Presentamos un caso de quiste óseo aneurismático (QOA) de ubicación infrecuente y comportamiento agresivo en un paciente masculino de 28 años, en que la resección quirúrgica es controversial por el riesgo de iatrogenia y eventual recurrencia. El tratamiento con denosumab ha sido recientemente propuesto como una alternativa para el manejo de QOAs irresecables o recurrentes; sin embargo, la literatura disponible es escasa. Reportamos nuestra experiencia en un caso y analizamos la bibliografía disponible
We present a case of aneurysmal bone cyst (ABC) of infrequent location and aggressive behavior in a 28-year-old male patient, in which surgical resection is controversial due to the risk of iatrogenicity and eventual recurrence. Clival giant cell tumors (GCTs) are an extremely rare type of tumor, with only 13 cases reported to date. Despite being histologically considered benign, these tumors can be locally aggressive and have a high rate of local recurrence, as well as the potential for distant metastasis. Due to their rarity and the difficulty of treating them due to their critical location, the ideal treatment protocol for managing clival GCTs remains controversial. The present report describes a 22-year-old female who experienced ptosis on her right eye and visual disturbance for one month. Magnetic resonance imaging (MRI) revealed a large, heterogenous, lobulated mass lesion arising from the clivus, extending anteriorly and compressing the optic chiasm and optic tract, with both parasellar extension and extension into the sphenoid sinus. An endoscopic endonasal trans-sphenoidal procedure was performed to excise the lesion. Post-operative MRI revealed residual disease, so a revision surgery was done using a combined microscopic and endoscopic procedure. Following surgery, the patient was given Denosumab for one year as well as has been symptom-free for the past 18 months of follow-up. This report contributes to the limited literature on the GCTs involving the clivus.

OBJECTIVE@#To evaluate the effect of denosumab on bone mineral density around proximal femoral prosthesis after total hip arthroplasty(THA) in the postmenopausal osteoporotic patients.@*METHODS@#Fifty-four consecutive patients underwent unilateral primary THA were included in this retrospective study. Twenty-five patients received denosumab for osteoporosis as the treatment group, and the twenty-nine without denosumab were the control group. At 1 week, 3month, 6 months, and 12 months after THA, bone turnover markers and proximal femoral periprosthetic bone mineral density (BMD) were measured.@*RESULTS@#At 3, 6 and 12 months after operation, the level of TRACP-5b in the control group was significantly higher than that in the treatment group (P<0.05);the level of bone-specific alkaline phosphatase (BALP) between two groups showed significant difference in 12 months after operation (control group was higher than treatment group, P<0.05). The BMD of Gruen 1 and Gruen 7 decreased at 3, 6 and 12 months after operation compared with 1 week after operation. Comparing the treatment group and the control group, the differences of the the decrease of BMD in Gruen 1 and Gruen 7 were no significant at 3 months after surgery. In Gruen 1, Gruen 7 at 6 months after operation and Gruen 1, Gruen 7 at 12 months after operation, the decrease of BMD in the control group was significantly higher than that in the treatment group(P<0.05). It is suggested that desudumab could inhibit the loss of BMD after 6 months, and continuously show a protective effect on bone mass at 12 months after operation.@*CONCLUSION@#After THA in postmenopausal patients with osteoporotic femoral neck fracture, Desuzumab can reduce the loss of BMD around the proximal femoral prosthesis and effectively inhibit bone resorption.

Osteoporosis is a comorbidity or complication of chronic kidney disease (CKD). Low bone mineral density, osteoporosis, and fractures are risk factors of poor prognosis in patients with CKD. Active prevention and treatment of osteoporosis is essential for patients with CKD. Denosumab is a fully human monoclonal antibody that targets receptor activator of nuclear factor-κB ligand (RANKL) and is not metabolized or excreted by the kidney. Clinical studies have shown that the use of denosumab in CKD (including dialysis and renal transplantation) patients with osteoporosis, can increase bone mineral density and reduce the incidence of fractures with acceptable safety. As hypocalcemia has been observed in patients with CKD, close monitoring and early supplementation of calcium and vitamin D are required before denosumab initiation, and serum calcium levels need to be monitored during treatment. The review summarized the published literature of denosumab in bone protection of CKD.

Denosumab is a monoclonal antibody against the receptor activator of nuclear factor-κB (RANK) ligand (RANKL) that significantly inhibits osteoclast activity and has been approved to treat osteoporosis, giant cell tumor of bone, and prophylactic as well as therapeutic entities for bone metastasis. However, the imbalance of RANKL/RANK/OPG has also been implicated in the pathogenesis of several other rare bone diseases and tumor-like disorders, including aneurysmal bone cyst, fibrous dysplasia of bone, and Langerhans cell histiocytosis. Nevertheless, there have been various clinical reports although these diseases have not been approved for indications. The review aims to summarize the available evidence for the off-label use of denosumab in metabolic bone diseases and tumor-like disorders and provide a reference for clinical diagnosis and treatment.

