,and standards from other countries and international organizations.Results In recent years,pharmacopoeias in the world had continuously increased the citation of non pharmacopoeial standards.The types,quantities,and fields of the United States Pharmacopoeia-National Formulary referencing other standards far exceed those of other pharmacopoeias.The Chinese Pharmacopoeia cites the least number of other standards.Conclusion It is suggested that the Chinese Pharmacopoeia should enhance the interoperability with other standard systems in the standards of various professional fields,enhance the openness,harmonization and advantages,and form a more complete standard system.

ABSTRACT

Objective:To compare the clinical efficacy and safety of water jet-assisted dermabrasion versus electric dermabrasion in combination with suction blister epidermal grafting in the treatment of vitiligo.Methods:A total of 60 vitiligo patients were enrolled from the Department of Dermatology, Xijing Hospital from March 2020 to March 2022. Thirty patients firstly received water jet-assisted dermabrasion, 30 firstly received electric dermabrasion, and then all were treated with suction blister epidermal grafting. Follow-up visits were conducted once a month, and the repigmentation of skin lesions and efficacy were evaluated and compared between the two groups 6 months after surgery.Results:There were 30 patients with 312 skin lesions in the water jet-assisted dermabrasion group, including 13 males and 17 females, with the ages and disease duration being 24.41 ± 3.12 years and 5.13 ± 2.34 years respectively; there were 30 patients with 301 skin lesions in the electric dermabrasion group, including 11 males and 19 females, with the ages and disease duration being 22.73 ± 5.11 years and 4.88 ± 2.21 years respectively. No significant differences were observed in the age, gender, disease duration, and dermabrasion sites between the two groups (all P > 0.05). Six months after the operation, 187 (59.94%) skin lesions were healed, 103 (33.01%) were markedly improved, and 22 (7.05%) were improved in the water jet-assisted dermabrasion group; in the electric dermabrasion group, 166 (55.15%) lesions were healed, 108 (35.88%) were markedly improved, and 27 (8.97%) were improved; there was no significant difference in the total response rate between the water jet-assisted dermabrasion group (92.95%) and the electric dermabrasion group (91.03%; χ2 = 0.27, P = 0.602). The water jet-assisted dermabrasion group showed significantly higher degree of repigmentation (90.47% ± 2.53%), matching degree of skin color (3.53 ± 0.21 points), and patient satisfaction scores (3.32 ± 0.27 points) compared with the electric dermabrasion group (82.40% ± 5.33%, 2.71 ± 0.32 points, 2.68 ± 0.41 points, t = 5.30, 8.28, 5.09, respectively, all P < 0.05). No adverse reactions/events were seen in either group. Conclusions:The water jet-assisted dermabrasion combined with suction blister epidermal grafting and electric dermabrasion combined with suction blister epidermal grafting showed similar efficacy in the treatment of vitiligo, with good safety profiles. However, the degree of repigmentation, matching degree of skin color, and patient satisfaction rates were all higher in the patients receiving water jet-assisted dermabrasion than those receiving electric dermabrasion.

ABSTRACT

Objective:To explore the effect of different injection schemes on improving lower eyelid rejuvenation and avoiding complications such as ecchymosis around the orbit.Methods:From September 2021 to June 2022, 86 patients who underwent lower eyelid rejuvenation at the Plastic Surgery Department of Nanjing Medical University Friendship Hospital and Taizhou Medical Cosmetic Clinic of Nanjing Medical University Friendship Plastic Surgery Hospital were included in this study. There were 2 males and 84 females, aged 20-58 (34.27±7.27) years. The patients were divided into two groups: the Cannula Group (68 cases) received appropriate injections under the deep layer of muscle fat pad and superficial layer of subcutaneous tissue using a 25 G cannula at two entry points on the lateral orbital zygomatic groove and the extension line of tear trough buccal groove; the Needle Group (18 cases) received injections directly perpendicular to the skin top to the orbital arch border of the corresponding area of tear trough and eyelid commissure fossa and superficial layer of subcutaneous fat using a 30G needle. The therapeutic effects and complications such as periorbital ecchymosis and swelling were observed in both groups.Results:In the Cannula Group, all 68 patients showed significant improvement in tear trough groove and eyelid commissure groove, achieving eyelid rejuvenation. Four patients developed spotty ecchymosis, and one patient was dissatisfied with the treatment outcome. The other patients were satisfied with the results. In the Needle Group, all 18 patients showed improvement in tear trough groove and eyelid commissure groove, with 6 patients developing ecchymosis. Two patients with large range of ecchymosis expressed dissatisfaction, while the rest were satisfied with the treatment outcome.Conclusions:Tear trough groove depression and eyelid commissure groove depression have been improved to varying degrees in both the Cannula Group and the Needle Group, achieving periorbital rejuvenation. However, the Cannula Group has a lower probability of complications such as ecchymosis compared to the Needle Group in lower eyelid rejuvenation treatment.