OBJECTIVE To investigate the characteristics and regularity of denosumab-associated atypical fractures (AF), so as to provide references for clinical rational use of drugs. METHODS The case reports of AF related to denosumab were retrieved from PubMed, Web of Science, CNKI, Wanfang data and VIP databases, and the reports were descriptively analyzed. RESULTS A total of 19 references were retrieved, including 20 patients. There were 3 males and 17 females, with an average age of (69.80± 15.39) years. Among 20 patients, primary diseases of 14 patients were osteoporosis, and 6 cases were malignant tumor bone metastasis and giant cell tumor of bone. The occurrence time of AF ranged from 3 to 132 months after the administration of denosumab, with an average of (42.14±29.49) months. Fourteen cases had prodromal symptoms before AF. There were 3 cases of ulna fractures, and the remaining 17 cases were femoral fractures. The vast majority of patients recovered well after discontinuing medication and undergoing surgical fixation, but some patients experienced delayed fracture healing. CONCLUSIONS Long-term use of denosumab should be vigilant against AF. When patients experience prodromal symptoms such as thigh, groin, hip joint and forearm pain, they should seek medical attention in a timely manner to ensure medication safety.

ABSTRACT: Currently, there are no guidelines for treating osteoporosis in spinal surgery. The rate of complications such as screw loosening, proximal junction kyphosis, cage subsidence, and loss of reduction in fractures is high. Objective: To evaluate the use of teriparatide and denosumab in planning spinal surgery in an osteoporotic patient with degenerative pathology, emphasizing the fusion rate, bone mineral density, and decreased complications. Method: A systematic search was performed in medical reference databases for comparative studies of teriparatide and denosumab in spinal surgery to evaluate fusion, screw loosening, bone mineral density, and decrease in the incidence of vertebral fractures. χ2 was implemented for the statistical analysis, according to PRISMA (2020). Result: Fusion rate with teriparatide was 79.28% in the first six months, 95% CI (OR 2.62) and decreased screw loosening rate 81.9% 95% CI (OR 0.6). Increase in bone mineral density 15.5% OR 1.49 (0.77 - 2.86) and decrease in vertebral fracture rate 85.4% OR 0.5. Conclusions: Teriparatide and denosumab should be considered in perioperative spinal planning due to their effectiveness, synergism, and low adverse effects; to improve bone mineral density and decrease the rate of complications. Clinical, comparative, and statistically significant studies are required to confirm this. Level of Evidence II; Systematic Review and Meta-analysis.
RESUMO: Atualmente não existem diretrizes para o tratamento da osteoporose em cirurgia da coluna vertebral. A taxa de complicações como afrouxamento de parafuso, cifose da junção proximal, subsidência da gaiola e perda de redução nas fraturas é alta. Objetivo: Avaliar o uso de teriparatida e/ou denosumabe no planejamento da cirurgia da coluna vertebral em pacientes osteoporóticos com patologia degenerativa, enfatizando a taxa de fusão, densidade mineral óssea e diminuição de complicações. Método: Foi realizada uma busca sistemática em bases de dados de referência médica para estudos comparativos de teriparatida e denosumabe em cirurgia da coluna vertebral, a fim de avaliar fusão, soltura de parafuso, densidade mineral óssea e diminuição da incidência de fraturas vertebrais. O χ2 foi implementado para a análise estatística, de acordo com PRISMA (2020). Resultado: A taxa de fusão com teriparatida foi de 79,28% nos primeiros 6 meses IC 95% (OR 2,62) e diminuiu a taxa de afrouxamento do parafuso 81,9% IC 95% (OR 0,6). O aumento da densidade mineral óssea foi de 15,5% OR 1,49 (0,77 - 2,86) e a diminuição da taxa de fratura vertebral atingiu 85,4% OR 0,5. Conclusões: A teriparatida e o denosumabe devem ser considerados no planejamento espinhal perioperatório devido à sua efetividade, sinergismo e baixos efeitos adversos, melhorando a densidade mineral óssea e diminuir a taxa de complicações. RESUMEN: Actualmente no existen pautas para el tratamiento de la osteoporosis en cirugía espinal. La tasa de complicaciones como el aflojamiento de los tornillos, la cifosis de la unión proximal, el hundimiento del aparato Ilizarov y la pérdida de reducción de las fracturas es alta. Objetivo: Evaluar el uso de teriparatida y/o denosumab en la planificación de la cirugía de columna en el paciente osteoporótico con patología degenerativa haciendo hincapié en la tasa de fusión, la densidad mineral ósea y la disminución de las complicaciones. Método: Se realizó una búsqueda sistemática en bases de datos de referencia médica para estudios comparativos de teriparatida y denosumab en cirugía espinal con el fin de evaluar la fusión, el aflojamiento de tornillos, la densidad mineral ósea y la disminución de la incidencia de fracturas vertebrales. χ2 se implementó para el análisis estadístico, según PRISMA (2020). O edema tardio intermitente persistente (ETIP) é uma reação inflamatória imunomediada desencadeada pela aplicação de ácido hialurônico (AH). É classificado clinicamente como não depressível, difuso e de caráter tardio, por aparecer, no mínimo, 30 dias após a aplicação. Ademais, caracteriza-se como intermitente e recorrente, visto que a reação pode reaparecer enquanto a substância perdurar no tecido. O presente estudo tem como objetivo relatar um caso de ETIP desencadeado pelo medicamento Prolia, sua apresentação clínica e evolução do quadro, bem como o tratamento realizado. No presente relato de caso, foi observada essa reação após uso do medicamento Prolia (Denosumabe), um anticorpo monoclonal que reduz a reabsorção óssea, utilizado para o tratamento da osteoporose. A paciente em questão é uma mulher de 56 anos, procedente de Joaçaba (SC), a qual apresentou placas eritematosas infiltradas na região do lábio superior, inferior e sulconasomentual, após cinco meses da aplicação do AH. Após conhecimento das medicações de uso e posterior confirmação histopatológica, foi diagnosticada com ETIP e iniciou conduta médica. A ETIP geralmente apresenta uma progressão benigna, se diagnosticada e tratada corretamente. A paciente em questão apresentou completa melhora clínica e, apesar de ainda não existir um consenso na literatura sobre como manejar a ETIP, o tratamento que melhor se adaptou ao caso foi a administração de Prednisolona por 4 semanas. Por não haver sido descrito anteriormente, na literatura mundial, o aparecimento de ETIP desencadeado pelo Denosumabe, destaca-se a importância do presente estudo para elucidação desta possível inteiração entre os compostos.
Persistent intermittent delayed swelling (PIDS) is an inflammatory reaction immune-mediated by the application of hyaluronic acid (HA). It is clinically classified as non-depressible, diffuse, and delayed, as its onset is in at least 30 days following application. Moreover, it is characterized as intermittent and reocurring, as the reaction may reappear while the substance remains in the tissue. The present study aims to report a case of PIDS caused by the drug Prolia, including its clinical presentation and development, as well as the treatment performed. In this case report the reaction was observed after the use of Prolia (Denosumab), a monoclonal antibody that reduces bone reapsorption, indicated for the treatment of osteoporosis. The patient was 56 years-old woman from Joaçaba (SC) that presented erythematous plaques in the upper and lower lip regions and in the melomental folds five months after the application of HA. After acknowledging the medications under use and histopathological confirmation, the patient was dignosed with PIDS and started medical treatment. PIDS usually has a benign progression if correctly diagnosed and treated. The patient made a full clinical recovery and, despite the lack of consensus in the literature for the management of PIDS, the treatment with best responses for this case was the administration of Prednilosone for four weeks. As the emergence of PIDS triggered by Denosumab was not previously described in the global literature, the current study is important to elucidate the possible interaction between the compounds.
La hinchazón persistente intermitente retardada (PIDS) es una reacción inflamatoria inmunomediada por la aplicación de ácido hialurónico (AH). Clínicamente se clasifica como no depresible, difusa y retardada, ya que su aparición se produce en al menos 30 días tras la aplicación. Además, se caracteriza como intermitente y recidivante, ya que la reacción puede reaparecer mientras la sustancia permanece en el tejido. El presente estudio tiene como objetivo reportar un caso de PIDS causado por el medicamento Prolia, incluyendo su presentación clínica y desarrollo, así como el tratamiento realizado. En este caso clínico la reacción se observó tras el uso de Prolia (Denosumab), un anticuerpo monoclonal que reduce la reabsorción ósea, indicado para el tratamiento de la osteoporosis. La paciente era una mujer de 56 años de Joaçaba (SC) que presentó placas eritematosas en las regiones labial superior e inferior y en los pliegues melomentales cinco meses después de la aplicación del HA. Después del reconocimiento de los medicamentos en uso y de la confirmación histopatológica, la paciente fue dignosticada con PIDS e inició tratamiento médico. El PIDS suele tener una evolución benigna si se diagnostica y trata correctamente. La paciente tuvo una recuperación clínica completa y, a pesar de la falta de consenso en la literatura para el manejo de laIDS, el tratamiento con mejores respuestas para este caso fue la administración de Prednilosona durante cuatro semanas. Como la aparición de PIDS desencadenada por Denosumab no fue descrita anteriormente en la literatura mundial, el presente estudio es importante para dilucidar la posible interacción entre los compuestos.

Introdução:O Denosumabeé um fármaco antirreabsortivo indicado para o tratamento de osteoporose e doenças ósseas metastáticas. O seu uso está associado ao desenvolvimento de reações adversas em diferentes órgãos, como a osteonecrose dos maxilares, que é o evento adverso de interesseodontológico. Objetivo:Realizar um levantamento bibliográfico sobre o mecanismo de ação do Denosumabe no tecido ósseo e destacar a importância do cirurgião-dentista na prevenção, no diagnóstico e tratamento da osteonecrose nos maxilares.Metodologia:Trata-se de uma revisão integrativa elaboradaem duas etapas: inicialmente realizou-se uma busca de artigos publicados entre os anos 2010a 2022, sobre a osteonecrose em pacientes que fazem uso do Denosumabe nas plataformas de dados Pubmed, ScieloeBiblioteca Virtual em Saúde. Posteriormente, foi feita uma seleção de partes relevantes para a pesquisa, uma leitura analítica e a organização das informações coletadas pertinentes a cada tópico da pesquisa.Resultados:ODenosumabe inibea ligação da citocina RANKL ao seu receptor RANK, tal mecanismo de ação reduz o processo de reabsorção óssea execessiva. As osteonecroses podem apresentar-se em diferentes níveis de estadiamento e caracterizam-se como área de exposição óssea necrótica na região maxilofacial, permanecendo por mais de oito semanas e sem histórico de radioterapia ou doença metastática evidentes nos maxilares. Alguns fatores predispõem o desenvolvimento das osteonecroses, entre eles: procedimentos odontológicos cirúrgicos. Ainda não existe um protocolo de tratamento definitivo, entretanto, modalidades terapêuticas coadjuvantes são administradas de acordo com a condição clínicado paciente.Conclusões:O exame clínico deve ser minucioso, atentando-se a qualquer alteração na cavidade bucal, às doenças preexistentes e às medicações utilizadas pelo paciente. Em todos os casos deve-se, realizar orientações de higiene oral e adequação do meio bucal previamente ao tratamento oncológico e ao uso de drogas antirreabsortivas (AU).
Introduction:Denosumab is an antiresorptive drug indicated for the treatment of osteoporosis and metastatic bone diseases. Its use is associated with the development of adverse reactions in different organs, such as osteonecrosis of the jaws, which is an adverse event ofdentalinterest.Objective:Conducta bibliographic survey on the mechanism of action of Denosumab in bone tissue and to highlight the importance of the dentist in the prevention, diagnosis and treatment of osteonecrosis in the jaws. Methodology: This is an integrative review carried out in two stages: initially, a search was carried out for articles published between the years 2010to 2022, on osteonecrosis in patients using Denosumab in the data platforms Pubmed, Scieloand Virtual Health Library(BVS). Subsequently, a selection of relevant parts for the research was made, an analytical reading and the organization of the collected information pertinent to each research theme was carried out.Results:TheDenosumab inhibitsthe binding of the RANKL cytokine to its RANK receptor, this mechanism of action reduces the process of excessive bone resorption. Osteonecrosis can present at different staging levels and are characterized as an area of necrotic bone exposure in the maxillofacial region, lasting for more than eight weeks and without a history of radiotherapy or evident metastatic disease in the jaws. Some factors predispose the development of osteonecrosis, including: surgical dental procedures. There is still no definitive treatment protocol, however, supporting therapeutic modalities are administered according to the patient's clinical condition.Conclusions:The clinical examination must be thorough, paying attention to any changes in the oral cavity, pre existing diseases and medications used by the patient. In all cases, guidelines on oral hygiene and adequacy of the oral environment should be carried out prior to oncological treatment and the use of antiresorptive drugs (AU).
Introducción: Denosumab es un fármaco antirresortivo indicado para el tratamiento de la osteoporosis y enfermedades óseas metastásicas. Su usoestá asociado al desarrollo de reacciones adversas en diferentes órganos, comola osteonecrosis de los maxilares, que es un evento adverso de interés odontológico. Objetivo: Realizar un levantamiento bibliográfico sobre el mecanismo de acción de Denosumab en el tejido óseo y resaltar la importancia del odontólogo en la prevención, diagnóstico y tratamiento de la osteonecrosis en los maxilares. Metodología: Esta es una revisión integradora realizada en dos etapas: inicialmente se realizó una búsqueda de artículos publicados entre los años 2010 a 2022, sobre osteonecrosis en pacientes usuarios de Denosumab en las plataformas Pubmed, ScieloyBiblioteca Virtual en Salud(BVS).Posteriormente, se realizó una selección de partes relevantes para la investigación, se realizó una lectura analítica y la organización de la información recolectada relevante para cada tema de investigación. Resultados:Denosumab inhibela unión de la citoquina RANKL a su receptor RANK, este mecanismo de acción reduce el proceso de reabsorción ósea excesiva. La osteonecrosis puede presentarse en diferentes nivelesde estadificación y se caracterizan por un área de exposición ósea necrótica en la región maxilofacial, con una duración mayor a ocho semanas y sin antecedentes de radioterapia o enfermedad metastásica evidente en los maxilares. Algunos factores predisponen al desarrollo de osteonecrosis, entre ellos: procedimientos quirúrgicos dentales. Aún noexiste un protocolo de tratamiento definitivo, sin embargo, se administran modalidades terapéuticas de apoyo de acuerdo a la condición clínica del paciente.Conclusiones: El examen clínico debe ser minucioso, prestando atención a cualquier cambio en la cavidad bucal, enfermedades preexistentes y medicamentos utilizados por el paciente. En todos los casos se deben realizar pautas de higiene bucal y adecuación del medio bucal previo al tratamiento oncológico y al uso de fármacos antirresortivos (AU).

ABSTRACT Denosumab (DMAb) is a human monoclonal antibody used as an antiresorptive drug in the treatment of osteoporosis. Approval at a dosage of 60 mg every 6 months was based on the results of the randomized, placebo-controlled trial (FREEDOM). The design of this 3-year study included an extension for up to 10 years. Those who were randomized to DMAb continued on drug, while those who were randomized to placebo transitioned to DMAb. The 10-year experience with DMAb provides data on efficacy of drug in terms of reduced fractures and continued increases in bone mineral density (BMD). The 10-year experience with denosumab also provides information about rare complications associated with the use of DMAb, such as osteonecrosis of the jaw (ONJ), and atypical femoral fractures (AFF). This experience provided new insights into the reversibility of effects upon discontinuation without follow-on therapy with another agent. This review focuses upon prolonged treatment with DMAb, with regard to beneficial effects on fracture reduction and safety. Additionally, its use in patients with impaired renal function, compare its results with those of bisphosphonates (BPs), the occurrence/frequency of complications, in addition to the use of different tools, from imaging techniques to histological findings, to evaluate its effects on bone tissue.

Introducción: La administración de bifosfonatos y medicamentos antiangiogénicos en pacientes con cáncer es un esquema terapéutico usual en oncología. Existen reportes de osteonecrosis de los maxilares en pacientes sometidos a este esquema de tratamiento, luego de realizar un procedimiento dental invasivo. Objetivo: A partir de las características clínicas e imagenológicas de la patología, ilustrar al odontólogo sobre los medicamentos para el tratamiento del cáncer, susceptibles de generar osteonecrosis de los maxilares. Presentación de caso: Paciente masculino de 89 años, con cáncer de próstata tratado con denosumab, que desarrolló osteonecrosis del maxilar inferior posterior a una extracción dental. Es de vital importancia que el odontólogo identifique los medicamentos, factores de riesgo y las medidas para minimizar el riesgo de osteonecrosis de los maxilares en pacientes susceptibles(AU)
Introduction: The administration of bisphosphonates and antiangiogenic drugs in cancer patients is a usual therapeutic scheme in oncology. There are reports of osteonecrosis of the jaws in patients undergoing this treatment scheme, after performing an invasive dental procedure. Objective: Show the dentist from the clinical and imaging characteristics of the pathology on the drugs for the treatment of cancer sensitivity to generate osteonecrosis of the jaws. Case presentation: An 89-year-old male patient with prostate cancer treated with denosumab developed osteonecrosis of the lower jaw after tooth extraction. It is vitally important that the dentist identifies medications, risk factors and measures to minimize the risk of osteonecrosis of the jaws in sensitivy patients(AU)

SUMMARY OBJECTIVE: There are limited studies investigating the comparison of the efficacy of anti-osteoporotic drugs in different conditions resulting in osteoporosis in older adults. This study aimed to compare the effectiveness of anti-osteoporotic agents in older adults with or without glucocorticoid-induced osteoporosis. METHODS: This retrospective study included 364 patients with osteoporosis, aged 65 years and older. Bone mineral density measurement was performed, and the percent change from baseline was calculated at month 24. RESULTS: Of the 364 patients, 80 were glucocorticoid users. Similar changes in the bone mineral density of the lumbar spine and femoral neck and fracture risk were found in patients with or without glucocorticoid-induced osteoporosis. There was no significant difference in bone mineral density changes between the groups in terms of anti-osteoporotic agents used. CONCLUSIONS: This study demonstrated that the response to anti-osteoporotic agents was similar in older adults with glucocorticoid-induced osteoporosis and those without glucocorticoid-induced osteoporosis. The results of our study may guide osteoporosis treatment in older individuals with glucocorticoid-induced osteoporosis.

INTRODUCCIÓN Los quistes óseos aneurismáticos (QOAs) son tumores benignos, localmente agresivos, y con importante potencial de recidiva, que representan aproximadamente el 1% de todos los tumores óseos. Se describen múltiples tratamientos, como: escisión intralesional, embolización arterial selectiva, inyección de agentes esclerosantes, y radiación. Estos tratamientos tienen una tasa variable de eficacia, ya que la recurrencia puede llegar al 20% y puede estar asociada a comorbidades graves como la pérda funcional de la extremidad. OBJETIVO Realizar una revisión integradora de la literatura sobre el uso de denosumab para el tratamiento de QOAs, describiendo el perfil epidemiológico, la dosis utilizada, y las complicaciones. MÉTODO Se recopilaron artículos publicados en los últimos cinco años en la base de datos PubMed. La información recogida de los casos reportados fue la edad, el sexo, la ubicación del tumor, la realización de cirugía antes y/o después del tratamiento con denosumab, la dosis utilizada, las complicaciones, y la recurrencia. RESULTADOS Se analizaron 7 artículos, 4 reportes de casos y 3 series de casos, escritos en inglés, y publicados de 2014 a 2019. La mayoría de los pacientes eran del sexo femenino, con una edad promedio de 14 años, y el tumor localizado en la columna. CONCLUSIÓN El uso de denosumab en el tratamiento de QOAs ha tenido una buena respuesta, ya que tiene bajas tasas de recurrencia y complicaciones; sin embargo, hacen falta más estudios para definir el protocolo de tratamiento.
INTRODUCTION Aneurysmal Bone Cysts (ABCs) are locally-aggressive benign tumors with relevant potential for recurrence, representing approximately 1% of all bone tumors. Multiple treatments are described for them, such as: intralesional excision, selective arterial embolization, injection of sclerosing agents, and radiation. These treatments have a variable efficacy rate, can reach 20% and may be associated with serious comorbidities such as functional loss of the limb. OBJETIVE To perform an integrative review of the literature on the use of denosumab in the treatment of ABCs, describing the epidemiological profile, the dosage used, and the complications. METHODOLOGY Articles published in the past five years were retrieved from the PubMed database. The information collected from the cases reported was age, gender, tumor location, the performance of surgery before and/or after the denosumab treatment, the dose used, the complications, and recurrence. RESULTS We analyzed 7 articles, 4 case reports and 3 case series, written in English, and published from 2014 to 2019. Most patients were female, with an average age of 14 years, with the tumor located in the spine. CONCLUSION The use of denosumab in the treatment of ABCs yielded good results, with low rates of recurrence and complications. However, further studies are needed to define a treatment protocol.

OBJECTIVE To evaluate the cost-effectiveness of denosumab and zoledronic acid in the treatment of postmenopausal osteoporosis ,and to provide reference for relevant decision-making. METHODS From the perspective of Chinese health system ,Excel 2003 software was used to establish Markov model ,and cost-utility analysis was used to evaluate the cost-effectiveness of denosumab or zoledronic acid combined with calcium carbonate D 3 in the treatment of postmenopausal osteoporosis. Pharmacotherapy effects were obtained from the network Meta-analysis ,and cost and health-utility value data were obtained from the published literature or network ,etc. The model cycle was 1 year,and the simulation time limit was the patient ’s lifetime. One-way sensitivity analysis and probabilistic sensitivity analysis were used to evaluate the impact of model parameter changes on the robustness of the results ;and the cost-effectiveness of changing the medication cycle of zoledronic acid were explored through scenario analysis. RESULTS Denosumab regimen was more effective than zoledronic acid regimen (12.77 QALYs vs. 11.98 QALYs),and its cost was also higher than zoledronic acid regimen (51 224.56 yuan vs. 49 221.67 yuan), and the incremental cost-effectiveness ratio was 2 544.14 yuan/QALY. One-way sensitivity analysis showed that the cost of Zoledronic acid injection and that of Denosumab injection had great impact on the results. The results of probabilistic sensitivity analysis showed that when using 3 times of per capita gross domestic product (GDP)in China in 2021 as the threshold of willingness to pay ,the probability of Denosumab regimen being cost-effective was 85.4%. The results of the scenario analysis showed that the Denosumab regimen was still more cost-effective when the dosing cycle of zoledronic acid was changed. CONCLUSIONS Under the threshold of 1-3 times of Chinese per capita GDP in 2021,denosumab combined with calcium carbonate D 3 is more cost-effective than zoledronic acid combined with calcium carbonate D 3 in the treatment of postmenopausal osteoporosis.

Objective:To compare the numerical rating scales (NRS) and Oswestry disability index (ODI) of denosumab in Chinese postmenopausal osteoporosis patients after 3 months, and analyze the early adverse reactions to provide reference for clinical diagnosis and treatment.Methods:Using a prospective study method, 260 patients with postmenopausal osteoporosis who were outpatients and inpatients in the Second Affiliated Hospital of Soochow University from September 2020 to October 2021 were selected, and general information, including age, height, weight, bone mineral density, history of fragility fractures, and use of anti-osteoporosis drugs. All subjects received denosumab 60 mg subcutaneously, and were given calcium and vitamin D at the same time. Pain was scored by NRS before treatment and 3 months after treatment, and functional improvement was assessed by ODI.Results:After 3 months of denosumab treatment in postmenopausal women with osteoporosis, among patients with different age groups, different degrees of osteoporosis, history of fragility fractures, and history of use of anti-osteoporosis drugs, NRS score and ODI score were lower than those before treatment, and the difference was statistically significant ( P<0.05). In addition, in patients with a history of fragility fractures (mainly vertebral fractures), the NRS scores and the ODI score decreased more significantly, and the difference was statistically significant ( P<0.05); the NRS score and ODI score decreased more significantly in patients with severe osteoporosis than in patients with osteoporosis, and the difference was statistically significant ( P<0.05); the BMD value of lumbar spine was negatively correlated with the reduction of NRS score before and after treatment ( P=0.042). In this study, 260 patients had musculoskeletal pain in 6 (2.3%), fatigue in 5 (1.9%), rash in 4 (1.5%), urinary tract infection in 2 (0.7%), and dizziness in 2 (0.7%), 2 case of fever (0.7%), 1 case of hypocalcemia (0.4%), a total of 22 cases of adverse reactions were reported, and the overall adverse reaction rate was 8.5%. Conclusion:Denosumab can improve pain symptoms and functional disability early in the clinical application of Chinese postmenopausal women with osteoporosis, and the incidence of adverse reactions is low. Especially for postmenopausal female osteoporosis patients with severe osteoporosis, low lumbar spine bone density, and a history of fragility fractures (mainly vertebral fractures), the application effect is more significant.

Osteoporosis is characterized by low bone mass with deterioration of the tissue microarchitec- ture leading to fragility, which increases the risk of fractures. After menopause, estrogen deficiency increases tissue exposure to the RANK ligand, resulting in increased bone loss and resorption, which can lead to osteoporosis. (1) Bisphosphonates and denosumab are used for the treatment in low concentration, due to their anticatabolic capacity, which reduce remodeling, preventing loss of bone mass and fractures besides, antiresorptives drugs increase the mineral density of the tissue. ABSTRACT Introduction: Denosumab is a human monoclonal antibody that binds to the receptor activator of nuclear factor kB (RANKL), it is used in the treatment of Osteoporosis. The Giant Cell Tumor (GCT) and the Aneurysmal Bone Cyst (ABC) use the same RANKL, and for this reason this drug began to be used for its treatment. There is consensus on the use, dose-time and 12-month duration for Denosumab treatment of GCT. Not so for ABC. In unresectable, disabling or recurrent tumors, its use could be for life. The adverse events of the habitual use of the drug are known, but it is not known if these increase with time. The objective of the present work is to identify the possible adverse events of treatment with Denosumab for more than 12 months. Material and Method: Series of cases with a diagnosis of GCT or ABC in spine, treated with Denosumab for more than 12 months. Adverse events are: arthralgia, fatigue, spinal pain, pain in extremities, headache, hypokalaemia, hypocalcemia, osteonecrosis of the jaw, malignant transformation, pathological fractures. Results: Eight patients, 6 TCG and 2 ABC, with a mean age at diagnosis of 25,6 years; presenting a mean treatment of 4.18 years (range 1.7 - 8.7). Of 6 operated patients, 4 had recurrence (2 to 36 months after surgery). One patient had to suspend treatment due to necrosis of the jaw, another hypocalcemia, both returned to treatment when stabilized. Conclusions: A minor adverse event (hypocalcemia) and a major adverse event (jaw bone necrosis) were observed. Level of Evidence IV; Original.
RESUMO Introdução: O denosumab é um anticorpo monoclonal humano que se liga ao receptor ativador do fator nuclear kB (RANKL), sendo utilizado no tratamento da Osteoporose. O Tumor de Células Gigantes (TCG) e o Cisto Ósseo Aneurismático (CAO) utilizam o mesmo RANKL, por isso esse medicamento passou a ser utilizado para seu tratamento. Há consenso sobre o uso, o tempo de dosagem e a duração de 12 meses para o tratamento com Denosumabe de TCG. Não é assim para CAO. Em tumores irressecáveis, incapacitantes ou recorrentes, seu uso pode ser vitalício. Os eventos adversos do uso habitual do medicamento são conhecidos, mas não se sabe se aumentam com o tempo. O objetivo do presente trabalho é identificar os possíveis eventos adversos do tratamento com Denosumabe por mais de 12 meses. Material e Método: Série de casos com diagnóstico de TCG ou CAO na coluna, tratados com Denosumabe por mais de 12 meses. Os eventos adversos são: artralgia, fadiga, dor na coluna, dor nas extremidades, cefaleia, hipocalemia, hipocalcemia, osteonecrose da mandíbula, transformação maligna, fraturas patológicas. Resultados: Oito pacientes, 6 TCG e 2 LRA, com média de idade ao diagnóstico de 25,6 anos; apresentando um tratamento médio de 4,18 anos (variação 1,7 - 8,7). Dos 6 pacientes operados, 4 tiveram recorrência (2 a 36 meses após a cirurgia). Um paciente teve que suspender o tratamento por necrose da mandíbula, outro hipocalcemia, ambos voltaram ao tratamento quando estabilizados. Conclusões: Um evento adverso menor (hipocalcemia) e um evento adverso maior (necrose óssea da mandíbula) foram observados. Nível de Evidência IV; Original.

En tumores irresecables, incapacitantes o con recidiva, su uso podría ser de por vida. Se conocen los eventos adversos de la utilización habitual de la droga, pero no se sabe si estas aumentan con relación al tiempo. El objetivo del presente trabajo, es identificar los posibles eventos adversos del tratamiento con Denosumab por más de 12 meses. Material y Método: Serie de casos con diagnóstico de TCG o QOA de columna, tratados con Denosumab por más de 12 meses. Los eventos adversos son: artralgias, fatiga, raquialgia, dolor en extremidades, cefalea, hipopotasemia, hipocalcemia, osteonecrosis de mandíbula, transformación maligna, fractura patológica. Resultados: Ocho pacientes, 6 TCG y 2 QOA, con promedio de edad al diagnóstico de 25,6 años; presentando una media de tratamiento de 4.18 años (rango 1,7 - 8,7). De 6 pacientes operados, 4 presentaron recidiva (2 a 36 meses después de la cirugía). Abstract Antiresorptive drugs operate in the bone metabolism modulation and are widely used in the treatment of bone metastases and bone losses related to hormonal deficiency. Although this therapy shows satisfactory results, there are adverse effects associated with its use, such as osteonecrosis of the jaws. Medication-related osteonecrosis of the jaws (MRONJ) is, therefore, a serious and challenging condition with important implications in dentistry. The aim was to conduct a narrative literature review on anti-resorptive drugs and their latest repercussions on the maxillary bones. The review was carried out through a bibliographic search using Decs/Mesh descriptors of interest, in Portuguese and English, in the PubMed, Virtual Health Library (VHL) and Scielo databases. After applying the inclusion and exclusion criteria, a total of 33 studies were selected for analysis. It can be noticed that therapy with anti-resorptive agents is complex, especially in dental practice, since MRONJ is a complication that is difficult to manage. Regarding the therapeutic options, these are divided into conservative, surgical or adjuvant therapy, however, there are no protocols in the literature, and there is no consistency regarding the indication of the suspension of the drug administration - "Drug Holiday". Thus, it is important that the multidisciplinary team seeks strategies that minimize complications and promote control over the use of these drugs. In addition, there is a need for investigations that contribute with guidelines for the management and control of adverse effects resulting from therapy with antiresorptive drugs. (AU)
Resumo As drogas antirreabsortivas atuam na modulação do metabolismo ósseo e são indicadas para o tratamento de metástases ósseas e perdas ósseas relacionadas à deficiência hormonal. Ainda que esta terapia apresente resultados satisfatórios, observam-se efeitos adversos associados ao seu uso, como a osteonecrose dos maxilares. A osteonecrose dos maxilares associada ao uso de medicamentos (OMAM) é, portanto, uma condição séria e desafiadora com implicações importantes na Odontologia. O objetivo foi realizar uma revisão narrativa de literatura sobre as drogas antirreabsortivas e suas respectivas repercussões nos ossos maxilares. A revisão foi realizada através de busca bibliográfica utilizando descritores Decs/Mesh de interesse, em português e inglês, nas bases de dados PubMed, Biblioteca Virtual de Saúde (BVS) e Scielo. Após aplicação dos critérios de inclusão e exclusão, um total de 33 trabalhos foram selecionados para análise. Pode-se constatar que a terapia com agentes antirreabsortivos é complexa, sobretudo na prática odontológica, visto que a OMAM é uma complicação de difícil manejo. Em relação às condutas terapêuticas para esta condição, divide-se em terapia conservadora, cirúrgica ou adjuvante, todavia, não existem protocolos validados na literatura, bem como não há consistência quanto à indicação do intervalo de suspensão da administração da droga - "Drug Holiday". Desse modo, é importante que a equipe multidisciplinar busque estratégias que minimizem as complicações e promovam o controle no uso dessas drogas. Além disso, nota-se a necessidade de realizar investigações que contribuam com diretrizes para o manejo e controle dos efeitos adversos decorrentes da terapia com medicamentos antirreabsortivos. (AU)
ABSTRACT
El objetivo del presente caso fue investigar los beneficios de la terapia fotodinámica (TFD) en ciclos de aplicación como método de tratamiento de la osteonecrosis inducida por fármacos (ONMRM), reportando el caso de un paciente masculino de 75 años, blanco, con diagnóstico de neoplasia maligna renal con metástasis pulmonares y óseas en tratamiento con malato de sunitinib y denosumab. El paciente fue derivado para evaluación dental en un consultorio habilitado para atender pacientes con necesidades especiales, en la ciudad de Salvador-Ba. En el examen intraoral se observaron áreas de tejido óseo expuesto y necrótico en la mandíbula (cara medial) a ambos lados, próximas a la región molar, con aspecto ulcerativo, de tres milímetros de diámetro, y la hipótesis diagnóstica fue de ONMRM en estadio 2. Se inició el protocolo de aplicación de 20 ciclos de TFD, como única forma de tratamiento, en dos sesiones semanales con terapia láser preventiva para la mucositis oral. Durante y al final del tratamiento fue posible el secuestro óseo necrótico, promoviendo la bioestimulación y la curación locorregional, sin manifestación clínica de la patología durante el mantenimiento. Con base en este informe y la evidencia en la literatura científica, se cree que se deben promover los ensayos clínicos, debido a los registros prometedores y efectivos del tratamiento que la terapia hace posible.
The main of this case was to investigate the benefits of photodynamic therapy (PDT) in application cycles as a method of treatment of medication-related osteonecrosis of the jaw (MRONJ), reporting the case of a 75-year-old white male patient with a diagnosis of renal malignancy with lung and bone metastases treated with sunitinib malate and denosumab. The patient was referred for dental evaluation in an office set up to attend patients with special needs, in the city of Salvador-Ba. In the intraoral examination, areas of exposed and necrotic bone tissue were observed in the mandible (medial aspect) on both sides, close to the molar region, with an ulcerative appearance, three millimeters in diameter, and the diagnostic hypothesis was stage 2 ONMRM The application protocol of 20 PDT cycles was started, as the only form of treatment, in two weekly sessions with preventive laser therapy for oral mucositis. During and at the end of the treatment, necrotic bone sequestration was possible, promoting biostimulation and locoregional healing, without clinical manifestation of the pathology during maintenance. Based on this report and the evidence in the scientific literature, it is believed that clinical trials should be promoted, due to the promising and effective records of the treatment that the therapy makes possible.

Resumen El tumor de células gigantes óseo es una neoplasia de agresividad local intermedia, que raramente metastatiza. En los últimos años el denosumab, anticuerpo monoclonal humano, surgió como una alternativa de tratamiento para esta enfermedad, al bloquear el comportamiento lítico tumoral. El objetivo de este trabajo fue determinar sus indicaciones y efectos adversos, analizando también los resultados oncológicos, y las tasas de recurrencia local en pacientes con diagnóstico de tumor de células gigantes óseo que recibieron denosumab como tratamiento neoadyuvante. Entre 2010 y 2018 se analizaron 80 pacientes con tumor de células gigantes, de los cuales 14 recibieron denosumab como tratamiento neoadyuvante. El seguimiento mínimo fue 12 meses. En 8 pacientes se trató de un tumor primario, mientras que 6 fueron pacientes con recidiva tumoral. En todos los casos se evidenció una mejoría clínica. Trece presentaron cambios radiográficos, y 11 respuesta histológica completa. En 6 de 14 pacientes se evidenció una recurrencia local y en 7 se identificó al menos un efecto adverso relacionado con el denosumab (incluyendo una malignización tumoral). A pesar de ser una herramienta útil para el tratamiento del tumor de células gigantes, el uso de denosumab está asociado a mayor tasa de recurrencias locales y no está exento de efectos adversos.
Abstract Giant cell tumor of bone is an intermediate, locally aggressive and rarely metastasiz ing, primary bone neoplasia. In recent years denosumab emerged as a treatment alternative for this pathology. The objective of this work was to analyze its indications as well as the clinical outcomes, side effects and local recurrence rates in patients diagnosed with giant cell tumor of bone, who received denosumab as neoadjuvant treatment. Between 2010 and 2018, 80 patients with giant cell tumor were analyzed, of whom 14 received deno sumab as a neoadjuvant treatment. The minimum follow-up was 12 months. In 8 patients it was a primary tumor, while 6 showed tumor recurrence. In all cases, clinical improvement was evident. Thirteen patients presented radiographic changes, and 11 showed complete histological response. A local recurrence was evidenced in 6 of 14 patients, and at least one adverse effect related to denosumab (including tumor malignancy) was identified in 7. Despite being a useful tool for treating giant cell tumor, the use of denosumab is associated with a higher rate of local recurrences and is not free of adverse effects.
